Abstract:Editor, We read with great interest the recently published article by Choi et al.,1 which evaluated the effect of pre-operative diameter and collapsibility index of the subclavian vein or the infraclavicular axillary vein for predicting intra-operative hypotension after induction of anaesthesia. The authors stated that the collapsibility index of the infraclavicular axillary vein during deep inspiration was a significant predictor of intra-operative hypotension after induction of anaesthesia. We highly appreciate their efforts and their innovative study. However, there are several issues in this study that were not clearly addressed, which may affect the validity of the conclusions. First, according to the authors, general anaesthesia was induced with midazolam (1 to 3 mg), fentanyl (1 to 2 μg kg−1), propofol (1.5 to 2 μg kg−1), rocuronium (0.6 mg kg−1) and maintained with sevoflurane (1 to 2%). However, whether the depth of anaesthesia was equivalent in all patients and how depth of anaesthesia was monitored was not clearly described in this article. As excessive anaesthetic depth is closely related to intra-operative hypotension,2 we suggest that bispectral index should be used to monitor depth of anaesthesia to avoid excessively deep anaesthetic states and associated hypotension, especially in elderly patients. Second, to our knowledge, different modes of mechanical ventilation may have different haemodynamic effects and haemodynamic compromise may happen more frequently during a period with high positive end-expiratory pressure.3 In other words, a high positive end-expiratory pressure may be more likely to provoke intra-operative hypotension due to a reduction of venous return attributable to an increase in the intrathoracic pressure. However, it was not clearly described whether the mode of mechanical ventilation settings was identical in all patients. Thus, this may bias the interpretation of the findings. Finally, to ensure consistent infraclavicular axillary vein measurements, we suggest that measurements should be carried out three times in each patient, and the average value taken. We believe that addressing the above issues would further help to support the conclusions of this study.
Association of Central Arterial Blood Pressure and Left Ventricular Hypertrophy in Patients with Chronic Kidney Disease

Abstract:Aims In the general population, central arterial blood pressure has proved to be more closely related to left ventricular hypertrophy (LVH) than brachial arterial blood pressure. We aimed to investigate whether this relationship was true in patients with chronic kidney disease (CKD). Methods In this retrospective study, we reviewed the medical records of 289 adult patients with CKD from the Zhejiang Provincial People's Hospital in Zhejiang, China. Demographic, echocardiographic and brachial and central blood pressure parameters were retrieved from medical records. Central blood pressure was measured using the SphygmoCor (R) CvMS (AtCor, Australia) device and its corresponding software. Multivariate logistic regression analyses were performed to identify independent predictors of LVH. Receiver operating characteristic curves were used to determine the ability of central and brachial blood pressure to predict LVH. Results The left ventricular mass index was positively associated with both central and brachial blood pressures. However, multiple logistic regression analysis demonstrated that a central pulse pressure (CPP) >= 58 mm Hg was an independent risk factor for LVH (OR = 5.597, 95%CI 2.363-13.259, p < .001). Brachial pulse pressure is not superior to CPP in predicting LVH (area under the curve [AUC] = 0.695, 95%CI 0.634-0.756, p < .001 vs. AUC = 0.687, 95%CI: 0.626-0.748, p < .001, respectively; p = .4824). Conclusion Our results suggested that, similarly to the general population, CPP is a better parameter for predicting the occurrence of LVH in patients with CKD.

Abstract:Central venous pressure (CVP) serves as a direct approximation of right atrial pressure and is influenced by factors like total blood volume, venous compliance, cardiac output, and orthostasis. Normal CVP falls within 8-12 mmHg but varies with volume status and venous compliance. Monitoring and managing disturbances in CVP are vital in patients with circulatory shock or fluid disturbances. Elevated CVP can lead to fluid accumulation in the interstitial space, impairing venous return and reducing cardiac preload. While pulmonary artery catheterization and central venous catheter obtained measurements are considered to be more accurate, they carry risk of complications and their usage has not shown clinical improvement. Ultrasound-based assessment of the internal jugular vein (IJV) offers real-time, non-invasive measurement of static and dynamic parameters for estimating CVP. IJV parameters, including diameter and ratio, has demonstrated good correlation with CVP. Despite significant advancements in non-invasive CVP measurement, a reliable tool is yet to be found. Present methods can offer reasonable guidance in assessing CVP, provided their limitations are acknowledged.
Correlation between sizes and respiratory variation of internal jugular veins and central venous pressure in severe trauma patients

