Abstract:Background: The major intrinsic cause of facial skin degeneration is age, associated with extrinsic factors such as exposure to sun. Its major pathologic causes are degeneration of the elastin matrix, with loss of oxytalan and elaunin fibers in the subepidermal region, and actinic degeneration of elastin fibers that lose their functional properties in the deep dermis. Therapy using autologous adipose mesenchymal stem cells for regeneration of extracellular matrix in patients with solar elastosis was addressed in qualitative and quantitative analyses of the dermal elastic fiber system and the associated cells. Methods: Mesenchymal stem cells were obtained from lipoaspirates, expanded in vitro, and introduced into the facial skin of patients submitted after 3 to 4 months to a face-lift operation. In the retrieved skin, immunocytochemical analyses quantified elastic matrix components; cathepsin K; matrix metalloproteinase 12 (macrophage metalloelastase); and the macrophage M2 markers CD68, CD206, and hemeoxygenase-1. Results: A full de novo formation of oxytalan and elaunin fibers was observed in the subepidermal region, with reconstitution of the papillary structure of the dermal-epidermal junction. Elastotic deposits in the deep dermis were substituted by a normal elastin fiber network. The coordinated removal of the pathologic deposits and their substitution by the normal ones was concomitant with activation of cathepsin K and matrix metalloproteinase 12, and with expansion of the M2 macrophage infiltration. Conclusion: The full regeneration of solar elastosis was obtained by injection of in vitro expanded autologous adipose mesenchymal stem cells, which are appropriate, competent, and sufficient to elicit the full structural regeneration of the sun-aged skin. Skin Cellular Reprogramming as an Innovative Anti-Aging Strategy for Cosmetic Application: A Clinical Study of Sericoside

Abstract:Background: While our body ages, skin cells progressively lose their pluripotency and proliferative capacities, as well as remodeling driver role, among other activities. This loss of capacities leads to visible aging signs such as wrinkles, under-eye bags or even aging spots. We studied if the stimulation of cell pluripotency and proliferation by a natural molecule could be an innovative anti-ageing strategy for skin rejuvenation. Methods: The activity of sericoside, a compound extracted from the bark of Terminalia sericea roots, was evaluated at a concentration of 0.02% in vitro. This assessment involved transcriptomic analysis on fibroblasts after 24 hours, as well as proliferation tests on aged fibroblasts after 72 hours. A clinical study was then conducted on 40 volunteers between the ages of 35 and 55. For four weeks, volunteers applied a cream twice daily containing either sericoside or blank emulsion (control group). Skin elasticity was measured by cutometry with R2 parameter. Skin texture and roughness was analyzed by an in vivo 3D scanner. Results: Transcriptomic analysis showed that sericoside improved the set of gene expressions involved in cell cycle (+85% MKI67), cell proliferation (+250% IGF1), DNA repair (+56% OGG1), pluripotency transcription factors (+36% NANOG) and stem cells maintenance (+200% SOX2). We substantiated a decrease of proliferation factor with aged cells compared to young cells by 50%, while sericoside increased this proliferation factor by +46%, a similar rate to that of a 22-year-old donor. Clinically, the anti-aging effects of sericoside were evident: the use of sericoside resulted in a 17% increase in skin elasticity and a 10% reduction in skin roughness, underscoring the smoothing effect with sericoside. Conclusions: The study highlighted an innovative anti-aging strategy that involves re-activating cells' memory to reprogram cell pluripotency by stimulating the natural tools available in our DNA.

