Abstract:BackgroundDespite being considered the standard surgical procedure for symptomatic cervical disc disease, anterior cervical decompression and fusion invariably accelerates adjacent segment degeneration. Cervical total disc replacement is a motion-preserving procedure developed as a substitute to fusion. Whether cervical total disc replacement is superior to fusion remains unclear.MethodsWe comprehensively searched PubMed, EMBASE, Medline, and the Cochrane Library in accordance with the inclusion criteria to identify possible studies. The retrieved results were last updated on December 12, 2014. We classified the studies as short-term and midterm follow-up.ResultsNineteen randomized controlled trials involving 4516 cases were identified. Compared with anterior cervical decompression and fusion, cervical total disc replacement had better functional outcomes (neck disability index [NDI], NDI success, neurological success, neck pain scores reported on a numerical rating scale [NRS], visual analog scales scores and overall success), greater segmental motion at the index level, fewer adverse events and fewer secondary surgical procedures at the index and adjacent levels in short-term follow-up (P < 0.05). With midterm follow-up, the cervical total disc replacement group indicated superiority in the NDI, neurological success, pain assessment (NRS), and secondary surgical procedures at the index level (P < 0.05). The Short Form 36 (SF-36) and segmental motion at the adjacent level in the short-term follow-up showed no significant difference between the two procedures, as did the secondary surgical procedure rates at the adjacent level with midterm follow-up (P > 0.05).ConclusionsCervical total disc replacement presented favorable functional outcomes, fewer adverse events, and fewer secondary surgical procedures. The efficacy and safety of cervical total disc replacement are superior to those of fusion. Longer-term, multicenter studies are required for a better evaluation of the long-term efficacy and safety of the two procedures.

Abstract:Introduction: The treatment of skip-level cervical degenerative disease (CDD) with no degenerative changes observed in the intervening segment (IS) is complicated. This research aims to provide a reference basis for selecting treatment approaches for noncontiguous CDD. Methods: To establish accurate finite element models (FEMs), this study included computed tomography (CT) data from 21 patients with CDD (10 males and 11 females) for modeling. The study primarily discusses four cross-segment surgical approaches: upper (C3/4) anterior cervical discectomy and fusion (ACDF) and lower (C5/6) cervical disc arthroplasty (CDA), FA model; upper CDA (C3/4) and lower ACDF (C5/6), AF model; upper ACDF (C3/4) and lower ACDF (C5/6), FF model; upper CDA (C3/4) and lower CDA (C5/6), AA model. An initial axial load of 73.6 N was applied at the motion center using the follower load technique. A moment of 1.0 Nm was applied at the center of the C2 vertebra to simulate the overall motion of the model. The statistical analysis was conducted using STATA version 14.0. Statistical significance was defined as a p value less than 0.05. Results: The AA group had significantly greater ROM in flexion and axial rotation in other segments compared to the FA group ( p < 0.05). The FA group consistently exhibited higher average intervertebral disc pressure in C2/3 during all motions compared to the AF group ( p < 0.001); however, the FA group displayed lower average intervertebral disc pressure in C6/7 during all motions ( p < 0.05). The AA group had lower facet joint contact stresses during extension in all segments compared to the AF group ( p < 0.05). The FA group exhibited significantly higher facet joint contact stresses during extension in C2/3 ( p < 0.001) and C6/7 ( p < 0.001) compared to the AF group. Discussion: The use of skip-level CDA is recommended for the treatment of non-contiguous CDD. The FA construct shows superior biomechanical performance compared to the AF construct.

[Effect of inhibition of cytosolic phospholipase A₂ on Leptin release from human umbilical vein endothelial cells induced by lipopolysaccharide].

Abstract:OBJECTIVE To determine Leptin levels in supernatant fluid of culture of human umbilical vein endothelial cells (ECV-304) after being challenged by lipopolysaccharide (LPS) and calcium ion vector A23187, and to explore the possible relation between Leptin release and cytosolic phospholipase A(2) (cPLA(2)) activity in an inflammatory cell model. METHODS ECV-304 cells were cultured in vitro. Experiment 1: the cells were divided into seven groups: blank control group, LPS 5, 10, 20 μg/ml stimulation groups, A23187 0.1, 1.0, 10.0 μmol/L stimulation groups. The supernatants were collected at 6, 12 and 24 hours.Experiment 2: according to the results of experiment 1, the cells were divided into eight groups: blank control group, LPS 20 μg/ml stimulation group, the inhibitor of cPLA2 AACOCF3 0.1, 1.0, 10.0 μmol/L plus LPS stimulation groups, the inhibitor of mitogen-activated protein/extracellular signal-regulated protein kinase kinase 1/2 (MEK1/2) U0126 0.1, 1.0, 5.0 μmol/L plus LPS stimulation groups, with AACOCF3 or U0126 added 1 hour before the addition of LPS, and the supernatants were collected 24 hours after the addition of LPS. Leptin level was determined by radioimmunoassay. RESULTS Experiment 1: with increase in LPS concentration and prolongation of time, Leptin release was decreased gradually. After 24 hours of interaction the concentration of Leptin (ng/ml) in LPS 20 μg/ml group was decreased significantly compared with the blank control group (0.540±0.109 vs. 0.823±0.048,P<0.05). However, A23187 had no significant effect on Leptin release. Experiment 2: LPS rendered cells to release less Leptin (ng/ml: 0.558±0.069 vs. 0.825±0.067,P<0.05); by adding AACOCF3 or U0126 in different concentration before adding LPS rendered the cells to release more Leptin (ng/ml), and it showed concentration-dependent (the AACOCF3 0.1, 1.0, 10.0 μmol/L groups were 0.673±0.135, 0.723±0.055, 0.797±0.062, respectively; the U0126 0.1, 1.0, 5.0 μmol/L groups were 0.698±0.112, 0.862±0.184, 0.935±0.145, respectively). The release of Leptin in AACOCF3 1.0 μmol/L, 10.0 μmol/L and U0126 1.0 μmol/L, 5.0 μmol/L groups was significantly higher than LPS 20 μg/ml stimulation group (all P<0.05). CONCLUSION There is a possible relation between Leptin release and cPLA 2 activity in inflammatory cells induced by LPS.

