Abstract:PurposeTo demonstrate the feasibility of noninvasively measuring pancreatic perfusion using pseudocontinuous arterial spin labeling (ASL) and to derive quantitative blood‐flow and transit‐time measurements in healthy volunteers.MethodsA pseudocontinuous ASL sequence with background suppression and a single‐slice single‐shot fast‐spin‐echo readout was acquired at 3 T in 10 subjects with a single standard postlabeling delay (PLD) of 1.5 s and in 4 additional subjects with 4 PLD from 0.7 to 2 s. An imaging synchronized breathing approach was used to minimize motion artifacts during the 3 min of acquisition. Scan‐rescan reproducibility was assessed in 3 volunteers with single‐delay ASL. Quantitative blood flow and arterial transit time (ATT) were derived and the impact of ATT correction was studied using either subject‐specific ATT in the second group or an average ATT derived from the group with multidelay ASL for subjects with single‐delay ASL.ResultsSuccessful ASL acquisitions were performed in all volunteers. An average pancreatic blood flow of 201 ± 40 mL/100 g/min was measured in the single‐delay group using an assumed ATT of 750 ms Average ATT measured in the multidelay group was 1029 ± 89 ms Using the longer, measured ATT reduced the measured flow to 162 ± 12 and 168 ± 28 mL/100 g/min with subject‐specific or average ATT correction, respectively. ASL signal heterogeneities were observed at shorter PLD, potentially linked to its complex vascular supply and islet distribution.ConclusionsASL enables reliable measurement of pancreatic perfusion in healthy volunteers. It presents a valuable alternative to contrast‐enhanced methods and may be useful for diagnosis and characterization of several inflammatory, metabolic, and neoplastic diseases affecting the pancreas.

Abstract:Objective: Type 2 diabetes (T2DM) through various mechanisms increases CV risk, therefore early diagnosis is pivotal for patients prognosis. Microcirculation function and its’ structural changes may coincide or even precede overt cardiovascular disease in T2DM. Thus, microcirculation assessment holds potential to serve as early diagnostic window and/or basis for precision medicine strategies. Using novel microcirculation assessment techniques, in our study we focused on both structural and functional markers of abnormal microvascular bed performance in patients with glucose metabolism impairment. Design and method: Both, patients with (1) established diabetes, (2) prediabetes, and subjects (3) free of carbohydrates metabolism impairment were enrolled (n=180; 40% women). Microvascular phenotyping using RTX1 Adaptive Optics Retinal Camera (vessels remodeling denoted by wall-to-lumen ratio; WLR) and flow-mediated skin fluorescence (FMSF; hypoxia sensitivity; logHS) were performed. Results: Mean age of the studied population was 60.7 ±10.8 yo, BMI=29.9±4.8; Comparisons of WLR and logHS are presented in the figure. Patients with prediabetes had similar structural microcirculation phenotype as controls (left panel). However, prediabetic subjects were characterized by functional skin circulation abnormalities comparable to the ones evident in established T2DM (right panel). The observed differences were, however, lessened when age, and BMI status were included in the multivariate analyses. Conclusions: In patients with prediabetes, functional microcirculation alterations are present in the absence of structural changes of microvascular bed. The magnitude of these abnormalities are comparable to the ones seen in established diabetes. Our study however suggests that most of the differences in microcirculation misperformance in prediabetes are attributable to advancing age and higher body weight.