Abstract:OBJECTIVES To evaluate outbreaks of disease attributable to eastern equine encephalitis virus (EEEV) in horses in Michigan, and the associated environmental patterns and weather conditions, so that factors could be identified that may have predisposed horses in specific areas of the state to infections with EEEV. DESIGN Epidemiologic retrospective records analysis. ANIMALS Data on EEEV vectors, wild-bird reservoir hosts, and incidental hosts, including horses and human beings, obtained from census reports and medical records compiled between 1942 and 1991. PROCEDURE Patterns detected during outbreaks of disease attributable to EEEV infections in horses were compared to associated water drainage patterns, distributions of EEEV vectors, wild-bird reservoir and incidental hosts, and weather conditions. RESULTS Michigan has all of the elements required to sustain EEEV on a state-wide basis. Outbreaks of disease attributable to EEEV in horses have recurred in a similar regional distribution in Michigan. Regions of Michigan that have specific patterns for water drainage, specific mosquito species, and areas with higher than expected amounts of precipitation have been associated with outbreaks of disease attributable to EEEV in horses. CLINICAL IMPLICATIONS Evaluation of environmental patterns, weather conditions, and vector and reservoir host distributions may be useful to identify areas in Michigan and elsewhere in which horses and human beings are at increased risk for an outbreak of disease attributable to EEEV.

Abstract:Aging is characterized by a gradual decline in function, partly due to accumulated molecular damage. Human skin undergoes both chronological aging and environmental degradation, particularly UV-induced photoaging. Detrimental structural and physiological changes caused by aging include epidermal thinning due to stem cell depletion and dermal atrophy associated with decreased collagen production. Here, we present a comprehensive single-cell atlas of skin aging, analyzing samples from young, middle-aged, and elderly individuals, including both sun-exposed and sun-protected areas. This atlas reveals age-related cellular composition and function changes across various skin cell types, including epidermal stem cells, fibroblasts, hair follicles, and endothelial cells. Using our atlas, we have identified basal stem cells as a highly variable population across aging, more so than other skin cell populations such as fibroblasts. In basal stem cells, we identified ATF3 as a novel regulator of skin aging. ATF3 is a transcriptional factor for genes involved in the aging process, with its expression reduced by 20% during aging. Based on this discovery, we have developed an innovative mRNA-based treatment to mitigate the effects of skin aging. Cell senescence decreased 25% in skin cells treated with ATF3 mRNA, and we observed an over 20% increase in proliferation in treated basal stem cells. Importantly, we also found crosstalk between keratinocytes and fibroblasts as a critical component of therapeutic interventions, with ATF3 rescue of basal cells significantly enhancing fibroblast collagen production by approximately 200%. We conclude that ATF3-targeted mRNA treatment effectively reverses the effects of skin aging by modulating specific cellular mechanisms, offering a novel, targeted approach to human skin rejuvenation.

Abstract:Background: Blood Cell Secretome, BCS (also Autologous Conditioned Serum, ACS) is efficacious in treatment of musculoskeletal disorders. It contains inflammation resolving cytokines, growth factors, exosomes, and lipid mediators. Skin aging is associated with reduced TGF-ß signaling and collagen synthesis and chronic (sub-acute) inflammation, among other factors. Pre-clinically, BCS counteracts these mechanisms, suggesting it as a treatment against cutaneous aging. Objective: This 24-week study evaluated the effects of deep dermal to immediate sub-dermal micropuncture injections of cell-free BCS in patients with age-related reduced facial skin elasticity. Methods: In this prospective, single-armed, mono-center study, 21 women underwent 4 BCS treatment sessions over 12 weeks with follow-up at 24 weeks. The primary endpoint was skin elasticity measured by cutometry. Secondary endpoints were safety, skin hydration, and aesthetic assessments using global aesthetic improvement scale. Results: Skin firmness increased significantly between baseline and 12 weeks (P<0.001) and further increased by 24 weeks (P<0.001). Skin tiring was congruently reduced (P<0.001). Skin hydration and aesthetic ratings improved significantly. No BCS-related adverse reactions occurred. Conclusion: BCS treatment resulted in increased firmness and hydration, usually attributed to younger skin. Abstract:Introduction: The skin is a vital organ as the body's largest barrier, but its function declines with aging. Therefore, research into effective regeneration treatments must continue to advance. Stem cell transplantation, a cell-based therapy, has become a popular skin-aging treatment, although it comes with drawbacks like host immune reactions. Stem cell-derived cell-free therapies have emerged as an alternative, backed by promising preclinical findings. Stem cell secretomes and extracellular vesicles (EVs) are the key components in cell-free therapy from stem cells. However, comprehensive reviews on the mechanisms of such treatments for skin aging are still limited.Purpose: This review discusses stem cell-derived cell-free therapy's potential mechanisms of action related to skin aging prevention by identifying specific molecular targets suitable for the interventions.Methods: A search identified 27 relevant in vitro studies on stem cell-derived cell-free therapy interventions in skin aging model cells without restricting publication years using PubMed, Scopus, and Google Scholar.Results: Stem cell-derived cell-free therapy can prevent skin aging through various mechanisms, such as (1) involvement of multiple regenerative pathways [NFkb, AP-1, MAPK, P-AKT, NRF2, SIRT-1]; (2) oxidative stress regulation [by reducing oxidants (HO-1, NQO1) and enhancing antioxidants (SOD1, CAT, GP, FRAP)]; (3) preventing ECM degradation [by increasing elastin, collagen, HA, TIMP, and reducing MMP]; (4) regulating cell activity [by reducing cell senescence (SA-β-gal), apoptosis, and cell cycle arrest (P53, P12, P16); and enhancing autophagy, cell migration, and cell proliferation (Ki67)] (5) Regulating the inflammatory pathway [by reducing IL-6, IL-1, TNF-⍺, and increasing TGF-β]. Several clinical trials have also revealed improvements in wrinkles, elasticity, hydration, pores, and pigmentation.Conclusion: Stem cell-derived cell-free therapy is a potential novel treatment for skin aging by cell rejuvenation through various molecular mechanisms.

