Abstract:AIM: We report here, a case series of patients with acute kidney injury (AKI) after ingestion of paraphenylene diamine (PPD) a derivative of analine. It is used as a colouring agent to dye hair, fur and plastic and in photographic films.METHODS: Subjects for the study reported here comprised a cohort of 100 patients coming to this institution with AKI following PPD poisoning. AKI was defined according to RIFLE criteria and PPD poisoning on the basis of history and presenting features. All patients had normal sized kidneys on ultrasonography and no previous co- morbidity.RESULTS: One hundred patients with AKI after PPD exposure were brought to this institute between May 2010 and February 2015. Among these, 56 were females, with mean age of 23.11 ± 7.94 years. Cause of AKI was toxic rhabdomyolysis as indicated by marked rise in muscle enzymes with mean lactate dehydrogenase and creatinine phosphokinase levels of 6665.22 ± 6272.04 and 194 486.66 ± 301 905.80, respectively. Hepatotoxicity with raised aspartate aminotransferase and alanine aminotransferase was a main feature of the studied population. Renal replacement was required in 97% of patients. Complete renal recovery was observed in 77 patients, while 16 died during the acute phase of illness. Respiratory failure and recurrent hyperkalaemia were the main causes of mortality.CONCLUSION: Easy availability and low cost of PPD has lead to a remarkable increase in the use of this substance, especially for suicidal purposes. Awareness of its effects among health professionals, as well as at a societal and government level, is needed at this time.

Abstract:Background: Para phenylene diamine (PPD) or black stone, a common hair-dye ingredient in South Asia, has recently emerged as a new means of suicidal attempts in developing countries especially in Asia. Objective: To determine the outcome of deliberate Para phenylene di-amine ingestion among young women. Methodology: This descriptive analysis was conducted upon a sample of 400, young women (aged 18 to 35 years) presenting to the emergency department from June 2019 to December 2021, with deliberate Para phenylene di-amine ingestion. After taking written informed consent, the data was recorded onto a structured questionnaire containing inquiries pertaining to basic biodata, socioeconomic details, time elapsed since ingestion, clinical signs and symptoms and the outcome (within 24 hours). Results: The mean age of the sample stood at 23 (SD ± 2) years. Out of the total 400 cases of deliberate ingestion reporting to the study setting, 19 were asymptomatic and were discharged after initial care. Among the symptomatic patients, 212 (53%) recovered within 12 hours without need for intensive care, while 139 (34.75%) needed admission to intensive care unit, following which a full recovery was achieved within 24 hours (98 patients) or more (31 patients), while the remaining 10 patients succumbed to the poisoning. 30 patients reached the hospital in critical state and died in the emergency department. Conclusions: The cumulative mortality rate recorded in the research was 10%. Factors such as time between ingestion and presentation to the hospital, the quantity of ingestion and the general health status, affected the outcome of the patient.

Abstract:The use of hair dye has been emerging worldwide however usage of Paraphenylenediamine (PPD) in making hair dye is generally restricted to underdeveloped and developing countries. In particular, prevalence of accidental and suicidal ingestion is more in low socioeconomic areas. The spectra of hair dye toxicity is wide, however, it presents more commonly with severe angioedema of face and neck leading to respiratory failure, rhabdomyolysis complicating into acute kidney injury, myocarditis and acute liver injury. Here we present a unique case of PPD poisoning in a young female presented with laryngeal edema and marked rhabdomyolysis. Preemptive shifting to Critical care unit and elective endotracheal intubation for air way patency obviated the need of tracheostomy and precluded its related complications. Moreover, aggressive intravenous hydration prevented from renal failure despite markedly raised Creatine phospho kinase (CPK) levels.

