Abstract:Objective: This work focuses on development a new mini sensing module to directly measure periarterial pressure and reduce the effect of muscle action induced artifacts on blood pressure measurement. Design and method: The coupling of blood pressure signal with disturbances such as motion artifacts makes blood pressure measurement unreliable. It is significant to directly measure periarterial pressure and reduce the effect of motion artefacts in the applications of wearable blood pressure monitoring in daily life. The mini sensing module consists of two independent sensing channels (Fig.1). The two channels are closely aligned with the spacing only 1 mm in order to guarantee the blood pressure signal under the channels are same. The two channels have different sizes, one with 5 mm length and 3 mm width and the other one with 8 mm length and 3 mm width. The channels are filled with silicon oil, in the which a silicon die is fixed as pressure sensor unit. The channels are sealed by TPU flexible film with 0.2 mm thickness, which serves as the stimulation interface. Attach the sensor module to wrist and the channel direction is transverse to the arterial path. Specific design endows both channels with same sensitivity to global mechanical excitation and different sensitivity to local excitations. During BP measuring, external pressure and muscle motion artifact act on the entire channel surface serving as global excitation, so both channels have the same sensitivity to them. However, because the width of the radial artery is smaller than the length of the channel, the BP signal act on partial channel surface serving as local excitation. The BP signal has different sensitivity in two channels. The influence of motion artifacts is reduced by differential processing of the output signals of two channels, which also is presented in Fig.2. Results: Experiments have proved the effectiveness of the scheme. Conclusions: By comparison with the BP signal under optimal condition. The sensing module can directly measure periarterial pressure on blood pressure measurement.

Abstract:Evaluation of the oxygen-mediated effects of clinical and daily activities demands an on-skin device that can track multi-vital regional tissue hemodynamics simultaneously. For example, peripheral arterial disease (PAD) is the third most prevalent cardiovascular disease, but the means of diagnosing and monitoring this disease are limited because the affected area is usually in the non-pulsatile area away from the heart. Herein, we report on an ultrathin and ultralight multi-vital near-infrared optoelectronic biosensor for the diagnosis and rehabilitation monitoring of regional tissue hemodynamics, which is suitable for mounting on the skin for long-term measurement. The device can simultaneously detect tissue oxygen saturation, heart rate, arterial blood oxygen, and tissue perfusion and shows potential for various hypoxia monitoring applications. Moreover, the tissue hemodynamics detected by this device showed a highly accordance with the ankle-brachial index and CT angiography obtained by traditional clinical methods. Therefore, our design was able to accurately diagnose and effectively evaluate PAD patients before and after surgery. The on-skin optoelectronic biosensor shows potential in biological oxygen-mediated behavior evaluation, injury-state monitoring, PAD clinical diagnosis optimization, and after surgery care.

Abstract:In the medical field, it is important to monitor and evaluate the blood supply status of organs and tissues during the clinical surgery. However, this largely depends on the surgeon’s experience and naked eye, which is easy to misjudge due to the interference of blood stains or other factors. A portable and flexible photoplethysmographic (PPG) detection probe is developed in this paper. And a new evaluation methodology of blood supply status is proposed based on this probe. Three typical indicators based on PPG is proposed to comprehensively evaluate the blood supply status, which are the blood oxygen saturation and its pulsation, differential characteristics of different lights, and time-frequency energy spectral characteristic. The probe and its evaluation methodology are verified using the brain of rats as a model.

