Abstract:STUDY OBJECTIVES To investigate the effects of sleep extension over multiple weeks on specific measures of athletic performance as well as reaction time, mood, and daytime sleepiness. SETTING Stanford Sleep Disorders Clinic and Research Laboratory and Maples Pavilion, Stanford University, Stanford, CA. PARTICIPANTS Eleven healthy students on the Stanford University men's varsity basketball team (mean age 19.4 ± 1.4 years). INTERVENTIONS Subjects maintained their habitual sleep-wake schedule for a 2-4 week baseline followed by a 5-7 week sleep extension period. Subjects obtained as much nocturnal sleep as possible during sleep extension with a minimum goal of 10 h in bed each night. Measures of athletic performance specific to basketball were recorded after every practice including a timed sprint and shooting accuracy. Reaction time, levels of daytime sleepiness, and mood were monitored via the Psychomotor Vigilance Task (PVT), Epworth Sleepiness Scale (ESS), and Profile of Mood States (POMS), respectively. RESULTS Total objective nightly sleep time increased during sleep extension compared to baseline by 110.9 ± 79.7 min (P < 0.001). Subjects demonstrated a faster timed sprint following sleep extension (16.2 ± 0.61 sec at baseline vs. 15.5 ± 0.54 sec at end of sleep extension, P < 0.001). Shooting accuracy improved, with free throw percentage increasing by 9% and 3-point field goal percentage increasing by 9.2% (P < 0.001). Mean PVT reaction time and Epworth Sleepiness Scale scores decreased following sleep extension (P < 0.01). POMS scores improved with increased vigor and decreased fatigue subscales (P < 0.001). Subjects also reported improved overall ratings of physical and mental well-being during practices and games. CONCLUSIONS Improvements in specific measures of basketball performance after sleep extension indicate that optimal sleep is likely beneficial in reaching peak athletic performance.

Abstract:BACKGROUND AND PURPOSE: To measure the effects of prolonged sleep extension on daytime alertness, vigilance, and mood in healthy young adults. Little research has documented the effects of increased sleep on daytime function despite a high prevalence of daytime fatigue and sleepiness in the adult population. Past extension studies report conflicting results with regard to Multiple Sleep Latency Test (MSLT) scores, vigilance, and mood ratings. No study has challenged subjects to maximum sleep extension, defined by an MSLT score of 20.PATIENTS AND METHODS: Fifteen healthy college students reporting minimal daytime sleepiness were allowed to sleep as much as possible during a sleep extension period. MSLT scores, psychomotor vigilance task (PVT) reaction times, and profile of mood states (POMS) ratings were measured at baseline, mid-extension, and end-extension.RESULTS: There was a significant increase in both journal and actigraphy sleep totals during all extension segments (P<0.01). MSLT scores increased significantly from baseline to both mid- and end-extension (P<0.01). Five of eight tabulated PVT measures also improved significantly at mid- and end-extension with respect to baseline (P<0.05). POMS vigor and fatigue scores showed a similar improvement (P<0.01). Seven subjects achieved an MSLT score of 20. Six subjects showed substantial improvements while two subjects obtained relatively little extra sleep and showed little or no MSLT improvement. The maximum extension group displayed exceptional improvements in vigilance and POMS ratings.CONCLUSIONS: Extended sleep leads to substantial improvements in daytime alertness, reaction time, and mood.

