Abstract:Background: In an influenza pandemic, arrival times of cases are a proxy of the epidemic size and disease transmissibility. Because of intense surveillance of travelers from infected countries, detection is more rapid and complete than on local surveillance. Travel information can provide a more reliable estimation of transmission parameters. Method: We developed an Approximate Bayesian Computation algorithm to estimate the basic reproduction number (R0) in addition to the reporting rate and unobserved epidemic start time, utilizing travel, and routine surveillance data in an influenza pandemic. A simulation was conducted to assess the sampling uncertainty. The estimation approach was further applied to the 2009 influenza A/H1N1 pandemic in Mexico as a case study. Results: In the simulations, we showed that the estimation approach was valid and reliable in different simulation settings. We also found estimates of R0 and the reporting rate to be 1.37 (95% Credible Interval [CI]: 1.26-1.42) and 4.9% (95% CI: 0.1%-18%), respectively, in the 2009 influenza pandemic in Mexico, which were robust to variations in the fixed parameters. The estimated R0 was consistent with that in the literature. Conclusions: This method is useful for officials to obtain reliable estimates of disease transmissibility for strategic planning. We suggest that improvements to the flow of reporting for confirmed cases among patients arriving at different countries are required.

Abstract:Emerging and re-emerging infections such as SARS (2003) and pandemic H1N1 (2009) have caused concern for public health researchers and policy makers due to the increased burden of these diseases on health care systems. This concern has prompted the use of mathematical models to evaluate strategies to control disease spread, making these models invaluable tools to identify optimal intervention strategies. A particularly important quantity in infectious disease epidemiology is the basic reproduction number, R0. Estimation of this quantity is crucial for effective control responses in the early phase of an epidemic. In our previous study, an approach for estimating the basic reproduction number in real time was developed. This approach uses case notification data and the structure of potential transmission contacts to accurately estimate R0 from the limited amount of information available at the early stage of an outbreak. Based on this approach, we extend the existing methodology; the most recent method features intra- and inter-age groups contact heterogeneity. Given the number of newly reported cases at the early stage of the outbreak, with parsimony assumptions on removal distribution and infectivity profile of the diseases, experiments to estimate real time R0 under different levels of intra- and inter-group contact heterogeneity using two age groups are presented. We show that the new method converges more quickly to the actual value of R0 than the previous one, in particular when there is high-level intra-group and inter-group contact heterogeneity. With the age specific contact patterns, number of newly reported cases, removal distribution, and information about the natural history of the 2009 pandemic influenza in Hong Kong, we also use the extended model to estimate R0 and age-specific R0.

Abstract:The enzyme catalase (EC 1.11.1.6) is light sensitive and subject to a rapid turnover in light, similar to the D1 reaction center protein of photosystem II. After 3 h of preadaptation to darkness or to different light intensities (90 and 520 mumol m(-2) s(-1) photosynthetic photon flux density), sections of rye leaves (Secale cereale L.) were labeled for 4 h with l-[(35)S]methionine. From leaf extracts, catalase was immunoprecipitated with an antiserum prepared against the purified enzyme from rye leaves. Both incorporation into catalase and degradation of the enzyme polypeptide during a subsequent 16-h chase period increased with light intensity. At a photon flux density of 520 mumol m(-2) s(-1), the apparent half-time of catalase in rye leaves was 3 to 4 h, whereas that of the D1 protein was much shorter, about 1.5 h. Exposure to stress conditions, such as 0.6 m NaCl or a heat-shock temperature of 40 degrees C, greatly suppressed both total protein synthesis and incorporation of the label into catalase and into the D1 protein. Immunoblotting assays indicated that in light, but not in darkness, steady-state levels of catalase and of the D1 protein strongly declined during treatments with salt, heat shock, or translation inhibitors that block repair synthesis. Because of the common property of rapid photodegradation and the resulting dependence on continuous repair, declines in catalase as well as of the D1 protein represent specific and sensitive indicators for stress conditions that suppress the translational activities of leaves.

Abstract:For nearly a century, the initial reproduction number (R0) has been used as a one number summary to compare outbreaks of infectious disease, yet there is no `standard' estimator for R0. Difficulties in estimating R0 arise both from how a disease transmits through a population as well as from differences in statistical estimation method. We describe eight methods used to estimate R0 and provide a thorough simulation study of how these estimates change in the presence of different disease parameters. As motivation, we analyze the 2009 outbreak of the H1N1 pandemic influenza in the USA and compare the results from our eight methods to a previous study. We discuss the most important aspects from our results which effect the estimation of R0, which include the population size, time period used, and the initial percent of infectious individuals. Additionally, we discuss how pre-processing incidence counts may effect estimates of R0. Finally, we provide guidelines for estimating point estimates and confidence intervals to create reliable, comparable estimates of R0.

