Abstract:During tumor resection, doctors use intraoperative biopsies to determine the tumor margin. However, the pathological procedures of traditional diagnostic methods, such as imprint cytology and frozen section analysis, are complicated and time-consuming. As this is not conducive to surgeries, their applications are limited to a large extent. Therefore, novel fast microscopy imaging technologies with resolutions comparable to those of pathological tissue sections are necessary. Stimulated Raman scattering (SRS), photoacoustic microscopy (PAM), multiphoton microscopy (MPM), and optical coherence microscopy (OCM) exhibit the advantages of high spatial resolution, large imaging depth, avoiding damage to biological tissues, label-free detection, and the availability of biochemical information of tissues. Additionally, they are superior to intraoperative biopsies owing to their fast imaging speeds. Therefore, they possess broad application prospects in tumor resection surgeries and the diagnosis of other diseases. This study briefly introduces the basic principles, structural characteristics, advantages and disadvantages, and the existing research status of SRS, PAM, MPM, and OCM in biomedicine. Furthermore, we propose a multi-mode hybrid detection technology that can be used for surgeries. The combination of the proposed technology with deep learning-based artificial intelligence can form the basis for intraoperative diagnosis in the future.
Specimen Mammography for Intraoperative Margin Assessment in Breast Conserving Surgery: a Meta-Analysis

Abstract:In breast conserving surgery (BCS), specimen mammography is one of the most widely used intraoperative methods of assessing margin status. We performed a meta-analysis to evaluate the diagnostic accuracy of specimen mammography. Literature databases including PubMed, Cochrane Library, Web of Science, and EMBASE were searched prior to Jun 2022. A total of 1967 patients were included from 20 studies. A pooled analysis, heterogeneity testing, threshold effect testing, publication bias analysis, and subgroup analyses were performed from extracted data. The pooled weighted values were a sensitivity of 0.55 (95% confidence interval [CI], 0.47-0.63), a specificity of 0.85 (95% CI, 0.78-0.90), a diagnostic odds ratio of 7 (95% CI, 4-12), and a pooled positive likelihood ratio of 3.7 (95% CI 2.6-5.5). The area under the receiver operator characteristic curve was 0.75 (95% CI 0.71-0.78). In the subgroup analysis, the pooled specificity in the positive margin defined as tumor at margin subgroup was lower than the other positive margin definition subgroup (0.82 [95% CI: 0.71, 0.92] vs. 0.87 [95% CI: 0.80, 0.94], p = 0.01). Our findings indicated that specimen mammography was an accurate intraoperative imaging technique for margin assessment in BCS.

Abstract:Abstract Successful treatment of solid cancers relies on complete surgical excision of the tumor either for definitive treatment or before adjuvant therapy. Intraoperative and postoperative radial sectioning, the most common form of margin assessment, can lead to incomplete excision and increase the risk of recurrence and repeat procedures. Mohs Micrographic Surgery is associated with complete removal of basal cell and squamous cell carcinoma through real-time margin assessment of 100% of the peripheral and deep margins. Real-time assessment in many tumor types is constrained by tissue size, complexity, and specimen processing / assessment time during general anesthesia. We developed an artificial intelligence platform to reduce the tissue preprocessing and histological assessment time through automated grossing recommendations, mapping and orientation of tumor to the surgical specimen. Using basal cell carcinoma as a model system, results demonstrate that this approach can address surgical laboratory efficiency bottlenecks for rapid and complete intraoperative margin assessment.

Abstract:There is an unmet clinical need for an accurate, rapid and reliable tool for margin assessment during breast-conserving surgeries. Ultrasound offers the potential for a rapid, reproducible, and non-invasive method to assess margins. However, it is challenged by certain drawbacks, including a low signal-to-noise ratio, artifacts, and the need for experience with the acquirement and interpretation of images. A possible solution might be computer-aided ultrasound evaluation. In this study, we have developed new ensemble approaches for automated breast tumor segmentation. The ensemble approaches to predict positive and close margins (distance from tumor to margin ≤ 2.0 mm) in the ultrasound images were based on 8 pre-trained deep neural networks. The best optimum ensemble approach for segmentation attained a median Dice score of 0.88 on our data set. Furthermore, utilizing the segmentation results we were able to achieve a sensitivity of 96% and a specificity of 76% for predicting a close margin when compared to histology results. The promising results demonstrate the capability of AI-based ultrasound imaging as an intraoperative surgical margin assessment tool during breast-conserving surgery.