Abstract:Background . Stem cells from adipose tissue (ADSCs) and platelet-rich plasma (PRP) are innovative modalities that arise due to their regenerative potential. Objective . The aim of this study was to characterize possible histological changes induced by PRP and ADSC therapies in photoaged skin. Methods . A prospective randomized study involving 20 healthy individuals, showing skin aging. They underwent two therapeutic protocols (protocol 1: PRP; protocol 2: ADSCs). Biopsies were obtained before and after treatment (4 months). Results . PRP protocol showed unwanted changes in the reticular dermis, mainly due to the deposition of a horizontal layer of collagen (fibrosis) and elastic fibers tightly linked. Structural analyses revealed infiltration of mononuclear cells and depot of fibrotic material in the reticular dermis. The ADSC protocol leads to neoelastogenesis with increase of tropoelastin and fibrillin. There was an improvement of solar elastosis inducing an increment of macrophage polarization and matrix proteinases. These last effects are probably related to the increase of elastinolysis and the remodeling of the dermis. Conclusions . The PRP promoted an inflammatory process with an increase of reticular dermis thickness with a fibrotic aspect. On the other hand, ADSC therapy is a promising modality with an important antiaging effect on photoaged human skin.

Autologous pure platelet-rich plasma injections for facial skin rejuvenation: Biometric instrumental evaluations and patient-reported outcomes to support antiaging effects

Abstract:Background: During skin aging, a degeneration of connective tissue and decrease in hyaluronic acid polymers occur. Since platelet-rich plasma (PRP) contains growth factors and various cytokines, it was hypothesized that it could play a role in fibroblast activation and type I collagen expression in human fibroblasts. Objectives: This study was performed to assess the efficacy of autologous PRP injections for facial skin rejuvenation, measured by biometric instrumental evaluations and patient-reported outcomes. Patients and methods: Patients signed an informed consent form. The EmCyte PurePRP® system technology was used to produce neutrophil-poor PurePRP. The efficacy of the procedures was assessed by biometric parameters, and a patient outcome a self-assessment questionnaire on each visit and at 6-month follow-up. Results: Eleven volunteers were included in the study, receiving 3 PurePRP® treatments. A significant decrease in brown spot counts and area (P < 0.05) was seen after 3 months. Wrinkle count and volume were significantly reduced (P < 0.05 for total wrinkle appearance). Skin firmness parameters were significantly improved. Skin redness was significantly improved after 169 days post-therapy for both the nasolabial and malar areas. A decrease in SLEB thickness was already noted at 2 months after the first injection, with an increase in SLEB density (P < 0.05 for both parameters), without affecting subcutaneous fat thickness. Self-assessment at 6-month follow-up revealed an average satisfaction score of >90%. Conclusions: A series of 3 PurePRP injections at 6-month follow-up resulted in significant skin rejuvenation as demonstrated by biometric parameters and confirmed by patient self-assessment score.
Clinical Evidence of Dermal and Epidermal Restructuring from a Biologically Active Growth Factor Serum for Skin Rejuvenation

