"Where my responsibility lies": Reflecting on medicine during the Holocaust to support personal and professional identity formation in health professions education

Abstract:Objectives: Physicians and the medical/scientific establishment during Nazism and the Holocaust committed egregious ethical violations including complicity with genocide. Critical reflection on this history serves as a powerful platform for scaffolding morally resilient professional identity formation (PIF) with striking relevance for contemporary health professions education and practice. Study aim was to explore the impact of an Auschwitz Memorial study trip within the context of a medicine during Nazism and the Holocaust curriculum on students' personal and PIF. Methods: The authors analyzed 44 medical and psychology students' reflective writings from a 2019 Auschwitz Memorial study trip using immersion-crystallization qualitative thematic analysis. Results: Six distinct themes and 22 subthemes were identified and mapped to a reflective learning process model: 1. "What am I bringing?" 2. "What am I experiencing through the curriculum?" 3. "What am I initially becoming aware of as a first response?" 4./5. "How and what am I processing?" 6. "What am I taking with me?" Particularly compelling subthemes of power of the place, emotional experience, reflection on myself as a moral person, and contemporary relevance referred to impactful course elements. Conclusions: This curriculum catalyzed a critically reflective learning/meaning-making process supporting personal and PIF including critical consciousness, ethical awareness, and professional values. Formative curriculum elements include narrative, supporting emotional aspects of learning, and guided reflection on moral implications. The authors propose Medicine during Nazism and the Holocaust curriculum as a fundamental health professions education component cultivating attitudes, values, and behaviors for empathic, moral leadership within inevitable healthcare challenges.

Abstract:The majority of the German medical institutions and nearly half of the clinicians during the Nazi regime provided unwavering support to the distorted Nazi views on health and disability. Leading physicians were an integral part of the atrocities carried out during that era, with some of them having a vital role in executing the ideology of the "final solution". The names of Carl Clauberg and Walter Stoeckel, two notable doctors in the fields of obstetrics and gynecology, were also linked with the third Reich. Both of these physicians, collaborated, each in his own way, with the Nazi regime, which tainted their legacy indefinitely. The two received professional honor for their contribution to the fields of obstetrics and gynecology, which were unrelated to their Nazi past, after their names were linked in the form of eponyms to surgical procedures and examinations, which they developed. In recent years, as a consequence of their disturbing past, there are increasing arguments which call for erasing their eponyms from the medical lexicon. We, on the contrary, believe that the opposite is true and that maintaining these eponyms will actually enable teaching future generations and serve a dual educational role to both highlight professional and scientific achievements as well as serve as a mark of Cain and warning that professional achievements do not prevent ethical decline. Preserving the name will provide an educational opportunity to teach about the responsibilities that come with professional leadership, which these doctors abused carelessly, in order to prevent history from repeating itself.
[Against oblivion: Remembering the first Jewish women doctors of the German Society for Internal Medicine]

Abstract:In 1933, the German Society for Internal Medicine (DGIM) willingly adapted to the ideology and politics of the Nazi regime. Seven members of the Society were Jewish women doctors, women making up 1 % of all members by that time. By pursuing a career in medicine, these women refused to take on the traditional woman's role, opting instead for an unusual path in life and making the medical profession their central mission despite difficult conditions. Under Nazi dictatorship, they were deprived of their livelihood, disenfranchised, persecuted and forced into exile. While this also applies to their male colleagues, Jewish women doctors are considerably less visible. This article presents and contextualises their biographies in order to increase their visibility and integrate them more explicitly into today's culture of remembrance.

Abstract:Human rights may feel self-apparent to us, but less than 80 years ago, one of the most advanced countries at the time acted based on an utterly contrary ideology. The view of social Darwinism that abandoned the idea of the intrinsic value of human lives instead argued that oppression of the inferior is not only inevitable but desirable. One of the many catastrophic outcomes is the medical data obtained from inhuman experiments at concentration camps. Ethical uncertainty over whether the resulting insights should be a part of the medical literature provides a chance to consider the seemingly irreplaceable social construct of human dignity. Would any medical benefit justify the utilization of this illicit data? Would utilization even qualify as an insult to the dignity of the exploited subjects, or is this a question about intersubjective meaning? This work discusses the wisdom in blind adherence to human dignity, the possibility of retrospective insults, moral complicity, contrary viewpoints, and possible resolutions.
Neuroscience research in the Max Planck Society and a broken relationship to the past: Some legacies of the Kaiser Wilhelm Society after 1948

