Abstract:Carcinosarcoma of the breast is an extremely rare and clinically aggressive tumor possessing carcinomatous components and malignant mesenchymal elements, with few cases reported in the literature. The prognosis of breast carcinosarcoma is poor when compared with more common types of breast cancer, such as ductal or lobular carcinoma. Furthermore, carcinosarcoma presents a diagnostic and therapeutic challenge due to the diversity in histology pattern and the lack of consensus regarding diagnostic criteria and effective systemic management. A 51-year-old female presented to the Department of Medical Oncology of the Second Affiliated Hospital of Zhejiang University (Hangzhou, China) with the complaint of lumbago and physical examination revealed a small mass in the right breast near the areola. Mammography and magnetic resonance imaging revealed two small masses, which were <1 cm in diameter. Positron emission tomography/computed tomography showed a maximum standardized uptake volume of 3.46 for one soft tissue nodule and multiple bones that were exhibiting radioactive enhancement. The patient was diagnosed with a breast carcinosarcoma with bone metastases and breast intraductal carcinoma. The response to chemotherapy was poor and the patient succumbed to the disease within one month of diagnosis.
A Clinicopathologic and Prognostic Analysis of 115 Cases of Endometrial Carcinoma

Abstract:Objective To explore the clinicopathologic character for endometrial carcinoma and prognostic factors. Methods One hundred and fifteen patients with endometrial carcinoma accepted primary surgical treatment from June 1997 to June 1999 were analyzed retrospectively. Results The average age of the patients was 56.7 years. There were 90 cases getting the follow up results. The 5 year survival rates of 90 patients were 97.6% and 90.9% in surgical-pathologic stage ⅠandⅡ61.9% and 0 in stage Ⅲ and Ⅳ.The differences of the 5 year survival rates between the stage Ⅰ,Ⅱand stage Ⅲ,Ⅳ were significant. The rate of extent of pelvic and abdominal cavity lymph in pathological grade Ⅰwas 3.1% (1/32),grade Ⅱ 5.6%(3/54),grade Ⅲ 33.3%(6/20) (P0.025 ). Conclusion The surgical-pathologic staging of endometrial carcinoma is able to assess prognosis objectively. The pathological grade is related to the extent of pelvic and abdominal cavity lymph metastasis.
Clinicopathologic features and surgical treatment prognosis of esophageal carcinosarcoma

Abstract:Background: Carcinosarcoma is a rare esophageal tumor, accounting for approximately 0.27-2.8% of malignant esophageal tumors. This study aims to investigate the clinical pathological characteristics, surgical treatment outcomes, and analysis of prognostic factors in esophageal carcinosarcoma (ECS). Methods: Clinical data from sixteen patients diagnosed with esophageal sarcomatoid carcinoma who underwent surgical interventions were retrospectively analyzed. Clinical and pathological features, treatment modalities, and postoperative outcomes were systematically examined. Results: Out of the 1261 patients who underwent surgical treatment for esophageal cancer, 16 cases were pathologically confirmed as carcinosarcoma. Among them, two underwent neoadjuvant chemotherapy, six received postoperative chemotherapy. Carcinosarcomas predominantly occurred in the middle (43.75%) and lower (50%) segments of the esophagus. Among the 16 cases, 10 presented as polypoid, 4 as ulcerative, and 2 as medullary types. Microscopic examination revealed coexistence and transitional transitions between sarcomatous and carcinoma components. Pathological staging showed 5 cases in stage T1, 2 in stage T2, and 9 in stage T3, with lymph node metastasis observed in 8 cases (50%). TNM staging revealed 2 cases in stage I, 9 in stage II, and 5 in stage III. The overall 1, 3, and 5-year survival rates were 86.67%, 62.5%, and 57.14%, respectively. Univariate analysis indicated that pathological N staging influenced survival rates, while multivariate analysis demonstrated that pathological N staging was an independent prognostic factor. Conclusions: Carcinosarcoma is a rare esophageal tumor, accounting for approximately 0.27-2.8% of malignant esophageal tumors. Histologically, the biphasic pattern is a crucial diagnostic feature, although the carcinomatous component may not always be evident, especially in limited biopsies, leading to potential misclassification as pure sarcoma or squamous cell carcinoma. Despite its large volume and cellular atypia, carcinosarcoma carries a favorable prognosis. Complete surgical resection of the tumor and regional lymph node dissection is the preferred treatment approach for esophageal carcinosarcoma.
Clinicopathological characteristics and survival outcomes in breast carcinosarcoma: A SEER population-based study

