Abstract:Background: Limited evidence is available regarding the effects of insoles on pediatric flexible flatfoot because of the heterogeneity and low methodological quality of previous studies. The purpose of this prospective trial is to examine the short-term effects of customized arch support insoles on symptomatic flexible flatfoot in children by using the International Classification of Functioning, randomized controlled Disability, and Health (ICF) framework. Methods: This study was conducted in a rehabilitation outpatient clinic of a teaching hospital. Fifty-two children with symptomatic flexible flatfoot were included. The children in the treatment group wore customized arch support insoles for 12 weeks, whereas those in the control group did not wear the insoles. Both clinical and radiographic measurements, including the navicular drop, foot posture index, Beighton hypermobility score, talonavicular coverage angle, calcaneal inclination angle, and calcaneal-first metatarsal angle, were used for diagnosing flexible flatfoot. Physical activity (10-m normal and fast walking, stair ascent, stair descent, and chair rising), physical function, and psychometric properties (Pediatric Outcome Data Collection Instrument and Pediatric Quality of Life Inventory) were evaluated at the baseline and 12 weeks after the intervention. Results: Compared with the control group, the treatment group exhibited significant improvement in pain/comfort (P = .048), physical health (P = .035), stair ascent time (P = .015), upper extremity and physical function (P = .016), and transfer and basic mobility (P = .042) during the intervention period. Conclusion: Children with flexible flatfoot who wore customized arch support insoles for 12 weeks exhibited significantly improved pain/comfort, physical health, stair ascent time, upper extremity and physical function, and transfer and basic mobility. These variables belong to the domains of body functions and structures and activity and participation in the ICF framework. However, because the groups were not comparable, additional studies with larger sample sizes should be conducted.

Abstract:This study aimed to investigate the effect of custom-made orthotic insoles on the lower limb biomechanics in children with flexible flat feet. A sample of 27 children (19 boys and eight girls) aged 7 to 13 years old with flexible flat feet participated in the study. Each custom-made orthotic insole was individually designed based on the foot of each subject, using ethylene-vinyl acetate (EVA) material with medium density. The study was conducted using a gait analysis laboratory, and participants were required to walk randomly at a self-selected speed with one of three insole conditions (custom-made orthotic insole, flat insole, and barefoot). The data was processed using Visual 3D software and analyzed by SPSS software. The results showed that using custom-made orthotic insoles resulted in a significant reduction in the maximum ankle inversion angle and ankle inversion moment while increasing the knee adduction moment during the stance phase when compared to wearing either flat insoles or being barefoot (p<0.05). This study found that custom-made orthotic insoles significantly altered the biomechanical characteristics of the lower limbs of children with flexible flat feet, reducing the movement of the ankle joint in the frontal plane, thereby enhancing ankle joint stability, but may increase knee joint load.

Abstract:System-level design issues become critical as implementation technology evolves toward increasingly complex integrated circuits and the time-to-market pressure continues relentlessly. To cope with these issues, new methodologies that emphasize re-use at all levels of abstraction are a "must", and this is a major focus of our work in the Gigascale Silicon Research Center. We present some important concepts for system design that are likely to provide at least some of the gains in productivity postulated above. In particular, we focus on a method that separates parts of the design process and makes them nearly independent so that complexity could be mastered. In this domain, architecture-function co-design and communication-based design are introduced and motivated. Platforms are essential elements of this design paradigm. We define system platforms and we argue about their use and relevance. Then we present an application of the design methodology to the design of wireless systems. Finally, we present a new approach to platform-based design called modern embedded systems, compilers, architectures and languages, based on highly concurrent and software programmable architectures and associated design tools.