Abstract:A digit image processing system was developed to quantitatively analyse two-dimension (2D) morphometric data on the microcirculatory network. Processing the digitized image by noise elimination, thresholding, primary improving, thinning and secondary improving, we can eliminate the tissue background effectively to get 2D morphometric information which cannot be readily obtained by manual methods, including length of microvessels, number of nodal points, distribution of diameter, perfused condition and branch angle, etc. A high speed and a high degree of consistency are found and human bias are avoided. Abstract:Oxygen uptake by a cell is indicative of metabolic processes which are modulated by extracellular stimuli and is characteristic of a cell's physiological status. In this work, a MEMS platform with an array of microsensors to monitor these processes at cell constructs is being developed. An array of Pt microring electrodes has been fabricated to quantitatively assess oxygen uptake by normal and pharmacologically manipulated BAC1.2F5 macrophage cells positioned at cell attachment sites inside the microrings. An amperometric pulsing protocol was implemented to reduce interference with cellular metabolism caused by depletion of oxygen during the measurement. We were able to detect changes as small as 1% in oxygen concentration in the vicinity of single cells and small cell clusters. Thus, the approach can be used for sensitive indication of oxygen uptake by cell(s) and its modulation by environmental factors such as pharmacological agents, and interaction with adjacent cells. Moreover, parallel experiments at different cell attachment sites are made feasible by the sensor array. This can provide multiple data sets for biostatistics in the same amount of time that is required to get a single data set.

Abstract:Many diseases involve either the formation of new blood vessels (e.g., tumor angiogenesis) or the damage of existing ones (e.g., diabetic retinopathy) at the microcirculation level. Optical-resolution photoacoustic microscopy (OR-PAM), capable of imaging microvessels in 3D in vivo down to individual capillaries using endogenous contrast, has the potential to reveal microvascular information critical to the diagnosis and staging of microcirculation-related diseases. In this study, we have developed a dedicated microvascular quantification (MQ) algorithm for OR-PAM to automatically quantify multiple microvascular morphological parameters in parallel, including the vessel diameter distribution, the microvessel density, the vascular tortuosity, and the fractal dimension. The algorithm has been tested on in vivo OR-PAM images of a healthy mouse, demonstrating high accuracy for microvascular segmentation and quantification. The developed MQ algorithm for OR-PAM may greatly facilitate quantitative imaging of tumor angiogenesis and many other microcirculation related diseases in vivo.

Abstract:Evaluation of the oxygen-mediated effects of clinical and daily activities demands an on-skin device that can track multi-vital regional tissue hemodynamics simultaneously. For example, peripheral arterial disease (PAD) is the third most prevalent cardiovascular disease, but the means of diagnosing and monitoring this disease are limited because the affected area is usually in the non-pulsatile area away from the heart. Herein, we report on an ultrathin and ultralight multi-vital near-infrared optoelectronic biosensor for the diagnosis and rehabilitation monitoring of regional tissue hemodynamics, which is suitable for mounting on the skin for long-term measurement. The device can simultaneously detect tissue oxygen saturation, heart rate, arterial blood oxygen, and tissue perfusion and shows potential for various hypoxia monitoring applications. Moreover, the tissue hemodynamics detected by this device showed a highly accordance with the ankle-brachial index and CT angiography obtained by traditional clinical methods. Therefore, our design was able to accurately diagnose and effectively evaluate PAD patients before and after surgery. The on-skin optoelectronic biosensor shows potential in biological oxygen-mediated behavior evaluation, injury-state monitoring, PAD clinical diagnosis optimization, and after surgery care.

Abstract:Objective Monitoring microcirculation and visualizing microvasculature are critical for providing diagnosis to medical professionals and guiding clinical interventions. Ultrasound provides a medium for monitoring and visualization, however, there are challenges due to the complex microscale geometry of the vasculature and difficulties associated with quantifying perfusion. Here, we studied established state‐of‐the‐art ultrasonic modalities (using different probes) to compare their detection of slow flow in small microvasculature. Methods Five ultrasonic modalities were studied: grayscale, color, power, superb microvascular imaging (SMI), and microflow imaging (MFI), using six linear probes across two ultrasound scanners. Image readability was blindly scored by radiologists and quantified for evaluation. Vasculature visualization was investigated both in vitro (resolution and flow characterization) and in vivo (fingertip microvasculature detection). Results SMI and MFI imaging modalities provided superior images when compared with conventional ultrasound imaging modalities both in vitro and in vivo. The choice of probe played a significant difference in detectability. The slowest flow detected was 0.1885 ml/s and small microvasculature of the fingertip were visualized. Conclusions Our data demonstrated SMI and MFI used with vascular probes operating at higher frequencies provided resolutions acceptable for microvasculature visualization, paving the path for future development of ultrasound devices for microcirculation monitoring.

