Abstract:Objectives To assess laser–tissue interactions through ablation, coagulation, and carbonisation characteristics in a non‐perfused porcine kidney model between three pulsed lasers: holmium (Ho): yttrium‐aluminium‐garnet (YAG), thulium fiber laser (TFL), and pulsed thulium (p‐Tm):YAG. Materials and Methods A 150‐W Ho:YAG, a 60‐W TFL, and a 100‐W p‐Tm:YAG lasers were compared. The laser settings that can be set identically between the three lasers and be clinically relevant for prostate laser enucleation were identified and used on fresh, unfrozen porcine kidneys. Laser incisions were performed using stripped laser fibers of 365 and 550 μm, set at distances of 0 and 1 mm from the tissue surface at a constant speed of 2 mm/s. Histological analysis evaluated shape, depth, width of the incision, axial coagulation depth, and presence of carbonisation. Results Incision depths, widths, and coagulation zones were greater with Ho:YAG and p‐Tm:YAG lasers than TFL. Although no carbonisation was found with the Ho:YAG and p‐Tm:YAG lasers, it was common with TFL, especially at high frequencies. The shapes of the incisions and coagulation zones were more regular and homogeneous with the p‐Tm:YAG laser and TFL than with Ho:YAG laser. Regardless of the laser used, short pulse durations resulted in deeper incisions than long pulse durations. Concerning the distance, we found that to be effective, TFL had to be used in contact with the tissue. Finally, 365‐μm fibers resulted in deeper incisions, while 550‐μm fibers led to wider incisions and larger coagulation zones. Conclusion Histological analysis revealed greater tissue penetration with the p‐Tm:YAG laser compared to the TFL, while remaining less than with Ho:YAG. Its coagulation properties seem interesting insofar as it provides homogeneous coagulation without carbonisation, while incisions remained uniform without tissue laceration. Thus, the p‐Tm:YAG laser appears to be an effective alternative to Ho:YAG and TFL lasers in prostate surgery.

Experimental Study of Diode-Laser Induced Thermocoagulation on Hepatic Tissue with Scanner Fiber Tip

Abstract:AIM:To explore a safe, efficient, and cost-effective technique for local thermo-ablation of hepatic tumors.METHODS:The livers of 16 healthy rabbits were thermocoagulated by diode-laser with a hand-made scanner fiber tip, 6 w for 10 min. At the same time, the temperature was measured at 5 and 10 mm from the laser tip. Liver function 7 days post-thermocoagulation was compared to pre-thermocoagulation. Pathological changes were also studied 1 month after laser thermocoagulation.RESULTS:All the rabbits lived and the temperature of hepatic tissues at 0 mm, 5 mm, 10 mm from laser tip reached 96.39+/-3.97 degrees, 60.79+/-6.21 degrees, 46.10+/-4.58 degrees, respectively after 10 min thermocoagulation. There was no significant change in liver function. The hepatic thermocoagulated necrosis and the surrounding fibrosis was 26.0 mm in diameter. Light microscopy observation revealed no surviving cells in the coagulated area.CONCLUSION:Hepatic tissue can be locally ablated safely and effectively by diode-laser with scanner fiber tip, and this technique may be a new method to treat hepatic tumors.
Safety of a Novel Thulium Fiber Laser for Lithotripsy: an in Vitro Study on the Thermal Effect and Its Impact Factor.

