Abstract:Objective: To show an original technique of a new combined vaginal-laparoscopic lateral suspension in Hysteropexy with cistocele and rectocele. In recent years, changes in attitudes toward sexuality, psychological value of reproductive organs and the desire to preserve fertility have led to a growing interest in uterine-preserving surgery for Pelvic Organ prolapse. Minimally invasive procedures derived from sacrocolpopexy are considered the gold standard in the treatment of apical Pelvic Organ prolapse. However, dissection at the level of the promontory may be challenging, particularly in obese women and when an anatomical variation exists. This may be associated with rare but serious neurological or ureteral morbidity as well as life-threatening vascular injury. Materials and methods: Stepwise demonstration of the technique with narrated video footage. Local institutional review board was consulted, and this study was exempted from approval. Results: Our technique entails 2 times. During the vaginal time, a polypropylene mesh is fixed to the cervical fascia and the 2 extremities are introduced in the abdominal cavity through the Douglas pouch. During the laparoscopic time, a retroperitoneal tunnel is made along the walls of the lateral abdominal walls; thereafter, each of the 2 extremities of the mesh is passed through the omolateral tunnel and "tension-free" suspended to the abdominal wall. Conclusion: Our combined technique may allow a safer approach, reducing the risks of serious complications. Moreover, it leads to a more physiological orientation of the vaginal axis. Further controlled studies are needed to confirm our suggestion.
Effectiveness of laparoscopic pectopexy for pelvic organ prolapse compared with laparoscopic sacrocolpopexy

Abstract:Study Objective To evaluate the clinical benefits of laparoscopic pectopexy versus laparoscopic sacrocolpopexy in women with pelvic organ prolapse (POP). Design Prospective cohort study. Setting A tertiary hospital. Patients We included 203 patients with POP. Interventions Laparoscopic pectopexy or laparoscopic sacrocolpopexy. Measurements and main results Anatomical effectiveness was measured using the POP-quantification (POP-Q) system, both pre- and post-operation. Functional recovery effectiveness was evaluated using complications and recurrence rates within one year. Quality of life (QOL) was assessed by the Pelvic Floor Distress Inventory-20 (PFDI-20) and Incontinence Quality of Life (I-QOL) questionnaires at enrollment and postoperative months 3, 6, and 12. Comparisons between groups were performed using t- test, Chi-square test, and mixed-effects model with repeated measures. The analysis included 203 eligible patients (sacrocolpopexy 101; pectopexy 102). The proportion of robotic-assisted surgeries was lower in the pectopexy group than in the sacrocolpopexy group (15.7% vs. 41.6%, p <0.001). The average operation time of pectopexy was shorter than that of sacrocolpopexy (174.2 vs. 187.7 mins) with a mean difference of 13.5 min (95%CI, 3.9-23.0; p =0.006). Differences of intraoperative blood loss, length of hospital stay and postoperative 7-day complications between groups were not significant. Anatomical successes were obtained in both groups with similar improvement in POP-Q scores. The rate of urinary symptoms recurrence was higher in the pectopexy group (13.7%) than in the sacrocolpexy group (5.0%) at the one-year follow-up (OR, 3.1; 95% CI, 1.1-8.8, p =0.032). The PFDI-20 and I-QOL scores were better improved at postoperative months 3, 6 and 12 for laparoscopic pectopexy than for sacrocolpopexy. Conclusion Laparoscopic pectopexy revealed comparable anatomical success, shorter operation time, and better improvement in QOL scores of prolapse, colorectal-anal and urinary symptoms at 1-year follow-up, possibly being an alternative when sacrocolpopexy is not practicable. However, clinicians should pay more attention to the recurrence of urinary symptoms after pectopexy.

