Abstract:BackgroundCarotid vulnerable plaque is a major cause of stroke and differs between men and women. Few studies have investigated the differences in carotid plaque features between sexes in a Chinese population.PurposeTo compare carotid atherosclerotic plaque features between men and women in a Chinese population using magnetic resonance imaging.Study typeCross‐sectional.SubjectsA total of 567 patients (mean age: 61.5 ± 10.1 years; 404 men) who had recent stroke or transient ischemia attack and atherosclerotic plaque in at least one carotid artery.Field strengthA 3.0 T.SequenceT1‐ and T2‐weighted turbo spin echo, three‐dimensional time‐of‐flight (TOF) fast field echo and magnetization‐prepared rapid acquisition gradient echo sequences.AssessmentPlaque characteristics including lumen area (LA), wall area (WA), total vessel area (TVA), mean wall thickness (MWT), and mean normalized wall index (NWI); presence of calcification, lipid‐rich necrotic core (LRNC), intraplaque hemorrhage (IPH), and fibrous cap rupture (FCR); and percent composition area (%area) were evaluated and compared between men and women.Statistical testsIndependent‐sample t test, Mann–Whitney U test, chi‐square test, and multiple linear and logistic regressions.ResultsIn symptomatic arteries, men had significantly greater LA (46.2 ± 15.6 mm2 vs. 40.7 ± 12.9 mm2, P < 0.05), WA (33.9 ± 11.5 mm2 vs. 26.3 ± 7.5 mm2, P < 0.05), and TVA (80.1 ± 20.4 mm2 vs. 67.0 ± 18.0 mm2, P < 0.05); higher MWT (1.2 ± 0.4 mm vs. 1.0 ± 0.2 mm, P < 0.05); and higher prevalence of LRNC (72.3% vs. 46.0%, P < 0.05) and IPH (18.6% vs. 4.9%, P < 0.05) compared with women. In asymptomatic arteries, men had significantly greater LA (48.3 ± 16.9 mm2 vs. 42.1 ± 12.6 mm2, P < 0.05), WA (32.9 ± 11.0 mm2 vs. 25.8 ± 6.1 mm2, P < 0.05), and TVA (81.2 ± 22.1 mm2 vs. 67.9 ± 16.5 mm2, P < 0.05); higher MWT (1.2 ± 0.3 mm vs. 1.0 ± 0.2 mm, P < 0.05); higher prevalence of LRNC (67.8% vs. 42.9%, P < 0.05), IPH (14.9% vs. 1.2%, P < 0.05), and FCR (6.4% vs. 1.2%, P < 0.05); and higher %LRNC area (24.8 ± 17.2% vs. 17.8 ± 14.1%, P < 0.05) compared with women.Data conclusionMen have similar plaque burden but more vulnerable atherosclerotic plaques compared with women in both symptomatic and asymptomatic carotid arteries in a Chinese population.Evidence Level4Technical EfficacyStage 3