Abstract:Introduction: To investigate the thermal effect on the water by a novel thulium fiber laser (TFL) designed for lithotripsy and evaluate the safety of this laser for clinical use. Materials and Methods: An in vitro experimental setup was constructed. A test tube filled with saline was immersed in an electric water bath, and a TFL fiber and a thermal probe were inserted into it. Saline was irrigated into the tube and pumped out synchronously at the same speed by two pumps, respectively, to maintain convection when needed. Then, continuous TFL firing of different power settings was imposed to saline in the tube for 60 seconds, on the conditions of different irrigation rates. The temperature was recorded every 5 seconds during the whole trial, and each trial was repeated five times. Safety threshold of temperature increase (STTI) was determined comparing with the deemed safe temperature of 43 degrees C in vivo. Results: On condition of 0 mL/min irrigation rate, STTI was 6.5 degrees C, and water temperature increase (WTI) caused by >= 15 W settings surpassed STTI after 20 seconds of laser firing; on condition of 15 mL/min irrigation rate, only WTI caused by the highest 30 W power setting surpassed STTI after 45 seconds of laser firing. When irrigation rate was added up to 25 and 50 mL/min, WTIs caused by all power settings were below STTIs in a 60-second experiment. High frequency and low pulse energy combinations caused a slightly higher WTI compared with low frequency and high pulse energy, given a constant power and irrigation rate. Conclusion: Power setting and irrigation rate collaboratively play a critical role in WTI during TFL lithotripsy, and it is safe to use TFL referring to the thermal effect as long as there is moderate irrigation, while TFL power should be lowered enough when irrigation is ceased.
A Fiber-Optic Thermal Source For Laser Surgery Applications

Abstract:A novel fiber-optic thermal source, consisting of a Nd3+ doped fiber tip and a pure YAG crystal fiber that grown by LHPG(Laser heated pedestal growth) method, had been studied for laser surgery applications. Pumping by a CuBr laser, the thermal source generated temperature as high as 520 degreesC with stability of better than 4% (absolute temperature), the thermal response of the thermal source is about 3 seconds, and the temperature can be controlled by the input laser power. Experimental results show that, the fiber-optic thermal source is promising for laser surgery applications, and is expected to avoid overheating that likely to happen in direct laser tissue interactions.

Abstract:Through an ex vivo experimental study, we aimed to compare the effects of the Ho: YAG laser Virtual Basket (VBTM) modulation and a Thulium fiber laser (TFL) on kidney tissue in different environments and using laser configurations. The 100 W Ho: YAG (Cyber Ho, Quanta System, Italy) and 60 W TFL (Fiber Dust, Quanta System, Italy) laser devices were used. The following laser settings were selected: power in the range of 10–60 W, frequency of 20–40 Hz, and energy of 0.5–1–1.5 J. A medium pulse duration of 600 μsec was used for VBTM, while short (spdTFL; 50 μsec) and long (lpdTFL; 15,000 μsec) were used for TFL. The tissue's incision depth (ID), vaporization area (VA), coagulation area (CA), total laser area (TLA = VA + CA), surface section (SS), and lateral effect (LE) were measured. In total, 108 experiments were conducted. No statistically significant difference in mean VA, TLA, ID, LE, or SS was observed between VBTM, spdTFL, and lpdTFL in the low-power output group in saline ( p > 0.05). However, the mean CA was statistically significantly higher for VBTM ( p = 0.005). In saline and high-power output group, the mean VA, CA, TLA, LE, and ID were higher when using lpdTFL than other pulse durations ( p = 0.001, p = 0.001, p = 0.001, p = 0.006, and p = 0.001, respectively). Similar to lpdTFL, VBTM may provide controlled dissection and incision as well as haemostasis. At different laser settings, the individual effects of laser properties (such as pulse length, energy and frequency) on tissue may be more significant.