Abstract:Background: Nowadays, Cardiovascular disease is a severe threat to human health. It is necessary to monitor the cardiovascular parameters such as blood oxygen saturation reliably and conveniently. Non-contact measurement has been widely studied. However, some inevitable factors limit the use of the platform and even lead to inaccurate estimation. Hence, this study proposed a new method to estimate blood oxygen saturation. Methods: This paper introduced a new IPPG device consisting of a dual-wavelength light source system, an enclosed illumination chamber and an RGB camera. In order to get a high-precision SpO2 calculation model, the data obtained and processed from the system are compared to the output of a traditional figure blood volume pulse sensor that was employed on 20 subjects at the same time. The most suitable region of interest (ROI) area was determined based on previous research. Findings: The comparison showed that the data from the new IPPG system is consistent and comparable with the traditional pulse oximeter. Compared to other IPPG-based measuring methods, our new IPPG device can remove the motion artifacts caused by body movement well. What's more, the consistency of the environment was maintained and reduce the interference of the external environment during the experiment because of the device.

Abstract:Patients with diabetes mellitus (DM) may experience problems such as peripheral tissue necrosis and gradual hardening of blood vessels; these may prevent normal metabolic behavior and may even require amputation under severe conditions. Therefore, evaluating the peripheral blood circulation state of patients with DM is crucial to enabling physicians to perform timely interventional therapy to prevent symptoms from worsening. To improve the general problems of treatment, such as high cost, the infection risk or misjudgment of specific groups, a smart blood perfusion monitoring system is proposed to noninvasively evaluate patients' peripheral blood circulation state. This system uses near-infrared spectroscopy to noninvasively monitor real-time changes in peripheral blood perfusion with force is applied on the arm. On the basis of changes in peripheral blood perfusion with pressure, several indexes related to blood circulation state are proposed. Finally, a neural network technique was successfully applied to classify patients' blood circulation state. From the experimental results, F-measure, sensitivity, positive predictive value and accuracy are 82.75%, 80.00%, 85.71% and 83.33%, respectively. The experimental results show that the proposed indexes (Indexes I-IV) are significantly related to blood circulation state and can be used to effectively evaluate peripheral blood circulation.

Abstract:Systemic blood circulation is one of life activity’s most important physiological functions. Continuous noninvasive hemodynamic monitoring is essential for the management of cardiovascular status. However, it is difficult to achieve systemic hemodynamic monitoring with the daily use of current devices due to the lack of multichannel and time-synchronized operation capability over the whole body. Here, we utilize a soft microfiber Bragg grating group to monitor spatiotemporal hemodynamics by taking advantage of the high sensitivity, electromagnetic immunity, and great temporal synchronization between multiple remote sensor nodes. A continuous systemic hemodynamic measurement technique is developed using all-mechanical physiological signals, such as ballistocardiogram signals and pulse waves, to illustrate the actual mechanical process of blood circulation. Multiple hemodynamic parameters, such as systemic pulse transit time, heart rate, blood pressure, and peripheral resistance, are monitored using skin-like microfiber Bragg grating patches conformally attached at different body locations. Relying on the soft microfiber Bragg grating group, the spatiotemporal hemodynamic monitoring technique opens up new possibilities in clinical medical diagnosis and daily health management.

Abstract:Perfusion index (PI), the ratio between variable pulsatile (AC) and non-pulsatile (DC) components in a photoplethysmographic (PPG) signal, is an indirect and non-invasive measure of peripheral perfusion. PI has been widely used in assessing sympathetic block success, and monitoring hemodynamics in anesthesia and intensive care. Based on the principle of dual-wavelength depolarization (DWD) of skin tissues, we propose to investigate its opportunity in quantifying the skin perfusion contactlessly. The proposed method exploits the characteristic changes in chromaticity caused by skin depolarization and chromophore absorption. The experimental results of DWD, obtained with the post occlusive reactive hyperemia test and the local cooling and heating test, were compared to the PI values obtained from the patient monitor and photoplethysmography imaging (PPGI). The comparison demonstrated the feasibility of using DWD for PI measurement. Clinical trials conducted in the anesthesia recovery room and operating theatre further showed that DWD is potentially a new metric for camera-based non-contact skin perfusion monitoring during clinical operations, such as the guidance in anesthetic surgery.

