Abstract:Sewage effluent, storm runoff, discharge from polluted rivers, and inputs of groundwater have all been suggested as potential sources of land derived nutrients into Hanalei Bay, Kauai. We determined the nitrogen isotopic signatures (delta(15)N) of different nitrate sources to Hanalei Bay along with the isotopic signature recorded by 11 species of macroalgal collected in the Bay. The macroalgae integrate the isotopic signatures of the nitrate sources over time, thus these data along with the nitrate to dissolved inorganic phosphate molar ratios (N:P) of the macroalgae were used to determine the major nitrate source to the bay ecosystem and which of the macro-nutrients is limiting algae growth, respectively. Relatively low delta(15)N values (average -0.5% per hundred) were observed in all algae collected throughout the Bay; implicating fertilizer, rather than domestic sewage, as an important external source of nitrogen to the coastal water around Hanalei. The N:P ratio in the algae compared to the ratio in the Bay waters imply that the Hanalei Bay coastal ecosystem is nitrogen limited and thus, increased nitrogen input may potentially impact this coastal ecosystem and specifically the coral reefs in the Bay. Identifying the major source of nutrient loading to the Bay is important for risk assessment and potential remediation plans.
Augmented Reality Surgical Navigation in Minimally Invasive Spine Surgery: A Preclinical Study

Abstract:Abstract Background : Augmented reality (AR) is a technology that combines virtual information with the real-world environment. In the medical field, AR technology can be used to show information inside the body. Methods : We combined augmented reality, artificial intelligence and optical tracking to enhance the augmented reality minimally invasive spine surgery (AR-MISS) system. The system has three functions: augmented reality radiograph superimposition, AR real-time puncture needle tracking and AR intraoperative navigation. The three functions of the system were preliminarily evaluated through beagle animal experiments. Results: The AR radiographs were successfully superimposed on the real intraoperative videos. The anteroposterior and lateral errors of superimposed AR radiographs were 0.74 ± 0.21 mm and 1.13 ± 0.40 mm, respectively. The puncture needles could be tracked by the AR-MISS system in real time. The anteroposterior and lateral errors of the real-time AR needle tracking were 1.26 ±0.20 mm and 1.22 ± 0.25 mm, respectively. With the help of AR radiographsand AR puncture needles, the puncture procedure could be guided visually by the system in real time. The anteroposterior and lateral errors of AR-guided puncture were 2.47 ±0.86 mm and 2.85± 1.17 mm, respectively. Conclusion : Theresults indicate that the AR-MISS system is accurate and feasible in a beagle model. Additional clinical assessment is needed.

Abstract:Background: Due to the considerable risk of trauma, traditional open surgery has been gradually replaced by minimally invasive surgery using tunnel screws. However, during minimally invasive pelvic surgery, surgical navigation can be improved by effectively using the augmented reality technique. This can further improve the accuracy of the surgery and reduce risks.Methods: This study adopts a registration approach based on the patient coordinate system and uses a specific manual setting comparison method to select the suitable 3D corner detection parameters.Results: After experiments, this strategy can effectively improve the accuracy of positioning during surgery. It achieves a balance in detection time and accuracy, which further reduces risks and injuries during surgery.Conclusions: Our technique can achieve more precise and safer results during minimally invasive pelvic surgery, which shows its great value and potential in the clinic.