Abstract:Incidence of ischemic stroke differs between men and women, with substantially higher rates in men. The underlying mechanism of this difference remains poorly understood but may be because of differences in carotid atherosclerosis. Using an in-depth imaging-based approach, we investigated differences between carotid plaque composition and morphology in male and female patients with stroke, taking into account differences in total plaque burden. Additionally, we investigated all possible within-artery combinations of plaque characteristics to explore differences between various plaque phenotypes. We included 156 men and 68 women from the PARISK (Plaque At Risk) study, a prospective cohort study of patients with recent ischemic cerebrovascular symptoms and <70% ipsilateral carotid stenosis. Plaque characteristics (intraplaque hemorrhage [IPH], lipid-rich necrotic core [LRNC], calcifications, thin-or-ruptured fibrous cap, ulcerations, total plaque volume) were assessed with magnetic resonance imaging and multidetector-row computed tomography angiography. We used multivariable logistic and linear regression analyses to assess sex differences in plaque characteristics. We found significant difference in total plaque volume between men and women (β=22.9 mm 3 [95% CI, 15.4–30.5]; mean volume in men 1399±425 mm 3 , in women 1011±242 mm 3 ). Additionally, men were more likely to have IPH (odds ratio [OR]=2.8 [95% CI, 1.3–6.3]; IPH proportion in men 49%, in women 16%) and LRNC (OR=2.4 [95% CI, 1.2–4.7]; LRNC proportion in men 73%, in women 41%) even after adjustment for total plaque volume. We found no sex-specific differences in plaque volume-corrected volumes of IPH, LRNC, and calcifications. In terms of coexistence of plaque characteristics, we found that men had more often a plaque with coexistence of calcifications, LRNC, and IPH (OR=2.7 [95% CI, 1.2–7.0]), with coexistence of thin-or-ruptured fibrous cap/ulcerations, LRNC, and IPH (OR=2.4 [95% CI, 1.1–5.9]), and with coexistence of all plaque characteristics (OR=3.0 [95% CI, 1.2–8.6]). In symptomatic patients with mild-to-moderate carotid stenosis, men are more likely to have a high-risk carotid plaque with IPH and LRNC than women, regardless of total plaque burden. Abstract:To evaluate the gender differences of local vascular and plaque characteristics in patients with symptomatic middle cerebral artery (MCA) atherosclerotic stenosis by high-resolution magnetic resonance imaging (HR-MRI). Methods: We retrospectively collected patients admitted for an ischemic event in the MCA and grouped them according to gender; within each group, the symptomatic and the asymptomatic sides were designated based on the presence or absence of ischemic signs. The local vascular and plaque characteristics acquired from HR-MRI images were compared between the groups and within the symptomatic and asymptomatic divisions of each group. Results: A total of 71 patients were eligible for inclusion and exclusion, including 46 males (64.7% ). There was a statistically significant difference in minimum lumen area between male and female patients (P=0.039), and there was a significant correlation between gender and symptomatic sides (P=0.026). There was no significant difference in the stenosis rate between male and female patients (P=0.056), but the correlation between gender and symptomatic sides of this indicator was significant (P=0.011). The male plaque load was greater (P=0.006), and the correlation between the affected sides was significant (P=0.011). Plaque enhancement was more common in males than in females (P=0.026). Multiple regression analysis suggested that the main factors affecting minimum lumen area and plaque burden were hypertension and diabetes, and gender mainly affected plaque enhancement (OR 6.75, 95% CI 1.60-28.49, P=0.009). Conclusion: There are gender differences in local vascular and plaque characteristics in patients with symptomatic MCA atherosclerosis. In male patients, the plaque volume is larger and local vascular lumen area is smaller, and vulnerable plaques are more likely to occur. Gender has no significant effect on the degree of local vascular stenosis, and ischemic events are more likely to be caused by altering the stability of local plaque.

Abstract:It remains unclear whether direct vessel wall imaging can identify carotid high-risk lesions in symptomatic subjects and whether carotid plaque characteristics are more effective indicators for cerebral infarct severity than stenosis. This study sought to determine the associations of carotid plaque characteristics by MR imaging with stenosis and acute cerebral infarct (ACI) sizes on diffusion weighted imaging (DWI).One hundred and fourteen symptomatic patients underwent carotid and brain MRI. ACI volume was determined from symptomatic internal carotid artery territory on DWI images. Ipsilateral carotid plaque morphological and compositional characteristics, and stenosis were also determined. The relationships between carotid plaque characteristics, stenosis and ACIs size were then evaluated.In carotid arteries with 30-49% stenosis, 86.7% and 26.7% were found to have lipid-rich necrotic core (LRNC) and intraplaque hemorrhage, respectively. Furthermore, 45.8% of carotid arteries with 0-29% stenosis developed LRNCs. Carotid morphological measurements, such as % wall volume, and the LRNC size were significantly associated with ipsilateral ACIs volume before and after adjustment for significant demographic factors (age and LDL) or stenosis in patients with carotid plaque (all p<0.05).A substantial number of high-risk plaques characterized by vessel wall imaging exist in carotid arteries with lower grade stenosis. In addition, carotid plaque characteristics, particularly the % wall volume and LRNC size, are independently associated with cerebral infarction as measured by DWI lesions. Our findings indicate that characterizing atherosclerotic plaque by MR vessel wall imaging might be useful for stratification of plaque risk and infarction severity.

