Abstract:Inadequate sleep is an on-going risk to the health and mission readiness of U.S. Armed Forces, with estimates of sleep problems high above U.S. civilian populations. Intervening early in the career of active duty Air Force personnel (or "Airmen") with education and the establishment of healthy behaviors may prevent short and long term-detriments of sleep problems. This paper describes the results of a qualitative study seeking to understand the facilitators and barriers to achieving good sleep in a technical training school during the first year of entry into the United States Air Force. Using the social ecological framework and content analysis, three focus groups with Airmen were conducted to explore themes at the individual, social, environmental, and organizational/policy level. Overall, results indicated a cohort motivated to achieve good sleep, and also struggling with a number of barriers across each level. This paper highlights opportunities for population health interventions during technical training aimed at supporting Airmen in developing healthy sleep behaviors early in the course of their career.

Abstract:Introduction: Social workers, the largest group of mental health clinicians in the United States, play a pivotal role in mental health promotion. Despite the importance of sleep for mental health, there is no empirical research on sleep education interventions for social workers. Method: We designed an online sleep health education intervention to equip social work students to promote healthy sleep practices among their clients. An interdisciplinary team of experts devised the 90 min intervention using an empirically supported behavioral change theoretical model (COM-B). The intervention discusses multi-level factors that impact sleep and emphasizes considerations for health disparities in populations commonly served by social workers (e.g., unhoused populations, clients with substance use disorders, etc.). We assessed sleep knowledge, sleep quality, and acceptability using survey and focus group data. Results: Ninety social work students (92.2% female, 38.8% non-Hispanic white) completed pre- and post-intervention assessments. Participants demonstrated significant improvements in sleep health knowledge and their personal sleep quality. Quantitative and qualitative data revealed perceived usefulness for social work practice. Conclusions: A short online sleep education intervention can improve sleep health knowledge, offering a practical method to expand social workers' understanding of healthy sleep promotion that can be readily implemented in clinical training and practice.

Abstract:Abstract Introduction Chronic sleep insufficiency is a serious threat to military readiness in the United States. Many military members develop poor sleep habits early in their career. Career fields such as Air Traffic Control (ATC) are at greater risk for sleep and performance concerns. Sleep health is determined by individual, social, and systemic factors. Thus, a multilevel approach is warranted, including leveraging leaders’ influence to promote sleep health within high-risk communities. The present study examined baseline sleep and sleep leadership behaviors among Air Force ATC training leaders. Sleep leadership is defined as awareness about benefits of good sleep health and promotion of helpful sleep habits among peers/subordinates. Methods Participants were 44 ATC instructors and military training leaders who completed an anonymous pre-training survey and then received an hour-long sleep leadership intervention. The baseline survey included the Split Week Self-Assessment of Sleep Survey as well as the Sleep Leadership Questionnaire, a self-report instrument measuring how often leaders engage in behaviors to promote healthy sleep among students. Participants then rated satisfaction and relevancy of the training and provided qualitative feedback on action steps they were considering Results Summary statistics indicated that leaders’ total sleep time (TST), on average, was 6.04 hours (SD=1.63, IQR=1.87) on weekdays and 7.06 hours (SD=2.44, IQR=2.81) on weekends. Ninety-one percent reported they regularly (i.e., “sometimes”/“often”/“always”) instruct students to get adequate sleep, but only 55% reported they inform Airmen about healthy sleeping habits (Fisher’s exact p=0.036) Leaders agreed the training was interesting (endorsed by 77%) and relevant (endorsed by 68%), and 64% reported the training would help them as a leader. Qualitative data showed that many leaders identified specific action steps they planned to implement with ATC students. Conclusion Results suggested leaders tell students to prioritize sleep, but rarely provide specific behavioral tips for improving sleep or model optimal sleep behavior. Leaders noted the training was relevant to their role and identified concrete steps to promote sleep among their students. ATC leaders are highly respected by their students, and targeting sleep leadership appears to be a feasible and acceptable solution within a multi-level approach for improving sleep among high-risk military career fields Support (if any)