Abstract:Our finite element analysis research revealed that anterior controllable antedisplacement and fusion (ACAF) can preserve a similar spinal stability to anterior cervical discectomy and fusion (ACDF), which is far more satisfying than anterior cervical corpectomy decompression and fusion (ACCF). ACAF also showed a lower risk of internal fixation failure and cage subsidence than ACCF, while higher rates of subsidence, screw loosening and internal fixation failure might still be detected when compared with ACDF, especially in axial rotation. Purpose Multilevel cervical spondylotic myelopathy poses significant challenges in selecting optimal surgical approaches, warranting a comprehensive understanding of their biomechanical impacts. Given the lack of consensus regarding the most effective technique, this study aims to fill this critical knowledge gap by rigorously assessing and comparing the biomechanical properties of three distinct surgical interventions, including anterior controllable antedisplacement and fusion (ACAF), anterior cervical corpectomy decompression and fusion (ACCF), and anterior cervical discectomy and fusion (ACDF). The study offers pivotal insights to enhance treatment precision and patient outcomes. Methods The construction of the cervical spine model involved a detailed process using CT data, specialized software (Mimics, Geomagic Studio, and Hypermesh) and material properties obtained from prior studies. Surgical instruments were modeled (titanium mesh, anterior cervical plate, interbody cage, and self‐tapping screws) to simulate three surgical approaches: ACAF, ACCF, and ACDF, each with specific procedures replicating clinical protocols. A 75‐N follower load with 2 Nm was applied to simulate biomechanical effects. Results The range of motion decreased more after surgery for ACAF and ACDF than for ACCF, especially in flexion and lateral bending. ACCF have higher stress peaks in the fixation system than those of ACAF and ACDF, especially in flexion. The maximum von Mises stresses of the bone–screw interfaces at C3 of ACCF were higher than those of ACAF and ACDF. The maximum von Mises stresses of the bone–screw interfaces at C6 of ACDF were much higher than those of ACAF and ACCF. The maximum von Mises stresses of the grafts of ACCF and ACAF were much higher than those of ACDF. The maximum von Mises stresses of the endplate of ACCF were much higher than those of ACAF and ACDF. Conclusion The ACAF and ACDF models demonstrated superior cervical reconstruction stability over the ACCF model. ACAF exhibited lower risks of internal fixation failure and cage subsidence compared to ACCF, making it a promising approach. However, while ACAF revealed improved stability over ACCF, higher rates of subsidence and internal fixation failure persisted compared to ACDF, suggesting the need for further exploration of ACAF's long‐term efficacy and potential improvements in clinical outcomes.

Abstract:Background: Long-term follow-up of the patients undergoing anterior cervical discectomy and fusion (ACDF) shows some incidence of degeneration at the segments adjacent to the fusion level. Cervical total disc replacement may resolve this problem. Objective: To compare the effects of cervical total disc replacement versus ACDF on the biomechanics of the segment adjacent to the operation level. Design, Time and Setting: The randomized controlled trial was performed at biomechanical laboratory in Beijng Trauma and Orthopaedic Institute from August 2007 to February 2008. Materials: Eighteen samples of fresh goat cervical spine; Bryan artificial cervical disk (Medtronic); CSLP-VA cervical plate (Synthes); coral bone (Beijing Yihuajian Science And Trade Co., Ltd.). Methods: The goat spines were divided into 3 groups: control group with no specific treatment; total disc replacement group, in which C 5/6 segment underwent Bryan artificial cervical disk replacement and fusion; internal fixation group, in which the C 5/6 segment underwent cervical discectomy, coral bone implantation and internal fixation with CSLP-VA cervical plate. Main Outcome Measures: The adjacent intervertebral disk pressure (IDP) was measured in flexion, extension, rotation and lateral bending under certain loading, and the pressure changes were compared among three groups. Results: At level C 4/5, the IDP between total disk replacement group and the control group had no statistical significances in flexion (P < 0.05), but that of internal fixation group was significantly higher than that of control group (P=0.035). At level C 6/7, there were no significant differences in IDP between total disk replacement group and control group in forward flexion (P > 0.05), but internal fixation group was significantly higher than control group (P=0.014). There were no significant differences among the three groups in IDP in extension, rotation and lateral bending. Conclusion: Compared with ACDF, the total disk replacement can reduce the adjacent segment IDP and may avoid the degeneration in adjacent cervical segment.
Biomechanical evaluation of adjacent segment degeneration after one- or two-level anterior cervical discectomy and fusion versus cervical disc arthroplasty: A finite element analysis