Abstract:INTRODUCTION: Control of risk factors that are associated with carotid high-risk plaques will play important role in plaque stabilization and prevention of ischemic events. However, the relationship between traditional risk factors and carotid high-risk plaques is unclear. This study sought to determine the risk factors that are associated with carotid high-risk plaques. Methods: Eighty-one patients (mean age 61 years, 60 males) with recent ischemic stroke and carotid plaque on ultrasound underwent MR imaging using multicontrast sequences (T1W, T2W, 3D TOF, and MP-RAGE). The clinical risk factors, such as age, gender, hypertension, diabetes, current smoking, blood pressure, pulse pressure (PP), BMI, and cholesterol levels were collected. The presence/absence and the maximum percentage of carotid lipid-rich necrotic core (LRNC) and intaplaque hemorrhage (IPH) to wall area for each artery were determined. Lesions with either IPH or large LRNC (% LRNC ≥20%) were defined as high-risk. The patients were divided into three groups according to the symmetrical distribution of carotid LRNC or IPH at none, unilateral, and bilateral carotid arteries. The risk factors in three different groups were compared. Results: Of 81 patients, 71.6%, 18.5%, and 9.9% had none, unilateral, and bilateral IPH at carotid arteries, respectively. We also found that 54.3%, 27.2%, and 18.5% of patients had none, unilateral, and bilateral large carotid LRNC, respectively. There was significant difference in BMI and PP among three groups of patients with none, unilateral, and bilateral carotid IPH or large LRNC (all P<0.05) (Fig. 1A-B). Conclusions: BMI and pulse pressure are associated with symmetrical disrtibution of carotid high-risk plaque indicating that control of these risk factors might be helpful for plaque stabilization.
Associations Between Carotid Plaques and White Matter Hyperintensities in Cerebral Small Vessel Disease

Abstract:Background: Cerebral small vessel disease (CSVD) can lead to stroke and cognitive impairment. Small vessels cannot be visualized by neuroimaging directly, and CSVD can only be evaluated by cerebral parenchymal changes in MRI. Therefore, a convenient screening method for identifying high-risk and susceptible patients is needed. Recently, some studies found that CSVD was related to large atherosclerosis, and atherosclerosis was an essential pathological feature of CSVD. Therefore, we aimed to investigate the association between carotid plaque size characteristics and white matter hyperintensities (WMHs) in patients with CSVD. Methods: We continuously enrolled patients with CSVD. Carotid plaque features were evaluated using carotid ultrasound, and WMHs were evaluated using brain magnetic resonance imaging. Plaque characteristics were compared between patients with no/mild WMHs and those with severe WMHs. Associations between the plaque characteristics and WMH severity were analyzed using logistic regression. Results: In total, 180 patients were recruited, of whom 92 had severe WMHs. The severe WMHs group had a higher sum of the bilateral maximum intima-media thickness (4.15 mm vs. 3.30 mm), longer maximum plaque length (17.20 mm vs. 13.90 mm), thicker plaques (2.70 mm vs. 2.30 mm), and more plaques (3 vs. 2) than the no/mild WMHs group. Adjusted logistic regression analyses revealed that maximum plaque length and thickness were associated with WMHs. Conclusions: Carotid atherosclerotic plaque features, such as plaque length and thickness, were associated with the severity of WMHs, which suggested that carotid atherosclerotic plaque characteristics measured using ultrasound might be helpful indicators for identifying high-risk patients with CSVD.

Abstract:Background The rupture of carotid atherosclerotic plaque is one of the major causes of ischemic stroke, and largely determined by the composition of the plaque. To date, most studies on carotid plaque composition focused on individual plaque components, whilst the interplay between different components within the same plaque may be a critical driving force for plaque evolution and rupture. Objectives To determine the interplay between carotid plaque components over time and describe the dynamics of plaque composition with a specific focus on age- and sex-specific changes in a population-based setting. Methods From a population-based cohort in Netherlands, 802 participants (mean age ± SD 68 ± 8 years, 43% women) with subclinical carotid atherosclerosis underwent repeat magnetic resonance imaging (MRI) of carotid plaque (first scan between 2007 and 2012, second scan between 2014 to 2017). We evaluated carotid plaque composition, including calcification, lipid-rich necrotic core (LRNC) and intraplaque hemorrhage (IPH). To assess the relationship between the incidence of one specific component and baseline plaque composition, plaques with this specific component present at the baseline were excluded. Generalized estimation equation models were used with adjustments for baseline plaque size and cardiovascular risk factors. We demonstrated the evolution of plaque composition along age and by sex. Results Baseline presence of calcification in carotid plaque is independently associated with a higher incidence of IPH (adjusted odds ratio: 2.25; 95% confidence interval: 1.40 to 3.63). No associations were found between baseline plaque composition and the new onset of calcification or LRNC. The proportion of complex plaques with multiple components significantly increased at follow-up (Figure 1a). The plaque composition dramatically evolved over time and the proportion of plaques with two or more components started with 10% around 55 years and increased to over 50% after 70 years of age (Figure 1b). Although the age distribution was comparable between men and women, men were more likely to have and develop plaques with multiple vulnerable plaque components such as IPH (Figure 2). Conclusions A higher incidence of IPH was found in plaques with baseline calcification. The pre-existing plaque composition strongly determines how the plaque will evolve over time. Given the interplay between individual components, the overall plaque composition exhibits significant age- and sex- specific characteristics.