Abstract:Background: Compared to civilians and non-medical personnel, military medical doctors are at increased risk for sleep disturbances and impaired psychological well-being. Despite their responsibility and workload, no research has examined sleep disturbances and psychological well-being among the medical doctors (MDs) of the Swiss Armed Forces (SAF). Thus, the aims of the proposed study are (1) to conduct a cross-sectional study (labeled 'Survey-Study 1') of sleep disturbances and psychological well-being among MDs of the SAF; (2) to identify MDs who report sleep disturbances (insomnia severity index >8), along with low psychological well-being such as symptoms of depression, anxiety and stress, but also emotion regulation, concentration, social life, strengths and difficulties, and mental toughness both in the private/professional and military context and (3) to offer those MDs with sleep disturbances an evidence-based and standardized online interventional group program of cognitive behavioral therapy for insomnia (eCBTi) over a time lapse of 6 weeks (labeled 'Intervention-Study 2'). Method: All MDs serving in the SAF (N = 480) will be contacted via the SAF-secured communication system to participate in a cross-sectional survey of sleep disturbances and psychological well-being ('Survey-Study 1'). Those who consent will be provided a link to a secure online survey that assesses sleep disturbances and psychological well-being (depression, anxiety, stress, coping), including current working conditions, job-related quality of life, mental toughness, social context, family/couple functioning, substance use, and physical activity patterns. Baseline data will be screened to identify those MDs who report sleep disturbances (insomnia severity index >8); they will be re-contacted, consented, and randomly assigned either to the eCBTi or the active control condition (ACC) ('Intervention-Study 2'). Individuals in the intervention condition will participate in an online standardized and evidence-based group intervention program of cognitive behavioral therapy for insomnia (eCBTi; once the week for six consecutive weeks; 60-70 min duration/session). Participants in the ACC will participate in an online group counseling (once the week for six consecutive weeks; 60-70 min duration/session), though, the ACC is not intended as a bona fide psychotherapeutic intervention. At the beginning of the intervention (baseline), at week 3, and at week 6 (post-intervention) participants complete a series of self-rating questionnaires as for the Survey-Study 1, though with additional questionnaires covering sleep-related cognitions, experiential avoidance, and dimensions of self-awareness. Expected outcomes: Survey-Study 1: We expect to describe the prevalence rates of, and the associations between sleep disturbances (insomnia (sleep quality); sleep onset latency (SOL); awakenings after sleep onset (WASO)) and psychological well-being among MDs of the SAF; we further expect to identify specific dimensions of psychological well-being, which might be rather associated or non-associated with sleep disturbances.Intervention-Study 2: We expect several significant condition-by-time-interactions. Such that participants in the eCBTi will report significantly greater improvement in sleep disturbances, symptoms of depression, anxiety, stress reduction both at work and at home (family related stress), and an improvement in the overall quality of life as compared to the ACC over the period of the study. Conclusion: The study offers the opportunity to understand the prevalence of sleep disturbances, including factors of psychological well-being among MDs of the SAF. Further, based on the results of the Intervention-Study 2, and if supported, eCBTi may be a promising method to address sleep disturbances and psychological well-being among the specific context of MDs in the SAF.

Abstract:Introduction: Adolescents are vulnerable to experiencing insufficient sleep which can increase the risk of developing insomnia, mental/physical health problems and mood regulation. School-based sleep improvement interventions (SBSII) have been developed utilising Cognitive Behavioural Therapy for Insomnia techniques and found improvements in sleep knowledge and behaviour. This study aimed to evaluate the effectiveness, feasibility and acceptability of a school-based sleep improvement intervention (iSLEEP) for improving sleep and psychological wellbeing in adolescents.Methods: A mixed-methods approach was adopted. The participants (115, aged 12-15) were recruited from secondary schools in Scotland. iSLEEP comprised of three lessons, combining formal sleep education and CBT-I, and were delivered over three consecutive weeks. Baseline (1 week before lesson 1) and follow-up (1 week following lesson 3) assessments comprised of self-report questionnaires to measure change in sleep, depression, anxiety and stress. Two weeks following the final lesson, a convenience sample of 19 pupils (14 females) were recruited to participate in focus groups.Results: There were significant improvements in overall sleep and marginal differences between those who were categorised with ‘probable insomnia’ at baseline and likewise for ‘good sleepers’. There were no significant improvements in symptoms of depression, anxiety or stress. Two themes and two sub-themes were generated. Participants reported that iSLEEP was acceptable and improved their sleep.Conclusions: These findings indicate that iSLEEP is an effective, feasible and acceptable SBSII. Future research should aim to utilise a controlled design to further investigate the efficacy of iSLEEP.

