Abstract:Purpose:To investigate the clinical value of machine learning in predicting osteoporosis based on demographic and routine biochemical indicators.Methods:The demographic and routine biochemical indicators of 2053 healthy subjects(906 females and 1 147 males)over 50 years who underwent low-dose CT scans were retrospectively analyzed.Diagnosis of osteoporosis is based on volumetric bone mineral density concluded from quantitative CT.The subjects were divided into training set and test set in a ratio of 7∶3.Five different algorithms were used to build the model,including logistic regression,support vector machine,decision tree,random forest and multilayer perceptron.Results:In women,the random forest was the best model in both training(AUC=0.90)and test set(AUC=0.80),with age as the most important feature,followed by alkaline phosphatase,triglycerides,and body mass index.In men,the logistic regression performed best in the test set(AUC=0.81),and the importance of demographic characteristics was higher than that of routine biochemical indicators.Conclusion:The sex-specific machine learning model based on demographic and routine biochemical indicators provides a possibility for osteoporosis screening in clinical scenarios such as physical examination.
A hierarchical opportunistic screening model for osteoporosis using machine learning applied to clinical data and CT images

Abstract:Abstract Background Osteoporosis is a common metabolic skeletal disease and usually lacks obvious symptoms. Many individuals are not diagnosed until osteoporotic fractures occur. Bone mineral density (BMD) measured by dual-energy X-ray absorptiometry (DXA) is the gold standard for osteoporosis detection. However, only a limited percentage of people with osteoporosis risks undergo the DXA test. As a result, it is vital to develop methods to identify individuals at-risk based on methods other than DXA. Results We proposed a hierarchical model with three layers to detect osteoporosis using clinical data (including demographic characteristics and routine laboratory tests data) and CT images covering lumbar vertebral bodies rather than DXA data via machine learning. 2210 individuals over age 40 were collected retrospectively, among which 246 individuals’ clinical data and CT images are both available. Irrelevant and redundant features were removed via statistical analysis. Consequently, 28 features, including 16 clinical data and 12 texture features demonstrated statistically significant differences ( p < 0.05) between osteoporosis and normal groups. Six machine learning algorithms including logistic regression (LR), support vector machine with radial-basis function kernel, artificial neural network, random forests, eXtreme Gradient Boosting and Stacking that combined the above five classifiers were employed as classifiers to assess the performances of the model. Furthermore, to diminish the influence of data partitioning, the dataset was randomly split into training and test set with stratified sampling repeated five times. The results demonstrated that the hierarchical model based on LR showed better performances with an area under the receiver operating characteristic curve of 0.818, 0.838, and 0.962 for three layers, respectively in distinguishing individuals with osteoporosis and normal BMD. Conclusions The proposed model showed great potential in opportunistic screening for osteoporosis without additional expense. It is hoped that this model could serve to detect osteoporosis as early as possible and thereby prevent serious complications of osteoporosis, such as osteoporosis fractures.

Abstract:Screening for osteoporosis is crucial for early detection and prevention, yet it faces challenges due to the low accuracy of calcaneal quantitative ultrasound (QUS) and limited access to dual-energy X-ray absorptiometry (DXA) scans. Recent advances in AI offer a promising solution through opportunistic screening using existing medical images. This study aims to utilize deep learning techniques to develop a model that analyzes chest X-ray (CXR) images for osteoporosis screening. This study included the AI model development stage and the clinical validation stage. In the AI model development stage, the combined dataset of 5122 paired CXR images and DXA reports from the patients aged 20 to 98 years at a medical center was collected. The images were enhanced and filtered for hardware retention such as pedicle screws, bone cement, artificial intervertebral discs or severe deformity in target level of T12 and L1. The dataset was then separated into training, validating, and testing datasets for model training and performance validation. In the clinical validation stage, we collected 440 paired CXR images and DXA reports from both the TCVGH and Joy Clinic, including 304 pared data from TCVGH and 136 paired data from Joy Clinic. The pre-clinical test yielded an area under the curve (AUC) of 0.940, while the clinical validation showed an AUC of 0.946. Pearson's correlation coefficient was 0.88. The model demonstrated an overall accuracy, sensitivity, and specificity of 89.0%, 88.7%, and 89.4%, respectively. This study proposes an AI model for opportunistic osteoporosis screening through CXR, demonstrating good performance and suggesting its potential for broad adoption in preliminary screening among high-risk populations.