Seasonal transmission potential and activity peaks of the new influenza A(H1N1): a Monte Carlo likelihood analysis based on human mobility

Abstract:Background: On 11 June the World Health Organization officially raised the phase of pandemic alert (with regard to the new H1N1 influenza strain) to level 6. As of 19 July, 137,232 cases of the H1N1 influenza strain have been officially confirmed in 142 different countries, and the pandemic unfolding in the Southern hemisphere is now under scrutiny to gain insights about the next winter wave in the Northern hemisphere. A major challenge is pre-emptied by the need to estimate the transmission potential of the virus and to assess its dependence on seasonality aspects in order to be able to use numerical models capable of projecting the spatiotemporal pattern of the pandemic. Methods: In the present work, we use a global structured metapopulation model integrating mobility and transportation data worldwide. The model considers data on 3,362 subpopulations in 220 different countries and individual mobility across them. The model generates stochastic realizations of the epidemic evolution worldwide considering 6 billion individuals, from which we can gather information such as prevalence, morbidity, number of secondary cases and number and date of imported cases for each subpopulation, all with a time resolution of 1 day. In order to estimate the transmission potential and the relevant model parameters we used the data on the chronology of the 2009 novel influenza A(H1N1). The method is based on the maximum likelihood analysis of the arrival time distribution generated by the model in 12 countries seeded by Mexico by using 1 million computationally simulated epidemics. An extended chronology including 93 countries worldwide seeded before 18 June was used to ascertain the seasonality effects. Results: We found the best estimate R0 = 1.75 (95% confidence interval (CI) 1.64 to 1.88) for the basic reproductive number. Correlation analysis allows the selection of the most probable seasonal behavior based on the observed pattern, leading to the identification of plausible scenarios for the future unfolding of the pandemic and the estimate of pandemic activity peaks in the different hemispheres. We provide estimates for the number of hospitalizations and the attack rate for the next wave as well as an extensive sensitivity analysis on the disease parameter values. We also studied the effect of systematic therapeutic use of antiviral drugs on the epidemic timeline. Conclusion: The analysis shows the potential for an early epidemic peak occurring in October/November in the Northern hemisphere, likely before large-scale vaccination campaigns could be carried out. The baseline results refer to a worst-case scenario in which additional mitigation policies are not considered. We suggest that the planning of additional mitigation policies such as systematic antiviral treatments might be the key to delay the activity peak in order to restore the effectiveness of the vaccination programs.

Abstract:Effectively predicting the evolution of COVID-19 is of great significance to contain the pandemic. Extensive previous studies proposed a great number of SIR variants, which are efficient to capture the transmission characteristics of COVID-19. However, the parameter estimation methods in previous studies are based on data from epidemiological investigations, which inevitably have caused a large delay. The popularity of digital trajectory data world-wide makes it possible to understand epidemic spreading from human mobility perspective. The major advantage of digital trajectory data lies in that the co-location level of a population is reflected at every moment, making it possible to forecast the evolution in advance. We showed that the mobility data contributed by mobile phone users could be exploited to estimate the contact probability between individuals, thus revealing the dynamic transmission of COVID-19. Specifically, we developed an estimation method to obtain human co-location levels and quantified the variations of human mobility during the epidemic. Then, we extended the infection rate with a real-time co-location level to further forecast the transmission of an epidemic, predicting the epidemic size much more accurately than conventional methods. Finally, the proposed method was applied to evaluate the quantitative effect of different non-pharmacological interventions by predicting the epidemic situations with various mobility characteristics. The empirical results and simulations corroborated our theoretical analysis, providing effective guidance to contain the pandemic.

Abstract:With a simple phenomenological metapopulation model, which characterizes the invasion process of an influenza pandemic from a source to a subpopulation at risk, we compare the efficiency of inter- and intrapopulation interventions in delaying the arrival of an influenza pandemic. We take travel restriction and patient isolation as examples, since in reality they are typical control measures implemented at the inter- and intrapopulation levels, respectively. We find that the intrapopulation interventions, e.g., patient isolation, perform better than the interpopulation strategies such as travel restriction if the response time is small. However, intrapopulation strategies are sensitive to the increase of the response time, which might be inevitable due to socioeconomic reasons in practice and will largely discount the efficiency.