Abstract:Background: Topical skin care products use various technologies to promote skin repair. Growth factors of human, animal, and plant-derived origins have clinically demonstrated the ability to repair skin by promoting collagen, elastin, and glycosaminoglycan (GAG) production to reconstruct and reinforce skin’s extracellular matrix (ECM). Human skin cells respond to instructions from highly specialized proteins or hormones referred to as growth factors. These growth factors initiate cellular communication that instigates cellular replication, production, or proliferation. The production of elastin and collagen dermal connective fibers slows, and, with age, the regenerative rates of GAGs become delayed. These biological issues can be exacerbated by extrinsic factors such as sun exposure, pollutants, and various other factors. Growth factor-based products have become important topical treatment modalities for addressing signs of skin aging such as fine lines, deep wrinkles, dryness, laxity, and textural irregularities. Objective: The aim of a 12-week clinical trial of a growth factor composition was to assess its effectiveness at restoring skin health through dermal and epidermal restructuring of aged skin. Results: Data from expert grading, and from corneometer and cutometer evaluations, as well as 2D and 3D image analysis, reflected significant improvements in facial skin appearance, firmness, elasticity, and hydration. Elements that improved most dramatically in investigators’ assessments included radiance, firmness, tactile elasticity, textural smoothness, overall appearance, and crow’s feet. Ultrasound imaging showed continual increases in dermal and epidermal restructuring throughout the study duration. Subject assessments reflected positive product tolerability and positive perception across a broad range of efficacy attributes through 12 weeks of usage. Conclusion: The results verified the ability of a multi-modal plant and enzymatically derived growth factor-based product to achieve skin rejuvenation improvements by stimulating dermal ECM and fibrous tissue regeneration to reduce fine lines and coarse wrinkles, and improve skin firmness and elasticity, while restoring skin to a properly hydrated state. Abstract:Skin aging is a biological process leading to visible skin alterations. The mechanism of action, clinical efficacy and tolerance of a novel anti-wrinkle technology were evaluated in two skin care products formulated for different skin types. Two single-arm monocentric, open-label observational clinical studies, which were 56 days long, evaluated a cream-gel (n = 30) and a cream (n = 33) on the face and neck. Morphometric analyses of five types of wrinkles were performed at 0, 7, 28 and 56 days. Structural changes in extracellular matrix (ECM) including collagen, elastin and hyaluronic acid (HA) were visualized and quantified by histochemical imaging after daily treatment of skin explants for 6 days. Protein and gene expression related to barrier and hydration were analyzed using ELISA and qRT-PCR, respectively, in a reconstituted human skin model treated daily for 48 h. A decrease in wrinkle dimensions was found in the majority of parameters after 28 days of treatment. Collagen, elastin, HA, procollagen type I, hyaluronan synthases, HAS2 and HAS3 were all stimulated. Based on significant and consistent changes in our investigations, we conclude that the underlying mechanism of action of the novel anti-wrinkle technology could be the remodeling of dermal ECM, and both the test formulations were efficacious and well tolerated.

Phenytoin in pregnancy: a review of the reported risks.

Abstract:Over the last 12 years several studies have found that the frequency of certain anomalies in children born to mothers treated with antiepileptic drugs during pregnancy has almost doubled. In 1968 Meadow reported certain malformations that suggest a congenital syndrome involving craniofacial abnormalities and that add to the high level of suspicion pointing to phenytoin. Moreover, multi-drug antiepileptics including phenytoin have been reported to induce carcinogenic damage and coagulation defects. In such instances it is always very difficult to distinguish the individual toxic effects of each component. Nevertheless, the possible role of phenytoin in the development of these diseases must be investigated. In addition, the eventual role of the pathologic state itself, epilepsy, cannot be disregarded. Two main aspects must be considered: the disease and its severity which determines the necessity of high dosages of antiepileptic drugs. The administration of phenytoin to pregnant epileptic patients is clearly questionable. Thus the aim of this paper is to evaluate the risks of this drug and compare them with its expected therapeutic benefits.