Abstract:Sir, Aspirin resistance is a phenomenon where the expected inhibition of platelet responses is not obtained as evaluated by different biological tests [1]. In addition to non-compliance and other patient-related factors, one of the main reasons for aspirin resistance is its inability to inhibit thromboxane A2 (TXA2) biosynthesis in vivo. Many studies have also shown that patients with aspirin resistance are more likely to have an increased rate of recurrence of vascular events [2,3]. Interestingly, in a recent systematic review by Krasopoulos et al., the relationship between resistance to aspirin and a history of renal impairment was observed (P < 0.03) [4]. This was considered as possibly a chance finding, mainly because of lack of substantial data. However, an abnormality of platelet arachidonic acid metabolism has been well documented to exist in patients with renal impairment [5]. This leads to altered thromboxane synthesis that is a key factor for the development of resistance to aspirin. Initially thought to be due to a ‘functional cyclo-oxygenase defect’, it is now considered to be due to the increased activity of phospholipase A2 in the platelets of patients with uraemia [5,6]. Thromboxane has also been shown to play an important role in the physiological function of the kidney, and TXA2 receptors have been shown to exist in renal vasculature and other nephron segments in animal models [7,8]. Various studies have shown that TXA2 plays a key role in the regulation of renal haemodynamics mainly acting in conjunction with angiotensin II. TXA2, in addition to angiotensin II and arginine–vasopressin constrict larger vessels within the renal vascular tree via activation of a rho-associated kinase pathway [9]. Thromboxane receptor knockout mice demonstrated reduced renal blood flow and increased filtration fraction and renal vascular resistance, despite normal basal mean arterial blood pressure and glomerular filtration rate [10]. Enhanced production of thromboxane in the kidney has been demonstrated in several diseases including lupus nephritis, ureteral obstruction and nephrotoxic renal injury [11,12,13]. In a normal kidney, the production of TXA2 and prostaglandin I2 is well controlled, and the balance between them is important in maintaining homeostasis in vivo. In patients with the above conditions, however, TXA2 synthesis is higher compared to that of prostaglandin I2. The administration of thromboxane antagonists decreased the severity of these diseases, supporting the important role of thromboxane in their pathogenesis. Kwag et al. demonstrated that dietary vitamin E decreased the elevated phospholipase A2 in the kidney tissues of diabetic rats and improved the prostaglandin I2/TXA2 balance in the kidney microsomes thus improving vascular complications [14]. Chronic kidney disease is now recognized as an independent risk factor for cardiovascular events, and cardiovascular disease is the major cause of mortality in patients with the disease [15]. Possibly, the increased aspirin resistance in patients with renal failure may indicate that a similar vascular pathology, involving among others thromboxane, exists in these two different vascular beds. More work on the thromboxane pathway is required in the patients with renal impairment, who develop recurrent cardiovascular events, despite being on aspirin. This would pave the way for novel treatments that would help in preventing the progression of both the renal and cardiovascular pathologies. Conflict of interest statement. None declared.
[Multiple myeloma in renal transplant recipients].

Abstract:Severe renal failure (RF) may be the first and only clinical manifestation of multiple myeloma (MM). Occasionally the disease remains long unrecognized and the patients receive renal function replacement therapy, including renal transplantation (RT). To treat MM in renal transplant recipients is a complex medical and ethical problem. The paper presents the authors' experience in treating 3 patients with MM diagnosed after RT and evolving transplant lesion. Various morphological types of grafted kidney lesion were detected. These included fibrillar glomerulonephritis, cast nephropathy, and the latter concurrent with light-chain deposition disease. RF most rapidly progressed in cast nephropathy. The natural history of the disease was unfavorable in all patients; VAD and PAD chemotherapy programs proved to be ineffective. It is concluded that RT should not be performed in patients with extended-stage MM due to the fact that there is a considerable risk for renal transplant lesion and severe infectious complications that may occur during chemotherapy. Blood and urine immunochemical studies should be conducted in all the patients who are to undergo RT.

Abstract:Flatfoot is characterized by the collapse of the medial longitudinal arch, eversion of the rearfoot and abduction of the loaded forefoot. Orthopedic insoles are the frequently recommended treatment to support the arch of the foot, adjust the structure of the foot, reduce pain, improve stability and new techniques have been applied to the design of orthopedic insoles in recent years. However, the effectiveness of orthopedic insoles in different motions is still debated from the perspective of biomechanics. Therefore, this study aimed to explore the impact of orthopedic insoles on the kinematics and kinetics of lower limb motion, and to verify effectiveness and propose possible future research directions. We conducted a literature search across three databases employing Boolean operations and filtered results based on eligibility criteria. A total of 671 relevant literature were searched in this review, and 19 literature meeting the requirements were finally included. The results showed that: 1) orthopedic insoles were effective when patients walk, run and jump from the perspective of biomechanics; 2) orthopedic insoles had different result on the change of ankle sagittal angle, moment and peak pressure in the metatarsal region; 3) Whether the effect of insoles, which uses new techniques such as different 3D printed technologies and adds various accessories, can be further improved remains to be further studied; 4) Follow-up studies can pay more attention to the differences between diverse populations, increase the breadth of running and jumping and other movements research and long-term intervention.