A Randomised Trial on the Effects of the Addition of Uterosacral Ligament Plication to Pectopexy Operation on Anatomical Restoration and Female Sexual Functions

Abstract:Introduction and hypothesis: The objective was to determine whether the addition of uterosacral ligament plication to pectopexy for pelvic organ prolapse increases anatomical improvement and female sexual functioning. Methods: This is a prospective randomised study. Forty patients who underwent laparoscopic pectopexy (LP) and laparoscopic pectopexy with uterosacral ligament plication (LPUSL) were included in the study. A total of 38 patients were evaluated in the study. Patients were assessed by the Pelvic Organ Prolapse Quantification (POP-Q) system and the patients who had symptomatic apical prolapse POP-Q ≥ II were included in the study. Patients were evaluated preoperatively and postoperatively at the 3rd, 6th and 12th month, with respect to anatomical changes as well as sexual function. Results: A statistically significant difference was found when the preoperative C points was compared with the C points postoperatively, at 3rd, 6th and 12th months in both the LP and LPUSL groups (p < 0.001). Additionally, there were statistically significant difference between the LP and LPUSL groups in terms of C and Aa points at the 6th and 12th postoperative months in favour of the LPUSL group (p = 0.007, p = 0.005 respectively). There was also a statistically significant difference when Ba points were compared between the two groups at the 12th postoperative month, in favour of the LPUSL group (p = 0.002). There were no statistically significant differences between the groups with respect to the variables of the Female Sexual Function Index. Additionally, all parameters were significantly better in both groups at the 3rd and 12th months than the preoperative values in post hoc analysis; only desire also had a significant improvement between the 3rd and 12th months in the LPUSL group in post hoc analysis. Conclusion: Pectopexy operation seems to be a successful surgical approach as an alternative to sacrocolpopexy. The addition of uterosacral ligament plication to pectopexy operation improves the anatomical restoration more drastically than with LP on its own. Moreover, either LP or LPUSL has improved the majority of sexual function indices.
Laparoscopic Suture Uterosacral Ligament Hysteropexy for Uterine Prolapse

Abstract:OBJECTIVE:To investigate possibility and effect of laparoscopic suture uterosacral ligament hysteropexy or colpopexy for women with uterine prolapse.METHODS:Thirty-two women with symptomatic uterine prolapse underwent laparoscopic suture uterosacral ligament hysteropexy. At the laparoscopic suture hysteropexy or colpopexy, the pouch of Douglas was closed and the uterosacral ligaments were plicated and reattached to the cervix. All patients were multipara and menopausal with prolapse of anterior wall of vagina. Additionally, 4 patients were with prolapse of posterior wall of vagina, 15 with stress urinary incontinence, and 4 with myomas.RESULTS:All procedures were successfully completed laparoscopically. The mean operating time for the laparoscopic suture hysteropexy or colpopexy alone was (32 +/- 11) min (range 20 approximately 80 min), and the mean blood loss was less than 50 ml. After a mean follow-up of (12 +/- 6) months (range 4 approximately 28 months), 23 women had no symptoms of uterine prolapse and seven had no objective evidence of uterine prolapse. Two women presented recurrence of uterine prolapse 3 months after operation.CONCLUSIONS:The laparoscopy suture hysteropexy or colpopexy is effective and safe in the management of symptomatic uterine prolapse. It may be an appropriate procedure for women with uterine prolapse hoping for uterine preservation.

Abstract:The application of hyperbaric oxygenation (HBO2) has been recommended for correction of neurological injury in severely CO-poisoned patients. However, the mechanisms of HBO2 action on brain mitochondrial function under the circumstances is not yet understood completely. In the present study, the effect of HBO2 on the rat brain after CO exposure was evaluated by measuring the intramitochondrial NADH and its responses to anoxic test or repetitive induction spreading depression (SD) leading to brain activation. A unique monitoring system for bilateral monitoring of brain NADH redox state was used. Rats were exposed to 3000 ppm CO for 30 (group A) or 60 min (C). In groups B and D, after CO exposure, the rats were exposed to HBO2 (3 atm abs for 30 min). Following CO exposure in groups A and C, a definite decrease in the amplitude of the NADH response and significant increase in the number of waves of NADH was noted during induced cortical SD. Anoxic test in these two groups led to a significant decrease of maximum levels of NADH (reduction) at the end of observation. The amplitude, and the number of SD waves and magnitude of NADH deviation during anoxic test in group B after application of HBO2, was not significantly different from the values measured under the initial conditions. However, in group D, tendency of maintenance of the parameter's initial level was weaker or absent. The results obtained indicated that suppression of brain energy metabolism is a characteristic manifestation of CO poisoning in rats. Restoration of cerebral energy metabolism by adequate dosage of HBO2 may become an important factor for recovery of brain activities after CO poisoning.

