Abstract:Small area ecological studies are commonly used in epidemiology to assess the impact of area level risk factors on health outcomes when data are only available in an aggregated form. However the resulting estimates are often biased due to unmeasured confounders, which typically are not available from the standard administrative registries used for these studies. Extra information on confounders can be provided through external datasets such as surveys or cohorts, where the data are available at the individual level rather than at the area level; however such data typically lack the geographical coverage of administrative registries. We develop a framework of analysis which combines ecological and individual level data from different sources to provide an adjusted estimate of area level risk factors which is less biased. Our method (i) summarises all available individual level confounders into an area level scalar variable, which we call ecological propensity score (EPS), (ii) implements a hierarchical structured approach to predict the values of EPS whenever they are missing, (iii) includes the estimated and predicted EPS into the ecological regression linking the risk factors to the health outcome. Through a simulation study we show that integrating individual level data into small area analyses via EPS is a promising method to reduce the bias intrinsic in ecological studies due to unmeasured confounders; we also apply the method to a real case study to evaluate the effect of air pollution on coronary heart disease hospital admissions in Greater London.

Abstract:The health impact of long-term exposure to air pollution is now routinely estimated using spatial ecological studies, due to the recent widespread availability of spatial referenced pollution and disease data. However, this areal unit study design presents a number of statistical challenges, which if ignored have the potential to bias the estimated pollution-health relationship. One such challenge is how to control for the spatial autocorrelation present in the data after accounting for the known covariates, which is caused by unmeasured confounding. A second challenge is how to adjust the functional form of the model to account for the spatial misalignment between the pollution and disease data, which causes within-area variation in the pollution data. These challenges have largely been ignored in existing long-term spatial air pollution and health studies, so here we propose a novel Bayesian hierarchical model that addresses both challenges, and provide software to allow others to apply our model to their own data. The effectiveness of the proposed model is compared by simulation against a number of state of the art alternatives proposed in the literature, and is then used to estimate the impact of nitrogen dioxide and particulate matter concentrations on respiratory hospital admissions in a new epidemiological study in England in 2010 at the Local Authority level.

Abstract:Accounting for exposure measurement errors has been recognized as a crucial problem in environmental epidemiology for over two decades. Bayesian hierarchical models offer a coherent probabilistic framework for evaluating associations between environmental exposures and health effects, which take into account exposure measurement errors introduced by uncertainty in the estimated exposure as well as spatial misalignment between the exposure and health outcome data. While two-stage Bayesian analyses are often regarded as a good alternative to fully Bayesian analyses when joint estimation is not feasible, there has been minimal research on how to properly propagate uncertainty from the first-stage exposure model to the second-stage health model, especially in the case of a large number of participant locations along with spatially correlated exposures. We propose a scalable two-stage Bayesian approach, called a sparse multivariate normal (sparse MVN) prior approach, based on the Vecchia approximation for assessing associations between exposure and health outcomes in environmental epidemiology. We compare its performance with existing approaches through simulation. Our sparse MVN prior approach shows comparable performance with the fully Bayesian approach, which is a gold standard but is impossible to implement in some cases. We investigate the association between source-specific exposures and pollutant (nitrogen dioxide (NO$_2$))-specific exposures and birth outcomes for 2012 in Harris County, Texas, using several approaches, including the newly developed method.

Abstract:The accurate assignment of geocodes to the residences of subjects in a study population is an important component of the data acquisition/assimilation stage of many spatial epidemiological investigations. Unfortunately, however, when residential address geocoding is performed by the most common method of street-segment matching to a georeferenced road file and subsequent interpolation, positional errors of hundreds of meters are commonplace, especially in rural locations. Ignoring these errors in a statistical analysis may lead to biased estimators, a reduction in power, and incorrect conclusions. This article develops modifications to existing likelihood-based procedures for estimating the intensity of a Poisson spatial point process and the relative risk function relating two such processes, from locations ascertained without error, so as to permit valid inferences to be made from locations observed with error. The performance of the modified methods relative to methods that ignore positional errors is investigated by simulation. The methodology is applied to respiratory disease data from an Iowa county. Our investigation indicates that the magnitude of the positional error standard deviation relative to the rate of change in intensity or relative risk across the study area determines whether an analysis that accounts for positional errors will improve upon an analysis that does not; errors must be sufficiently large for an improvement to be realized.

