Abstract:Background: The ultimate goal of advanced procedures in prosthetic dentistry is to construct a prosthesis based on the accurate reproduction of condylar guidance. Aim: To compare the sagittal condylar inclination obtained using the cone-beam computed tomography (CBCT), panoramic image, and two articulator systems. Settings and design: A cross-sectional study was carried out in 20 dentate patients in the age group of 20-40 years. Materials and methods: Sagittal condylar guidance angles (SCGAs) were measured on two semi-adjustable articulators (Hanau Wide Vue and Denar Mark 320) using protrusive interocclusal record. SCGAs were also measured on the CBCT scans (CBCT reconstructed panoramic image and CBCT sagittal cross section) and the panoramic images obtained from the patients. The angles were determined on the radiographs by joining two lines: Frankfort's horizontal plane and the other plane were drawn by connecting the superior-most point on the glenoid fossa and the inferior-most point on the articular eminence. All the measurements were done twice by two operators independently. Statistical analysis used: The data were analyzed using the paired t-test and Pearson's correlation coefficient with a P ≤ 0.05. Results: There was no significant difference between the right and the left side in any of the groups (P ≥ 0.05). In addition, there was no significant difference between the clinical methods (P ≥ 0.05). However, there was a significant difference between the clinical and the radiographic methods (P ≤ 0.05). Furthermore, there was a strong correlation between the clinical and radiographic methods with Pearson's Correlation coefficient above 0.67. Conclusion: There is a strong correlation between the clinical and radiographic methods. Thus, CBCT and OPG can be used an adjunct to clinical methods to record sagittal condylar guidance.
An investigation of the impact of speech transcript errors on HMM voices

Abstract:Toward automatic creation of web-based voice fonts at low cost, automatic speech transcription technology is used to obtain the linguistic features for building HMM-based voices from audio web contents. This paper presents an investigation of the influences of erroneous transcripts on such voices. We simulate varied speech transcript errors by using a large vocabulary automatic speech recognizer (LVASR) to dictate thousands of Japanese utterances from two speakers (a male and a female). A set of experiments is conducted on dozens of HMM voices built upon both dictated and correct transcripts. The results indicate a significant impact of speech transcript errors on the voices. One direct impact is increasing the number of leaf nodes of the decision trees associated with both state duration and F0 but decreasing that with cepstrum in comparison with the reference voices by correct transcripts. The HMM voice quality in mean opinion scores (MOS) is closely related to the word and phone accuracy of speech transcriptions. To achieve fair voice quality with limited training samples, for example, the word and phone accuracy must be higher than 50% and 80%, respectively.

Abstract:INTRODUCTION Can a person who has lost the ability to communicate following a stroke (dysphasic) make a will? The answer to this question depends very much on an understanding of what dysphasia is and a knowledge of the legal requirements for making a will. The prevalence of dysphasia, 150 per 100,000 population, along with an increasingly affluent ageing society, necessitates that doctors, speech and language therapists and legal advisers become more familiar with this area. When considering this situation, Critchley, in 1970 (1), concluded that "the literature of both law and neurology proves to be surprisingly unhelpful". Conflicts can occur as there is a common perception that those who cannot speak cannot think. On the other hand, there have been cases where an over simple view of dysphasia has been taken, ignoring the covert difficulties frequently found accompanying this language deficit. This paper will outline the problems associated with dysphasia, as well as the legal requirements for making a will. A recommended procedure is given to assist those involved in ensuring that a patient in this position is given the most appropriate advice and assistance.
Renal effects of ketanserin in essential hypertension.

Abstract:The effects of maintenance treatment with ketanserin 40 mg twice daily on renal haemodynamics were studied in 13 essential hypertensives. To establish whether activation of the renin system modifies the response to ketanserin the patients were studied under three conditions: on a sodium restricted diet combined with hydrochlorothiazide, on a sodium restricted diet without hydrochlorothiazide and on a liberal sodium diet. Blood pressure fell in a proportion of the patients on all three regimens. Neither effective renal plasma flow (ERPF; clearance of 131I-hippuran) nor glomerular filtration rate (GFR; clearance of 125I-iothalamate) was significantly altered by ketanserin on either regimen. Neither the blood pressure response nor the renal response was modified by renin stimulation. The renal response was not related to the fall in blood pressure observed in some patients. Therefore ketanserin probably does not affect renal autoregulation.