Abstract:—In northern areas of their expanded range, information on Merriam’s turkeys (Meleagris gallopavo merriami) is lacking, specifically pertaining to wintering behavior and factors associated with winter habitat selection. Forest managers need detailed quantification of the effects of logging and other management practices on wintering habitats needed by Wild Turkeys and other wildlife. Therefore, we examined winter habitat selection patterns within ponderosa pine (Pinus ponderosa) forests and determined factors associated with use of farmsteads by Merriam’s turkeys in the southern Black Hills, South Dakota. We radio-marked 86 female Merriam’s turkeys (70 adults and 16 yearlings) and monitored them during winter (1 December–31 March), 2001–2004. Female Wild Turkeys used recently burned pine forest less than expected but selected farmsteads and stands of mature ponderosa pine (<70% overstory canopy cover, >22.9 cm diameter at breast height [DBH] trees) for foraging sites. Within forests, female Wild Turkeys selected foraging sites with less understory vegetation and visual obstruction, and larger-diameter ponderosa pine. Ponderosa pine seed abundance varied among years, and pine seeds were most abundant in stands of 30–35 cm DBH with basal area of 22–28 m2 ⋅ ha–1. Abundance of pine seeds may have influenced use of farmsteads by Wild Turkeys, more so than ambient temperatures or snow depth. In the southern Black Hills, management should emphasize opento mid-canopy and mature-structural-stage pine stands, where seed production was greatest. Abstract:Infants, children and adults are capable of extracting recurring patterns from their environment through statistical learning (SL), an implicit learning mechanism that is considered to have an important role in language acquisition. Research over the past 20 years has shown that SL is present from very early infancy and found in a variety of tasks and across modalities (e.g., auditory, visual), raising questions on the domain generality of SL. However, while SL is well established for infants and adults, only little is known about its developmental trajectory during childhood, leaving two important questions unanswered: (1) Is SL an early-maturing capacity that is fully developed in infancy, or does it improve with age like other cognitive capacities (e.g., memory)? and (2) Will SL have similar developmental trajectories across modalities? Only few studies have looked at SL across development, with conflicting results: some find age-related improvements while others do not. Importantly, no study to date has examined auditory SL across childhood, nor compared it to visual SL to see if there are modality-based differences in the developmental trajectory of SL abilities. We addressed these issues by conducting a large-scale study of children's performance on matching auditory and visual SL tasks across a wide age range (5-12y). Results show modality-based differences in the development of SL abilities: while children's learning in the visual domain improved with age, learning in the auditory domain did not change in the tested age range. We examine these findings in light of previous studies and discuss their implications for modality-based differences in SL and for the role of auditory SL in language acquisition. Inferring country-specific import risk of diseases from the world air transportation network

Abstract:Disease propagation between countries strongly depends on their effective distance, a measure derived from the world air transportation network (WAN). It reduces the complex spreading patterns of a pandemic to a wave-like propagation from the outbreak country, establishing a linear relationship to the arrival time of the unmitigated spread of a disease. However, in the early stages of an outbreak, what concerns decision-makers in countries is understanding the relative risk of active cases arriving in their country—essentially, the likelihood that an active case boarding an airplane at the outbreak location will reach them. While there are data-fitted models available to estimate these risks, accurate mechanistic, parameter-free models are still lacking. Therefore, we introduce the 'import risk' model in this study, which defines import probabilities using the effective-distance framework. The model assumes that airline passengers are distributed along the shortest path tree that starts at the outbreak's origin. In combination with a random walk, we account for all possible paths, thus inferring predominant connecting flights. Our model outperforms other mobility models, such as the radiation and gravity model with varying distance types, and it improves further if additional geographic information is included. The import risk model's precision increases for countries with stronger connections within the WAN, and it reveals a geographic distance dependence that implies a pull- rather than a push-dynamic in the distribution process. For the spread of a contagious disease, human mobility puts distant places in proximity and geographically closer targets may be effectively much further away. The worldwide flight network is crucial for long distance travels and the previously proposed 'effective distance' translates this mobility into a distance measure that correlates with the disease arrival time. We use the effective distance to generate a bottom-up and thus parameter-free distribution process of passengers on the flight network, which takes into account all possible flight routes. This allows us to determine the import probability of a disease. Our 'import risk' model outperforms or matches established mobility models, some of which require calibration with scarce or costly data. In contrast, our approach relies on minimal flight network data, that is the number of planes between airports and their passenger capacities, but not on passenger data. Its bottom-up approach enables future studies on country-specific measures for controlling and containing infected passengers, a challenge with existing models. Thus, the 'import risk' model's strength lies in its data simplicity, this relevance to pandemics, and parameter-free design.