Abstract:Cardiovascular disease remains a leading global health challenge, necessitating precise and continuous monitoring of blood pressure (BP) and cardiac function. Traditional non-invasive measurement techniques often present operational complexities and discomfort, while photoplethysmography (PPG) technology lacks the ability to capture comprehensive hemodynamic parameters. Herein, we introduce an innovative Internet of Medical Things (IoMT) framework for personalized hemodynamic assessment, driven by advanced flexible sensing technologies and multi-scale modeling. Specifically, we propose a pulse wave detection system utilizing micro-fiber Bragg grating (lFBG) sensors for comprehensive spatiotemporal monitoring. The system leverages a Multi-scale Perception Network (MSP-Net) to achieve precise BP estimation, with mean errors (ME) and standard deviations (SD) of -0.05 ± 2.15 mmHg for systolic blood pressure (SBP) and -0.19 ± 3.48 mmHg for diastolic blood pressure (DBP). The system’s performance surpasses traditional PPG-based methods across multiple hemodynamic parameters. Leveraging pre-training and fine-tuning strategies, this study realizes personalized estimation models, furnishing technical support for precision medicine. Furthermore, we utilize a cloud-edge collaborative framework to achieve hemodynamic assessment across multiple IoMT devices via LAN and the RV1126 platform, providing real-time feedback, thereby establishing a closed-loop control system. Our study presents an innovative approach for the comprehensive assessment of cardiovascular function and the realization of personalized medicine, potentially serving as a promising solution for Healthcare 5.0 applications.

Abstract:Continuous monitoring of variations in blood flow is vital in assessing the status of microvascular and macro-vascular beds for a wide range of clinical and research scenarios. Although a variety of techniques exist, most require complete immobilization of the subject, thereby limiting their utility to hospital or clinical settings. Those that can be rendered in wearable formats suffer from limited accuracy, motion artifacts, and other shortcomings that follow from an inability to achieve intimate, noninvasive mechanical linkage of sensors with the surface of the skin. We introduce an ultrathin, soft, skin-conforming sensor technology that offers advanced capabilities in continuous and precise blood flow mapping. Systematic work establishes a set of experimental procedures and theoretical models for quantitative measurements and guidelines in design and operation. Experimental studies on human subjects, including validation with measurements performed using state-of-the-art clinical techniques, demonstrate sensitive and accurate assessment of both macrovascular and microvascular flow under a range of physiological conditions. Refined operational modes eliminate long-term drifts and reduce power consumption, thereby providing steps toward the use of this technology for continuous monitoring during daily activities.
Wearable Skin-Like Optoelectronic Systems with Suppression of Motion Artifacts for Cuff-Less Continuous Blood Pressure Monitor

Abstract:According to the statistics of the World Health Organization, an estimated 17.9 million people die from cardiovascular diseases each year, representing 31% of all global deaths. Continuous non-invasive arterial pressure (CNAP) is essential for the management of cardiovascular diseases. However, it is difficult to achieve long-term CNAP monitoring with the daily use of current devices due to irritation of the skin as well as the lack of motion artifacts suppression. Here, we report a high-performance skin-like optoelectronic system integrated with ultra-thin flexible circuits to monitor CNAP. We introduce a theoretical model via the virtual work principle for predicting the precise blood pressure and suppressing motion artifacts, and propose optical difference in the frequency domain for stable optical measurements in terms of skin-like devices. We compare the results with the blood pressure acquired by invasive (intra-arterial) blood pressure monitoring for >1500 min in total on 44 subjects in an intensive care unit. The maximum absolute errors of diastolic and systolic blood pressure were 7/+/- 10 mm Hg, respectively, in immobilized, and +/- 10/+/- 14 mm Hg, respectively, in walking scenarios. These strategies provide advanced blood pressure monitoring techniques, which would directly address an unmet clinical need or daily use for a highly vulnerable population.