Abstract:Environmental epidemiologists are increasingly interested in establishing causality between exposures and health outcomes. A popular model for causal inference is the Rubin Causal Model (RCM), which typically seeks to estimate the average difference in study units' potential outcomes. An important assumption under RCM is no interference; that is, the potential outcomes of one unit are not affected by the exposure status of other units. The no interference assumption is violated if we expect spillover or diffusion of exposure effects based on units' proximity to other units and several other causal estimands arise. Air pollution epidemiology typically violates this assumption when we expect upwind events to affect downwind or nearby locations. This paper adapts causal assumptions from social network research to address interference and allow estimation of both direct and spillover causal effects. We use propensity score-based methods to estimate these effects when considering the effects of the Environmental Protection Agency's 2005 nonattainment designations for particulate matter with aerodynamic diameter less than 2.5 micrograms per cubic meter (PM2.5) on lung cancer incidence using county-level data obtained from the Surveillance, Epidemiology, and End Results (SEER) Program. We compare these methods in a rigorous simulation study that considers both spatially autocorrelated variables, interference, and missing confounders. We find that pruning and matching based on the propensity score produces the highest probability coverage of the true causal effects and lower mean squared error. When applied to the research question, we found protective direct and spillover causal effects.

Abstract:When assessing the short-term effect of air pollution on health outcomes, it is common practice to consider one pollutant at a time, due to their high correlation. Multi pollutant methods have been recently proposed, mainly consisting of collapsing the different pollutants into air quality indexes or clustering the pollutants and then evaluating the effect of each cluster on the health outcome. A major drawback of such approaches is that it is not possible to evaluate the health impact of each pollutant. In this paper we propose the use of the Bayesian hierarchical framework to deal with multi pollutant concentrations in a two-component model: a pollutant model is specified to estimate the 'true' concentration values for each pollutant and then such concentration is linked to the health outcomes in a time-series perspective. Through a simulation study we evaluate the model performance and we apply the modelling framework to investigate the effect of six pollutants on cardiovascular mortality in Greater London in 2011-2012.

Abstract:Recent developments in linkage procedures and exposure modelling offer great prospects for cohort analyses on the health risks of environmental factors. However, assigning individual-level exposures to large population-based cohorts poses methodological and practical problems. In this contribution, we illustrate a linkage framework to reconstruct environmental exposures for individual-level epidemiological analyses, discussing methodological and practical issues such as residential mobility and privacy concerns. The framework outlined here requires the availability of individual residential histories with related time periods, as well as high-resolution spatio-temporal maps of environmental exposures. The linkage process is carried out in three steps: (1) spatial alignment of the exposure maps and residential locations to extract address-specific exposure series; (2) reconstruction of individual-level exposure histories accounting for residential changes during the follow-up; (3) flexible definition of exposure summaries consistent with alternative research questions and epidemiological designs. The procedure is exemplified by the linkage and processing of daily averages of air pollution for the UK Biobank cohort using gridded spatio-temporal maps across Great Britain. This results in the extraction of exposure summaries suitable for epidemiological analyses of both short and long-term risk associations and, in general, for the investigation of temporal dependencies. The linkage framework presented here is generally applicable to multiple environmental stressors and can be extended beyond the reconstruction of residential exposures.

