Abstract:BACKGROUND: Laparoscopic partial nephrectomy (LPN) is a standard surgical treatment option for renal tumors. Prior to LPN, it is necessary to evaluate the tumor condition and develop a surgical plan using precise imaging. OBJECTIVE: To explore the clinical application value of 3D virtual model (3DVM) in LPN. METHODS: A total of 80 patients with renal tumor who underwent LPN were measured. Patients were divided into three cohorts (A, B, C) according to the difficulty of surgery. Each group was further divided into the test and control groups based on the application of preoperative 3DVM. Surgical safety and efficacy were assessed, and a questionnaire was developed to investigate the opinions of patients and physicians on 3DVM. RESULTS: The duration of LPN and intraoperative renal artery occlusion were significantly different between the test and control groups in both cohorts A and B (P< 0.05). In cohort C, the surgical duration, duration of intraoperative renal artery occlusion, length of stay, time to postoperative ambulation, intraoperative bleeding, incidence of postoperative bleeding were significantly between the two groups (P< 0.05). The analysis both of patients and physicians questionnaire scores were statistically significant (P< 0.05). CONCLUSIONS: 3DVM contributes to safer and more effective LPN. It benefits both doctors and patients.

Abstract:While minimally invasive laparoscopic surgery can help reduce blood loss, reduce hospital time, and shorten recovery time compared to open surgery, it has the disadvantages of limited field of view and difficulty in locating subsurface targets. Our proposed solution applies an augmented reality (AR) system to overlay pre-operative images, such as those from magnetic resonance imaging (MRI), onto the target organ in the user's real-world environment. Our system can provide critical information regarding the location of subsurface lesions to guide surgical procedures in real time. An infrared motion tracking camera system was employed to obtain real-time position data of the patient and surgical instruments. To perform hologram registration, fiducial markers were used to track and map virtual coordinates to the real world. In this study, phantom models of each organ were constructed to test the reliability and accuracy of the AR-guided laparoscopic system. Root mean square error (RMSE) was used to evaluate the targeting accuracy of the laparoscopic interventional procedure. Our results demonstrated a registration error of 2.42 ± 0.79 mm and a procedural targeting error of 4.17 ± 1.63 mm using our AR-guided laparoscopic system that will be further refined for potential clinical procedures.
Effect of dietary fat saturation on LDL oxidation and monocyte adhesion to human endothelial cells in vitro.

Abstract:Forty-two healthy men and women were subjected to four consecutive dietary periods differing in the fat content of saturated fatty acids (SFAs), monounsaturated fatty acids (MUFAs), and polyunsaturated fatty acids (n-6) [PUFA(n-6)] and (n-3) [PUFA(n-3)]. Plasma lipids, vitamin E, and in vitro LDL oxidation were examined during each period. Adhesion of human monocytes to cultured human endothelial cells was used as a functional test to identify differences in the biological properties of LDL from each dietary period. Consumption of an SFA-rich diet resulted in higher LDL cholesterol (4.06 +/- 0.85 mmol/L, P < .05) than did consumption of MUFA- (3.59 +/- 0.75 mmol/L), PUFA(n-6)- (3.44 +/- 0.77 mmol/L), or PUFA(n-3)- (3.31 +/- 0.8 mmol/L) rich diets. HDL cholesterol was lower during both PUFA-rich diets (1.24 +/- 0.28 and 1.27 +/- 0.28 mmol/L for n-6 and n-3, respectively) than during the SFA-(1.32 +/- 0.36 mmol/L) and MUFA- (1.32 +/- 0.34 mmol/L) rich diets. LDL resistance to copper-induced oxidation, expressed as lag time, was highest during the MUFA-rich diet (55.1 +/- 7.3 minutes) and lowest during the PUFA(n-3)- (45.3 +/- 7 minutes) and SFA- (45.3 +/- 6.4 minutes) rich diets. LDL induction of monocyte adhesion to endothelial cells was lower during the MUFA-rich diet than the other periods. The highest monocyte adhesion was obtained during the PUFA(n-3) and SFA dietary periods. In conclusion, an MUFA-rich diet benefits plasma lipid levels compared with an SFA-rich diet. Furthermore, this diet results in an increased resistance of LDL to oxidation and a lower rate of monocyte adhesion to endothelial cells than the other dietary fats examined.