Recovery of Cognitive Performance Following Multi-Stressor Military Training

Abstract:Objective: This project aimed to assess the impact of an 8-day military training exercise on cognitive performance, and track its recovery in periods of reduced training load and partially restored sleep. Background: Military personnel often work in challenging multi-stressor environments, where sleep loss is inevitable. Sleep loss can impair multiple cognitive domains, which can have disastrous consequences in military contexts. Method: A total of 57 male and female soldiers undergoing the Australian Army combat engineer Initial Employment Training course were recruited and tracked over a 16-day study period which included an 8-day field-based military training exercise. Cognitive performance was assessed via a computerised battery at seven time points across four sequential study periods; 1) baseline (PRE), 2) military field training exercise which included total sleep deprivation (EX-FIELD), 3) training exercise at simulated base with restricted sleep opportunities (EX-BASE), and 4) a 3-day recovery period (REC). Subjective load, fatigue, and sleep were evaluated continuously via questionnaire and actigraphy. Results: Psychomotor speed, reaction time, visual tracking and vigilance were impaired following the EX-FIELD period (p < 0.05). The majority of affected measures recovered 2 days following EX-FIELD, being no different in EX-BASE compared to PRE. Conclusion: The sensitivity of the cognitive tests to sleep restriction, and recovery, indicates they can help assess operational readiness in military personnel. Future studies should explore other indicators of, and strategies to preserve, operational readiness in military personnel. Application: This study highlights the impact of work-induced fatigue on cognitive performance, and would interest authorities seeking to preserve operational readiness.
Effects of sleep deprivation with reference to military operations

Abstract:This review discusses the need for sleep, effects of sleep deprivation on behaviour and performance in the military, and sleep management recommendations to optimise combat effectiveness. Most people, regardless of sex or race, prefer 7 to 8 hours of sleep each night. Sleeping during the day is less recuperative. Continuous sleep is more effective than multiple short naps-even when the total hours for naps is more. Ten to 20 minute naps are useful when continuous sleep is not possible. Sleep inertia is the 5 to 30 minute period of sluggishness after awakening and important military tasks should be avoided. Previously, continuous work episodes (CWEs) duration was restricted by limited night vision, unreliable equipment and reduced endurance of military personnel. With improved technology, CWEs are now restricted primarily by endurance which is affected by sleep deprivation. This was one of the experiences noted in recent conflicts (e.g. Desert Storm) by personnel in the air force, army and navy. Since there will be changes in operational requirements, several work-rest-sleep plans must be prepared. Sleeping the preferred 7 to 8 hours per 24 hours the week before an operation may help prepare for optimal performance. Personnel should be familiarised with conditions under which they may sleep. During combat, sleep management should ideally avoid situations where all personnel are exhausted at the same time. As sleep debt accumulates, a person's mood, motivation, attention, alertness, short-term memory, ability to complete routines, task performance (errors of omission more than errors of commission) and physical performance will become more negatively affected. Counter measures must then be taken (e.g. time for sleep or naps, changing routines or rotating jobs). Drugs like caffeine and amphetamine can help personnel stay awake. However, they may also keep them awake when they need to sleep- and on awakening, they could suffer from "hang-overs" and are less efficient. Sleep lost need not be replaced hour-for-hour. Therefore, after operations, personnel need continuous sleep for only 10 to 12 hours as longer sleep increases sleep inertia and delays getting back to normal schedules.
Less daytime sleepiness and slow wave activity during sleep predict better physical readiness in military personnel

Abstract:Background: Military personnel must maintain physical performance despite exposure to operational stressors such as sleep loss, caloric restriction and high cognitive load. Habitual sleep and specific sleep features are positively associated with fitness and may contribute to physical performance in operational settings. Further, by affecting muscle recovery, sleep may contribute to the ability to maintain performance across multiple days of exposure to operational stressors. Objectives: We examined the role of individual differences in baseline sleep on baseline physical performance and on change in physical performance throughout exposure to simulated military operational stress (SMOS). Methods: Military personnel (36 male, 9 female, 26.3 ± 5.3 years) completed a 5-day SMOS protocol during which they completed a tactical mobility test daily. Sleep questionnaires were administered at intake and sleep was monitored each night with polysomnography. Lasso regressions were used to identify meaningful predictors of physical performance at baseline and of change in physical performance across SMOS. Results: Better aerobic fitness, lower daytime sleepiness (Epworth Sleepiness Scale), and lower absolute slow wave activity (0.5-4 Hz) predicted better physical performance at baseline (66.1% of variance explained), but did not relate to changes in performance. Conclusions: Collectively, higher daytime sleepiness and slow wave activity may reflect more chronic exposure to insufficient sleep and higher baseline sleep drive, which in turn led to compromised physical performance. The findings suggest that low self-report sleepiness and low objective slow wave activity may reflect two quantifiable markers of healthy sleep behaviors that have implications for operational performance.