Abstract:Rationale and Objectives To develop and validate a predictive model for osteoporosis and osteopenia prediction by fusing deep transfer learning (DTL) features and classical radiomics features based on single-source dual-energy computed tomography (CT) virtual monochromatic imaging. Methods A total of 606 lumbar vertebrae with dual-energy CT imaging and quantitative CT (QCT) evaluation were included in the retrospective study and randomly divided into the training ( n = 424) and validation ( n = 182) cohorts. Radiomics features and DTL features were extracted from 70-keV monochromatic CT images, followed by feature selection and model construction, radiomics and DTL features models were established. Then, we integrated the selected two types of features into a features fusion model. We developed a two-level classifier for the hierarchical pairwise classification of each vertebra. All the vertebrae were first classified into osteoporosis and non-osteoporosis groups, then non-osteoporosis group was classified into osteopenia and normal groups. QCT was used as reference. The predictive performance and clinical usefulness of three models were evaluated and compared. Results The area under the curve (AUC) of the features fusion, radiomics and DTL models for the classification between osteoporosis and non-osteoporosis were 0.981, 0.999, 0.997 in the training cohort and 0.979, 0.943, 0.848 in the validation cohort. Furthermore, the AUCs of the previously mentioned models for the differentiation between osteopenia and normal were 0.994, 0.971, 0.996 in the training cohort and 0.990, 0.968, 0.908 in the validation cohort. The overall accuracy of the previously mentioned models for two-level classifications was 0.979, 0.955, 0.908 in the training cohort and 0.918, 0.885, 0.841 in the validation cohort. Decision curve analysis showed that all models had high clinical value. Conclusion The feature fusion model can be used for osteoporosis and osteopenia prediction with improved predictive ability over a radiomics model or a DTL model alone.

Proximal Humeral Bone Density Assessment and Prediction Analysis Using Machine Learning Techniques: an Innovative Approach in Medical Research

Abstract:Background: Patients with fractures of the proximal humerus often local complications and failures attributed to osteoporosis. Currently, there is a lack of straight forward screening methods for assessing the extent of local osteoporosis in the proximal humerus. This study utilizes machine learning techniques to establish a diagnostic approach for evaluating local osteoporosis by analyzing the patient's demographic data, bone density, and X-ray ratio of the proximal humerus. Methods: A cohort comprising a total of 102 hospitalized patients admitted during the period spanning from 2021 to 2023 underwent random selection procedures. Resulting in exclusion of 5 patients while enrolling 97 patients for analysis encompassing patient demographics, shoulder joint anteroposterior radiographs, and bone density information. Using the modified Tingart index methodology involving multiple measurements denoted as M1 through M4 obtained from humeral shafts. Within this cohort comprised 76 females (78.4 %) and 21 males (21.6 %), with an average age of 73.0 years (range: 43-98 years). There were 25 cases with normal bone density, 35 with osteopenia, and 37 with osteoporosis. Machine learning techniques were used to randomly divide the 97 cases into training (n = 59) and validation (n = 38) sets with a ratio of 6:4 using stratified random sampling. A decision tree model was built in the training set, and significant diagnostic indicators were selected, with the performance of the decision tree evaluated using the validation set. Multinomial logistic regression methods were used to verify the strength of the relationship between the selected indicators and osteoporosis. Results: The decision tree identified significant diagnostic indicators as the humeral shaft medullary cavity ratio M2/M4, age, and gender. M2/M4 >= 1.13 can be used as an important screening criterion; M2/M4 < 1.13 was predicted as local osteoporosis; M2/M4 >= 1.13 and age >= 83 years were also predicted as osteoporosis. M2/M4 >= 1.13 and age <64 years or males aged between 64 and 83 years were predicted as the normal population; M2/M4 >= 1.13 and females aged between 64 and 83 years were predicted as having osteopenia. The decision tree's accuracy in the training set was 0.7627 (95 % CI (0.6341, 0.8638)), and its accuracy in the test set was 0.7895 (95 % CI (0.6268, 0.9045)). Multinomial logistic regression results showed that humeral shaft medullary cavity ratios M2/M4, age, and gender in X-ray images were significantly associated with the occurrence of osteoporosis. Conclusion: Utilizing X-ray data of the proximal humerus in conjunction with demographic information such as gender and age enable the prediction of localized osteoporosis, facilitating physicians' rapid comprehension of osteoporosis in patients and optimization of clinical treatment plans.
Predicting osteoporosis from kidney-ureter-bladder radiographs utilizing deep convolutional neural networks