Abstract:Treatment of degenerative spinal disorders by fusion produces abnormal mechanical conditions at mobile segments above or below the site of spinal disorders and is clinically referred to as adjacent segments disc disease (ASDD) or transition syndrome in the case of a previous surgical treatment. The aim of the current study is to understand with the help of poro‐elastic finite element models how single or two level degeneration of lower lumbar levels influences motions at adjacent levels and compare the findings to motions produced by single or two level fusions when the adjacent disk has varying degree of degeneration. Validated grade‐specific finite element models including varying grades of disc degeneration at lower lumbar levels with and without fusion were developed and used to determine motions at all levels of the lumbar spine due to applied moment loads. Results showed that adjacent disc motions do depend on severity of disc degeneration, number of disc degenerated or fused, and level at which degeneration or fusion occurred. Furthermore, single level degeneration and single level fusion produced similar amount of adjacent disc motions. The pattern of increase in adjacent segment motions due to disc degeneration and increase in motions at segment adjacent to fusion was similar. Based on the current study, it can be concluded that disc degeneration should also be considered as a risk factor in addition to fusion for generating adjacent disc degeneration. © 2016 Orthopaedic Research Society. Abstract:Introduction Organophosphate ingestion is the commonest cause of self-harm encountered at poison control centers in Pakistan. It usually affects a young populous. Organophosphates are found in various forms and formulations that are easily accessible to the general public. These compounds are extremely potent poisons causing rapid clinical deterioration with minimal ingestion or exposure. Signs and symptoms can range from mild or none to severe such as bradycardia, miosis, fasciculations, seizures and altered level of consciousness. Poisoning severity is measured using the Peradeniya Organophosphorus Poisoning (POP) scale. Mortality rates are relatively low for mild to moderate disease. Severe disease as calculated by the POP carries an exceptionally high mortality rate. The National Poisoning Control Centre (NPCC) at Jinnah Postgraduate Medical Centre, Karachi treats an extraordinary number of poisoning cases on a daily basis. Despite this data pertaining specifically to OP ingestion is nearly absent. There have been no studies analyzing the various aspects of organophosphate poisoning in the last 30 years to the best of our knowledge. Here, we look to rectify this. Aims To evaluate the demographics, severity scores and outcomes of organophosphate poisoning cases in the last year from the NPCC, Karachi. Methods This was a retrospective study. It was held from 1<sup>st</sup> January 2019 to 31<sup>st</sup> December 2019. All data was recorded from patients admitted to the NPCC with a proven diagnosis of organophosphate poisoning. Results Three thousand and three hundred patients were inducted into this study. Over 3/4th of the patients were teenagers or aged less than 30 years. Almost all referrals were made from within the city. Overall survival rate at 28 days was 89.45%. Most patients presented with mild to moderate disease as calculated by the POP; severe disease had a mortality rate of nearly 50%. Conclusion Organophosphates make up a significant portion of all cases of poisoning treated at the NPCC. The POP is an excellent tool to evaluate disease severity. Overall survival rates are good but mortality rate is high for severe disease even in young patients.

Abstract:Akshaya Srikanth Bhagavathula, Deepak Kumar Bandari, Moien Khan, Abdulla Shehab Indian Journal of Pharmacology 2019 51(5):302-315 BACKGROUND: Suicide is a public health problem, and the number of paraphenylenediamine (PPD)-containing hair dye poisoning with suicidal intentions is increasing in developing countries. In order to better understand this situation, we aimed to conduct a systematic review and meta-analysis to estimate the prevalence and complications associated with hair dye poisoning in developing countries. METHODS: We conducted a systematic review of epidemiological studies using MeSh terms and text keywords to identify studies from the inception to March 2016 about hair dye poisoning with suicidal intentions in developing countries. A meta-analysis was used to calculate the pooled prevalence proportion of hair dye poisoning and its major complications. Data extraction, data analysis, and risk of bias assessment were performed. RESULTS: Thirty-two studies were included in the systematic review and 29 of these studies containing 5,559 subjects covering six countries were included in the meta-analysis. The pooled prevalence proportion of hair dye poisoning with suicidal intentions was 93.5% (95% confidence interval [CI] = 91.6–95.4) with a mortality rate of 14.5% (95% CI = 11.1–17.9). Of these, 73.8% were female, and 26.2% were male (sex ratio: 2.7:1). The occurrence of angioneurotic edema in hair poisoning patients was 67.1% (95% CI = 56.6–77.6), and tracheostomy intervention was considered in 47.9% (95% CI = 22.7–73.2) patients with respiratory distress. Acute renal failure was noticed in 54.7% (95% CI = 34.5–74.9) of the pooled samples and mortality rates were 14.5% (95% CI = 11.1–17.9). The pooled rate of the population studied from Asia and Africa showed 94.6% (95% CI = 92.5–96.7) and 82.9% (95% CI = 70.6–95.3), respectively, ingested hair dye with suicidal intentions. Further, studies carried out in Africa showed slightly higher mortality of 15.1% (95% CI = 6.56–23.7) than the Asians 14.3% (95% CI = 10.5–18.1). CONCLUSION: This meta-analysis provided clear evidence of the prevalence of hair dye poisoning among individuals with suicidal intentions and had given robust evidence for policy making to curtail emerging PPD-containing hair dye poisoning in developing countries.