Abstract:Exocrine and endocrine pancreas are interconnected anatomically and functionally, with vasculature facilitating bidirectional communication. Our understanding of this network remains limited, largely due to two-dimensional histology and missing combination with three-dimensional imaging. In this study, a multiscale 3D-imaging process was used to analyze a porcine pancreas. Clinical computed tomography, digital volume tomography, micro-computed tomography and Synchrotron-based propagation-based imaging were applied consecutively. Fields of view correlated inversely with attainable resolution from a whole organism level down to capillary structures with a voxel edge length of 2.0 μm. Segmented vascular networks from 3D-imaging data were correlated with tissue sections stained by immunohistochemistry and revealed highly vascularized regions to be intra-islet capillaries of islets of Langerhans. Generated 3D-datasets allowed for three-dimensional qualitative and quantitative organ and vessel structure analysis. Beyond this study, the method shows potential for application across a wide range of patho-morphology analyses and might possibly provide microstructural blueprints for biotissue engineering.

Label-free oxygen-metabolic photoacoustic microscopy in vivo

Abstract:Almost all diseases, especially cancer and diabetes, manifest abnormal oxygen metabolism. Accurately measuring the metabolic rate of oxygen (MRO(2)) can be helpful for fundamental pathophysiological studies, and even early diagnosis and treatment of disease. Current techniques either lack high resolution or rely on exogenous contrast. Here, we propose label-free metabolic photoacoustic microscopy (mPAM) with small vessel resolution to noninvasively quantify MRO(2) in vivo in absolute units. mPAM is the unique modality for simultaneously imaging all five anatomical, chemical, and fluid-dynamic parameters required for such quantification: tissue volume, vessel cross-section, concentration of hemoglobin, oxygen saturation of hemoglobin, and blood flow speed. Hyperthermia, cryotherapy, melanoma, and glioblastoma were longitudinally imaged in vivo. Counterintuitively, increased MRO(2) does not necessarily cause hypoxia or increase oxygen extraction. In fact, early-stage cancer was found to be hyperoxic despite hypermetabolism.

Abstract:Continuous monitoring of variations in blood flow is vital in assessing the status of microvascular and macro-vascular beds for a wide range of clinical and research scenarios. Although a variety of techniques exist, most require complete immobilization of the subject, thereby limiting their utility to hospital or clinical settings. Those that can be rendered in wearable formats suffer from limited accuracy, motion artifacts, and other shortcomings that follow from an inability to achieve intimate, noninvasive mechanical linkage of sensors with the surface of the skin. We introduce an ultrathin, soft, skin-conforming sensor technology that offers advanced capabilities in continuous and precise blood flow mapping. Systematic work establishes a set of experimental procedures and theoretical models for quantitative measurements and guidelines in design and operation. Experimental studies on human subjects, including validation with measurements performed using state-of-the-art clinical techniques, demonstrate sensitive and accurate assessment of both macrovascular and microvascular flow under a range of physiological conditions. Refined operational modes eliminate long-term drifts and reduce power consumption, thereby providing steps toward the use of this technology for continuous monitoring during daily activities.

Abstract:Pancreatic cancer is a common cancer with poor odds of survival for the patient, with surgical resection offering the only hope of cure. Current surgical practice is time-consuming and, due to time constraints, does not sample the whole cut surface sufficiently to check for remaining cancer. Although microscopy with hematoxylin and eosin (H&E) stain is the gold standard for microscopic evaluation, multiphoton microscopy (MPM) has emerged as an alternative tool for imaging tissue architecture and cellular morphology without labels. We explored the use of multimodal MPM for the label-free identification of normal and cancerous tissue of the pancreas in a mouse model by comparing the images to H&E microscopy. Our early studies indicate that MPM using second-harmonic generation, third-harmonic generation, and multiphoton excitation of endogenous fluorescent proteins can each contribute to the label-free analysis of the pancreatic surgical margin.
3D evaluation of the extracellular matrix of hypoxic pancreatic islets using light sheet fluorescence microscopy

