Abstract:For nearly a century, the initial reproduction number (R0) has been used as a one number summary to compare outbreaks of infectious disease, yet there is no `standard' estimator for R0. Difficulties in estimating R0 arise both from how a disease transmits through a population as well as from differences in statistical estimation method. We describe eight methods used to estimate R0 and provide a thorough simulation study of how these estimates change in the presence of different disease parameters. As motivation, we analyze the 2009 outbreak of the H1N1 pandemic influenza in the USA and compare the results from our eight methods to a previous study. We discuss the most important aspects from our results which effect the estimation of R0, which include the population size, time period used, and the initial percent of infectious individuals. Additionally, we discuss how pre-processing incidence counts may effect estimates of R0. Finally, we provide guidelines for estimating point estimates and confidence intervals to create reliable, comparable estimates of R0.

Abstract:We compare several popular methods of estimating the basic reproduction number, R 0 , focusing on the early stages of an epidemic, and assuming weekly reports of new infecteds. We study the situation when data is generated by one of three standard epidemiological compartmental models: SIR, SEIR, and SEAIR; and examine the sensitivity of the estimators to the model structure. As some methods are developed assuming specific epidemiological models, our work adds a study of their performance in both a well-specified (data generating model and method model are the same) and miss-specified (data generating model and method model differ) settings. We also study R 0 estimation using Canadian COVID-19 case report data. In this study we focus on examples of influenza and COVID-19, though the general approach is easily extendable to other scenarios. Our simulation study reveals that some estimation methods tend to work better than others, however, no singular best method was clearly detected. In the discussion, we provide recommendations for practitioners based on our results.

Abstract:Emerging and re-emerging infections such as SARS (2003) and pandemic H1N1 (2009) have caused concern for public health researchers and policy makers due to the increased burden of these diseases on health care systems. This concern has prompted the use of mathematical models to evaluate strategies to control disease spread, making these models invaluable tools to identify optimal intervention strategies. A particularly important quantity in infectious disease epidemiology is the basic reproduction number, R0. Estimation of this quantity is crucial for effective control responses in the early phase of an epidemic. In our previous study, an approach for estimating the basic reproduction number in real time was developed. This approach uses case notification data and the structure of potential transmission contacts to accurately estimate R0 from the limited amount of information available at the early stage of an outbreak. Based on this approach, we extend the existing methodology; the most recent method features intra- and inter-age groups contact heterogeneity. Given the number of newly reported cases at the early stage of the outbreak, with parsimony assumptions on removal distribution and infectivity profile of the diseases, experiments to estimate real time R0 under different levels of intra- and inter-group contact heterogeneity using two age groups are presented. We show that the new method converges more quickly to the actual value of R0 than the previous one, in particular when there is high-level intra-group and inter-group contact heterogeneity. With the age specific contact patterns, number of newly reported cases, removal distribution, and information about the natural history of the 2009 pandemic influenza in Hong Kong, we also use the extended model to estimate R0 and age-specific R0.
Approximate Bayesian algorithm to estimate the basic reproduction number in an influenza pandemic using arrival times of imported cases

Abstract:Background: In an influenza pandemic, arrival times of cases are a proxy of the epidemic size and disease transmissibility. Because of intense surveillance of travelers from infected countries, detection is more rapid and complete than on local surveillance. Travel information can provide a more reliable estimation of transmission parameters. Method: We developed an Approximate Bayesian Computation algorithm to estimate the basic reproduction number (R0) in addition to the reporting rate and unobserved epidemic start time, utilizing travel, and routine surveillance data in an influenza pandemic. A simulation was conducted to assess the sampling uncertainty. The estimation approach was further applied to the 2009 influenza A/H1N1 pandemic in Mexico as a case study. Results: In the simulations, we showed that the estimation approach was valid and reliable in different simulation settings. We also found estimates of R0 and the reporting rate to be 1.37 (95% Credible Interval [CI]: 1.26-1.42) and 4.9% (95% CI: 0.1%-18%), respectively, in the 2009 influenza pandemic in Mexico, which were robust to variations in the fixed parameters. The estimated R0 was consistent with that in the literature. Conclusions: This method is useful for officials to obtain reliable estimates of disease transmissibility for strategic planning. We suggest that improvements to the flow of reporting for confirmed cases among patients arriving at different countries are required.
The Estimated Time-Varying Reproduction Numbers during the Ongoing Pandemic of the Coronavirus Disease 2019 (COVID-19) in 12 Selected Countries outside China