Abstract:The aim of this clinical study was to demonstrate that through a micrograft of viable adipose tissue cells microfiltered at 50 microns to exclude fibrous shoots and cell debris in a suspension of cross-linked hyaluronic acid, we were able to improve visible imperfections of the dermis and to improve clinically observable wrinkles, with a beneficial effect also in the extracellular matrix (ECM). Background and Objectives: With the passage of time, the aging process begins, resulting in a progressive impairment of tissue homeostasis. The main reason for the formation of wrinkles is the involution of the papillary dermis, as well as the loss of stem cell niches with compromise of the extra-cytoplasmic matrix (ECM), and the loss of hyaluronic acid, which helps to maintain the shape and resistance and that is contained in the connective tissue. Materials and Methods: This study involved 14 female patients who underwent dermal wrinkle correction and bio-regeneration over the entire facial area through a suspension containing 1.0 mL of viable micrografts from adipose tissue in a 1.0 mL cross-linked hyaluronic acid. To verify the improvement of the anatomical area concerned over time, the various degrees of correction obtained for wrinkles, and in general for texture, were objectively evaluated by using a Numeric Rating scale (NRS) 10-0, a modified Vancouver scale and a Berardesca scale. Results: The Berardesca, NRS and Modified Vancouver scales showed that with this technique it was possible to obtain excellent results both when the suspension was injected into wrinkles with the linear retrograde technique, and when it was injected with the micropomphs technique to correct furrows, with the intent to revitalize the tissue through progenitors with adult stemness markers. Conclusions: The combination of microfragmented and microfiltered adipose tissue and cross-linked hyaluronic acid at 50 microns is safe new method to treat soft tissue defects such as deep wrinkles.

Abstract:that an embolectomy or thrombolytic treatment would not salvage the kidney. Anticoagulant treatment was therefore commenced. An electrocardiogram confirmed sinus rhythm and an echocardiogram showed no evidence of mural thrombus. On review at 3 months she was normotensive, and repeat isotope renogram confirmed complete left renal infarction. This case emphasizes the need to consider renal colic as a symptom and not a diagnosis. Although rare, renal artery embolism should be considered in the differential diagnosis of renal colic, particularly when there is a history of cardiac disease, so that appropriate management may be given.
A Double-Blind, 12-Week Study to Evaluate the Antiaging Efficacy of a Cream Containing the NFκB Inhibitor 4-Hexyl-1, 3-Phenylenediol and Ascorbic Acid-2 Glucoside in Adult Females

Abstract:The 5 main physical manifestations of aged skin are wrinkles, uneven tone, brown spots, loss of elasticity, and dryness. One mechanism resulting in these physical manifestations is increased activity of the nuclear factor kappa B (NFκB) protein. This 12-week, double-blind, placebo-controlled, randomized split-face study compared the antiaging effect and safety of a face cream containing 4-Hexyl-1, 3-phenylenediol, an NFκB inhibitor, and ascorbic acid-2 glucoside versus placebo in adult females aged 45-70 years old. Subjects (n=42) applied active treatment or placebo to the same half face twice daily at home for 12 weeks. Clinical evaluation was carried out by a dermatologist. Subjects carried out similar self-grading assessments. Colorimetric measurements analyzed skin color, and biomechanical skin properties were evaluated. Clinical grading showed that most wrinkle parameters were significantly improved after 8 weeks of active treatment compared with baseline and placebo (P≤.05), with improvements maintained after 12 weeks. Only Marionette wrinkles did not show a significant improvement. Brown spots (color intensity/number), overall photodamage, and most complexion parameters improved significantly after 8 and 12 weeks compared with baseline and placebo (P≤.05). Self-grading yielded similar results compared with baseline. Self-grading did not demonstrate improvements with active treatment versus placebo, except for skin firmness at 8 and 12 weeks (P≤.05). A significant difference was seen with active treatment compared with placebo in all colorimetric parameters (L*, b*, and ITA°) after 8 weeks, and in spot coloration (b*) after 12 weeks (P<.05). Improvements in skin elasticity were not significantly different between treatments. Overall tolerability of active treatment was judged as good. In conclusion, a cream containing 4-Hexyl-1, 3-phenylenediol and ascorbic acid-2 glucoside improves the clinical appearance of aged skin, validating the potential use of NFκB inhibition as a novel, effective method of topical antiaging. Novel Strategy for Strengthening Dermatoporotic Skin by Managing Cellular Senescence