Abstract:Abstract Introduction Suicide represents a public health emergency among veterans, who die by suicide at alarming rates and show elevated sleep disturbances. Selective interventions for military suicide indication are scarce, and available treatments remains mismatched to the acute nature of a suicidal crisis. We sought to develop and test preliminary efficacy of a fast-acting, behavioral insomnia intervention for antisuicidal response in the context of a sham-controlled, randomized trial among veterans. Methods A multicomponent behavioral sleep intervention was developed, integrating: CBT for Insomnia (CBTi) and Imagery Rehearsal Treatment (IRT) within a rapid (4-session) format (SERVE: Sleep Enhancement for Returning Veterans). This was compared to active control, Arousal-Based Treatment of Insomnia (ABTI). Treatments were manualized and matched for therapist contact, materials (manuals, powerpoints, guidesheets), and passage of time. Raters were blind to treatment assignment. Inclusion criteria: (a) age>18, (b) clinically-significant sleep disturbance (Insomnia Severity Index (ISI>10)), (c) DSM-V-Defined Depression, or (d) current suicidal ideation (Columbia Suicidal Severity Rating Scale (CSSRS>1)). A comprehensive data and safety monitoring plan supported standardized risk assessment, safety planning, and triage. Outcomes were assessed at Baseline and Posttreatment (1,3 mos) according to primary (suicidal ideation), secondary (sleep indices), and exploratory (mood, stress indices) outcomes. Results Recruitment occurred by way of comprehensive military partnerships, clinic referrals, and community flyering. Of n=753 new contacts, n=436 participants were screened for inclusion. Of these, n=112 were interviewed by full-battery eligibility assessment, resulting in n=77 veterans enrolled and randomized to: Active Treatment (n=39) vs. Active Control (n=38). Feasibility analyses supported high rates of acceptance, tolerability, and safety. For the full sample, t-tests revealed large posttreatment reductions in suicidal ideation (CSSRS, p<.001; Depressive Symptom Inventory Suicidality Subscale (DSISS)), p<.001), sleep, mood, and stress outcomes (ISI, p<.01; DDNSI, p<.01; QIDS-SR, p<.01; PCL, p<.001). Effects were significant compared to control (DSISS, p<.01; ISI, p<.01). Conclusion A brief, non-pharmacological insomnia intervention resulted in antisuicidal response and large posttreatment improvements in sleep and well-being. This is the first known report testing non-medication insomnia treatment within a sham-controlled, randomized trial for military suicidal behaviors, where results support feasibility, safety, and therapeutic impact to suicidal risk. Sleep duration and perceptions of sleep quality in British Army recruits during basic training - an observational analysis

Abstract:Introduction: Sleep is critical to the health, wellbeing and performance of military personnel during basic training. This two-part study evaluated sleep-wake patterns and sleep disturbances in junior soldiers (JS) and infantry recruits in Autumn 2021 (study 1), and non-infantry recruits in spring 2022 (study 2). Methods: During studies 1 and 2, validated wearable technology combined with a sleep diary was used to quantify sleep-wake indices, sleep disturbances and perceptions of sleep quality. Sleep diary data was analysed descriptively. A series of repeated-measures ANOVAs examined differences in objective sleep-wake indices. Correlation analysis determined associations between time in bed (TIB) and total sleep time (TST). Results: Significant ( p < 0.05) differences in most sleep-wake indices were observed between weeks of basic training for all cohorts. Strong positive correlations between TIB and TST were observed for each cohort across basic training ( r = 0.681 – 0.970, p < 0.001), with longer TST associated with greater TIB. The mean±SD sleep duration (hours and mins [hm]) for JS (06:22 ± 00:27hm), non-infantry (05:41 ± 00:47hm) and infantry (05:46 ± 00:34hm) recruits across basic training was consistently below national recommendations. The mean±SD bed and wake times for JS (bedtime: 23:01 ± 00:32hm; awake: 05:34 ± 00:10hm), non-infantry (bedtime: 23:38 ± 01:09hm; awake: 04:47 ± 00:58hm), and infantry (bedtime: 23:13 ± 00:29hm; awake: 05:38 ± 00:26hm) recruits varied across weeks of basic training, with over 80% reporting "fairly bad" or "very bad" sleep quality and frequent periods of "dozing off" during daytime activity. The most commonly reported sleep disturbing factors identified during basic training involved: late-night military admin (e.g., ironing, boot cleaning, kit set up etc), early morning wake times, extraneous noise, light and hot room temperatures within the primary sleeping environment, bed/mattress discomfort, muscle soreness and feelings of stress and anxiety. Discussion/Conclusion: Our findings contribute to the existing evidence that long-term sleep loss is pervasive during initial military training programmes. The average sleep durations indicate chronic and unrecoverable sleep loss which would be expected to significantly impair physical and cognitive military performance, and increase the risk of injury, illness and attrition rates during basic training. Changes in the design and scheduling of basic training programmes to enable, at the least, minimum sleep recommendations to be met, and to improve sleep hygiene in the primary sleeping environment are warranted.