Abstract:Background: How can we anticipate the progression of the ongoing pandemic of the coronavirus disease 2019 (COVID-19)? As a measure of transmissibility, we aimed to estimate concurrently the time-varying reproduction number, R0(t), over time during the COVID-19 pandemic for each of the following 12 heavily-attacked countries: Singapore, South Korea, Japan, Iran, Italy, Spain, Germany, France, Belgium, United Kingdom, the United States of America, and South Africa. Methods: We downloaded the publicly available COVID-19 pandemic data from the WHO COVID-19 Dashboard website (https://covid19.who.int/) for the duration of January 11, 2020 and May 1, 2020. Then, we specified two plausible distributions of serial interval to apply the novel estimation method implemented in the incidence and EpiEstim packages to the data of daily new confirmed cases for robustly estimating R0(t) in the R software. Results: We plotted the epidemic curves of daily new confirmed cases for the 12 selected countries. A clear peak of the epidemic curve appeared in 10 of the 12 selected countries at various time points, and then the epidemic curve declined gradually. However, the United States of America and South Africa happened to have two or more peaks and their epidemic curves either reached a plateau or still climbed up. Almost all curves of the estimated R0(t) monotonically went down to be less than or close to 1.0 up to April 30, 2020 except Singapore, South Korea, Japan, Iran, and South Africa, of which the curves surprisingly went up and down at various time periods during the COVID-19 pandemic. Finally, the United States of America and South Africa were the two countries with the approximate R0(t) ≥ 1.0 at the end of April, and thus they were now facing the harshest battles against the coronavirus among the 12 selected countries. By contrast, Spain, Germany, and France with smaller values of the estimated R0(t) were relatively better than the other 9 countries. Conclusion: Seeing the estimated R0(t) going downhill speedily is more informative than looking for the drops in the daily number of new confirmed cases during an ongoing epidemic of infectious disease. We urge public health authorities and scientists to estimate R0(t) routinely during an epidemic of infectious disease and to report R0(t) daily to the public until the end of the epidemic.

Abstract:OBJECTIVES: The outbreak of coronavirus disease 2019 (COVID-19) is one of the main public health challenges currently facing the world. Because of its high transmissibility, COVID-19 has already caused extensive morbidity and mortality in many countries throughout the world. An accurate estimation of the basic reproduction number (R0) of COVID-19 would be beneficial for prevention programs. In light of discrepancies in original research on this issue, this systematic review and meta-analysis aimed to estimate the pooled R0 for COVID-19 in the current outbreak.METHODS: International databases (including Google Scholar, Science Direct, PubMed, and Scopus) were searched to identify studies conducted regarding the R0 of COVID-19. Articles were searched using the following keywords: "COVID-19" and "basic reproduction number" or "R0." The heterogeneity among studies was assessed using the I2 index, the Cochran Q test, and T2. A random-effects model was used to estimate R0 in this study.RESULTS: The mean reported R0 in the identified articles was 3.38±1.40, with a range of 1.90 to 6.49. According to the results of the random-effects model, the pooled R0 for COVID-19 was estimated as 3.32 (95% confidence interval, 2.81 to 3.82). According to the results of the meta-regression analysis, the type of model used to estimate R0 did not have a significant effect on heterogeneity among studies (p=0.81).CONCLUSIONS: Considering the estimated R0 for COVID-19, reducing the number of contacts within the population is a necessary step to control the epidemic. The estimated overall R0 was higher than the World Health Organization estimate.
Inhibition of human salivary alpha-amylase by glucopyranosylidene-spiro-thiohydantoin.