Abstract:Objectives: Carcinosarcoma of the breast is a rare disease. Its clinicopathological features and prognosis are not well defined. The aim of this study was to compare the clinicopathological features and clinical outcome between breast carcinosarcoma and breast invasive ductal carcinoma (IDC). Materials and methods: Patients with breast carcinosarcoma and breast IDC were identified through the Surveillance, Epidemiology, and End Results (SEER) database from 2010 to 2015. Then a comparison was conducted between these two groups. Propensity score matching (PSM) was performed to balance the effects of baseline clinicopathological differences. The Cox proportional hazard model was used to identify potential prognostic factors of breast carcinosarcoma. Results: In total, we identified 63 patients with breast carcinosarcoma and 200,596 cases with breast IDC. Comparing with IDC, breast carcinosarcoma was significantly correlated with higher grading, higher staging, larger tumor size, lower lymph node involvement, and a higher proportion of triple negative breast cancer (TNBC), suggesting a significantly worse clinical outcome. After adjusting for the uneven clinicopathological variables with PSM, significant differences were still observed between these two histology types. Subgroup analysis further showed that carcinosarcoma-TNBC has an inferior clinical outcome compared with IDC-TNBC. Finally, we identified independent prognostic factors, namely, stage, tumor size, and distant metastasis. Conclusion: It is concluded that breast carcinosarcoma has distinct clinicopathological features and a significantly worse clinical outcome than common IDC. (C) 2019 The Authors. Carcinosarcoma of the pancreas: comprehensive clinicopathological and molecular characterization

Abstract:BACKGROUND: Carcinosarcoma of pancreas is a rare subtype of pancreatic cancer. The aim of this study was to comprehensively elaborate the clinicopathological and molecular features of this rare malignancy.METHODS: Patients who were diagnosed with carcinosarcoma of the pancreas were retrospectively identified from pathology database of a single institution between 2012 and 2018.RESULTS: A total of nine patients were identified. Pathological examination of tumor tissues from included patients showed coexisting carcinomatous and sarcomatous components. These two components were distinguished by mutually exclusive expression of cytokeratin and vimentin. The sarcomatous tissue exhibited more extensive proliferation, as revealed by Ki67 staining, and necrosis compared with the carcinomatous counterpart. Genomic analysis of tumor tissues for two patients demonstrated hotspot mutation at KRAS and TP53. Carcinomatous and sarcomatous components were separately obtained via laser captured microdissection in one patient, and mutations of driving genes were highly concordant between them. Besides, immunostaining of frequently-altered tumor suppressor genes for these two components suggested consistent expression patterns. The median overall survival for six patients with adequate follow-up was 14 months.CONCLUSION: Carcinosarcoma of the pancreas represent a rare malignancy with distinct histological characteristics. Genomic and molecular analysis suggested monoclonal origin of carcinomatous and sarcomatous components.