Modeling the initial transmission dynamics of influenza A H1N1 in Guangdong Province, China

Abstract:Background: The novel influenza A H1N1 (2009) virus, identified in mid-2009, spread rapidly in Guangdong Province. The accurate estimation of epidemiological parameters is of vital significance in decision-making for coping with pandemic influenza. Methods: We used influenza A H1N1 epidemic data from local cases in Guangdong Province, China, in conjunction with a complex SEIR model (susceptible, exposed, infectious, recovered) to estimate the basic reproduction number. The transmission rate was obtained by fitting the model to the cumulative number of local daily infected cases using the nonlinear ordinary least squares method. The latent period and duration of infectiousness were obtained from the published literature, and the proportion of symptomatic infected cases was obtained from the serological survey conducted by the Center for Disease Control and Prevention of Guangdong Province. We determined the variance of model parameters via a simulation study. Results: The model was in keeping with the observed epidemic data (coefficient of determination=0.982). The basic reproduction number was estimated preliminarily to be R0=1.525 (95% confidence interval 1.448-1.602), with the possible range of true R0 being 1.30-1.85. We estimated the transmission rate β to be between 0.390 and 0.432. Conclusions: With the help of the serological survey, useful estimates of key epidemiological parameters for the influenza A H1N1 outbreak in Guangdong Province were obtained. The sensitivity analysis suggests that different latent periods and infectious periods, which specify different mean durations of generation time, have a significant impact on R0. Our proposed model and findings provide a relevant contribution towards understanding the characteristics of influenza A H1N1 in Guangdong Province.
Estimating the generation time for influenza transmission using household data in the United States

Abstract:The generation time, representing the interval between infections in primary and secondary cases, is essential for understanding and predicting the transmission dynamics of seasonal influenza, including the real-time effective reproduction number (Rt). However, comprehensive generation time estimates for seasonal influenza, especially post the 2009 influenza pandemic, are lacking. We estimated the generation time utilizing data from a 7-site case-ascertained household study in the United States over two influenza seasons, 2021/2022 and 2022/2023. More than 200 individuals who tested positive for influenza and their household contacts were enrolled within 7 days of the first illness in the household. All participants were prospectively followed for 10 days completing daily symptom diaries and collecting nasal swabs, which were tested for influenza via RT-PCR. We analyzed these data by modifying a previously published Bayesian data augmentation approach that imputes infection times of cases to obtain both intrinsic (assuming no susceptible depletion) and realized (observed within household) generation times. We assessed the robustness of the generation time estimate by varying the incubation period, and generated estimates of the proportion of transmission before symptomatic onset, infectious period, and latent period. We estimated a mean intrinsic generation time of 3.2 (95% credible interval, CrI: 2.9-3.6) days, with a realized household generation time of 2.8 (95% CrI: 2.7-3.0) days. The generation time exhibited limited sensitivity to incubation period variation. Estimates of the proportion of transmission that occurred before symptom onset, the infectious period, and the latent period were sensitive to variation in incubation periods. Our study contributes to the ongoing efforts to refine estimates of the generation time for influenza. Our estimates, derived from recent data following the COVID-19 pandemic, are consistent with previous pre-pandemic estimates, and will be incorporated into real-time Rt estimation efforts.

Abstract:Pandemics have the potential to cause immense disruption and damage to communities and societies. In this paper, we model the Influenza Pandemic of 2009. We propose a hybrid model to determine how the pandemic spreads through the world. The model considers both the SEIR-based model for local areas and the network model for global connection between countries referring to data on international travelers. Our interest is to reproduce the situation using the data of early stage of pandemic and to predict the future transition by extending the simulation cycle. Without considering the tendency of seasonal flu, the simulation does not predict the second peak of the pandemic in the real world. However, considering the seasonal tendency, the simulation result predicts the next peak in winter. Thus we consider the seasonal tendency is an important factor for the spreading of the pandemic.

