Abstract:Background: The frequency of mass gatherings is increasing. Such events often involve many people and carry the risk of mass casualty incidents, which require substantial medical resources from various healthcare institutions. The current medical system, while meeting daily needs, struggles to address the demand for a high volume of emergency resources and real-time data exchange. Objective: The aim of this study was to develop an emergency medical information system for mass gatherings. Methods: We developed an emergency medical information system for mass gatherings. Based on a unified prehospital and intrahospital emergency data exchange protocol, we can directly standardize medical information data and provide data support for the evacuation decision support algorithms of multiple institutions. Wearable devices, vehicle-mounted devices, video calling systems and surveillance systems are connected to capture real-time scenes. Results: We constructed the system via mobile applications and online platforms and deployed it in 3 hospitals, 5 ambulances and 17 on-site medical locations. We constructed a set of electronic medical records covering the whole first aid process according to the basic principles of first aid. The simulation results show that the proposed algorithm is suitable for mass gatherings. The overall survival rate of patients can be improved by 5 %, and the average evacuation efficiency of patients can be improved by 50 %. Furthermore, in a real-world environment, this method can ensure patient survival and achieve good convergence. Conclusions: Our system is capable of providing robust medical information support for emergency medical services during large-scale assembly events, ensuring a visualized full-process emergency response and decision-making for the diversion and subsequent transport of a large patient population.

Abstract:Planning in advance to prepare for and respond to a natural hazard-induced disaster-related emergency is a key action that allows decision makers to mitigate unexpected impacts and potential damage. To further this aim, a collaborative, modular, and information and communications technology-based Spatial Data Infrastructure (SDI) called SIRENE—Sistema Informativo per la Preparazione e la Risposta alle Emergenze (Information System for Emergency Preparedness and Response) is designed and implemented to access and share, over the Internet, relevant multisource and distributed geospatial data to support decision makers in reducing disaster risks. SIRENE flexibly searches and retrieves strategic information from local and/or remote repositories to cope with different emergency phases. The system collects, queries, and analyzes geographic information provided voluntarily by observers directly in the field (volunteered geographic information (VGI) reports) to identify potentially critical environmental conditions. SIRENE can visualize and cross-validate institutional and research-based data against VGI reports, as well as provide disaster managers with a decision support system able to suggest the mode and timing of intervention, before and in the aftermath of different types of emergencies, on the basis of the available information and in agreement with the laws in force at the national and regional levels. Testing installations of SIRENE have been deployed in 18 hilly or mountain municipalities (12 located in the Italian Central Alps of northern Italy, and six in the Umbria region of central Italy), which have been affected by natural hazard-induced disasters over the past years (landslides, debris flows, floods, and wildfire) and experienced significant social and economic losses.

Abstract:Background: In the last decades an increasing number of West Nile Disease cases was observed in equines and humans in the Mediterranean basin and surveillance systems are set up in numerous countries to manage and control the disease. The collection, storage and distribution of information on the spread of the disease becomes important for a shared intervention and control strategy. To this end, a Web Geographic Information System has been developed and disease data, climatic and environmental remote sensed data, full genome sequences of selected isolated strains are made available. This paper describes the Disease Monitoring Dashboard (DMD) web system application, the tools available for the preliminary analysis on climatic and environmental factors and the other interactive tools for epidemiological analysis. Methods: WNV occurrence data are collected from multiple official and unofficial sources. Whole genome sequences and metadata of WNV strains are retrieved from public databases or generated in the framework of the Italian surveillance activities. Climatic and environmental data are provided by NASA website. The Geographical Information System is composed by Oracle 10g Database and ESRI ArcGIS Server 10.03; the web mapping client application is developed with the ArcGIS API for Javascript and Phylocanvas library to facilitate and optimize the mash-up approach. ESRI ArcSDE 10.1 has been used to store spatial data. Results: The DMD application is accessible through a generic web browser at https://netmed.izs.it/networkMediterraneo/. The system collects data through on-line forms and automated procedures and visualizes data as interactive graphs, maps and tables. The spatial and temporal dynamic visualization of disease events is managed by a time slider that returns results on both map and epidemiological curve. Climatic and environmental data can be associated to cases through python procedures and downloaded as Excel files. Conclusions: The system compiles multiple datasets through user-friendly web tools; it integrates entomological, veterinary and human surveillance, molecular information on pathogens and environmental and climatic data. The principal result of the DMD development is the transfer and dissemination of knowledge and technologies to develop strategies for integrated prevention and control measures of animal and human diseases.

