Abstract:Editor, We read with great interest the recently published article by Choi et al.,1 which evaluated the effect of pre-operative diameter and collapsibility index of the subclavian vein or the infraclavicular axillary vein for predicting intra-operative hypotension after induction of anaesthesia. The authors stated that the collapsibility index of the infraclavicular axillary vein during deep inspiration was a significant predictor of intra-operative hypotension after induction of anaesthesia. We highly appreciate their efforts and their innovative study. However, there are several issues in this study that were not clearly addressed, which may affect the validity of the conclusions. First, according to the authors, general anaesthesia was induced with midazolam (1 to 3 mg), fentanyl (1 to 2 μg kg−1), propofol (1.5 to 2 μg kg−1), rocuronium (0.6 mg kg−1) and maintained with sevoflurane (1 to 2%). However, whether the depth of anaesthesia was equivalent in all patients and how depth of anaesthesia was monitored was not clearly described in this article. As excessive anaesthetic depth is closely related to intra-operative hypotension,2 we suggest that bispectral index should be used to monitor depth of anaesthesia to avoid excessively deep anaesthetic states and associated hypotension, especially in elderly patients. Second, to our knowledge, different modes of mechanical ventilation may have different haemodynamic effects and haemodynamic compromise may happen more frequently during a period with high positive end-expiratory pressure.3 In other words, a high positive end-expiratory pressure may be more likely to provoke intra-operative hypotension due to a reduction of venous return attributable to an increase in the intrathoracic pressure. However, it was not clearly described whether the mode of mechanical ventilation settings was identical in all patients. Thus, this may bias the interpretation of the findings. Finally, to ensure consistent infraclavicular axillary vein measurements, we suggest that measurements should be carried out three times in each patient, and the average value taken. We believe that addressing the above issues would further help to support the conclusions of this study.

Abstract:Dry or moist skin-contact thermal stimulation for vein puncture (VP) and vein cannulation (VC) may not be feasible for sensitive skin. For a damaged, burned, or dark skin, near-infrared (NIR) imaging is preferred to visualize a vein. Postprocessing of NIR images is always required because the skin is a reflective material and veins need segmentation for quantitative analysis. Our pilot study aims to observe the effect of noncontact local heating on the superficial metacarpal veins in the dorsal surface of the hand and to visualize vein dynamics using an NIR imaging system. Our experiment consists of studies A and B at two ambient temperatures, 19°C and 25°C. A simple reflection-based NIR imaging system was installed to acquire sequential vein images for 5 min before and after applying 10 min of radiant thermal stimulation. To measure the vein diameter (VD), we trained a convolutional neural network (CNN) on sequential raw images to predict vein-segmentation masks as output images. Later these masked images were postprocessed for the VD measurements. The average VD was significantly increased after thermal stimulation in study A. The maximum increments in VD were 39.3% and 9.19%, 1 min after thermal stimulation in studies A and B, respectively. Both the VD and skin temperature (Tskin) follow negative exponentials in time, and the VD is proportional to Tskin. A multiple linear-regression model was made to predict the final VD. A significant difference was observed in the change of the VD. NIR imaging with CNN can be used for quantitative analyses of vein dynamics. This finding can be further extended to develop real-time, image-guided medical devices by integrating them with a radiant heater and to assist medical practitioners in achieving high success rates for VP or VC.

Abstract:OBJECTIVES: To evaluate the effectiveness of a vein visualization display system using near-infrared light ("Vein Display") for the safe and proper selection of venipuncture sites for indwelling needle placement in the forearm.METHODS: Ten second year nursing students were recruited to apply an indwelling needle line with and without Vein Display. Another ten participants were recruited from various faculty to serve as patients. The quality of the venipuncture procedure at various selected sites was evaluated according to a scale developed by the authors. Time, scores and patterns of puncture-site selection were compared with respect to three different methods: [1] attempt 1 (tourniquet only), [2] attempt 2 (Vein Display only) and [3] attempt 3 (both). To validate the effectiveness of Vein Display, 52 trials were conducted in total.RESULTS: We found that venipuncture site selection time was significantly improved with the Vein Display, particularly in the case of difficult to administer venipuncture sites. Overall, we found no significant difference with respect to venipuncture quality, as determined by our scale.CONCLUSION: These results suggest that equipment such as the Vein Display can contribute immensely to the improvement of practical skills, such as venipuncture, especially in the context of elderly patients.

Microbiological safety of household membrane water filter.