A Brief Sleep Intervention Improves Outcomes in the School Entry Year: A Randomized Controlled Trial

Abstract:OBJECTIVE: To determine the feasibility of screening for child sleep problems and the efficacy of a behavioral sleep intervention in improving child and parent outcomes in the first year of schooling. METHODS: A randomized controlled trial was nested in a population survey performed at 22 elementary schools in Melbourne, Australia. Intervention involved 2 to 3 consultations that covered behavioral sleep strategies for children whose screening results were positive for a moderate/severe sleep problem. Outcomes were parent-reported child sleep problem (primary outcome), sleep habits, psychosocial health-related quality of life, behavior, and parent mental health (all at 3, 6, and 12 months) and blinded, face-to-face learning assessment (at 6 months). RESULTS: The screening survey was completed by 1512 parents; 161 (10.8%) reported a moderate/severe child sleep problem, and 108 of 136 (79.2% of those eligible) entered the trial. Sleep problems tended to resolve more rapidly in intervention children. Sleep problems affected 33% of 54 intervention children versus 43% of 54 control children at 3 months (P = .3), 25.5% vs 46.8% at 6 months (P = .03), and 32% vs 33% at 12 months (P = .8). Sustained sleep-habit improvements were evident at 3, 6, and 12 months (effect sizes: 0.33 [P = .03]; 0.51 [P = .003]; and 0.40 [P = .02]; respectively), and there were initial marked improvements in psychosocial scores that diminished over time (effect sizes: 0.47 [P = .02]; 0.41 [P = .09]; and 0.26 [P = .3]; respectively). Better prosocial behavior was evident at 12 months (effect size: 0.35; P = .03), and learning and parent outcomes were similar between groups. CONCLUSIONS: School-based screening for sleep problems followed by a targeted, brief behavioral sleep intervention is feasible and has benefits relevant to school transition.

Abstract:This randomized-controlled trial examined the efficacy of Sleep Scholar, a brief, internet-based insomnia intervention tailored to the needs of college students. College students commonly experience insomnia and various other mental health symptoms, including suicide ideation. Cognitive-Behavioral Therapy for Insomnia (CBT-I) is a promising suicide prevention effort for college students because insomnia is robust risk factor for suicide ideation and CBT-I reduces suicide ideation. Moreover, CBT-I can be brief, self-guided, and internet-based. CBT-I also may elicit less stigma than treatment specifically targeting suicide ideation. However, existing forms of brief CBT-I are neither self-guided nor internet-based, and existing forms of self-guided, internet-based CBT-I are not brief. In addition, previous iterations of CBT-I are not typically designed to address the unique sleep needs of college students. For this registered clinical trial, we recruited 61 college students with at least subclinical insomnia symptoms. Participants were randomized to either Sleep Scholar or a control condition, Building Healthy Habits. Participants completed pre-treatment daily sleep diaries and surveys, a post-treatment assessment of acceptability and satisfaction, a one-week and one-month survey follow-up, and daily sleep diaries throughout the one-month follow-up period. Results showed that Sleep Scholar was more acceptable and satisfactory compared to the control condition at post-treatment. However, Sleep Scholar was not more effective for improving sleep or mental health symptoms compared to the control condition. These findings suggest that modifications to Sleep Scholar are needed to improve its efficacy.