Abstract:Background and ObjectivesTo evaluate the feasibility of using a novel blue diode laser (blue laser), a thulium fiber laser (Tm laser), and their combination as a directed‐energy surgical tool in laparoscopic partial nephrectomy (LPN). Study Design/Materials and MethodsThe blue laser emitting at 442 nm, the Tm laser emitting at 1,940 nm wavelengths, and the combination of them were tested. First, cutting and coagulative abilities of the lasers were characterized ex vivo on porcine kidney in air and CO2. Histological staining was performed to assess the efficiency of ablation and coagulation. Next, experimental LPN was performed on a porcine model at zero ischemia. Upper and lower segments of both kidneys were resected. Total operation time and resection time were measured; bleeding and carbonization were evaluated. ResultsEx vivo data show that laser‐induced ablation and coagulation processes do not differ substantially between CO2 and air environments. Histological analysis of ex vivo incisions demonstrates that the blue laser produced deep ablation with relatively narrow coagulation zone, whereas Tm laser was less efficient in terms of ablation but possessed excellent coagulative properties. Experimental LPN revealed that the blue laser provided fast cutting with minimal carbonization, whereas Tm laser induced slow cutting with strong carbonization. The combination of the blue and Tm lasers provided the most promising results demonstrating the highest resection rate, almost carbonization free resection surface and clinically acceptable hemostasis enabling LPN without the need for vessel clamping. ConclusionsThe blue laser can be efficiently utilized in LPN. Furthermore, the combination of the blue and Tm lasers into a single modality may be beneficial for further development of successful laser‐assisted LPN. Lasers Surg. Abstract:Introduction: Several preclinical studies about a novel pulsed-thulium:yttrium-aluminum-garnet (p-Tm:YAG) device have been published, demonstrating its possible clinical relevance. Methods: We systematically reviewed the reality and expectations for this new p-Tm:YAG technology. A PubMed, Scopus and Embase search were performed. All relevant studies and data identified in the bibliographic search were selected, categorized, and summarized. Results: Tm:YAG is a solid state diode-pumped laser that emits at a wavelength of 2013 nm, in the infrared spectrum. Despite being close to the Ho:YAG emission wavelength (2120 nm), Tm:YAG is much closer to the water absorption peak and has higher absorption coefficient in liquid water. At present, there very few evaluations of the commercially available p-Tm:YAG devices. There is a lack of information on how the technical aspects, functionality and pulse mechanism can be maximized for clinical utility. Available preclinical studies suggest that p-Tm:YAG laser may potentially increase the ablated stone weight as compared to Ho:YAG under specific condition and similar laser parameters, showing lower retropulsion as well. Regarding laser safety, a preclinical study observed similar absolute temperature and cumulative equivalent minutes at 43° C as compared to Ho:YAG. Finally, laser-associated soft-tissue damage was assessed at histological level, showing similar extent of alterations due to coagulation and necrosis when compared with the other clinically relevant lasers. Conclusions: The p-Tm:YAG appears to be a potential alternative to the Ho:YAG and TFL according to these preliminary laboratory data. Due to its novelty, further studies are needed to broaden our understanding of its functioning and clinical applicability.
Treatment for Ulnar Neuritis Around the Elbow in Adolescent Baseball Players: Factors Associated With Poor Outcome

Abstract:Background: Ulnar neuritis around the elbow is one of the injuries seen in throwing athletes. Outcomes of nonsurgical treatment and factors associated with failure outcomes have not been reported. Purpose: To investigate the outcomes of treatments for ulnar neuritis in adolescent baseball players. Study Design: Case series; Level of evidence, 4. Methods: We assessed 40 male baseball players with a mean age of 15.0 years (range, 13-17 years) who presented with ulnar neuritis. There were 19 pitchers and 21 fielders whose throwing side was affected. All patients had elbow pain, and 13 patients had hand numbness on the ulnar side. The mean Kerlan-Jobe Orthopaedic Clinic (KJOC) overhead athlete shoulder and elbow score was 52.5 at the first follow-up visit (n = 36 patients). Thirteen patients were identified with ulnar nerve subluxation, and 23 patients had concomitant elbow ulnar collateral ligament (UCL) injury. All patients underwent nonsurgical treatment, which included rehabilitation exercises and prohibition of throwing. If the nonsurgical treatment failed, we recommended surgical treatment. We investigated the outcomes of the nonsurgical and surgical treatments. Return to sports was evaluated, combined with factors associated with return to sports in nonsurgical treatment by univariate and multivariate statistical analysis. Results: The mean follow-up period was 23.6 months (range, 6-39 months). After nonsurgical treatment, 24 patients (60%) returned to the previous competition level after a mean of 2.4 months. Two patients returned to a recreational level. One patient gave up playing baseball at 2 months. The remaining 13 patients underwent surgery and returned to sports after a mean of 2.0 months postoperatively, and 12 had no limitation of sports activities. Multivariate logistical regression analysis demonstrated that hand numbness, ulnar nerve subluxation, and UCL injury were associated with failure of nonsurgical treatment (P < .05). In addition, KJOC score of <45 at the first follow-up tended to be associated with poor outcomes of nonsurgical treatment (P = .06). Conclusion: Hand numbness on the ulnar side, ulnar nerve subluxation, and UCL injury are strong predictors of poor outcomes after nonsurgical treatment for ulnar neuritis, and surgery provides excellent results.