Abstract:The development of the brain sciences (Hirnforschung) in the Max Planck Society (MPG) during the early decades of the Federal Republic of Germany (FRG) was influenced by the legacy of its precursor institution, the Kaiser Wilhelm Society for the Advancement of Science (KWG). The KWG's brain science institutes, along with their intramural psychiatry and neurology research programs, were of considerable interest to the Western Allies and former administrators of the German science and education systems in their plans to rebuild the extra-university research society-first in the British Occupation Zone and later in the American and French Occupation Zones. This formation process occurred under the physicist Max Planck (1858-1947) as acting president, and the MPG was named in his honor when it was formally established in 1948. In comparison to other international developments in the brain sciences, it was neuropathology as well as neurohistology that initially dominated postwar brain research activities in West Germany. In regard to its KWG past, at least four historical factors can be identified that explain the dislocated structural and social features of the MPG during the postwar period: first, the disruption of previously existing interactions between German brain scientists and international colleagues; second, the German educational structures that countered interdisciplinary developments through their structural focus on medical research disciplines during the postwar period; third, the moral misconduct of earlier KWG scientists and scholars during the National Socialism period; and, fourth, the deep rupture that appeared through the forced migration of many Jewish and oppositional neuroscientists who sought to find exile after 1933 in countries where they had already held active collaborations since the 1910s and 1920s. This article examines several trends in the MPG's disrupted relational processes as it sought to grapple with its broken past, beginning with the period of reinauguration of relevant Max Planck Institutes in brain science and culminating with the establishment of the Presidential Research Program on the History of the Kaiser Wilhelm Society in National Socialism in 1997.

Abstract:This article presents the findings of an ongoing supervision group (founded in 1999) researching the after-effects of the Nazi period on people in psychotherapy in Germany today. The unacknowledged collective shadow hidden behind half-truths, prevarications, and silence itself prevents a genuine working through of the Nazi past. Patients' lack of knowledge concerning their families' own past leads to unconscious guilt, which often then leads to psychosomatic disturbances. But this is not only a problem in Germany. Unacknowledged collective shadows are prevalent in many countries worldwide. Psychological difficulties on the individual and societal levels result.

Abstract:Our understanding of the history of psychiatry has been warped by the way history is remembered – collectively and personally; this is the point of departure from which Wynter, Wallis and Ellis have embarked upon this splendid essay anthology. The introduction provokes something of a liminal space by describing psychiatry as a specialty that is ever existing between 'the terrible past and the ideal future'. They argue that barbarity has become a cultural trope for psychiatry, especially when historical treatments are viewed through modern framing without the scaffolding of contemporaneous societal values. Progress seems to always be in response to wanting to heal this iniquity and provide a change in direction. The authors endeavour to show us that the actuality of psychiatry may exist somewhere between these two opposing poles, and furthermore it is interested in the idea that modern mental health services have arisen from within this in-between space. The essays loosely stick to a theme of memory and remembrance, and are arranged within five sections: governance; practitioners; case books; oral histories; and personal recollections. The various essays look at different aspects of psychiatry through different eras and across the globe, with very distinct voices and forms, but this thematic arrangement provides a sense of cohesion. However, there is no onus on the reader to read the whole collection all at once, with most of the pieces having enough strength to be considered in their own right. We see Britain's colonial footprint in the particularly provocative 'Madness, memory and delusion in late nineteenth-century Barbados', whereby Leonard Smith skilfully introduces readers to the cultural matrix of colonial Barbados, where many people were living in destitution, and then goes on to consider a series of nineteenth-century lunatic asylum case records held in the Barbados National Archive. A re-examination of these material impressions of the past acknowledges the colonial gaze through which patients were described, but also provides a new way of seeing patients and their problems, beyond the hermeneutic trouble entailed by the passage of time. A chapter about Bombay's lunatic asylums around the turn of the twentieth century readily relates to this one, but other works describing topics as far reaching as the remodelling of the Sigmund Freud Museum in Vienna and the nostalgia experienced by staff in the wake of an American psychiatric hospital ensure a significant breadth of content. The book ends with a devastatingly honest reflective essay by Allan Beveridge that focuses sharply upon his experience of psychiatric training in Edinburgh in the 1980s. He shares his personal sense of excitement upon embarking upon his career that gives way to unease as he faces the reality of clinical practice, and the discombobulating effect of competing explanations and opposing views that continue to persist, to a greater or lesser extent, to this day. The book grasps onto the Derridean idea that there is an ever-transforming inheritance of the past in light of changing contexts of the future, and that the key to history lies in how it is remembered. Through an exploration of remembrance, this work generates novel insights, which allow us to recast how we understand previous insights into psychiatry's past. This thought-provoking essay collection will generate enthusiasm for new ways of considering the past and reframing the present through troubling our interpretation of what has been written before. It is certainly worth reading to generate reflections on what the legacy of our own work may be.