Abstract:Objective: Understanding the three-dimensional (3D) spatial position and orientation of vessels and tumor(s) is vital in laparoscopic liver resection procedures. Augmented reality (AR) techniques can help surgeons see the patient's internal anatomy in conjunction with laparoscopic video images. Method: In this paper, we present an AR-assisted navigation system for liver resection based on a rigid stereoscopic laparoscope. The stereo image pairs from the laparoscope are used by an unsupervised convolutional network (CNN) framework to estimate depth and generate an intraoperative 3D liver surface. Meanwhile, 3D models of the patient's surgical field are segmented from preoperative CT images using V-Net architecture for volumetric image data in an end-to-end predictive style. A globally optimal iterative closest point (Go-ICP) algorithm is adopted to register the pre- and intraoperative models into a unified coordinate space; then, the preoperative 3D models are superimposed on the live laparoscopic images to provide the surgeon with detailed information about the subsurface of the patient's anatomy, including tumors, their resection margins and vessels. Results: The proposed navigation system is tested on four laboratory ex vivo porcine livers and five operating theatre in vivo porcine experiments to validate its accuracy. The ex vivo and in vivo reprojection errors (RPE) are 6.04 +/- 1.85 mm and 8.73 +/- 2.43 mm, respectively. Conclusion and Significance: Both the qualitative and quantitative results indicate that our AR-assisted navigation system shows promise and has the potential to be highly useful in clinical practice. Abstract:OBJECTIVE:Fluoroscopic guidance is a critical step for the puncture procedure in percutaneous endoscopic transforaminal discectomy (PETD). However, two-dimensional observations of the three-dimensional anatomic structure suffer from the effects of projective simplification. To accurately assess the spatial relations between the patient vertebra tissues and puncture needle, a considerable number of fluoroscopic images from different orientations need to be acquired by the surgeons. This process significantly increases the radiation risk for both the patient and surgeons.METHODS:In this paper, we propose an augmented reality (AR) surgical navigation system for PETD based on multi-modality information, which contains fluoroscopy, optical tracking, and depth camera. To register the fluoroscopic image with the intraoperative video, we design a lightweight non-invasive fiducial with markers and detect the markers based on the deep learning method. It can display the intraoperative video fused with the registered fluoroscopic images. We also present a self-adaptive calibration and transformation method between a 6-DOF optical tracking device and a depth camera, which are in different coordinate systems.RESULTS:With the substantially reduced frequency of fluoroscopy imaging, the system can accurately track and superimpose the virtual puncture needle on fluoroscopy images in real-time. From operating theatre in vivo animal experiments, the results illustrate that the system average positioning accuracy can reach 1.98mm and the orientation accuracy can reach 1.19∘. From the clinical validation results, the system significantly lower the frequency of fluoroscopy imaging (42.7%) and reduce the radiation risk for both the patient and surgeons.CONCLUSION:Coupled with the user study, both the quantitative and qualitative results indicate that our navigation system has the potential to be highly useful in clinical practice. Compared with the existing navigation systems, which are usually equipped with a variety of large and high-cost medical equipments, such as O-arm, cone-beam CT, and robots, our navigation system does not need special equipment and can be implemented with common equipment in the operating room, such as C-arm, desktop, etc., even in small hospitals.

Abstract:Journal of Mechanics in Medicine and Biology, Ahead of Print. In oral and maxillofacial surgery, computer-assisted navigation technologies have been widely used to achieve intraoperative positioning. The traditional methods mainly rely on the experience of doctors and the difference between the locations of key points in the surgical area and the preoperative planning, which have certain limitations. In this paper, a new intraoperative surgical navigation framework based on mobile laser scanner is proposed, which ensures that surgery is performed accurately according to the preoperative planning. The framework mainly includes two parts. First, the real-time surface reconstruction of the anatomy should be realized during the operation. Second, the acquired image is matched to the planned image in real time. Although the most common method of surface reconstruction is to render the volume directly from raw data or render the surface from the segmented data using computed tomography/magnetic resonance (CT/MR) data, this method is too complicated for performing the real-time operation during surgery. Furthermore, a new surface registration technique is proposed for image-guided oral and maxillofacial surgery based on the point sets. To improve the registration accuracy and robustness, the point sets are modeled by Mixed Student's t-Distribution model. In the experiments, the point sets of CT data are from 10 patients with craniomaxillofacial diseases and the surface point set is from the LRS. The TRE of 10 data was less than 1[math]mm. Compared with the paired-point registration method and Iterative Closest Point algorithm, the results demonstrated better performance of the proposed method, the surgical situation can be displayed in real time during the surgical process, and any differences from the surgical plan can also be reflected.

