Abstract:Following a major shortage of 99 Mo in the 2009–2010 period, concern grew that the aging reactor production facilities needed to be replaced. Most producers were using highly enriched 235 U (HEU) as the target material. The Organisation for Economic Co-operation and Development and the International Atomic Energy Agency sought to remedy these issues by removing HEU from medical isotope production and implementing full cost recovery to enable new production entities to compete with the existing multipurpose reactor facilities, which were heavily subsidized by their respective governments. This review examines the various approaches to producing 99 Mo and/or 99m Tc with a critical eye toward their potential success in ( a) producing the medical isotopes and ( b) being able to successfully enter and compete in the market. Because many of the new approaches are adapting existing technologies for commercial businesses, some of the details are of a proprietary nature and not available for in-depth technical review.

Developing a new target design for producing 99Mo in a MTR reactor

Abstract:99Mo is an important radioisotope and mainly produced using uranium fission reaction in a nuclear reactor. Investigation for probable improvements, especially on target geometry and in-core location of target is the main goal of this research. This is for producing more efficient of 99Mo in a typical Material Testing Reactors (MTRs). A parametric investigation is done focused on the target characteristics such as geometry, location, material, density, heat flux (power density) and also usability in Tehran Research Reactor (TRR) as a MTR case study. Stochastic code MCNPX 2.6.0 along with CFD code ANSYS are used to perform neutronic and thermal-hydraulic analyses. A target with plate type design is specified and proposed as a final and most favorable design. Taking into account the safety criteria, the production yield, the chemical process and radioactive waste, it is demonstrated that the new target design meets the key design requirements without compromising the reactor safety. This research results indicate that the new target gives rise to a higher production of 99Mo using less amount of initial material causing to a reduction in nuclear waste and process difficulties.
Production of Molybdenum-99 by (n, ) activation and direct separation of Technetium-99m without column generator fabrication: A viable strategy for enhanced availability of technetium-99m

Abstract:Fission-produced 99Mo (F 99Mo) is traditionally used for fabrication of 99Mo/99mTc adsorption-type column generators. In this paper, several emerging strategies that are being pursued or have been suggested to overcome the continuing shortages of F 99Mo are discussed. To provide an alternative source of 99Mo, the principal focus of this analysis is a detailed discussion of the advantages and strategies for enhanced production of low-specific-activity 99Mo (LSA 99Mo) by direct activation of molybdenum targets in nuclear reactors. In order to enhance the availability of 99Mo, development of an increased network of reactors for production of LSA 99Mo is described, as well as utilization of currently unused reactors. The time spent in manufacturing of 99Mo/99mTc column generators is responsible for the loss of more than 50% of F99Mo produced. Hence, the authors propose a paradigm shift in the use of 99Mo by recovering clinical-grade 99mTc from 99Mo solution as an alternative to use of 99Mo/99mTc column generators, thereby avoiding substantial decreased availability of 99Mo from radioactive decay. Implementation of the suggested strategies would be expected to increase availability of 99mTc to the clinical user community by several folds. Nicorandil-induced severe oral ulceration.

Abstract:Nicorandil has been shown to precipitate persistent ulcerative stomatitis in a small percentage of patients. To date over 50 cases have been reported, initially in France and, more recently, in the United Kingdom and Portugal, but this is the first report of nicorandil-induced ulceration in Irish medical or dental literature. It has been suggested that a previous history of aphthous ulceration may predispose the development of nicorandil-induced ulcers, although onlyone of these patients reported a history of recurrent oral ulceration (ROU).