The blood from Auschwitz and the silence of the scholars

Abstract:The Kaiser Wilhelm Institute for Anthropology, Human Genetics and Eugenics in Berlin-Dahlem was the centre of scientific racism in Nazi Germany. Its bad history culminated in a research project to analyse the molecular basis of racial differences in the susceptibility to various infectious diseases such as tuberculosis. Josef Mengele, a former postdoc of the director of the institute, Otmar von Verschuer, collected blood samples and other material in Auschwitz from families and twins of Jews and Gypsies. The blood samples were analysed by Günther Hillmann in the Berlin laboratory of Nobel Prize winner Adolf Butenandt. Butenandt had just moved to Tübingen. The project was paid for by the Deutsche Forschungsgemeinschaft. Butenandt, Hillmann and von Verschuer made scientific careers in the Federal Republic. To the present day this past has not been acknowledged by the Max-Planck-Gesellschaft as part of its history.
Bearing witness: Medical education and reflecting on the Holocaust then and now

Abstract:Background: Despite advocacy from the Association of American Medical Colleges (AAMC) and The Lancet Commission on medicine, Nazism, and the Holocaust, Holocaust education is lacking in medical education. To address this gap, students at Geisinger Commonwealth School of Medicine (GCSOM) viewed an Association of American Medical College (AAMC) webinar about medicine during the Holocaust as part of the required curriculum for first year medical students introduced in 2022. Methods: As part of their doctoring course, Physician and Patient Centered Care (PPCC), students viewed the AAMC webinar "The legacy of the role of medicine during the Holocaust and its contemporary relevance" and participated in two structured reflections: a written reflection on how webinar topics inform students' professional development and a verbal reflection on learning from the Holocaust to develop a sense of moral courage, advocacy, and activism in medicine. Researchers conducted qualitative analysis of written reflections and analyzed session surveys to determine key themes and impact of the session. Results: Of the 108 enrolled in PPCC, 59 (54.6%) completed a post session Likert scale survey assessing the impact of the webinar on their personal and professional development. As an average, respondents moderately agreed that the webinar impacted their personal and professional development, with 91% slightly, moderately, or strongly agreeing. Additionally, thematic analysis of required written reflections indicated a majority of students (62.5%) identified the need for additional medical humanities education about the Holocaust and its relevance to medicine. Conclusion: Holocaust education encourages medical students to bear witness to past medical atrocities and critically assess the profession and their personal-professional growth. Continued structured integration of the Holocaust in medical education supports critical self-reflection and the development of morally courageous physicians who endorse and practice social accountability in medicine.

Abstract:There are increasing calls for coverage of medicine during the Holocaust in medical school curricula. This article describes outcomes from a Holocaust and medicine educational program featuring a study trip to Poland, which focused on physician complicity during the Holocaust, as well as moral courage in health professionals who demonstrated various forms of resistance in the ghettos and concentration camps. The trip included tours of key sites in Krakow, Oswiecim, and the Auschwitz-Birkenau concentration camps, as well as meeting with survivors, lectures, reflective writings, and discussions. In-depth interviews and reflective writings were qualitatively analyzed. Resulting themes centered on greater understanding of the relationship between bioethics and the Holocaust, recognizing the need for moral courage and social awareness, deeper appreciation for the historical roles played by dehumanization and medical power and their contemporary manifestations, and the power of presence and experiential learning for bioethics education and professional identity formation. These findings evidence the significant impact of the experience and suggest broader adoption of pedagogies that include place-based and experiential learning coupled with critical reflection can amplify the impact of bioethics and humanism education as well as the process of professional identity formation of medical students.