Abstract:A positive margin may result in an increased risk of local recurrences after breast retention surgery for any malignant tumour. In order to reduce the number of positive margins would offer surgeon real-time intra-operative information on the presence of positive resection margins. This study aims to design an intra-operative tumour margin evaluation scheme by using specimen mammography in breast-conserving surgery. Total of 30 cases were evaluated and compared with the manually determined contours by experienced physicians and pathology report. The proposed method utilizes image thresholding to extract regions of interest and then performs a deep learning model, i.e. SegNet, to segment tumour tissue. The margin width of normal tissues surrounding it is evaluated as the result. The desired size of margin around the tumor was set for 10 mm. The smallest average difference to manual sketched margin (6.53 mm +- 5.84). In the all case, the SegNet architecture was utilized to obtain tissue specimen boundary and tumor contour, respectively. The simulation results indicated that this technology is helpful in discriminating positive from negative margins in the intra-operative setting. The aim of proposed scheme was a potential procedure in the intra-operative measurement system. The experimental results reveal that deep learning techniques can draw results that are consistent with pathology reports.

Abstract:Positive surgical margin (PSM) is a frequent concern for surgeons performing radical prostatectomy for prostate cancer (PCa). PSM are recognized as risk factors for earlier biochemical recurrence and expose patients to adjuvant or salvage treatments such as external radiotherapy and hormonotherapy. Several strategies have been established to reduce PSM rate, while still allowing safe nerve-sparing surgery. Precise preoperative staging by multiparametric magnetic resonance imaging (mpMRI) and fusion biopsy is recommended to identify suspicious areas of extracapsular extension (ECE) that warrant special attention during dissection. However, even with optimal imaging, ECE can be missed, some cancers are not well defined or visible, and capsular incision during surgery remains an issue. Hence, intraoperative frozen section techniques, such as the neurovascular structure-adjacent frozen section examination (NeuroSAFE) have been developed and lately widely disseminated. The NeuroSAFE technique reduces PSM rate while allowing higher rate of nerve-sparing surgery. However, its use is limited to high volume or expert center because of its high barrier-to-entry in terms of logistics, human resources and expertise, as well as cost. Also, NeuroSAFE is a time-consuming process, even in expert hands. To address these issues, several technologies have been developed for an ex vivo and in vivo use. Ex vivo technology such as fluorescent confocal microscopy and intraoperative PET-CT require the extraction of the specimen for preparation, and digital images acquisition. In vivo technology, such as augmented reality based on mpMRI images and PSMA-fluorescent guided surgery have the advantage to provide an intracorporeal analysis of the completeness of the resection. The current manuscript provides a narrative review of established techniques, and details several new and promising techniques for intraoperative PSM assessment.

Abstract:Background: The assessment information of tumor margins is extremely important for the success of the breast cancer surgery and whether the patient undergoes a second operation. However, conducting surgical margin assessments is a time-consuming task that requires pathology-related skills and equipment, and often cannot be provided in a timely manner. To address this challenge, digital breast tomosynthesis technology was utilized to generate detailed cross-sectional images of the breast tissue and integrate deep learning algorithms for image segmentation, achieving an assessment of tumor margins during surgery. Methods: this study utilized post-operative tissue samples from 46 patients who underwent breast-conserving treatment, and generated image sets using digital breast tomosynthesis for the training and evaluation of deep learning models. Results: Deep learning algorithms effectively identifying the tumor area. They achieved a Mean Intersection over Union (MIoU) of 0.91, global accuracy of 99%, weighted IoU of 44%, precision of 98%, recall of 83%, F1 score of 89%, and dice coefficient of 93% on the training dataset; for the testing dataset, MIoU was at 83%, global accuracy at 97%, weighted IoU at 38%, precision at 87%, recall rate at 69%, F1 score at 76%, dice coefficient at 86%. Conclusions: The initial evaluation suggests that the deep learning-based image segmentation method is highly accurate in measuring breast tumor margins. This helps provide information related to tumor margins during surgery, and by using different datasets, this research method can also be applied to the surgical margin assessment of various types of tumors.