Abstract:Tracking pathogen transmissibility during infectious disease outbreaks is essential for assessing the effectiveness of public health measures and planning future control strategies. A key measure of transmissibility is the time-dependent reproduction number, which has been estimated in real-time during outbreaks of a range of pathogens from disease incidence time series data. While commonly used approaches for estimating the time-dependent reproduction number can be reliable when disease incidence is recorded frequently, such incidence data are often aggregated temporally (for example, numbers of cases may be reported weekly rather than daily). As we show, commonly used methods for estimating transmissibility can be unreliable when the timescale of transmission is shorter than the timescale of data recording. To address this, here we develop a simulation-based approach involving Approximate Bayesian Computation for estimating the time-dependent reproduction number from temporally aggregated disease incidence time series data. We first use a simulated dataset representative of a situation in which daily disease incidence data are unavailable and only weekly summary values are reported, demonstrating that our method provides accurate estimates of the time-dependent reproduction number under such circumstances. We then apply our method to two outbreak datasets consisting of weekly influenza case numbers in 2019-20 and 2022-23 in Wales (in the United Kingdom). Our simple-to-use approach will allow accurate estimates of time-dependent reproduction numbers to be obtained from temporally aggregated data during future infectious disease outbreaks.

Global Spatio-temporal Patterns of Influenza in the Post-pandemic Era

Abstract:We study the global spatio-temporal patterns of influenza dynamics. This is achieved by analysing and modelling weekly laboratory confirmed cases of influenza A and B from 138 countries between January 2006 and May 2014. The data were obtained from FluNet, the surveillance network compiled by the the World Health Organization. We report a pattern of {\it skip-and-resurgence} behavior between the years 2011 and 2013 for influenza H1N1/09, the strain responsible for the 2009 pandemic, in Europe and Eastern Asia. In particular, the expected H1N1/09 epidemic outbreak in 2011 failed to occur (or"skipped") in many countries across the globe, although an outbreak occurred in the following year. We also report a pattern of {\it well-synchronized} 2010 winter wave of H1N1/09 in the Northern Hemisphere countries, and a pattern of replacement of strain H1N1/77 by H1N1/09 between the 2009 and 2012 influenza seasons. Using both a statistical and a mechanistic mathematical model, and through fitting the data of 108 countries (108 countries in a statistical model and 10 large populations with a mechanistic model), we discuss the mechanisms that are likely to generate these events taking into account the role of multi-strain dynamics. A basic understanding of these patterns has important public health implications and scientific significance.

Abstract:Pandemic influenza A (H1N1) 2009 virus spread rapidly around the world in 2009. We used multiple data sources from surveillance systems and specific investigations to characterize the transmission patterns of this virus in China during May-November 2009 and analyze the effectiveness of border entry screening and holiday-related school closures on transmission. In China, age distribution and transmission dynamic characteristics were similar to those in Northern Hemisphere temperate countries. The epidemic was focused in children, with an effective reproduction number of ≈1.2-1.3. The 8 days of national holidays in October reduced the effective reproduction number by 37% (95% credible interval 28%-45%) and increased underreporting by ≈20%-30%. Border entry screening detected at most 37% of international travel-related cases, with most (89%) persons identified as having fever at time of entry. These findings suggest that border entry screening was unlikely to have delayed spread in China by >4 days.
Modeling H1N1 Influenza Transmission and Control: Epidemic Theory Insights Across Mexico, Italy, and South Africa

Abstract:This study incorporates mathematical analysis, focusing on developing theories and conducting numerical simulations of Influenza virus transmission using real-world data. The terms in the equations introduce parameters which are determined by fitting the model for matching clinical data sets using non-linear least-square method. The purpose is to determine the wave trend, critical illness factors and forecast for Influenza in national levels of Mexico, Italy, and South Africa and to investigate the effectiveness of control policy and making some suggestions of alternative control policies. Data were extracted from the following sources: published literature, surveillance, unpublished reports, and websites of Centres For Disease Control and Prevention (CDC) \cite{CDC}, Natality report of U.S. clinics and World Health Organization (WHO) Influenza Disease Dashboard \cite{WHO}. We included total 120 weeks data (which are calculated as per thousand) from October 01, 2020 to March 31, 2023 \cite{CDC}, throughout this study. Numerical and sensitivity analysis are carried out to determine some prevent strategies. The objectives of local and global sensitivity analysis is to determine the dominating parameters and effective correlation with $\mathcal{R}_0$. We presented data fitting, Latin hypercube sampling, sensitivity indices, Partial Rank Correlation Coefficient, p-value, estimation of the nature of $\mathcal{R}_0$ from available data to show validation of the model with these counties. The aim is to determine optimal control strategies with drug administration schemes, treatments which represent the efficacy of drug inhabiting viral production and preventing new infections, minimizes the systematic cost based on the percentage effect of the drug. Finally, we present series of numerical examples and the effect of different parameters on the compartments to verify theoretical results.