Abstract:目的了解严重创伤患者颈内静脉截面积及呼吸变异与中心静脉压的关系,为临床应用床旁超声快速评估严重创伤患者的血容量提供依据.方法对急诊监护室收治的50例严重创伤患者,在完全控制通气下以超声测定双侧颈内静脉于吸气末和呼气末的最大和最小截面积,计算静脉的呼吸变异指数.同时记录患者脱开呼吸机时的中心静脉压及机械通气的气道压力.并与27例正常人比较颈内静脉截面积的大小,观察静脉截面积的大小、呼吸变异指数与中心静脉压的相关性,确定预测中心静脉压的最佳指标和相应的阈值.结果严重创伤患者的颈内静脉截面积较对照组明显增大,中心静脉压与颈内静脉截面积的大小、呼吸变异指数存在相关性.调整机械通气的影响,右侧颈内静脉最小截面积、左侧颈内静脉的呼吸变异指数预测中心静脉压＞12cm H2O具有较好的价值,相应的阈值分别为1.37 cm2(敏感性84.6%,特异性66.7%)和10.8%(敏感性66.7%,特异性84.6%).结论应用超声测定颈内静脉截面积的大小及呼吸变异指数能在一定程度上预测中心静脉压水平,是评估严重创伤患者血容量的可选方法。
Correlation of peripheral venous pressure and central venous pressure in kidney recipients

Abstract:Background and objective: Previous studies in adults have demonstrated a clinically useful correlation between central venous pressure (CVP) and peripheral venous pressure (PVP). The current study prospectively compared CVP measurements from a central versus a peripheral catheter in kidney recipients during renal transplantation. Methods: With ethics committee approval and informed consent, 30 consecutive kidney recipients were included in the study. We excluded patients who had significant valvular disease or clinically apparent left ventricular failure. For each of 30 patients, CVP and PVP were measured on five different occasions. The pressure tubing of the transducer system was connected to the distal lumen of the central or to the peripheral venous catheter for measurements following induction of anesthesia, after induction, 1 hour after induction, reperfusion of the kidney, and the end of the operation, yielding 150 hemodynamic data points. Each hemodynamic measurement included heart rate, mean arterial pressure, mean CVP, and mean PVP determined at end-expiration. Results: The mean PVP was 13.5 +/- 1.8 mm Hg and the mean CVP was 11.0 +/- 1.5 mm Hg during surgery. The mean difference was 2.5 +/- 0.5 (P < .01). Repeated-measures analysis of variance indicated a highly significant relationship between PVP and CVP (P < .01) with a Pearson correlation coefficient of 0.97. Conclusion: Under the conditions of this study, PVP showed a consistently high agreement with CVP in the perioperative period among patients without significant cardiac dysfunction.
Peripheral Venous Pressure As an Alternative to Central Venous Pressure in Patients Undergoing Laparoscopic Colorectal Surgery.

Abstract:BACKGROUND:Peripheral venous pressure (PVP) is strongly correlated with central venous pressure (CVP) during various surgeries. Laparoscopic surgery in the Trendelenburg position with pneumoperitoneum typically increases CVP. To determine whether PVP convincingly reflects changes in CVP, we evaluated the correlation between PVP and CVP in patients undergoing laparoscopic colorectal surgery.METHODS:Both CVP and PVP were measured simultaneously at predetermined time intervals during elective laparoscopic colorectal surgery in 42 patients without cardiac disease. The pairs of venous pressure measurements were analysed for correlation, and the Bland-Altman plots of repeated measures were used to evaluate the agreement between CVP and PVP.RESULTS:A total of 420 data pairs were obtained. The overall mean CVP was 11.3 (sd 4.5) mm Hg, which was significantly lower than the measured PVP of mean 12.1 (4.5) mm Hg (P=0.005). There was a strong positive correlation between overall CVP and PVP (correlation coefficient=0.96, P<0.0001). The mean bias (PVP-CVP) corrected for repeated measurements using random-effects modelling was 0.9 mm Hg [95% confidence interval (CI) 0.54-1.19 mm Hg] with 95% limits of agreement of -1.2 mm Hg (95% CI -1.75 to -0.62 mm Hg) to 2.9 mm Hg (95% CI 2.35-3.48 mm Hg).CONCLUSIONS:PVP displays a strong correlation and agreement with CVP under the increased intrathoracic pressure of pneumoperitoneum in the Trendelenburg position and may be used as an alternative to CVP in patients without cardiac disease undergoing laparoscopic colorectal surgery.