Abstract:The beat frequency, flow rate, and oxygen content of human pulsating blood are important health indicators. For early diagnosis of diseases and real-time monitoring of cardiovascular health in specific practical scenarios, noninvasive methods are used that reduce pain and the risk of infection, but they are not conducive to wound healing. In this field, blood oxygen saturation in humans is obtained based on the use of the medium absorption principle of Lambert Beer's law. Owing to the absorption of light by different substances in the blood, the pulse rate constitutes an important index that is used to assess the status of the cardiovascular system. This study summarizes the research outcomes of sensor devices used for blood oxygen and pulse rate signal monitoring, based on the photoplethysmographic (PPG) test principle, and the main technical means and intelligent application scenarios for improving the shape adaptability, test stability, and biological compatibility of these devices. Nowadays, fabrication technologies are capable of realizing real-time stability in complex practical applications. Based on the research results and the related development status in the field of flexible electronics, some enlightening prospects for the future development of flexible PPG sensors are presented.

Abstract:Well-functioning vascular access (VA) is essential for hemodialysis treatment in patients with end-stage renal disease (ESRD). However, continuous and accurate monitoring of blood flow to assess high-flux VA during hospitalization or at home is not feasible for either clinical instruments or wearable sensors. Here, we report the design and preclinical validation of a high-precision, long-term, epidermal blood flow sensor that self-adapts to unavoidable sensor-mounting deviations on the skin and is compatible with individual tissue differences. Specifically, the technology is based on thermal dissipation of the skin, and improves the signal-to-error ratio surpassing 4 times when measuring high-flux blood (100–600 mL/min). In preclinical validation, the sensor is compared with the Doppler ultrasound and demonstrate a blood flow resolution of 10–50 mL/min. Furthermore, it is highly-integrated and wearable, measuring 36 × 50 mm 2 . The sensor paves the way for accurate, convenient, high-flux blood monitoring, offering significant potential to extend the lives of patients with ESRD.

Abstract:Hemodynamic activities, as an essential measure of physiological and psychological characteristics, can be used for cardiovascular and cerebrovascular disease detection. Photoplethysmography imaging (iPPG) can be applied for such purposes with non-contact advances, however, most cardiovascular hemodynamics of iPPG systems are developed for laboratory research, which limits the application in pervasive healthcare. In this study, a video-based facial iPPG detecting equipment was devised to provide multi-dimensional spatiotemporal hemodynamic pulsations for applications with high portability and self-monitoring requirements. A series of algorithms have also been developed for physiological indices such as heart rate and breath rate extraction, facial region analysis, and visualization of hemodynamic pulsation distribution. Results showed that the new device can provide a reliable measurement of a rich range of cardiovascular hemodynamics. Combined with the advanced computing techniques, the new non-contact iPPG system provides a promising solution for user-friendly pervasive healthcare.

Abstract:A system combining the capabilities of spectrometry, photoplethysmography, and arterial pressure measurement is demonstrated to extract hemodynamic parameters from the fingertip. The optical channels effectively split into three groups, representing: i) the smallest blood vessels (microcirculation), ii) an abrupt transition band at the wavelength range where the absorption coefficient of whole blood decreases, and iii) large arteries. Minimally invasive and non‐invasive hemodynamic monitoring technologies have recently gained more attention, driven by technological advances and the inherent risk of complications in invasive techniques. In this article, an experimental non‐invasive system is presented that effectively combines the capabilities of spectrometry, photoplethysmography (PPG), and arterial pressure measurement. Both time‐ and wavelength‐resolved optical signals from the fingertip are measured under external pressure, which gradually increased above the level of systolic blood pressure. The optical channels measured at 434–731 nm divided into three groups separated by a group of channels with wavelengths approximately between 590 and 630 nm. This group of channels, labeled transition band, is characterized by abrupt changes resulting from a decrease in the absorption coefficient of whole blood. External pressure levels of maximum pulsation showed that shorter wavelengths ( 630 nm) probe high‐pressure arteries. The results on perfusion indices and DC component level changes showed clear differences between the optical channels, further highlighting the importance of wavelength selection in optical hemodynamic monitoring systems. Altogether, the results demonstrated that the integrated system presented has the potential to extract new hemodynamic information simultaneously from macrocirculation to microcirculation.