Estimation of influenza incidence and analysis of epidemic characteristics from 2009 to 2022 in Zhejiang Province, China

Abstract:Background: Influenza infection causes a huge burden every year, affecting approximately 8% of adults and approximately 25% of children and resulting in approximately 400,000 respiratory deaths worldwide. However, based on the number of reported influenza cases, the actual prevalence of influenza may be greatly underestimated. The purpose of this study was to estimate the incidence rate of influenza and determine the true epidemiological characteristics of this virus. Methods: The number of influenza cases and the prevalence of ILIs among outpatients in Zhejiang Province were obtained from the China Disease Control and Prevention Information System. Specimens were sampled from some cases and sent to laboratories for influenza nucleic acid testing. Random forest was used to establish an influenza estimation model based on the influenza-positive rate and the percentage of ILIs among outpatients. Furthermore, the moving epidemic method (MEM) was applied to calculate the epidemic threshold for different intensity levels. Joinpoint regression analysis was used to identify the annual change in influenza incidence. The seasonal trends of influenza were detected by wavelet analysis. Results: From 2009 to 2021, a total of 990,016 influenza cases and 8 deaths were reported in Zhejiang Province. The numbers of estimated influenza cases from 2009 to 2018 were 743,449, 47,635, 89,026, 132,647, 69,218, 190,099, 204,606, 190,763, 267,168 and 364,809, respectively. The total number of estimated influenza cases is 12.11 times the number of reported cases. The APC of the estimated annual incidence rate was 23.33 (95% CI: 13.2 to 34.4) from 2011 to 2019, indicating a constant increasing trend. The intensity levels of the estimated incidence from the epidemic threshold to the very high-intensity threshold were 18.94 cases per 100,000, 24.14 cases per 100,000, 141.55 cases per 100,000, and 309.34 cases per 100,000, respectively. From the first week of 2009 to the 39th week of 2022, there were a total of 81 weeks of epidemics: the epidemic period reached a high intensity in 2 weeks, the epidemic period was at a moderate intensity in 75 weeks, and the epidemic period was at a low intensity in 2 weeks. The average power was significant on the 1-year scale, semiannual scale, and 115-week scale, and the average power of the first two cycles was significantly higher than that of the other cycles. In the period from the 20th week to the 35th week, the Pearson correlation coefficients between the time series of influenza onset and the positive rate of pathogens, including A(H3N2), A (H1N1)pdm2009, B(Victoria) and B(Yamagata), were − 0.089 ( p = 0.021), 0.497 ( p < 0.001), −0.062 ( p = 0.109) and − 0.084 ( p = 0.029), respectively. In the period from the 36th week of the first year to the 19th week of the next year, the Pearson correlation coefficients between the time series of influenza onset and the positive rate of pathogens, including A(H3N2), A (H1N1)pdm2009, B(Victoria) and B(Yamagata), were 0.516 ( p < 0.001), 0.148 ( p < 0.001), 0.292 ( p < 0.001) and 0.271 ( p < 0.001), respectively. Conclusion: The disease burden of influenza has been seriously underestimated in the past. An appropriate method for estimating the incidence rate of influenza may be to comprehensively consider the influenza-positive rate as well as the percentage of ILIs among outpatients. The intensity level of the estimated incidence from the epidemic threshold to the very high-intensity threshold was calculated, thus yielding a quantitative standard for judging the influenza prevalence level in the future. The incidence of influenza showed semi-annual peaks in Zhejiang Province, including a main peak from December to January of the next year followed by a peak in summer. Furthermore, the driving factors of the influenza peaks were preliminarily explored. While the peak in summer was mainly driven by pathogens of A(H3N2), the peak in winter was alternately driven by various pathogens. Our research suggests that the government urgently needs to address barriers to vaccination and actively promote vaccines through primary care providers.