Abstract:Waterborne pathogens outbreaks are major reasons of diarrhea disease worldwide. Detecting and monitoring emerging waterborne pathogens (EWPs) is important for drinking water microbiological safety. The microbiological safety of household water hollow fiber membrane filter which is the end of drinking water treatment process was studied with heterotrophic plate count (HPC) and real-time PCR method. The effect of the flow rate, idle time and washing fashion were investigated. Among the selected filters from three manufacturers, only the PVDF membrane water filter (Brand B) could achieve a good water purification criteria. Brand A was found a certain degree of EWPs in its effluent. The lowest bacteria-removing efficiency of the PVC membrane water filter was found Brand C. Our study showed that the microorganisms could reach up to 10(6) CFU ml(-1) and the 16s rDNA could reach up to 10(6) copies ml(-1) in the initial filtrate of Brand C. More species and amounts of EWPs were detected in the washing water. These results suggested that the popular household membrane filters might cause microbiological risks at certain circumstances such as the shock load of EWPs and leakage of the membranes in the case of abnormal source water or poor membrane filter quality.

Abstract:Moderate Resolution Imaging Spectroradiometer (MODIS) snow-cover maps have been available since September 13, 2000. These products, at 500-m spatial resolution, are available through the National Snow and Ice Data Center Distributed Active Archive Center in Boulder, Colorado. By the 2001-02 winter, 5-km climate-modeling grid (CMG) products will be available for presentation of global views of snow cover and for use in climate models. All MODIS snow-cover products are produced from automated algorithms that map snow in an objective manner. In this paper, we describe the MODIS snow products, and show snow maps from the fall of 2000 in North America.

Abstract:Objective To assess the value of high-frequency ultrasound guided venipuncture of forearm superficial veins. Methods Forty patients with failure of direct venipuncture were given high-frequency ultrasound guided venipuncture. After the direction and depth of superficial veins were identified by high-frequency ultrasonography, a re-puncture was performed. Real-time ultrasonic guidance was employed in case of necessity. Results High-frequency ultrasound identification was used in 11 patients, and real-time ultrasonic guidance was used in 29 patients. All the procedures were successfully completed on one session. Conclusions High-frequency ultrasound guided venipuncture of superficial veins is a simple method without complications, being worthy of wide clinical application.

Abstract:Human blood specimen contains information about health and possible diseases that help the physician identifying the appropriate medical diagnosis. Venepuncture and intravenous cannulation are among the most common medical procedures that were performing on patients. However, there is difficulty to find the visualization of vein structures. The use of infrared radiation will be the right preference since it can penetrate the tissue and a non-invasive method. Many studies have focused on the characteristics of NIR on human skin, but the effects of exposure time as one of the design parameter in NIR exposure was not discovered. This research proposes studies that ease the handling operation and minimize the operating cost of NIR imaging in visualizing vein-structure. The study aims to measure and compare the effect of exposure time of the near infrared light emitting diodes on the vein visualization. The working principle is started with the haemoglobin in the blood absorbs the infrared light, so the vein appears darker than other areas. Then, a detection system consists of an infrared camera to capture the vein digital images. This study will then process the overall quality of the images with different exposure time by highlighting the vein-morphological structure using hessian and contrast method. The results revealed that increasing time of exposure does not increase the absorption of the NIR in both palm and arm area. Image processing further confirms this result by showing the extracted and highlighted vein. For all images, the numbers of vein appeared are the most significant factors that contribute to the vein visualization. This study can add to the process of developing a vein visualization system.

Abstract:Objective: To identify the factors related to failed peripheral venipuncture from the perspective of the situation that nurses had a difficulty.Methods: Ten nurses who were experienced in peripheral venipuncture participated in the group interview. All participants were allocated to two groups. A semi-structured interview was conducted with each group. The findings were analyzed qualitatively from the viewpoint of the situation of difficult or failed venipuncture.Results: The mean experience of nurse was 9.9±6.1 years and the mean experience of peripheral venipuncture was 5.4±3.7 years. The situations led to difficult or failed venipuncture were categorized into four groups; “cannot have confidence in venipuncture”, “low flexibility of choosing the best vein”, “being unable to calm down”, and “no support system for venipuncture”.Conclusion: This study shows that nurses cannot keep themselves from imaging failing venipuncture when they face difficult veins. Moreover, nurses may conduct venipuncture without concentrating on it under the several conditions. These factors may lead failure of venipuncture conducted by nurses. In order to increase success rate of venipuncture, we need to improve the venipuncture techniques for difficult vein and to create environment for concentrating on venipuncture.

