Abstract:143 Background: A trend for the increased usage of mastectomy to treat breast cancer and for prophylaxis has been reported. Postmastectomy reconstruction (PMR) may improve body image and quality of life for breast cancer survivors. This prompted an investigation of the current pattern of PMR in our breast surgery practice. METHODS An IRB-approved retrospective chart review of women who had mastectomies performed by dedicated breast surgeons from 2006 through 2009 was completed. Data collected included age, unilateral (ULM) or bilateral mastectomy (BLM), type and timing of reconstruction, BRCA status, presence of an in-breast tumor recurrence (IBTR). Two-sided Fisher exact test and t-test were used in comparing groups. RESULTS From 2006 to 2009, 231 patients ranging in age from 27 to 91 years had ULM (n=155) or BLM (n=76). 38% of the patients had PMR. Patients <50 years old were more likely to undergo PMR than those >50 (67.5% versus 22.7%, p< 0.0001). 4 patients >70 years old had PMR. PMR followed ULM 27.7% of the time and BM 57.9% (p < 0.0001). Only 6 patients underwent delayed PMR. 30 patients had mastectomies after IBTR with 36.7% having PMR and 36.7% opting for BLM. These rates were not significantly different from women with a unilateral de novo breast cancer (33.1% and 21.3%, respectively). In 15 patients with a BRCA mutation, one had ULM. The 14 having BLM were more likely to have BMR than BLM patients without a mutation (92.9% versus 50.0%, respectively, p=0.0029) and were younger (44.6 versus 52.3 years, respectively, p=0.0166). Types of reconstructions were divided between tissue expander/implant (TE/I)(70%) and autologous tissue flaps (AT) techniques. After BLM, TE/I was employed more frequently than after UM (84% versus 56%, respectively, p=0.005). From 2006 to 2009, there was no significant change in type or frequency of PMR. CONCLUSIONS From this data it appears that a majority of women do not have PMR and few women return for a delayed PMR. Fewer women seek PMR after ULM. Rates and types of PMR have not changed over this time period. Further investigation to determine if women feel informed about PRM and to identify factors influencing their choices may be helpful in improving a PMR program.

Abstract:‐The luminescence at 1.27 μm from the 3→→1δg transition of the oxygen molecule has been detected from a variety of liquid systems. A Q‐switched laser delivering pulses of 532 nm light was the excitation source, a germanium photodiode was the detector and substituted porphyrins were used as photosensitizers. Protio‐ and deutero‐ forms of several solvents were studied and the singlet oxygen lifetimes determined directly agreed well with published values. Tδ in D2O was found to be 55 μs and, by extrapolation from a series of H2O ‐ D2O mixtures, a value of 3.3 μs was obtained for Tδ in H2O. The technique was shown to be useful in measuring Tδ values in several microheterogeneous systems such as surfactant micelles, vesicles and human serum albumin.
Guided tissue regeneration demineralized freeze-dried bone allograft: treatment of furcation defects in mandibular molars

Abstract:The present study evaluated the effects of combined guided tissue regeneration (GTR) and demineralized freeze-dried bone allograft (DFDBA) therapy on the healing of grade III furcation lesions in mandibular molars of seven periodontitis patients. De novo surgical debridement of furcation roofs by fine diamond bur was introduced. Routine presurgical preparation of teeth and a strict plaque control program were performed for at least six weeks before surgery. A papillary conserved full thickness mucoperiosteal flap was used in all cases. In addition to conventional debridement, odontoplasty was performed on the furcation areas with a diamond bur to eradicate inaccessible fissures or grooves and ensure calculus-free root surfaces. Following debridement, the bony defects were filled with DFDBA and covered with polytetrafluoroethylene (ePTFE) membranes. The flaps were then closed by interproximal sutures coronally positioned through the contact point. The ePTFE membranes were removed 6 to 7 weeks after operation. Clinical parameters such as probing depth (PD), gingival recession (GR), probing attachment level (PAL), tooth mobility (TM), and periapical x-ray were recorded at the baseline and 0, 3, 6, 9, and 12 months after removal of the ePTFE membrane. The results showed a significant increase in the probing attachment level and radiographic evidence of bone fill at the furcation sites. Thus, the addition of fine diamond bur debridement on the furcation in the GTR procedure with DFDBA grafting may be effective in the treatment of grade III furcation involvement.
Guided Tissue Regeneration Treatment Yields Better Results in Class II Furcations in the Mandible Than in the Maxilla: A Retrospective Study