Abstract:The risks associated with zoonotic infections transmitted by companion animals are a serious public health concern: the control of zoonoses incidence in domestic dogs, both owned and stray, is hence important to protect human health. Integrated dog population management (DPM) programs, based on the availability of information systems providing reliable data on the structure and composition of the existing dog population in a given area, are fundamental for making realistic plans for any disease surveillance and action system. Traceability systems, based on the compulsory electronic identification of dogs and their registration in a computerised database, are one of the most effective ways to ensure the usefulness of DPM programs. Even if this approach provides many advantages, several areas of improvement have emerged in countries where it has been applied. In Italy, every region hosts its own dog register but these are not compatible with one another. This paper shows the advantages of a web-based-application to improve data management of dog regional registers. The approach used for building this system was inspired by farm animal traceability schemes and it relies on a network of services that allows multi-channel access by different devices and data exchange via the web with other existing applications, without changing the pre-existing platforms. Today the system manages a database for over 300,000 dogs registered in three different Italian regions. By integrating multiple Web Services, this approach could be the solution to gather data at national and international levels at reasonable cost and creating a traceability system on a large scale and across borders that can be used for disease surveillance and development of population management plans.

Abstract:Objective Coronavirus disease 2019 demonstrated the inconsistencies in adequately responding to biological threats on a global scale due to a lack of powerful tools for assessing various factors in the formation of the epidemic situation and its forecasting. Decision support systems have a role in overcoming the challenges in health monitoring systems in light of current or future epidemic outbreaks. This paper focuses on some applied examples of logistic planning, a key service of the Earth Cognitive System for Coronavirus Disease 2019 project, here presented, evidencing the added value of artificial intelligence algorithms towards predictive hypotheses in tackling health emergencies. Methods Earth Cognitive System for Coronavirus Disease 2019 is a decision support system designed to support healthcare institutions in monitoring, management and forecasting activities through artificial intelligence, social media analytics, geospatial analysis and satellite imaging. The monitoring, management and prediction of medical equipment logistic needs rely on machine learning to predict the regional risk classification colour codes, the emergency rooms attendances, and the forecast of regional medical supplies, synergically enhancing geospatial and temporal dimensions. Results The overall performance of the regional risk colour code classifier yielded a high value of the macro-average F1-score (0.82) and an accuracy of 85%. The prediction of the emergency rooms attendances for the Lazio region yielded a very low root mean square error (<11 patients) and a high positive correlation with the actual values for the major hospitals of the Lazio region which admit about 90% of the region's patients. The prediction of the medicinal purchases for the regions of Lazio and Piemonte has yielded a low root mean squared percentage error of 16%. Conclusions Accurate forecasting of the evolution of new cases and drug utilisation enables the resulting excess demand throughout the supply chain to be managed more effectively. Forecasting during a pandemic becomes essential for effective government decision-making, managing supply chain resources, and for informing tough policy decisions.

First Prototype of the Infectious Diseases Seeker (IDS) Software for Prompt Identification of Infectious Diseases

Abstract:The rapid detection of ongoing outbreak - and the identification of causative pathogen - is pivotal for the early recognition of public health threats. The emergence and re-emergence of infectious diseases are linked to several determinants, both human factors - such as population density, travel, and trade - and ecological factors - like climate change and agricultural practices. Several technologies are available for the rapid molecular identification of pathogens [e.g. real-time polymerase chain reaction (PCR)], and together with on line monitoring tools of infectious disease activity and behaviour, they contribute to the surveillance system for infectious diseases. Web-based surveillance tools, infectious diseases modelling and epidemic intelligence methods represent crucial components for timely outbreak detection and rapid risk assessment. The study aims to integrate the current prevention and control system with a prediction tool for infectious diseases, based on regression analysis, to support decision makers, health care workers, and first responders to quickly and properly recognise an outbreak. This study has the intention to develop an infectious disease regressive prediction tool working with an off-line database built with specific epidemiological parameters of a set of infectious diseases of high consequences. The tool has been developed as a first prototype of a software solution called Infectious Diseases Seeker (IDS) and it had been established in two main steps, the database building stage and the software implementation stage (MATLAB® environment). The IDS has been tested with the epidemiological data of three outbreaks occurred recently: severe acute respiratory syndrome epidemic in China (2002-2003), plague outbreak in Madagascar (2017) and the Ebola virus disease outbreak in the Democratic Republic of Congo (2018). The outcomes are promising and they reveal that the software has been able to recognize and characterize these outbreaks. The future perspective about this software regards the developing of that tool as a useful and user-friendly predictive tool appropriate for first responders, health care workers, and public health decision makers to help them in predicting, assessing and contrasting outbreaks.
A Geospatial Artificial Intelligence and satellite-based earth observation cognitive system in response to COVID-19