Abstract:The effects of superficial venous intervention on hemodynamics can be quantified using two-dimensional phase-contrast magnetic resonance imaging (2D PC-MRI). Twelve patients received pre- and postintervention 2D PC-MRI analysis using quantitative hemodynamic parameters. Fifteen healthy volunteers served as controls. The 2D PC-MRI results of the target limbs (limbs scheduled for intervention for venous reflux) differed from those of the controls in terms of stroke volume (SV), forward flow volume (FFV), absolute stroke volume (ASV), and mean flux (MF) in all venous segments. The velocity time integral (VTI) and mean velocity (MV) of the popliteal vein (PV) segments were similar between the target limbs and controls preoperatively. After intervention, the target limbs exhibited an increase in VTI and MV in the femoral vein (FV) and PV segments. We compared the target and nontreated limbs of the individual patients preoperatively and postoperatively to minimalize individual bias. All QFlow parameter ratios in the FV segment increased after venous intervention (VTI, p = 0.025; MV, p = 0.024). In the PV segment, FFV and ASV increased significantly (p = 0.035 and 0.024, respectively). After interventions, the volume (FFV and ASV) of the PV segment and the efficiency (VTI and MV) of the FV segment significantly increased.

Abstract:Background: The great saphenous vein is the major superficial vein of the lower limb, and also the most often used as arterial graft material for lower limb revascularization. Prior knowledge of the quality of the vein can guide choice of therapeutic strategy, avoiding surgery that is doomed to failure. Discrepancies between intraoperative findings of the quality of the great saphenous vein and imaging tests are also frequently observed. Objectives: To evaluate the diameter of the great saphenous vein using two imaging methods (Duplex Ultrasound and Computed Tomography) and the gold-standard (intraoperative direct measurement of the vein), comparing the results. Methods: Prospective, observational study of data obtained during routine medical procedures performed by the Vascular Surgery team. Results: 41 patients were evaluated, with a 12-month follow-up. 27 (65.85%) were male and mean age was 65.37 years. 19 (46.34%) patients had femoropopliteal grafts and 22 (53.66%) had distal grafts. Preoperative saphenous vein internal diameters measured with the patient supine were on average 16.4% smaller on CT and 33.8% smaller on US than the external diameters measured after intraoperative hydrostatic dilatation. There were no statistical differences in measurements when sex, weight, and height were considered. Conclusions: Saphenous vein diameters were underestimated by preoperative US and CT scans when compared to intraoperative measurements. Thus, in patients undergoing graft planning for revascularization, the choice of conduit should take this data into consideration, so that use of the saphenous vein is not ruled out unnecessarily during planning.
The utility of quantitative calf muscle near-infrared spectroscopy in the follow-up of acute deep vein thrombosis

Abstract:Background: To investigate patterns of venous insufficiency and changes in calf muscle deoxygenated hemoglobin (HHb) levels after an acute deep vein thrombosis (DVT). Methods: A total of 78 limbs with an acute DVT involving 156 anatomic segments were evaluated with duplex scanning and near-infrared spectroscopy (NIRS) at 1, 3, 6 and 12 months. Venous segments were examined whether they were occluded, partially recanalized, and totally recanalized, and the development of venous reflux was noted. The NIRS was used to measure calf muscle HHb levels. Calf venous blood filling index (HHbFI) was calculated on standing, then the calf venous ejection index (HHbEI), and the venous retention index (HHbRI) were obtained after exercise. Results: The segments investigated were the common femoral vein (CFV; 38 segments), femoral vein (FV; 37), popliteal vein (POPV; 44), and calf veins (CV; 37). At 1 year, thrombi had fully resolved in 67% of the segments, 27% remained partially recanalized, 6% were occluded. The venous occlusion was predominant in the FV (24%) at 1 year. On the contrary, rapid recanalization was obtained in CV than proximal veins at each examination (P < 0.01). Venous reflux was predominant in POPV (55%), followed by FV (19%), and no reflux was found in CV. At 1 year, the HHbFI in POPV reflux patients was significantly higher than those with resolution (0.19 +/- 0.14, 0.11 +/- 0.05 microm s, P = 0.009, respectively). Similarly, there was a significant difference in the HHbRI between the two groups (3.08 +/- 1.91, 1.42 +/- 1.56, P = 0.002, respectively). In patients with FV occlusion, the value of HHbRI was significantly higher than those with complete resolution (2.59 +/- 1.50, 1.42 +/- 1.56, P = 0.011, respectively). Conclusions: The lower extremity venous segments show different proportions of occlusion, partial recanalization, and total recanalization. The CV shows more rapid recanalization than proximal veins. The NIRS-derived HHbFI and HHbRI could be promising parameters as the overall venous function in the follow-up of acute DVT. These findings might be very helpful for physician in detecting patients who require much longer follow-up studies.