Abstract:As laser technology has advanced, high-power lasers have become increasingly common. The Holmium: yttrium–aluminum-garnet (Ho:YAG) laser has long been accepted as the standard for laser lithotripsy. The thulium fiber laser (TFL) has recently been established as a viable option. The aim of this study is to evaluate thermal dose and temperature for the Ho:YAG laser to the TFL at four different laser settings while varying energy, frequency, operator duty cycle (ODC). Utilizing high-fidelity, 3D-printed hydrogel models of a pelvicalyceal collecting system (PCS) with a synthetic BegoStone implanted in the renal pelvis, laser lithotripsy was performed with the Ho:YAG laser or TFL. At a standard power (40W) and irrigation (17.9 ml/min), we evaluated four different laser settings with ODC variations with different time-on intervals. Temperature was measured at two separate locations. In general, the TFL yielded greater cumulative thermal doses than the Ho:YAG laser. Thermal dose and temperature were typically greater at the stone when compared away from the stone . Regarding the TFL, there was no general trend if fragmentation or dusting settings yielded greater thermal doses or temperatures. The TFL generated greater temperatures and thermal doses in general than the Ho:YAG laser with Moses technology. Temperatures and thermal doses were greater closer to the laser fiber tip. It is inconclusive as to whether fragmentation or dusting settings elicit greater thermal loads for the TFL. Energy, frequency, ODC, and laser-on time significantly impact thermal loads during ureteroscopic laser lithotripsy, independent of power.

Abstract:Elderly people who are beginning to " fail " often show changes in mentality or behaviour as the first sign of their deterioration. During the last two years a number of such patients have been studied at the Royal Hospital, Chelsea. Among them were two groups of cases which seemed to be associated with the presence of cerebral ischaemia. One of these, in which the condition remained stationary, appeared to correspond to the 'relative hypotension " of Stieglitz (1935). This was one of the commoner forms of senile dementia. The other, described below, was progressive through three distinct stages to a fatal termination. Clinical Picture Of the 27 patients in this series all but two were Chelsea pensioners. Their ages ranged from 70 to 87. Fifteen had but recently recovered from an infection of the respiratory tract; 14 were suffering from congestive heart failure, which followed bronchitis or bronchopneumonia in 7 of them; 9 had had previous cerebral thrombosis; I had infection, cerebral thrombosis, and heart failure; while 1 had no known predisposing factor apart from hypertension.

Abstract:Objectives: Photoangiolytic lasers have yielded significant innovation in laryngeal surgery in the last 25 years. After the discontinuation of the potassium titanyl phosphate (KTP) laser, a novel 445-nm blue laser was developed. The optimal balance between a laser's desired tissue effects and collateral tissue damage is a major determinant of laser selection in microlaryngeal surgery. The shell-less incubation system for the chick chorioallantoic membrane (CAM) simulates the microvasculature of the human vocal fold and is useful for testing effects of laser settings and in simulated surgery. The aim of this study is to compare the tissue effects of the KTP and blue lasers using the shell-less CAM model. Methods: The shell-less incubation system contains: polymethylpentene film (used as a culture vessel), calcium lactate and distilled water supplementations. By using this system, the chick chorioallantoic membrane (CAM) can be fully exposed with a good field for surgery simulation. The effects of the 2 lasers (532 nm KTP and 445 nm blue) were quantified at clinically relevant energy settings and laser distances from target. Measures included imaging real-time vascular reactions in the CAM model, post-procedure histologic analysis of CAM tissue and temperature changes. Results: Vessel coagulation and rupture rates were less common with the blue laser compared with the KTP laser. Histologic analysis demonstrated less tissue disruption with the blue laser. Temperature changes were less with the blue laser. Conclusion: In this CAM model with specific conditions, the blue laser reveals less tissue damage than the KTP laser. Suitable working distance and power setting of the laser are necessary for desired tissue effects.Level of Evidence: Level 3.
Ex Vivo Experimental Study on the Thulium Laser System: New Horizons for Interventional Endoscopy (with Videos).