Abstract:Background and objective: To compare the biomechanical changes of adjacent segment degeneration (ASD) after one- or two-level anterior cervical discectomy and fusion (ACDF) versus cervical disc arthroplasty (CDA). Methods: A three-dimensional finite element (FE) model of intact C2-C7 segments was constructed and validated. In the one-level surgery model, the cage with plate implant or Prestige LP cervical disc prosthesis were integrated at C5-C6 segment into the FE model; while in the two-level surgery model, the prostheses were integrated at both C4-C5 and C5-C6 segments into the FE model. A pure moment of 1.0 Nm combined with a follower load of 73.6 N were imposed on C2 to investigate the flexion-extension, lateral bending, and axial rotation of different segments in the FE model. The segmental range of motion (ROM) and intradiscal pressure of the surgery models were investigated and compared with the intact model. Results: In the one-level model of ACDF, the ROM at C5-C6 was decreased, the ROM and intradiscal pressure at C4-C5 and C6-C7 segments were increased. In the two-level model of ACDF, the ROM at C4-C5 and C5-C6 were decreased, the ROM and intradiscal pressure at C3-C4 and C6-C7 were increased. However, in both one- and two-level models of CDA, the ROM of surgery segments were preserved, avoiding the increase of the ROM and intradiscal pressure at the adjacent segments. Conclusions: Abnormal ROM and intradiscal pressure at the adjacent segments may contribute to the higher risk of ASD after ACDF compared with CDA.

Abstract:Abstract Background To analyze and compare the biomechanical characteristics of the new combined cervical fusion device (NCCFD) and the traditional cage-plate construct (CPC) to ascertain its effectiveness in anterior cervical discectomy and fusion (ACDF) using finite element analysis. Methods A finite element model of the cervical spine, inclusive of the occipital bone was created and validated. In the ACDF model, either CPC or NCCFD was implanted at the C2–C3 segment of the model. A pure moment of 1.0 Nm combined with a follower load of 50 N was directed onto the superior surfaces of the occipital bone to determine flexion, extension, lateral bending (left and right), and axial rotation (left and right). The range of motion (ROM), stress distribution at the bone-implant interface, and facet joint forces were investigated and compared between CPC and NCCFD systems. Result The results showed that the ROMs of the fused levels in both models were nearly zero, and the motions of the unfused segments were similar. In addition, the maximum displacement exhibited nearly identical values for both models. The maximum stress of NCCFD screws in lateral bending and rotational conditions is significantly higher than that of the CPC, while the NCCFD model’s maximum stress remains within an acceptable range. Comparing the maximum fusion stress, it was found that the CPC experiences much lower fusion stress in anterior flexion and extension than the NCCFD, with no significant difference between the two in lateral bending and rotational states. Stress on the cage was mainly concentrated on both sides of the wings. Comparing the maximum IDP in the CPC and NCCFD, it was observed that maximum stresses rise in extension and lateral bending for both models. Lastly, stress distributions of the facet joints were generally similar across the two devices. Conclusion NCCFD not only provides the same level of biomechanical stability as CPC but also avoids postoperative complications associated with uneven force damage to the implant. The device offers a novel surgical alternative for ACDF in C2–C3 level.

Motion Analysis of Single‐Level Cervical Total Disc Arthroplasty: A Meta‐Analysis

Abstract:OBJECTIVE:The purpose of this review was to investigate whether cervical total disc arthroplasty (CTDA) preserves motion of the treated level and what its effect is on adjacent segments.METHODS:Relevant published reports were collected from PubMed, Medline and Cochrane library. The original studies were considered eligible only if the range of motion (ROM) of the index or adjacent level had been investigated. A meta-analysis was then performed on the collected data. Statistical heterogeneity across the various trials was tested using Cochran's Q statistic and I (2) ; in the case of heterogeneity, a random effect model was used.RESULTS:The weighted mean differences (WMDs) of the index level were 0.34 (95% confidence interval [CI], -0.53∼1.21, P = 0.440) and 0.23 (95% CI, -1.92∼2.38, P = 0.834) in all included studies and randomized control trials (RCTs), respectively. The WMDs of the cranial adjacent levels, caudal adjacent levels and whole cervical spines were 1.01 (95% CI, 0.55∼1.47, P = 0.000), 1.10 (95% CI, 0.61∼1.59, P = 0.000) and 3.40 (95% CI, -6.02∼12.82, P = 0.479), respectively.CONCLUSION:These findings suggest that the protective effect against adjacent segment degeneration provided by cervical arthroplasty might not be as good as has been believed. Long-term supporting evidence is still needed.