Abstract:In its groundbreaking series of reports on patient safety, the Institute of Medicine (IOM) quotes Goethe, “Knowing is not enough; we must apply. Willing is not enough; we must do.” This commitment to knowledge and to action is essential to the magnitude of change transforming health care entails. The 10-year goal of an electronic health record for all Americans in 2014 is driving the move to the new health system for the 21st century—safe, effective, patientcentered, timely, efficient, and equitable—defined by the IOM. This carries forward the IOM’s vision of the computer-based patient record as “an essential technology for health care” set forth in 1991, when using the term “patient record” rather than “medical record” was a major step forward. Today, patient-centeredness is acknowledged as a key attribute of safer, higher-quality health care, as evident in the growing emphasis on personal health records.1 This shift in the center of care will change roles and responsibilities for patients and clinicians.

Abstract:Objective. This study evaluated the impact of a novel venipuncture method on vascular parameters of vena basilica to provide a new technical option for the clinical management of difficult venipuncture. Methods. 32 volunteers examined by ultrasound at the Medical Imaging Center of The First People's Hospital of Nantong, Jiangsu province, from February 2020 to April 2020 were randomly selected. One hand of each patient was ligated with the traditional single tourniquet and the new ligation method for 20 seconds. Then, the distance between the left or right vena basilica and the skin and the diameter of blood vessels were measured by ultrasound. 98 patients with peripheral venipuncture difficulty who were examined by ultrasound of a third-grade hospital from April 2019 to March 2020 were selected and randomly divided into control group () and experimental group (). The traditional method was used in the control group, and the new peripheral venipuncture was used in the experimental group. The success rate of single puncture, preparation time before puncture, time required for puncture operation, pain reaction during puncture, intravascular congestion after puncture, subcutaneous tissue injury, and vascular reuse rate after puncture were compared between the two methods. Results. The new ligation and puncture method increased the diameter of peripheral superficial vein vessels by , and the change of vena basilica diameter was not significantly associated with gender and age of patient. The success rate of single puncture and the vascular reuse rate were significantly higher in the experimental group than in the control group, while the preparation time and venipuncture time, as well as the patient's pain response, were significantly lower in the experimental group compared to the control group, and the intravascular congestion and subcutaneous tissue injury were lower. In addition, the mean satisfaction score of patients in the experimental group was higher than that of the control group. Conclusion. The new ligation and puncture method was an effective vena basilica dilation technique for filling the peripheral superficial veins, improving puncture success rate of peripheral difficult vein, and reducing patient pain, which was worth popularizing and applying in clinic.

Anatomical Observation of Femoral Vein to Femoral Artery and Influence Factors on Its Relation: an Ultrasonic Study

Abstract:Objective To observe the anatomic relationship of femoral vein(FV) to femoral artery(FA),and assess the effects of age,gender and side on these anatomical structures.Methods A total of 275 adult patients were enrolled in this observation.The FV and FA were investigated at 2 cm distal to the inguinal ligament by portable ultrasonography.Results A posteromedial location of the FV was the most common configuration observed on both sides(left 57.45%,right 66.18%) followed by the medial(left 37.09%,right 28.00%) and posterolateral(4.36% both side) locations.A few FV was located at lateral(left 0.36%,right 1.09%) and anteromedial(left 0.73%,right 0.36%)to the FA.Compared with left side,right FV at medial location of the FA was significantly decreased(P0.05),and at posteromedial location of the FA was significantly increased(P0.05).Subjects with a posteromedial located FV(50.74±9.92) yrs were older than those with a medial(44.78±10.28) yrs configuration(P0.05),and subjects with a posterolateral located FV(53.00±11.60) yrs were older than those with a medial configuration(P0.05).Compared with male,the posteromedial located FV on female was significantly increased(male 39.71%,female 60.29%,P0.05).Conclusion The following factors are important for dertamine the FV anatomy relationship: age,gender and left-right side effect.It is very important for the operator choose appropriate puncture point and puncture direction to potentially reduce the chance of laceration of the FA and avoid placement of a large catheter within a critical artery.