Abstract:Background: Dermatoporosis (DP) is a condition associated with thinning skin layers and resultant fragility. Much of the thinning is related to fibroblast dysfunction, production of destructive inflammatory cytokines, breakdown of the extracellular matrix (ECM), and weakening of the dermo-epidermal junction. A major contributor to this change in the ECM milieu, previously under-considered, is cellular senescence, particularly involving the papillary dermal fibroblasts. Methods: A series of experiments were undertaken to explore the impact of a combination of known actives on senescent cell status. Human keratinocytes and fibroblasts were cultured, and cytotoxicity tests were performed to determine the ideal concentration to avoid cell toxicity. Microdoses of Centella asiatica (0.005%) and mandelic acid (0.05%) were found to be ideal in avoiding any cytotoxicity. However, the challenge was then to assess the efficacy of these actives in this microdosed form. After exposing the cells to the compounds, RNA was isolated and sequenced. Moreover, a well-described ex vivo model using photodamaged skin was subjected to immunofluorescence to identify senescent cells (via p16INK4a), particularly in the papillary dermis, using the microdose formulation compared to untreated skin. In addition, JAG/NOTCH expression in the epidermal basal cells was evaluated to further understand the cellular senescence signaling mechanism. Results: Microdosing these two well-known agents had surprisingly significant synergistic effects in vitro, decreasing senescence-associated secretory phenotype (SASP) cytokines and the associated inflammation involved in the process. The ex vivo model revealed a significant (P<0.05) decrease in senescent cells in the papillary dermis and a significant increase (P<0.001) of JAG/NOTCH expression in the basal cells of the epidermis. Conclusion: Using microdoses of two known agents, a novel approach produced an unexpected effect of reversal of dermal senescent cells and promoting an anti-inflammatory milieu. A gene expression analysis of the individual and combined actives validated these observations, followed by full formulation testing in an ex vivo model. The approach of limiting cellular senescence in dermal fibroblasts for managing DP is novel and provides an exciting new direction to address dermatoporosis. Clinical studies will follow. Double‐blind, vehicle‐controlled clinical investigation of peptide OS‐01 for skin rejuvenation

Abstract:Introduction Senescent cells contribute to age‐related tissue deterioration, including the skin, which plays important roles in overall health and social interactions. This study aimed to assess the effects of the senotherapeutic peptide, OS‐01 (a.k.a. Pep 14), on skin. Methods A 12‐week split‐face, double‐blinded, vehicle‐controlled study involving 22 participants was conducted. The OS‐01‐containing formulation was applied to one side of the face, while the other side received an identical control formulation lacking the peptide. Skin characteristics were assessed using instrumental measurements, expert clinical grading, and subjective questionnaires. Results Results showed that the OS‐01 formulation significantly improved one aspect of skin barrier function, as evidenced by reduced trans‐epidermal water loss compared to both baseline and vehicle control. Expert grading and Antera 3D image analysis revealed a reduction in wrinkle appearance and indentation in the periorbital area, and improved skin texture and radiance on both sides of the face, with the OS‐01‐containing formulation demonstrating superior results. Participants also perceived improvements in skin hydration, smoothness, radiance, and overall appearance. Conclusion The findings suggest that the OS‐01 formulation promotes skin health by strengthening the skin barrier, protecting against dehydration, reducing the appearance of wrinkles, and improving skin texture and radiance. These effects are likely attributed to the senotherapeutic properties of OS‐01 in reducing cellular senescence and its associated detrimental effects.