Abstract:Objectives: To verify the validity of the recently described sciatic functional index for mice to monitor neuronal functional recovery over time using a blinded, randomized, and controlled evaluation. Study Design: Surgery was performed on the left sciatic nerves of 62 C57/BL mice after randomly assigning them to one of four surgical groups: sham surgery, sciatic nerve crush, nerve transection without repair, and nerve transection followed by epineurial suture repair. Sciatic functional indices were measured before surgery and then after surgery at 10‐day intervals for 90 days, using a previously described formula. Results: Sham surgery did not affect nerve function when compared with preoperative values (P > .24). Crush surgery produced a reversible nerve injury that fully recovered after 20 days. Nerve transection without repair resulted in complete functional disability without recovery of function during the 90‐day follow‐up interval. When transected nerves were repaired, complete functional disability was noted at day 10, with subsequent functional recovery to 26% of function at day 30. This level of recovery persisted until the 60th postoperative day when muscle contractures resulted in progressive worsening of the index. There were statistically significant differences between the sciatic functional indices of each of the groups (P < .05). Conclusions: The previously described sciatic functional index for mice is an accurate indicator of the level of sciatic neuronal function during recovery. This index represents a method of evaluating neuronal function that may provide a better reflection of the recovery parameters that are important in clinical situations. The sciatic functional index will allow for study of sciatic nerve functional recovery in genetically engineered transgenic mice.

Abstract:This study aimed to examine the effect of nonsleep deep rest (NSDR) on physical and cognitive performance, as well as sleepiness, acute readiness, recovery, stress, and mood state in physically active participants. A total of 65 physically active participants (42 male, 23 female) were randomly assigned into two groups: an experimental group (NSDR, n = 34), in which participants completed a 10-min NSDR intervention, and a control group (CON, n = 31), whereby participants sat passively for 10 min. Testing measures were assessed immediately pre and 10 min post each condition and comprised completing a hand grip strength dynamometer test and a countermovement jump test on force plates, cognitive function measures via a psychomotor vigilance task (PVT-B), and a Simon task test, along with four questionnaires to assess sleep, recovery, and mood state. A significant Group × Time interaction favored the NSDR condition for handgrip strength, median reaction time during the PVT-B, and accuracy percentage during the Simon task. Questionnaire responses demonstrated NSDR to be associated with significant benefits to physical readiness, emotional balance, overall recovery, negative emotional state, overall stress, and tension in comparison to CON (p < .05). The NSDR intervention could be a valuable strategy for acutely enhancing overall well-being and readiness.
Effects of a 1-hour per night week-long sleep extension in college students on cardiometabolic parameters, hydration status, and physical activity: A pilot study

Abstract:Objectives: Short sleep duration is associated with poor physical health in college students. Few studies examine the effects of sleep extension on physical health in this population, who are susceptible to sleep loss. We examined health effects of a 1-week, 1-hour nightly sleep extension in college students. Methods: Twelve healthy undergraduate college students (83% female; age 20.2 ± 1.5years) completed a study consisting of sleeping typically for 1week ("Habitual"), then extending sleep by ≥1 hour/night during the second week ("Extension"). Sleep and physical activity actigraphy were collected throughout. Following each week, participants completed cardiometabolic assessments including a meal response and provided a urine sample for markers of hydration. Results: In Extension compared to Habitual, average sleep duration increased (mean change±SEM, +42.6 ± 15.1 minutes; p = .005), while subjective sleepiness (-1.8 ± 0.8 units; p = .040), systolic blood pressure (-6.6 ± 2.8 mmHg; p = .037), postprandial glucose area under the curve (-26.5 ± 10.2 mg/dL × h; p = .025) and time to baseline (-83.0 ± 46.4 minutes; p = .031) after the meal response, sedentary time (-44.3 ± 15.7 minutes; p = .018), and percentage of wake in moderate-to-vigorous activity (-0.89% ± 0.35%; p = .030) decreased. Participants who increased average sleep duration by ≥20 minutes (n = 9) were better hydrated according to urine osmolality (-187.0 ± 68.4 mOsm/kg; p = .026) and specific gravity (-0.01 ± 0.002 g/mL; p = .012) and had reduced odds of dehydration according to urine osmolality (≥800 mOsm/kg; -67%; OR=0.03; p = .035). Conclusions: This pilot study's findings suggest that sleep extension may improve cardiometabolic functioning and hydration, and alter sedentary behavior and physical activity, in college students. Sleep extension may be employed to improve multiple aspects of health in this sleep-deprived population.
Operation early‐bird: Investigating altered light exposure in military barracks on sleep and performance—a placebo‐controlled study