Abstract:In surgery, the implantation of a central venous catheter through the internal jugular or subclavian vein is crucial.[1] Typically, uids, blood, or inotropic medications are delivered through the catheter's numerous passageways. In addition, monitoring of central venous pressure and pulmonary artery pressure is accomplished via central veins. [2] The indirect measurement of venous pressure (VP) by the physical examination of blood arteries in the neck is one of the most important components of assessment, but it is not without its problems. The jugular vein is difcult to palpate in more than 20% of patients; hence, measures of normal, low, and high levels of VP are erroneous, particularly in critically ill patients. [3] These difculties also occur in individuals who are scheduled for surgery; sometimes, even dramatic changes in VP are not identied. Consequently, the direct assessment of VP is frequently necessary for patients with hemodynamic instability and those undergoing major surgery. [2,3]

Abstract:Objective: An optical imaging system is proposed for quantitatively assessing jugular venous response to altered central venous pressure. Methods: The proposed system assesses sub-surface optical absorption changes from jugular venous waveforms with a spatial calibration procedure to normalize incident tissue illumination. Widefield frames of the right lateral neck were captured and calibrated using a novel flexible surface calibration method. A hemodynamic optical model was derived to quantify jugular venous optical attenuation (JVA) signals, and generate a spatial jugular venous pulsatility map. JVA was assessed in three cardiovascular protocols that altered central venous pressure: acute central hypovolemia (lower body negative pressure), venous congestion (head-down tilt), and impaired cardiac filling (Valsalva maneuver). Results: JVA waveforms exhibited biphasic wave properties consistent with jugular venous pulse dynamics when time-aligned with an electrocardiogram. JVA correlated strongly (median, interquartile range) with invasive central venous pressure during graded central hypovolemia (r=0.85, [0.72, 0.95]), graded venous congestion (r=0.94, [0.84, 0.99]), and impaired cardiac filling (r=0.94, [0.85, 0.99]). Reduced JVA during graded acute hypovolemia was strongly correlated with reductions in stroke volume (SV) (r=0.85, [0.76, 0.92]) from baseline (SV: 7915 mL, JVA: 0.560.10 a.u.) to -40 mmHg suction (SV: 5918 mL, JVA: 0.470.05 a.u.; p<0.01). Conclusion: The proposed non-contact optical imaging system demonstrated jugular venous dynamics consistent with invasive central venous monitoring during three protocols that altered central venous pressure. <->Significance: This system provides non-invasive monitoring of pressure-induced jugular venous dynamics in clinically relevant conditions where catheterization is traditionally required, enabling monitoring in non-surgical environments.

Video PPG and cPPG signals were captured simultaneously on the left and right sides of the neck, respectively. ECG was also captured using the same clinical monitor as cPPG. Two volunteers underwent AJT and BH with head up/down, each with: baseline (15s), experiment (15s), and recovery (15s). Video PPG was split into remote PPG (rPPG) and micromotion detection. All signal modalities were significantly affected by physiological testing. Moreover, cPPG and micromotion waveforms exhibited primary features of jugular vein waveforms and, therefore, have great potential for venous blood flow monitoring. Specifically, remote patient monitoring applications may be enabled by this methodology, facilitating physical collection without a specially trained care provider.