Abstract:Background: Flatfoot (pes planus) is a common foot deformity, and its causes are mainly related to age, gender, weight, and genetics. Previous studies have shown that custom-made insoles could have a positive effect in improving plantar pressure and symptoms in individuals with flexible flatfeet, but it remains to be explored whether they can still show benefits in daily walking on different slopes. Objective: This study aims to investigate a custom-made insole based on plantar pressure redistribution and to verify its effectiveness by gait analysis on different slopes. Methods: We recruited 10 subjects and compared the peak pressure and impulse in each area between custom-made insole (CI) and ordinary insole (OI) groups. Results: The results illustrate that CI raises the pressure in T area, improves the ability of the subjects to move forward in the slope walking, which was beneficial to gait stability. Conclusion: The redistribution of pressure in MF and MH area is promoted to provide active protection for subjects. Meanwhile, CI could decrease the impulse in MF area during uphill and level walking, which effectively reduces the accumulation of fatigue during gait. Moreover, avoiding downhill walking could be able to protect foot from injury in daily life.

Abstract:Rickettsiae tend to have a rapid decrease of viability outside living cells. Therefore, the transport of samples containing viable rickettsiae for culturing in cell culture for diagnostic purposes is challenging. The viability of rickettsiae in different transport media (commercially available transport medium COPAN "UTM-RT transport medium for viruses, chlamydia, mycoplasma, and ureaplasma," minimal essential medium (MEM) with and without 10% foetal calf serum) at various time points at 4 °C and at ambient temperature (22 °C) was compared. Rickettsia honei was used as model organism. After 2 weeks of storage at room temperature, no viable rickettsiae were detectable any more while storage at 4 °C kept rickettsiae viable for up to 4 weeks. The commercially available COPAN medium showed similarly good or slightly better stabilizing effects on rickettsiae compared with MEM + 10% foetal calf serum, pure MEM demonstrated the poorest results. It is important to transport and store media with potentially rickettsiae-containing samples at 4 °C to prevent inactivation. MEM + 10% foetal calf serum can be used if no commercial medium is available with similarly good results.
Effectiveness of Gait Plate Insole and Lateral Sole Wedged Shoes on Foot Progression Angle in Children with In-Toeing Gait: A Prospective Randomized Control Trial

Abstract:Background In-toeing gait is defined as the internal rotation of the long axis of the foot to the line of progression. Although most of the time it is corrected spontaneously but sometimes causes torsional misalignment syndrome and maybe patellofemoral instability and arthritis of the knee. Research question What is the effect of gait plate insoles and lateral sole wedged shoes on foot progression angle (FPA) in children with in-toeing? Method In this study, a randomized control trial was conducted with 11 participants (18 feet) who were girls aged between seven and ten years old. They were randomly assigned to either the gait plate group (6 girls, 11 feet) or the lateral sole wedge group (5 girls, 11 feet). The foot progression angle was measured using the RS scanner pressure platform before and after four weeks of intervention, both with and without interventions. The interaction effects of time and group on outcomes were analyzed using Mixed ANOVA and post-hoc complementary tests with a confidence interval of 95%. Results It was observed that none of the interventions had an immediate impact on the FPA ( P >0.05). However, after four weeks, the FPA increased by 9.96 degrees with the lateral sole wedge and by 3.51 degrees with the gate plate insole. During the immediate and short-term evaluation, no significant difference was noticed between the two groups ( P >0.05). However, there was a large effect size (eta square = 0.269) observed in the time group interaction between the two groups ( P =0.028). Significance Conservative methods like using a gait plate insole or modifying shoes with a lateral sole wedge can improve the foot progression angle in children with in-toeing gait. However, longer studies with larger sample sizes are needed to reach a conclusion.

Abstract:Based on the Maximum Degree Construction algorithm, a new select algorithm is proposed in this paper. In the algorithm, each node and its neighbors issue the certificates each other to generate the local In-degree and Out-degree certificate repository. Similar to the ant colony algorithm, it finds the certificate chain between the source node and destination node by selecting the node of the maximum certificated times from the beginning. The algorithm reduces the complexity of the selection, provides a guarantee to find the certificate chain, and saves the spending of space as well. Next, this paper gives the simulation of the algorithm and the simulated results show that this is an optimized select algorithm for local certificate repository.