Abstract:Summary The manipulation of light exposure in the evening has been shown to modulate sleep, and may be beneficial in a military setting where sleep is reported to be problematic. This study investigated the efficacy of low‐temperature lighting on objective sleep measures and physical performance in military trainees. Sixty‐four officer‐trainees (52 male/12 female, mean ± SD age: 25 ± 5 years) wore wrist‐actigraphs for 6 weeks during military training to quantify sleep metrics. Trainee 2.4‐km run time and upper‐body muscular‐endurance were assessed before and after the training course. Participants were randomly assigned to either: low‐temperature lighting (LOW, n = 19), standard‐temperature lighting with a placebo "sleep‐enhancing" device (PLA, n = 17), or standard‐temperature lighting (CON, n = 28) groups in their military barracks for the duration of the course. Repeated‐measures ANOVAs were run to identify significant differences with post hoc analyses and effect size calculations performed where indicated. No significant interaction effect was observed for the sleep metrics; however, there was a significant effect of time for average sleep duration, and small benefits of LOW when compared with CON (d = 0.41–0.44). A significant interaction was observed for the 2.4‐km run, with the improvement in LOW (Δ92.3 s) associated with a large improvement when compared with CON (Δ35.9 s; p = 0.003; d = 0.95 ± 0.60), but not PLA (Δ68.6 s). Similarly, curl‐up improvement resulted in a moderate effect in favour of LOW (Δ14 repetitions) compared with CON (Δ6; p = 0.063; d = 0.68 ± 0.72). Chronic exposure to low‐temperature lighting was associated with benefits to aerobic fitness across a 6‐week training period, with minimal effects on sleep measures.

Abstract:Introduction: This observational study investigated the effects of sleep deprivation and ad libitum caffeine consumption on cognitive performance, risk behavior, and mood among 28 Israeli Special Forces (SF) soldiers (mean age: 20.57 ± 0.92 years) during a 96-hour combat exercise. Methods: Actigraphy was used to monitor sleep and activity; cognitive function, risk-taking propensity, mood states, and self-reported sleepiness were assessed using the Psychomotor Vigilance Task (PVT), Evaluation of Risks Scale (EVAR), Profile of Mood States (POMS), Karolinska Sleepiness Scale (KSS); and caffeine consumption by questionnaire at 0, 50, and 96 hours. For analyses, participants were divided into Low (<400 mg) and High (≥400 mg) caffeine consumption groups. Results: The soldiers hiked 108.5 ± 0.52 km and slept for 12.7 ± 0.5 h, with a notable transition from multiple short sleep epochs in the initial 50 hours to a consolidated 5-hour sleep period subsequently. In the High caffeine group, PVT reaction time was faster ( p = 0.024) compared to the Low caffeine group, with fewer premature response errors ( p = 0.026). However, this group showed increased risk-taking ( p = 0.037), particularly reduced Self-Control ( p = 0.010). No significant impact of ad libitum caffeine intake on mood was observed. However, degradation over the course of the exercise in both groups in mood states, including anger, fatigue, tension, and vigor, was noted ( p < 0.05). KSS scores increased significantly at 50 and 96 h ( p < 0.001). Discussion: These results suggest that while caffeine enhances cognitive function, its ad libitum consumption did not consistently improve these measures in this cohort of SF soldiers. The study highlights the complex relationship between sleep deprivation and caffeine intake and their combined effects on soldiers' cognitive and behavioral functions, indicating a need for evidence-based caffeine use guidelines for using caffeine in military settings.