Estimating Spatial Variation in Disease Risk from Locations Coarsened by Incomplete Geocoding

Abstract:Inference for spatial variation in relative risk of disease is an important problem in spatial epidemiologic studies. A standard component of data assimilation in these studies is the assignment of a geocode, i.e. point-level spatial coordinates, to the address of each subject in the study population. Unfortunately, when geococling is performed by the standard procedure of street-segment matching to a georeferenced road file and subsequent interpolation, it is rarely completely successful. Typically, 10-30% of the addresses in the study population fail to geocode, which can adversely affect relative risk estimation, especially if one of the disease groups (e.g. cases) has a different geocoding success rate than another (e.g. controls). The possibility exists, however, for ameliorating this effect by incorporating geographic information coarser than a point (e.g. a Zip code) that is measured for the observations that fail to geocode. This article develops coarsened-data methods for relative risk estimation from incompletely geocoded data. Nonparametric (kernel smoothing) estimation procedures are featured; parametric (likelihood-based) procedures are described as well, but their applicability is much more limited. We demonstrate, via simulation and a real example of childhood asthma cases in an Iowa county that substantial improvements in the quality of relative risk estimates are possible using the proposed nonparametric coarsened-data methods. Abstract:In the presence of unmeasured spatial confounding, spatial models may actually increase (rather than decrease) bias, leading to uncertainty as to how they should be applied in practice. We evaluated spatial modeling approaches through simulation and application to a big data electronic health record study. Whereas the risk of bias was high for purely spatial exposures (e.g., built environment), we found very limited potential for increased bias for individual-level exposures that cluster spatially (e.g., smoking status). We also proposed a novel exposure-penalized spline approach that selects the degree of spatial smoothing to explain spatial variability in the exposure. This approach appeared promising for efficiently reducing spatial confounding bias.

Abstract:We propose a new approach for estimating causal effects when the exposure is measured with error and confounding adjustment is performed via a generalized propensity score (GPS). Using validation data, we propose a regression calibration (RC)-based adjustment for a continuous error-prone exposure combined with GPS to adjust for confounding (RC-GPS). The outcome analysis is conducted after transforming the corrected continuous exposure into a categorical exposure. We consider confounding adjustment in the context of GPS subclassification, inverse probability treatment weighting (IPTW) and matching. In simulations with varying degrees of exposure error and confounding bias, RC-GPS eliminates bias from exposure error and confounding compared to standard approaches that rely on the error-prone exposure. We applied RC-GPS to a rich data platform to estimate the causal effect of long-term exposure to fine particles (PM2.5) on mortality in New England for the period from 2000 to 2012. The main study consists of 2202 zip codes covered by 217,660 1 km × 1 km grid cells with yearly mortality rates, yearly PM2.5 averages estimated from a spatio-temporal model (error-prone exposure) and several potential confounders. The internal validation study includes a subset of 83 1 km × 1 km grid cells within 75 zip codes from the main study with error-free yearly PM2.5 exposures obtained from monitor stations. Under assumptions of noninterference and weak unconfoundedness, using matching we found that exposure to moderate levels of PM2.5 (8 < PM2.5 ≤ 10 μg/m3) causes a 2.8% (95% CI: 0.6%, 3.6%) increase in all-cause mortality compared to low exposure (PM2.5 ≤ 8 μg/m3).

Abstract:In this work we present a statistical approach to distinguish and interpret the complex relationship between several predictors and a response variable at the small area level, in the presence of i) high correlation between the predictors and ii) spatial correlation for the response. Covariates which are highly correlated create collinearity problems when used in a standard multiple regression model. Many methods have been proposed in the literature to address this issue. A very common approach is to create an index which aggregates all the highly correlated variables of interest. For example, it is well known that there is a relationship between social deprivation measured through the Multiple Deprivation Index (IMD) and air pollution; this index is then used as a confounder in assessing the effect of air pollution on health outcomes (e.g. respiratory hospital admissions or mortality). However it would be more informative to look specifically at each domain of the IMD and at its relationship with air pollution to better understand its role as a confounder in the epidemiological analyses. In this paper we illustrate how the complex relationships between the domains of IMD and air pollution can be deconstructed and analysed using profile regression, a Bayesian non-parametric model for clustering responses and covariates simultaneously. Moreover, we include an intrinsic spatial conditional autoregressive (ICAR) term to account for the spatial correlation of the response variable.