No Silence, but Whispers: Postwar Exhibitions on Nazi Crimes and the Shoah in Austria, 1945–1949

Abstract:Until the 1990s, the myth that Austria had been a victim of National Socialism reigned. Recent historiography has turned this discourse diametrically around, highlighting, among other things, the limited willingness of large segments of Austrian society after 1945 to address their guilt and responsibility for Nazi crimes. However, these issues have seldom been explored in depth. The exhibitions of the early postwar era shine light on precisely these tentative attempts. Those initiated by the Allies, aimed to confront the locals with Nazi crimes, were followed by local exhibitions, which were nevertheless unable to find a means of expression transcending a mere documentation of the horrors that had occurred. The Viennese exhibition Niemals vergessen! – a spectacular show from a curatorial perspective – already moved in a twilight zone between serious engagement with the subject and projecting guilt onto Germany. Yet the show’s motto − ‘We are all guilty!’ − demonstrates that such statements were possible in 1946 that would later become suppressed. This phase of direct confrontation with National Socialism ended with a watered-down attempt to stage an anti-racist exhibition at the Natural History Museum in 1949, although further exhibitions were mounted by resistance groups, albeit with different aims in mind. One reason for the failure of these early initiatives may have been the absence of an appropriate language with which to discuss the Holocaust, a form of ‘speechlessness’ in light of the dimensions of this crime. Not until 1965 would the Holocaust be exhibited to the Viennese public, in a traveling exhibition at the Chamber of Labor. The great silence that is presumed to have dominated in the early postwar period turns out to be no less of a myth: Whoever listened closely would at least have heard a whisper.
The case of Robert Herrlinger: a unique postwar controversy on the ethics of the anatomical use of bodies of the executed during National Socialism

Abstract:Historical evidence shows that German anatomists used bodies of executed victims of the National Socialist (NS) regime for anatomical purposes. However, there has been little direct information on these anatomists' thoughts and motivations, and a public discussion of their activities and ethics only started in the late 1980s. The present study documents a unique postwar controversy surrounding the promotion of the anatomist and medical historian Robert Herrlinger at the university of Würzburg in the late 1950s. This intramural debate had originally been mentioned by Goetz Aly in 1987. Herrlinger's files record his career as a representative of the discipline of medical history at the university of Würzburg from 1951 to 1960. He never worked there as an active anatomist. When the university senate applied for his appointment as full professor in 1957, the internist Ernst Wollheim, the pediatrician Joseph Ströder, and the psychiatrist Heinrich Scheller strongly opposed this move in a dissenting opinion based on Herrlinger's anatomical work on bodies of executed NS-victims. They claimed that he lacked the moral prerequisites required in a teacher of medical ethics. A highly controversial debate followed and was remarkable for addressing most of the questions of the ethical and political attitudes and responsibilities of anatomists in NS-Germany that are still being discussed today and are relevant for modern anatomy. It was also significant that Wollheim, Ströder and Scheller objected to Herrlinger in his role as a medical historian, not as an anatomist. The senate finally rejected the dissenting opinion and Herrlinger was promoted.
Beyond a Year of Sanctions in Science

Abstract:While sanctions in political and economic areas are now part of the standard repertoire of Western countries (not always endorsed by UN mandates), sanctions in science and culture in general are new. Historically, fundamental research as conducted at international research centers such as CERN has long been seen as a driver for peace, and the Science4Peace idea has been celebrated for decades. However, much changed with the war against Ukraine, and most Western science organizations put scientific cooperation with Russia and Belarus on hold immediately after the start of the war in 2022. In addition, common publications and participation in conferences were banned by some institutions, going against the ideal of free scientific exchange and communication. These and other points were the topics of an international virtual panel discussion organized by the Science4Peace Forum together with the "Natural Scientists Initiative - Responsibility for Peace and Sustainability" (NatWiss e.V.) in Germany and the journal "Wissenschaft und Frieden" (W&F) (see the Figure). Fellows from the Hamburg Institute for Peace Research and Security Policy (IFSH), scientists collaborating with the large physics research institutes DESY and CERN, as well as from climate and futures researchers were represented on the panel. In this Dossier we document the panel discussion, and give additional perspectives. The authors of the individual sections present their personal reflections, which should not be taken as implying that they are endorsed by the Science4Peace Forum or any other organizations. It is regrettable that some colleagues who expressed support for this document felt that it would be unwise for them to co-sign it.