Abstract:PURPOSE:During the minimally invasive knee surgery, surgeons insert surgical instruments and arthroscopy through small incisions, and implement treatment assisted by 2D arthroscopic images. However, this 2D arthroscopic navigation faces several problems. Firstly, the guidance information is displayed on a screen away from the surgical area, which makes hand/eye coordination difficult. Secondly, the small incision limits the surgeons to view the internal knee structures only from an arthroscopic camera. In addition, arthroscopic images commonly appear obscure visions.METHODS:To solve these problems, we proposed a novel in-situ augmented reality navigation system with the enhanced arthroscopic information. Firstly, intraoperative anatomical locations were obtained by using arthroscopic images and arthroscopy calibration. Secondly, tissue properties-based model deformation method was proposed to update the 3D preoperative knee model with anatomical location information. Then, the updated model was further rendered with glasses-free real 3D display for achieving the global in-situ augmented reality view. In addition, virtual arthroscopic images were generated from the updated preoperative model to provide the anatomical information of the operation area.RESULTS:Experimental results demonstrated that virtual arthroscopic images could reflect the correct structure information with a mean error of 0.32 mm. Compared with 2D arthroscopic navigation, the proposed augmented reality navigation reduced the targeting errors by 2.10 mm and 2.70 mm for the experiments of knee phantom and in-vitro swine knee, respectively.CONCLUSION:Our navigation method is helpful for minimally invasive knee surgery since it can provide the global in-situ information and detail anatomical information.

Abstract:The minimally invasive surgery in spinal surgery has become increasingly popular in recent years as it reduces the chances of complications during post-operation. However, the procedure of spinal surgery is complicated and the surgical vision of minimally invasive surgery is limited. In order to increase the quality of percutaneous pedicle screw placement, the O-arm that is a mobile intraoperative imaging system is used to assist surgery. The robot navigation system combined with O-arm is also increasing, with the extensive use of O-arm. One of the major problems in the surgical navigation system is to associate the patient space with the intra-operation image space. This study proposes a spatial registration method of spinal surgical robot navigation system, which uses the O-arm to scan a calibration phantom with metal calibration spheres. First, the metal artifacts were reduced in the CT slices and then the circles in the images based on the moments invariant could be identified. Further, the position of the calibration sphere in the image space was obtained. Moreover, the registration matrix is obtained based on the ICP algorithm. Finally, the position error is calculated to verify the feasibility and accuracy of the registration method.
A multi-view interactive virtual-physical registration method for mixed reality based surgical navigation in pelvic and acetabular fracture fixation

Abstract:Purpose: The treatment of pelvic and acetabular fractures remains technically demanding, and traditional surgical navigation systems suffer from the hand-eye mis-coordination. This paper describes a multi-view interactive virtual-physical registration method to enhance the surgeon's depth perception and a mixed reality (MR)-based surgical navigation system for pelvic and acetabular fracture fixation. Methods: First, the pelvic structure is reconstructed by segmentation in a preoperative CT scan, and an insertion path for the percutaneous LC-II screw is computed. A custom hand-held registration cube is used for virtual-physical registration. Three strategies are proposed to improve the surgeon's depth perception: vertices alignment, tremble compensation and multi-view averaging. During navigation, distance and angular deviation visual cues are updated to help the surgeon with the guide wire insertion. The methods have been integrated into an MR module in a surgical navigation system. Results: Phantom experiments were conducted. Ablation experimental results demonstrated the effectiveness of each strategy in the virtual-physical registration method. The proposed method achieved the best accuracy in comparison with related works. For percutaneous guide wire placement, our system achieved a mean bony entry point error of 2.76 ± 1.31 mm, a mean bony exit point error of 4.13 ± 1.74 mm, and a mean angular deviation of 3.04 ± 1.22°. Conclusions: The proposed method can improve the virtual-physical fusion accuracy. The developed MR-based surgical navigation system has clinical application potential. Cadaver and clinical experiments will be conducted in future.

Abstract:To improve the precision,safety and usability of the robot assisted spinal operation,registration between preop-erative CT images and intraoperative X-ray images is utilized.Conventional single planar registration is processed with large navigation error due to lack of depth information along the optical axis,therefore anterior-posterior(AP) and lateral views are calculated simultaneously to improve the registration and navigation accuracy.Based on the analysis on the transforma-tion model of anterior-posterior(AP) and lateral projection spaces,initial registration parameters are automatically estimated and the mapping from the 3D surgical space to the CT space is established by using the biplanar geometric constraint.The real-time postures of surgical instrument in CT space are calculated by using stereo-vision-based positioning technique based on virtual X-ray navigation,and 3D navigation is realized.A vertebral phantom experiment validates the accuracy of the registration and navigation method,and animal lamina milling experiments prove that the operation safety could be improved and the real-time monitoring is accomplished.