Abstract:Cardiovascular health is typically monitored by measuring blood pressure. Here we describe a wireless on-skin system consisting of synchronized sensors for chest electrocardiography and peripheral multispectral photoplethysmography for the continuous monitoring of metrics related to vascular resistance, cardiac output and blood-pressure regulation. We used data from the sensors to train a support-vector-machine model for the classification of haemodynamic states (resulting from exposure to heat or cold, physical exercise, breath holding, performing the Valsalva manoeuvre or from vasopressor administration during post-operative hypotension) that independently affect blood pressure, cardiac output and vascular resistance. The model classified the haemodynamic states on the basis of an unseen subset of sensor data for 10 healthy individuals, 20 patients with hypertension undergoing haemodynamic stimuli and 15 patients recovering from cardiac surgery, with an average precision of 0.878 and an overall area under the receiver operating characteristic curve of 0.958. The multinodal sensor system may provide clinically actionable insights into haemodynamic states for use in the management of cardiovascular disease.
Development of a Continuous Blood Pressure Monitoring System based on Pulse Transit Time and Hemodynamic Covariates

Abstract:There were many studies showing the relation between pulse transit time (PTT) and blood pressure (BP). Besides, hemodynamic covariates may also contribute to BP values. Our previous study has proposed a BP model based on PTT, HR, stiffness index (SIx) and descent time (DT), which has been validated on the Multiparameter Intelligent Monitoring for Intensive Care (MIMIC) database. In this article, we present a prototype cuff-less monitoring device for non-invasive estimation of BP, which can obtain both electrocardiogram (ECG) and finger plethysmograph (PPG) signals synchronously. The model proposed above has been validated by using ECG and PPG records from 22 healthy subjects with no cardiovascular disease and hypertension, and the error of BP estimation was 0.002 +/- 8.544 mmHg for SBP, 0.005 +/- 6.690 mmHg for DBP. The reliability of this method in long-term BP monitoring was further verified by studying the data of one individual for 28 days, while the error was 5.204 +/- 5.462 mmHg for SBP, 2.714 +/- 4.756 mmHg for DBP without calibration. The results show that the model could estimate the BP value within the acceptable error range based on this study, which is extremely close to AAMI's standard (5 +/- 8nunHg) and consistent with the cuff-method. The proposed ultra-low power, wearable, time-synchronized prototype monitoring device with an embedded hemodynamic covariate model, can measure SBP and DBP values accurately, which is expected to estimate continuous blood pressure better.

Abstract:Photoplethysmography (PPG) is a non-invasive optical technique that provides valuable physiological information, including heart rate (HR), pulse wave (PW), and blood oxygen saturation (SpO2). However, wearable devices often have variable signal quality, lacking monitoring capabilities or precision in daily use. To address these limitations, we propose a multi-wavelength PPG-based patch physiological monitoring system. The system utilizes three PPG wavelengths of enhanced signal quality, leading to improved accuracy and potentially extracting a wider range of physiological information compared to standard devices. We evaluated the system's performance by extracting PPG signals from three locations on the human body and analyzing their quality. The results demonstrate that the system successfully extracts high-quality PW, enabling accurate estimation of HR, SpO2, and even blood pressure (BP). The BP estimation model, particularly with personalized adaptation, exhibits good performance. The system offers the potential for continuous, non-invasive measurement of a wider range of physiological parameters compared to traditional wearable devices, and its portability and ease of use make it a compelling solution for user-friendly daily healthcare monitoring.
Non-Invasive Capillary Blood Pressure Measurement Enabling Early Detection and Classification of Venous Congestion