Abstract:The pandemic emergency caused by the spread of COVID-19 has stressed the importance of promptly identifying new epidemic clusters and patterns, to ensure the implementation of local risk containment measures and provide the needed healthcare to the population. In this framework, artificial intelligence, GIS, geospatial analysis and space assets can play a crucial role. Social media analytics can be used to trigger Earth Observation (EO) satellite acquisitions over potential new areas of human aggregation. Similarly, EO satellites can be used jointly with social media analytics to systematically monitor well-known areas of aggregation (green urban areas, public markets, etc.). The information that can be obtained from the Earth Cognitive System 4 COVID-19 (ECO4CO) are both predictive, aiming to identify possible new clusters of outbreaks, and at the same time supervisorial, by monitoring infrastructures (i.e. traffic jams, parking lots) or specific categories (i.e. teenagers, doctors, teachers, etc.). In this perspective, the technologies described in this paper will allow us to detect critical areas where individuals can be involved in risky aggregation clusters. The ECO4CO data lake will be integrated with ad hoc data obtained by health care structures to understand trends and dynamics, to assess criticalities with respect to medical response and supplies, and to test possibilities useful to tackle potential future emergencies. The System will also provide geographical information on the spread of the infection which will allow an appropriate context-specific public health response to the epidemic. This project has been co-funded by the European Space Agency under its Business Applications programme.

Abstract:The agricultural, forestry, and animal production domain (AFA domain) plays an essential role in meeting global needs and supporting livelihoods while facing escalating challenges from climate change-induced impacts and extreme natural events. This perspective advocates for urgent strategies to enhance resilience through effective emergency management and prevention measures tailored to this critical domain. The analysis here exposed, which includes elements of ontology and the conceptual approach of an emergency management system encompassing both restoration and prevention aspects, entails three case studies across the AFA domain. Each case study, described by location, timing, nature, and consequences, critically evaluates the implemented risk prevention measures, details the emergency and recovery actions, and highlights shortcomings in response efforts. The analysis, incorporating a retrospective comparative component based on the proposed conceptual model, highlights the importance of identifying lessons learned and potential future applications. It emphasizes the urgent need for a well-structured emergency management strategy that integrates risk mapping and advanced technology to ensure timely and effective responses. The active engagement of domain professionals (agronomists, foresters, animal production doctors) and scholars of AFA domain sciences, as either farm owners or technical advisors, is crucial to optimize intervention strategies. This engagement is especially important for enhancing resilience during recovery phases, aligning with the best international practices such as making use of local knowledge and citizen engagement strategies. Comprehensive training initiatives, also adopting innovative formats and tools including micro-credentials, e-learning platforms, and the applications of generative Artificial Intelligence for learning assistance, as well as new research insights are strategic for coordinated and effective emergency responses across all stakeholders. Collaboration between the different production systems and areas of expertise, raising awareness of the distinction between Civil Protection and Production Protection and fostering their close interconnection, is essential for effective emergency response and long-term resilience.

Assessing the use of animal health platforms: User's needs, preferences and constraints

Abstract:Animal health information systems or risk analysis tools are indispensable not only for animal health surveillance, but also to observe the evolution and risk of disease incursion into a disease-free area. Given the various information that can be derived from these both animal information systems and risk analysis tools, different international and national organizations have customized or created their own systems/tools to provide specific information for use by the respective countries. Moreover, with the increase of technology and data storage, they have become more accessible and widely used by professionals in animal and human health sciences. This study aimed to establish user's preferences, needs and constraints in respect of these animal information systems and risk analysis tools. An online survey was conducted and answered by 213 respondents from 132 countries. The respondents were animal health or public health professionals in different employment sectors (mostly in government, research and university institutions) and various fields of competency (highest for animal and public health). The majority of respondents used the animal health information systems frequently and on a weekly basis, with prevention measures of diseases being regarded as the most useful information. Descriptive epidemiology was more used/needed than analytical epidemiology. Risk analysis was performed by the majority of the respondents (70%), using a qualitative approach more than a quantitative or semi-qualitative. The primary objectives were to produce risk assessment and preparedness in areas involving origin and spread of animal diseases. The features most sought after in risk analysis tools were pathways of introduction and spread assessment. The level of satisfaction was higher for the platform which is most used by the respondents. Overall, these results could be taken into consideration when improving an already available platform, or when creating a new efficient tool.
Integrated Satellite System for Fire Detection and Prioritization