Abstract:This study is the first report on the effectiveness and specificity of glucopyranosylidene-spiro-thiohydantoin (G-TH) inhibitor on the 2-chloro-4-nitrophenyl-4-O-beta-D-galactopyranosyl-maltoside (GalG(2)CNP) hydrolysis catalysed by human salivary alpha-amylase (HSA). The inhibition of hydrolysis is a mixed-noncompetitive type. In any case, only one molecule of inhibitor binds to HSA. Since our substrate and inhibitor are small molecules the long enough active site facilitates accommodating both of them simultaneously. However, the product formation can be excluded from enzyme-substrate-inhibitor complex (ESI) since Dixon plots are linear. Kinetic constants calculated from secondary plots and nonlinear regression are almost entirely equal, confirming the fidelity of the suggested model. Kinetic constants (K(1i)=7.3mM, L(1i)=2.84 mM) show that G-TH is not such a potent inhibitor of HSA as acarbose and indicate higher stability for ESI than for enzyme-inhibitor complex.

Abstract:Abstract Background As the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic continues its rapid global spread, quantification of local transmission patterns has been, and will continue to be, critical for guiding the pandemic response. Understanding the accuracy and limitations of statistical methods to estimate the basic reproduction number, R0, in the context of emerging epidemics is therefore vital to ensure appropriate interpretation of results and the subsequent implications for control efforts. Methods Using simulated epidemic data, we assess the performance of 7 commonly used statistical methods to estimate R0 as they would be applied in a real-time outbreak analysis scenario: fitting to an increasing number of data points over time and with varying levels of random noise in the data. Method comparison was also conducted on empirical outbreak data, using Zika surveillance data from the 2015–2016 epidemic in Latin America and the Caribbean. Results We find that most methods considered here frequently overestimate R0 in the early stages of epidemic growth on simulated data, the magnitude of which decreases when fitted to an increasing number of time points. This trend of decreasing bias over time can easily lead to incorrect conclusions about the course of the epidemic or the need for control efforts. Conclusions We show that true changes in pathogen transmissibility can be difficult to disentangle from changes in methodological accuracy and precision in the early stages of epidemic growth, particularly for data with significant over-dispersion. As localized epidemics of SARS-CoV-2 take hold around the globe, awareness of this trend will be important for appropriately cautious interpretation of results and subsequent guidance for control efforts.

Abstract:We assessed public health use of R0, the basic reproduction number, which estimates the speed at which a disease is capable of spreading in a population. These estimates are of great public health interest, as evidenced during the 2009 influenza A (H1N1) vírus pandemic. We reviewed methods commonly used to estimate R0, examined their practical utility, and assessed how estimates of this epidemiological parameter can inform mitigation strategy decisions. In isolation, R0 is a suboptimal gauge of infectious disease dynamics across populations; other disease parameters may provide more useful information. Nonetheless, estimation of R0 for a particular population is useful for understanding transmission in the study population. Considered in the context of other epidemiologically important parameters, the value of R0 may lie in better understanding an outbreak and in preparing a public health response.

Abstract:The effective reproduction number is one of the most important epidemiological parameters, providing suggestions for monitoring the development trend of diseases and also for adjusting the prevention and control policies. However, a few studies have focused on the performance of some common computational methods for R t . The purpose of this article is to compare the performance of three computational methods for R t : the time-dependent (TD) method, the new time-varying (NT) method, and the sequential Bayesian (SB) method. Four evaluation methods—accuracy, correlation coefficient, similarity based on trend, and dynamic time warping distance—were used to compare the effectiveness of three computational methods for R t under different time lags and time windows. The results showed that the NT method was a better choice for real-time monitoring and analysis of the epidemic in the middle and late stages of the infectious disease. The TD method could reflect the change of the number of cases stably and accurately, and was more suitable for monitoring the change of R t during the whole process of the epidemic outbreak. When the data were relatively stable, the SB method could also provide a reliable estimate for R t , while the error would increase when the fluctuation in the number of cases increased. The results would provide suggestions for selecting appropriate R t estimation methods and making policy adjustments more timely and effectively according to the change of R t .