Abstract:Toxoplasmosis is a potentially fatal opportunistic infection of immunocompromised hosts. Improved animal models of toxoplasmosis are needed to more nearly approximate conditions that occur in immunocompromised humans. The development of models of toxoplasmosis using human peripheral blood lymphocytes (hu‐PBL) transplanted into severe combined immunodeficiency (SCID) mice is described here. Transplantation of hu‐PBL into SCID mice without prior conditioning of the mice resulted in detectable differences in quantitative histological scores of brain inflammation due to Toxoplasma gondii infection, but did not alter mortality when compared to SCID mouse controls. The lack of detectable differences in survival were due to inadequate engraftment of hu‐PBL, as assessed by flow cytometry. Unconditioned hu‐PBL SCID mice had low titre T. gondii‐ specific antibody detectable after infection. When pretransplantation conditioning with irradiation and antiasialo GM 1 (n‐glucolyl neuraminic acid) antibody was used, prolonged hu‐PBL engraftment was observed in SCID mice, which was associated with worsened histopathology and usually impaired survival when compared with SCID mouse controls. When pretransplantation conditioning with irradiation, antiasialo GM antibody and polyethylene glycol‐conjugated IL‐2 was used, prolonged hu‐PBL engraftment was also documented, but this did not affect survival from T. gondii infection when compared with similarly conditioned SCID mouse controls. The latter conditioning protocol resulted in hu‐PBL SCID mice producing high titre T. gondii‐specific antibody after infection. Conditioned hu‐PBL SCID mice had evidence of increased T. gondii‐induced inflammatory scores when compared with conditioned SCID mice. These models show promise for the study of the pathogenesis of toxoplasmosis and conditioned hu‐PBL SCID mice may have applications for the evaluation of novel therapies for toxoplasmosis in immunocompromised humans.

Abstract:Aim: The Aim of the study was to evaluate the reproducibility of condylar inclination values obtained using three different interocclusal recording materials in Hanau Wide-Vue semi-adjustable articulator and to compare those values of condylar inclination with panoramic radiographic images.Settings and Design: In Vivo Comparative study.Materials and Methods: The clinical trial was carried out in the Department of Prosthodontics, M. R Ambedkar Dental College and Hospital, Bengaluru. A total of 30 dentulous subjects aged between 20 and 40 years were selected, without any signs and symptoms of temporomandibular joint disorders. With the help of a custom-made protrusive guide, protrusive records for three materials were made. These records were used for programming the articulator and the respective horizontal condylar inclination (HCI) value, and panoramic radiographs were made. The values obtained were statistically analyzed.Statistical Analysis Used: Independent t-test, multiple comparisons using Bonferroni method followed by Analysis of variances (ANOVA) were used.Results: Among each pair of groups there exists a significant difference, i.e., the mean condylar inclination (degree) was found to be statistically significant between wax and polyvinyl siloxane (P < 0.01) as well as between wax and polyether (P < 0.01). No significant difference was observed between polyvinyl siloxane and polyether (P > 0.05).Conclusion: HCI values differed depending on the type of material employed. The study concluded that the protrusive condylar guidance angles obtained by panoramic radiograph can be used in programming the semi-adjustable articulator and among the materials polyether gave the highest Condylar Guidance values with less variability and wax gave lowest values with greater variation.
Complicated Relationships between Anterior and Condylar Guidance and Their Clinical Implications-Comparison by Cone Beam Computed Tomography and Electronic Axiography-An Observational Cohort Cross-Sectional Study