Abstract:Noncontiguous cervical disc degenerative disease (CDDD) is a unique form of multilevel CDDD, characterized by segments with significant degeneration at both ends (C3/4, C5/6, C6/7) and an intermediate segment (C4/5) showing no apparent degeneration. The utilization of cervical disc arthroplasty with or without fusion is a potential effective approach for its treatment. Objective The long‐term results of cervical disc arthroplasty (CDA) for noncontiguous cervical degenerative disc disease (CDDD) are still uncertain. Moreover, it is unclear whether CDA delays or avoids the degeneration of the intermediate segment (IS), leading to controversy in the field. Therefore, this study aimed to investigate the mid‐ to long‐term clinical and radiographic outcomes of CDA in treating noncontiguous CDDD and to explore whether the IS degenerated faster after CDA than other non‐surgically treated adjacent segments. Methods We retrospectively analyzed patients with noncontiguous CDDD who underwent CDA in our department between January 2008 and July 2018. The patients were divided into the CDA and hybrid surgery (HS) groups, and clinical and radiographic outcomes were evaluated at routine postoperative intervals. Clinical outcomes were assessed using the Japanese Orthopaedic Association (JOA), neck disability index (NDI), and visual analogue scale (VAS), while radiographic outcomes included cervical lordosis (CL), C2‐C7 range of motion (ROM), segmental ROM, and disc angle (DA) at the arthroplasty level. Complications were also evaluated.Pre‐ and postoperative values were compared using paired t‐tests or Wilcoxon rank‐sum tests. Independent Student t‐tests or Mann–Whitney U tests analyzed continuous data between CDA and HS groups, while chi‐square or Fisher exact tests assessed categorical data. Results Sixty‐four patients with noncontiguous CDDD, with 31 in the CDA group and 33 in the HS group, were evaluated. The mean follow‐up time was over 70 months. The most frequently involved levels were C4/5 and C5/6. Both groups showed significant improvements in JOA, NDI, and VAS values after surgery. Abstract:Objective To evaluate the efficacy of cervical total disc replacement(TDR) with Prodisc-C and anterior cervical discectomy and fusion(ACDF).Methods Forty-seven patients with cervical symptomatic disc disease underwent implantation with the Prodisc-C prosthesis or MC + fusion after anterior cervical discectomy.At the follow-up periods,the clinical outcome was evaluated by Visual Analogue Score(VAS).The Neck Disability Index (NDI) and SF-36,radiographical evaluation include Range of Motoon (ROM) of the implanted level.Results All the patients were assessed 5 years after surgery,VAS scores of Pre-Op of Prodisc-C was (7.0 ± 1.0) score,ACDF was(6.6 ± 1.4)score; 5 years Post-Op of Prodisc-C was (2.1 ±0.9) score,ACDF was (2.9 ±0.9) score,ProDisc-C patients had statistically significantly less neck pain(P ＜ 0.05) ; At 5 years,NDI and SF-36 scores were significantly improved compared with preoperative scores.Heterotopic ossification in 13 Prodisc-C patients occurred,accounted for 56.5％.There were no reports of device failures or implant migration in both groups.Conclusion The results of this clinical series demonstrate that TDR with Prodisc-C is a safe and effective surgical treatment for patients with disabling single level cervical disease.
Changes in the Centers of Rotation in Different Prosthetic Segments Following Continuous Two-Level Cervical Disc Replacement: A Retrospective Study