Abstract:Background and study aims The Thulium laser system (TLS) is an emerging interventional tool adopted in many surgical specialties. Its 2.0-mu m wavelength allows precise coagulation (0.2-0.4mm in depth) and cutting, limiting the possibilities of collateral injuries. We tested the impact of the TLS for gastric endoscopic submucosal dissection (ESD) and per oral endoscopic myotomy (POEM) ex vivo in pigs. Materials and methods Ex vivo porcine stomach and esophagus models underwent 2 POEMs, and 3 ESDs (mean diameter 3.5 cm) with TLS using a 272-mu m and a 365-mu m thick optical fibers. Both continuous and pulsed laser emission were evaluated. Subsequent histopathological analysis was performed by an expert GI pathologist on the whole porcine models. Results Complete POEMs and gastric ESDs were successfully performed in all cases in 30 to 70 and 15 to 20 minutes. Both optical fibers were equally effective and precise. The best power output for mucosal incision was 25 to 30W during ESD and 25W for POEM using continuous laser emission. During submucosal dissection and tunneling the favorite power output was 20W and 15 to 20W, respectively, operating in continuous mode. No transmural perforation occurred throughout the operations and histopathology confirmed the absence of accidental muscular layer damage. Conclusions The TLS stands out as a precise and manageable instrument in ex vivo models. This technique appears to be a promising tool for advanced interventional endoscopy.
Evaluation of Different Dental Lasers' Ability to Congeal Pooled Blood: An In Vitro Study

Abstract:Most dental lasers claim that they can aid in hemostasis during oral and periodontal surgery. To date, there are no studies that compare different lasers' ability to congeal pooled blood. The aim of the study was to see if there was a difference in dental lasers' ability to congeal pooled human blood in vitro. Whole blood was collected from donors, with 0.5 mL (premolar socket volume for all tests) aliquoted into microcentrifuge tubes. Different dental lasers (810-nm diode, 940-nm diode, 1,064-nm Nd:YAG, 2,790-nm Er,Cr:YSGG, 2,940-nm Er:YAG, and 10,600-nm CO2) were applied to the whole blood for 0, 15, 30, and 45 seconds. The sample tubes were centrifuged, and the supernatant color was scored to assess the degree of congealing. Additional samples of blood were tested for time needed for maximum congealing and temperature change. Analysis of supernatant colors showed that there were significant differences in the degrees of congealing for the 810-nm diode, 940-nm diode, and 2,790-nm Er,Cr:YSGG lasers when compared to the 1,064-nm Nd:YAG, 2,940-nm Er:YAG, and 10,600-nm CO2 lasers, but not within those groupings. Additionally, the 1,064-nm Nd:YAG laser increased the temperature of the blood samples more than the other lasers and had a shorter time for maximum congealing. There were differences in the dental lasers' ability to congeal pooled human blood in an in vitro model. Nd:YAG, Er:YAG, and 10,600-nm CO2 lasers were able to achieve a greater degree of congealing at an earlier time point. The Nd:YAG laser produced the most heat and was the fastest to complete coagulation.

Abstract:This article describes bent π-conjugated systems composed of alternating o-phenylene and Z-vinylene units. all-Z-[n]Benzo[4n]annulenes are higher homologues of dibenzocyclooctatetraene (DBCOT) with a concave π system, and attempts were made to convert [20]- and [24]annulenes having partial belt structures of fullerenes and carbon nanotubes into [10]- and [12]phenacenes. A bent π-conjugated system composed of two DBCOT units showed dynamic syn-anti equilibrium in solution and behaved as dynamic molecular tweezers (DMTs). The syn isomers of the DMTs formed blue charge-transfer complexes with DDQ in solution, and this complexation and color change were applied to thermochromism. Furthermore, DMTs having two CN groups on each COT ring exhibited crystalline-state emission in the presence of solvent molecules, although almost no emission was observed in solution or the solid state. Based on this crystalline-state emission, a vapochromic system was established. The multifunctional properties of DMTs caused by the flexibility of the nonplanar π system are summarized.