Abstract:Background: The number of patients who suffer from glioma has been increasing, and this malignancy is a serious threat to human health. The mainstream treatment for glioma is surgical resection; therefore, accurate resection can improve postoperative patient recovery. Purpose: Many studies have investigated surgical navigation guided by mixed reality, with good outcomes. However, the limitations of mixed reality, such as spatial drift caused by environmental changes, limit its clinical application. Therefore, we present a mixed reality surgical navigation system for glioma resection. Preoperative information can be fused precisely with the real patient with the spatial compensation method to achieve clinically suitable accuracy. Methods: A head-mounted device was used to display virtual information, and a markerless spatial registration method was applied to precisely align the virtual anatomy with the real patient preoperatively. High-accuracy preoperative and intraoperative movement and spatial drift compensation methods were used to increase the positional accuracy of the mixed reality-guided glioma resection system when the patient's head is fixed to the bed frame. Several experiments were designed to validate the accuracy and efficacy of this system. Results: Phantom experiments were performed to test the efficacy and accuracy of this system under ideal conditions, and clinical tests were conducted to assess the performance of this system in clinical application. The accuracy of spatial registration was 1.18 mm in the phantom experiments and 1.86 mm in the clinical application. Conclusions: Herein, we present a mixed reality-based multimodality-fused surgical navigation system for assisting surgeons in intuitively identifying the glioma boundary intraoperatively. The experimental results indicate that this system has suitable accuracy and efficacy for clinical usage.
A Pilot Human Cadaveric Study on Accuracy of the Augmented Reality Surgical Navigation System for Thoracolumbar Pedicle Screw Insertion Using a New Intraoperative Rapid Registration Method

Abstract:To evaluate the feasibility and accuracy of AR-assisted pedicle screw placement using a new intraoperative rapid registration method of combining preoperative CT scanning and intraoperative C-arm 2D fluoroscopy in cadavers. Five cadavers with intact thoracolumbar spines were employed in this study. Intraoperative registration was performed using anteroposterior and lateral views of preoperative CT scanning and intraoperative 2D fluoroscopic images. Patient-specific targeting guides were used for pedicle screw placement from Th1-L5, totaling 166 screws. Instrumentation for each side was randomized (augmented reality surgical navigation (ARSN) vs. C-arm) with an equal distribution of 83 screws in each group. CT was performed to evaluate the accuracy of both techniques by assessing the screw positions and the deviations between the inserted screws and planned trajectories. Postoperative CT showed that 98.80% (82/83) screws in ARSN group and 72.29% (60/83) screws in C-arm group were within the 2-mm safe zone ( p < 0.001). The mean time for instrumentation per level in ARSN group was significantly shorter than that in C-arm group (56.17 ± 3.33 s vs. 99.22 ± 9.03 s, p < 0.001). The overall intraoperative registration time was 17.2 ± 3.5 s per segment. AR-based navigation technology can provide surgeons with accurate guidance of pedicle screw insertion and save the operation time by using the intraoperative rapid registration method of combining preoperative CT scanning and intraoperative C-arm 2D fluoroscopy.

