Abstract:The International Journal of Artificial Organs, Ahead of Print. Musculoskeletal diseases are often related with postural changes in the neck region that can be caused by prolonged cervical flexion. This is one of the contributing factors. When determining the prevalence, causes, and related risks of neck discomfort, having a solid understanding of the biomechanics of the cervical spine (C1–C7) is absolutely necessary. The objective of this study is to make predictions regarding the intervertebral disc (IVD) stress values across C2–C7 IVD, the ligament stress, and the variation at 0°, 15°, 30°, 45°, and 60° of cervical neck angle using finite element analysis (FEA). In order to evaluate the mechanical properties of the cervical spine (particularly, C2–C7), this investigation makes use of computed tomography (CT) scans to develop a three-dimensional FEA model of the cervical spine. A preload of 50 N compression force was applied at the apex of the C2 vertebra, and all degrees of freedom below the C7 level were constrained. The primary objective of this investigation is to assess the distribution of von Mises stress within the IVDs and ligaments spanning C2–C7 at various flexion angles: 0°, 15°, 30°, 45°, and 60°, utilizing FEA. The outcomes derived from this analysis were subsequently compared to previously published experimental and FEA data to validate the model's ability to replicate the physiological motion of the cervical spine across different flexion angles.

Biomechanics of the upper cervical spine ligaments in axial rotation and flexion-extension: Considerations into the clinical framework

Abstract:Context: The motion of the upper cervical spine (UCS) has a great interest for analyzing the biomechanical features of this joint complex, especially in case of instability. Although investigators have analyzed numerous kinematics and musculoskeletal characteristics, there are still little data available regarding several suboccipital ligaments such as occipito-atlantal, atlantoaxial, and cruciform ligaments. Objective: The aim of this study is to quantify the length and moment arm magnitudes of suboccipital ligaments and to integrate data into specific 3D-model, including musculoskeletal and motion representation. Materials and methods: Based on a recent method, suboccipital ligaments were identified using UCS anatomical modeling. Biomechanical characteristics of these anatomical structures were assessed for sagittal and transversal displacements regarding length and moment arm alterations. Results: Outcomes data indicated length alterations >25% for occipito-atlantal, atlanto-axial and apical ligaments. The length alteration of unique ligaments was negligible. Length variation was dependent on the motion direction considered. Regarding moment arm, larger magnitudes were observed for posterior ligaments, and consistent alteration was depicted for these structures. Conclusion: These outcomes supply relevant biomechanical characteristics of the UCS ligaments in flexion-extension and axial rotation by quantifying length and moment arm magnitude. Moreover, 3D anatomical modeling and motion representation can help in the process of understanding of musculoskeletal behaviors of the craniovertebral junction.

Abstract:The greatest challenge to human health of recent times is the pandemic caused by the retroviruses known as HIV1 and HIV2. These have unleashed a plethora of opportunistic infections with organisms ranging from other viruses to helminths, some of which are newly recognised taxa or organisms never before identified as serious pathogens of man. Animal and bird migration as well as human travel and international trade have extended, to an alarming degree, the dissemination of other viral infections. Malaria is an increasing global challenge to human health, a reversal of the downward trend that was achieved in the latter half of the 20th century. Moreover, evidence is currently accumulating for the existence of a fifth species of Plasmodium in man. In the bacterial and protozoal fields, other novel organisms are being described. Powerful new tools of molecular biology are being deployed to probe parasite genetics, for example to reveal the genome of Plasmodium falciparum. These and molecular immunological techniques are also helping to resolve puzzling taxonomic questions, as well as aiding epidemiological studies that are revealing the unforeseen scale of human infection with a number of parasitic helminths in both tropic and temperate climatic areas. This paper presents a brief review of the major, as well as some of the relatively minor changes and advances of recent decades.
Coupled rotation patterns in cervical spine axial rotation can change when the head is kept level