Abstract:Osteoporosis is a common condition that can lead to fractures, mobility issues, and death. Although dual-energy X-ray absorptiometry (DXA) is the gold standard for osteoporosis, it is expensive and not widely available. In contrast, kidney-ureter-bladder (KUB) radiographs are inexpensive and frequently ordered in clinical practice. Thus, it is a potential screening tool for osteoporosis. In this study, we explored the possibility of predicting the bone mineral density (BMD) and classifying high-risk patient groups using KUB radiographs. We proposed DeepDXA-KUB, a deep learning model that predicts the BMD values of the left hip and lumbar vertebrae from an input KUB image. The datasets were obtained from Taiwanese medical centers between 2006 and 2019, using 8913 pairs of KUB radiographs and DXA examinations performed within 6 months. The images were randomly divided into training and validation sets in a 4:1 ratio. To evaluate the model's performance, we computed a confusion matrix and evaluated the sensitivity, specificity, accuracy, precision, positive predictive value, negative predictive value, F1 score, and area under the receiver operating curve (AUROC). Moderate correlations were observed between the predicted and DXA-measured BMD values, with a correlation coefficient of 0.858 for the lumbar vertebrae and 0.87 for the left hip. The model demonstrated an osteoporosis detection accuracy, sensitivity, and specificity of 84.7 %, 81.6 %, and 86.6 % for the lumbar vertebrae and 84.2 %, 91.2 %, and 81 % for the left hip, respectively. The AUROC was 0.939 for the lumbar vertebrae and 0.947 for the left hip, indicating a satisfactory performance in osteoporosis screening. The present study is the first to develop a deep learning model based on KUB radiographs to predict lumbar spine and femoral BMD. Our model demonstrated a promising correlation between the predicted and DXA-measured BMD in both the lumbar vertebrae and hip, showing great potential for the opportunistic screening of osteoporosis.

Abstract:ObjectiveTo explore the application of deep learning in patients with primary osteoporosis, and to develop a fully automatic method based on deep convolutional neural network (DCNN) for vertebral body segmentation and bone mineral density (BMD) calculation in CT images.Materials and methodsA total of 1449 patients were used for experiments and analysis in this retrospective study, who underwent spinal or abdominal CT scans for other indications between March 2018 and May 2020. All data was gathered from three different CT vendors. Among them, 586 cases were used for training, and other 863 cases were used for testing. A fully convolutional neural network, called U-Net, was employed for automated vertebral body segmentation. The manually sketched region of vertebral body was used as the ground truth for comparison. A convolutional neural network, called DenseNet-121, was applied for BMD calculation. The values post-processed by quantitative computed tomography (QCT) were identified as the standards for analysis.ResultsBased on the diversity of CT vendors, all testing cases were split into three testing cohorts: Test set 1 (n = 463), test set 2 (n = 200), and test set 3 (n = 200). Automated segmentation correlated well with manual segmentation regarding four lumbar vertebral bodies (L1–L4): the minimum average dice coefficients for three testing sets were 0.823, 0.786, and 0.782, respectively. For testing sets from different vendors, the average BMDs calculated by automated regression showed high correlation (r > 0.98) and agreement with those derived from QCT.ConclusionsA deep learning–based method could achieve fully automatic identification of osteoporosis, osteopenia, and normal bone mineral density in CT images.Key Points• Deep learning can perform accurate fully automated segmentation of lumbar vertebral body in CT images.• The average BMDs obtained by deep learning highly correlates with ones derived from QCT.• The deep learning–based method could be helpful for clinicians in opportunistic osteoporosis screening in spinal or abdominal CT scans.