Abstract:Pelvic organ prolapse is a prevalent condition, with up to 12 % of women requiring surgery in their lifetime. This manuscript reviews the treatment options for apical prolapse, specifically. Both conservative and surgical management options are acceptable and should be based on patient preferences. Pessaries are the most commonly used conservative management options. Guided pelvic floor muscle training is more beneficial than self-taught Kegel exercises, though may not be effective for high stage or apical prolapse. Surgical treatment options include abdominal and vaginal approaches, the latter of which can be performed open, laparoscopically, and robotically. A systematic review has demonstrated that sacrocolpopexy has better long-term success for treatment of apical prolapse than vaginal techniques, but vaginal surgery can be considered an acceptable alternative. Recent data has demonstrated equal efficacy between uterosacral ligament suspension and sacrospinous ligament suspension at 1 year. To date, two randomized controlled trials have demonstrated equal efficacy between robotic and laparoscopic sacrocolpopexy. Though abdominal approaches may have increased long-term durability, when counseling their patients, surgeons should consider longer operating times and increased pain and cost with these procedures compared to vaginal surgery. Key points: • Pelvic floor physical therapy (PFPT) with a physical therapist is the best approach to conservative management of apical prolapse [10]. • Pessaries should be managed with regular follow-up care to minimize complications [14•]. • Minimally invasive sacrocolpopexy appears as effective as the gold standard abdominal sacrocolpopexy (ASC) [42•]. • Robotic assisted sacrocolpopexy (RASC) and laparoscopic assisted sacrocolpopexy (LASC) are equally effective and should be utilized by pelvic floor surgeons based on their skill level and expertise in laparoscopy [44, 45•]. • Uterosacral ligament suspension (USLS) and sacrospinous ligament suspension (SSLS) are considered equally effective procedures and can be combined with a vaginal hysterectomy. • Obliterative procedures are effective but are considered definitive surgery [24••]. • The use of transvaginal mesh has been shown in some studies to be superior to native tissue repairs with regard to anatomic outcomes, but complication rates are higher. Transvaginal mesh should be reserved for surgeons with adequate training so that complications are minimized.
Laparoscopic Lateral Hysteropexy versus Hysterosacropexy in Women with Stage III Uterine Prolapse

Abstract:Background: Minimally invasive techniques in gynecological pathology have well-known benefits, the "gold standard" of uterine prolapse being currently managed laparoscopically. Laparoscopic lateral hysteropexy and hysterosacropexy are surgical techniques that can be performed for uterine prolapse. Laparoscopic management of such cases is recommended, but requires well-trained teams in laparoscopic surgery. Methods: This study is a prospective analysis of patients who required surgical treatment for stage III uterine prolapse, hospitalized in the Surgery Department of Constanta County Hospital, for which laparoscopic lateral hysteropexy or laparoscopic hysterosacropexy was performed. Results: Between 2016-2020, 61 patients were hospitalized with stage III uterine prolapse that required surgery. All patients underwent laparoscopic surgery. Symptomatology was dominated by urinary incontinence (50%, 44.89%) and obstructive defecation (16.66%, 18.36%). Intraoperative complications were encountered in 33.3% of cases undergoing laparoscopic hysterosacropexy and in 8.16% undergoing laparoscopic lateral hysteropexy. At one year, the recurrence rate was 2.04% for patients who underwent lateral hysteropexy and 8.33% for patients who underwent hysterosacropexy. No patient had a recurrence at the 3-year visit. Conclusions: Laparoscopic lateral hysteropexy is emerging as an appropriate, safe, and effective procedure to treat advanced apical prolapse that requires further clinical attention and development to fully understand its surgical place in the treatment of pelvic defects.
Laparoscopic Management of Pelvic Organ Prolapse: A Case Series