Abstract:Abstract Introduction Insomnia is prevalent in older adults with mild cognitive impairment. Current treatment options are limited due to the nature of cognitive impairment and a paucity of providers. We sought to determine the preliminary effects of a multicomponent behavioral sleep intervention (MBSI) compared to a sleep education control condition on sleep latency, wake after sleep onset, total sleep time and efficiency. Methods Participants were enrolled and randomized in a 1:1 ratio to intervention or control in this pilot randomized controlled trial. Inclusion criteria were: self-reported and diary based evidence of insomnia (latency ≥ 30 minutes or wake after sleep onset ≥ 60 minutes) and Modified Telephone Interview for Cognitive Status scores between 28-36. The intervention was delivered virtually via tablet and consisted of sleep hygiene education, meaningful activity in the daytime and relaxation therapy in the evening for 4 weeks. The active control received sleep hygiene education only. Data collection occurred at baseline and immediately post intervention. Sleep variables were averaged over one week at each time point from daily sleep diaries. We used non-parametric Kruskal-Wallis tests to compare intervention and control post-intervention. Results We screened 180 potential participants and enrolled 27 older adults with mild cognitive impairment. Our sample was balanced on all key variables, had a mean age of 65.63 ± 6.39, was primarily (81.5%) female, and mixed between white (51.9%), Black (44.4%). Retention rate was high at almost 90%. At baseline, average latency was 46.5 (27.3) minutes for control and 41.4 (33.10) for intervention group. Post intervention, average latency for control group was 47.0 (34.6) and 28.3 (31.8) for intervention (p=0.076) Standardized mean difference (SMD) was 0.56. Average efficiency for control group was 65% (11%) and 70% (12%) for intervention at baseline. Post intervention, efficiency was 68% (14%) for control group and 78% (12%) for intervention (p=0.044). SMD was 0.76. There were no significant differences between groups for wake after sleep onset or total sleep time. Conclusion Preliminary evidence supports testing MBSI on a larger scale to improve insomnia symptoms in older adults with mild cognitive impairment and to determine if these effects are sustained over time. Abstract:Abstract Introduction Insomnia is the most prevalent sleep disorder among active duty service members (ADSMs) and compromises readiness. Cognitive-behavioral therapy for insomnia (CBTI) is the DoD/VA recommended treatment, but access and delivery of CBTI is impeded by gaps between high patient demand for care and clinical capabilities. We evaluated the effectiveness of a digital clinical decision support (CDS) platform to overcome current CBTI capability gaps. Methods Mental healthcare providers (MHCPs) at military clinics were offered access to a novel CDS platform (COAST, NOCTEM® Health, Inc) to treat patients they deemed appropriate for this CBTI delivery mode. The platform consists of (1) clinician portal to remotely monitor and manage patients’ symptoms, progress, and adherence to algorithm-based, MHCP-approved treatment recommendations; (2) patient app that prospectively collects sleep diaries and displays MHCP-approved treatment recommendations (e.g., stimulus control, sleep restriction). The primary outcomes were treatment response (50% reduction in sleep latency (SL) or wake after sleep onset (WASO); increase of > 10% in sleep efficiency (SE)) and insomnia remission (response & SL & WASO < 30 minutes with SE > 85%) at the end of treatment. The magnitude of changes in diary-based SL, WASO, and %SE were compared from baseline and at the final intervention was quantified using Cohen’s d effect sizes. Results Nineteen MHCPs at 7 Air Force Bases, 2 Naval hospitals, and 1 Army medical center utilized the platform with 245 ADSMs presenting with insomnia (M age =32.6 + 8.0 y.o.; 27.3% women, 78% Sailors/Marines, 19% Airmen, 3% Soldiers). Average treatment duration was 5 ±1 weeks. The mean completion rate of diaries was 77% + 20%. Post-treatment, 83.5% of ADSMs met treatment response criteria, and 70.3% met remission criteria. From baseline to the last intervention, clinically meaningful improvements in SL, WASO, and %SE were observed (all Cohen’s d > .5). Conclusion Augmentation of CBTI capabilities among MHCP using a digital CDS platform is feasible and yields rapid and clinically meaningful improvements in sleep among ADSMs with insomnia. Low-intensity sleep intervention in a youth mental health service: a case series analysis