Abstract:Background: Due to the considerable risk of trauma, traditional open surgery has been gradually replaced by minimally invasive surgery using tunnel screws. However, during minimally invasive pelvic surgery, surgical navigation can be improved by effectively using the augmented reality technique. This can further improve the accuracy of the surgery and reduce risks.Methods: This study adopts a registration approach based on the patient coordinate system and uses a specific manual setting comparison method to select the suitable 3D corner detection parameters.Results: After experiments, this strategy can effectively improve the accuracy of positioning during surgery. It achieves a balance in detection time and accuracy, which further reduces risks and injuries during surgery.Conclusions: Our technique can achieve more precise and safer results during minimally invasive pelvic surgery, which shows its great value and potential in the clinic.

Abstract:Purpose We present a novel augmented reality (AR) surgical navigation method with ultrasound-assisted point cloud registration for percutaneous ablation of liver tumors. A preliminary study is carried out to verify its feasibility. Methods Two three-dimensional (3D) point clouds of the liver surface are derived from the preoperative images and intraoperative tracked US images, respectively. To compensate for the soft tissue deformation, the point cloud registration between the preoperative images and the liver is performed using the non-rigid iterative closest point (ICP) algorithm. A 3D AR device based on integral videography technology is designed to accurately display naked-eye 3D images for surgical navigation. Based on the above registration, naked-eye 3D images of the liver surface, planning path, entry points, and tumor can be overlaid in situ through our 3D AR device. Finally, the AR-guided targeting accuracy is evaluated through entry point positioning. Results Experiments on both the liver phantom and in vitro pork liver were conducted. Several entry points on the liver surface were used to evaluate the targeting accuracy. The preliminary validation on the liver phantom showed average entry-point errors (EPEs) of 2.34 ± 0.45 mm, 2.25 ± 0.72 mm, 2.71 ± 0.82 mm, and 2.50 ± 1.11 mm at distinct US point cloud coverage rates of 100%, 75%, 50%, and 25%, respectively. The average EPEs of the deformed pork liver were 4.49 ± 1.88 mm and 5.02 ± 2.03 mm at the coverage rates of 100% and 75%, and the average covered-entry-point errors (CEPEs) were 4.96 ± 2.05 mm and 2.97 ± 1.37 mm at 50% and 25%, respectively. Conclusion Experimental outcomes demonstrate that the proposed AR navigation method based on US-assisted point cloud registration has achieved an acceptable targeting accuracy on the liver surface even in the case of liver deformation.
Augmented Reality Surgical Navigation System Integrated with Deep Learning

Abstract:Most current surgical navigation methods rely on optical navigators with images displayed on an external screen. However, minimizing distractions during surgery is critical and the spatial information displayed in this arrangement is non-intuitive. Previous studies have proposed combining optical navigation systems with augmented reality (AR) to provide surgeons with intuitive imaging during surgery, through the use of planar and three-dimensional imagery. However, these studies have mainly focused on visual aids and have paid relatively little attention to real surgical guidance aids. Moreover, the use of augmented reality reduces system stability and accuracy, and optical navigation systems are costly. Therefore, this paper proposed an augmented reality surgical navigation system based on image positioning that achieves the desired system advantages with low cost, high stability, and high accuracy. This system also provides intuitive guidance for the surgical target point, entry point, and trajectory. Once the surgeon uses the navigation stick to indicate the position of the surgical entry point, the connection between the surgical target and the surgical entry point is immediately displayed on the AR device (tablet or HoloLens glasses), and a dynamic auxiliary line is shown to assist with incision angle and depth. Clinical trials were conducted for EVD (extra-ventricular drainage) surgery, and surgeons confirmed the system’s overall benefit. A “virtual object automatic scanning” method is proposed to achieve a high accuracy of 1 ± 0.1 mm for the AR-based system. Furthermore, a deep learning-based U-Net segmentation network is incorporated to enable automatic identification of the hydrocephalus location by the system. The system achieves improved recognition accuracy, sensitivity, and specificity of 99.93%, 93.85%, and 95.73%, respectively, representing a significant improvement from previous studies.