A New Framework and Software to Estimate Time-Varying Reproduction Numbers During Epidemics

Abstract:The quantification of transmissibility during epidemics is essential to designing and adjusting public health responses. Transmissibility can be measured by the reproduction number R, the average number of secondary cases caused by an infected individual. Several methods have been proposed to estimate R over the course of an epidemic; however, they are usually difficult to implement for people without a strong background in statistical modeling. Here, we present a ready-to-use tool for estimating R from incidence time series, which is implemented in popular software including Microsoft Excel (Microsoft Corporation, Redmond, Washington). This tool produces novel, statistically robust analytical estimates of R and incorporates uncertainty in the distribution of the serial interval (the time between the onset of symptoms in a primary case and the onset of symptoms in secondary cases). We applied the method to 5 historical outbreaks; the resulting estimates of R are consistent with those presented in the literature. This tool should help epidemiologists quantify temporal changes in the transmission intensity of future epidemics by using surveillance data.

Abstract:Coronavirus disease 2019 (COVID-19) is an emerging infectious diseases caused by a new coronavirus named SARS-CoV-2. In the epidemiology of infectious diseases, the basic reproduction number (R-0) is a central quantitative parameter that can be used to measure the transmission potential of infectious diseases. R-0 represents the average number of new infections generated by an infectious person in a totally naive population. If R-0>1, the number infected is likely to increase. If R-0<1, transmission is likely to die out. In this study, we present a review on the estimation of basic reproduction number of SARS-CoV-2, in order to provide a scientific basis for better understanding the dynamic characteristics of SARS-CoV-2 transmission. R-0 is usually estimated with various types of complex mathematical models. It can be calculated and estimated by differential equations in the propagation dynamics model (such as SIR model and SEIR model), or estimated using maximum likelihood method, stochastic model, etc. Modeled R-0 values are dependent on model structures and assumptions. With the rapid spread of the global COVID-19 epidemic, there is an increasing evidence on R-0 of SARS-CoV-2. However, the results of R-0 estimation varied a lot in different studies, due to different model assumptions, parameters settings, and data used in the models. The estimated median R-0 is about 3.15 (95%CI: 2.26-6.20) in studies published by peer-reviewed journals, 3.01 (95%CI: 1.99-5.44) in preprinted platform without peer-reviewed and online reports, and 2.55 (95%CI: 1.61-3.55) in studies published in Chinese language, and 3.10 (95%CI: 2.09-6.05) in studies published in English language. With the implementation of comprehensive intervention measures, R-0 showed a downward trend. In the basic propagation dynamics model, R-0 might be estimated with biases, due to the limitations of its basic assumptions (such as fixed population). Regardless of the methods used, characteristics of the virus should be considered fully during estimation. To better estimate R-0, some researchers improved the basic propagation dynamics model and considered the assumption of unfixed population, population mobility, infectivity of the virus during incubation period, and quarantine measures, to make it more in line with the characteristics of SARS-CoV-2. When adopting other methods such as exponential growth model and stochastic model to estimate R-0, the estimated results is relatively stable because that it is not restricted by assumptions of the propagation dynamics model. However, the estimation are more susceptible to the data used in the model (such as distribution of the data) because that the inherent characteristics of the spread of infectious diseases are not considered. Thus, it is particularly critical to use appropriate data distribution and parameters during estimation.
Estimation of the Basic Reproductive Number and Mean Serial Interval of a Novel Pathogen in a Small, Well-Observed Discrete Population

Abstract:Background: Accurately assessing the transmissibility and serial interval of a novel human pathogen is public health priority so that the timing and required strength of interventions may be determined. Recent theoretical work has focused on making best use of data from the initial exponential phase of growth of incidence in large populations. Methods: We measured generational transmissibility by the basic reproductive number R0 and the serial interval by its mean Tg. First, we constructed a simulation algorithm for case data arising from a small population of known size with R0 and Tg also known. We then developed an inferential model for the likelihood of these case data as a function of R0 and Tg. The model was designed to capture a) any signal of the serial interval distribution in the initial stochastic phase b) the growth rate of the exponential phase and c) the unique combination of R0 and Tg that generates a specific shape of peak incidence when the susceptible portion of a small population is depleted. Findings: Extensive repeat simulation and parameter estimation revealed no bias in univariate estimates of either R0 and Tg. We were also able to simultaneously estimate both R0 and Tg. However, accurate final estimates could be obtained only much later in the outbreak. In particular, estimates of Tg were considerably less accurate in the bivariate case until the peak of incidence had passed. Conclusions: The basic reproductive number and mean serial interval can be estimated simultaneously in real time during an outbreak of an emerging pathogen. Repeated application of these methods to small scale outbreaks at the start of an epidemic would permit accurate estimates of key parameters.
Modeling H1N1 Influenza Transmission and Control: Epidemic Theory Insights Across Mexico, Italy, and South Africa