Abstract:The biomechanical literature describes axial rotation occurring coupled with lateral bending and flexion in the cervical spine. Since the head is kept level during some activities of daily living, we set out to investigate the changes in total and segmental motion that occur when a level gaze constraint is applied to cadaveric cervical spine specimens during axial rotation. 1.5Nm of left and right axial rotation moment was applied to sixteen C2-T1 cadaveric specimens with C2 unconstrained and C2 constrained to simulate level gaze. Overall and segmental motions were determined using optoelectronic motion measurement and specimen-specific kinematic modeling. Without a kinematic constraint on C2, motions were as described in the literature; namely, flexion and lateral bending to the same side as axial rotation. Keeping C2 level reduced that total axial rotation range of motion of the specimens. Changes were also produced in segmental coupled rotation in all specimens. The observed changes included completely opposite coupled motion than in the uncoupled specimens, and traditional coupled behavior at one load extreme and the opposite at the other extreme. Constraining C2 during axial rotation to simulate level gaze can produce coupled motion that differs from the classically described flexion and lateral bending to the same side as axial rotation. Statement of Clinical Significance: Activities of daily living that require the head to be kept level during axial rotation of the cervical spine may produce segmental motions that are quite different from the classically described motions with implications for biomechanical experiments and implant designers.

Abstract:Spinal intervertebral joints are complex structures allowing motion in multiple directions, and many experimental studies have reported moment-rotation response. However, experimental methods, reporting of results, and levels of the spine tested vary widely, and a comprehensive assessment of moment-rotation response across all levels of the spine is lacking. This review aims to characterize moment-rotation response in a consistent manner for all levels of the human spine. A literature search was conducted in PubMed for moment versus rotation data from mechanical testing of intact human cadaveric intervertebral joint specimens in flexion-extension, lateral bending, and axial rotation. A total of 45 studies were included, providing data from testing of an estimated 1,648 intervertebral joints from 518 human cadavers. We used mixed-effects regression analysis to create 75 regression models of moment-rotation response (25 intervertebral joints x 3 directions). We found that a cubic polynomial model provides a good representation of the moment-rotation behavior of most intervertebral joints, and that compressive loading increases rotational stiffness throughout the spine in all directions. The results allow for the direct evaluation of intervertebral ranges of motion across the whole of the spine for given loading conditions. The random-effects outcomes, representing standard deviations of the model coefficients across the dataset, can aid understanding of normal variations in moment-rotation responses. Overall these results fill a large gap, providing the first realistic and comprehensive representations of moment-rotation behavior at all levels of the spine, with broad implications for surgical planning, medical device design, computational modeling, and understanding of spine biomechanics. Analysis of three-dimensional facet joint displacement during two passive upper cervical mobilizations

Abstract:Background: Understanding the 3D-kinematics of the upper cervical spine during manual mobilization is essential for clinical examination and therapy. Some information about rotational motion is available in literature but translational components are often ignored, complicating the understanding of the complex inter-segmental motions. Objectives: This study aims to describe the amount, trajectories and reproducibility of atlanto-occipital facet joints' displacement during a flexion-extension mobilization and of the atlanto-axial facet joints during an axial rotation mobilization. Design: Original research using quantitative data. Methods: 20 fresh frozen human cervical specimens were examined with a Zebris® CMS20 ultrasound-based motion tracking system. Two physiotherapists performed regionalmobilizations in flexion-extension and axial rotation. The amount of displacement and the trajectories were calculated along the XYZ axes. Difference between measurements was evaluated with the Friedman two-way ANOVA test. Intra- and inter-rater reliability were estimated through ICC scores. Results: 3D-displacement (2.6-23.4 mm) was larger at C1-C2 during axial rotation, Atlanto-occipital flexion displayed the greatest variability in the C0 trajectory. During a right rotation, the left C1 facet moved mainly forward, and the right C1 facet moved backward. During a left rotation, the left C1 facet moved backward, while the right C1 facet moved forward. Intra-tester and Inter-tester ICCs varied between 0.5 and 0.90 (p < 0.005). Conclusions: During passive spinal motion, there is an important variability in magnitude and trajectory of joints' displacement. Nevertheless, different clinicians may be able to achieve the same position at the end of the mobilization.
In vivo three-dimensional intervertebral kinematics of the subaxial cervical spine during seated axial rotation and lateral bending via a fluoroscopy-to-CT registration approach