The Influence of Joining Central Venous Catheter and Pressure Transducer with T-junctions on Central Venous Pressure

Abstract:OBJECTIVE:To investigate the influence of the number of T-junctions between central venous catheter and pressure transducer on measurement of central venous pressure ( CVP ) in patients.METHODS:A prospective controlled study was conducted. The patients with CVP monitoring in Department of Critical Care Medicine of the Fifth Center Hospital in Tianjin from February to October in 2014 were enrolled. The patients were divided into three groups according to the number of T-junction between central venous catheter and pressure transducer: without T-junction control group and 1, 2, 3 T-junctions groups. In each patient, corresponding CVP values with different number of T-junctions placed between the central venous catheter and pressure sensors were determined within a certain period, and a square-wave graphic was obtained and preserved on the monitor. The own frequency ( fn ) and the attenuation coefficient ( D ) of the system of pressure measurement were calculated after measurement of the shock wave following a square-wave to obtain the distance between two vibrations and the amplitude of the shock wave. The difference in CVP, fn and D were compared among the groups.RESULTS:A total of 20 cases were enrolled, and 150 groups of data were collected. (1) With the increase in the number of T-junction, CVP showed a tendency of gradual reduction. The CVP of the groups of control and 1, 2, 3 T-junctions was ( 7.00±1.60 ), ( 7.00±3.00 ), ( 5.00±2.00 ), and ( 4.00±1.00 ) mmHg ( 1 mmHg = 0.133 kPa ), respectively. The CVP of 3 T-junctions group was significantly lower than that of the control group ( F = 9.333, P = 0.015 ). (2) With an increase in the number of T-junction, fn showed a tendency of gradual increase. The fn of groups control and 1, 2, 3 T-junctions was ( 12.30±0.79 ), ( 16.00±0.91 ), ( 18.10±1.75 ), ( 20.90±2.69 ) Hz, respectively. The fn of 1, 2, 3 T-junctions group was significantly higher than that of the control group ( F1 = 45.962, F2 = 45.414, F3 = 46.830, all P = 0.000 ); the fn of groups 2 and 3 T-junctions was significantly higher than that of 1 T-junction group ( F1= 5.827, P1= 0.042; F2 = 15.038, P2 = 0.004 ), but there was no significant difference between the groups of 2 T-junctions and 3 T-junctions ( F = 3.800, P = 0.087 ). (3) With an increase of the number of T-junction, D also showed a tendency of gradual increase. The D of 1, 2, 3 T-junction group was 1.62±0.27, 1.60±0.22, 1.82±0.25, and 2.15±0.58, respectively. There were no differences among four groups.CONCLUSIONS:After the application of T-junctions between central venous catheter and pressure transducer, CVP values will be underestimated, the reason of which is considered to be the increase in length and thinner lumen of the T-junctions.
Correlation and Consistency Analysis Between Various Indirect Portal Pressure Gradients and Actual Portal Pressure Gradient

Abstract:Objective: To evaluate the correlation and consistency between hepatic venous pressure gradient(F-HVPG) calculated as the wedged hepatic venous pressure (WHVP) minus free hepatic venous pressure (FHVP), I-HVPG calculated as WHVP minus inferior vena cava pressure (IVCP) in the hepatic segment, and portal pressure gradient (PPG). Methods: Data were collected from 112 patients with portal hypertension undergoing transjugular intrahepatic portosystemic shunt (TIPS) along with HVPG measurement. FHVP, IVCP, WHVP, and portal venous pressure (PVP) were collected intraoperatively. Pearson’s correlation and Bland–Altman method were used to assess correlation and consistency. Results:A total of 112 patients were retrospectively collected. The correlation coefficient (r) velues (p < 0.001) between FHVP and IVCP, F-HVPG and I-HVPG, F-HVPG and PPG, I-HVPG and PPG were 0.835, 0.946, 0.667 and 0.698, respectively; the determination coefficient (R2) values were 0.697, 0.895, 0.445 and 0.487, respectively. Bland–Altman plots showed that F-HVPG and I-HVPG had the narrowest 95% limits of agreement. Among patients with FHVP-IVCP > 2 mmHg, the (r) vlues (p < 0.05) between F-HVPG and I-HVPG, F-HVPG and PPG,I-HVPG and PPG were 0.907, 0.648 and 0.807, respectively; the (R2) values were 0.822, 0.420 and 0.651, respectively. Bland–Altman plots showed that I-HVPG had the narrower 95% limits of agreement with PPG. Conclusion: F-HVPG and I-HVPG demonstrated high correlation and consistency. When the difference between FHVP and IVCP is greater than 2 mmHg, IVCP should be used to calculate HVPG instead of FHVP.
Comparative evaluation between external jugular and internal jugular venous catheterization through landmark technique