Abstract:Abstract Introduction Sleep deprivation leads to detriments in performance, including alertness and vigilance. Less is known, however, about the cognitive impact of mild sleep restriction, including whether negative outcomes are time-dependent and cumulative. We tested the impact of sleep restriction on sustained attention in a multi-week, crossover experiment in a controlled laboratory setting. Methods Sixty-six young adults (Mage = 20.03; 53.0% female; 51.5% non-white) completed seven nights of in-laboratory testing, spread across 3-weeks. On the first week, participants completed a polysomnography adaptation night and baseline assessments including the psychomotor vigilance task (PVT) in the evening (9:00PM) and in the morning (7:30AM) with buffer time to prevent sleep inertia following initial awakening. During the following week, participants completed either three nights of sleep restriction (5 hours of sleep/night) or three nights of normal sleep (8 hours of sleep/night), with PVT testing each morning and evening. After a four-night washout period, participants returned to complete the other sleep condition (order counterbalanced). Research assistants who administered the PVT were kept blinded to the participant’s sleep condition. Results Relative to normal sleep, three nights of sleep restriction significantly worsened PVT mean reaction times (RTs; M = 361 ms versus M = 390 ms; t(65) = 4.38, p < .001, d = .54) and number of lapses (M= 3.78 versus M = 5.80; t(65) = 5.00, p < .001, d = .62]. There were no main effects of time of day or interactions with sleep condition (ps>.05). However, study day significantly interacted with sleep condition (F(2, 128) = 3.42, p = .036, η2 = .051) such that the negative impact of sleep restriction on lapses worsened across the week (night 1: Mdiff = 1.59, night 2: Mdiff = 0.88; night 3: Mdiff = 3.26). Conclusion Mild sleep restriction quickly produces detriments in sustained attention, with effects being cumulative across repeated days of sleep loss. Abstract:Abstract Introduction The military works under 24/7 operations which can lead to sleep loss and Soldier fatigue. Both sleep loss and fatigue impact functionality and success of the Soldier and ultimately, the entire unit. The functional impact of sleepiness and fatigue can be present differently. Therefore, clarifying the relationship between subjective sleepiness and fatigue is important for military readiness and provides leadership with critical information to ensure mission success. Methods Fifty-one healthy, young male participants were recruited from a sample of active-duty Soldiers. The study lasted 5 days with a 72-hour live-fire mission simulation from days 2 to 4. On each of these days, Soldiers completed daily morning surveys, including measures of sleepiness using the Epworth Sleepiness Scale (an 8-question measure to identify clinical sleepiness) as well as subjective fatigue through the Visual Analog Scale. Results A repeated measures ANOVA showed a significant main effect for subject sleepiness and fatigue among mission days (p < 0.05). Linear regression indicated sleepiness scores were significantly predictive of fatigue each mission day. Conclusion On mission day 3 during the 72-hour live fire mission, soldiers were significantly more sleepy and more fatigued compared to other mission days. Additionally, for each mission day, subjective sleepiness was predictive of soldier fatigue. Understanding this relationship and its predictive nature can help military leadership plan accordingly for mission and training events to maximize success in operational contexts. Abstract:Typically, the effects of sleep duration on cognition are examined in isolation.OBJECTIVE: This study examined the effects of restricted sleep and related symptoms on neurocognitive performance.METHOD: Baseline Immediate Post-Concussion Assessment and Cognitive Testing (ImPACT) and postconcussion symptom scale (PCSS) were administered to athletes (N = 7,150) ages 14-17 (M = 15.26, SD = 1.09) prior to sport participation. Three groups of athletes were derived from total sleep duration: sleep restriction (≤5 hours), typical sleep (5.5-8.5 hours), and optimal sleep (≥9 hours). A MANCOVA (age and sex as covariates) was conducted to examine differences across ImPACT/PCSS. Follow-up MANOVA compared ImPACT/PCSS performance among symptomatic (e.g., trouble falling asleep, sleeping less than usual) adolescents from the sleep restriction group (n = 78) with asymptomatic optimal sleepers (n = 99).RESULTS: A dose-response effect of sleep duration on ImPACT performance and PCSS was replicated (Wilk's λ = .98, F2,7145 = 17.25, p < .001, η2 = .01). The symptomatic sleep restricted adolescents (n = 78) had poorer neurocognitive performance: verbal memory, F = 11.60, p = .001, visual memory, F = 6.57, p = .01, visual motor speed, F = 6.19, p = .01, and reaction time (RT), F = 5.21, p = .02, compared to demographically matched controls (n = 99). 1108 Prior-night Sleep Predicts Next-day Symptoms over Ten Days Among Military Personnel with Sleep Problems