Background:Despite being the gold standard for diagnosing osteoporosis, dual-energy X-ray absorptiometry (DXA) is an underutilized screening tool for osteoporosis.Objectives:This study proposed and validated a controllable feature layer of a convolutional neural network (CNN) model with a preprocessing image algorithm to classify osteoporosis and predict T-score on the proximal hip region via simple hip radiographs.Design:This was a single-center, retrospective study.Methods:An image dataset of 3460 unilateral hip images from 1730 patients (age ⩾50 years) was retrospectively collected with matched DXA assessment for T-score for the targeted proximal hip regions to train (2473 unilateral hip images from 1430 patients) and test (497 unilateral hip images from 300 patients) the proposed CNN model. All images were processed with a fully automated CNN model, X1AI-Osteo.Results:The proposed screening tool illustrated a better performance (sensitivity: 97.2%; specificity: 95.6%; positive predictive value: 95.7%; negative predictive value: 97.1%; area under the curve: 0.96) than the open-sourced CNN models in predicting osteoporosis. Moreover, when combining variables, including age, body mass index, and sex as features in the training metric, there was high consistency in the T-score on the targeted hip regions between the proposed CNN model and the DXA (r = 0.996, p < 0.001).Conclusion:The proposed CNN model may identify osteoporosis and predict T-scores on the targeted hip regions from simple hip radiographs with high accuracy, highlighting the future application for population-based opportunistic osteoporosis screening with low cost and high adaptability for a broader population at risk.Trial registration:TMU-JIRB N201909036.

Abstract:Although the number of patients with osteoporosis is increasing worldwide, diagnosis and treatment are presently inadequate. In this study, we developed a deep learning model to predict bone mineral density (BMD) and T-score from chest X-rays, which are one of the most common, easily accessible, and low-cost medical imaging examination methods. The dataset used in this study contained patients who underwent dual-energy X-ray absorptiometry (DXA) and chest radiography at six hospitals between 2010 and 2021. We trained the deep learning model through ensemble learning of chest X-rays, age, and sex to predict BMD using regression and T-score for multiclass classification. We assessed the following two metrics to evaluate the performance of the deep learning model: (1) correlation between the predicted and true BMDs and (2) consistency in the T-score between the predicted class and true class. The correlation coefficients for BMD prediction were hip = 0.75 and lumbar spine = 0.63. The areas under the curves for the T-score predictions of normal, osteopenia, and osteoporosis diagnoses were 0.89, 0.70, and 0.84, respectively. These results suggest that the proposed deep learning model may be suitable for screening patients with osteoporosis by predicting BMD and T-score from chest X-rays.

Prediction of bone mineral density in CT using deep learning with explainability

Abstract:Bone mineral density (BMD) is a key feature in diagnosing bone diseases. Although computational tomography (CT) is a common imaging modality, it seldom provides bone mineral density information in a clinic owing to technical difficulties. Thus, a dual-energy X-ray absorptiometry (DXA) is required to measure bone mineral density at the expense of additional radiation exposure. In this study, a deep learning framework was developed to estimate the bone mineral density from an axial cut of the L1 bone on computational tomography. As a result, the correlation coefficient between bone mineral density estimates and dual-energy X-ray absorptiometry bone mineral density was .90. When the samples were categorized into abnormal and normal groups using a standard (T-score =−1.0 ), the maximum F1 score in the diagnostic test was .875. In addition, it was identified using explainable artificial intelligence techniques that the network intensively sees a local area spanning tissues around the vertebral foramen. This method is well suited as an auxiliary tool in clinical practice and as an automatic screener for identifying latent patients in computational tomography databases.