Abstract:Several studies have shown the relevance of satellite systems in detecting, monitoring, and characterizing fire events as support to fire management activities. On the other hand, up to now, only a few satellite-based platforms provide immediately and easily usable information about events in progress, in terms of both hotspots, which identify and localize active fires, and the danger conditions of the affected area. However, this kind of information is usually provided through separated layers, without any synthetic indicator which, indeed, could be helpful, if timely provided, for planning the priority of the intervention of firefighting resources in case of concurrent fires. In this study, we try to fill these gaps by presenting an Integrated Satellite System (ISS) for fire detection and prioritization, mainly based on the Robust Satellite Techniques (RST), and the Fire Danger Dynamic Index (FDDI), an original re-structuration of the Índice Combinado de Risco de Incêndio Florestal (ICRIF), for the first time presented here. The system, using Moderate Resolution Imaging Spectroradiometer (MODIS), Advanced Very High Resolution Radiometer (AVHRR), and Spinning Enhanced Visible and InfraRed Imager (SEVIRI) data, provides near real-time integrated information about both the fire presence and danger over the affected area. These satellite-based products are generated in common formats, ready to be ingested in Geographic Information System (GIS) technologies. Results shown and discussed here, on the occasion of concurrent winter and summer fires in Italy, in agreement with information from independent sources, demonstrate that the ISS system, operating at a regional/national scale, may provide an important contribution to fire prioritization. This may result in the mitigation of fire impact in populated areas, infrastructures, and the environment.

Abstract:New crisis response and management approaches that incorporate the latest information technologies are essential in all phases of emergency preparedness and response, including the planning, response, recovery, and assessment phases. Accurate and timely information is as crucial as is rapid and coherent coordination among the responding organizations. We are working towards a multipronged emergency response tool that provide stakeholders timely access to comprehensive, relevant, and reliable information. The faster emergency personnel are able to analyze, disseminate and act on key information, the more effective and timelier their response will be and the greater the benefit to affected populations. Our tool consists of encoding multiple layers of open source geospatial data including flood risk location, road network strength, inundation maps that proxy inland flooding and computer vision semantic segmentation for estimating flooded areas and damaged infrastructure. These data layers are combined and used as input data for machine learning algorithms such as finding the best evacuation routes before, during and after an emergency or providing a list of available lodging for first responders in an impacted area for first. Even though our system could be used in a number of use cases where people are forced from one location to another, we demonstrate the feasibility of our system for the use case of Hurricane Florence in Lumberton, North Carolina.

Abstract:Objective To build a decision-making system for prevention and treatment of hemorrhagic fever with renal syndrome(HFRS) using spatial information technology and information visualization technology.Methods Under the support of Windows XP system,ArcGIS Engine 9.2 and Visual Basic 6.0 were used to develop modules for a visualization system,which compiled geographic database,attribute database,and knowledge database with each other closely by Activex Data Object.This visualization system,with geographic information system(GIS) as the core of information,can transform HFRS-related information into knowledge which is important for decision makers.Results The decision-making system for the prevetion and treatment of HFRS,which could comprehensively integrate HFRS monitoring data,animal host data and related geographical environment data and provide tools for field epidemiology investigation,data management and analysis,differential diagnosis,animal host identification,mapping and data export functionality for facilitating multi-departmental data sharing,was established.Conclusion Combining the geospatial data with the diversity of epidemiological data and combining the traditional GIS with multi-information visualization technologies could lead to effective application of spatial information technology in the decision-making of disease prevention and treatment.

Abstract:Background/Introduction: The World Health Organization (WHO) declared climate change a defining issue in the 21st century with more intense heatwaves, higher risks of flooding and damaging storms, and a changing pattern of emerging infectious diseases. In this scenario, the response of Emergency Medical Teams (EMTs) to disasters represents a fundamental resource. Objectives: To expand EMT2-ITA-Regione Piemonte operational independence and to minimize its environmental footprint. Method/Description: A multiphasic and prospective project is planned in order to: (1) Reduce water consumption: use of a sterilizer designed with a set of high-efficiency heat exchangers enabling a substantial saving in water consumption by the vacuum pump and a significant reduction of total water usage through a recirculation system. (2) Reduce demand for diesel: photovoltaic (PV) system to integrate the current energy production system based on diesel generators. (3) Reduce paper consumption: use of sterilization management and traceability system and computerized medical record in order to be paperless. (4) Improve staff awareness and education on greening practices: educational program for the staff focused on waste segregation/management and energy and water saving both in the hospital and in the Base of Operation (BoO). Results/Outcomes: EMT2-ITA-Regione Piemonte aims to reduce energy and water consumption by 30% and to become paperless. Conclusion: Advances in greening initiatives offer to EMT2-ITA-Regione Piemonte the potential to improve its disaster medical response capabilities and to reduce its ecological footprint.