Augmented reality calibration using feature triangulation iteration-based registration for surgical navigation

Abstract:Background: Marker-based augmented reality (AR) calibration methods for surgical navigation often require a second computed tomography scan of the patient, and their clinical application is limited due to high manufacturing costs and low accuracy. Methods: This work introduces a novel type of AR calibration framework that combines a Microsoft HoloLens device with a single camera registration module for surgical navigation. A camera is used to gather multi-view images of a patient for reconstruction in this framework. A shape feature matching-based search method is proposed to adjust the size of the reconstructed model. The double clustering-based 3D point cloud segmentation method and 3D line segment detection method are also proposed to extract the corner points of the image marker. The corner points are the registration data of the image marker. A feature triangulation iteration-based registration method is proposed to quickly and accurately calibrate the pose relationship between the image marker and the patient in the virtual and real space. The patient model after registration is wirelessly transmitted to the HoloLens device to display the AR scene. Results: The proposed approach was used to conduct accuracy verification experiments on the phantoms and volunteers, which were compared with six advanced AR calibration methods. The proposed method obtained average fusion errors of 0.70 ± 0.16 and 0.91 ± 0.13 mm in phantom and volunteer experiments, respectively. The fusion accuracy of the proposed method is the highest among all comparison methods. A volunteer liver puncture clinical simulation experiment was also conducted to show the clinical feasibility. Conclusions: Our experiments proved the effectiveness of the proposed AR calibration method, and revealed a considerable potential for improving surgical performance.
2D/3D registration based on biplanar X-ray and CT images for surgical navigation

Abstract:Background and objectives: Image-based 2D/3D registration is a crucial technology for fluoroscopy-guided surgical interventions. However, traditional registration methods relying on a single X-ray image into surgical navigation systems. This study proposes a novel 2D/3D registration approach utilizing biplanar X-ray images combined with computed tomography (CT) to significantly reduce registration and navigation errors. The method is successfully implemented in a surgical navigation system, enhancing its precision and reliability. Methods: First, we simultaneously register the frontal and lateral X-ray images with the CT image, enabling mutual complementation and more precise localization. Additionally, we introduce a novel similarity measure for image comparison, providing a more robust cost function for the optimization algorithm. Furthermore, a multi-resolution strategy is employed to enhance registration efficiency. Lastly, we propose a more accurate coordinate transformation method, based on projection and 3D reconstruction, to improve the precision of surgical navigation systems. Results: We conducted registration and navigation experiments using pelvic, spinal, and femur phantoms. The navigation results demonstrated that the feature registration errors (FREs) in the three experiments were 0.505±0.063 mm, 0.515±0.055 mm, and 0.577±0.056 mm, respectively. Compared to the point-to-point (PTP) registration method based on anatomical landmarks, our method reduced registration errors by 31.3%, 23.9%, and 26.3%, respectively. Conclusion: The results demonstrate that our method significantly reduces registration and navigation errors, highlighting its potential for application across various anatomical sites. Early Feasibility Studies of Augmented Reality Navigation for Lateral Skull Base Surgery

Abstract:Hypothesis: An augmented reality (AR) platform using a head mounted display (HMD) can successfully display and register virtual computerized tomography (CT) data to corresponding real world anatomy using fiducial markers. Background: AR is a growing field in surgical planning, and has been used by this group to aide in orthopedic surgery applications. Intraoperative AR registration of preoperative CT images could improve identification of critical anatomical structures during lateral skull base surgery. Methods: An AR based HMD platform was created to allow for virtual display and real time manipulation of a temporal bone phantom model. Four fiducial markers were incorporated into this model and used to allow manual alignment of surface markers in real-time. To test the accuracy of this platform virtual lines were created in the AR environment running through the visualized real-world fiducial marker points. Target registration error (TRE) was calculated by measuring the orthogonal distance between these virtual lines and the corresponding virtual fiducial marker for each of the four markers from varying angles. Ten consecutive experiments were performed. Results: The AR based platform successfully displayed CT renderings in the AR environment and allowed real time translation and rotation for manual alignment. A TRE of 10.62 ± 5.90 mm was found. Conclusions: Our results suggest that AR visualization of CT imaging can be registered to patient surface landmarks, but current limitations of AR hardware resulted in TREs too large to be used in the lateral skull base. Future advancements in AR technology will hopefully allow for reduction of registration error.
Optical augmented reality assisted nagivation system for neurosurgery teaching and planning