Abstract:Background: There is increasing evidence of a strong association between sleep and mental health in both adolescents and adults. CBT for insomnia is being applied to good effect with adults with mental health difficulties but there are few studies examining its applicability to adolescents within mental health services. Method: We carried out a case series analysis (n = 15) looking at the feasibility, accessibility and impact of a low-intensity sleep intervention for young people (14-25 years) being seen by a secondary care Youth Mental Health team in the UK. The intervention was based on cognitive behavioural therapy for insomnia (CBTi) and acceptance and commitment therapy (ACT) approaches and involved six individual sessions delivered on a weekly basis by a graduate psychologist. Routine outcome measures were used to monitor insomnia, psychological distress and functioning with assessments at baseline, session 3, session 6 and at 4 weeks after end of intervention. All participants scored in the clinical range for insomnia at the start of the study. Results: High uptake, attendance and measure completion rates were observed. Large effect sizes were observed for insomnia, psychological distress and functioning. Twelve of the fifteen participants (80%) no longer scored above threshold for insomnia at follow-up. All seven under-18s no longer met threshold for clinical 'caseness' on the Revised Child Anxiety and Depression Scale (RCADS) at follow-up. Discussion: The findings suggest that the intervention was well accepted by young people and feasible to apply within a secondary care setting. Strong effect sizes are encouraging but are probably inflated by the small sample size, uncontrolled design and unblinded assessments.

Abstract:Abstract Introduction Insufficient sleep and circadian disruption have both psychological and physiological consequences that impact the health and safety of military service members, yet research shows only about one-third of service members meet recommended sleep duration guidelines. To address this issue, interventions must be developed that are tailored to meet the specific needs of this community. Although substantial research has identified the adverse impacts of poor sleep, limited research has examined the underlying contextual factors that contribute to it between service branches or in different operational settings. Thus, the objective of this qualitative study was to identify factors that impede sleep health among active duty Marines. Methods Four focus groups were conducted with 23 active duty Enlisted Marines (87% male; 13% female). The semi-structured focus group guide assessed factors that impact sleep in Marines. Interview transcripts were analyzed using applied thematic analysis by two independent coders. Results All focus group participants reported low satisfaction with their sleep quality or duration. The most commonly reported barriers to sleep were work schedules. Many participants reported regularly working long and/or varied hours as part of their standard schedule, and others described unpredictable schedules due to frequently being kept at work late to complete unfinished tasks or for punitive reasons. Participants also described how Marine Corps culture influenced their attitudes towards sleep and sleep-related behavior — even when acknowledging the adverse effects of insufficient sleep on performance and mental health, participants described forgoing it for the sake of the mission, e.g., “It's very important to keep in mind that mission accomplishment is more important than, like, a night of sleep.” Conclusion Poor sleep and circadian health were viewed by study participants as inherent to military service. However, the most salient barriers reported by Marines are potentially modifiable through scheduling interventions and interventions designed to reduce sleep stigma and promote healthy sleep hygiene. Abstract:Objectives To evaluate the impact of a mentoring program to encourage staff-delivered sleep-promoting strategies on sleep, function, depression, and anxiety among skilled nursing facility (SNF) residents. Design Modified stepped-wedge unit-level intervention. Setting and Participants Seventy-two residents (mean age 75 ± 15 years; 55.9% female, 41% non-Hispanic White, 35% Black, 20% Hispanic, 3% Asian) of 2 New York City urban SNFs. Methods Expert mentors provided SNF staff webinars, in-person workshops, and weekly sleep pearls via text messaging. Resident data were collected at baseline, post-intervention (V 1 ), and 3-month follow-up (V 2 ), including wrist actigraphy, resident behavioral observations, Pittsburgh Sleep Quality Index (PSQI), Patient Health Questionnaire-9 (PHQ-9) depression scale, Brief Anxiety and Depression Scale (BADS), Brief Cognitive Assessment Tool (BCAT), and select Minimum Data Set 3.0 (MDS 3.0) measures. Linear mixed models were fit for continuous outcomes and mixed-effects logistic models for binary outcomes. Outcomes were modeled as a function of time. Planned contrasts compared baseline to V 1 and V 2 . Results There was significant improvement in PSQI scores from baseline to V 1 ( P = .010), and from baseline to V 2 ( P = .007). Other significant changes between baseline and V 1 included decreased depression (PHQ-9) ( P = .027), increased daytime observed out of bed ( P ≤ .001), and increased daytime observed being awake ( P < .001). At V 2 (vs baseline) being observed out of bed decreased ( P < .001). Daytime sleeping by actigraphy increased from baseline to V 1 ( P = .005), but not V 2 . MDS 3.0 activities of daily living and pain showed improvements by the second quarter following implementation of SLUMBER ( P 's ≤ .022). There were no significant changes in BADS or BCAT between baseline and V 1 or V 2 . Conclusions and Implications SNF residents had improvements in sleep quality and depression with intervention, but improvements were not sustained at 3-month follow-up. The COVID-19 pandemic led to premature study termination, so full impacts remain unknown.