Circulating Sex-Specific Markers of Plaque Instability in Women and Men With Severe Carotid Atherosclerosis

BACKGROUND:Sex-specific differences in plaque composition and instability underscore the need to explore circulating markers for better prediction of high-risk plaques. This cross-sectional study aims to (1) investigate differences in lipid, immune, and adipokine circulating profiles between men and women with stable versus unstable plaques and (2) identify circulating markers that can better classify men and women according to plaque instability.METHODS:Preoperative blood samples and plaque specimens were collected from consecutive men and women with carotid artery stenosis ≥50% and who underwent a carotid endarterectomy between 2009 and 2018. Adipokine, lipid, and immune profiling was conducted. Plaque stability was determined by gold-standard histological classifications. Statistical analyses, including χ2, ANOVA, Kruskal–Wallis, and logistic regression, assessed differences in plaque features and blood parameters between men and women with stable and unstable plaques.RESULTS:Of 470 recruited patients (aged 70.8±9.2 years), the final study analyses included 317 men and 143 women (aged 71.0±9.0 years). Men exhibited more unstable plaques (P<0.001), characterized by increased plaque hemorrhage, larger lipid core, and inflammation (P<0.001), along with less favorable circulating profiles. Antagonistic interactions between sex and white blood cell (WBC) counts, basophil-to-WBC ratio, and platelet counts influenced plaque instability. In men, low WBC counts, high monocyte-to-WBC ratio, low basophil-to-WBC ratio, and high LDL-C (low-density lipoprotein cholesterol) levels were associated with greater plaque instability (odds ratio, 0.827 [95% CI, 0.713–0.926], 1.158 [95% CI, 1.027–1.305], 0.495 [95% CI, 0.281–0.871], and 1.564 [95% CI, 1.001–2.443], respectively) and more unstable features (ie, inflammation, foam cells, and neovascularization). In women, a high basophil-to-WBC ratio was associated with greater plaque instability (3.142 [95% CI, 1.220–8.093]), hemorrhage, and thrombosis, while a high molecular weight-to-total adiponectin ratio was associated with decreased instability (0.014 [95% CI, 0.000–0.646]) and inflammation.CONCLUSIONS:Our findings demonstrated sex-specific differences, with women displaying more stable plaque phenotypes and favorable circulating profiles compared with men. This proof-of-concept study was also designed as the key first step in exploring novel sex-specific associations between circulating lipid, immune, and adipokine profiles and carotid plaque instability.

Abstract:The purpose of this study was to evaluate the differences in carotid arterial morphology and plaque composition between patients with acute coronary syndrome (ACS) and patients with stable coronary artery disease (SCAD). Twenty-eight patients (12 ACS patients and 16 SCAD patients) underwent carotid high-resolution MRI examination using a 3.0-Tesla (3.0T) MRI scanner. The indicators of carotid arterial morphology included the maximum total vessel area (Max-TVA), mean TVA, minimum lumen area (Min-LA), mean LA, maximum wall area (Max-WA), mean WA, maximum wall thickness (Max-WT), mean WT, maximum normalized wall index (Max-NWI), mean NWI, and maximum stenosis (Max-stenosis). The indicators of plaque composition included the prevalence and mean area percentage (%) of lipid-rich necrotic core (LRNC), calcification (Ca), intraplaque hemorrhage (IPH), and fibrous cap rupture (FCR). None of the indicators of carotid arterial morphology had significant differences (all P > 0.05) between the ACS and SCAD patients. The prevalence and plaque composition area percentage of LRNC, Ca, and IPH did not exhibit significant differences between the two groups. However, carotid plaques in the ACS patients presented a higher prevalence of FCR than SCAD patients (P < 0.05). This study revealed a similar carotid arterial morphology between ACS and SCAD patients. However, FCR is more common in carotid plaques with ACS than in those with SCAD. Ruptured carotid plaques may be a forewarning factor for those patients who are at high risk of ACS.
Discriminating Carotid Atherosclerotic Lesion Severity By Luminal Stenosis And Plaque Burden A Comparison Utilizing High-Resolution Magnetic Resonance Imaging At 3.0 Tesla