Abstract:Absorbable porcine collagen membrane with a bovine bone graft can be considered for regenerative treatment in periodontal class II furcation defects. We evaluated the clinical efficacy of guided tissue regeneration (GTR) treatment with bovine bone xenograft and a porcine collagen membrane in molars with class II furcations. Probing depth (PD), clinical attachment level (CAL), and bone level (BL) were recorded at baseline and at 3, 6, and 9 months postoperatively. Thirty class II furcation defects from the lower and upper molars were assessed. Significant improvements in PD and CAL were observed from baseline to 9 months in all groups (p < 0.01). BL improved in all groups except group A in the upper molars in radiographic assessment (p < 0.05). The lower and upper molars showed PD reduction of 50.5% ± 7.44% and 46.2% ± 11.2%, respectively, at 9 months (p = 0.044). In furcations of 1–3 mm, the lower and upper molars showed PD reductions of 51.2% ± 4.49% and 36.5% ± 16.14%, respectively (p = 0.035). The lower and upper molars showed a CAL gain of 51.1% ± 4.64% and 33.6% ± 18.8%, respectively (p = 0.037). Thus, GTR with bovine bone graft and porcine collagen membrane yielded good results in class II furcations, with better results in the lower than in the upper molars.

Abstract:The overall goal of my career as an academic chemist has always been the design and creation of advanced therapeutics and diagnostics that address unmet medical need in the management of chronic diseases. Realising this goal has been an immensely difficult process involving multidisciplinary problem-driven research at the chemistry-biology-medicine interfaces. With success in the laboratory, I started seriously to question the value of remaining an academic whose career is spent in the pursuit of knowledge and understanding alone without making any significant effort to translate knowledge and understanding gained into products of genuine utility for public benefit. Therefore, I elected by choice to become an academic entrepreneur, seeking opportunities wherever possible for the translation of the best of my personal and collaborative academic research work into potentially valuable and useful products. This choice has brought with it many unexpected difficulties and challenges. Nevertheless, progress bas been made and sufficient learnt to suggest that this would be an appropriate moment to take stock and provide some personal reflections on what it takes to design and create advanced therapeutics and diagnostics in the laboratory then seek to develop, innovate and translate the best towards market.

Abstract:Numerically significant clinical experience relative to the surgical treatment of arterial occlusive disease of the gastrointestinal tract has not been published. Attempted revascularization of the celiac and mesenteric arteries has been documented only by isolated case reports, and in these most of the operative procedures were performed under emergency situations for acute mesenteric thrombosis. 6,11-15 Even rough appraisal as to incidence of celiac and superior mesenteric arterial occlusive disease is difficult because until recently 2 essential factors for this have been lacking—namely, general awareness of the problem and application of techniques for confirming the diagnosis. Although the abdominal angina syndrome will probably never rival the clinical importance of such occlusive arterial lesions as those affecting the brain, heart, and legs, it most certainly will prove to be far more significant than presently considered. Guided tissue regeneration in human Class II furcation defects using a diphenylphosphorylazide-cross-linked collagen membrane: a consecutive case series

Abstract:Background: Several bioabsorbable collagen membranes are either currently available or under investigation for use in guided tissue regeneration (GTR) procedures. The aim of this 12-month reentry study was to evaluate a diphenylphosphorylazide (DPPA)-cross-linked bovine type I collagen membrane in the healing of mandibular Class II furcation defects. The DPPA technique prevents the inclusion of foreign cytotoxic cross-linking substances in the reticulated molecules, thus providing a more biocompatible product. Methods: Forty non-smoking periodontally compromised adult subjects volunteered for this study, and one defect was randomly selected in each patient. At baseline, plaque index (PI), gingival index (GI), probing depth (PD), gingival margin location (GML), clinical attachment level (CAL), and width of the attached gingiva (AG) were measured. Intrasurgical hard tissue measurements consisted of the vertical open probing attachment level (V-OPAL) and horizontal open probing attachment level (H-OPAL), as well as furcation entrance width (FW) and height (FH) and subclassification of defects according to vertical bone loss. All soft tissue measurements were repeated 12 months after membrane placement using conservative flap reentry surgeries; hard tissue parameters were also assessed at this time. Results: DPPA-cross-linked collagen membranes demonstrated good handling characteristics, and no postoperative adverse tissue reactions or clinically detectable localized allergic responses were observed. GI and PI remained low during the entire study period. When the one year reentry values were compared to the baseline measurements, a significant reduction in PD (1.9 +/- 1.4), FW (2.1 +/- 1.7), FH (2.9 +/- 1.6), and gain in CAL (1.7 +/- 1.5), V-OPAL (2.1 +/- 1.9), and H-OPAL (4.7 +/- 1.4) were observed (P < or = 0.05). However, there were no significant differences in GML or AG. Hard tissue improvements resulted in the complete closure of 30% (12) of the defects, and transformation of 67.5% (27) of the defects to Class I, while one defect remained a Class II (2.5%). Conclusions: Based on this 12-month clinical reentry study, a DPPA-cross-linked collagen membrane could be beneficial in GTR treatment of Class II mandibular furcation involvements, both in terms of soft and hard tissue improvements.