Abstract:This paper proposed a convenient navigation system for neurosurgeon's pre-operative planning and teaching with augmented reality (AR) technique, which maps the three-dimensional reconstructed virtual anatomy structures onto a skull model. This system included two parts, a virtual reality system and a skull model scence. In our experiment, a 73 year old right-handed man initially diagnosed with astrocytoma was selected as an example to vertify our system. His imaging data from different modalities were registered and the skull soft tissue, brain and inside vessels as well as tumor were reconstructed. Then the reconstructed models were overlayed on the real scence. Our findings showed that the reconstructed tissues were augmented into the real scence and the registration results were in good alignment. The reconstructed brain tissue was well distributed in the skull cavity. The probe was used by a neurosurgeon to explore the surgical pathway which could be directly posed into the tumor while not injuring important vessels. In this way, the learning cost for students and patients' education about surgical risks reduced. Therefore, this system could be a selective protocol for image guided surgery(IGS), and is promising for neurosurgeon's pre-operative planning and teaching.

Abstract:Most current surgical navigation methods rely on optical navigators with images displayed on an external screen. However, minimizing distractions during surgery is critical and the spatial information displayed in this arrangement is non-intuitive. Previous studies have proposed combining optical navigation systems with augmented reality (AR) to provide surgeons with intuitive imaging during surgery, through the use of planar and three-dimensional imagery. However, these studies have mainly focused on visual aids and have paid relatively little attention to real surgical guidance aids. Moreover, the use of augmented reality reduces system stability and accuracy, and optical navigation systems are costly. Therefore, this paper proposed an augmented reality surgical navigation system based on image positioning that achieves the desired system advantages with low cost, high stability, and high accuracy. This system also provides intuitive guidance for the surgical target point, entry point, and trajectory. Once the surgeon uses the navigation stick to indicate the position of the surgical entry point, the connection between the surgical target and the surgical entry point is immediately displayed on the AR device (tablet or HoloLens glasses), and a dynamic auxiliary line is shown to assist with incision angle and depth. Clinical trials were conducted for EVD (extra-ventricular drainage) surgery, and surgeons confirmed the system’s overall benefit. A “virtual object automatic scanning” method is proposed to achieve a high accuracy of 1 ± 0.1 mm for the AR-based system. Furthermore, a deep learning-based U-Net segmentation network is incorporated to enable automatic identification of the hydrocephalus location by the system. The system achieves improved recognition accuracy, sensitivity, and specificity of 99.93%, 93.85%, and 95.73%, respectively, representing a significant improvement from previous studies.
Visualization, Registration and Tracking Techniques for Augmented Reality Guided Surgery: a Review

Abstract:Augmented reality (AR) surgical navigation has developed rapidly in recent years. This paper reviews and analyzes the visualization, registration, and tracking techniques used in AR surgical navigation systems, as well as the application of these AR systems in different surgical fields. The types of AR visualization are divided into two categories of in situ visualization and non in situ visualization. The rendering contents of AR visualization are various. The registration methods include manual registration, point-based registration, surface registration, marker-based registration, and calibration-based registration. The tracking methods consist of self-localization, tracking with integrated cameras, external tracking, and hybrid tracking. Moreover, we describe the applications of AR in surgical fields. However, most AR applications were evaluated through model experiments and animal experiments, and there are relatively few clinical experiments, indicating that the current AR navigation methods are still in the early stage of development. Finally, we summarize the contributions and challenges of AR in the surgical fields, as well as the future development trend. Despite the fact that AR-guided surgery has not yet reached clinical maturity, we believe that if the current development trend continues, it will soon reveal its clinical utility.
A practical marker-less image registration method for augmented reality oral and maxillofacial surgery