Abstract:Accurate measurement of the coupled intervertebral motions is helpful for understanding the etiology and diagnosis of relevant diseases, and for assessing the subsequent treatment. No study has reported the in vivo, dynamic and three-dimensional (3D) intervertebral motion of the cervical spine during active axial rotation (AR) and lateral bending (LB) in the sitting position. The current study fills the gap by measuring the coupled intervertebral motions of the subaxial cervical spine in ten asymptomatic young adults in an upright sitting position during active head LB and AR using a volumetric model-based 2D-to-3D registration method via biplane fluoroscopy. Subject-specific models of the individual vertebrae were derived from each subject's CT data and were registered to the fluoroscopic images for determining the 3D poses of the subaxial vertebrae that were used to obtain the intervertebral kinematics. The averaged ranges of motion to one side (ROM) during AR at C3/C4, C4/C5, C5/C6, and C6/C7 were 4.2°, 4.6°, 3.0° and 1.3°, respectively. The corresponding values were 6.4°, 5.2°, 6.1° and 6.1° during LB. Intervertebral LB (ILB) played an important role in both AR and LB tasks of the cervical spine, experiencing greater ROM than intervertebral AR (IAR) (ratio of coupled motion (IAR/ILB): 0.23-0.75 in LB, 0.34-0.95 in AR). Compared to the AR task, the ranges of ILB during the LB task were significantly greater at C5/6 (p=0.008) and C6/7 (p=0.001) but the range of IAR was significantly smaller at C4/5 (p=0.02), leading to significantly smaller ratios of coupled motions at C4/5 (p=0.0013), C5/6 (p<0.001) and C6/7 (p=0.0037). The observed coupling characteristics of the intervertebral kinematics were different from those in previous studies under discrete static conditions in a supine position without weight-bearing, suggesting that the testing conditions likely affect the kinematics of the subaxial cervical spine. While C1 and C2 were not included owing to technical limitations, the current results nonetheless provide baseline data of the intervertebral motion of the subaxial cervical spine in asymptomatic young subjects under physiological conditions, which may be helpful for further investigations into spine biomechanics.

Abstract:This issue of the journal brings a paper with strong perspective in practice - “Blood Viscosity, Lipid Profile, and Lipid Peroxidation in Type-1 Diabetic Patients with Good and Poor Glycemic Control.”[1] Patients with diabetes show lipid disorders that promote atherogenesis, which in turn feed forward to cardiovascular disease (CVD) complications. Though the pathophysiology of these dyslipidemia is not totally explained, hyperglycemia and peripheral hyperinsulinemia are deemed to play a role. For instance, insulin has an antilipolytic action as well as the potential to promote storage of triglycerides. Hence, it is necessary to pay attention to lipid profile abnormalities in patients with type 1 diabetes in order to reduce CVD complications.[2] In referring to diabetes in this context, it is regardless whether patient is type 1 or type 2, because the biochemistry and physiology in either type has CVD implications and associated with lipid oxidation and viscosity of blood. It has been reported that erythrocytes from type 1 diabetes patients have increased membrane lipid peroxidation associated with composition and function of lipid profile, including that only individuals who do not respond to supplementation have low serum lipids.[3] That is, individuals who are not at risk of lipid peroxidation vis-a-vis oxidative stress may not show remarkable response to antioxidant. Indeed, blood viscosity in diabetic type 1 children show a trend of increasing compared with apparently healthy individual; as well as positive relationship with blood pressure.[4] Antioxidants in supplements or medical nutritional therapies are commonly used in diabetes management. However, discretion is required to avoid a paradoxical adverse effect. A “perspective in practice” papers has been previously published on this journal indicating that some guideline recommendations are yet to be adopted in clinical practice; and emphasized that since antioxidant supplement is also an alternative to aspirin and influences whole blood viscosity (WBV), there is a need to raise awareness for laboratory monitoring to be followed.[5] Thus, the promise of the blood viscosity, lipid profile and lipid peroxidation in type-1 diabetic patients with good and poor glycemic control report is in its perspective in practice. It is known that like any other pharmacotherapeutic, antiplatelets have a safety concern. Aspirin is still one of the main therapies in diabetes management and its effect is modulated by WBV, which in turn is influenced by antioxidant nutritional therapy. Utilizing the available clinical laboratory tests to determine lipid peroxidation status in patients during dietician's assessment would be keeping with guidelines. Determining WBV levels would be invaluable to optimize medical nutrition outcomes, in addition to keeping with the guidelines for antiplatelet therapy. However, the reality is that test methods for lipid peroxidation have yet to be available to most diagnostic pathology settings and also yet to be validated for diagnostic use.
Intersegmental Kinematics of the Upper Cervical Spine: Normal Range of Motion and Its Alteration After Alar Ligament Transection