Abstract:Background: Pelvic static disorders have an important impact on patients' quality of life, constituting a real public health problem, despite the fact that they are not life-threatening. Minimally invasive procedures of pelvic organ prolapse has many advantages, laparoscopic hysteropexy and colpopexy being a standard with real benefits: minimal incisions, reduced postoperative complications, shorter hospital stay and a low recurrence rate. Laparoscopic management of such cases is recommended, but requires teams well trained in minimally invasive surgery. Case presentation: We presented a series of successful cases of two patients with grade III hysterocele, respectively vaginal vault prolapse, who were treated minimally invasively with a lateral laparoscopic hysteropexy, respectively lateral laparoscopic colpopexy. Given the well-known benefits of minimally invasive surgery, we chose laparoscopic surgery because of the smaller surgical impact and faster return to normal life. The surgical procedures were performed successfully, without complications, with rapid recovery, without recurrence. Conclusion: Hysteropexy and laparoscopic colpopexy are safe and effective surgical procedures in selected cases.
[Laparoscopic repair of pelvic organ prolapse by lateral suspension with mesh: a continuous series of 218 patients]

Abstract:Objectives: To evaluate the technique of laparoscopic lateral colpo-uterine suspension using a mesh to treat pelvic organ prolapse, with a sufficient follow-up. Patients and methods: The technique consists of two steps. First, the lateral suspension of the vaginal vault and of the uterus is performed using a polypropylene mesh placed in the vesicovaginal septum as a transversal hammock. The second step is the application of a polypropylene patch to the posterior surface of the vagina and the rectovaginal septum. The transversal hammock is placed laterally by the tension-free fixation of the mesh to the lateral abdominal wall above the iliac crests. Between January 2004 and December 2007, 218 patients were treated. It is a continuous series including all the patients needing a surgical procedure to treat a genital prolapse. We excluded, from the study, the patients with a severe cardiorespiratory disease and the cases of isolated rectocele. Results: We observed a recurrence of the prolapse in thirty patients (13.76%). A reoperation was performed in 10 patients (4.6%). One complication was related to the technique of lateral suspension (bladder injury immediately sutured 0.46%). A mesh erosion was noted in 13 cases (5.96%), nine were treated by vaginal excision of the mesh (4.12%). Conclusions: The laparoscopic lateral colpo-uterine suspension using a mesh corrects the pelvic organ prolapse with a predominant cystocele or rectocele. It is an interesting alternative to the other procedures because of the low risk of complications and the satisfactory results.
Laparoscopic Burch Colposuspension for the Treatment of Stress Urinary Incontinence

Abstract:Objective To investigate the possibility and efficacy o f the laparoscopic Burch colposuspension treated for stress urinary incontinence .Methods 11 patients had stress incontinence were treated by lap aroscopic Burch suspension.Preoperative testing included cystoscopy, multichanne l urodynamics with pressure-flow studies.Mean patient age was 63.2 years(range 55～77).All patients had prolapse of uterus.Results Of the 11 lap a roscopic Burch procedures were performed transperitoneally.Mean operative time w as 196 minutes(range 130 to 300),mean blood loss 96.3ml (range 50 to 120)and mea n postoperative hospital stay 4.3 d(range 3 to 6).Mean followup was 6.3 months ( range 2 to 10).Nine patients (81%) had complete resolution of symptoms,2 patient s(19%) reported significant improvement.There are no complications in all patien ts.Conclusion The Burch suspension performed laparoscopically app ears to have a favorable morbidity profile with a successful continence outcome, and a decrease in the length of hospital stay, faster recovery, and fewer traum as and scarring due to the smaller incisions.