Abstract:Objective: To observe the short-term to medium-term therapeutic effects and radiological outcomes of adjacent 2-level cervical spondylosis treated with artificial cervical disc replacement (ACDR) using Mobi-C and Bryan discs. To observe changes and discrepancies in the flexion-extension centers of rotation (FE-COR) of different ACDR segments. Methods: Twenty-four patients undergoing 2-level (C4/5 and C5/6) ACDR were analyzed retrospectively (11 patients in the Mobi-C group and 13 patients in the Bryan group). Clinical outcomes were assessed using modified Japanese Orthopedic Association score, Neck Disability Index, and visual analog scale. Preoperative and 2 years' follow-up radiographs were collected to compare total cervical spine curvature (C2-C7) range of motion (ROM), upper and lower ACDR segmental ROM, and the operated adjacent segmental ROM. The FE-COR of the ACDR segment was measured using the mid-plumb line method. The degree of disc degeneration in the adjacent segment was observed. Results: At the 2 years' follow-up, in both group, the modified Japanese Orthopedic Association score increased significantly, and the Neck Disability Index and neck and upper extremity visual analog scale scores decreased significantly compared with preoperative (P < 0.05). There were no significant statistical differences in postoperative scores between the 2 groups (P > 0.05). The overall cervical ROM, the upper and lower segmental ROM at the 2 years' follow-up showed no significant statistical differences compared with the preoperative period (P > 0.05). There was no statistically significant difference in the adjacent segmental ROMs compared with the preoperative period (P > 0.05). Furthermore, the statistical analysis revealed no significant differences in the measurements of the ROM at each time points between the Mobi-C Group and the Bryan Group (P > 0.05). There was no significant difference in the preoperative FE-COR-X (indicating the horizontal position of the point in the coordinate system) and FE-COR-Y (indicating the vertical position of the point in the coordinate system) of upper or lower ACDR segments between the 2 groups (P > 0.05). At the 2 years' follow-up, there were significant differences both in FE-COR-X and FE-COR-Y between the 2 groups (P < 0.05). For the Mobi-C group, in both the upper and lower segment, the FE-COR-X significantly increased compared with preoperative (P < 0.05), while the FE-COR-Y decreased compared with preoperative (P < 0.05). For the Bryan group, no significant changes were observed in the upper or lower segment in both FE-COR-X and FE-COR-Y compared with preoperative (P > 0.05). Fourty-eight adjacent segments (24 superior and 24 inferior segments) were included in the studies of adjacent segment disease. Four segments showed imaging adjacent segment disease (4/48, 8.33%) in 4 patients, of which 2 were mild and 2 were moderate according to the grading criteria. Among them, 2 were from the Mobi-C group and 2 were from the Bryan group. No severe imaging degeneration was observed. Conclusions: In continuous 2-level cervical ACDR surgery, both Mobi-C and Bryan artificial cervical discs achieved satisfactory clinical outcomes in the short to medium term postoperatively. The FE-COR exhibited different trends of change. In the Mobi-C group, the FE-COR for both upper and lower segments shifted anteriorly and inferiorly, whereas in the Bryan group, whether upper or lower, the FE-COR remained closer to the preoperative state. The changes in FE-COR did not significantly affect the short-term to medium-term clinical outcomes postoperatively.
Biomechanics of Hybrid Anterior Cervical Fusion and Artificial Disc Replacement in 3-Level Constructs: an in Vitro Investigation.

Abstract:BACKGROUND:The ideal surgical approach for cervical disk disease remains controversial, especially for multilevel cervical disease. The purpose of this study was to investigate the biomechanics of the cervical spine after 3-level hybrid surgery compared with 3-level anterior cervical discectomy and fusion (ACDF). MATERIAL AND METHODS:Eighteen human cadaveric spines (C2-T1) were evaluated under displacement-input protocol. After intact testing, a simulated hybrid construct or fusion construct was created between C3 to C6 and tested in the following 3 conditions: 3-level disc plate disc (3DPD), 3-level plate disc plate (3PDP), and 3-level plate (3P). RESULTS:Compared to intact, almost 65~80% of motion was successfully restricted at C3-C6 fusion levels (p<0.05). 3DPD construct resulted in slight increase at the 3 instrumented levels (p>0.05). 3PDP construct resulted in significant decrease of ROM at C3-C6 levels less than 3P (p<0.05). Both 3DPD and 3PDP caused significant reduction of ROM at the arthrodesis level and produced motion increase at the arthroplasty level. For adjacent levels, 3P resulted in markedly increased contribution of both upper and lower adjacent levels (p<0.05). Significant motion increases lower than 3P were only noted at partly adjacent levels in some conditions for 3DPD and 3PDP (p<0.05). CONCLUSIONS:ACDF eliminated motion within the construct and greatly increased adjacent motion. Artificial cervical disc replacement normalized motion of its segment and adjacent segments. While hybrid conditions failed to restore normal motion within the construct, they significantly normalized motion in adjacent segments compared with the 3-level ACDF condition. The artificial disc in 3-level constructs has biomechanical advantages compared to fusion in normalizing motion.

Abstract:BACKGROUND The ideal procedure for multilevel cervical degenerative disc diseases remains controversial. Recent studies on hybrid surgery combining anterior cervical discectomy and fusion (ACDF) and artificial cervical disc replacement (ACDR) for 2-level and 3-level constructs have been reported in the literature. The purpose of this study was to estimate the biomechanics of 3 kinds of 4-level hybrid constructs, which are more likely to be used clinically compared to 4-level arthrodesis. MATERIAL AND METHODS Eighteen human cadaveric spines (C2-T1) were evaluated in different testing conditions: intact, with 3 kinds of 4-level hybrid constructs (hybrid C3-4 ACDR+C4-6 ACDF+C6-7ACDR; hybrid C3-5ACDF+C5-6ACDR+C6-7ACDR; hybrid C3-4ACDR+C4-5ACDR+C5-7ACDF); and 4-level fusion. RESULTS Four-level fusion resulted in significant decrease in the C3-C7 ROM compared with the intact spine. The 3 different 4-level hybrid treatment groups caused only slight change at the instrumented levels compared to intact except for flexion. At the adjacent levels, 4-level fusion resulted in significant increase of contribution of both upper and lower adjacent levels. However, for the 3 hybrid constructs, significant changes of motion increase far lower than 4P at adjacent levels were only noted in partial loading conditions. No destabilizing effect or hypermobility were observed in any 4-level hybrid construct. CONCLUSIONS Four-level fusion significantly eliminated motion within the construct and increased motion at the adjacent segments. For all 3 different 4-level hybrid constructs, ACDR normalized motion of the index segment and adjacent segments with no significant hypermobility. Compared with the 4-level ACDF condition, the artificial discs in 4-level hybrid constructs had biomechanical advantages compared to fusion in normalizing adjacent level motion.