Abstract:Study design: Biomechanical study using cadaveric cervical spines. Objective: To evaluate joint mobility and stiffness at the craniovertebral junction. Summary of background data: Data on the intersegmental kinematics of the craniovertebral joints are available in the literature with a widespread range of values. The effect that alar ligament injuries have on intersegmental kinematics remains unclear and requires further biomechanical investigation. Methods: Ten occipito-atlanto-axial (C0-C1-C2) human specimens were articulated to flexion, extension, bilateral lateral bending, and bilateral axial rotation. The moment-rotation response was continuously tracked through the entire range of motion before and after unilateral alar ligament transection of the right side. Results: The intersegmental (C0-C1/C1-C2) moment-rotation response was continuously quantified in full flexion (7.2 ± 6.6°/12.1 ± 5.8°), extension (11.1 ± 6.4°/3.0 ± 2.8°), lateral bending to the right (3.1 ± 2.2°/1.6 ± 1.2°) and left sides (3.3 ± 1.6°/2.1 ± 1.5°), and axial rotation to the right (1.2 ± 3.5°/32.3 ± 9.3°) and left sides (2.7 ± 2.6°/25.3 ± 8.3°). Unilateral alar ligament transection increased the range of motion of C0-C2 in the three planes of movement; however, intersegmental motion alterations were not always observed. Conclusion: Increases in the range of extension and lateral bending at C0-C1, which had not been reported previously, were observed. Further, the range of rotation on the right and left sides increased, in conjunction with the increased ranges at C0-C1 and C1-C2.Level of Evidence: N/A.
Biomechanical analysis of the cervical spine segment as a method for studying the functional and dynamic anatomy of the human neck

Abstract:Background: Traditionally, dynamic and functional anatomy, in particular the dynamic anatomy of the neck, is studied on cadaveric material. However, the development of in vivo visualization technologies and in silico modeling has made it possible to expand these possibilities. Despite significant progress in the study of dynamic and functional anatomy of the neck by means of in silico methods, the issues of validating the developed models and taking into account the pronounced nonlinearity of soft tissues as well as local anisotropy remain open. The aim of this study was to develop a virtual dynamic anatomical model of the human neck and reproduce the dynamic processes in the cervical spine from this model using the finite element method. Materials and methods: Reverse engineering was used to generate a dynamic anatomical model of the neck from CT data (both male, 24 and 22 years old). Two segments of the cervical spine (C3-C5, C2-T1) were isolated from the resulting model for finite element analysis. Finite element mesh generation and contact interactions were performed using the HyperMesh software (Altair Engineering Inc, Troy, Michigan, USA). The anisotropic hyperelastic Holzapfel-Gasser-Ogden model was used to describe the material behavior of the fibrous rings of the disc. Material modeling and finite element analysis were performed using Abaqus CAE 6.14 software (Simulia, Johnston, Rhode Island, USA). Results: A technique for creating a virtual dynamic anatomical model of the neck was elaborated and implemented. The model includes 79 major anatomical structures of the neck segmented from radiological data. A finite element analysis of the cervical spine was performed. The results of finite element analysis of the C3-C5 segment under axial load were compared with in vitro data. The proposed model shows nonlinear deformation of the disc under static loading; the model predicted displacement values agree well with the experimental ones. The displacement of the С3-С5 central vertebra with an axial load of 800 N reaches a value of 0.65 mm. For the segment C2-T1, data on intradiscal pressure, stress plots and displacements during flexion were obtained. The maximum stress value of 10.036 MPa is observed in the C3-C4 disc. Conclusion: Simulation results using the proposed methodology are in good agreement with experimental data. The generated biomechanical models allow describing dynamic phenomena in the cervical spine and obtaining a wide range of quantitative properties of anatomical objects, which are otherwise inaccessible to classical methods for studying dynamic and functional anatomy.