Jewish Spaces in Present Vienna: A Relational, Hybrid Approach

Abstract:In October 2017, Vienna's Leopoldstadt community succeeded in reinstalling a Hebrew street sign in a public space of the second district. This achievement became possible in large part due to the efforts of an active online community that encouraged many people to share their wish to have visible signs of the former historic Jewish quarter in the present urban space. Through vigorous Facebook and other social media activities, the interest that the group generated put pressure on the city, leading to the support of an art project. The placement of the Hebrew street sign marked a hybrid way of constructing Jewish urban spaces. The dialogue between virtual and physical spaces added new layers to the historic Jewish quarter of Vienna; this way of relational space making is, I wish to argue, paradigmatic for today's Europe as it witnesses the heyday of Holocaust tourism and klezmer revivals. In this article, I investigate this space-making process in Jewish public history in present Vienna and examine how the virtual community frames the way urban Jewish spaces are constructed.

Abstract:A LEAFLET on “Science in Austria”, presented on the occasion of the meeting of British men of science and the Association of Austrian Chemists, Engineers and Scientific Workers in Great Britain in support of the restoration of science in Austria, on November 3, 1945 (see Nature, Jan. 12, 1946, p. 53), includes a survey of the present position (from the Association, 133 Hatherley Court, London, W.2. Pp. 28). It refers to material destruction due to the War-heavy damage to the main buildings of the University in Vienna, the loss of a large part of the books of the University Library, and the total loss of the Institute of Experimental Biology are mentioned -and of moral devastation due to Nazi rule and resistance to the Germans in the universities. The Provisional Government and the Secretary for Education are attributing the greatest importance to repairing the intellectual damage of seven years of Nazi rule. A short survey of past scientific achievements in Austria gives an impressive picture of our debt to Austria in physics, radioactivity, chemistry, zoology and botany, even in the difficult period through which this small and impoverished country passed between the world wars. The concluding part of the pamphlet, “What Austrian Scientists Desire”, expresses a hope for early restoration of contact between Austrian and Allied men of science, and includes a note by Emil Abel on the pioneers of physical chemistry in Austria, a tribute by F. Eirich to the work of H. Mark and W. Pauli, sen., in colloid chemistry, and other notes on Austrian work by L. Janossy (on cosmic rays), St. Jellinek (electro-pathology), H. Steiner (physical chemistry) and K. Weissenberg (on Guido Holzknecht's work on the application of X-rays to medicine).

Abstract:The European Holocaust Research Infrastructure (EHRI) has been set up by the European Union to create a sustainable complex of services for researchers. EHRI will bring together information about dispersed collections, based on currently more than 20 partner organisations in 13 countries and many other archives. EHRI, which brings together historians, archivists and specialists in digital humanities, strives to develop innovative on-line tools for finding, researching and sharing knowledge about the Holocaust. The paper describes its current progress and collaboration across the disciplines involved.