Abstract:Multi-layer optical networks have recently evolved towards IP-over-WDM networks. Therein, in order to avoid protection/restoration redundancies against either single or multiple failures, synergies need to be developed between IP and optical layers in order to reduce the costs and the energy consumption of the future IP-over-WDM networks. We propose two new optimization models. The first one is an enhanced cutset model, relying on a column generation reformulation. The second one is a path model, based on a multi-flow formulation. Both models can solve exactly most benchmark instances, which were only solved heuristically so far.
Smart insole-based analysis of gait biomechanics for insoles in patients with flatfoot

Abstract:Background: Assessing the effect of insoles on gait biomechanics and foot comfort remains challenging. Our novel in-insole-type wearable sensor device (smart insole) enables accurate quantitative evaluation of gait parameters without affecting the subject's foot comfort. Research question: What are the effects of insoles on gait biomechanics and foot comfort in patients with flatfoot, as evaluated using a novel smart insole? Methods: Thirty-three subjects with 61 flatfeet were recruited. Three different types of prefabricated insoles were tested: a control insole as an experimental control, a flat insole with only cushion pads for the shock absorbing function, and an arch support insole with both cushioning pads and arch support functions. Gait parameters and visual analog scale (VAS) scores for foot comfort were measured during 30 m of straight walking with each insole incorporating the wearable sensor device. The differences in gait parameters and foot comfort between the flat and arch support insoles relative to the control insole were analyzed. Additionally, the correlations between gait parameters and foot comfort were evaluated. Results: Maximum plantarflexion angle significantly decreased (p = 0.03) and the toe-out angle significantly increased (p < 0.01) with arch support insoles compared to flat insoles. Significantly better foot comfort was demonstrated when walking with arch support insoles than with flat insoles (p < 0.01). The only gait parameter correlated with foot comfort was foot lift height while walking with an arch support insole (r = -0.45, p < 0.01). Significance: A novel smart insole revealed that foot lift height was a key gait parameter for determining foot comfort while walking with an arch support insole. Our findings provide important evidence for selecting a comfortable flatfoot insole based on gait data measured using a smart insole.

Abstract:Temporal constraints on the perception of variable-size speech fragments produced by interruption rates between 0.5 and 16 Hz were investigated by contrasting the intelligibility of gated sentences with and without silent intervals. Concatenation of consecutive speech fragments produced a significant decrease in intelligibility at 2 and 4 Hz, while having little effect at lower and higher rates. Consistent with previous studies, these findings indicate that (1) syllable-sized intervals associated with intermediate-rate interruptions are more susceptible to temporal distortions than the longer word-size or shorter phoneme-size intervals and (2) suggest qualitative differences in underlying perceptual processes at different rates.

Abstract:The adenine analogue, 2-aminopurine (2-AP), induces G2 arrest in the human promonocyte-macrophage cell line, U937. The arrest is reversible and cells enter mitosis to resume normal logarithmic growth upon removal of the drug. These physiological changes are accompanied by markedly stimulated expression of eukaryotic gene constructs stably integrated in the chromosomes or introduced into the cells by transient transfection. Induction by 2-AP has two components: one involves increased transcription of the introduced genes as shown by run-on transcription experiments. The other involves markedly increased mRNA half-life that affects mRNA transcribed from transiently transfected DNA but apparently does not affect mRNA transcribed from the same chromosomally integrated sequences. Together, these two components could account for the 100- to 1000-fold inductions observed with various transfected gene constructs reported here and elsewhere. Maximum induction by 2-AP is promiscuous with respect to eukaryotic promoter origins or sequences, but appears to require a minimum of two such promoter elements. Thus, G2 cell cycle arrest induced by 2-AP is also associated with transcriptional and post-transcriptional alterations in gene expression. The data also suggest that transiently transfected DNAs may enter spatial or biochemical compartments of the nucleus that are different from those of normal genes in their native locations. These differences may affect the abundance and fate of the transcribed mRNA and, in some circumstances, introduce serious discordances into studies of gene regulation.
The effects of foot orthoses on radiological parameters and pain in children with flexible flat feet: a systematic review and meta-analysis