Abstract:Epidemiological and clinical studies show that the magnitude and scope of cervical disease are on the rise, along with the world's rising aging population. From a biomechanical perspective, the cervical spine presents a wide inter-individual variability, where its motion patterns and load sharing strongly depend on the anatomy. This study aimed to first develop and validate a geometrically patient-specific model of the lower cervical spine for clinical applications, and secondly to use the model to investigate the spinal biomechanics associated with typical cervical disorders. Based on measurements of 30 parameters from X-ray radiographs, the 3D geometry of the vertebrae and intervertebral discs (IVDs) were developed, and detailed finite element models (FEMs) of the lower ligamentous cervical spine for 6 subjects were constructed and simulated. The models were then used for the investigation of different grades of IVD alteration. The multi directional range of motion (ROM) results were in alignment with the in-vitro and in-Silico studies confirming the validity of the model. Severe disc alteration (Grade 3) presented a significant decrease in the ROM and intradiscal pressure (flexion, extension, and axial rotation) on the C5-C6 and slightly increase on the adjacent levels. Maximum stress in Annulus Fibrosus (AF) and facet joint forces increased for Grade 3 for both altered and adjacent levels. The novel validated geometrically-personalized FEM presented in this study potentially offers the clinical community a valuable quantitative tool for the noninvasive analyses of the biomechanical alterations associated with cervical spine disease towards improved surgical planning and enhanced clinical outcomes.
Estimating Facet Joint Apposition with Specimen-Specific Computer Models of Subaxial Cervical Spine Kinematics

Abstract:Computational models of experimental data can provide a noninvasive method to estimate spinal facet joint biomechanics. Existing models typically consider each vertebra as one rigid-body and assume uniform facet cartilage thickness. However, facet deflection occurs during motion, and cervical facet cartilage is nonuniform. Multi rigid-body computational models were used to investigate the effect of specimen-specific cartilage profiles on facet contact area estimates. Twelve C6/C7 segments underwent non-destructive intervertebral motions. Kinematics and facet deflections were measured. Three-dimensional models of the vertebra and cartilage thickness estimates were obtained from pre-test CT data. Motion-capture data was applied to two model types (2RB: C6, C7 vertebrae each one rigid body; 3RB: left and right C6 posterior elements, and C7 vertebrae, each one rigid body) and maximum facet mesh penetration was compared. Constant thickness cartilage (CTC) and spatially-varying thickness cartilage (SVTC) profiles were applied to the facet surfaces of the 3RB model. Cartilage apposition area (CAA) was compared. Linear mixed-effects models were used for all quantitative comparisons. The 3RB model significantly reduced penetrating mesh elements by accounting for facet deflections (p = 0.001). The CTC profile resulted in incongruent facet articulation, whereas realistic congruence was observed for the SVTC profile. The SVTC profile demonstrated significantly larger CAA than the CTC model (p < 0.001).