Abstract:Purpose To investigate the clinical and pathological features of breast carcinosarcoma and its differential diagnosis. Methods The histopathology and immunophenotype of a breast carcinosarcoma was studied and the relative literature were reviewed in order to understand the characteristics of the tumor. Results Microscopically, the lesion was relatively well circumscribed. The tumor was predominantly composed of chondrosarcoma with invasive ductal carcinoma at the periphery of the tumor. The chondrosarcoma elements showed vimentin(+), S-100 protein(+), ER(-), PR(-), c-erbB-2(-), and CK(AE1/AE3)(-). The carcinoma cells showed CK (AE1/AE3)(+), ER(+), PR(+), c-erbB2(+), vimentin(-), and S-100 protein (-). Conclusions Primary carcinosarcoma of the breast is a rare tumor. More attention should be paid in the daily practice in order to differentiate this tumor from malignant phyllodes tumor and primary soft tissue sarcoma of the breast.

Abstract:BACKGROUND Sarcomatoid carcinoma of the pancreas (SCP) is a rare and aggressive epithelial tumor that has both epithelial and mesenchymal features. It is characterized by sarcomatous elements with evidence of epithelial differentiation. And the term “sarcomatoid carcinoma” is often confused with “carcinosarcoma”. CASE SUMMARY We present a case of SCP with lymph node metastasis in a 59-year-old male patient. He had experienced darkening of the urine, scleral icterus, and fatigue for 4 weeks. Computed tomography and magnetic resonance imaging revealed a mass in the pancreatic head, and laboratory tests revealed elevated serum bilirubin levels. The patient underwent pancreaticoduodenectomy after biliary decompression. Histologically, spindle cells with marked nuclear atypia and brisk mitotic activity arranged in a storiform or fascicular pattern were present in the bulk of the tumor. Immunohistochemical analysis found that the spindle cells exhibited strong diffuse positivity for epithelial markers, indicative of epithelial differentiation. Accordingly, the pathologic diagnosis of the pancreatic neoplasm was SCP. CONCLUSION Although sarcomatoid carcinomas and carcinosarcomas have different pathologic features, both have epithelial origin.
Metaplastic Carcinoma (Carcinosarcoma) Of The Breast: A Case Report

Abstract:Metaplastic breast carcinoma (MBC) is a rare malignancy and accounting for less than 1% of all breast carcinomas, which is often composed of epithelial and mesenchymal components. The present study describes a case of a 43-year-old female patient with MBC, which is diagnosed according to the combination of the gross examination, ultrasonic, mammographic, magnetic resonance (MR), breast-specific gamma-imaging (BSGI) and sentinel lymph node biopsy. After undergoing lumpectomy of right breast, the patient received six courses of adjuvant chemotherapy and now undergoing radiotherapy, without any sign of recurrence or metastasis. MBC may have poorer prognosis compared with invasive breast cancer (IBC) but there is no standardized therapeutic strategy for it. It is urgently needed that large studies are conducted to investigate different treatment.

Abstract:Background and purpose:Pure epithelial breast metaplastic carcinoma is a rare disease.In this study,our purpose was to evaluate clinical and pathological features of the pure epithelial subtype of breast metaplastic carcinoma.Methods:Nine cases with pure epithelial subtype of breast metaplastic carcinoma treated in our hospital were retrospectively analyzed.Results:All patients were female with a median age of 50 years and underwent radical or modified radical mastectomy.Six cases were diagnosed as spindle cell carcinomas and three cases were squamous cell carcinoma.Estrogen receptor(ER)or/and progesterone receptor(PR)were expressed only in 2 cases.Two patients showed lymph node metastasis.After follow-up(median 25 months),1 patient developed local recurrence,and 3 patients had distant metastases,2 patients died of widespread metastases and 1 patient was still alive with lung metastasis.Conclusions:The spindle subtype is the most common form of pure epithelial breast metaplastic carcinoma that can express epithelial and mesenchymal markers simultaneously,but usually lack ER/PR expression.These patients are characterized by low risk of node metastasis and high risk of local recurrence after local excision,and lung is common site of metastasis.Simple mastectomy plus sentinel node biopsy may be the optimal choice of the treatment.