Abstract:Ziel Die Überlebenden des Holocaust trafen in der Nachkriegszeit bei ihren Anträgen auf Entschädigung für psychische Schädigungen oft auf ablehnend begutachtende deutsche Psychiater, was auf Kritik stieß. Dieser Beitrag untersucht den nachfolgenden Wandel der herrschenden Lehrmeinung zu Traumafolgestörungen. Methodik Deutschsprachige fachwissenschaftliche Beiträge von 1946 bis 1969 werden auf die darin vertretenen paradigmatischen Auffassungen von der Ätiologie psychischer Störungen nach Krieg und nationalsozialistischer Verfolgung analysiert. Ergebnisse Erst eine neue Generation von Psychiatern konnte unter Rückgriff auf anthropologische und teilweise auch psychoanalytische Theorieelemente adäquatere psychotraumatologische Konzepte entwickeln. Diese stellten zugleich auch den medizinischen Krankheitsbegriff der Psychiatrie in Frage. Schussfolgerung Weniger das Fortbestehen antisemitischer Ressentiments, sondern vor allem innerwissenschaftliche Gründe wie ein erfahrungsferner Dogmatismus standen der wissenschaftlichen Fortentwicklung im Weg. Objective After WW II, Holocaust survivors often faced negative assessments by German psychiatrists when claiming compensation for psychological damage, a fact that was met with criticism. This study examines how as a result the prevailing doctrine on trauma sequelae underwent a transformation. Methods Academic contributions in German language from 1946 to 1969 are analyzed for their para-digmatic views on the etiology of mental disorders after war and Nazi persecution. Results Only a new generation of psychiatrists was able to develop more adequate psychotraumatological concepts by using anthropological and occasionally psychoanalytic theoretical elements. At the same time this also questioned the medically oriented concept of illness in psychiatry. Conclusion Less the persistence of anti-Semitic sentiments but rather internal scientific reasons such as an unevidenced dogmatism obstructed the the advancement of science. Abstract:The Guest Editors of this issue of Seminars in Neurology are Drs. Sarah Wahlster and Aaron Berkowitz. Dr. Sarah Wahlster is Associate Professor of Neurology, Neurosurgery (adj), and Anesthesiology (adjunct) at the University of Washington, Seattle. She is the founding fellowship director and section head for Neurocritical Care. She works as a neurointensivist and clinician educator at Harborview Medical Center, where she serves as the medical staff president (as of July 2024). Dr. Wahlster obtained her medical degree at the Ruprecht-Karls Universität Heidelberg, Germany, and completed her neurology residency in the Massachusetts General Hospital/Brigham and Women's Hospital (MGH/BWH) Partners Neurology Program, where she served as chief resident. She then completed a neurocritical care fellowship at the MGH/BWH Partners program before joining the faculty at the University of Washington. Her academic interests include mechanical ventilation in acute brain injury, long-term outcomes of patients and their families after severe acute brain injury, neurological care in resource-limited settings, and medical education. She currently chairs the Neurocritical Care Society Fellowship Director Section. She has collaborated with clinicians in Peru, Ethiopia, Bhutan, Ecuador, Nigeria, and Uganda to build educational material for resource-limited settings. She has published over 40 peer-reviewed articles, and several book chapters, including for Pocket Medicine and Pocket Primary Care. Dr. Aaron Berkowitz is Professor of Clinical Neurology at University of California San Francisco (UCSF) where he serves as a neurohospitalist, general neurologist, and clinician educator at the San Francisco Veterans Administration and San Francisco General Hospital. He previously served as director of global neurology at Brigham and Women's Hospital and Associate Professor of Neurology at Harvard Medical School, where he designed and directed the clinical neuroanatomy/neuroscience course for first year medical students. He completed medical school at Johns Hopkins and neurology residency in the MGH/BWH Partners Neurology Program, where he served as chief resident. As Health and Policy Advisor to Partners In Health, Senior Specialist Consultant to Doctors Without Borders, and prior Chair of the American Academy of Neurology (AAN) Global Health Section and prior Co-Chair of the American Neurological Association Global Health Section, he has worked tirelessly to improve access to neurologic care and education worldwide. This work has been recognized by the Mridha Humanitarian Award from the American Brain Foundation in 2018 and the Viste Patient Advocate of the Year Award from the AAN in 2019. His work as a neurology educator has been recognized by the Residency Teacher of the Year Award from the Harvard Neurology Residency program in both 2018 and 2019, the O'Hara Excellence in Preclinical Teaching award from Harvard Medical School in 2016, appointment to the editorial boards of Continuum and Practical Neurology, and frequent invitations to discuss neurology cases on the popular Clinical Problem Solvers podcast. He has published over 80 peer-reviewed articles, several book chapters including the neurology chapter for the Oxford Manual of Humanitarian Medicine , and four books including the neurology textbook Clinical Neurology and Neuroanatomy: A Localization-Based Approach and One by One by One: Making a Small Difference Amid a Billion Problems about his work caring for patients with brain tumors in rural Haiti. We greatly appreciate the efforts of Drs. Wahlster and Berkowitz, as well as all of the contributing authors, for their wonderful work in this special issue of Seminars in Neurology . We have not put forth an issue like this before, committed to underserved and marginalized populations, and one could argue that this is long overdue. I wish to personally thank Sarah and Aaron for their dedication to this issue, bringing attention to the worldwide perspective on neurological health and illness, and the very real, unique challenges underserved and marginalized populations face.