Abstract:The military lifestyle often includes continuous operations whether in training or deployed environments. These stressful environments present unique challenges for service members attempting to achieve consolidated, restorative sleep. The significant mental and physical derangements caused by degraded metabolic, cardiovascular, skeletomuscular, and cognitive health often result from insufficient sleep and/or circadian misalignment. Insufficient sleep and resulting fatigue compromises personal safety, mission success, and even national security. In the long-term, chronic insufficient sleep and circadian rhythm disorders have been associated with other sleep disorders (e.g., insomnia, obstructive sleep apnea, and parasomnias). Other physiologic and psychologic diagnoses such as post-traumatic stress disorder, cardiovascular disease, and dementia have also been associated with chronic, insufficient sleep. Increased co-morbidity and mortality are compounded by traumatic brain injury resulting from blunt trauma, blast exposure, and highly physically demanding tasks under load. We present the current state of science in human and animal models specific to service members during- and post-military career. We focus on mission requirements of night shift work, sustained operations, and rapid re-entrainment to time zones. We then propose targeted pharmacological and non-pharmacological countermeasures to optimize performance that are mission- and symptom-specific. We recognize a critical gap in research involving service members, but provide tailored interventions for military health care providers based on the large body of research in health care and public service workers.

1108 Prior-night Sleep Predicts Next-day Symptoms over Ten Days Among Military Personnel with Sleep Problems

Abstract:Abstract Introduction Insufficient and disturbed sleep are highly prevalent and associated with adverse consequences in the U.S. military. The purpose of this study was to employ remote monitoring/ecological momentary assessment methods to test the hypothesis that prior-night sleep is associated with next-day subjective symptoms among military personnel. Methods Participants (N=270) with sleep problems were recruited from two military treatment facilities and completed an intensive ten-day remote monitoring assessment. Prior night sleep was measured using standardized sleep diaries and a commercial wearable sleep tracker (Fitbit Inspire 2). Next-day symptoms were measured using twice-daily symptom surveys (i.e., 20 surveys over ten days). Individual items assessed subjective cognition, energy level, and mood using an “I feel tired” format and scored from 0-4. To determine the lagged impact of prior-night sleep on next-day symptoms, a total of 40 mixed models (MMs) were developed. Individual prior-night sleep diary (total sleep time [TST], sleep efficiency [SE], and sleep quality [QUAL]) and Fitbit (TST, SE) parameters were entered as independent variables, and individual daytime symptoms (feeling alert, clear-headed, refreshed, fatigued, happy, sad, stressed, and relaxed) were entered as dependent variables. All models controlled for age and sex. The Benjamini-Hochberg procedure for correcting false discovery rate (BH-FDR) method was used to correct for multiple comparisons. Results Lagged MM analyses revealed that all prior-night sleep diary variables were significantly associated with next-day symptoms (all ps< 0.001 with df=2081) over ten days. Specifically, prior-night sleep diary parameters (TST, SE, QUAL) were positively associated with next-day feeling alert, clear-headed, refreshed, happy, and relaxed; and negatively associated with next-day feeling fatigued, sad, and stressed. Prior-night Fitbit sleep parameters were significantly associated with most next-day symptoms (largest p=0.023 with df=1956). Specifically, Fitbit TST and SE were positively associated with next-day feeling clear-headed, refreshed, happy, and relaxed and negatively associated with next-day fatigue. Fitbit TST was negatively associated with next-day stress. Conclusion Among military personnel with sleep problems, prior-night sleep predicts next-day subjective cognition, energy level, and mood over ten days. Future research should employ remote monitoring approaches to predict treatment response and deliver personalized care to improve sleep and daytime symptoms. Abstract:Sleep is a major health issue for young adults. While sleep hygiene interventions have small-to-moderate effects, better outcomes might be achieved by delivering interventions individually to those reporting sleep difficulties, or in the presence of a supportive other or companion. To examine the feasibility, acceptability, and initial effects of an adapted sleep hygiene intervention for young adults with poor sleep, we conducted an ORBIT Phase IIb pilot randomized controlled trial (n = 51). Young adults with poor sleep were randomized to receive a sleep hygiene intervention individually or with a companion. Sleep patterns were measured at baseline and 4 weeks. Participants completed daily dairies and wore wearable devices for the month following the intervention. Attrition at follow-up was minimal (3.9%), and missing rates were low for diary data (8%), and in line with prior studies for wearable devices (14%). Participants expressed high levels of satisfaction with the study, with no differences between study arms. Significant effects were found postintervention for all participants, indicating improved sleep quality (t = 6.14, P < .001), greater sleep duration (t = 3.8, P < .001), and reduced daytime sleepiness (t = 2.13, P = .039). There were no significant differences in sleep efficiency pre and postintervention, or between study arms for any sleep outcome based on self-reported and objective data. Results from this pilot provide evidence for the feasibility and acceptability of a sleep hygiene intervention for young adults and lay the groundwork for a larger, fully powered study.
Treating sleep problems in young children: A randomised controlled trial of a group-based, parent-focused behavioural sleep intervention