Abstract:Objectives: It is feasible to evaluate bone mineral density (BMD) and detect osteoporosis through an artificial intelligence (AI)-assisted system by using quantitative computed tomography (QCT) as a reference without additional radiation exposure or cost. Methods: A deep-learning model developed based on 3312 low-dose chest computed tomography (LDCT) scans (trained with 2337 and tested with 975) achieved a mean dice similarity coefficient of 95.8% for T1-T12, L1, and L2 vertebral body (VB) segmentation on test data. We performed a model evaluation based on 4401 LDCT scans (obtained from scanners of 3 different manufacturers as external validation data). The BMD values of all individuals were extracted from three consecutive VBs: T12 to L2. Line regression and Bland‒Altman analyses were used to evaluate the overall detection performance. Sensitivity and specificity were used to evaluate the diagnostic performance for normal, osteopenia, and osteoporosis patients. Results: Compared with the QCT results as the diagnostic standard, the BMD assessed had a mean error of (- 0.28, 2.37) mg/cm3. Overall, the sensitivity of a normal diagnosis was greater than that of a diagnosis of osteopenia or osteoporosis. For the diagnosis of osteoporosis, the model achieved a sensitivity > 86% and a specificity > 98%. Conclusion: The developed tool is clinically applicable and helpful for the positioning and analysis of VBs, the measurement of BMD, and the screening of osteopenia and osteoporosis. Clinical relevance statement: The developed system achieved high accuracy for automatic opportunistic osteoporosis screening using low-dose chest CT scans and performed well on CT images collected from different scanners. Key points: Osteoporosis is a prevalent but underdiagnosed condition that can increase the risk of fractures. This system could automatically and opportunistically screen for osteoporosis using low-dose chest CT scans obtained for lung cancer screening. The developed system performed well on CT images collected from different scanners and did not differ with patient age or sex.
The Use of CT Hounsfield Unit Values to Identify the Bone Quality in Patients with Cervical Degenerative Diseases.

Abstract:Background It is well known that osteoporosis may lead to the failure of spinal surgery. As the gold standard, dual-energy X-ray absorptiometry (DXA) is used to evaluate the overall bone mineral density (BMD). Previous studies have used CT (Computed tomography) value to evaluate local bone mineral density. The objective of this study was to investigate the application value of cervical CT value in preoperative bone quality evaluation of cervical degenerative diseases. Methods A total of 939 patients who received surgical treatment for cervical degenerative diseases in our center from January 2015 to December 2017 were retrospectively reviewed. The Hounsfield unit (HU) values were measured in middle transverse CT images of the C2–C7 on the picture archiving and communication system (PACS), and the total bone mineral density T-score of L1–L4 was obtained by dual-energy X-ray absorptiometry. The changes in the HU values of C2–C7 were observed, the correlation between the HU value of C2–C7 and the total BMD T-score of L1–L4 was analyzed, and the HU thresholds of C2–C7 for different T-scores (-2.5 < T-score <-1 or T-score ≤2.5, respectively) were identified. Results The HU values of C2–C7 show a decreasing trend. The mean HU value of C2–C7 was 322.52 ± 89.27 HU. The average BMD T-score of L1-L4 was -0.73. The average HU value of C2–C7 was positively correlated with the average BMD T-score of L1–L4 (r = 0.487, P < 0.001). The HU threshold of C2–C7 was 269 HU when -2.5＜T-score＜-1, and it had a sensitivity of 75.7% and a specificity of 59.8% when used for screening for osteopenia; the HU threshold of C2–C7 was 269H U when T-score ≤-2.5, and it had a sensitivity of 63.8% and a specificity of 80.8% when used for screening for osteoporosis. Conclusions The HU values of cervical vertebrae gradually decrease from C2 to C7 in patients undergoing surgical treatment for cervical degenerative disorders. The CT HU value of cervical vertebrae is positively correlated with the BMD T-score provided by lumbar DXA, which is helpful for clinical evaluation of bone quality before surgery.
Prediction of osteoporosis using MRI and CT scans with unimodal and multimodal deep-learning models