Abstract:In a large-scale environmental health population study that is composed of subprojects, often different fractions of participants out of the total enrolled have measures of specific outcomes. It's conceptually reasonable to assume the association study would benefit from utilizing additional exposure information from those with a specific outcome not measured. Partial least squares regression is a practical approach to determine the exposure-outcome associations for mixture data. Like a typical regression approach, however, the partial least squares regression requires that each data observation must have both complete covariate and outcome for model fitting. In this paper, we propose novel adjustments to the general partial least squares regression to estimate and examine the association effects of individual environmental exposure to an outcome within a more complete context of the study population's environmental mixture exposures. The proposed framework takes advantage of the bilinear model structure. It allows information from all participants, with or without the outcome values, to contribute to the model fitting and the assessment of association effects. Using this proposed framework, incorporation of additional information will lead to smaller root mean square errors in the estimation of association effects, and improve the ability to assess the significance of the effects.

Methodology

Abstract:We develop new methodology to improve our understanding of the causal effects of multivariate air pollution exposures on public health. Typically, exposure to air pollution for an individual is measured at their home geographic region, though people travel to different regions with potentially different levels of air pollution. To account for this, we incorporate estimates of the mobility of individuals from cell phone mobility data to get an improved estimate of their exposure to air pollution. We treat this as an interference problem, where individuals in one geographic region can be affected by exposures in other regions due to mobility into those areas. We propose policy-relevant estimands and derive expressions showing the extent of bias one would obtain by ignoring this mobility. We additionally highlight the benefits of the proposed interference framework relative to a measurement error framework for accounting for mobility. We develop novel estimation strategies to estimate causal effects that account for this spatial spillover utilizing flexible Bayesian methodology. Empirically we find that this leads to improved estimation of the causal effects of air pollution exposures over analyses that ignore spatial spillover caused by mobility.

Abstract:In epidemiological disease mapping one aims to estimate the spatio-temporal pattern in disease risk and identify high-risk clusters, allowing health interventions to be appropriately targeted. Bayesian spatio-temporal models are used to estimate smoothed risk surfaces, but this is contrary to the aim of identifying groups of areal units that exhibit elevated risks compared with their neighbours. Therefore, in this paper we propose a new Bayesian hierarchical modelling approach for simultaneously estimating disease risk and identifying high-risk clusters in space and time. Inference for this model is based on Markov chain Monte Carlo simulation, using the freely available R package CARBayesST that has been developed in conjunction with this paper. Our methodology is motivated by two case studies, the first of which assesses if there is a relationship between Public health Districts and colon cancer clusters in Georgia, while the second looks at the impact of the smoking ban in public places in England on cardiovascular disease clusters.
A Bayesian localised conditional auto-regressive model for estimating the health effects of air pollution

Abstract:Estimation of the long-term health effects of air pollution is a challenging task, especially when modelling small-area disease incidence data in an ecological study design. The challenge comes from the unobserved underlying spatial correlation structure in these data, which is accounted for using random effects modelled by a globally smooth conditional autoregressive model. These smooth random effects confound the effects of air pollution, which are also globally smooth. To avoid this collinearity a Bayesian localised conditional autoregressive model is developed for the random effects. This localised model is flexible spatially, in the sense that it is not only able to model step changes in the random effects surface, but also is able to capture areas of spatial smoothness in the study region. This methodological development allows us to improve the estimation performance of the covariate effects, compared to using traditional conditional auto-regressive models. These results are established using a simulation study, and are then illustrated with our motivating study on air pollution and respiratory ill health in Greater Glasgow, Scotland in 2010. The model shows substantial health effects of particulate matter air pollution and income deprivation, whose effects have been consistently attenuated by the currently available globally smooth models.