Abstract:Background: Laparoscopic sacral colpopexy (LSC) is the gold standard treatment for women with apical/anterior pelvic organ prolapse (POP). For isolated posterior vaginal prolapse, instead, the literature suggests fascial native tissue repair. This is a retrospective 2-year quality-of-life follow-up study after laparoscopic posterior plication (LPP) combined with LSC in patients with anterior/apical prolapse combined with severe posterior colpocele. The primary endpoint was to evaluate the subjective outcomes quality of life (QoL), sexual function, and patient satisfaction rate. The secondary endpoint was to evaluate perioperative and anatomical outcomes at the 2-year follow-up. Methods: A total of 139 consecutive patients with anterior and/or apical prolapse (POP-Q stage ≥ II) and severe posterior vaginal prolapse (posterior POP-Q stage ≥ III) were retrospectively selected from our database among women who underwent, from November 2018 to February 2021, a "two-meshes" LSC. The patients were classified into Group A (81 patients; LSC plus LPP) and Group B (67 patients; LSC alone). The primary endpoint was evaluated using the Patient Global Impression of Improvement (PGI-I), the Pelvic Organ Prolapse Distress Inventory-6 (POPDI-6), the Pelvic Floor Impact Questionnaire-7 (PFIQ-7), the Female Sexual Distress Scale (FSDS), the Pelvic Organ Prolapse/Urinary Incontinence Sexual Questionnaire (PISQ-12), and the EuroQol (EQ-5D). The secondary endpoint was studied using the POP-Q study and an intra-, peri-, and post-operative complications assessment. Two-year follow-up data were analyzed for the study. Results: At 2 years, all women showed a statistically significant amelioration of their symptoms on the QoL questionnaires. We found a statistical difference in favor of posterior plication in terms of the PGI-I successful outcome rate (Group A versus B: 85.3% versus 67.1%), FSDS (median 11 versus 21), and PISQ-12 (median 89 versus 62) (p < 0.05 for all comparisons). A significant improvement of all EQ-5D values was observed from baseline to 2-year follow-up, and only for the "pain/discomfort" domains did we observe a significant improvement in LSC plus LPP patients versus LSC alone (p < 0.05). LSC plus LPP women showed, at 2 years, a significant amelioration of their Ap and GH POP-Q points. We observed no statistical differences in terms of intra-post-operative complications or anatomic failure rate between groups. Conclusions: Our LPP approach to LSC appears to be a safe, feasible, and effective treatment for advanced pelvic organ prolapse with a significant impact on the patient's general health and sexual quality of life. Adding laparoscopic posterior vaginal plication to "two-meshes" sacral colpopexy is recommended in patients with apical/anterior prolapse and concomitant severe posterior colpocele. This surgical approach, in addition to improving the anatomical results of these patients, is associated with a significant improvement in sexual and quality of life indexes.

Abstract:Cells from goat's milk and peripheral blood taken during the lactation period were analysed by flow cytometry. The investigated cells were populations of leucocytes, lymphocyte subpopulations (T, T-helper, T-cytotoxic, B, WC1-N2) and all MHC class II positive cells. Labelling of cells was performed on whole blood and milk cell suspensions. Statistically significant differences were found between percentages of B and WC1-N2 lymphocytes and MHC II positive cells from peripheral blood during the lactation period and all of examined milk cells during the same time.
Medium-term Outcomes of Laparoscopic Pubocervical Fascia Reconstruction and Sacrospinous Ligament Fixation with Posterior Approach for a Pelvic Organ Prolapse: A Retrospective Study of 124 Cases