Abstract:This study incorporates mathematical analysis, focusing on developing theories and conducting numerical simulations of Influenza virus transmission using real-world data. The terms in the equations introduce parameters which are determined by fitting the model for matching clinical data sets using non-linear least-square method. The purpose is to determine the wave trend, critical illness factors and forecast for Influenza in national levels of Mexico, Italy, and South Africa and to investigate the effectiveness of control policy and making some suggestions of alternative control policies. Data were extracted from the following sources: published literature, surveillance, unpublished reports, and websites of Centres For Disease Control and Prevention (CDC) \cite{CDC}, Natality report of U.S. clinics and World Health Organization (WHO) Influenza Disease Dashboard \cite{WHO}. We included total 120 weeks data (which are calculated as per thousand) from October 01, 2020 to March 31, 2023 \cite{CDC}, throughout this study. Numerical and sensitivity analysis are carried out to determine some prevent strategies. The objectives of local and global sensitivity analysis is to determine the dominating parameters and effective correlation with $\mathcal{R}_0$. We presented data fitting, Latin hypercube sampling, sensitivity indices, Partial Rank Correlation Coefficient, p-value, estimation of the nature of $\mathcal{R}_0$ from available data to show validation of the model with these counties. The aim is to determine optimal control strategies with drug administration schemes, treatments which represent the efficacy of drug inhabiting viral production and preventing new infections, minimizes the systematic cost based on the percentage effect of the drug. Finally, we present series of numerical examples and the effect of different parameters on the compartments to verify theoretical results.

Abstract:Objective This study uses four COVID-19 outbreaks as examples to calculate and compare merits and demerits, as well as applicational scenarios, of three methods for calculating reproduction numbers. Method The epidemiological characteristics of the COVID-19 outbreaks are described. Through the definition method, the next-generation matrix-based method, and the epidemic curve and serial interval ( SI )-based method, corresponding reproduction numbers were obtained and compared. Results Reproduction numbers ( R eff ), obtained by the definition method of the four regions, are 1.20, 1.14, 1.66, and 1.12. Through the next generation matrix method, in region H R eff = 4.30, 0.44; region P R eff = 6.5, 1.39, 0; region X R eff = 6.82, 1.39, 0; and region Z R eff = 2.99, 0.65. Time-varying reproduction numbers ( R t ), which are attained by SI of onset dates, are decreasing with time. Region H reached its highest R t = 2.8 on July 29 and decreased to R t < 1 after August 4; region P reached its highest R t = 5.8 on September 9 and dropped to R t < 1 by September 14; region X had a fluctuation in the R t and R t < 1 after September 22; R t in region Z reached a maximum of 1.8 on September 15 and decreased continuously to R t < 1 on September 19. Conclusion The reproduction number obtained by the definition method is optimal in the early stage of epidemics with a small number of cases that have clear transmission chains to predict the trend of epidemics accurately. The effective reproduction number R eff , calculated by the next generation matrix, could assess the scale of the epidemic and be used to evaluate the effectiveness of prevention and control measures used in epidemics with a large number of cases. Time-varying reproduction number R t , obtained via epidemic curve and SI , can give a clear picture of the change in transmissibility over time, but the conditions of use are more rigorous, requiring a greater sample size and clear transmission chains to perform the calculation. The rational use of the three methods for reproduction numbers plays a role in the further study of the transmissibility of COVID-19.