Abstract:PURPOSE: Osteoporosis is the systematic degeneration of the human skeleton, with consequences ranging from a reduced quality of life to mortality. Therefore, the prediction of osteoporosis reduces risks and supports patients in taking precautions. Deep-learning and specific models achieve highly accurate results using different imaging modalities. The primary purpose of this research was to develop unimodal and multimodal deep-learning-based diagnostic models to predict bone mineral loss of the lumbar vertebrae using magnetic resonance (MR) and computed tomography (CT) imaging.METHODS: Patients who received both lumbar dual-energy X-ray absorptiometry (DEXA) and MRI (n = 120) or CT (n = 100) examinations were included in this study. Unimodal and multimodal convolutional neural networks (CNNs) with dual blocks were proposed to predict osteoporosis using lumbar vertebrae MR and CT examinations in separate and combined datasets. Bone mineral density values obtained by DEXA were used as reference data. The proposed models were compared with a CNN model and six benchmark pre-trained deep-learning models.RESULTS: The proposed unimodal model obtained 96.54%, 98.84%, and 96.76% balanced accuracy for MRI, CT, and combined datasets, respectively, while the multimodal model achieved 98.90% balanced accuracy in 5-fold cross-validation experiments. Furthermore, the models obtained 95.68%-97.91% accuracy with a hold-out validation dataset. In addition, comparative experiments demonstrated that the proposed models yielded superior results by providing more effective feature extraction in dual blocks to predict osteoporosis.CONCLUSION: This study demonstrated that osteoporosis was accurately predicted by the proposed models using both MR and CT images, and a multimodal approach improved the prediction of osteoporosis. With further research involving prospective studies with a larger number of patients, there may be an opportunity to implement these technologies into clinical practice.

Automatic Opportunistic Osteoporosis Screening Using Low-Dose Chest Computed Tomography Scans Obtained for Lung Cancer Screening

Abstract:Objective Osteoporosis is a prevalent and treatable condition, but it remains underdiagnosed. In this study, a deep learning-based system was developed to automatically measure bone mineral density (BMD) for opportunistic osteoporosis screening using low-dose chest computed tomography (LDCT) scans obtained for lung cancer screening. Methods First, a deep learning model was trained and tested with 200 annotated LDCT scans to segment and label all vertebral bodies (VBs). Then, the mean CT numbers of the trabecular area of target VBs were obtained based on the segmentation mask through geometric operations. Finally, a linear function was built to map the trabecular CT numbers of target VBs to their BMDs collected from approved software used for osteoporosis diagnosis. The diagnostic performance of the developed system was evaluated using an independent dataset of 374 LDCT scans with standard BMDs and osteoporosis diagnosis. Results Our deep learning model achieved a mean Dice coefficient of 86.6% for VB segmentation and 97.5% accuracy for VB labeling. Line regression and Bland-Altman analyses showed good agreement between the predicted BMD and the ground truth, with correlation coefficients of 0.964–0.968 and mean errors of 2.2–4.0 mg/cm 3 . The area under the curve (AUC) was 0.927 for detecting osteoporosis and 0.942 for distinguishing low BMD. Conclusion The proposed deep learning-based system demonstrated the potential to automatically perform opportunistic osteoporosis screening using LDCT scans obtained for lung cancer screening. Key Points • Osteoporosis is a prevalent but underdiagnosed condition that can increase the risk of fracture. • A deep learning-based system was developed to fully automate bone mineral density measurement in low-dose chest computed tomography scans. • The developed system achieved high accuracy for automatic opportunistic osteoporosis screening using low-dose chest computed tomography scans obtained for lung cancer screening.