Abstract:Objective : To evaluate using an inanimate model the thermal injury and laser efficiency on high frequency, high energy and its combination in hands of junior and experienced urologists during holmium YAG (Ho:YAG) and Thulium fiber laser (TFL) lithotripsy. Methods : Cyber: Ho 150 WTM and Fiber Dust TFL (Quanta System) with 200μm core-diameter laser fibers (LF) were used in a saline in vitro ureteral model. Each participant (5 junior -5 experienced urologist) performed 32 sessions of 5 minutes lasering (125 mm3 phantom BegoStonesTM), comparing four modes (3J/5Hz (1.5W), 0.3J/20Hz (6W), 1.2 J/5Hz (6W), and 1.2J/20Hz (24W)). Transparent tip and cleaved LF, and digital and fiberoptic ureteroscopes were also compared. Ureteral damage was classified in a scale (0-5) according to the burns and holes seen in the ureteral model's surface. Results : High-power setting (24W) was associated with higher delivered energy and higher ablation rates in both lasers (p<0.001). For the same power setting (6W), there were no differences in delivered energy nor stone ablation rates. Regardless the settings, higher ablation rate was observed with TFL than with Ho:YAG ((0.5Δmg/s ± 0.33 vs 0.39 Δmg/s ± 0.31, p:0.002) laser. Higher mean ablation rate was found with cleaved tip vs transparent tip (p:0.03) in TFL. For both lasers, higher ureteral damage was observed in the 24W group (p: 0.006) and in the junior urologists (p: 0.03). Between 6W groups, different type of lesions were found and junior urologist have more lesions when high frequency was used, for both Ho:YAG (p=0.05) and TFL (p=0.04). Conclusion : High-power settings have more stone ablation rate and may reduce the operative time; however, more ureteral thermic-related damage is produced. When comparing the same power, higher energy or frequency does not modify the ablation rate. Nonetheless, more ureteral thermic-related thermal damage is observed in high frequency settings in unexperienced hands.

Efficiency of soft tissue incision with a novel 445-nm semiconductor laser

Abstract:Using a 445-nm semiconductor laser for tissue incision, an effective cut is expected due to the special absorption properties of blue laser light in soft tissues. The aim of the present study was the histological evaluation of tissue samples after incision with a 445-nm diode laser. Forty soft tissue specimens were obtained from pork oral mucosa and mounted on a motorized linear translation stage. The handpiece of a high-frequency surgery device, a 970-nm semiconductor laser, and a 445-nm semiconductor laser were connected to the slide, allowing a constant linear movement (2 mm/s) and the same distance of the working tip to the soft tissue’s surface. Four incisions were made each: (I) 970-nm laser with conditioned fiber tip, contact mode at 3-W cw; (II–III): 445-nm laser with non-conditioned fiber tip, contact mode at 2-W cw, and non-contact mode (1 mm) at 2 W; and (IV): high-frequency surgery device with straight working tip, 90° angulation, contact mode at 50 W. Histological analysis was performed after H&E staining of the embedded specimens at 35-fold magnification. The comparison of the incision depths showed a significant difference depending on the laser wavelength and the selected laser parameters. The highest incision depth was achieved with the 445-nm laser contact mode (median depth 0.61 mm, min 0.26, max 1.17, interquartile range 0.58) (p < 0.05) with the lowest amount of soft tissue denaturation (p < 0.05). The lowest incision depth was measured for the high-frequency surgical device (median depth 0.36 mm, min 0.12, max 1.12, interquartile range 0.23) (p < 0.05). Using a 445-nm semiconductor laser, a higher cutting efficiency can be expected when compared with a 970-nm diode laser and high-frequency surgery. Even the 445-nm laser application in non-contact mode shows clinically acceptable incision depths without signs of extensive soft tissue denaturation.