Abstract:Background: Cannulation of a central vein is crucial in patients for hemodynamic management and when insertion of a peripheral line is not possible. The internal jugular vein is the preferred access site; however, in the case of not being accessible, the second cannulation site of choice is still unclear. The study aimed to access the feasibility of external jugular vein cannulation versus internal jugular vein cannulation in terms of success, cannulation time, number of attempts and complications. Materials and methods: In this prospective, randomised double-blinded study, 100 patients received 7Fr size triple-lumen central vein catheter either in the internal jugular vein (n = 50) or external jugular vein (n = 50) through the landmark technique as a part of anaesthetic care in the operation theatre. The number of attempts, cannulation time and incidence of complications in both the techniques were observed and recorded. Results: The success rate in internal jugular vein cannulation was 88%, while in the external jugular vein, it was 78% (p = 0.17). The external jugular vein cannulation was comparatively quicker in our study (p = 0.01). Similarly, elevated body mass index did not affect the success rate in the number of cannulations attempts in either group (p = 0.08). In terms of complications, the internal jugular vein group had a total of 20% and the external jugular vein group had 28% complications; however, the complications were more severe in the internal jugular vein group. Conclusion: Anaesthetists should rethink alternate routes for the insertion of central vein cannulation. The external jugular cannulation route is one of them.
Anatomical Variations of the Internal Jugular Vein in the Context of Central Line Placement: A Visual Approach to Data Processing

Abstract:Introduction Central line placement in the internal jugular vein (IJV) can result in complications. Previous studies that examined variations in geometric anatomical parameters in paediatric populations, have reduced these risks in children. The aim of this study was to establish possible anatomical variations to improve central line placement in the adult population and demonstrate the use of a correlation heatmap in processing large amounts of data. Materials and Methods 27 volunteers were imaged using ultrasound at three different neck levels on right and left sides and various anatomical parameters were measured. Demographic information was also collected and included in the data processing. The Pearson coefficient was derived from each possible relationship between the measured parameters and was plotted in a correlation heatmap. Results Strong correlations were observed between the body mass index (BMI) and the depth of the IJV and common carotid artery (CCA), the relative depth of the IJV to the CCA. No significant correlations were found in the dimensions of both vessels. Conclusions Anatomical variations were more common than anticipated and should be taken into account when performing central line catheterization in order to minimize post‐procedure complications. Comparison of Central and Peripheral Arterial Blood Pressure Gradients in Critically Ill Patients: A Systematic Review and Meta-Analysis

Abstract:Objectives: Measurement of blood pressure taken from different anatomical sites, are often perceived as interchangeable, despite them representing different parts of the systemic circulation. We aimed to perform a systematic review and meta-analysis on blood pressure differences between central and peripheral arterial cannulation in critically ill patients. Data sources: We searched MEDLINE, Cochrane Central Register of Controlled Trials, and Embase from inception to December 26, 2023, using Medical Subject Headings (MeSH) terms and keywords. Study selection: Observation study of adult patients in ICUs and operating rooms who underwent simultaneous central (femoral, axillary, or subclavian artery) and peripheral (radial, brachial, or dorsalis pedis artery) arterial catheter placement in ICUs and operating rooms. Data extraction: We screened and extracted studies independently and in duplicate. We assessed risk of bias using the revised Quality Assessment for Studies of Diagnostic Accuracy tool. Data synthesis: Twenty-four studies that enrolled 1598 patients in total were included. Central pressures (mean arterial pressure [MAP] and systolic blood pressure [SBP]) were found to be significantly higher than their peripheral counterparts, with mean gradients of 3.5 and 8.0 mm Hg, respectively. However, there was no statistically significant difference in central or peripheral diastolic blood pressure (DBP). Subgroup analysis further highlighted a higher MAP gradient during the on-cardiopulmonary bypass stage of cardiac surgery, reperfusion stage of liver transplant, and in nonsurgical critically ill patients. SBP or DBP gradient did not demonstrate any subgroup specific changes. Conclusions: SBP and MAP obtained by central arterial cannulation were higher than peripheral arterial cannulation; however, clinical implication of a difference of 8.0 mm Hg in SBP and 3.5 mm Hg in MAP remains unclear. Our current clinical practices preferring peripheral arterial lines need not change.