Abstract:OBJECTIVE Tissues loss due to periodontal disease is typically treated by a variety of regenerative treatment modalities, including bone grafts, guided tissue regeneration (GTR) and growth factors, to reform the supporting tissues of teeth. Concentrated growth factors (CGF) are produced by centrifuging blood samples at alternating and controlled speeds using a special centrifuge. The purpose of this study was to evaluate whether GTR could improve the effect of CGF combined with bone graft in the treatment of classII furcations of mandibular molars. METHODS In the present study, thirty-five classII furcation involvements were included and randomly divided into two groups. The experimental group (n=17) accepted GTR combined with CGF and bone graft therapy, and the controlled group (n=18) accepted CGF combined with bone graft therapy. The clinical examinations and cone beam computed tomography (CBCT) were performed at baseline and 1 year post-surgery. Comparisons of clinical and CBCT data before and after operation between the experimental group and the control group were made. RESULTS The clinical and CBCT data of both groups were not statistically different at baseline (P>0.05). At the end of 1 year post-surgery, the clinical parameters of both groups were significantly improved (P<0.001). The probing depths of the experimental group were (4.81±1.95) mm and (3.56±1.94) mm, respectively, significantly higher than the changes of the control group (P<0.001). The vertical and horizontal attachment gains of the experimental group were (4.11±1.98) mm and (3.84±1.68) mm, respectively, significantly higher than the changes of the control group (P<0.001). At the end of 1 year post-surgery, the experimental group showed significantly higher bone gain at vertical and horizontal directions compared with those of the control group: (3.84±1.68) and (3.88±2.12) mm, respectively (P<0.001). CONCLUSION Within the limitation of the present study, GTR showed positive role in the effect of CGF combined with bone graft in the treatment of classII furcation involvements of mandibular molars.

Abstract:The purpose of the present investigation was to evaluate the use of bone grafts in combination with the GTR-procedure in reinstituting periodontal support around teeth with horizontal bone loss. Orthodontic elastic bands were placed around mandibular and maxillary premolars in 3 dogs in order to induce breakdown of the periodontal tissues. When the destruction had reached a level corresponding to approximately half the root length, the elastic bands were removed and the teeth were scaled. After a period with plaque control, mucoperiosteal flaps were elevated on the buccal and lingual aspects of the experimental teeth. The exposed root surfaces were curetted, and a notch was made at the level of the reduced bone crest. In randomly selected test quadrants, Kielbone mixed with fibrin sealant (Tisseel) was placed in the interproximal space and in the bifurcations. Both the buccal and lingual aspect of the test teeth were then covered by a teflon membrane (Zitex) and the raised tissue flaps sutured in a coronally displaced position. The contralateral teeth (controls) were treated the same way, except that fibrin sealant without Kielbone was applied in the bifurcations and the interproximal space prior to the placement of the membranes. The membranes were removed by a 2nd operation after 35 days. Clinically, various complications were observed during healing. The buccal and lingual flap margins consistently failed to join in the interproximal area, and increasing recession of the flap margins and exposure of the membranes, resulting in accumulation of bacterial deposits and food debris, occurred during the period the membranes were maintained.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract:Background: Class II furcations present difficult treatment problems. Several treatment approaches to obtain furcation fill have been used with varying success. Methods: The response of mandibular Class II furcations to treatment with either a microporous biocompatible composite of PMMA (poly-methyl-methacrylate), PHEMA (poly-hydroxyl-ethyl methacrylate) and calcium hydroxide graft synthetic bone (HTR) replacement graft material; ePTFE barrier membrane; or a combination of the two was evaluated in trios of mandibular molars with Class II furcations in 8 patients with moderate to advanced periodontitis. Following initial preparation, full-thickness flaps were raised in the areas being treated, the bone and furcation defects debrided of granulomatous tissue, and the involved root surfaces mechanically prepared and chemically conditioned. By random allocation, HTR, ePTFE, or a combination of both was placed into and/or fitted over the furcations, packed and/or secured in place, and the host flap replaced or coronally positioned with sutures. Postsurgical deplaquing was performed every 10 days leading up to ePTFE removal at approximately 6 weeks. Continuing periodontal maintenance therapy was provided until surgical reentry at 6 months for documentation and any further necessary treatment. Results: Direct clinical measurements demonstrated essentially similar clinical results with all 3 treatments for bone and soft tissue changes. There were no statistically or clinically significant differences except for better horizontal amount and percent defect fill with HTR alone. Four of 8 furcations became Class I clinically with HTR alone, 5 of 8 became Class I with ePTFE alone, and 5 of 8 with combination treatment. The only complete furcation closure occurred with HTR. Conclusion: The findings of this study suggest essentially equal clinical results with HTR bone replacement graft material alone, ePTFE barrier alone, and a combination of the two in mandibular molar Class II furcations. However, a real difference may not have been detected based on the small number of subjects in the study.
Evaluation of periosteal membranes and coronally positioned flaps in the treatment of Class II furcation defects: a comparative clinical study in humans