Abstract:A complex prosthodontic treatment is believed to be more successful when the condylar path is replicated using the articulator. However, there is an ongoing major disagreement between the researchers as the exact relationship between the posterior and anterior determinants has not been clear. The purpose of this study was to investigate whether the protrusive movement of the mandible does correlate with the temporomandibular joint (TMJ) anatomy or with incised features. Subjects (15 males and 15 females) were qualified for this study based on an initial interview including the following criteria: age 21-23 (+/-1), no history of trauma, orthodontic treatment, or temporomandibular disorders (TMD). For each patient, the angle of the condylar path, incisal guidance angle (IGA), interincisal angle, as well as overbite and overjet were measured on cone beam computed tomography (CBCT). This was followed by the examination with the Modjaw® electronic axiograph recording and calculating the functional sagittal condylar guidance angle (SCGA) for the right and left TMJ during the protrusion. The results show that the mean functional axiographic measurement of SCGA in protrusion significantly correlates with the TMJ anatomy presented on CBCT. Moreover, a significant correlation was found between the values of SCGA in the functional and anatomical measurements in all its variants. It turned out that, statistically, the AB measurement was the most accurate. Finally, results showed that incisal relationships of permanent teeth such as overbite, overjet, incisal guidance angle and interincisal angle do not correlate with TMJ anatomy, and therefore, regarding an analyzed study group, do not affect the TMJ formation in young adults.

Abstract:Background: Temporomandibular joint (TMJ) is a ginglymo-diarthroidial joint with fibroelastic cartilage. The chondrogenesis initiates from the 12th week of intrauterine life and the development of condyle is associated with growth. The condylar cortication shows distinct morphological variation for each individual in each stage of their life. The cortical bone around the condyle could be used as a factor for chronological age assessment and it can act as a tool in forensic medicine. Aim and Objective: The study was carried out to evaluate the cortical grading in mandibular condyle using two different applications and to correlate their grades with chronological age. Setting and Design: Hospital-based retrospective observational cross-sectional study. Materials and Methods: The study was carried out in 40 patients and 80 TMJs were assessed for cortication grades in Carestream 3D imaging and Image J applications. These grading from both the applications were correlated with the chronological age. Statistical Analysis: SPSS (Statistical Analysis for the Social Science) – Cohen's Kappa inter-examiner reliability and Spearman's correlation coefficient were used. Results: The radiological assessment of condylar cortication in individual application showed significant results and the relationship of cortication with chronological age showed a significant correlation. Conclusion: The condylar cortication grading is a simple technique and can be used as a factor for chronological age assessment. This is an initial study which used two different applications to view the cortication of the mandibular condyle and to correlate the cortication with chronological age. Hence, a large sample size-based study is required for further research.

Exactly what is early diagnosis; how is it measured; and by what criteria is it to be evaluated? Surely it is more than just recognition of secondary complications such as acute abdomen from volvulus or of intractable anemia from hemorrhage in colon cancer. And surely its value is more than just economic or social gains to the individual or his community. We find ourselves rather like Alice reposed on the chessboard between the Red Queen and White Queen with the White King in check and neither side seeming to notice. Collaboration between industry and research institutes/universities on industrial innovation in Beijing, China

Abstract:Abstract Most R&D resources in China were allocated to public research institutes/universities until the economic transition of the mid 1990s. To maximize the return from these resources, it is important to have a healthy collaboration between industry and research institutes/universities on industrial innovation. This paper examines that relationship and discusses some empirical evidence on its efficiency with particular reference to industry in Beijing. Following a survey of 950 industrial enterprises, the influences of the collaboration relationship on industrial innovation were analyzed. The main findings indicate that the technology novelty of industrial innovation is positively related to that relationship, i.e. the more the collaboration, the higher the technology novelty of the innovation. However, the collaboration relationship is less efficient in terms of economic performance indicators such as innovation sales and profit ratios, to measure innovation. Moreover, the collaboration relationship is still far from efficient in stimulating industrial innovation in China. The major barriers to successful collaboration have also been addressed in this paper with the aim of devising policies and suggesting possible improvements to collaboration efficiency.