Abstract:Focal laser ablation is a minimally invasive method of treating cancerous lesions in organs such as prostate, liver and brain. Oncologic control is achieved by inducing hyperthermia throughout the target while minimizing damage to surrounding tissue. Consequently, successful clinical outcomes are contingent upon achieving desired ablation volumes. Magnetic resonance thermometry is frequently used to monitor the formation of the induced thermal damage zone and inform the decision to terminate energy delivery. However, due to the associated cost and complexity there is growing interest in the development of alternative approaches. Here we investigate the utility of real-time interstitial interrogation of laser-tissue interaction as an inexpensive alternative monitoring modality that provides direct assessment of tissue coagulation without the need for organ specific calibration. The optical contrast mechanism was determined using a Monte Carlo model. Subsequently, four interstitial probe designs were manufactured and assessed in a tissue mimicking phantom under simultaneous magnetic resonance imaging. Finally, the optimal probe design was evaluated in ex vivo bovine muscle. It was found to be capable of providing sufficient feedback to achieve pre-defined ablation radii in the range 4-7 mm with a mean absolute error of 0.3 mm. This approach provides an inexpensive monitoring modality that may facilitate widespread adoption of focal laser ablation.

Abstract:Purpose: Little is known about the soft tissue destruction by holmium laser clinically used for holmium laser enucleation of the prostate (HoLEP), subject to the distance between the laser fiber tip and the tissue surface. We aimed to investigate the impact of the distance between the laser fiber tip and the phantom surface (DLP) on a soft tissue phantom (STP) in relation to the surgical modes of HoLEP. Methods: STP responses to the laser pulses produced by a commercial holmium:yttrium aluminum garnet (Holmium:YAG) laser at an output setting 2 J were observed at different values of the DLP (0, 1, 2, 3, and 4 mm) to look at (1) the single laser pulse-induced cavitation bubble and its penetration into the STP, (2) the STP destruction by a single pulse, (3) the STP destruction by 60 pulses repeated at 12 Hz, and (4) the thermal effect by the multiple pulses visualized on a thermosensitive bovine serum albumin (BSA) STP. Results: We observed that the laser pulse produced a heated gas bubble in water centered at the laser fiber tip. The bubble shape depended on the DLP. The bubble completely penetrated into the STP at the DLP of 0 mm and the penetration decreased with the DLP. The size of the destruction of the STP by the laser pulses was shown to decrease as the DLP increased. Test with the BSA STP showed that, at the DLP of 3 mm, the destruction became insignificant while the thermal effects were still effective. Conclusion: We illustrated that soft tissue destruction by the Holmium:YAG laser is associated with cavitation effects. We provide for the first time experimental evidence for various surgical modes in HoLEP such as incision and hemostasis in relation to the DLP.
Ablation Effect Comparison of Solid State Midinfrared Lasers Applied to TMLR

Abstract:Transmyocardial laser revascularization was found to be a new emerging technique for the treatment of end-stage coronary artery diseases. In our study, comparison of the ablation effect of the various infrared solid-state laser radiations on myocardial tissue was studied. Tm:YAG, CTH:YAG, CTE:YAG and Er:YAG lasers with the fiber and waveguide delivery systems were prepared and the ablation coefficients for the radiation 2.01 mum (Tm:YAG), 2.1 mum (CTH:YAG), 2.67 mum (CTE:YAG) and 2.94 mum (Er:YAG) interacting with myocardial tissue were measured. From the histological examination follows that the channels ablated by Er:YAG laser radiation are without any thermal damage of the surrounding tissue and with the minimal mechanical injury, therefore Er:YAG laser could be considered as a good candidate for the TMLR.

Abstract:X-linked lymphoproliferative disease (XLP) is a rare genetic syndrome that continues to serve as a useful model to understand more broadly the role of immunodeficiency and the pathogenetic mechanisms for the spectrum of Epstein-Barr virus (EBV)-induced diseases to which XLP is predisposed. Apart from XLP, EBV infection is related to the high frequency of non-Hodgkin's lymphoma in children with various primary immune deficiency diseases and in allograft recipients. More recently, EBV has been implicated in several lymphoproliferative diseases in individuals with acquired immune deficiency syndrome. Studies thus far on patients with XLP suggest that immune deficiency is a major determinant of these diseases. Additional molecular aberrations must be necessary in the pathogenesis of lymphoma to convert polyclonal to monoclonal disease.