Abstract:Carcinosarcoma is a rare malignant neoplasm, consisting of both epithelial and mesenchymal component. Primary gastric carcinosarcoma is rare and poorly understood. We reported clinicopathologic features of 4 cases and analysis of 76 cases published in the literature. Clinical symptoms were nonspecific with epigastric pain, weight loss, and melena, as the most common complaints. The prognosis of patients was dismal with high mortality. The tumor commonly occurred in the upper stomach in Chinese patients, whereas, it was more prevalent at the lower stomach in Japanese and other populations. The two malignant components of this rare cancer showed considerable histological heterogeneity with a wide range of differentiation. We propose that carcinosarcomas be divided in two main subtypes: conventional carcinosarcoma and carcinosarcoma, not-otherwise-specified (NOS). Such distinction may provide useful information for targeted treatment of various sarcomatous components of this tumor. Immunohistochemistry should be routinely applied in the diagnosis of this rare tumor.

J Strength Cond Res 24(7): 1782-1789, 2010-The objective of this study was to investigate the acute effects of performing traditional set (TS) vs. complex set (CS) agonist-antagonist training over 3 consecutive sets, on bench press throw (BPT) throw height (TH), peak velocity (PV), peak power (PP), bench pull volume load (VL), and electromyographic (EMG) activity. Eighteen trained men performed 2 testing protocols: TS comprising 3 sets of Bpull followed by 3 sets of BPT performed in approximately 20 minutes or CS comprising 3 sets of both Bpull and BPT performed in an alternating manner in approximately 10 minutes. Throw height, PV, PP, and EMG activity were not different within, or between, the 2 conditions. Bench pull VL decreased significantly from set 1 to sets 2 and 3, under both conditions. Decreases from set 1 to set 2 were 14.55 ± 26.11 and 9.07 ± 13.89% and from set 1 to set 3 were 16.87 ± 29.90 and 14.17 ± 18.37% under CS and TS, respectively. There was no difference in VL per set, or session, between the conditions. Although there was no augmentation of the power measures, CS was determined to have approximately twice the efficiency (ouput/time) as compared to TS. Efficiency calculations for VL, TH, PV, and PP are 103.47 kg·min−1, 26.25 cm·min−1, 1.98 m·s−1·min−1, 890.39 W·min−1 under CS and 54.71 kg·min−1, 13.02 cm·min−1, 0.99 m·s−1·min−1, 459.28 W·min−1 under TS. Comparison of EMG activity between the protocols suggests the level of neuromuscular fatigue did not differ under the 2 conditions. Complex set training would appear to be an effective method of exercise with respect to efficiency and the maintenance of TH, PV, PP, and VL.

Abstract:Esophageal carcinosarcoma is a rare cancer type, which accounts for approximately 3% in esophageal neoplasms with low 5-year overall survival. Carcinosarcoma is biphasic in nature which consists of both carcinomatous and sarcomatous elements. Of these sarcomatous components, fibrosarcoma is the most common type and others include leiomyosarcoma, rhabdomyosarcoma, chondrosarcoma, osteosarcoma, liposarcoma, chondrosarcoma and so on. Since the first description of this neoplasm by Chapman in 1877, esophageal carcinosarcoma cases have been reported, but its pathogenesis is still unclear. Here, we report a case with complete follow-up data and integrated high-throughput data, which could contribute to elucidating the pathophysiological mechanisms of diseases accurately and provide novel therapeutic strategies to the clinical research. To the best of our knowledge, this is the first reported case to characterize its characteristics at multi-omics levels.
Epidemiology, site-specific characteristics and survival of carcinosarcoma: a retrospective study based on SEER database