Abstract:OBJECTIVE:The aim of this study was to comprehensively compare the clinical and biomechanical efficiency of anterior cervical discectomy and fusion (ACDF) with anterior cervical disc replacement (ACDR) for treatment of multilevel cervical disc disease using a meta-analysis and systematical review.METHODS:A literature search was performed using PubMed, MEDLINE, EMBASE, and the Cochrane Library for articles published between January 1960 and December 2017. Both clinical and biomechanical parameters were analyzed. Statistical tests were conducted by Revman 5.3. Nineteen studies including 10 clinical studies and 9 biomechanical studies were filtered out.RESULTS:The pooled results for clinical efficiency showed that no significant difference was observed in blood loss (P = 0.09; mean difference [MD], 7.38; confidence interval [CI], -1.16 to 15.91), hospital stay (P = 0.33; MD, -0.25; CI, -0.76 to 0.26), Japanese Orthopaedic Association scores (P = 0.63; MD, -0.11; CI, -0.57 to 0.34), visual analog scale (P = 0.08; MD, -0.50; CI, -1.06 to 0.05), and Neck Disability Index (P = 0.33; MD, -0.55; CI, -1.65 to 0.56) between the 2 groups. Compared with ACDF, ACDR did show increased surgical time (P = 0.03; MD, 31.42; CI, 2.71-60.14). On the other hand, ACDR showed increased index range of motion (ROM) (P < 0.00001; MD, 13.83; CI, 9.28-18.39), lower rates of adjacent segment disease (ASD) (P = 0.001; odds ratio [OR], 0.27; CI, 0.13-0.59), complications (P = 0.006; OR, 0.62; CI, 0.45-0.87), and rate of subsequent surgery (P < 0.00001; OR, 0.25; CI, 0.14-0.44). As for biomechanical performance, ACDR maintained index ROM and avoided compensation in adjacent ROM and tissue pressure.CONCLUSIONS:Multilevel ACDR may be an effective and safe alternative to ACDF in terms of clinical and biomechanical performance. However, further multicenter and prospective studies should be conducted to obtain a stronger and more reliable conclusion.

Abstract:Purpose: Adjacent segment degeneration (ASDeg) after anterior cervical discectomy and fusion (ACDF) seriously affects the long-term efficacy of the operation. Therefore, our team has done a lot of research on allograft intervertebral disc transplantation (AIDT) to prove its feasibility and safety. This study will compare the efficacy between AIDT and ACDF in the treatment of cervical spondylosis. Methods: All patients who received ACDF or AIDT in our hospital from 2000 to 2016 and followed up for at least 5 years were recruited and divided into ACDF and AIDT groups. The clinical outcomes including functional scores and radiological data of both groups were collected and compared preoperatively and postoperatively at 1 week, 3 months, 6 months, 12 months, 24 months, 60 months and last follow-up. Functional scores included Japanese Orthopedic Association score (JOA), Neck Disability Index (NDI), Visual Analog Scale of Neck (N-VAS) and Arms (A-VAS) pain, the Short Form Health Survey-36 (SF-36) and imaging dates including digital radiographs in the lateral, hyperextension and flexion positions to assess the stability, sagittal balance and mobility of the cervical spine and magnetic resonance imaging (MRI) scans to assess the degeneration of adjacent segment. Results: There were 68 patients with 25 in AIDT group and 43 in ACDF group. Satisfactory clinical results were obtained in both groups, but the long-term NDI score and N-VAS score in the AIDT group were better. The AIDT obtained the same stability and sagittal balance of the cervical spine as fusion surgery. The range of motion of adjacent segments can be restored to the preoperative level after transplantation, but this increases significantly after ACDF. There were significant differences in the superior adjacent segment range of motion (SROM) between two groups at 12 months (P = 0.039), 24 months (P = 0.035), 60 months (P = 0.039) and the last follow-up (P = 0.011). The inferior adjacent segment range of motion (IROM) and SROM had a similar trend in the two groups. The ratio value of the greyscale (RVG) of adjacent segments showed a downward trend. At the last follow-up, the RVG decreased more significantly in the ACDF group. At the last follow-up, there was a significant difference in the incidence of ASDeg between the two groups (P = 0.000). And the incidence of adjacent segment disease (ASDis) is 22.86% in the ACDF group. Conclusion: The allograft intervertebral disc transplantation may be as an alternative technique to traditional anterior cervical discectomy and fusion for the management of cervical degenerative diseases. For the more, the results showed it would improve cervical kinematics and reduce the incidence of adjacent segment degeneration.
Biomechanical Analysis of Cervical Artificial Disc Replacement Using Cervical Subtotal Discectomy Prosthesis