Abstract:INTRODUCTION: For early-stage breast cancer, breast-conserving surgery (BCS) plus radiation is standard-of-care. Nationwide, >20% of BCS patients require re-excision for positive margins, resulting in delayed adjuvant therapy, increased complications, emotional and financial stress for patients, and additional cost to the healthcare system. Although several methods may be employed to mitigate positive margins, no technique can fully address the need. MarginProbe® is an adjunctive tool for real-time intraoperative margin assessment and is shown to reduce positive margins by >50%.AREAS COVERED: Discussion of the impact of re-excision following BCS, a review of currently available methods for intraoperative margin management, followed by a technology and literature review of the MarginProbe® Radiofrequency Spectroscopy System.EXPERT OPINION: Re-excision significantly impacts patients, providers and payers. Limitations in the ability to assess margins at time of surgery warrant more advanced methods of residual disease detection. MarginProbe facilitates the most efficient pathway for breast cancer patients through the surgical phase of treatment. The device is well-suited for adoption as the healthcare focus shifts from volume to value and supports the three pillars of the US Department of Health and Human Services' 'Triple-Aim' strategy: improve population health, improve patient experience of care, and reduce per-capita costs.

Abstract:Purpose There are a wide variety of intraoperative techniques available in breast surgery to achieve low rates for positive margins of excision. The objective of this systematic review was to determine the pooled diagnostic accuracy of intraoperative breast margin assessment techniques that have been evaluated in clinical practice. Methods This study was performed in accordance with PRISMA guidelines. A systematic search of the literature was conducted to identify studies assessing the diagnostic accuracy of intraoperative margin assessment techniques. Only clinical studies with raw diagnostic accuracy data as compared with final permanent section histopathology were included in the meta-analysis. A bivariate model for diagnostic meta-analysis was used to determine overall pooled sensitivity and specificity. Results Sixty-one studies were eligible for inclusion in this systematic review and meta-analysis. Cytology demonstrated the best diagnostic accuracy, with pooled sensitivity of 0.92 (95 % CI 0.77–0.98) and a pooled specificity of 0.95 (95 % CI 0.90–0.97). The findings also indicate good diagnostic accuracy for optical spectroscopy, with a pooled sensitivity of 0.86 (95 % CI 0.76–0.93) and a pooled specificity of 0.92 (95 % CI 0.82–0.97). Conclusion Pooled data indicate that optical spectroscopy, cytology and frozen section have the greatest diagnostic accuracy of currently available intraoperative margin assessment techniques. However, long turnaround time for results and their resource intensive nature has prevented widespread adoption of these methods. The aim of emerging technologies is to compete with the diagnostic accuracy of these established techniques, while improving speed and usability.