Abstract:Objectives Carcinosarcoma (CS) is a rare and biphasic malignancy characterised by a highly invasive biological nature and poor prognosis. This study explored the epidemiology, site-specific characteristics and survival outcome of CS. Design We conducted a retrospective study in the Surveillance, Epidemiology and End Results (SEER) database (1975–2018) for primary CS. Setting and participants SEER database includes publicly available information from regional and state cancer registries in the US centres. A total of 5042 CS patients were identified. We selected the top five anatomic CS (uterus, double adnexa, lung, bladder and breast) patients for further analysis. Primary outcome measures Incidence was estimated by geographical region, age, sex, race, stage and primary site. Trends were calculated using joinpoint regression. The cancer-specific survival (CSS) rate and initial treatment were summarised. Results Nearly 80% of CS occurred in the uterus and double adnexa, followed by lung, bladder and breast. The elderly and black population presented the highest age-adjusted rate of CS. The rates of distant metastasis in CS progressively increased from 1989 to 2018. Atlanta was the area with the highest incidence at 0.7 per 100 000. Pulmonary and bladder CS more frequently occurred in men and were diagnosed with regional stage. Distant metastasis was mostly found in ovary/fallopian tube CS. Radiotherapy was more commonly applied in uterine CS, while adnexa CS cases were more likely to receive chemotherapy. Multiple treatments were more used in breast CS. Pulmonary CS seemed to suffer worse CSS (median: 9.92 months), for which radiotherapy might not provide survival benefits (HR 0.60, 95% CI 0.42 to 0.86). Compared with the common histological types in each site, CS had the shortest survival. Conclusions CS has unique clinical features in each primary site. Substantial prognosis variances exist based on tumour locations. The aggressive course is the common feature in CS at all sites.

Uterine carcinosarcoma:a clinicopathological analysis of 41 cases

Abstract:Objective:To investigate the prognostic effect of various clinicopathological features of uterine carcinosarcoma. Methods:Medical records of 41 patients with uterine carci-nosarcoma between Jan. 2000 to June 2011 in Obstetrics and Gynecology Hospital of Fudan U-niversity were retrieved. Kaplan-Meier and Cox Regress were used to analyze the prognostic fac-tors,as well as the relationship between the prognosis and carcinomatous or sarcomatous ele-ments. Results:Three-year overall survival (OS) and disease free survival (DFS) were 57. 5%and 52. 5%,respectively. Advanced Federation International Gynecology and Obstetrics ( FI-GO) stage(Ⅲ~Ⅳ),deep myometrial invasion,cervical stromal involvement and lymph node metastasis were related to worse outcomes, while lymph node metastasis was an independent prognostic factor. Immunohistochemical expression of p53 in the epithelial and mesenchymal components was similar. A type of non-endometrioid carcinomatous element may indicate worse outcome compared with endometrioid carcinomatous element(P=0. 074). Nevertheless,the sar-comatous component seemed to bear no relationship with the prognosis. Conclusion:Patients with advanced FIGO stage,deep myometrial invasion,cervical stromal involvement and lymph node metastasis have worse prognosis. The lymph node metastasis is an independent prognostic factor. Carcinosarcomas are monoclonal tumors,and the carcinomatous component might be the driving force.
A Retrospective Study of Clinicopathological Characteristics, Treatment, and Outcomes of Chinese Uterine and Ovarian Carcinosarcoma Patients

Abstract:This study aimed to determine the clinicopathological and prognostic factors, and to assess the impact of cytoreduction to complete gross resection in Chinese uterine and ovarian carcinosarcoma patients. Materials and Methods: The authors reviewed 86 consecutive patients treated for uterine (n= 60) or ovarian (n= 26) carcinosarcomas between February 2006 and August 2013 at Fudan University Shanghai Cancer Center. Results: The median follow- up time was 24.0 months [ interquartile range (IQR): 12.4 to 48.7]. The median age was 59 years (IQR: 53 to 66). The three- and five- year overall survival rates were 60.1% and 42.8%, respectively. Extent of tumor dissemination, mesenchymal component, and adjuvant therapy were predictive of overall survival. Among advanced carcinosarcoma patients, complete gross resection (R-0) had a three- year overall survival rate of 52.5%, vs. 38.9% and 20.8% in patients with gross \residual disease <= 1cm and > 1cm (p = 0.004). On multivariate analysis, only heterologous mesenchymal component (HR= 4.8; 95% CI, 1.4-16.7; p = 0.015) and gross residual disease (HR= 2.6, 95% CI: 1.2- 5.9; p = 0.019) were predictors of increased mortality. Conclusions: Heterologous tumor and incomplete gross resection were significantly predictive of a poor overall survival in carcinosarcoma patients. We recommend cytoreduction to R0 for upfront treatment in CS patients.