Abstract:Pancreatic islet transplantation is a promising treatment for type 1 diabetes, but the survival and function of transplanted islets are hindered by the loss of extracellular matrix (ECM) during islet isolation and by low oxygenation upon implantation. This study aimed to evaluate the impact of hypoxia on ECM using a cutting-edge imaging approach based on tissue clearing and 3D microscopy. Human and rat islets were cultured under normoxic (O 2 21%) or hypoxic (O 2 1%) conditions. Immunofluorescence staining targeting insulin, glucagon, CA9 (a hypoxia marker), ECM proteins (collagen 4, fibronectin, laminin), and E-cadherin (intercellular adhesion protein) was performed on fixed whole islets. The cleared islets were imaged using Light Sheet Fluorescence Microscopy (LSFM) and digitally analyzed. The volumetric analysis of target proteins did not show significant differences in abundance between the experimental groups. However, 3D projections revealed distinct morphological features that differentiated normoxic and hypoxic islets. Under normoxic conditions, ECM could be found throughout the islets. Hypoxic islets exhibited areas of scattered nuclei and central clusters of ECM proteins, indicating central necrosis. E-cadherin was absent in these areas. Our results, demonstrating a diminution of islets' functional mass in hypoxia, align with the functional decline observed in transplanted islets experiencing low oxygenation after grafting. This study provides a methodology combining tissue clearing, multiplex immunofluorescence, Light Sheet Fluorescence Microscopy, and digital image analysis to investigate pancreatic islet morphology. This 3D approach allowed us to highlight ECM organizational changes during hypoxia from a morphological perspective.

SliceChip: a benchtop fluidic platform for organotypic culture and serial assessment of human and rodent pancreatic slices

Abstract:Enzymatically isolated pancreatic islets are the most commonly used ex vivo testbeds for diabetes research. Recently, precision-cut living slices of human pancreas are emerging as an exciting alternative because they maintain the complex architecture of the endocrine and exocrine tissues, and do not suffer from the mechanical and chemical stress of enzymatic isolation. We report a fluidic pancreatic SliceChip platform with dynamic environmental controls that generates a warm, oxygenated, and bubble-free fluidic pathway across singular immobilized slices with continuous deliver of fresh media and the ability to perform repeat serial perfusion assessments. A degasser ensures the system remains bubble-free while systemic pressurization with compressed oxygen ensures slice medium remains adequately oxygenated. Computational modeling of perfusion and oxygen dynamics within SliceChip guide the system's physiomimetic culture conditions. Maintenance of the physiological glucose dependent insulin secretion profile across repeat perfusion assessments of individual pancreatic slices kept under physiological oxygen levels demonstrated the culture capacity of our platform. Fluorescent images acquired every 4 hours of transgenic murine pancreatic slices were reliably stable and recoverable over a 5 day period due to the inclusion of a 3D-printed bioinert metallic anchor that maintained slice position within the SliceChip. Our slice on a chip platform has the potential to expand the useability of human pancreatic slices for diabetes pathogenesis and the development of new therapeutic approaches, while also enabling organotypic culture and assessment of other tissue slices such as brain and patient tumors.
The Physiological Characteristics of the Basal Microvilli Microvessels in Pancreatic Cancers.