Abstract:Abstract Introduction Insufficient and disturbed sleep are highly prevalent and associated with adverse consequences in the U.S. military. The purpose of this study was to employ remote monitoring/ecological momentary assessment methods to test the hypothesis that prior-night sleep is associated with next-day subjective symptoms among military personnel. Methods Participants (N=270) with sleep problems were recruited from two military treatment facilities and completed an intensive ten-day remote monitoring assessment. Prior night sleep was measured using standardized sleep diaries and a commercial wearable sleep tracker (Fitbit Inspire 2). Next-day symptoms were measured using twice-daily symptom surveys (i.e., 20 surveys over ten days). Individual items assessed subjective cognition, energy level, and mood using an “I feel tired” format and scored from 0-4. To determine the lagged impact of prior-night sleep on next-day symptoms, a total of 40 mixed models (MMs) were developed. Individual prior-night sleep diary (total sleep time [TST], sleep efficiency [SE], and sleep quality [QUAL]) and Fitbit (TST, SE) parameters were entered as independent variables, and individual daytime symptoms (feeling alert, clear-headed, refreshed, fatigued, happy, sad, stressed, and relaxed) were entered as dependent variables. All models controlled for age and sex. The Benjamini-Hochberg procedure for correcting false discovery rate (BH-FDR) method was used to correct for multiple comparisons. Results Lagged MM analyses revealed that all prior-night sleep diary variables were significantly associated with next-day symptoms (all ps< 0.001 with df=2081) over ten days. Specifically, prior-night sleep diary parameters (TST, SE, QUAL) were positively associated with next-day feeling alert, clear-headed, refreshed, happy, and relaxed; and negatively associated with next-day feeling fatigued, sad, and stressed. Prior-night Fitbit sleep parameters were significantly associated with most next-day symptoms (largest p=0.023 with df=1956). Specifically, Fitbit TST and SE were positively associated with next-day feeling clear-headed, refreshed, happy, and relaxed and negatively associated with next-day fatigue. Fitbit TST was negatively associated with next-day stress. Conclusion Among military personnel with sleep problems, prior-night sleep predicts next-day subjective cognition, energy level, and mood over ten days. Future research should employ remote monitoring approaches to predict treatment response and deliver personalized care to improve sleep and daytime symptoms. Abstract:Study Objectives: The Psychomotor Vigilance Test (PVT) is reported to be free of practice effects that can otherwise confound the effects of sleep loss and circadian misalignment on performance. This differentiates the PVT from more complex cognitive tests. To the best of our knowledge, no study has systematically investigated practice effects on the PVT across multiple outcome domains, depending on administration interval, and in ecologically more valid settings.Methods: We administered a validated 3-minute PVT (PVT-B) 16 times in 45 participants (23 male, mean ± SD age 32.6 ± 7.3 years, range 25-54 years) with administration intervals of ≥10 days, ≤5 days, or 4 times per day. We investigated linear and logarithmic trends across repeated administrations in 10 PVT-B outcome variables.Results: The fastest 10% of response times (RT; plin = .0002), minimum RT (plog = .0010), and the slowest 10% of reciprocal RT (plog = .0124) increased while false starts (plog = 0.0050) decreased with repeated administration, collectively decreasing RT variability (plog = .0010) across administrations. However, the observed absolute changes were small (e.g., -0.03 false starts per administration, linear fit) and are probably irrelevant in practice. Test administration interval did not modify the effects of repeated administration on PVT-B performance (all p > .13 for interaction). Importantly, mean and median RT, response speed, and lapses, which are among the most frequently used PVT outcomes, did not change systematically with repeated administration.Conclusions: PVT-B showed stable performance across repeated administrations. Combined with its high sensitivity, this corroborates the status of the PVT as the de facto gold standard measure of the neurobehavioral effects of sleep loss and circadian misalignment.