Abstract:Accidental or suicidal poisoning with yellow phosphorus or metal phosphides (YPMP) such as aluminum (AlP) zinc phosphide (Zn3P2) commonly cause acute liver failure (ALF) and cardiotoxicity. These are used as household, agricultural and industrial rodenticides and in production of ammunitions, firecrackers and fertilizers. In absence of a clinically available laboratory test for diagnosis or toxin measurement or an antidote, managing their poisoning is challenging even at a tertiary care center with a dedicated liver intensive care unit (LICU) and liver transplant facility. Patients and methods: Patients with YPMP related ALF were monitored using standardized clinical, hemodynamic, biochemical, metabolic, neurological, electrocardiography (ECG) and SOFA score and managed using uniform intensive care, treatment and transplant protocols in LICU. Socio-demographic characteristics, clinical and biochemical parameters and scores were summarized and compared between 3 groups i.e. spontaneous survivors, transplanted patients and non-survivors. Predictors of spontaneous survival and the need for liver transplant are also evaluated. Results: Nineteen patients with YPMP related ALF were about 32 years old (63.2% females) and presented to us at a median of 3 (0 - 10) days after poisoning. YPMP related cardiotoxicity was rapidly progressive and fatal whereas liver transplant was therapeutic for ALF. Spontaneous survivors had lower dose ingestion (<17.5 grams), absence of cardiotoxicity, < grade 3 HE, lactate < 5.8, SOFA score < 14.5, and increase in SOFA score by < 5.5. Patients with renal failure need for CVVHDF and KCC positivity on account of PT-INR > 6.5 had higher mortality risk. Patients undergoing liver transplant and with spontaneous recovery required longer ICU and hospital stay. At median follow-up of 3.4 (2.6 - 5.5) years, all spontaneous survivors and transplanted patients are well with normal liver function. Conclusions: Early transfer to a specialized center, pre-emptive close monitoring, and intensive care and organ support with ventilation, CVVHDF, plasmapheresis and others may maximize their chances of spontaneous recovery, allow accurate prognostication and a timely liver transplant.

Abstract:Objective: To determine the risk factors and outcome of acute poisoning in children at a tertiary care hospital of Karachi. Study design: Cross sectional study Place and duration of study: Department of Emergency Medicine, Dr Ziauddin Hospital, Karachi, Pakistan. From January 2019 to January 2021 Methodology: The patients (n=136) were selected of either gender with the precise age group of twelve years. All the patients selected were meeting the inclusion criteria and enrolled with a history of poisoning exposure within 24 hours of arriving at the hospital's emergency department. The name of the toxin, the time since ingestion of the toxic substance, factors prompting coincidental harming like age, ill-advised capacity of hurtful specialists, for example, lamp oil and blanch in soda bottles, simple access of kids to meds utilized by different individuals from the family, mother's schooling, working moms, financial status were completely assembled from the guardians or orderlies as referenced in the Performa. In general, people were poisoned primarily from within their own homes. Non-accidental poisoning cases were omitted. SPSS version 22 was used to analyze the data. Results: Out of 136 patient enrolled in this study mean age was 5.2+2.9 years, 77(56.6%) were male and 59(43.4%) were female and mean time of presentation was 2.3+1.7 hours. Most common risk factor was unsafe storage of chemicals (n=98) (72.1%), followed by education level of mother (n=)65 (47.8%), inadequate supervision of child (n=60) (44.1%), non authorative parenting (n=51) (37.5%), working mothers (n=41) (30.1%), lack of family support (n=30) (22.1%), developmental delay (n=11) (8.1%) and poisonous plant in home garden (n=6) (4.4%). Outcome mainly depends upon type of poisoning and time interval between poisoning and presentation to the hospital, 26 (19.11%) were discharged safely from ER, while 45 (33.08%) admit in wards and 48 (35.29%) were admitted in Intensive Care and 17 (12.5%) were expired in ER. Conclusion: In this study we have concluded that number of accidental poisoning are often occur in toddler and school going children. The major risk factor of acute poisoning found was unsafe storage or easy accessibility of house hold chemicals, second one was low level of education of parents, than non-authorative parenting, and working mothers and lack of family support. The outcome mainly depend upon time interval from ingestion of poison to presentation at hospital and type poisonous agent taken, that can lead to the need of specialized care, prolong hospital stay and sometimes results in death of a child.