Abstract:Endometrial carcinosarcoma is a rare and aggressive high-grade endometrial carcinoma with secondary sarcomatous trans-differentiation (conversion theory). The clinical presentation and diagnostic work-up roughly align with those of the more common endometrioid counterpart, although endometrial carcinosarcoma is more frequently diagnosed at an advanced stage. Endometrial carcinosarcoma is not a single entity but encompasses different histological subtypes, depending on the type of carcinomatous and sarcomatous elements. The majority of endometrial carcinosarcomas are characterized by p53 abnormalities. The proportion of POLE and microsatellite instablity-high (MSI-H) is directly related to the epithelial component, being approximately 25% and 3% in endometrioid and non-endometrioid components.The management of non-metastatic disease is based on a multimodal approach with optimal surgery followed by (concomitant or sequential) chemotherapy and radiotherapy, even for early stages. Palliative chemotherapy is recommended in the metastatic or recurrent setting, with carboplatin/paclitaxel doublet being the first-line regimen. Although the introduction of immunotherapy plus/minus a tyrosine kinase inhibitor shifted the paradigm of treatment of patients with recurrent endometrial cancer, patients with endometrial carcinosarcoma were excluded from most studies evaluating single-agent immunotherapy or the combination. However, the US Food and Drug Administration (FDA) and the European Medicines Agency (EMA) approved the use of pembrolizumab and lenvatinib in endometrial cancer (all histotypes) after progression on chemotherapy and single-agent immunotherapy in MSI-H cancers. In the era of precision medicine, emerging knowledge on molecular endometrial carcinosarcoma is opening new promising therapeutic options for more personalized treatment. The present review outlines state-of-the-art knowledge and future directions for patients with endometrial carcinosarcoma.
Carcinosarcoma of the Stomach with Alpha-Fetoprotein-producing Hepatoid Adenocarcinoma: an Unexpected Combination of Two Rare Subtypes of Gastric Cancer in One Tumor

Abstract:Carcinosarcoma is a rare malignant neoplasm comprising both epithelial and mesenchymal components. Hepatoid adenocarcinoma (HAC) is another rare type of cancer. To date, there are only four reported cases of concurrent carcinosarcomas with HAC across all tumor types, all of which were observed in uterine tumors. Here, we report an unusual case of gastric carcinosarcoma associated with alpha-fetoprotein (AFP)-producing HAC in a 76-year-old woman. Upon admission, the patient had an elevated serum AFP concentration (448 µg/L), a necrotic polypoid tumor of the central gastric cardia revealed by endoscopy, and no evidence of distant metastasis indicated by computed tomography (CT). Owing to malignancy indicated by biopsy, the patient underwent proximal subtotal gastrectomy. The resected tumor was composed of both an HAC component and a sarcoma component, microscopically. The sample was positive for AFP, hepatocyte paraffin (Hep-Par) 1, glypican-3, SALL4, CDX2, cytokeratin (CK) (pan), CK18, desmin, and vimentin staining immunohistochemically. In summary, the tumor was diagnosed as carcinosarcoma of the stomach with AFP-producing HAC. To our knowledge, this is the first report of gastric carcinosarcoma with AFP-producing HAC in the English literature describing gastric tumors.
Mesonephric-like Carcinosarcoma of the Uterine Corpus: Clinicopathological, Molecular and Prognostic Characteristics in Comparison With Uterine Mesonephric-like Adenocarcinoma and Conventional Endometrial Carcinosarcoma