Abstract:PURPOSE:During the minimally invasive knee surgery, surgeons insert surgical instruments and arthroscopy through small incisions, and implement treatment assisted by 2D arthroscopic images. However, this 2D arthroscopic navigation faces several problems. Firstly, the guidance information is displayed on a screen away from the surgical area, which makes hand/eye coordination difficult. Secondly, the small incision limits the surgeons to view the internal knee structures only from an arthroscopic camera. In addition, arthroscopic images commonly appear obscure visions.METHODS:To solve these problems, we proposed a novel in-situ augmented reality navigation system with the enhanced arthroscopic information. Firstly, intraoperative anatomical locations were obtained by using arthroscopic images and arthroscopy calibration. Secondly, tissue properties-based model deformation method was proposed to update the 3D preoperative knee model with anatomical location information. Then, the updated model was further rendered with glasses-free real 3D display for achieving the global in-situ augmented reality view. In addition, virtual arthroscopic images were generated from the updated preoperative model to provide the anatomical information of the operation area.RESULTS:Experimental results demonstrated that virtual arthroscopic images could reflect the correct structure information with a mean error of 0.32 mm. Compared with 2D arthroscopic navigation, the proposed augmented reality navigation reduced the targeting errors by 2.10 mm and 2.70 mm for the experiments of knee phantom and in-vitro swine knee, respectively.CONCLUSION:Our navigation method is helpful for minimally invasive knee surgery since it can provide the global in-situ information and detail anatomical information.

Abstract:Surgical registration that maps surgical space onto image space plays an important role in surgical navigation. Accurate surgical registration can help surgeons efficiently locate surgical instruments. The complicated marker-based surgical registration method is highly accurate, but it is time-consuming. Therefore, a marker-less surgical registration method with high-precision and high-efficiency is proposed without human intervention. Firstly, the surgical navigation system based on the multi-vision system is calibrated by using a specially-designed calibration board. When extracting the abdominal point cloud acquired by the structured light vision system, the constraint is constructed by using Computed Tomography (CT) image to filter out the points in irrelevant areas to improve the computational efficiency. The Coherent Point Drift (CPD) algorithm based on Gaussian Mixture Model (GMM) is applied in the registration of abdominal point cloud with lack of surface features. To enhance the efficiency of the CPD algorithm, firstly, the system calibration result is used in rough registration of the point cloud, and then the proper point cloud pretreatment method and its parameters are studied through experiments. Finally, the puncturing simulation experiments were carried out by using the abdominal phantom. The experimental results show that the proposed surgical registration method has high accuracy and efficiency, and has potential clinical application value.

Abstract:Partial nephrectomy, a standard treatment for small renal cancers, has evolved through minimally invasive procedures such as laparoscopic and robot-assisted partial nephrectomy. The use of three-dimensional (3D) kidney models derived from preoperative computed tomography (CT) images has been investigated to improve surgical outcomes. This review explores various navigation techniques, such as 3D printing, virtual reality (VR), and augmented reality (AR), to address organ movement and deformation challenges during surgery. Despite the promising positive impact of these methods, as revealed by a systematic review in 2022, achieving the desired navigation accuracy remains elusive. The use of Virtual Reality and Augmented Reality, capable of overlaying the 3D model onto the surgical image in real-time, has shown potential. Still, we need advanced techniques, for instance, non-rigid 3D models employing nonlinear parametric deformation, to adapt to organ deformation. Additionally, the application of deep learning from artificial intelligence for high accuracy 3D navigation is an emerging area of interest. Although considerable progress has been achieved, a comprehensive, widely adoptable solution has yet to be discovered. The paper underscores the necessity for ongoing research and development in 3D navigation methods, anticipating their substantial contribution to future surgical procedures.
Augmented Reality Navigation for Stereoscopic Laparoscopic Anatomical Hepatectomy of Primary Liver Cancer: Preliminary Experience

Abstract:Background Accurate determination of intrahepatic anatomy remains challenging for laparoscopic anatomical hepatectomy (LAH). Laparoscopic augmented reality navigation (LARN) is expected to facilitate LAH of primary liver cancer (PLC) by identifying the exact location of tumors and vessels. The study was to evaluate the safety and effectiveness of our independently developed LARN system in LAH of PLC. Methods From May 2018 to July 2020, the study included 85 PLC patients who underwent three-dimensional (3D) LAH. According to whether LARN was performed during the operation, the patients were divided into the intraoperative navigation (IN) group and the non-intraoperative navigation (NIN) group. We compared the preoperative data, perioperative results and postoperative complications between the two groups, and introduced our preliminary experience of this novel technology in LAH. Results There were 44 and 41 PLC patients in the IN group and the NIN group, respectively. No significant differences were found in preoperative characteristics and any of the resection-related complications between the two groups (All P > 0.05). Compared with the NIN group, the IN group had significantly less operative bleeding ( P = 0.002), lower delta Hb% ( P = 0.039), lower blood transfusion rate ( P < 0.001), and reduced postoperative hospital stay ( P = 0.003). For the IN group, the successful fusion of simulated surgical planning and operative scene helped to determine the extent of resection. Conclusions The LARN contributed to the identification of important anatomical structures during LAH of PLC. It reduced vascular injury and accelerated postoperative recovery, showing a potential application prospects in liver surgery.