Abstract:Assessing the surgical margin during breast lumpectomy operations can avoid the need for additional surgery. Optical coherence tomography (OCT) is an imaging technique that has been proven to be efficient for this purpose. However, to avoid overloading the surgeon during the operation, automatic cancer detection at the surface of the removed tissue is needed. This work explores automated margin assessment on a sample of patient data collected at the Pathology Department, Severance Hospital (Seoul, South Korea). Some methods based on the spatial statistics of the images have been developed, but the obtained results are still far from human performance. In this work, we investigate the possibility to use deep neural networks (DNNs) for real time margin assessment, demonstrating performance significantly better than the reported literature and close to the level of a human expert. Since the goal is to detect the presence of cancer, a patch-based classification method is proposed, as it is sufficient for detection, and requires training data that is easier and cheaper to collect than for other approaches such as segmentation. For that purpose, we train a DNN architecture that was proved to be efficient for small images on patches extracted from images containing only cancer or only normal tissue as determined by pathologists in a university hospital. As the number of available images in all such studies is by necessity small relative to other deep network applications such as ImageNet, a good regularization method is needed. In this work, we propose to use a recently introduced function norm regularization that attempts to directly control the function complexity, in contrast to classical approaches such as weight decay and DropOut. As neither the code nor the data of previous results are publicly available, the obtained results are compared with reported results in the literature for a conservative comparison. Moreover, our method is applied to locally collected data on several data configurations. The reported results are the average over the different trials. The experimental results show that the use of DNNs yields significantly better results than other techniques when evaluated in terms of sensitivity, specificity, F1 score, G-mean and Matthews correlation coefficient. Function norm regularization yielded higher and more robust results than competing regularization methods. We have demonstrated a system that shows high promise for (partially) automated margin assessment of human breast tissue, Equal error rate (EER) is reduced from approximately 12% (the lowest reported in the literature) to 5% - a 58% reduction. The method is computationally feasible for intraoperative application (less than 2 s per image) at the only cost of a longer offline training time.

Abstract:Positive resection margins occur in approximately 25% of breast cancer (BCa) surgeries, requiring re-operation. Margin status is not routinely available during surgery; thus, technologies that identify residual cancer on the specimen or cavity are needed to provide intraoperative decision support that may reduce positive margin rates. Rapid evaporative ionization mass spectrometry (REIMS) is an emerging technique that chemically profiles the plume generated by tissue cauterization to classify the ablated tissue as either cancerous or non-cancerous, on the basis of detected lipid species. Although REIMS can distinguish cancer and non-cancerous breast tissue by the signals generated, it does not indicate the location of the classified tissue in real-time. Our objective was to combine REIMS with spatio-temporal navigation (navigated REIMS), and to compare performance of navigated REIMS with conventional histopathology examination (gold standard) to assess margin status (positive or negative) in patients undergoing surgery for BCa. A multivariate model was trained using pathology-validated mass spectra from ex vivo resection specimens from 11 patients, including 36 spectra from invasive cancer, and 118 spectra from normal breast adipose. This model was subsequently tested retrospectively on intraoperative data from 25 BCa cases that were conducted using navigated REIMS. Navigation was facilitated by an electromagnetic sensor placed on the cautery. To record the position of the tumor, a localization wire fitted with an electromagnetic sensor was placed into the tumor using ultrasound prior to surgery, and a 3D map of the tumor was created. This enabled real-time knowledge of the cautery position relative to the tumor during surgery. Spectra from the plume classified as BCa were mapped onto a display of the tumor region and compared with the pathology report. Our multivariate classifier exhibited >90% accuracy on cross-validation, driven by an elevated ratio of glycerophospholipid:triglyceride in cancer as compared with normal adipose. In the intraoperative testing cohort, 4/150 margins were assessed by pathology as positive, all of which were correctly identified by REIMS (Sensitivity=100%). Notably, two of these cases were positive for ductal carcinoma in situ. 146/150 margins were negative, of which 131/146 negative margins were consistent with histopathology (Specificity=90%). There were 13 negative margins that were determined to be positive by REIMS (false positive), of which 8 margins were noted as either 'close margins' (with cancer cells being detected within 1 mm of the inked margin on histopathology), or as high-density normal breast tissue. Other normal tissue such as skin and muscle also exhibited high phospholipid content but these spectra could be rationalized by relative distance from the tumor region using the navigation data and/or information provided by the surgeon in real-time. We have demonstrated the importance of spatio-temporal tracking and histopathology to validate intraoperative REIMS. Furthermore, we have shown the feasibility of navigated REIMS in identifying margins containing cancer in real-time during BCa surgery, where a wider excision may be desirable. Abstract:BackgroundPositive/close margins are associated with higher in-breast failure rates after breast-conserving surgery (BCS). We investigated whether intraoperative margin assessment aids in obtaining negative margins, and to evaluate the local control thus achieved.MethodsBetween 1994 and 1996, 264 patients underwent BCS for stages 0–III breast cancer [invasive, n = 200; ductal carcinoma in situ (DCIS), n = 64]. Intraoperative margin assessment included gross tissue inspection, specimen radiography, with or without frozen section.ResultsNinety-two patients (46%) with invasive cancer and 24 (38%) with DCIS had positive/close margins on the permanent section analysis of their initial surgical specimens. Fifty-eight patients (29%) with invasive cancer and six (9%) with DCIS had initial positive/close margins, and were rendered margin-negative by intraoperative analysis and immediate re-excision. Final margins on permanent pathology were positive/close in 52 patients (20%): 34 patients (17%) with invasive cancer and 18 patients (28%) with DCIS. By multivariate analysis, excisional biopsy for diagnosis, larger tumor size, and multifocality were associated with final positive/close margins. Of these 52 patients, 23 underwent a second operation to achieve widely negative margins (13 completion mastectomies, 10 re-excisions). The 5-year ipsilateral breast recurrence-free survival rates after BCS and radiation were 99% for invasive cancer (n = 167) and 100% for DCIS (n = 27).ConclusionsIntraoperative assessment of margins assisted in identifying positive/close margins and allowed over a quarter of the patients to be rendered margin-negative with intraoperative re-excision at their original operation. This approach resulted in excellent local control in patients treated with BCS and radiation.