Abstract:Pancreatic cancer (PC) is a highly lethal tumor with controversial high glucose uptake and hypomicrovascularity, and the hypomicrovasculature, which is considered to have poor perfusion, blocks the delivery of drugs to tumors. The preferential existence of a novel endothelial projection with trafficking vesicles in PCs, referring to basal microvilli, was described previously. However, the perfusion and nutrients delivering status of the basal microvilli microvessels are unknown. Here, we used the perfusion of fluorescently labeled CD31 antibody, lectin, and 2-NBDG to autochthonous PC-bearing mice, immunostaining, probe-based confocal laser endoscopy and three-dimensional (3D) reconstruction to study the nutrient trafficking, and perfusion status of the basal microvilli microvasculature in PC. Our data showed that the coperfusion of lectin and CD31 is an efficient way to show the microcirculation in most healthy organs. However, coperfusion with lectin and CD31 is inefficient for showing the microcirculation in PCs compared with that in healthy organs and immunostaining. This method does not reflect the nutrient trafficking status in the microvessels, especially in basal microvilli microvessels of PCs. In basal microvilli microvessels that were poorly labeled by lectin, we observed large vesicle-like structures with 2-NBDG preferentially located at the base of the basal microvilli or in basal microvilli, and there were long filopodia on the luminal surface of the human PC microvasculature. Our observations suggest that the PC microvasculature, especially basal microvilli microvessels, is well perfused and might be highly efficient in the trafficking of glucose or other nutrients, indicating that macropinocytosis might participate in the nutrient trafficking.
Integrated Pancreatic Blood Flow: Bidirectional Microcirculation Between Endocrine and Exocrine Pancreas

Abstract:The pancreatic islet is a highly vascularized endocrine micro-organ. The unique architecture of rodent islets, a so-called core-mantle arrangement seen in two-dimensional images, led researchers to seek functional implications for islet hormone secretion. Three models of islet blood flow were previously proposed, all based on the assumption that islet microcirculation occurs in an enclosed structure. Recent electrophysiological and molecular biological studies using isolated islets also presumed unidirectional flow. Using intravital analysis of the islet microcirculation in mice, we found that islet capillaries were continuously integrated to those in the exocrine pancreas, which made the islet circulation rather open, not self-contained. Similarly in human islets, the capillary structure was integrated with pancreatic microvasculature in its entirety. Thus, islet microcirculation has no relation to islet cytoarchitecture, which explains its well-known variability throughout species. Furthermore, tracking fluorescent-labeled red blood cells at the endocrine-exocrine interface revealed bidirectional blood flow, with similar variability in blood flow speed in both the intra- and extra-islet vasculature. To date, the endocrine and exocrine pancreas have been studied separately by different fields of investigators. We propose that the open circulation model physically links both endocrine and exocrine parts of the pancreas as a single organ through the integrated vascular network.
Continuous Monitoring of Diabetes with an Integrated Microneedle Biosensing Device Through 3D Printing.

Abstract:Diabetes is a prevalent chronic metabolic disease with multiple clinical manifestations and complications, and it is among the leading causes of death. Painless and continuous monitoring of interstitial glucose is highly desirable for diabetes management. Here we unprecedentedly show continuous monitoring of diabetes with an integrated microneedle biosensing device. The device was manufactured with a 3D printing process, a microfabrication process, an electroplating process, and an enzyme immobilization step. The device was inserted into the dermis layer of mouse skin and showed accurate sensing performance for monitoring subcutaneous glucose levels in normal or diabetic mice. The detection results were highly correlated with those obtained from a commercial blood glucose meter. We anticipate that the study could open exciting avenues for monitoring and managing diabetes, alongside fundamental studies of subcutaneous electronic devices.

Abstract:In vivo imaging of microcirculation can improve our fundamental understanding of cerebral microhemodynamics under various physiological challenges, such as hypoxia and hyperoxia. However, existing techniques often involve the use of invasive procedures or exogenous contrast agents, which would inevitably perturb the intrinsic physiologic responses of microcirculation being investigated. We report ultrahigh sensitive optical microangiography (OMAG) for label-free monitoring of microcirculation responses challenged by oxygen inhalation. For the first time, we demonstrate that OMAG is capable of showing the impact of acute hypoxia and hyperoxia on microhemodynamic activities, including the passive and active modulation of microvascular density and flux regulation, within capillary and noncapillary vessels in rodents in vivo. The ability of OMAG to functionally image the intact microcirculation promises future applications for studying cerebral diseases. (C) 2011 Society of Photo-Optical Instrumentation Engineers (SPIE).