Abstract:Objective: Pelvic organ prolapse (POP) is a common gynecological disease in middle-aged and older women that seriously affects patients' physical health and quality of life, increases the financial burden for patients, and becomes a major public health concern. The aim of this study was to investigate the medium-term outcomes of laparoscopic pubocervical fascia reconstruction and sacrospinous ligament fixation with a posterior approach for patients with severe POP. Methods: Patients with severe POP quantitation stage III-IV who underwent laparoscopic pubocervical fascia reconstruction and sacrospinous ligament fixation with a posterior approach at the Department of Obstetrics and Gynecology, Sir Run Run Shaw Hospital, Zhejiang University School of Medicine between September 2016 and December 2020 were enrolled in this study. The results and complications were recorded. Data were retrospectively reviewed. Results: In total, 124 patients were analyzed. Patients were followed up for 32.27 +/- 12.90 months. The objective cure rate for patients who underwent hysterectomy was 91.7% (100/109), with 7 (6.4%) patients had anterior vaginal wall prolapse and 2 (1.8%) patients had posterior vaginal wall prolapse. The objective cure rate for patients who retained uterus was 66.7% (10/15). All 5 patients with recurrence had uterine prolapse, and 3 (20.0%) of them also had anterior vaginal wall prolapse. Conclusions: Laparoscopic pubocervical fascia reconstruction and sacrospinous ligament fixation with the posterior approach is a safe, minimally invasive, and effective method for patients with severe POP. Abstract:The laparoscopic sacrocolpopexy is the treatment of choice of pelvic organ prolapses since more than twenty years. The laparoscopic lateral suspension with mesh is an alternative technique. Its originality is the subperitoneal passing of the lateral arm of the mesh in the lateral abdominal wall, leaving the skin above the iliac crest, in a place without risks of vascular, nerve, bowel injuries. We report in this article the results of the three main publications on the subject. The indications are cystocele and apical descent. It can be envisaged when the access of the promontory is difficult; for instance in the presence of obesity, adhesions, sigmoid megacolon, or low position of the left common iliac vein, partially covering the promontory. It is also a practical technique for surgeons having a moderate experience of the promontory access.
Laparoscopic Sacral Colpopexy for Severe Uterine Prolapse

Abstract:Objective To explore the characteristics and clinical effectiveness of laparoscopic sacral colpopexy ( LSC ) by using synthetic mesh in the treatment of severe uterine prolapse . Methods From January 2011 to April 2013, 22 patients withⅢorⅣstage severe uterine prolapse , according to the pelvic organ prolapse quantification ( POP-Q) , were given LSC by using Y-shaped polypropylene mesh .Patients complicated with anterior and posterior vaginal wall prolapse were given prolonged mesh placement in the vesicovaginal and rectovaginal clearance gap , and patients complicated with severe stress urinary incontinence ( SUI ) were given transobturator tension-free vaginal tape ( TVT-O).The anatomic and functional recovery was evaluated by POP-Q, the pelvic floor distress inventory short form 20 (PFDI-20), pelvic floor impact questionnaire short form 7 (PFIQ-7), and the pelvic organ prolapse/urinary incontinence sexual function questionnaire 12 ( PISQ-12 ) . Results All the operations were accomplished successfully .A concomitant TVT-O was also completed for treating severe SUI .No injuries of bladder , urethra, rectum, and great vessels occurred . The mean operation time was (134.3 ±36.6) min, and the mean blood loss was (77.3 ±37.8) ml.All the patients were able to micturate 2-3 days after LSC.The follow-up duration was (17.5 ±8.2) months.All the cases were cured without recurrence or failure of surgery .None of them experienced mesh erosion .Pelvic structure assessed with POP-Q returned to normal , with median values recovering from 1.5, 3.0, 2.0, -2.3, and -1.5 cm preoperatively to -3.0, -3.0, -8.0, -3.0, and -3.0 cm postoperatively at anatomic sites of Aa , Ba, C, Ap, and Bp, respectively.Questionnaire median scores of PFIQ-7 and PFDI-20 were recovered from 66.7 and 66.2 preoperatively to 12.0 and 14.6 postoperatively.The PISQ-12 scores were increased from (69.4 ± 10.3) points to (86.9 ±10.0) points (t =12.351,P =0.000).The UDI-6 was not improved (Z =-0.337,P =0.736).Postoperatively , 3 cases suffered from de novo SUI and 1 case developed urgent incontinence .The symptoms of SUI were more serious after LSC in 4 out of 6 cases with mild SUI , and one of them was given TVT-O eight months after LSC .The patient subjective satisfaction rate was 95.4%(21/22). Conclusion LSC is a safe, effective, and minimally invasive treatment for severe uterine prolapse, but may induce or exacerbate incontinence .
Incidence of stress urinary incontinence after laparoscopic sacrocolpopexy