Clinical Relevance of Cervical Kinematic Quality Parameters in Planar Movement

Abstract:Comprehending cervical spinal motion underlies the understanding of the mechanisms of cervical disorders. We aimed to better define the clinical relevance of cervical spine kinematics, focusing on quality parameters describing cervical spine planar motion. The most common study focuses were kinematic quality parameters after cervical arthroplasty and in normal subjects, patients with cervical degeneration, and patients with cervical deformities. Kinematic quality parameters are important for cervical degeneration prevention, being detected sooner than differences on imaging examinations and being significantly related to the degree of cervical degeneration. Kinematic quality parameters are effective for evaluating the changes of cervical motion pattern after cervical fusion and non-fusion, assessing operative and adjacent segments in the early stages, and predicting adjacent segment degeneration. However, owing to current research limitations, and controversy about the changes of kinematic quality parameters after different surgical procedures, current assessments are limited to cervical spine flexion and extension. Different osteotomy methods of cervical deformity have different effects on cervical motion patterns and quality parameters. Choosing the most effective surgical method remains a challenge and kinematic quality parameters in cervical deformity are important future research topics. This review highlights the instantaneous center of rotation, the center of rotation, and the instantaneous axis of rotation as being important kinematic quality parameters of cervical spinal motion. These can be used to detect abnormal cervical mobility, to diagnose cervical degeneration, to design disc protheses, and to evaluate surgical effects earlier than other methods. Owing to limitations of research methods there is variation in the way parameters are defined by various researchers. No uniform standard exists for defining degenerative motion quality parameters in normal asymptomatic, degenerative, and postoperative patients. Therefore, further study is required. New study techniques and defining kinematic quality parameters in normal subjects will clarify the definitions of these parameters, enhancing their future clinical usefulness.

Abstract:To compare the biomechanical characters of three-level anterior cervical fusion and hybrid constructs by measuring coupled motion changes of the cervical spine, Eighteen adult human cadaveric cervical spines were biomechanically investigated under eccentric displacement control in lateral bending and axial rotation by measuring vertebral motion (X, Y, Z -axis).The 3DPD condition displayed similar coupled motion compared to the intact condition both in lateral bending and in axial rotation, while the 3PDP condition was similar only in lateral bending. However, the coupled motion of the cervical spine under 3P conditions markedly changed in both lateral bending and axial rotation. Considering the coupled motion characteristics, the 3DPD hybrid construct is biomechanically advantageous in three types of reconstructions (3DPD, 3PDP, 3P).
Intervertebral kinematics during neck motion 6.5 years after fusion and artificial disc replacement

Abstract:Background: Arthroplasty with artificial disc replacement for surgical treatment of cervical spine degeneration was introduced with the notion that motion-preserving approaches would prevent development of adjacent segment disease. Though clinical outcomes favor arthroplasty over the commonly used anterior cervical discectomy with fusion approach, clinical studies confirming the biomechanical basis of these results are lacking. The aim of this study was to compare intervertebral kinematics between arthroplasty and fusion patients 6.5 years post-surgery during physiological motion of the neck. Methods: Using a biplane dynamic X-ray system, computed tomography imaging and model based tracking algorithms, three dimensional intervertebral kinematics were measured during neck axial rotation and extension in 14 patients treated for cervical radiculopathy with fusion (n = 8) or arthroplasty (n = 6). The measurements were performed at 2-year (baseline) and 6.5 year post-surgical time points, with the main interest being in the interaction between surgery types and time points. 3 translations and 3 rotations were investigated for the index (C5C6), and upper- (C4C5) and lower adjacent levels (C6C7). Findings: Surgery-time interaction was significant for axial rotation (P < 0.04) and flexion-extension rotation (P < 0.005) in C4C5 during neck axial rotation, left-right translation (P < 0.04) in C5C6 and anterior-posterior translation in C6C7 (P < 0.04) during neck extension. In contrast with the expectations, axial rotation and flexion-extension decreased in C4C5 during neck rotation and anterior-posterior translation decreased in C6C7 during neck extension for fusion. Interpretation: The findings do not support the notion that adjacent segment motion increases after fusion.