Abstract:New statistical methods were employed to improve the ability to distinguish benign from malignant breast tissue ex vivo in a recent study. The ultimately aim was to improve the intraoperative assessment of positive tumour margins in breast-conserving surgery (BCS), potentially reducing patient re-operation rates. A multivariate Bayesian classifier was applied to the waveform samples produced by a Terahertz Pulsed Imaging (TPI) handheld probe system in order to discriminate tumour from benign breast tissue, obtaining a sensitivity of 96% and specificity of 95%. We compare these results to traditional and to state-of-the-art methods for determining resection margins. Given the general nature of the classifier, it is expected that this method can be applied to other tumour types where resection margins are also critical.

Effect of Intraoperative Imprint Cytology Followed by Frozen Section on Margin Assessment in Breast-Conserving Surgery

Abstract:PurposeIntraoperative margin assessment can reduce positive margins in patients undergoing breast-conserving surgery. However, reports on intraoperative margin assessment have described only the use of either imprint cytology or frozen section. This study was designed to elucidate the effect of intraoperative margin assessment using imprint cytology followed by frozen section.MethodsOverall, 522 patients were enrolled. First, the entire surgical margin was subjected to imprint cytology. Frozen section was performed only in cases with “positive” or “suspicious” imprint cytology results. An additional intraoperative excision was performed in patients with frozen section-positive lesion sites. All margins were evaluated using postoperative permanent sections after excision.ResultsAmong 522 patients, 136 (26.1%) were imprint cytology-positive, and 386 (73.9%) were imprint cytology-negative. Among the 386 imprint cytology-negative patients not subjected to frozen section, 11 (2.1%) were permanent sections-positive (imprint cytology-false-negative). In 47 of the 136 imprint cytology-positive patients, additional intraoperative excision was unnecessary due to the frozen section-negative diagnosis. Moreover, these patients could avoid reoperation, because they were permanent section-negative. The false-positive rate of imprint cytology alone was 13.4%, but adding frozen section to imprint cytology decreased the overall false-positive rate to 2.5%. After undergoing excision, four patients still had positive margins. The overall positive margin rate in the final pathology based on permanent sections was 2.9% (15/522).ConclusionsImprint cytology followed by frozen section led to a markedly decreased positive margin rate. This is considered the best method for intraoperative margin assessment, as it can overcome the limitations of cytology and histology performed individually.