Abstract:The ubiquitous brachial cuff method gained widespread clinical acceptance for blood pressure recording after confirmation of its prognostic value in 1917. This method displaced radial pulse waveform analysis by sphygmography, which also gave prognostic Information but was difficult to use. Since that time, brachial cuff sphygmomanometry has migrated from the physician's office to 24-hour monitoring and home use, with electronic methods replacing the Korotkov sound technique for determining systolic and diastolic pressure. Detailed instrumental studies, required by regulatory bodies, revealed inaccuracies of all cuff methods for recording true intra-arterial pressure. A major source of inaccuracy in assessing left ventricular load is the amplification of the pressure wave in its transit from the central aorta to upper limb arteries, as extensively studied by Earl H. Wood at the Mayo Clinic in Rochester, Minn, in the 1950s. This limitation can be overcome by combining newer methods using radial artery waveform analysis in conjunction with conventional cuff sphygmomanometry to noninvasively measure the central aortic pressure waveforms. Recent studies using radial tonometry have proved that this is more effective than conventional manometry in predicting cardiovascular events and gauging response to therapy. Measurement of central as well as peripheral arterial pressure and physiology is becoming increasingly used as an office practice and a laboratory procedure.

Effects of Four Different Positive Airway Pressures on Right Internal Jugular Vein Catheterisation

Abstract:Context The right internal jugular vein (RIJV) is often used for central venous catheterisation in patients undergoing major surgery. The efficacy of this route correlates with the diameter of the vein which can be influenced by airway pressure.Objective To investigate four positive airway pressures (PAPs) in mechanically ventilated patients and to determine the most suitable of these for RIJV catheterisation.Design Prospective, randomised, controlled study.Setting Two Chinese medical centres.Patients Two hundred and forty patients scheduled for gastrointestinal tract surgery under general anaesthesia (male-to- female ratio, 135 : 105; American Society of Anesthesiologists' physical status class, I-III; age range, 19-81 years); patients were excluded from the study if they had a history of haematological disease, external neck injury, RIJV catheterisation, severe cardiovascular disease, RIJV thrombosis, injection-site infection, pneumothorax and pulmonary bulla.Interventions : They were randomised into four groups of 60, each to undergo RIJV catheterisation at a PAP of 0 (1cmH(2)O 0.098 = kPa, group A); 15 (group B); 20 (group C); and 25cmH(2)O (group D).Main outcome measures Primary outcomes were central venous pressure (CVP) and RIJV cross-sectional area which were measured before and during PAP; the number of catheterisations completed in 30 s; the number of first pass punctures; heart rate and mean arterial pressure which were monitored continuously; and the incidence of complications which included local haematoma, pneumothorax and internal carotid artery (ICA) puncture.Results Two patients were excluded following ICA puncture, leaving 238 for analysis. In groups C and D, the CVP and the cross-sectional area of RIJV were significantly larger; the number completing catheterisation within 30 s and the number of first pass punctures increased significantly and was comparable between these two groups. Compared with groups A and B, the incidences of hypotension and bradycardia were significantly increased in groups C and D. The incidence of local haematoma was significantly increased in group D compared with other groups. RIJV catheterisation was completed successfully in all but two patients in group A.Conclusion A PAP of 20cmH(2)O seems most suitable for successful RIJV catheterisation in mechanically ventilated patients. It is associated with an increase in cross-sectional RIJV area and CVP, which facilitate cannulation, and results in fewer puncture-related complications. Safety of Subclavian Venous Catheterization Using Anatomical Landmark Technique Versus Ultrasound-Guided Internal Jugular Central Venous Method Inserted in The Neuro-Intensive Care Unit

Abstract:Background:Central venous (CV) catheterization is one of the important interventions used for various purposes. Accordingly, this study aimed to compare the complication rates of internal jugular vein (IJV) assisted by ultrasound with those of subclavian vein (SCV) catheterization using the anatomical landmark technique.Methods:Prospective, nonrandomized, single center study. Data from patients undergoing central venous catheterization either via IJV or SCV were recorded. The overall complication rate as well as number of attempts and catheter misplacement were analyzed comparing both methodsResults:Data from 398 (IJV: 206; SCV:192) patients were analyzed. The rate of pneumothorax did not demonstrate a significant difference between the IJV (0.5 %) and SCV (2.1 %) groups, p = 0.2. The success rate on the first attempt was similar in both groups (64%; p = 1.0). More catheter misplacements were observed in the SCV group (4.7%) than in the IJV group (3%), p = 0.4.Conclusions:Our results demonstrated that the complication rates of anatomical landmark guided SCV is very low and comparable to US guided IJV method.