Abstract:Osteoporosis is a prevalent chronic disease worldwide, particularly affecting the aging population. The gold standard diagnostic tool for osteoporosis is Dual-energy X-ray Absorptiometry (DXA). However, the expensive cost of the DXA machine and the need for skilled professionals to operate it restrict its accessibility to the general public. This paper builds upon previous research and proposes a novel approach for rapidly screening bone density. The method involves utilizing near-infrared light to capture local body information within the human body. Deep learning techniques are employed to analyze the obtained data and extract meaningful insights related to bone density. Our initial prediction, utilizing multi-linear regression, demonstrated a strong correlation (r = 0.98, p-value = 0.003**) with the measured Bone Mineral Density (BMD) obtained from Dual-energy X-ray Absorptiometry (DXA). This indicates a highly significant relationship between the predicted values and the actual BMD measurements. A deep learning-based algorithm is applied to analyze the underlying information further to predict bone density at the wrist, hip, and spine. The prediction of bone densities in the hip and spine holds significant importance due to their status as gold-standard sites for assessing an individual's bone density. Our prediction rate had an error margin below 10% for the wrist and below 20% for the hip and spine bone density.

Abstract:Objective: This study aimed to develop and validate a predictive model for osteoporotic vertebral fractures (OVFs) risk by integrating demographic, bone mineral density (BMD), CT imaging, and deep learning radiomics features from CT images. Methods: A total of 169 osteoporosis-diagnosed patients from three hospitals were randomly split into OVFs (n = 77) and Non-OVFs (n = 92) groups for training (n = 135) and test (n = 34). Demographic data, BMD, and CT imaging details were collected. Deep transfer learning (DTL) using ResNet-50 and radiomics features were fused, with the best model chosen via logistic regression. Cox proportional hazards models identified clinical factors. Three models were constructed: clinical, radiomics-DTL, and fusion (clinical-radiomics-DTL). Performance was assessed using AUC, C-index, Kaplan-Meier, and calibration curves. The best model was depicted as a nomogram, and clinical utility was evaluated using decision curve analysis (DCA). Results: BMD, CT values of paravertebral muscles (PVM), and paravertebral muscles' cross-sectional area (CSA) significantly differed between OVFs and Non-OVFs groups (P < 0.05). No significant differences were found between training and test cohort. Multivariate Cox models identified BMD, CT values of PVM, and CSAPS reduction as independent OVFs risk factors (P < 0.05). The fusion model exhibited the highest predictive performance (C-index: 0.839 in training, 0.795 in test). DCA confirmed the nomogram's utility in OVFs risk prediction. Conclusion: This study presents a robust predictive model for OVFs risk, integrating BMD, CT data, and radiomics-DTL features, offering high sensitivity and specificity. The model's visualizations can inform OVFs prevention and treatment strategies.

Abstract:BackgroundDual-energy X-ray absorptiometry (DEXA) scan is an easy and cost-effective method of assessing bone mineral density (BMD). However, in patients with degenerative changes of the spine, overestimation of T-score on DEXA scan can occur despite low BMD during pedicle screw placement in spine surgery. The goal of this study is to assess BMD using Hounsfield unit (HU) measurements from computed tomography (CT) and to correlate these with DEXA-assessed T-scores in non-degenerative and degenerative patients.MethodsThis study included 80 non-degenerative and 30 degenerative patients who underwent DEXA and spine CT assessment. The HU value on the mid-body axial images of CT and DEXA-assessed T-scores were measured from the L1-4 vertebrae.ResultsIn the non-degenerative group, HU values had a strong positive correlation with BMD and T-score, exhibiting correlation coefficients (r) greater than 0.7: the r-value (p value) between HU and T-score of the L1 vertebra was 0.701 (<0.001); 0.709 (<0.001) for L2; 0.709 (<0.001) for L3; 0.649 (<0.001) for L4; and 0.734 (<0.001) for L1-4. BMD assessed as +100 HU matched a T-score of -2.0 while +150, +200 HU matched T-scores of -1.0, 0.0. The differences were significant (p < 0.001). In the degenerative group, there was a weak positive correlation with r of approximately 0.4: the r-value (p value) was 0.300 (0.104); 0.457 (0.013); 0.433 (0.017); 0.447 (0.013) at each segment and 0.398 (0.031) for L1-4.ConclusionsHU values provide a meaningful assessment of BMD and have a strong correlation with T-score. However, in degenerative patients, the T-score tended to be higher than the actual BMD. BMD assessment using HU might be helpful in predicting real BMD in patients undergoing instrumentation surgery with degenerative changes of the spine.