Abstract:Background and Purpose-To determine associations between stenosis, measures of plaque burden, and compositional features of carotid atherosclerosis, including high-risk features of intraplaque hemorrhage (IPH) and surface disruption.Methods-Institutional Review Board approval and informed consent for all participants were obtained before study initiation. Patients with either carotid stenosis >50% by duplex ultrasound or suspected coronary artery disease underwent multi-contrast carotid MRI at 3.0 T. For each artery, stenosis, percent wall volume (PWV=100% x wall volume/total vessel volume), and mean wall thickness (MWT) were measured. Presence or absence of a lipid-rich necrotic core, calcification, IPH, and surface disruption were recorded.Results-One hundred eighty-one patients were included in the final analysis. The area under the curve (AUC) calculated from receiver-operating-characteristics analysis found the presence of IPH was similarly classified by stenosis (AUC=0.82), PWV (AUC=0.88), and MWT (AUC=0.88). Notably, IPH was present in the lowest category of each parameter. Prevalence of IPH in arteries with 0% stenosis was 4.4%. In arteries with PWV <40%, prevalence was 3.2%; in arteries with MWT <1.0 mm, prevalence was 2.3%. Strength of classification for surface disruption was similarly classified by stenosis (AUC=0.87), PWV (AUC=0.93), and MWT (AUC=0.94).Conclusions-Measures of plaque burden do not substantially improve disease assessment compared to stenosis. The finding of IPH in all categories of stenosis and plaque burden suggests that direct characterization of plaque composition and surface status is necessary to fully discriminate disease severity. Abstract:It is unknown whether gender influences the atherosclerotic plaque characteristics (APCs) of lesions of varying angiographic stenosis severity. This study evaluated the imaging data of 303 symptomatic patients from the derivation arm of the CREDENCE (Computed TomogRaphic Evaluation of Atherosclerotic Determinants of Myocardial IsChEmia) trial, all of whom underwent coronary computed tomographic angiography and clinically indicated nonemergent invasive coronary angiography upon study enrollment. Index tests were interpreted by 2 blinded core laboratories, one of which performed quantitative coronary computed tomographic angiography using an artificial intelligence application to characterize and quantify APCs, including percent atheroma volume (PAV), low-density noncalcified plaque (LD-NCP), noncalcified plaque (NCP), calcified plaque (CP), lesion length, positive arterial remodeling, and high-risk plaque (a combination of LD-NCP and positive remodeling ≥1.10); the other classified lesions as obstructive (≥50% diameter stenosis) or nonobstructive (<50% diameter stenosis) based on quantitative invasive coronary angiography. The relation between APCs and angiographic stenosis was further examined by gender. The mean age of the study cohort was 64.4 ± 10.2 years (29.0% female). In patients with obstructive disease, men had more LD-NCP PAV (0.5 ± 0.4 vs 0.3 ± 0.8, p = 0.03) and women had more CP PAV (11.7 ± 1.6 vs 8.0 ± 0.8, p = 0.04). Obstructive lesions had more NCP PAV compared with their nonobstructive lesions in both genders, however, obstructive lesions in women also demonstrated greater LD-NCP PAV (0.4 ± 0.5 vs 1.0 ± 1.8, p = 0.03), and CP PAV (17.4 ± 16.5 vs 25.9 ± 18.7, p = 0.03) than nonobstructive lesions. Comparing the composition of obstructive lesions by gender, women had more CP PAV (26.3 ± 3.4 vs 15.8 ± 1.5, p = 0.005) whereas men had more NCP PAV (33.0 ± 1.6 vs 26.7 ± 2.5, p = 0.04). Men had more LD-NCP PAV in nonobstructive lesions compared with women (1.2 ± 0.2 vs 0.6 ± 0.2, p = 0.02). In conclusion, there are gender-specific differences in plaque composition based on stenosis severity.