Abstract:Background: Anterior cervical discectomy and fusion (ACDF) sacrifices segmental mobility, which can lead to the acceleration of adjacent segment degeneration. The challenge has promoted cervical artificial disc replacement (CADR) as a substitute for ACDF. However, CADR has revealed a series of new issues that are not found in ACDF, such as hypermobility, subsidence, and wear phenomenon. This study designed a cervical subtotal discectomy prosthesis (CSDP) consisting of a cervical disc prosthesis structure (CDP structure), cervical vertebra fixation structure (CVF structure), link structure, and locking screw, aiming to facilitate motion control and reduce subsidence. The aim of this study was to assess the biomechanics of the CSDP using finite element (FE) analysis, friction-wear test, and non-human primates implantation study. Study Design: For the FE analysis, based on an intact FE C 2 -C 7 spinal model, a CSDP was implanted at C 5 -C 6 to establish the CSDP FE model and compare it with the Prestige LP prosthesis (Medtronic Sofamor Danek, Minneapolis, MN, United States). The range of motion (ROM), bone-implant interface stress, and facet joint force were calculated under flexion extension, lateral bending, and axial rotation. In addition, CSDP was elevated 1 mm to mimic an improper implantation technique to analyze the biomechanics of CSDP errors in the FE model. Moreover, the friction-wear test was conducted in vitro to research CSDP durability and observe surface wear morphology and total wear volume. Finally, the CSDP was implanted into non-human primates, and its properties were evaluated and verified by radiology. Results: In the FE analysis, the ROM of the CSDP FE model was close to that of the intact FE model in the operative and adjacent segments. In the operative segment, the CSDP error FE model increased ROM in flexion extension, lateral bending, and axial rotation. The maximum stress in the CSDP FE model was similar to that of the intact FE model and was located in the peripheral cortical bone region. The facet joint force changes were minimal in extension, lateral bending, and axial rotation loads in CSDP. In the friction-wear test, after the 150-W movement simulation, both the CVF-link-junction and the CDP-link-junction had slight wear. In the CSDP non-human primate implantation study, no subsidence, dislocation, or loosening was observed. Conclusion: In the FE analysis, the biomechanical parameters of the CSDP FE model were relatively close to those of the intact FE model when compared with the Prestige LP FE model. In terms of CSDP error FE models, we demonstrated that the implantation position influences CSDP performance, such as ROM, bone-implant interface stress, and facet joint force. In addition, we performed a friction-wear test on the CSDP to prove its durability. Finally, CSDP studies with non-human primates have shown that the CSDP is effective.

Abstract:Cervical disc degenerative disease (CDDD) is a common spinal pathology that is often treated with anterior cervical discectomy and fusion (ACDF), cervical disc arthroplasty (CDA), and/or hybrid cervical surgery (HCS). The purpose of this first-time systematic review is to examine the biomechanical outcomes associated with three types of non-contiguous cervical surgeries—ACDF, CDA, and HCS—to provide a greater understanding of non-contiguous cervical surgical biomechanics. A systematic review was performed using PubMed, Cumulated Index to Nursing and Allied Health Literature (CINAHL), MEDLINE, and Web of Science from database inception until June 6th, 2023. The inclusion criteria was any article that reported biomechanical or kinematic outcomes, outcomes for any of the three non-contiguous cervical surgeries, and human-derived and/or human cadaver subjects. A total of 5 biomechanical articles were included from a total of 523 articles. Non-contiguous two-level HCS experienced less drastic range-of-motion (ROM) changes throughout the cervical spine and decreased intervertebral disc pressure (IDP) compared to non-contiguous two-level ACDF. Non-contiguous two-level CDA resulted in more cervical ROM and less non-operative segment facet contact force compared to non-contiguous two level ACDF. There was less cephalad and caudal non-operative segment ROM in non-contiguous two-level ACDF compared to contiguous three-level ACDF.
In vitro study on biomechanical comparison between cervical arthroplasty and fusion