Abstract:At the Research Laboratories of AB Leo in Helsingborg Sweden information was sought about the hormonal dependence of phosphatases in the rat ventral prostate. For treatment of rats 4 long-acting estrogens and 2 short-acting estrogens were used. The long-acting estrogens were polyestradiol phosphate (PEP) polydiethylstilbestrol phosphate (PSP) polyestriol phosphate (SEP) and estrodiol-17-undecylate (EU). The short-acting estrogens were honvan and estracyt. In intact rats treated with PEP PSP EU H or E testis weight decreased. Thus these compounds had pronounced antigonadotropic effects. Atrophy of the ventral prostate accompanied the weight decreases this indicating marked reduction in the release of androgen. SEP had only a weak antigonadotropic effect. Alkaline and acid phosphatase activities in the ventral prostate were decreased after castration. Although injection of PEP PSP OR EU into intact rats brought ventral atrophy as great as occurred with castration phosphatase activities remained largely unchanged. Thus estrogens may affect the ventral prostate in ways other than by merely causing a secondary decrease in androgen release. Supporting this hypothesis were the facts that 1) phosphatase activities remained largely unchanged in castrated rats after treatment with PEP PSP EU or E and 2) estrogen increased acid and alkaline phosphatase activities in androgen-treated castrated rats.
Initial development of an alkaline process for recovery of uranium from 99Mo production process waste residue

Abstract:An ammonium carbonate-based process is being developed for the recovery of uranium from the residue generated by an alkaline dissolution process used in the production of the medical isotope 99Mo. As a unique application of this dissolution medium, this is the first attempt at uranium recovery from a commercial 99Mo production process. Dissolution parameters were optimized with unirradiated residue material and applied to irradiated residue in a small hot cell. Complete recovery of uranium from the residue was achieved with three successive ammonium carbonate/peroxide leaches with final decontamination factors ranging from low values of 1–10 (137Cs, 106Ru, 125Sb) to infinity (lanthanides).

Abstract:Past and prospective shortages of medical radioisotopes have driven recent developments in the direct production of 99mTc via the 100Mo(p,2n)99mTc reaction. The cyclotron-based production method has been shown to successfully produce 99mTc, however trace impurities present in the enriched molybdenum target can also lead to the unintended creation of other radioisotopes which constitute waste. The isotopic composition of the waste has to be investigated in order to determine how it can be handled, transported and safely stored. In this article, we report which waste radioisotopes are created alongside 99mTc during target irradiation. Results are based on the gamma spectroscopy of waste produced. Significant complexities in the emission spectra made automated identification of radioisotopes inaccurate; complexities were resolved using a manual radioisotope identification procedure. The impact of target composition, integrated beam current and duration of target irradiation on the waste produced was studied. Results indicate that an average of 0.059 ± 0.003 GBq of waste is generated per 1 GBq of 99mTc produced. Two-thirds of the total waste activity produced was attributed to 99Mo (T 1/2 = 66 h) alone, while a total of fifty radioisotopes were found in the waste. Long-lived isotopes (T 1/2 > 2 months) constituted only 1% of the total waste activity at end of beam (EOB). In conclusion, it was determined that the waste generated during cyclotron-based 99mTc production was acceptably low for routine clinical production.

Abstract:The concept of theranostics in nuclear medicine is based on the use of a pair of radioisotopes to label radiopharmaceuticals for both diagnosis and therapy and is essential for nuclear medicine developments. The production of novel medical radioisotopes using solid target stations is challenging and new instruments and methods are needed. A research program is ongoing at the 18 MeV Bern medical cyclotron, equipped with a Solid Target Station and a 6.5 m Beam Transfer Line ending in a separate bunker. To irradiate isotope-enriched materials in form of compressed powder pellets, a novel target coin was conceived and realized together with methods to assess the beam energy and the production cross sections. To optimize the irradiation procedure, a novel ultra-compact active irradiation system based on a specific magnetic lens and a two-dimensional beam detector was conceived, constructed and tested. The system allows to control on-line the size and position of the beam and to correct its characteristics by steering and focusing it in order to keep it on target. The first results on the production of several radionuclides ( 43 Sc, 44 Sc, 47 Sc, 61 Cu, 64 Cu, 67 Cu, 68 Ga, 165 Er, 165 Tm, 167 Tm and 155 Tb) are presented.