Abstract:Purpose This study was a retrospective and non-randomized study to assess the safety and reliability of identifying the surgical margin in breast cancer breast-conserving surgery (BCS) by using intraoperative ultrasonic location and specimen mammography instead of traditional intraoperative frozen pathological section. Methods Among the patients who underwent BCS from May 2019 to October 2021, according to the different methods of evaluating the intraoperative margin, 104 breast cancer patients were included in the frozen edge group, 53 breast cancer patients were included in the freeze-free group, and the surgeon judged whether extended resection was needed based on the results of pathological section or evaluation of intraoperative ultrasound and mammography. The surgical margins of the two groups were judged by postoperative pathological results as the gold standard. Results The median waiting pathology results time in the frozen edge group was 64 minutes, while the waiting time in the freeze-free group was 30 minutes, and the difference was statistically significant (P<0.0001). The postoperative pathological results showed that the positive rate of the surgical margin in the frozen edge group was 0.96%. The coincidence rate of intraoperative frozen and postoperative pathological results was 99.04%. The coincidence rate between intraoperative mammography and postoperative pathological results was 100%. Conclusions In BCS, the method of using intraoperative staining markers combined with mammography to evaluate the resection margin is highly accurate, reliable, economical and convenient, and at the same time reduces the waiting time of the operator during the operation. However, this was not a randomized controlled study, and there was patient selection bias, and its safety needs to be confirmed by long-term follow-up. In the future, it is expected to become the mainstream means of evaluating. Micro Breast-conserving surgery is recommended as a treatment for patients with early-stage breast cancer, but the positive margin is an important factor for recurrence. This study proposed a new method and assessed the safety and reliability of identifying the surgical margin in breast cancer breast-conserving surgery by using intraoperative ultrasonic location and specimen mammography.

Abstract:In this narrative review, we discuss some of the familiar limitations of intraoperative frozen section for margin assessment and compare these to the attractive counterpoints of modern digital imaging technologies such as confocal laser microscopy. We also delve into the new questions posed by these technologies that use en‐face margin assessment as opposed to traditional formalin‐fixed paraffin‐embedded cross‐sectional margin assessment. Intraoperative frozen section (IFS) is used with the intention to improve functional and oncological outcomes for patients undergoing radical prostatectomy (RP). High resource requirements of IFS techniques such as NeuroSAFE may preclude widespread adoption, even if there are benefits to patients. Recent advances in fresh‐tissue microscopic digital imaging technologies may offer an attractive alternative, and there is a growing body of evidence regarding these technologies. In this narrative review, we discuss some of the familiar limitations of IFS and compare these to the attractive counterpoints of modern digital imaging technologies such as the speed and ease of image generation, the locality of equipment within (or near) the operating room, the ability to maintain tissue integrity, and digital transfer of images. Confocal laser microscopy (CLM) is the modality most frequently reported in the literature for margin assessment during RP. We discuss several imitations and obstacles to widespread dissemination of digital imaging technologies. Among these, we consider how the 'en‐face' margin perspective will challenge urologists and pathologists to understand afresh the meaning of positive margin significance. As a part of this, discussions on how to describe, categorize, react to, and evaluate these technologies are needed to improve patient outcomes. Limitations of this review include its narrative structure and that the evidence base in this field is relatively immature but developing at pace.

Abstract:Artificial intelligence (AI) has been implemented in multiple fields of medicine to assist in the diagnosis and treatment of patients. AI implementation in radiology, more specifically for breast imaging, has advanced considerably. Breast cancer is one of the most important causes of cancer mortality among women, and there has been increased attention towards creating more efficacious methods for breast cancer detection utilizing AI to improve radiologist accuracy and efficiency to meet the increasing demand of our patients. AI can be applied to imaging studies to improve image quality, increase interpretation accuracy, and improve time efficiency and cost efficiency. AI applied to mammography, ultrasound, and MRI allows for improved cancer detection and diagnosis while decreasing intra- and interobserver variability. The synergistic effect between a radiologist and AI has the potential to improve patient care in underserved populations with the intention of providing quality and equitable care for all. Additionally, AI has allowed for improved risk stratification. Further, AI application can have treatment implications as well by identifying upstage risk of ductal carcinoma in situ (DCIS) to invasive carcinoma and by better predicting individualized patient response to neoadjuvant chemotherapy. AI has potential for advancement in pre-operative 3-dimensional models of the breast as well as improved viability of reconstructive grafts.
Evaluation of Incision Margin and Radiotherapy Adjustment in Breast Cancer-Conserving Surgery Based on VR Pathology 3D Reconstruction