Abstract:The outbreak of COVID‐19 was first reported from China, and on February 19, 2020, the first case was confirmed in Qom, Iran. The basic reproduction number (R0) of infection is variable in different populations and periods. The present study aimed to estimate the R0 of COVID‐19 in Qom, Iran and compare it with other countries. For estimation of the serial interval, we used data of the 51 confirmed cases of COVID‐19 and their 318 close contacts in the Qom, Iran. The daily number of confirmed cases in the early phase of the outbreak and estimated serial interval were used for R0 estimation. We used the time‐varying method as a method with the least bias to estimate R0 in Qom, Iran, China, Italy, and South Korea. The serial interval was estimated with gamma distribution, mean of 4.55 days and a standard deviation of 3.30 days for the COVID‐19 epidemic based on Qom data. The number of R0 in the present study was estimated to be between 2 and 3 in Qom. Of the four countries studied, the lowest R0 was estimated for South Korea (1.5‐2) and the highest for Iran (4‐5). Sensitivity analyses demonstrated that R0 is sensitive to the applied mean generation time. To the best of the authors' knowledge, the current study is the first to estimate R0 number in Qom. To control the epidemic, the number of reproduction number should be reduced by decreasing the contact rate, decreasing transmission probability and decreasing the duration of the infectious period.

Abstract:Background/Objective: Since reported firstly in November 2021, Severe Acute Respiratory Syndrome Coronavirus2(SARS-CoV-2) Omicron Variant of Concern (VoC) has spread worldwide at an unprecedented speed. However, scholars have different research results on the transmissibility of this VoC. This study was to estimate the communication power of SARS-CoV-2 Omicron VoC through calculating the time-varying effective reproduction number Re(t) using an outbreak data in Shenyang, China to, as well as to provide data support for other scholars to carry out further transmissibility research of SARS-CoV-2 Omicron VoC.Methods: The daily incidence data of COVID-19 outbreak during March 5 and April 25 in Shenyang, China was downloaded from the Nationwide Notifiable Infectious Diseases Reporting Information System (NIDRIS). The infector-infectee pair was identified through epidemiological investigation and analysis. Then make a list of series interval(SI) distribution and a table of generation time(GT) distribution for each infector-infectee pair. Convert series interval in SI list to generation time distribution through 'est.GT' function included in 'R0' package. At last, estimate Re(t) by R Package 'EpiEstim' based on bayesian framework through parameter and non parameter method respectively.Results: 1134 infections were found in this outbreak with 20 confirmed cases and 1124 asymptomatic infections. 54 infector-infectee pairs were identified and form a SI list, but only 15 infector-infectee pairs were included in the GT table at last. The generation time distribution converted from series interval in SI list met the best fitting distribution of LogNorm distribution with mean = 2.84 and sd = 2.22. Re(t) calculated by parameter method and nonparametric method all peaked on March 17 with a value of 2.58 and 2.54, Re(t) calculated by parameter method decreased to less than 1 on March 29 then remained less than 1, Re(t) calculated by the nonparametric method decreased to less than 1 on March 28 then remained less than 1. There was no statistical difference in Re(t) distribution calculated by the two methods (t = 0.001, P = 0.999).Conclusions: It was reasonable to calculate Re (t) through parameter method in which the SI list was converted into GT distribution by 'est.GT' function in the 'R0' package when we did not have access to generational time distribution of infection-infectee pair data.Funding Information: This work was not supported by any funding.Declaration of Interests: The authors declare that they have no conflict of interest.Ethics Approval Statement: Not applicable.

Abstract:Background: The 2019-nCoV outbreak in Wuhan, China has attracted world-wide attention. As of February 5, 2020, a total of 24433 cases of novel coronavirus-infected pneumonia associated with 2019-nCov were confirmed by the National Health Commission of China. Methods: Three approaches, namely Poisson likelihood-based method (ML), exponential growth rate-based method (EGR) and stochastic Susceptible-Infected-Removed dynamic model-based method (SIR), were implemented to estimate the basic and controlled reproduction numbers. Results: A total of 71 chains of transmission together with dates of symptoms onset and 67 dates of infections were identified among 5405 confirmed cases outside Hubei as reported by February 2, 2020. Based on this information, we find the serial interval having an average of 4.41 days with a standard deviation of 3.17 days and the infectious period having an average of 10.91 days with a standard deviation of 3.95 days. Although the estimated controlled reproduction numbers R_c produced by all three methods in all different regions are significantly smaller compared with the basic reproduction numbers R_0, they are still greater than one. Conclusions: Although the controlled reproduction number is declining, it is still larger than one. Additional efforts are needed to further reduce the R_c to below one in order to end the current epidemic.