Correlation between the VExUS score and right atrial pressure: a pilot prospective observational study

Abstract:Venous congestion is an under-recognized contributor to mortality in critically ill patients. Unfortunately, venous congestion is difficult to measure, and right heart catheterization (RHC) has been considered the most readily available means for measuring venous filling pressure. Recently, a novel "Venous Excess Ultrasound (VExUS)" score was developed to noninvasively quantify venous congestion using inferior vena cava (IVC) diameter and Doppler flow through the hepatic, portal, and renal veins. A preliminary retrospective study of post-cardiac surgery patients showed promising results, including a high positive-likelihood ratio of high VExUS grade for acute kidney injury. However, studies have not been reported in broader patient populations, and the relationship between VExUS and conventional measures of venous congestion is unknown. To address these gaps, we prospectively assessed the correlation of VExUS with right atrial pressure (RAP), with comparison to inferior vena cava (IVC) diameter. Patients undergoing RHC at Denver Health Medical Center underwent VExUS examination before their procedure. VExUS grades were assigned before RHC, blinding ultrasonographers to RHC outcomes. After controlling for age, sex, and common comorbidities, we observed a significant positive association between RAP and VExUS grade (P < 0.001, R2 = .68). VExUS had a favorable AUC for prediction of a RAP ≥ 12 mmHg (0.99, 95% CI 0.96-1) compared to IVC diameter (0.79, 95% CI 0.65-0.92). These results suggest a strong correlation between VExUS and RAP in a diverse patient population, and support future studies of VExUS as a tool to assess venous congestion and guide management in a spectrum of critical illnesses.
Design and Haemodynamic Analysis of a Novel Anchoring System for Central Venous Pressure Measurement

Abstract:Background/Objective: In recent years, treatment of heart failure patients has proved to benefit from implantation of pressure sensors in the pulmonary artery (PA). While longitudinal measurement of PA pressure profoundly improves a clinician's ability to manage HF, the full potential of central venous pressure as a clinical tool has yet to be unlocked. Central venous pressure serves as a surrogate for the right atrial pressure, and thus could potentially predict a wider range of heart failure conditions. However, it is unclear if current sensor anchoring methods, designed for the PA, are suitable to hold pressure sensors safely in the inferior vena cava. The purpose of this study was to design an anchoring system for accurate apposition in inferior vena cava and evaluate whether it is a potential site for central venous pressure measurement. Materials and Methods: A location inferior to the renal veins was selected as an optimal site based on a CT scan analysis. Three anchor designs, a 10-strut anchor, and 5-struts with and without loops, were tested on a custom-made silicone bench model of Vena Cava targeting the infra-renal vena cava. The model was connected to a pulsatile pump system and a heated water bath that constituted an in-vitro simulation unit. Delivery of the inferior vena cava implant was accomplished using a preloaded introducer and a dilator as a push rod to deploy the device at the target area. The anchors were subjected to manual compression tests to evaluate their stability against dislodgement. Computational Fluid Dynamics (CFD) analysis was completed to characterize blood flow in the anchor's environment using pressure-based transient solver. Any potential recirculation zones or disturbances in the blood flow caused by the struts were identified. Results: We demonstrated successful anchorage and deployment of the 10-strut anchor in the Vena Cava bench model. The 10-strut anchor remained stable during several compression attempts as compared with the other two 5-strut anchor designs. The 10-strut design provided the maximum number of contact points with the vessel in a circular layout and was less susceptible to movement or dislodgement during compression tests. Furthermore, the CFD simulation provided haemodynamic analysis of the optimum 10-strut anchor design. Conclusions: This study successfully demonstrated the design and deployment of an inferior vena cava anchoring system in a bench test model. The 10-strut anchor is an optimal design as compared with the two other 5-strut designs; however, substantial in-vivo experiments are required to validate the safety and accuracy of such implants. The CFD simulation enabled better understanding of the haemodynamic parameters and any disturbances in the blood flow due to the presence of the anchor. The ability to place a sensor technology in the vena cava could provide a simple and minimally invasive approach for heart failure patients.