Abstract:This study tested the efficacy of a 5 × 1.5 h/session, group-based, parent-focused, behavioural intervention (BI) targeting sleep problems in preschool children. Parents were randomised to either the BI (N = 62) or care as usual (CAU; N = 66) conditions. Outcomes included sleep, anxiety, behavioural problems, internalising and externalising symptoms, transition to school and academic achievement. Assessments were conducted at pre- and post-BI intervention (in the year prior to formal schooling), and then at follow-ups 1 and 2 in the first year of formal schooling. Relative to the CAU, the BI condition demonstrated significantly greater improvements in sleep, anxiety, behaviour problems and internalising and externalising symptoms from pre-to post-intervention. Improvements in sleep, anxiety, and internalising symptoms were maintained, while behaviour and externalising symptoms were further improved upon at school follow-up 2. For the BI group, improvements in sleep at post-intervention were found to mediate improvements in anxiety, internalising, and externalising symptoms, but not behaviour problems, at school follow-ups 1 and 2. There were no significant effects of condition on school transition or academic outcome measures. The results suggest that the BI is effective for sleep, anxiety, behaviour, internalising and externalising symptoms, but not for school transition or academic outcomes. SleepSync: Early Testing of a Personalised Sleep–Wake Management Smartphone Application for Improving Sleep and Cognitive Fitness in Defence Shift Workers

Abstract:Shift work, long work hours, and operational tasks contribute to sleep and circadian disruption in defence personnel, with profound impacts on cognition. To address this, a digital technology, the SleepSync app, was designed for use in defence. A pre-post design study was undertaken to examine whether four weeks app use improved sleep and cognitive fitness (high performance neurocognition) in a cohort of shift workers from the Royal Australian Air Force. In total, 13 of approximately 20 shift-working personnel from one base volunteered for the study. Sleep outcomes were assessed using the Insomnia Severity Index (ISI), the Patient-Reported Outcomes Measurement Information System (PROMIS), Sleep Disturbance and Sleep-Related Impairment Scales, the Glasgow Sleep Effort Scale, the Sleep Hygiene Index, and mental health was assessed using the Depression, Anxiety, and Stress Scale-21. Sustained attention was measured using the 3-min Psychomotor Vigilance Task (PVT) and controlled response using the NBack. Results showed significant improvements in insomnia (ISI scores 10.31 at baseline and 7.50 after app use), sleep-related impairments (SRI T-scores 53.03 at baseline to 46.75 post-app use), and healthy sleep practices (SHI scores 21.61 at baseline to 18.83 post-app use; all p < 0.001). Trends for improvement were recorded for depression. NBack incorrect responses reduced significantly (9.36 at baseline; reduced by −3.87 at last week of app use, p < 0.001), but no other objective measures improved. These findings suggest that SleepSync may improve sleep and positively enhance cognitive fitness but warrants further investigation in large samples. Randomised control trials with other cohorts of defence personnel are needed to confirm the utility of this intervention in defence settings.