Abstract:ABSTRACT Environmental epidemiologic studies routinely utilize aggregate health outcomes to estimate effects of short-term (eg, daily) exposures that are available at increasingly fine spatial resolutions. However, areal averages are typically used to derive population-level exposure, which cannot capture the spatial variation and individual heterogeneity in exposures that may occur within the spatial and temporal unit of interest (eg, within a day or ZIP code). We propose a general modeling approach to incorporate within-unit exposure heterogeneity in health analyses via exposure quantile functions. Furthermore, by viewing the exposure quantile function as a functional covariate, our approach provides additional flexibility in characterizing associations at different quantile levels. We apply the proposed approach to an analysis of air pollution and emergency department (ED) visits in Atlanta over 4 years. The analysis utilizes daily ZIP code-level distributions of personal exposures to 4 traffic-related ambient air pollutants simulated from the Stochastic Human Exposure and Dose Simulator. Our analyses find that effects of carbon monoxide on respiratory and cardiovascular disease ED visits are more pronounced with changes in lower quantiles of the population’s exposure. Abstract:Introduction: The environmental health effects have been studied for decades, where model-based methods have been standard for analyzing exposure-outcome associations. Methods: We propose a complementary approach to examine causal effects of environmental exposures on health outcomes based on insights from classical experimental design that rely on modern computing. We illustrate our approach using two examples, the effect of extreme hot temperatures on cardiovascular mortality, and the effect of parental smoking on children's lung function. Our approach proposes an analysis strategy with four transparent, distinct, and ordered stages: 1) The precise formulation of the causal question (and related assumptions) in terms of a hypothetical randomized experiment using a potential outcomes framework. 2) A design stage that attempts to reconstruct the ideal conditions for a randomized experiment (that is, with unconfounded exposure assignment). 3) A statistical analysis comparing the outcomes of interest in the exposed versus unexposed units of the hypothetical randomized experiment. 4) A conclusion about evidence for or against the hypothesized causal question. Results: When investigating the causal effect of extreme temperatures on cardiovascular mortality in Los Angeles, we find similar effect estimates using the standard regression approach and our alternative approach (e.g., the estimated difference in daily mortality is 0.05, comparing days with Tmax>30C to milder days). However, we find different conclusions when we examine the causal effect of parental smoking in a classical dataset on children's FEV-1 (the estimated causal effect equals to -0.34 [95%CI:-0.59; -0.09] using our approach (here, matched-sampling) vs. -0.09 [95%CI:-0.20; 0.03] using standard regression). Conclusions: Because of its conceptual links to (hypothetical) interventions, our approach can suggest the effects of policies for reducing environmental pollutants and thereby preventing diseases.
Accounting for survey design in Bayesian disaggregation of survey-based areal estimates of proportions: an application to the American Community Survey

Abstract:Understanding the effects of social determinants of health on health outcomes requires data on characteristics of the neighborhoods in which subjects live. However, estimates of these characteristics are often aggregated over space and time in a fashion that diminishes their utility. Take, for example, estimates from the American Community Survey (ACS), a multi-year nationwide survey administered by the U.S. Census Bureau: estimates for small municipal areas are aggregated over 5-year periods, whereas 1-year estimates are only available for municipal areas with populations $>$65,000. Researchers may wish to use ACS estimates in studies of population health to characterize neighborhood-level exposures. However, 5-year estimates may not properly characterize temporal changes or align temporally with other data in the study, while the coarse spatial resolution of the 1-year estimates diminishes their utility in characterizing neighborhood exposure. To circumvent this issue, in this paper we propose a modeling framework to disaggregate estimates of proportions derived from sampling surveys which explicitly accounts for the survey design effect. We illustrate the utility of our model by applying it to the ACS data, generating estimates of poverty for the state of Michigan at fine spatio-temporal resolution.

Abstract:Summary With the rise in popularity of digital Atlases to communicate spatial variation, there is an increasing need for robust small area estimates. However, current small area estimation methods suffer from various modelling problems when data are very sparse or when estimates are required for areas with very small populations. These issues are particularly heightened when modelling proportions. Additionally, recent work has shown significant benefits in modelling at both the individual and area levels. We propose a two‐stage Bayesian hierarchical small area estimation approach for proportions that can account for survey design, reduce direct estimate instability and generate prevalence estimates for small areas with no survey data. Using a simulation study, we show that, compared with existing Bayesian small area estimation methods, our approach can provide optimal predictive performance (Bayesian mean relative root mean squared error, mean absolute relative bias and coverage) of proportions under a variety of data conditions, including very sparse and unstable data. To assess the model in practice, we compare modelled estimates of current smoking prevalence for 1,630 small areas in Australia using the 2017–2018 National Health Survey data combined with 2016 census data.