Abstract:Paraphenylenediamine (PPD) is a chemical product used in hair dye preparations and temporary tattoos to fasten the dyeing process and intensify its results. PPD can evoke local and/or systemic toxic effects either when utilized topically, inhaled or ingested. The aim of this study was to evaluate the toxic effects of PPD on liver of adult female albino rats. Thirty-two adult female albino rats were divided into 3 groups. Group I (control group) 16 rats: which subdivided into 2 equal subgroups: Subgroup IA (negative control) 8 rats: received only regular diet and water, Subgroup IB (vehicle control) 8 rats: received 1 ml distilled water once daily by oral gavage, 5 days/week for 8 weeks. Group II (PPD treated group) 8 rats: Each rat received 10 mg/kg B.W of PPD in 1 ml distilled water once daily for 8 weeks by oral gavage. Group III (Follow up group) 8 rats: Each rat received 10 mg/kg B.W of PPD in 1 ml distilled water once daily for 8 weeks by oral gavage, followed by two weeks without treatment. After 8 weeks in group I & group II and after 2 weeks of follow up in group III, rats were subjected to blood sample collection for estimating AST, ALT & ALP levels. Then livers were dissected out and divided into three parts. The first part was used for estimating oxidative stress markers MDA & GSH. Second part was subjected for histopathological examination under light microscope. The third part was examined by comet assay to determine DNA damage. The results revealed that PPD induced a significant increase in (AST, ALT & ALP), hepatic MDA and a significant decrease in hepatic GSH. It also induced several histopathological alterations and significant DNA damage in liver cells, and cessation of treatment for 2 weeks ameliorates its damaging effects.