Abstract:Introduction: Large, prospective, randomized trials have identified an improvement in outcome by offering surgery to neurologically symptomatic patients with severe carotid stenosis [1]. In symptomatic patients with moderate carotid stenosis (<70%) or in asymptomatic patients with carotid disease, the optimal treatment strategy remains ambiguous. Recently, multi-contrast, in vivo MR vessel wall imaging has found a correspondence between carotid plaque characteristics and both previous and future ipsilateral thromboembolic events [2-4]. With the advent of 3.0 T MR imaging, highresolution in vivo carotid plaque studies with improved signal-to-noise ratio [5,6] have been enabled as well as very high-resolution (0.28 mm ) contrastenhanced carotid MR angiography (CE MRA) [7]. Purpose: We sought to identify features detected on carotid MRI and CE MRA associated with recent cerebrovascular symptoms in a population of subjects with known carotid atherosclerotic disease. Beyond evaluating the group as a whole, we utilized the CE MRA data to isolate the subgroups of individuals with <70% and 70-99% stenosis to identify differences in imaging results between symptomatic and asymptomatic groups that may discriminate between stable and unstable lesions within each particular cohort. Methods: Ninety-seven consecutive patients with carotid artery stenosis between 50%-99% in at least one artery identified with non-invasive imaging such as duplex ultrasound or CT angiography underwent carotid MRI at 3.0 T after filling-out a healthcare questionnaire and having their height and weight measured. A standardized protocol optimized for 3.0 T imaging [5,6] was utilized to obtain 3D time-of-flight (TOF), 2D T2-weighted FSE [8], and 2D T1-weighted preand post-contrast quadruple inversion-recovery [9] FSE sequences. After bolus gadolinium contrast intravenous injection and prior to the post-contrast T1-weighted acquisition, a coronal elliptical-centric carotid CE MRA with 0.28 mm resolution was performed [7]. Total scan time for all sequences was < 45 minutes. Two experienced reviewers, blinded to clinical outcome, interpreted all images by reaching a consensus opinion. Custom designed software (CASCADE [10], Seattle, WA, USA) was used to draw the lumen and outer wall boundaries of the carotid artery at each axial location. In addition, calcification, necrotic core, and type I / type II hemorrhage were identified as present or absent using previously published criteria that have been validated with histology [11]. When a component was present, the proportion of the arterial wall occupied by that component was recorded (e.g. percent calcification = calcification area / wall area). When a necrotic core was present, fibrous caps status was categorized as either “thick” versus “thin or ruptured” using previously published, histologically validated criteria [12]. In addition, the presence of smooth, irregular, or ulcerated lumen on CE MRA was determined based upon previously published standards. Finally, the NASCET percent carotid stenosis was measured from the high resolution carotid CE MRA. Univariate logistic regression analysis was used to identify associations between clinical data and metrics identified with carotid MRI/MRA and recent cerebrovascular symptoms for both the group as a whole and for the subgroup with < 70% stenosis. Meaningful multivariate analysis could not be performed because of the small number of cases with recent symptoms. Results from logistic regression analysis are presented as odds ratios (OR) with 95% confidence interval (CI). Results: Among the 97 patients (97 arteries) evaluated, 7 (7.2%; 100% asymptomatic) arteries were excluded due to poor image quality. Of the remaining 90 subjects (male gender, 58%; mean±SD age, 71±8.7 years), 13 (14.4%) had a recent transient ischemic attack or stroke. Presence of a thin/ruptured fibrous cap (OR, 4.22; 95%CI, 1.08-16.53; p=0.037), type I hemorrhage (OR, 5.74; 95%CI, 1.60-20.59; p=0.007), and maximum % area of type I hemorrhage (OR for 10% increase, 2.78; 95%CI, 1.45-5.34; p=0.002) were identified as significant predictors of a neurological event. All the other variables, including MRA stenosis and clinical data, were not significantly associated with recent symptoms. Of the 49 patients with < 70% carotid stenosis by MRA, 6 (12.2%) were symptomatic. Results of metrics identified with a significant association during univariate analysis are presented in Table 1. In this subgroup, we did not identify a significant association between symptom status and MRA features and/or clinical data. Abstract:Background: Magnetic resonance imaging (MRI) has the potential in assessing the inflammation of perivascular adipose tissue (PVAT) due to its excellent soft tissue contrast. However, evidence is lacking for the association between carotid PVAT measured by MRI and carotid vulnerable atherosclerotic plaques. This study aimed to investigate the association between signal intensity of PVAT and vulnerable plaques in carotid arteries using multi-contrast magnetic resonance (MR) vessel wall imaging.Methods: In this cross-sectional study, a total of 104 patients (mean age, 64.9 +/- 7.0 years; 86 men) with unilateral moderate-to-severe atherosclerotic stenosis referred to carotid endarterectomy (CEA) were recruited from April 2018 to December 2020 at Department of Neurosurgery of Peking University Third Hospital. All patients underwent multi-contrast MR vessel wall imaging including time-of-flight (ToF) MR angiography, black-blood T1-weighted (T1w) and T2-weighted (T2w) and simultaneous non-contrast angiography and intraplaque hemorrhage (IPH) imaging sequences. Patients with contraindications to endarterectomy or MRI examinations were excluded. The signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) of PVAT were measured on ToF images and vulnerable plaque characteristics including IPH, large lipid-rich necrotic core (LRNC), and fibrous cap rupture (FCR) were identified. The SNR and CNR of PVAT were compared between slices with and without vulnerable plaque features using Mann-Whitney U test and their associations were analyzed using the generalized linear mixed model (GLMM).Results: Carotid artery slices with IPH (30.93 +/- 14.56 vs. 27.34 +/- 10.02; P<0.001), FCR (30.35 +/- 13.82 vs. 27.53 +/- 10.37; P=0.006), and vulnerable plaque (29.15 +/- 12.52 vs. 27.32 +/- 10.05; P=0.016) had significantly higher value of SNR of PVAT compared to those without. After adjusting for clinical confounders, the SNR of PVAT was significantly associated with presence of IPH [odds ratio (OR) =0.627, 95% confidence interval (CI): 0.465-0.847, P-uncorr=0.002, PFDR=0.016] and vulnerable plaque (OR =0.762, 95% CI: 0.629-0.924, P-uncorr=0.006, P-FDR=0.020). However, no significant association was found between the CNR of PVAT and presence of vulnerable plaque features (all P>0.05).Conclusions: The SNR of carotid artery PVAT measured by ToF MR angiography is independently associated with vulnerable atherosclerotic plaque features, suggesting that the signal intensity of PVAT might be an effective indicator for vulnerable plaque.
Predictors of carotid atherosclerotic plaque progression as measured by noninvasive magnetic resonance imaging