Abstract:Background: To understand the facet capsular ligament's (FCL) role in cervical spine mechanics, the interactions between the FCL and other spinal components must be examined. One approach is to develop a subject-specific finite element (FE) model of the lower cervical spine, simulating the motion segments and their components' behaviors under physiological loading conditions. This approach can be particularly attractive when a patient's anatomical and kinematic data are available. Methods: We developed and demonstrated methodology to create 3D subject-specific models of the lower cervical spine, with a focus on facet capsular ligament biomechanics. Displacement-controlled boundary conditions were applied to the vertebrae using kinematics extracted from biplane videoradiography during planar head motions, including axial rotation, lateral bending, and flexion-extension. The FCL geometries were generated by fitting a surface over the estimated ligament-bone attachment regions. The fiber structure and material characteristics of the ligament tissue were extracted from available human cervical FCL data. The method was demonstrated by application to the cervical geometry and kinematics of a healthy 23-year-old female subject. Results: FCL strain within the resulting subject-specific model were subsequently compared to models with generic: (1) geometry, (2) kinematics, and (3) material properties to assess the effect of model specificity. Asymmetry in both the kinematics and the anatomy led to asymmetry in strain fields, highlighting the importance of patient-specific models. We also found that the calculated strain field was largely independent of constitutive model and driven by vertebrae morphology and motion, but the stress field showed more constitutive-equation-dependence, as would be expected given the highly constrained motion of cervical FCLs. Conclusions: The current study provides a methodology to create a subject-specific model of the cervical spine that can be used to investigate various clinical questions by coupling experimental kinematics with multiscale computational models.
Influence of Cervical Total Disc Replacement on Motion in the Target and Adjacent Segments

Abstract:Background Context The motion limitation after cervical discectomy and fusion alters the spine ́s kinematics. Unphysiological strains may be the result and possible explanation for adjacent segment degeneration. Alterations to cervical kinematics due to cervical total disc replacement (TDR), especially two-level, are still underinvestigated. Purpose To investigate cervical motion including coupled motions after one-level and two-level TDR in the treated and also the adjacent segments. Study Design An in-vitro study using pure moment loading of human donor spines. Methods Seven fresh frozen human cervical spine specimens (C4-T1, median age 46 with range 19-60 years, four female) were included in this study. Specimens were tested in the intact condition first, followed by one-level TDR at C5-6 which was subsequently extended one level further caudal (C5-7). Each specimen was quasistatically loaded with pure moments up to 1.5 Nm in flexion/extension (FE), lateral bending (LB) and axial rotation (AR) in a universal spine tester for 3.5 cycles at 1 °/s. During the tests three dimensional motion tracking was performed for each vertebral body individually. From that the primary and coupled ROM of each spinal level during the third full cycle of motion were evaluated. Non-parametric statistical analysis was performed using a Friedman-test and post-hoc correction with Dunn-Bonferroni-tests (p < 0.05). Ethics approval was obtained in advance. Results In FE, one-level TDR (C5-6) moderately increased primary FE in all four segments, but only significantly at the cranial adjacent level C4-5. Additional TDR at C6-7 further increased the ROM at the target segment without much influence on the other levels. Increasing implant height at C6-7 partially counteracted the increased FE. Coupled motions were minimal in all test conditions at all levels. In LB, coupled AR was observed in all test conditions at all levels. One-level TDR decreased primary LB at the target segment C5-6 significantly, without much influence on the other levels. Extending TDR to C6-7 decreased ROM in the target segment but without gaining statistical significance. Increasing implant height at C6-7 further decreased primary LB at the target segment, still without significance. Notably, coupled AR was significantly decreased at the cranial adjacent segment C4-5 compared to the intact condition. In AR, coupled LB was observed in all test conditions at the levels C4-5, C5-6 and C6-7, while the transition level to the thoracic spine C7-T1 showed only little coupled LB. Both one-level and two-level TDR showed little influence on primary AR or coupled motions at any level. Only after increasing implant height at C6-7 was the motion of the caudally adjacent level C7-T1 significantly altered. Conclusion Evaluating primary FE, LB and AR together with the associated coupled motions revealed widespread influence of cervical TDR not only on the motion of the treated level but also at the adjacent segments. The influence of two-level TDR is more widespread and involves more levels than one-level TDR. Clinical Significance The prevention of unphysiological strains due to altered kinematics after cervical fusion, which could possibly explain adjacent segment degeneration, were a driving factor in the development of TDR. These experimental findings suggest cervical TDR influences the whole cervical spine, not only the treated segment. The effect becomes more extensive, involving more levels and motion directions, after two-level than after one-level TDR.