Abstract:Background: During prosthodontic treatment, the averaged values of the transversal condylar inclination (TCI) and the sagittal condylar inclination (SCI) are used for articulator settings. This study evaluated different parameters of measurable mandibular movements according to skeletal classes. Methods: Seventy-five patients (mean age 30.8 ± 5.49) had a condylography using the Cadiax Compact device (Gamma Dental, Klosterneuburg, Austria) and cephalometric analysis performed. Statistical analysis was performed using R statistical software. Results: There was no statistical evidence to state that the value of SCI angle is different in I compared to II Skeletal Class. There were no statistically significant differences when comparing the I vs. III and II vs. III Skeletal Class. The lowest mean SCI angle values were found in subjects with Skeletal Class III. There were statistically significant differences in left-sided TCI between Class I and II. There was a statistically significant linear relationship between ANB angle value and SCI value. Conclusions: Due to the wide individual variation in SCI and TCI values, it is advisable to use individual measurable parameters of mandibular movements during prosthetic reconstructions. The statistically significant relationship between SCI and ANB angle can be used to individualize the articulating parameters, especially in significant skeletal disproportions.
Condylar guidance: correlation between articular morphology and panoramic radiographic images in dry human skulls

Abstract:Statement of problem: Several studies have shown the unreliability of recording and reproducing the condylar guidance in semiadjustable articulators. Purpose: The purpose of this study was to determine the correlation between the anatomic shape of the articular eminence and the corresponding panoramic radiographic image in dry skulls. Material and methods: Two metal wires were adapted and fixed to the inner and outer surfaces of the articular eminences in 25 human skulls. The inner (thicker) wire was fixed to the middle of the most concave aspect of the articular eminence in an anterior-posterior direction. The outer (thinner) wire was attached to the inferior aspect of the zygomatic arch adjacent to the articular eminence. Panoramic radiographic images were recorded by the same operator at the same time and with the same panoramic radiographic unit (Orthoralix SD). Impressions were made of the condylar fossae in 25 human dry skulls. Tracings of the incline of the articular eminence on the panoramic radiographs and the impression sections were compared. Each measurement was repeated twice by 2 operators. The results were subjected to the Pearson correlation statistical analysis (alpha=.05). Results: The outline of the articular eminence in a panoramic radiographic image was identified as a thinner superior radio-opaque line above a thicker radio-opaque outline of the inferior-lateral aspect of the zygomatic arch. A significant correlation was found between the sagittal inclination of the anatomic articular eminence outline and the corresponding panoramic radiographic image for both right (R=0.802; P=.001) and left (R=0.561; P=.004) sides. A significant correlation was found between articular eminences on left and right sides in the same skull (P=.025). Conclusions: The panoramic radiographic image of the sagittal inclination of the articular eminence consistently replicated the eminence inclinations in the 25 human skulls evaluated.

Reliability and accuracy of a method for measuring temporomandibular joint condylar volume

Abstract:Objective: The aim of this study was to develop and validate a technique for mandibular condyle segmentation and volume determination by using cone beam computed tomography (CBCT). Study design: A dry skull was used to generate 3 dimensional (3-D)-printed mandible models that were then imaged by using CBCT. Semiautomatic segmentation of condyles was completed. The Frankfurt plane was established and translated to the most inferior point of the sigmoid notch, and the condylar volume superior to the plane was determined. This procedure was repeated on 3-D-printed mandibles by using physical landmarks and the water displacement method to obtain the physical volume. This was repeated 3 times to evaluate reliability. Sensitivity analysis was performed to demonstrate the effect of discrepancies in locating landmarks in the Frankfurt plane. Condylar volume measurements obtained from CBCT were compared with physical measurements through repeated-measures analysis of variance (ANOVA) to determine accuracy. Results: Condylar volume obtained from CBCT and physical measurements resulted in an intraclass correlation coefficient of 0.988 (0.918, 0.998) (P < .01) with both modalities, demonstrating excellent intrarater reliability. The mean difference of volume measurements between the modalities was not statistically significant (P = .365). Potential discrepancies in porion coordinates had minimal impact on condylar volume change. Conclusions: The condylar segmentation technique proved to be a reliable and accurate method for evaluating condylar volume.
Evaluation of mandibular condyle position in Class III patients after bimaxillary orthognathic surgery: A cone-beam computed tomography study