Abstract:The purpose of this in vivo MRI study was to quantify changes in atherosclerotic plaque morphology prospectively and to identify factors that may alter the rate of progression in plaque burden. Sixty-eight asymptomatic subjects with >or=50% stenosis, underwent serial carotid MRI examinations over an 18-month period. Clinical risk factors for atherosclerosis, and medications were documented prospectively. The wall and total vessel areas, matched across time-points, were measured from cross-sectional images. The normalized wall index (NWI=wall area/total vessel area), as a marker of disease severity, was documented at baseline and at 18 months. Multiple regression analysis was used to correlate risk factors and morphological features of the plaque with the rate of progression/regression. On average, the wall area increased by 2.2% per year (P=0.001). Multiple regression analysis demonstrated that statin therapy (P=0.01) and a normalized wall index >0.64 (P=0.001) were associated with a significantly reduced rate of progression in mean wall area. All other documented risk factors were not significantly associated with changes in wall area. Findings from this study suggest that increased normalized wall index and the use of statin therapy are associated with reduced rates of plaque progression amongst individuals with advanced, asymptomatic carotid atherosclerosis.

Characteristics of Symptomatic Intra- and Extracranial Atherosclerotic Plaques in Anterior Circulation Based on Multi-contrast Magnetic Resonance Vascular Wall Imaging

Abstract:Objective To compare the characteristics of intra- and extracranial plaques using 3D multi-contrast magnetic resonance vessel wall imaging in patients with symptomatic intra- and extracranial atherosclerotic diseases. Methods The data of patients with symptomatic intra-and extracranial carotid artery culprit plaques identified by MR vessel wall imaging were retrospectively analyzed. The distribution, burden and components of plaques were analyzed. The plaque burden including the maximum wall thickness (Max WT), plaque length and vessel stenosis were measured. Plaque components such as lipid-rich necrotic core (LRNC), intraplaque hemorrhage (IPH) and calcification on MRI were evaluated. The imaging characteristics of plaques between intracranial and extracranial arteries were compared. Results In total, 45 patients were recruited and 203 atherosclerotic plaques were detected, including 156 extracranial plaques (76.8%) and 47 intracranial plaques (23.2%). Max WT [(3.2±1.0) mm vs (1.9±0.4) mm, P<0.001] and plaque length [10.7 (7.5-13.7) mm vs 4.3 (2.6-6.3) mm, P<0.001] of extracranial plaques were significantly larger than that of intracranial plaques. Stenosis degree [28.0 (20.0-38.5)% vs 18.8 (10.8-30.5)%, P<0.001] of intracranial plaques were significantly larger than that of extracranial plaques. Compared with intracranial plaques, extracranial plaques showed more higher rate of LRNC (66.0% vs 21.3%, P<0.001) and lower rate of IPH (13.5% vs 27.7%, P=0.027). Conclusions For patients with atherosclerotic culprit plaques and symptomatic intracranial and extracranial artery diseases, extracranial plaques commonly have greater Max WT, higher rate of LRNC, less severe stenosis and lower rate of IPH than intracranial plaques.