Abstract:Varicose veins in the lower extremities are dilated subcutaneous varicose veins with a diameter of ≥ 3 mm, caused by increased venous pressure resulting from backflow of blood due to venous valve insufficiency (Gloviczki in Handbook of venous disorders: guidelines of the American venous forum, Hodder Arnold, London, 2009). When diagnosing varicose veins, the shape and thickness of the blood vessels should be accurately visualized in three dimensions. In this study, we investigated a new method for numerical evaluation of vascular morphology related to varicose veins in the lower extremities, using a photoacoustic imaging (PAI) system, which can acquire high-resolution and three-dimensional images noninvasively.

Abstract:This report describes a rare occurrence of bilateral, spontaneous, nontraumatic hyphema in a 6 weeks old infant, associated with a small, multiple skin lesions. The diagnosis of juvenile xanthogranuloma was confirmed by histopathological examination of the cutaneous lesions. The hyphaema cleared gradually in 2 weeks time with conservative management.

A total of 87 patients with primary deep venous insufficiency (PDVI) using ascending venography were randomly divided into group A (44 patients) and group B (43 patients). All patients underwent encircling constriction of the superficial femoral vein, high ligation and ablation of the great saphenous vein and perforator vein. The duration of venous reflux at operation was monitored with colour Doppler in group A (but not group B) to evaluate the immediate effects. Clinical grading and scoring of the clinical, etiological, anatomical, pathophysiological (CEAP) classification system were used to evaluate the follow‐up curative effect.3. In four cases from group A, completely destroyed valves were identified at the time of operation and autografting of the vein segment with a valve was carried out. The intraoperative examination of colour Doppler in group A showed a much shorter duration of vein reflux after the encircling constriction procedure than the presurgery condition. According to the results of CEAP grading, the success rate of group A (95.0%, 38/40) was significantly higher than that of group B (76.7%, 33/43). Postoperative clinical scores were markedly lower than preoperative scores in both groups A and B.4. In conclusion, our data suggest that application of colour Doppler in encircling constriction of superficial femoral vein might enhance surgical pertinence and improve surgical effect for PDVI.
The intravascular ultrasound morphometry of iliac veins in subjects without severe chronic venous insufficiency and its implications for treatment indications and stent size selection

Abstract:Objectives: The purpose of this study is to report the intravascular ultrasound morphometry of iliac veins and its relation to demographic and anthropometric factors in subjects without chronic venous insufficiency. Methods: Thirty-three patients, without chronic venous insufficiency - qualified to great saphenous vein stripping due to unilateral, primary varicose veins - participated in the study. During the surgery, left and right external iliac veins, common iliac veins and inferior vena cava were interrogated with intravascular ultrasound. The morphometric analysis included measurement of a cross-sectional area at normal, non-stenosed vein segments (ref-CSA) and at the point of the most prominent narrowing (minimal lumen area (MLA)). Based on these measurements, a percentage of stenosis (S%) and calculated lumen diameter of interrogated veins were determined according to the following formulas, S% = (ref-CSA-MLA)/ref-CSA × 100 and CLD = 2 × √(ref-CSA/π), respectively. Results: Median ref-CSA, S% and calculated lumen diameter were 265.3 mm2, 45.8% and 18.4 mm for inferior vena cava; 193.9 mm2, 62.4% and 15.7 mm for left common iliac veins; 166.9 mm2, 35.7% and 14.2 mm for right common iliac veins; 136.5 mm2, 48.0% and 12.8 mm for left external iliac veins and 140.9 mm2, 46.3% and 13.5 mm for right external iliac veins. There were statistically significant differences between left and right common iliac veins ref-CSA, common iliac veins S% and common iliac veins calculated lumen diameter (p = 0.03, p < 0 and p = 0.03, respectively). The S% of left external iliac veins was greater in women 52.2 versus 37.2% in men (p = 0.04). Neither age nor anthropometric factors had any influence on the calculated lumen diameter of the analysed veins. A negative correlation between the left common iliac veins S% and the age was observed (p = 0.03). Conclusions: In adult subjects, the calculated lumen diameter of the common iliac veins is greater on the left side and is not influenced by age and body size. Common iliac vein stenosis occurs more frequently on the left side, decreases with age and tends to be more frequent in women.