Abstract:Objective: The aim of the study was to evaluate the occurrence of stress urinary incontinence after pelvic organ prolapse surgery using the laparoscopic sacrocolpopexy method. This is a retrospective multicenter study. Methods: The study included 131 patients who underwent laparoscopic sacrocolpopexy and underwent at least a one-year follow-up in the form of a clinical examination. Results: The group included patients with an average age of 63.4 years (38-80 years), BMI 26.6 kg/m2 (19.4-36 kg/m2), and parity 2 (0-4). Before surgery, 30 (22.9%) patients showed stress incontinence, and after surgery there were 50 (38.2%); the difference was statistically significant (P = 0.0007). Thirty (22.9%) patients underwent subsequent anti-incontinence surgery, while 10 (7.6%) patients experienced significant improvement of stress incontinence. Conclusion: Pelvic organ prolapse reconstruction surgery by laparoscopic sacrocolpopexy is associated with the risk of postoperative stress incontinence in 38.2%, of which de novo in 22.9% of cases.

Abstract:Aim Laparoscopic lateral suspension is a novel approach for repairing anterior and apical pelvic organ prolapse (POP). According to integral theory, urinary symptoms and pelvic pain are believed to originate from suspensory ligaments. We aimed to investigate the objective and subjective outcomes of adding sacroterine plication to apical prolapse surgery. Methods Sixty patients with Grade 2 or higher symptomatic apical POP were included in the study. The study sample was categorized into two groups: Group 1 underwent lateral suspension and Group 2 underwent lateral suspension and sacroterine plication. Anatomical cure was defined separately for the apical and anterior compartments as POP‐Q scores for sites C and Ba of less than −1 cm for each compartment. A subjective cure was defined as the absence of bulge symptoms. Patient satisfaction, sexual function, prolapse‐related quality of life, voiding dysfunction, nocturia, and constipation were assessed. Results In Group 1, anatomical cure rates for apical and anterior prolapse were 100% and 70%, respectively (p <0.001). In Group 2, these rates were 100% for apical prolapse and 73.3% for anterior prolapse (p <0.001). The subjective cure was 96.6% in both groups. Furthermore, improvement in sexual and urinary symptoms was more significant in the group that underwent sacroterine plication (p <0.001). Conclusions The additional sacroterine plication (shortening) procedure with lateral suspension proved to be an effective and successful surgical approach for apical prolapse. Its routine addition to existing lateral suspension surgery can contribute significantly to the improvement of urinary and prolapse symptoms.

Abstract:The selectivity of aquaporins for water and solutes is determined by pore diameter. Paradoxically, the wider pores of glycerol facilitators restrict water passage by an unknown mechanism. Earlier we characterized an aquaglyceroporin from Plasmodium falciparum with high permeability for both glycerol and water. We use point mutations to demonstrate that amino acids directly lining the pore are not responsible for the excellent water permeability of the Plasmodium aquaglyceroporin but affect permeability of pentitols. Within a conserved WET triad in the extracellular C-loop we identified a Plasmodium aquaglyceroporin-specific glutamate (E125) located in proximity to a conserved arginine (R196) at the pore mouth. Mutation of E125 to serine largely abolished water permeability. Concomitantly, the activation energy for water permeation was increased by 4 kcal/mol. Mutation of the adjacent tryptophan to cysteine led to irreversible inhibition of water passage by Hg(2+). This unequivocally proves the proximity of the couple W124/E125 close to the pore mouth. We conclude that in the Plasmodium aquaglyceroporin the electrostatic environment at the extracellular pore entry regulates water permeability.