Abstract:Objective: This study aimed to investigate the impact of foot orthoses on foot radiological parameters and pain in children diagnosed with flexible flatfoot. Methods: A comprehensive search was conducted across several databases, including PubMed, Web of Science, EMBASE, Cochrane Library, and EBSCO, covering publications from the inception of each database up to 8 June 2024. The study focused on randomized controlled trials investigating the use of foot orthoses for treating flexible flat feet in children. Four researchers independently reviewed the identified literature, extracted relevant data, assessed the quality of the studies, and performed statistical analyses using RevMan 5.4 software. Results: Six studies involving 297 participants were included. The methodological quality of the included literature ranged from moderate to high. Radiological parameters of the foot improved significantly in older children with flexible flat feet following foot orthotic intervention compared to controls, particularly in the lateral talar-first metatarsal angle [mean difference (MD) = -2.76, 95% confidence interval (95% CI) -4.30 to -1.21, p = 0.0005], lateral talo-heel angle (MD = -5.14, 95% CI -7.76 to -2.52, p = 0.0001) and calcaneal pitch angle (MD = 1.79, 95% CI 0.88-2.69, p = 0.0001). These differences were statistically significant. Additionally, foot orthoses significantly improved the ankle internal rotation angle and reduced foot pain in children with symptomatic flexible flatfoot (MD = -2.51, 95% CI -4.94 to -0.07, p = 0.04). Conclusion: The use of foot orthoses positively impacts the improvement of radiological parameters of the foot and reduces pain in older children with flexible flat feet. However, in younger children with flexible flat feet, the improvement from foot orthoses was not significant, likely due to challenges in radiological measurements caused by the underdevelopment of the ossification centers in the foot. Further studies are needed. Consequently, the results of this meta-analysis support the implementation of an early intervention strategy using foot orthoses for the management of symptomatic flat feet in older children. Abstract:Insoles play an important role in the conservative treatment of functional flat foot. The features of 3D-printed insoles are high customizability, low cost, and rapid prototyping. However, different designed insoles tend to have different effects. The study aimed to use 3D printing technology to fabricate three different kinds of designed insoles in order to compare the biomechanical effects on the lower extremities in flat foot participants. Ten participants with functional flat foot were recruited for this study. Data were recorded via a Vicon motion capture system and force plates during walking under four conditions: without insoles (shoe condition), with auto-scan insoles (scan condition), with total contact insoles (total condition), and with 5-mm wedge added total contact insoles (wedge condition). The navicular height, eversion and dorsiflexion angles of the ankle joint, eversion moment of the ankle joint, and adduction moment of the knee joint were analyzed, and comfort scales were recorded after finishing the analysis. Compared to the shoe condition, all three 3D printed insoles could increase the navicular height and ankle dorsiflexion angle and improve comfort. Among the three insoles, the wedge condition was the most efficient in navicular height support and increasing the ankle dorsiflexion angle.

Abstract:Objective: To observe the effects of scoliosis-specific exercise therapy combined with braces and orthopedic insoles on improved spinal deformity and walking ability in adolescents with idiopathic scoliosis (AIS). Method: From September 2019 to September 2020, 60 outpatient AIS patients were distributed into brace group (n = 30) at random and brace combined orthopedic insole group (n = 30). Both groups underwent brace dryness, and the observation group used scoliosis-specific exercise therapy combined with brace therapy, and on this basis, orthopedic insole intervention was added for 8 h per day for 2 months. At the same time, 20 adolescents of the same age with normal spinal development were recruited as a healthy group. GaitScan instruments were used to collect gait and plantar pressure measurements from study subjects. First, the gait and plantar pressure data of AIS patients and healthy groups were compared horizontally to ascertain the abnormal indicators, and then the spinal deformity and the above abnormal indicators were compared between the brace group and the brace combined orthopedic insole group. Outcome: The plantar pressure center drift index (CPEI) in the AIS group was higher than that in the healthy group (F = 3.120, P < 0.05), and there were significant differences in the ratio of medial and lateral heel pressure (M/l) and total foot pressure (P < 0.05) between the AIS group and the healthy group, and no noticeable variations were found in the support phase period, walking speed, and proportion of each phase (P > 0.05). After treatment, the Cobb angle was significantly reduced in both the brace group and the brace combined with orthopedic insole group (P < 0.05), and there was no significant difference between the groups (P > 0.05). There were no significant changes in the pressure ratio of CPEI, M/l and bilateral full foot in the brace group (P > 0.05). The CPEI decreased in the brace combined with orthopedic insole group (P < 0.05), and the pressure ratio of M/l and bilateral full foot tended to 1 (P < 0.05), and was better than that in the brace group (P < 0.05). Conclusion: Patients with AIS may have local and worldwide asymmetric changes in plantar pressure distribution. The addition of orthopedic insoles has limited effect on improving scoliosis deformity in patients with AIS, but it can effectively improve the abnormal biomechanics of patients with AIS, so that the patient's force tends to be balanced.