Abstract:Background/aim: This study aimed to investigate the clinicopathological, prognostic and molecular characteristics of uterine mesonephric-like carcinosarcoma (MLCS). Patients and methods: We collected clinical, pathological, and genetic information from 12 MLCS patients, and analyzed their differences from mesonephric-like adenocarcinoma (MLA) and conventional endometrial carcinosarcoma (CECS). Results: The epithelial component was exclusively MLA in all MLCS cases. Metastatic and recurrent tumors consisted predominantly or exclusively of MLA in the majority of MLCS cases. Patients with MLCS and MLA presented with more advanced-stage disease than those with CECS. They also exhibited post-treatment recurrence and lung metastases more frequently than CECS. Disease-free survival rates of MLCS and MLA were shorter than those of CECS. Tumor protein 53 gene mutations were detected in four MLCS cases. Conclusion: The predominance or exclusive presence of MLA in metastatic and recurrent tumors highlights the possibility that MLA may determine the clinical outcomes of patients with MLCS. Further studies are required to provide direct molecular evidence of the monoclonal origin of uterine MLCS.
Carcinosarcoma of breast - A chimera among breast neoplasms

Abstract:Malignant phyllodes, along with ductal carcinoma, is known as metaplastic cancer of the breast. This tumor is additionally known as breast carcinosarcoma. Malignant phyllodes in conjunction with ductal carcinoma is a rare finding in routine clinical practice. We describe the case report of a 47-year-old female patient who arrived with a large right breast mass. A core biopsy was performed, and histopathological examination indicated that the tumor was a malignant phyllodes tumor. A positron emission tomography (PET) scan revealed hyper-metabolic and hypo-metabolic tumors with perilesional stranding and satellite nodularity. There were a few metastatic right axillary nodes visible. There was no indication of distant metastases. Due to the presence of both components, a modified radical mastectomy surgery with axillary dissection was undertaken for this patient. Histopathological analysis of paraffin sections revealed ductal carcinoma in situ (DCIS) comedo-epithelial component and spindle-shaped cells with hyper-chromatic oval nuclei and scanty cytoplasm.
Pancreatic carcinosarcoma with the same KRAS gene mutation in both carcinomatous and sarcomatous components: molecular evidence for monoclonal origin of the tumour

Abstract:Aims: To better understand the histogenesis, prognosis and feasible treatment of pancreatic carcinosarcoma, a rare type of neoplasia. Methods and results: We investigated eight additional cases of pancreatic carcinosarcoma at a single institution, including the clinicopathological, immunohistochemical, and KRAS mutation characteristics. We have also reviewed the current literature on this rare type of neoplasia, and summarized the clinicopathological features and feasible treatments. As a result, concordant strong nuclear immunoreactivity for P53 protein and the same type of KRAS gene mutation, c.35G>A (p.G12D) or c.35G>T (p.G12V), were showed in both carcinomatous and sarcomatous components in five of eight cases. Furthermore, we found that the patients treated with surgery plus postoperative chemotherapy had longer survival than those treated with surgery only (P = 0.034 and P = 0.131 for overall survival and disease-free survival, respectively), although Cox regression multivariate analysis indicated that it was not an independent predictor. In addition, we found KRAS mutant allele-specific imbalance in four of our five cases of pancreatic carcinosarcoma, which was associated with advanced disease and a worse prognosis. Conclusions: This is the largest panel of cases of pancreatic carcinosarcoma studied so far, including clinicopathological, immunohistochemical and molecular cytogenetic features. Our findings indicate that the tumour could have been of monoclonal origin, and that the sarcomatous components might have arisen from metaplastic transformation of the carcinomatous components. Our results also suggest that surgery plus POC including gemcitabine may be a good choice for patients with pancreatic carcinosarcoma.