Real-time segmentation and tracking of excised corneal contour by deep neural networks for DALK surgical navigation

Abstract:Objective: Corneal disease is one of the main causes of blindness for humans globally nowadays, and deep anterior lamellar keratoplasty (DALK) is a widely applied technique for corneal transplantation. However, the position of stitch points highly influences the success rate of such surgery, which would require accurate control and manipulation of surgical instruments. Methods: In this paper, we present a deep learning framework for augmented reality (AR) based surgery navigation to guide the suturing in DALK. It can robustly track the excised corneal contour by semantic segmentation and the reconstruction of occlusion. We propose a novel optical flow inpainting network to recover the missing motion caused by occlusion. The occluded regions are detected by weakly supervised segmentation of surgical instruments and reconstructed by key frame warping along the completed optical flow. Then we introduce two types of loss function to adapt the inpainting network in the optical flow space. Results: Our techniques are tested and evaluated by a number of real surgery videos from Shandong Eye Hospital in China. We compare our approaches with other typical methods in the corneal contour segmentation, optical flow inpainting and occlusion regions reconstruction. The tracking accuracy reachs 99.2% in average and PSNR reaches 25.52 for the reconstruction of the occluded frames. Conclusion: From the experimental evaluations and user study, both the qualitative and quantitative results indicate that our techniques can achieve accurate detection and tracking of corneal contour under complex disturbance in real-time surgical scenes. Our prototype AR navigation system would be highly useful in clinical practice.

Abstract:The immunological response of the human host to xenograft bone after combined xenografts of Kiel bone and autologous marrow has been studied by serological methods in twenty-eight patients and by the leucocytic reaction in the peripheral blood in fifteen. The clinical findings in these patients have already been reported. The results suggest that Kiel bone is very weakly antigenic in the human host. These results are discussed in the light of previous experimental findings and a possible explanation put forward. The advantages of osteogenesis by autologous marrow cells are also discussed.

Abstract:PurposeLaparoscopic nephrectomy (LN) has become the preferred method for renal cell carcinoma (RCC). Adequate preoperative assessment or intraoperative navigation is key to the successful implementation of LN. The aim of this study was to evaluate the clinical application value of mixed‐reality–assisted surgical navigation (MRASN) in LN. Patients and MethodsA total of 100 patients with stage T1N0M0 renal tumors who underwent laparoscopic partial nephrectomy (LPN) or laparoscopic radical nephrectomy (LRN) were prospectively enrolled and divided into a mixed‐reality‐assisted laparoscopic nephrectomy (MRALN) group (n = 50) and a non–mixed‐reality‐assisted laparoscopic nephrectomy (non‐MRALN) group (n = 50). All patients underwent renal contrast‐enhanced CT scans. The CT DICOM data of all patients in the MRALN group were imported into the mixed‐reality (MR) postprocessing workstation and underwent holographic three‐dimensional visualization (V3D) modeling and MR displayed, respectively. We adopted the Likert scale to evaluate the clinical application value of MRASN. The consistency of evaluators was assessed using the Cohen kappa coefficient (k). ResultsNo significant differences in patient demographic indicators between the MRALN group and the non‐MRALN group (P > .05). The subjective score of MRASN clinical application value in operative plan formulation, intraoperative navigation, remote consultation, teaching guidance, and doctor‐patient communication were higher in the MRASN group than in the non‐MRASN group (all P < .001). There were significantly more patients for whom LPN was successfully implemented in the MRALN group than in the non‐MRALN group (82% vs 46%, P < .001). The MRALN group had a shorter operative time (OT) and warm ischemia time (WIT) and less estimated blood loss (EBL) than the non‐MRALN group (all P < .001). ConclusionMRASN is helpful for operative plan formulation, intraoperative navigation, remote consultation, teaching guidance, and doctor‐patient communication. MRALN may effectively improve the successful implementation rate of LPN and reduce the OT, WIT, and EBL.