Abstract:Objective: This retrospective study evaluated the mandibular condyle position before and after bimaxillary orthognathic surgery performed with the mandibular condyle positioned manually in patients with mandibular prognathism using cone-beam computed tomography. Methods: Overall, 88 mandibular condyles from 44 adult patients (20 female and 24 male) diagnosed with mandibular prognathism due to skeletal Class III malocclusion who underwent bilateral sagittal split ramus osteotomy (BSSRO) and Le Fort I performed using the manual condyle positioning method were included. Cone-beam computed tomography images obtained 1-2 weeks before (T0) and approximately 6 months after (T1) surgery were analyzed in three planes using 3D Slicer software. Statistical significance was set at P < 0.05 level. Results: Significant inward rotation of the left mandibular condyle and significant outward rotation of the right mandibular condyle were observed in the axial and coronal planes (P < 0.05). The positions of the right and left condyles in the sagittal plane and the distance between the most medial points of the condyles in the coronal plane did not differ significantly (P > 0.05). Conclusions: While the change in the sagittal plane can be maintained as before surgery with manual positioning during the BSSRO procedure, significant inward and outward rotation was observed in the axial and coronal planes, respectively, even in the absence of concomitant temporomandibular joint disorder before or after the operation. Further long-term studies are needed to correlate these findings with possible clinical consequences.

Abstract:BACKGROUND: The optimal position of the condyle in glenoid fossa is a fundamental question in dentistry. There is no quantitative standard for the optimal position of mandibular condyle in the glenoid fossa in our population. The purpose of this study is to assess the position of the condyle by cone beam computed tomography (CBCT) images in patient with normal function of temporomandibular joint (TMJ).MATERIALS AND METHODS: In this cross-sectional study, CBCT images of 40 class I skeletal patients (15 males and 25 females) without history of TMJ disorders were selected. Next, the anterior, superior and posterior joint spaces (Ajs, Sjs, Pjs) were measured on the two true central sagittal slices. Then medial (M) and lateral (L) joint spaces on true coronal view were measured in the right and left sides, separately. After that, P/A ratio, S/A ratio and M/L ratio were calculated. Finally, a paired t-test and independent samples t-test were employed for analysis.RESULTS: The centric position of the condyle in glenoid fossa was more common (92.5%) than other positions. Significant differences in Ajs, Sjs, Pjs, Mjs and Ljs values between two sides were observed (P ≤ 0.05). Additionally, Sjs showed statistically significant differences between the sexes (P = 0.05). P/A ratio and S/A ratio had significant differences between two sides but not between those sexes.CONCLUSION: The assessment of joint spaces in right and left sides should be done independently. Overall, the measured joint spaces except Sjs are not different in two sexes. The data from this study could be a useful and comparable reference for the clinical assessment of condylar position in patients with normal functional joints.

Abstract:Identifying landscape elements that influence gene flow and migration in wild species is the current main topic of landscape genetics. Most landscape genetic studies infer gene flow and migration from genetic distances among populations or individuals and statistically relate these measurements to landscape composition and configuration. This approach assumes symmetrical gene flow between pairs of populations. Such an assumption, however, will often be violated, especially in source–sink systems. Source populations provide more emigrants than they receive immigrants, and sink populations get many immigrants, but release few emigrants. Source–sink dynamics cannot be explored using common landscape genetic approaches relying on genetic distances. In this issue of Molecular Ecology, Andreasen et al. ( ) apply an alternative approach allowing them to infer asymmetrical migration. They use a Bayesian assignment test among objectively defined populations of mountain lions (Puma concolor) in western USA to estimate recent and directional migration rates. The study shows that an area with a high amount of wildlife refuges and low hunting pressure harbours a source population for mountain lion dispersal, while areas with high hunting pressures form sink populations; a result helpful in making informed decisions in conservation management.