However, with our search for reports about acute poisoning, there was no data regarding a change in trends and outcomes of acute poisoning in Ethiopia and its districts. Objective: This study was to determine the burden of poison-related health hazards, identify poisoning substances, and clinical outcomes of poisoning cases admitted to Felege Hiwot Comprehensive Specialized Hospital- Medical intensive care unit (FHCSH-MICU), Bahir Dar, Ethiopia. Methods: A retrospective hospital-based study was conducted using patient data archived from March 2019 to February 2020 at FHCSH-MICU. Collected data were analyzed using Statistical Package for Social Sciences (SPSS) software version 26 and descriptive statistics such as measures of central tendency and proportions were computed. The results were reported as text narrations, figures and tables. Results: A total 121 poisoned patients were identified from the patient registration log books. Among the total FHCSH-MICU cases registered, acute poisoning accounted for 17.0%. Of 121 poison cases, 118 (98.3%) were due to suicide attempts with the female to male ratio being 1.5:1. The majority, 65 (54.0%), of cases were in the range of 21– 30 years age. Nearly two-thirds, 75 (62.0%), of the poisoning cases died, accounting for 21.6% of all deaths in the FHCSH-MICU. The most common poisoning substance was aluminum phosphide, 92 (76%). Conclusion and Recommendations: Aluminum phosphide became the leading poisoning substance in the study area. Nearly two-thirds of the poisoning cases died. Hence, aluminum phosphide must be placed in secured and inaccessible places and it should be replaced by less poisonous rodenticides. Health education and awareness creation about this lethal toxin are strongly advisable. A common type of poisoning substance was aluminum phosphide. Aluminum phosphide is used to kill rodents and pests to protect stored grains by fumigation. This fumigant was commonly named Phostoxin, and its formulation was dark gray tablets. Each tablet was 3 gm in weight with a composition of aluminum phosphide (56%) and aluminum carbamate (44%). 1,4 Its commercial formulation with 55% to 75% active ingredients were solid tablets. Pellets of solid ALP react with water and form highly toxic phosphine gas. The rate of phosphine gas formation depends on humidity and ambient temperature. 3,4 The other types of poisoning agents were organophosphates used as pesticides and insecticides. They are easily available in marketplaces without appropriate controlling and monitoring systems. Organophosphate poisoning is a health-related concern in underdeveloped areas, with high morbidity and mortality. 3,4 Poison affects most organ systems. Its effect on the nervous system is a loss of consciousness; on the respiratory system, it decreases reflexes that protect airways and aspirating from the gastro-intestinal tract into the airways and an absence ofrespiration derives from inadequate ventilation, resulting in respiratory failure. Massive aspiration of gastric contents into the lungs with chemical pneumonitis and pulmonary damage is a major cause of death. A retrospective 7-years study of aluminum phosphide poisoning in Tehran: opportunities for prevention

Abstract:The objective of this study was to survey aluminum phosphide (AIP) poisoning in a referral poisoning hospital in Tehran servicing an estimation of 10,000,000 populations. Records of all patients admitted and hospitalized during a period of 7 years from January 2000 to January 2007 were collected and analyzed according to gender, age, cause of intoxication, amount of AIP consumed, route of exposure, time between exposure and onset of treatment, signs and symptoms of intoxication at admission, therapeutic intervention, laboratory tests, and outcome. During the studied years, 471 patients were admitted to the hospital with AIP poisoning; 50% of them were men. The overall case fatality ratio was 31%. The mean age was 27.1 years, and most of the patients were between 20 and 40 years old. Self-poisoning was observed in 93% of cases. The average ingested dose was 5.1 g, and most of the patients (73%) consumed 1-3 tablets of AIP. A wide range of symptoms and signs was seen on admission, but the most common one was cardiovascular manifestations (78.12%). The majority (65%) of patients were from Tehran. Poisoning in spring and winter (34% and 24%, respectively) was more common than other seasons. Gastric decontamination with potassium permanganate, and administration of calcium gluconate, magnesium sulfate, sodium bicarbonate, and charcoal were considered for most of the patients. Mean arterial blood pH was 7.23 and bicarbonate concentration was 12.7 mEq/L. One-hundred percent of patients with blood pH <7 died and 100% of patients with blood pH >or= 7.35 survived. Electrocardiogram (EKG) abnormalities were noted in 65.6% of cases. There was a significant difference between survival and non-survival according to pH, HCO(3) concentration, and EKG abnormality. Even without an increase in resources, there appears to be significant opportunities for reducing mortality by better medical management and further restrictions on the AIP tablets usage. Arterial blood pH seems to be a prognostic factor for the outcome of AIP-poisoned patients.
Poisoning cases and their management in Amhara National Regional State, Ethiopia: Hospital-based prospective study