Abstract:The present paper aims at introducing the authors' development of an emergency management system for SARS, which is based on the GIS and decision support system. The paper has also given its discussion on the organization pattern of the system and the model of the system. When discussing the working principle of the emergency management system, first of all, the suggested emergency management system is to be recognized as a type of DSS, which is suggested to reduce the influence of the likely occuring disasters. The function of the system includes the potential incidence diagnosis, dynamic estimation of the epidemic spreading, and analysis on the outcome of controlling policies. Secondly, the system needs to be based on GIS, since the disaster development is different in space (the spatial dynamics has to be analyzed in an emergency system). For this purpose, the coupling strategy based on the Component Object Model (COM) technology is also suggested to be included into the system.The model system developed in Fig.1 based on the above principle is used to predict the epidemic situation of the following month on the regional scale and to simulate the influence of regional restriction policy. Other models include: the knowledge-based model for SARS prediction by analysing climatic risk; the epidemic spreading model based on regional reciprocity, aiming at analysing the tendency of the contagion of SARS between different regions; the regional economic spillover model to estimate the influence of restriction policy on the development of regional economy, and the dynamic estimate model to predict the future epidemic situation within each region .The parameters of the above models can be predicted by Chinese facts. Finally, technical problems associated with establishing emergency system based on GIS are also discussed in the paper. The results of our work reveal that the system is feasible by joining the COM technology (MapObjects -the standard GIS component used in the system) with programming environment Visual Basic 6.0 and data base software Microsoft Access technology; the flow chart of the system mode has been given in Fig.2. For example, we explore and evaluate the SARS situation in 2003, and the results were recommended to the corresponding governmental departments.

Abstract:SUMMARY We developed a syndromic surveillance (SyS) concept using emergency dispatch, ambulance and emergency-department data from different European countries. Based on an inventory of sub-national emergency data availability in 12 countries, we propose framework definitions for specific syndromes and a SyS system design. We tested the concept by retrospectively applying cumulative sum and spatio-temporal cluster analyses for the detection of local gastrointestinal outbreaks in four countries and comparing the results with notifiable disease reporting. Routine emergency data was available daily and electronically in 11 regions, following a common structure. We identified two gastrointestinal outbreaks in two countries; one was confirmed as a norovirus outbreak. We detected 1/147 notified outbreaks. Emergency-care data-based SyS can supplement local surveillance with near real-time information on gastrointestinal patients, especially in special circumstances, e.g. foreign tourists. It most likely cannot detect the majority of local gastrointestinal outbreaks with few, mild or dispersed cases.

Abstract:Due to increasing herd sizes and automation on dairy farms there is an important need for automated monitoring of cow production, health, and welfare. Despite much progress in automatic monitoring techniques, there is still a need to integrate data from multiple sources to create a comprehensive overview and accurate diagnosis of a cow's state. To aid the technological development of data integration, a prototype of an open and customizable automatic system that integrates data from multiple sensors relating to barn environment and cow behaviour was developed. The system integrates data from sensors that measure barn climate (e.g., temperature, humidity, wind speed), air quality (e.g., CO 2 concentration), water use and temperature, the moisture and temperature of the litter and cow behaviour (e.g., lying, eating, ruminating). An external weather system and video recording system are also included. The system's architecture consists of four main elements: sensors, nodes, gateways, and backend. The data are recorded by sensors, then locally processed on custom-developed sensor nodes, and then transmitted via radio channels to local gateways that combine the data from multiple nodes and transmit them to distributed digital storage ("the cloud") via a 3G/4G cellular network. On the cloud, the data are further processed and stored in a database. The data are then presented to the user continuously and in real time on a dashboard that can be accessed via the internet. In the design of the local wireless network, care was taken to avoid data packet collision and thus to minimize data loss. To test the system's performance, the system was installed and operated on three commercial dairy cattle farms for one year. The system provided high data stability with minimal loss and outliers, showing that the system is reliable and suitable for long term application on commercial dairy farms. The system's architecture, communication network, and data processing and visualization applications form an open framework for research and development purposes, allowing it to be customized and fine-tuned before being deployed as a management assistant on commercial dairy farms. Missing elements that should be added in the future are the integration of the data from the milking parlour and cow identification. Algorithms to integrate information from multiple sensors can be added to provide a comprehensive system that monitors all aspects related to cow welfare, health, and production automatically, remotely and in real time, thereby supporting farmers in important management decision-making.