Abstract:At present, the incidence of breast cancer is high worldwide and the most effective way to treat breast cancer is surgery. This study mainly explores the application of pathological three-dimensional reconstruction based on VR technology in the evaluation of the margins of breast-conserving surgery and the adjustment of radiotherapy for breast cancer. In the process of making pathological large slices, the slice markers are counted and the upper and outer cutting edges are located and placed in a large embedding box to prevent tissue deformation. ACDSEE6.0 is used to convert the aligned image BMP format into JPG format, the JPG images of each breast are cropped in batches under the same range and size, and then 3DDOCTOR1.0 is imported to edit the images. The boundary edit tool is activated to segment the target area layer by layer to create a boundary data set of the target structure. The stored file is opened and 3D rendering simple surface and volume rendering on the computer is run to display the surface data 3D display and the volume data 3D display. During VR image processing, the data are imported into the VR image workstation and the Radio Dexter software system is used for image adjustment, trimming, and sequence extraction. Then, the preliminary processed CT and MRI data are displayed in pairs. Through multiplane comparison, manual or automatic fusion tools are used to reduce the gap between image sequences. The surgical process is simulated under VR: by changing the window size and viewing angle; using virtual electric drills, cutting tools, and other virtual instruments; ad selecting the operating microscope perspective; and simulating steps such as breast opening, visual field exposure, and tumor resection. The breast tomographic image was reconstructed in multiple planes, and two-dimensional images of any plane could be observed. 10 cases showed high-density shadows and 5 cases showed isodensity shadows. This research contributes to the precise treatment of breast-conserving surgery.

The Usefulness of Intraoperative Circumferential Frozen-Section Analysis of Lumpectomy Margins in Breast-Conserving Surgery

Abstract:PURPOSE: Intraoperative frozen-section analysis of the lumpect-omy margin during breast-conserving surgery (BCS) is an excellent method in obtaining a clear resection margin. This study aimed to investigate the usefulness of intraoperative circumferential frozen-section analysis (IOCFS) of lumpectomy margin during BCS for breast cancer, and to find factors that increase the conversion into mastectomy.METHODS: From 2007 to 2011, 509 patients with breast cancer underwent IOCFS during BCS. The outer surfaces of the shaved lumpectomy margins were evaluated. A negative margin was defined as no ink on the tumor. All margins were evaluated using the permanent section analysis.RESULTS: Among the 509 patients, 437 (85.9%) underwent BCS and 72 (14.1%) finally underwent mastectomy. Of the 483 pathologically confirmed patients, 338 (70.0%) were true-negative, 24 (5.0%) false-negative, 120 (24.8%) true-positive, and 1 (0.2%) false-positive. Twenty-four patients (4.7%) among total 509 patients had undetermined margins as either atypical ductal hyperplasia or ductal carcinoma <i>in situ</i> in the first IOCFS. The IOCFS has an accuracy of 94.8% with 83% sensitivity, 99.7% specificity, 93.4% negative predictive value, and 99.2% positive predictive value. Sixty-three cases (12.4%) were converted to mastectomy, the first intraoperatively. Of the 446 (87.6%) patients who successfully underwent BCS, 64 patients received additional excisions and 32 were reoperated to achieve clear margin (reoperation rate, 6.3%). Twenty-three of the reoperated patients underwent re-excisions using the second intraoperative frozen section analysis, and achieved BCS. Nine cases were additionally converted to mastectomy. No significant differences in age, stage, and biological factors were found between the BCS and mastectomy cases. Factors such as invasive lobular carcinoma, multiple tumors, large tumor, and multiple excisions increased the conversion to mastectomy.CONCLUSION: The IOCFS analysis during BCS is useful in evaluating lumpectomy margins and preventing reoperation.