Abstract:Background: Poisoning is a significant public health problem globally. Ethiopia is a low-income country undergoing technological and social change that may increase access to drugs and chemicals, potentially increasing the incidence of poisoning. This study describes the epidemiology of hospital admissions due to poisoning in a region of Ethiopia. Methods: An institution based prospective observational study was employed, as a study design, in selected hospitals of the region from January to December 2018. Results: Of 442 poisoning cases, 78 (17.6%) died. Almost all poisoning cases were intentional self-poisonings. The most frequent poisonings were organophosphate compounds, 145 (32.8%), and metal phosphides (majorly aluminum phosphide), 115 (26.0%). The ingested poison was most frequently accessed from the patients' homes, 243 (55.0%), followed by purchases from local shops, 159 (36%). The median duration of admission was 24 hours. Of all the cases, 23 (5.2%) were admitted to intensive care units (ICU) requiring mechanical ventilation. Most of the cases admitted to the ICU were aluminum phosphide-poisoned patients. The majority of deaths (43 of 78) were due to metal phosphides. From the multivariate logistic regression analysis, altered level of consciousness on hospital arrival, metal phosphide poisoning, and no laboratory result as a part of the diagnosis process or investigation of the extent of toxicity were found to be significantly associated with the likelihood of poor treatment outcome. Conclusion: The majority of the poisoning cases were females. The most common reasons for the intent of self-poisoning were dispute-related, mainly family disharmonies, followed by psychiatric conditions. The poisoning agents were mostly obtained from households. Organophosphate compounds and metal phosphides were the first and the second most frequently encountered poisoning agents, respectively, and it was noted that the later ones were responsible for most of the fatal cases. Of the pharmacologic interventions, atropine was the only agent regarded as an antidote. The most commonly employed agent for supportive treatment was cimetidine followed by maintenance fluids, while gastric lavage was the only GI decontamination method used among others. The fatality rate of poisoning in this study was found to be much higher than in other similar studies. Impaired consciousness upon hospital arrival, metal phosphide poisoning, and no involvement of laboratory investigation were found to significantly associate with the likelihood of death. Generally, the results dictate the need for the design and implementation of strategies to create awareness, prevent, and manage poisoning incidences in the community.

In medullary thymocytes, the percentage of rosette formation increased significantly after neuraminidase treatment. Some parts of receptors for rosette formation in thymocytes might be masked by sialic acid in the course of maturation.

Abstract:Initial studies suggested that major histocompatibility complex class I-restricted viral epitopes could be predicted by the presence of particular residues termed anchors. However, recent studies showed that nonanchor positions of the epitopes are also significant for class I binding and recognition by cytotoxic T lymphocytes (CTLs). We investigated if changing nonanchor amino acids could increase class I affinity, complex stability, and T-cell recognition of a natural viral epitope. This concept was tested by using the HLA-A 0201-restricted human immunodeficiency virus type 1 epitope from reverse transcriptase (pol). Position 1 (P1) amino acid substitutions were emphasized because P1 alterations may not alter the T-cell receptor interaction. The peptide with the P1 substitution of tyrosine for isoleucine (I1Y) showed a binding affinity for HLA-A 0201 similar to that of the wild-type pol peptide in a cell lysate assembly assay. Surprisingly, I1Y significantly increased the HLA-A 0201-peptide complex stability at the cell surface. I1Y sensitized HLA-A 0201-expressing target cells for wild-type pol-specific CTL lysis as well as wild-type pol. Peripheral blood lymphocytes from three HLA-A2 HIV-seropositive individuals were stimulated in vitro with I1Y and wild-type pol. I1Y stimulated a higher wild-type pol-specific CTL response than wild-type pol in all three donors. Thus, I1Y may be an "improved" epitope for use as a CTL-based human immunodeficiency virus vaccine component. The design of improved epitopes has important ramifications for prophylaxis and therapeutic vaccine development.

Abstract:Background: Every year, 3 million cases of pesticide, mainly O.P. poisoning, occurs all over the world. Therefore, statistics regarding O.P. poisoning in a specific region will aid in the early diagnosis and treatment of cases. The main objective of this research project is to find out the pattern of O.P. poisoning cases at the tertiary care center. Methods: We conducted a retrospective cross-sectional study, in which we analyzed data from 40 cases of O.P. poisoning, including patient information regarding gender, age, time of ingestion, type of poison, mode of poisoning, any first aid received, treatment given, duration of hospital stay and outcome of the treatment. Results: There were 16 females (40%) and 24 males (60%). The most common age group was 21 to 40 years, with 19 participants (47.50%). The majority of the participants were from the rural area, 38 cases (95%), while in 36 cases, the poisoning was suicidal (90%). 28 participants (70%) received some form of first aid. In addition, 21 of the participants received Atropine (52.50%), 13 received only gastric lavage (32.50%), 5 received both Atropine and Pralidoxime (12.50%), while one patient only received Pralidoxime (2.5%) as treatment. I.C.U. admission was required in 39 participants (97.50%), and ventilatory support was required in 2 participants (5%). Our study observed 4 deaths (10%) amongst our participants. The multivariate analysis of deaths in our study showed that specifically not receiving any first aid (p = 0.039) and requirement of the ventilator (p<0.001) were seen to have a significant association with deaths in our study. Conclusion: O.P. poisoning is the commonest cause of suicidal deaths in developing countries. Creating awareness regarding O.P. poisoning and legislative action on using hazardous poisons will help reduce the deaths.