Abstract:We compared plaque characteristics between symptomatic and asymptomatic sides in patients with bilateral carotid vulnerable plaques using magnetic resonance imaging (MRI). Participants (n = 67; mean age: 65.8 ± 7.7 years, 61 males) with bilateral carotid vulnerable plaques were included. Vulnerable plaques were characterized by intraplaque hemorrhage (IPH), large lipid-rich necrotic core (LRNC), or fibrous cap rupture (FCR) on MRI. Symptomatic vulnerable plaques showed greater plaque burden, LRNC volume (median: 221.4 vs 134.8 mm 3 , P = .003), IPH volume (median: 32.2 vs 22.5 mm 3 , P = .030), maximum percentage (Max%) LRNC (median: 51.3% vs 41.8%, P = .002), Max%IPH (median: 13.4% vs 9.5%, P = .022), cumulative slices of LRNC (median: 10 vs 8, P = .005), and more juxtaluminal IPH and/or thrombus (29.9% vs 6.0%, P = .001) and FCR (37.3% vs 16.4%, P = .007) than asymptomatic ones. After adjusting for plaque burden, differences in juxtaluminal IPH and/or thrombus (odds ratio [OR]: 5.49, 95% CI: 1.61-18.75, P = .007) and FCR (OR: 2.90, 95% CI: 1.16-7.24, P = .022) between bilateral sides remained statistically significant. For patients with bilateral carotid vulnerable plaques, symptomatic plaques had greater burden, more juxtaluminal IPH and/or thrombus, and FCR compared with asymptomatic ones. The differences in juxtaluminal IPH and/or thrombus and FCR between bilateral sides were independent of plaque burden.

Abstract:Background and purposeTo investigate differences in the characteristics of carotid atherosclerotic plaques of symptomatic subjects in northern and southern China using MRI.MethodsSixty-three subjects in northern China (mean age: 59.1±8.6 years, 45 men) and 56 subjects in southern China (mean age: 60.4±8.6 years, 38 men) were included. All subjects underwent carotid artery multicontrast vessel wall MRI. Plaque morphology, calciﬁcation, lipid-rich necrotic core, intraplaque haemorrhage, luminal surface disruption and high-risk plaque were measured and identified. All plaque characteristics were compared between subjects in northern and southern China using Mann-Whitney U test or χ2 test.ResultsCompared with subjects in southern China, those in northern China had significantly greater areas for lumen (57.7±14.9 mm2 vs 50.4±18.3 mm2, p=0.009), wall (38.4±13.1 mm2 vs 31.9±11.7 mm2, p<0.001) and total vessel (96.1±20.2 mm2 vs 82.4±22.7 mm2, p=0.001) and mean wall thickness (1.25±0.43 mm vs 1.13±0.40 mm, p=0.019). χ2 analysis showed that subjects in northern China tended to have a higher prevalence of intraplaque haemorrhage (14.3% vs 5.4%, p=0.106) and high-risk plaque (20.6% vs 10.7%, p=0.140) than those in southern China, although these differences were not statistically significant (all p>0.05).ConclusionSubjects in northern China have significantly larger vessel size and may have a higher prevalence of vulnerable plaques than those in southern China. Our findings provide additional perspective to optimise the management of cerebrovascular disease in individuals in different regions in China.Trial registration numberNCT02017756

Abstract:Objectives: To characterize adventitial vasa vasorum (VV) in unilaterally symptomatic patients and evaluate its association with intraplaque hemorrhage (IPH) and symptom status. Background: Adventitial VV is thought to be a primary source of IPH and a potential marker for plaque instability. Dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) has been shown capable of quantifying adventitial VV in the carotid artery. Methods: Patients with ischemic cerebrovascular symptoms within the past 6 months and ipsilateral carotid plaques underwent MR imaging examination, which included a multi-contrast protocol for detecting IPH and a DCE-MRI protocol for measuring adventitial VV. Both the symptomatic and contralateral asymptomatic arteries were studied. Adventitial VV was measured at consecutive slices as adventitial Ktrans from kinetic modeling. Maximum adventitial Ktrans across all slices were calculated for each artery. Results: From the 27 patients (22 men; 69±10 years) recruited, 50 arteries had adventitial Ktrans measurements. IPH arteries (0.142±0.042 vs. 0.112±0.029 min-1, p<0.001) and symptomatic arteries (0.134±0.038 vs. 0.107±0.027 min-1, p=0.001) were shown to have higher adventitial Ktrans compared to their counterparts. In multivariate analysis, IPH, symptom status and male sex were independently associated with adventitial Ktrans. Conclusions: Adventitial VV demonstrated significant associations with IPH and symptom status. In vivo assessment of adventitial VV by DCE-MRI may be useful in studying plaque pathogenesis or risk stratification, but further examination through prospective studies is needed.