Abstract:The purpose of this study was to compare the clinical effectiveness of connective tissue grafts including periosteum used as a mechanical barrier for guided periodontal tissue regeneration and coronally positioned flaps in the treatment of Class II furcation defects. A total of 28 furcation defects were treated; 14 received a periosteal barrier and 14 received a coronally positioned flap. Reentry surgeries were performed at 6 months. No statistically significant differences were found preoperatively between the two treatment groups with respect to clinical parameters and osseous measurements. Postsurgically, both treatment modalities resulted in a significant decrease in probing depth and a significant gain in clinical attachment, but the differences observed were not statistically significant. The periosteal barrier group presented with a significantly better gain in vertical components of the alveolar bone (1.93 +/- 0.15 mm and 0.20 +/- 0.26 mm for periosteal barrier and coronally positioned flap groups, respectively; P < or = 0.001) and horizontal components of the alveolar bone (1.60 +/- 0.21 mm and 0.13 +/- 0.90 mm for periosteal barrier and coronally positioned flap groups, respectively; P < or = 0.001). The results of this trial indicate that similar clinical resolution of Class II furcation defects can be obtained with periosteal barriers and coronally positioned flaps. Periosteal barriers, however, are a better treatment alternative in achieving bone fill of the furcation area.

Abstract:In the present prospective clinical trial, the effect of various regenerative procedures performed at sites with angular bone defects were evaluated. The main outcome variable was probing attachment alteration. Material and methods: 40 subjects, aged 32-61 years participated. They met the following inclusion criteria: (i) presence of generalized, advanced periodontal tissue destruction; (ii) presence of 2 similar, contralateral, angular bone defects (experimental sites) located in either the maxilla or the mandible; (iii) the defect site must exhibit a probing pocket depth (PPD) of > or = 6 mm, a probing attachment level (PAL) of > or = 7 mm, and a depth of the intrabony component of > or = 3 mm. All subjects had a good oral hygiene standard, were in good general health and did not use any medication. Prior to the start of the study, all subjects received non-surgical treatment for periodontal disease. Baseline clinical measurements (plaque, gingivitis, PPD, PAL and soft tissue recession) of the selected experimental sites were obtained 6 months after the completion of basic therapy. The 40 subjects were randomly divided into 4 treatment groups including 10 subjects each: 3 membrane groups and one Emdogain group. 1 h before surgery, the patients were given 3 g of Amoxicillin. No other antibiotics were prescribed. The test and control sites were treated during the same surgical session. Full thickness flaps were elevated and the exposed root surfaces were planed. Membrane placement: The root surface was rinsed with saline. A barrier membrane (Guidor or Resolut or Periodontal (e-PTFE) material) was positioned to cover the defect and the adjacent 2-3 mm of bone tissue. The control treatment was identical to the test treatment with the exception of barrier placement. Emdogain placement: The exposed root surfaces at both the test and control sites were, during a 2-min period, conditioned with a 24% EDTA gel. Emdogain was applied to the exposed root surface of the test site. In the control site, the vehicle, the PGA gel, was used as placebo control. The flaps were closed and sutured to obtain a complete coverage of the intrabony defect. Results: Re-examinations, which were performed 12 months after surgery, disclosed that regenerative therapy, including either the use of barrier membranes or application of enamel matrix proteins to an instrumented root surface in an angular, intrabony defect, enhanced outcome variables such as probing pocket depth and probing attachment gain. It was furthermore demonstrated that clinical improvements were better at sites with deep, than at sites with shallow, intrabony defects. Conclusion: The 4 regenerative modalities tested appeared to be equally effective in terms of PPD reduction and PAL gain, and superior to open flap curettage alone.
Platelet-rich Plasma Induced with Guided Tissue Regeneration in the Treatment of Mandibular Grade II Molar Furcation Disease