Abstract:PURPOSEː: The current study aimed at assessing the effect of a trial of two nights of sleep deprivation (SDT) on mood, sleepiness, motivation and cognitive and motor performance. METHODSː: Thirty-six healthy young and physically active adult men (17 in the control group and 19 in the SDT group) completed a 48-h control or 48-h SDT. For the SDT, participants did not sleep for 48 h. Executive function (attention and inhibitory control) in the Go/No-Go and Stroop tests, mood, sleepiness, motivation, heart rate variability (HRV), motor performance in a hand grip strength test, and 60-s maximal isometric contraction (MVC-60 s) of knee extension were evaluated at 9-11 am on consecutive days 1, 2, and 3. RESULTS: One night of sleep deprivation increased sleepiness, decreased mood, motivation and motor endurance but did not affect executive function (as measured in the Stroop and Go/No-Go tests), the MVC for hand and leg knee extensor muscles, and peripheral motor fatigue in the leg MVC-60 s task. However, the central activation ratio (CAR) decreased significantly during the MVC-60 s. The SDT significantly contributed to the decrease in these functions. That is, the SDT reduced executive function (increased reaction time during Go/No-Go test), MVC of knee extension, and the CAR before and after the MVC-60 s. By contrast, the SDT did not increase CAR immediately after the MVC-60 s and did not decrease the rate of torque development (RTD). CONCLUSIONSː: The SDT significantly impaired mood, motivation and increased sleepiness and HRV, reduced MVC of knee extensor muscles (but not RTD) and motor performance during the MVC-60 s and worsened executive function (attention and inhibitory control) only during the Go/No-Go task. However, the SDT did not reduce hand grip strength and CAR immediately after the MVC-60 s of knee extensor muscles.

Abstract:The military lifestyle often includes continuous operations whether in training or deployed environments. These stressful environments present unique challenges for service members attempting to achieve consolidated, restorative sleep. The significant mental and physical derangements caused by degraded metabolic, cardiovascular, skeletomuscular, and cognitive health often result from insufficient sleep and/or circadian misalignment. Insufficient sleep and resulting fatigue compromises personal safety, mission success, and even national security. In the long-term, chronic insufficient sleep and circadian rhythm disorders have been associated with other sleep disorders (e.g., insomnia, obstructive sleep apnea, and parasomnias). Other physiologic and psychologic diagnoses such as post-traumatic stress disorder, cardiovascular disease, and dementia have also been associated with chronic, insufficient sleep. Increased co-morbidity and mortality are compounded by traumatic brain injury resulting from blunt trauma, blast exposure, and highly physically demanding tasks under load. We present the current state of science in human and animal models specific to service members during- and post-military career. We focus on mission requirements of night shift work, sustained operations, and rapid re-entrainment to time zones. We then propose targeted pharmacological and non-pharmacological countermeasures to optimize performance that are mission- and symptom-specific. We recognize a critical gap in research involving service members, but provide tailored interventions for military health care providers based on the large body of research in health care and public service workers.