Abstract:BackgroundImage registration lies in the core of augmented reality (AR), which aligns the virtual scene with the reality. In AR surgical navigation, the performance of image registration is vital to the surgical outcome.MethodsThis paper presents a practical marker-less image registration method for AR-guided oral and maxillofacial surgery where a virtual scene is generated and mixed with reality to guide surgical operation or provide surgical outcome visualization in the manner of video see-through overlay. An intraoral 3D scanner is employed to acquire the patient’s teeth shape model intraoperatively. The shape model is then registered with a custom-made stereo camera system using a novel 3D stereo matching algorithm and with the patient’s CT-derived 3D model using an iterative closest point scheme, respectively. By leveraging the intraoral 3D scanner, the CT space and the stereo camera space are associated so that surrounding anatomical models and virtual implants could be overlaid on the camera’s view to achieve AR surgical navigation.ResultsJaw phantom experiments were performed to evaluate the target registration error of the overlay, which yielded an average error of less than 0.50 mm with the time cost less than 0.5 s. Volunteer trial was also conducted to show the clinical feasibility.ConclusionsThe proposed registration method does not rely on any external fiducial markers attached to the patient. It performs automatically so as to maintain a correct AR scene, overcoming the misalignment difficulty caused by patient’s movement. Therefore, it is noninvasive and practical in oral and maxillofacial surgery.

Abstract:BACKGROUND:Medical robot is a promising surgical tool, but no specific one has been designed for interventional treatment of chronic pain. We developed a computed tomography-image based navigation robot using a new registration method with binocular vision. This kind of robot is appropriate for minimal invasive interventional procedures and easy to operate. The feasibility, accuracy and stability of this new robot need to be tested. AIM:To assess quantitatively the feasibility, accuracy and stability of the binocular-stereo-vision-based navigation robot for minimally invasive interventional procedures. METHODS:A box model was designed for assessing the accuracy for targets at different distances. Nine (three sets) lead spheres were embedded in the model as puncture goals. The entry-to-target distances were set 50 mm (short-distance), 100 mm (medium-distance) and 150 mm (long-distance). Puncture procedure was repeated three times for each goal. The Euclidian error of each puncture was calculated and statistically analyzed. Three head phantoms were used to explore the clinical feasibility and stability. Three independent operators conducted foramen ovale placement on head phantoms (both sides) by freehand or under the guidance of robot (18 punctures with each method). The operation time, adjustment time and one-time success rate were recorded, and the two guidance methods were compared. RESULTS:On the box model, the mean puncture errors of navigation robot were 1.7 ± 0.9 mm for the short-distance target, 2.4 ± 1.0 mm for the moderate target and 4.4 ± 1.4 mm for the long-distance target. On the head phantom, no obvious differences in operation time and adjustment time were found among the three performers (P > 0.05). The median adjustment time was significantly less under the guidance of the robot than under free hand. The one-time success rate was significantly higher with the robot (P < 0.05). There was no obvious difference in operation time between the two methods (P > 0.05). CONCLUSION:In the laboratory environment, accuracy of binocular-stereo-vision-based navigation robot is acceptable for target at 100 mm depth or less. Compared with freehand, foramen ovale placement accuracy can be improved with robot guidance.

Abstract:While minimally invasive laparoscopic surgery can help reduce blood loss, reduce hospital time, and shorten recovery time compared to open surgery, it has the disadvantages of limited field of view and difficulty in locating subsurface targets. Our proposed solution applies an augmented reality (AR) system to overlay pre-operative images, such as those from magnetic resonance imaging (MRI), onto the target organ in the user's real-world environment. Our system can provide critical information regarding the location of subsurface lesions to guide surgical procedures in real time. An infrared motion tracking camera system was employed to obtain real-time position data of the patient and surgical instruments. To perform hologram registration, fiducial markers were used to track and map virtual coordinates to the real world. In this study, phantom models of each organ were constructed to test the reliability and accuracy of the AR-guided laparoscopic system. Root mean square error (RMSE) was used to evaluate the targeting accuracy of the laparoscopic interventional procedure. Our results demonstrated a registration error of 2.42 ± 0.79 mm and a procedural targeting error of 4.17 ± 1.63 mm using our AR-guided laparoscopic system that will be further refined for potential clinical procedures.