Abstract:Objective To evaluate whether Bio-oss in combination with guided tissue regeneration(GTR),a combination of GTR and BPBM(GTR/BPBM),or a combination of platelet-rich plasma,bio-oss and GTR(GTR/BPBM/PRP) can improve the healing of mandibular grade Ⅱ molar furcation as compared with GTR alone.Methods A total of 18 grade Ⅱ mandibular molar furcation defects were divided into 3 groups and were surgically treated with GTR,GTR/BPBM,GTR/BPBM/PRP.Changes in probing depth(PD),clinical attachment level(CAL) were evaluated at baseline,6 months,12months,18months and 24months postoperatively.Results The results showed that 6 months postoperation 3 groups presented with significantly greater PD reduction,and clinical attachment gain than baseline(P<(0.01)).Interestingly,no significant difference was found between GTR alone,GTR/BPBM and GTR/BPBM/PRP(P>0.05).Conclusion This study demonstrate that GTR,GTR/BPBM and GTR/BPBM/PRP can improve the healing of mandibular grade Ⅱ molar furcation defects compared with GTR alone.PRP may be an effective modality of regenerative treatment for class Ⅱ furcation defects.
Guided tissue regeneration in the treatment of human infrabony defects. Clinical, radiographical and microbiological results: a pilot study

Abstract:The aim of the study was to evaluate the clinical, radiographical and microbiological outcome after using guided tissue regeneration (GTR) with a bioabsorbable membrane, Resolut. Subjects with bilateral infrabony defects at single rooted teeth were selected. A total of 22 teeth, 2 in each 1 of 7 patients and 4 in 2 patients, with probing pocket depth > or =5 mm, 3 months after scaling, participated. At baseline, assessments of plaque and gingival indices, bleeding on probing, probing pocket depth and probing attachment level were recorded and reproducible radiographs for computer-based bone level measurements were taken. Bacterial samples were collected to investigate the presence of periodontitis-associated bacteria, e.g., Porphyromonas/Prevotella- and Fusobactrium-like micro-organisms. One tooth was randomly treated with GTR and the contralateral with an open debridement procedure as a control. Clinical, radiographical and microbiological examinations were repeated 6 and 12 months postoperatively. Both procedures demonstrated a statistically significant improvement of gingival conditions, reduction of pocket depths and gain of attachment. When evaluating the differences between test and control teeth, none of the clinical parameters yielded statistical difference. Computer-based bone-level measurements showed only small differences in the majority of both test and control sites. The differences were not significant. Periodontitis-associated bacteria were present at baseline, but the appearance was not related to any specific site or patient and did not demonstrate any unwanted change in the 6- and 12-month samples. The findings suggest that the clinical, radiographical and microbiological improvements were not significantly enhanced with the GTR therapy.
[Effect of Regenerative Therapy for the Furcation Involvements of Mandibular Molars Evaluated by Cone-Beam Computed Tomography].

Abstract:OBJECTIVE:To evaluate the effect of regenerative therapy for the treatment of furcation involvements of mandibular molars with cone-beam computed tomography (CBCT).METHODS:In the study, 38 furcation involvements of mandibular molars were included and randomly divided into two groups. The experimental group accepted guided tissue regeneration and bone graft therapy, and the control group only flap surgery. The clinical examination and CBCT examination were performed at baseline and 1 year post-surgery.RESULTS:The clinical and CBCT data of both groups were not statistically different at baseline (P>0.05). At the end of 1 year post-surgery, except gingival recession, the clinical parameters of both groups were significantly improved (P<0.001). The vertical and horizontal attachment gains of the experimental group were (3.20 ± 1.82) mm and (2.05 ± 1.27) mm, respectively, and significantly higher than the changes of the control group (P<0.001). And at the end of 1 year post-surgery, the experimental group showed significantly higher bone gain at the vertical and horizontal directions compared with those of the control group: (2.82 ± 0.97) mm and (2.24 ± 0.92) mm, respectively (P<0.001).CONCLUSION:With the limitation of this study, the effect of guided tissue regeneration surgery and bone graft therapy for the treatment of furcation involvements of mandibular molars is significantly better than that of the flap surgery. CBCT can reflect the horizontal and vertical bone changes of furcation area, which is more comprehensive than traditional periapicals.