Abstract:Objective To analyze biomechanical properties of cervical spine after anterior cervical discectomy and fusion( ACDF) and total disc replacement( TDR) surgery. Methods Twelve cadaveric cervical spines( C2-T1) were adopted,and the motion and load distributions of the cervical segments under intact state and after ACDF and TDR surgery were tested using a three-dimensional( 3D) optoelectronics measurement system. All the tests were carried out with displacement control in directions of flexion( Flex),extension( Ext),left bending( LB), right bending( RB),left rotation( LR) and right rotation( RR). Motion characteristics of the normal cervical spine and the implant were also discussed. Results In TDR-treated specimens,range of motion( ROM) was well preserved and could restore to the normal ROM distributions,especially in Flex /Ext and LR /RR direction. While in ACDF-treated specimens,ROM presented a large decrease as much as to 73. 41% under the same condition compared with TDR,and ROM distributions were also changed obviously in other motions for the segments. Significant changes of ROM in LB /RB direction occurred in both TDR and ACDF group,which were up to 45. 92% and 108. 06%,respectively. The experimental data indicated that the normal motion of cervical spines was a 3D coupled motion,especially in LB /RB direction,where a 35% rotation around X-axis existed. The cervical spine could recover close to normal coupled motion after TDR surgery. Conclusions TDR surgery can restore the physiological motion of cervical spines more close to the normal state,especially in Flex /Ext and LR /RR direction. The study provides a theoretical basis and quantitative reference for TDR and ACDF surgery in clinic.
External and internal responses of cervical disc arthroplasty and anterior cervical discectomy and fusion: A finite element modeling study

Abstract:Surgical treatment for spinal disorders, such as cervical disc herniation and spondylosis, includes the removal of the intervertebral disc and replacement of biological or artificial materials. In the former case, bone graft is used to fill the space, and this conventional procedure is termed anterior cervical discectomy and fusion (ACDF). The latter surgery is termed as artificial disc replacement ADR) or cervical disc arthroplasty (CDA). Surgeries are most commonly performed at one or two levels. The present study was designed to determine the external (range of motion, ROM) and internal (anterior and posterior load sharing) responses of the spines with one-level and two-level surgeries in both models (ACDF and CDA) using a previously validated finite element model (FEM) of the subaxial cervical spinal column. The FEM simulated the vertebra (cancellous core and cortical shell of the body, posterior elements - laminae, pedicles and spinous processes), discs (anulus fibers, ground substance, and nucleus pulposus), anterior and posterior ligaments of the disc and facet joints, and interspinous and supraspinous ligaments. Appropriate material properties were assigned to the spinal components. The United States Food Drug Administration-approved Mobi-C was used for the CDA option. The FEM was exercised under pure flexion and extension moment loading of 2 Nm in the intact state. The overall ROM of the column was obtained. The hybrid loading protocol applied moments that matched the ROM in the intact spine for both one-level (C5-C6) and two-level (C5-C7) ACDF and CDA surgeries. ROM at the level(s) of surgery, termed the index level was obtained. These data along with anterior column load (ACL) and posterior column load (PCL) sharing were obtained for all surgical options at superior and inferior segments (termed adjacent segment outputs). Results for both one-level and two-level surgeries showed that ACDFs decreases ROM at the index level, while CDAs increase motions compared to the intact normal spine. The ROM, ACL, and PCL increased at both adjacent levels for the ACDF while CDA showed a decrease. Although two-level surgeries resulted in increased these biomechanical variables, greater changes to adjacent segment biomechanics in ACDF may accelerate adjacent segment disease. Decreased ROM and lower load sharing in CDAs may limit adjacent segment effects such as accelerated degeneration. Their increased posterior load sharing, however, may need additional attention for patients with suspected facet joint disease.
Comparison of the Clinical and Radiographic Results Between Cervical Artificial Disk Replacement and Anterior Cervical Fusion A 6-Year Prospective Nonrandomized Comparative Study

Abstract:Study Design: Prospective nonrandomized comparative study.Objective: To compare the long-term clinical and radiographic results of cervical artificial disk replacement (CADR) and anterior cervical discectomy and fusion (ACDF), and to provide our evidence if CADR could reduce adjacent segment degeneration (ASD).Summary of Background Data: CADR is widely used in spine surgery today. Despite the short-term results of it having been ascertained, the long-term results are still under observation. Meanwhile it is still debatable if CADR could reduce ASD in the long run.Materials and Methods: Sixty-three patients with cervical myelopathy who underwent CADR (28) or ACDF (35) with a minimum follow-up of 68 months were included. Japanese Orthopaedic Association score, neck disability index, and Odom's scale were used to evaluate the clinical outcomes. Radiographs, computed tomography, and magnetic resonance imaging were used to evaluate the radiographic outcomes.Results: Both CADR and ACDF groups showed significant improvement on Japanese Orthopaedic Association scores and neck disability index, and there was no significant difference between the 2 groups. The sagittal alignment was maintained for both the groups. The C2-C7 range of motion had no significant change for CADR group, whereas it significantly decreased for ACDF group. The range of motion at index level of CADR patients decreased from 9.5 +/- 3.7 degrees before surgery to 7.0 +/- 3.0 degrees at 3 months after surgery (P < 0.001), and it was maintained to 6.6 +/- 4.1 degrees at last follow-up without significant decrease (P = 0.448). We used radiographic data to evaluate ASD and we found the incidence of ASD was significantly lower for CADR group than ACDF group.Conclusions: The clinical and radiographic results of CADR over 6-year follow-up are basically satisfying. Compared with ACDF, CADR could better preserve physiological motion and biomechanics of cervical spine, and reduce the occurrence of ASD.