Abstract:Capillary blood pressure (CBP) is the primary driving force for fluid exchange across microvessels. Subclinical systemic venous congestion prior to overt peripheral edema can directly result in elevated peripheral CBP. Therefore, CBP measurements can enable timely edema control in a variety of clinical cases including venous insufficiency, heart failure and so on. However, currently CBP measurements can be only done invasively and with a complicated experimental setup. In this work, we proposed an opto-mechanical system to achieve non-invasive and automatic CBP measurements through modifying the widely implemented oscillometric technique in home-use arterial blood pressure monitors. The proposed CBP system is featured with a blue light photoplethysmography sensor embedded in finger/toe cuffs to probe skin capillary pulsations. The experimental results demonstrated the proposed CBP system can track local CBP changes induced by different levels of venous congestion. Leveraging the decision tree technique, we demonstrate the use of a multi-site CBP measurement at fingertips and toes to classify four categories of subjects (total N = 40) including patients with peripheral arterial disease, varicose veins and heart failure. Our work demonstrates the promising non-invasive CBP measurement as well as its great potential in realizing point-of-care systems for the management of cardiovascular diseases.

Abstract:A unconstrained and implantable piezoelectric vascular electronic system for real‐time, long‐term, and wireless hemodynamic monitoring is developed. Bioinspired by nature leaf sheath, a growable sheath with elasticity, and conformal shape adaptive to the dynamically growing arterial vessels is integrated with the piezoelectric biosensor. The patient‐centered healthcare requires timely disease diagnosis and prognostic assessment, calling for individualized physiological monitoring. To assess the postoperative hemodynamic status of patients, implantable blood flow monitoring devices are highly expected to deliver real time, long‐term, sensitive, and reliable hemodynamic signals, which can accurately reflect multiple physiological conditions. Herein, an implantable and unconstrained vascular electronic system based on a piezoelectric sensor immobilized is presented by a "growable" sheath around continuously growing arterial vessels for real‐timely and wirelessly monitoring of hemodynamics. The piezoelectric sensor made of circumferentially aligned polyvinylidene fluoride nanofibers around pulsating artery can sensitively perceive mechanical signals, and the growable sheath bioinspired by the structure and function of leaf sheath has elasticity and conformal shape adaptive to the dynamically growing arterial vessels to avoid growth constriction. With this integrated and smart design, long‐term, wireless, and sensitive monitoring of hemodynamics are achieved and demonstrated in rats and rabbits. It provides a simple and versatile strategy for designing implantable sensors in a less invasive way.

A New Physiological Control Strategy based on the CardioMEMS Pulmonary Artery Pressure Sensor for Rotary Blood Pumps

Abstract:The mechanical rotary blood pumps (RBP) have been designed to provide sufficient physiological perfusion and prevent ventricular collapse caused by suction events for the congestive heart failure (HF) patients. Various control systems could be one of the most crucial objectives for developing RBPs. Some control algorithms need implantable sensors, but they are unreliable due to sensor error, pump thrombus, or short lifespan, etc. Other sensorless control algorithms can eliminate any unreliable sensor but to date those strategies have not been successfully incorporated into the RBPs in clinical. To overcome the above limitations, in this study a new physiological control strategy for RBPs has been proposed. This method depends on a recently developed implantable wireless sensor called CardioMEMS, which can provide hemodynamic information for pulmonary artery pressure (PAP). In this algorithm, a gain-scheduled proportional-integral controller is used to maintain the actual mean PAP measurements close to a user-defined threshold to provide sufficient physiologic perfusion. The algorithm is tested in-silico with (1) reference mean PAP (MPAP) of 17 mmHg and the threshold of MPAP as 15 and 19 mmHg due to ±2 mmHg sensor drift during rest and exercise conditions with normal and increased pulmonary vascular resistance (PVR); (2) physiological state quickly changes between rest and exercise with normal PVR for (1); and (3) actual MPAP measurements with 5% and 10% uniformly distributed noise for (1) and (2). Simulation results demonstrated that with the CardioMEMS sensor the proposed control algorithm can achieve adequate physiologic perfusion with satisfied efficacy and robustness.