Abstract:BACKGROUND Tissue regeneration within the periodontally involved furcation area is one of the most challenging aspects of periodontal surgery. The aim of this study was to evaluate the additional benefit of leukocyte and platelet-rich fibrin (L-PRF) to autogenous bone grafts (ABGs) in the treatment of mandibular molar degree II furcation involvement, comparing the clinical outcomes with those from open flap debridement (OFD)+ABG and OFD alone treatments. METHODS Fifty-four patients, exhibiting one buccal or lingual mandibular molar furcation defect, were randomly assigned to three treatment groups: OFD+ABG+L-PRF(n = 18); OFD+ABG (n = 18); and OFD (n = 18). Clinical [probing pocket depth (PPD), horizontal clinical attachment level (HCAL), vertical clinical attachment level (VCAL), gingival recession (GR)] and radiographic [vertical bone level (VBL)] parameters were evaluated at baseline and 6 months after treatment. HCAL change was the primary outcome. RESULTS No significant differences within each group were reported for GR changes, but statistically significant improvements in HCAL, VCAL, PPD and VBL were observed in all groups, except for VBL in the OFD group. At 6 months, the mean HCAL gain was 2.29±0.18 mm in the OFD+ABG+L-PRF group, which was significantly greater than that in the OFD+ABG (1.61±0.18 mm) and OFD (0.86±0.18 mm) groups. Both OFD+ABG+L-PRF and OFD+ABG therapies produced a significantly greater clinical and radiographic improvement than OFD. CONCLUSION The addition of L-PRF to ABG produces a significantly greater HCAL gain and PPD reduction as compared to OFD+ABG treatment in mandibular degree II furcation involvements. Laparoscopic Approach for Small Bowel Tumors

Abstract:Aim To review the outcome of laparoscopic small bowel resections as compared with open resection for small bowel tumors. Method Retrospective review comparing total laparoscopic to open resections for small bowel tumors from October 1998 to July 2005 in a single centre. Results During the study period, 9 patients underwent laparoscopic resection, whereas 11 patients underwent open small bowel resection. Majority of the patients (75%) suffered from gastrointestinal stromal tumors. Median operative time in both groups was comparable, but the median operative blood loss was significantly less in the laparoscopic group. (5 vs. 165 mL, P=0.005). The amount of oral dologesic used postoperatively was similar, whereas the use of intravenous morphine was significantly less in the laparoscopic group (7.4 vs. 21.3 mg, P=0.007). The median postoperative hospital stay was significantly shorter in the laparoscopic group (7 vs. 12 d, P=0.018). For patients with gastrointestinal stromal tumors, 2 patients (29%) in the laparoscopic group and 3 patients (38%) in the open group had recurrence of disease during follow-up (P=0.573). Conclusions Laparoscopic resection resulted in favorable short-term outcome compared with open resection for small bowel tumors in selected cases. The oncologic outcome is not compromised with this approach.
Clinical effectiveness of regenerative periodontal surgery and orthodontic tooth movement with clear aligners in stage IV periodontitis: a case series

Abstract:Objectives: To evaluate the clinical effectiveness of regenerative treatment of intrabony defects in combination with consecutive orthodontic therapy with clear aligners in stage IV (type 2) periodontitis. Method and materials: Ten patients with a total of 103 intrabony defects were analyzed after regenerative surgery using collagen-deproteinized bovine bone mineral with or without collagen membrane or enamel matrix derivative followed by orthodontic therapy with clear aligners. Changes in radiographic bone level and probing pocket depths were evaluated after 1 year (T1) and at final splinting (T2) after orthodontic tooth movement. Results: Mean radiographic bone level gain was significant, with 2.13 ± 1.64 mm at T1 and 3.02 ± 2.00 mm at T2. Mean probing pocket depth was significantly reduced from 5.40 ± 1.80 mm at baseline to 3.78 ± 1.73 mm at T1, and remained stable with 3.73 ± 1.70 mm at T2. Pocket closure (≤ 4 mm probing pocket depth) was accomplished in 76% of all defects. Tooth loss amounted to 2.9%. Conclusion: Within the limitations of the retrospective study design, the findings suggest that the interdisciplinary treatment of periodontitis stage IV by regenerative periodontal surgery and consecutive orthodontic therapy with clear aligners can lead to favorable results.