Abstract:Introduction: Poison is any substance that obstructs with ordinary body functions and is capable of affecting adverse effects in living organisms. Self-poisoning from organophosphorus [OP] pesticides is a significant clinical issue in rural areas of developing countries and is responsible for an estimated 2,00,000 fatalities annually. Suspicion recognizable clinical symptoms the odour of pesticides or solvents and decreased butyrylcholinesterase activity in the blood are used for diagnosis. Case Study: A 30 years old female patient was admitted in a Tertiary Care Hospital, with an alleged history of dimethoate compound poisoning. On arrival the patient was drowsy responds to painful stimuli and afebrile. The treatment was begin with Gastric lavage with normal saline Inj.Pralidoxime in an intravenous route and Atropine was also administered intravenously and repeated every 5 minutes until the pupil dilated and Ranitidine was given to prevent ulceration. Discussion: The initial method to treat organophosphorus poisoning is to decontaminate the patient by removing and destroying all clothing and using drying agent such as flour sand or bentonite. The patient's irrational usage of medication therapy causes a worsening of the condition and a 10- to 15-day hospitalization. Conclusion: As per the standard treatment guidelines, atropine should be given as first-line therapy because muscarinic effects are reversed by atropine. By providing this to the patient, you will minimise the severity of their illness minimise down on their stay in the hospital and ultimately save their lives.

Abstract:Background The use of organophosphorus (OP) compounds as pesticides is widespread, particularly in developing nations such as India. These substances are easily accessible and often linked to suicide poisoning incidents, especially among distressed farmers. In India, OP poisoning remains a significant cause of emergency hospital admissions. This study aims to analyze the clinical profile of patients admitted with OP poisoning, focusing on the various clinically significant characteristics and aspects associated with these cases. Methods Throughout the course of two years (from June 2022 to May 2024), five hospitals with critical care units participated in this prospective observational study. These hospitals referred patients to the Prathima Institute of Medical Sciences, which served as the central facility for the study. The study focused on individuals who arrived at the emergency room within 24 hours of acute exposure to organophosphate pesticides. A total of 150 patients meeting the inclusion criteria were enrolled. Data collection included a comprehensive clinical examination, detailed patient history, and monitoring of the response to intravenous atropine. The severity of the poisoning was evaluated using the Peradeniya Organophosphorus Poisoning (POP) scale, adapted for this study. Statistical analysis was performed using IBM SPSS Statistics for Windows, Version 23 (Released 2015; IBM Corp., Armonk, New York), with descriptive statistics used to summarize the categorical and continuous variables. Results The results of the study demonstrated a notable relationship between the arrival time at the health facility and mortality rates among patients with OP poisoning. Patients who arrived within three hours had a mortality rate of 13.2% (five out of 38 cases), while those arriving between three and six hours had a slightly higher rate of 14.3% (10 out of 70 cases). The mortality rate sharply increased to 28.6% (12 out of 42 cases) for patients who arrived after six hours. Among the OP compounds, monocrotophos showed the highest mortality rate at 80% (eight out of 10 cases), followed by dimethoate with 42.9% (12 out of 28 cases) and dichlorvos with 62.5% (five out of eight cases). The overall mortality rate in the study was 22.7% (34 out of 150 cases). Conclusions Poisoning with OP has primarily impacted young, productive men. Considerable mortality results from a disease condition that is more severe at presentation. It is advised to selectively outlaw the riskier substances that account for a larger percentage of fatalities.