Abstract:Whatever the radionuclide to be used in nuclear medicine, it is essential to know the expected yield during the production process, but also of all the possible radionuclidic impurities coproduced, that can have an impact on the product final quality, as well as in the related waste management. The availability of the majority of emerging radioisotopes, including the theranostic ones or pairs, is mainly limited by the fact that, for most of them, the optimal production route still needs to be strengthened if not defined in some cases. The aim of this work is to present a review on the charged particle induced nuclear cross sections to produce some emerging radionuclides for medical applications to show that all types of projectiles should be considered in the quest of producing medical radionuclides. An accurate analysis of the production routes is presented for some radionuclides ( 67 Cu, 47 Sc, 89 Zr, 103 Pd, 186g Re, 97 Ru, 211 At) chosen as examples to highlight (i) how the quality of the final product strongly depends on the chosen target/projectile/energy parameters set, (ii) how deuteron production routes may sometimes be more effective than the proton ones or lead to a different impurity profile and (iii) how α-particle beams may allow to bypass the limitations occurring when using Z = 1 beams. An overview of possible advantages and drawbacks of the cited production routes and of potential cross sections that still need to be measured, is also reported.

Abstract:The relationship between the cultural and biochemical characteristics of 22 strains of Bacteroides nodosus and their virulence for sheep was examined. Virulent, intermediate and benign strains were recognised. Although there was some relationship between virulence and colony morphology on hoof medium with 4% agar, colonies of one virulent and 4 intermediate strains resembled those of benign strains. However, on hoof medium with 2% agar and on blood Euonagar, colonies of this virulent and one intermediate strain differed from each other and the other 3 intermediate strains, which in turn differed from the benign. The degree of piliation, as assessed by electron microscopy, was not a reliable indicator of virulence in strains not possessing a beaded colony type. Together, the results of colony morphology and proteolytic tests such as zymogram, degrading proteinase and elastin-agar tests allowed better discrimination of virulent and benign strains. Intermediate strains generally possessed virulent protease activity. In strains with benign zymogram patterns, activity bands 2 and 3 were more labile than in strains with virulent patterns. The addition of CaCl2 to the culture medium resulted in greater stability of proteolytic activity, particularly with benign strains, and prevented the disappearance of protease activity in the band 5 position in virulent, intermediate and benign strains during prolonged incubation. There were slight differences in the sodium dodecyl sulphate polyacrylamide gel electrophoresis (SDS-PAGE) patterns of outer membrane proteins of some benign strains but those of intermediate category resembled virulent strains. There was some relationship between the apparent Mr of the pilin monomer on SDS-PAGE gels and serogroup specificity.

Abstract:One of the most effective methods for producing radioisotopes aimed at Nuclear Medicine applications, such as 99mTc and 99Mo, is to use a compact proton accelerator located near hospitals. Target utilization efficiency is a critical factor in the production of these isotopes, through the 100Mo(p, 2n)99mTc and 100Mo(p, pn)99Mo nuclear reaction routes. To maximize the utilization efficiency, a multi-layer target structure was developed aimed at achieving a high radionuclidic purity (RNP) level. The yield and RNP of different target structures were calculated and compared by using FLUKA code, and benchmark experiments were conducted using a proton beam at the China Spallation Neutron Source (CSNS) Associated Proton Experimental Platform (APEP). Results showed that the multi-layer target structure has the potential to significantly reduce production costs compared to a single-layer target. The multi-layer target structure is being adopted for the production of both 99mTc and 99Mo at APEP, with an expected 99mTc yield of up to 60 MBq enough for preclinical studies after 100Mo target being irradiated for about 2 h at a beam intensity of 62.5 μA.

Abstract:Efficient and safe production of molybdenum-99 (99Mo) radiopharmaceutical at Tehran Research Reactor (TRR) via fission of LEU targets is studied. Neutronic calculations are performed to evaluate produced 99Mo activity, core neutronic safety parameters and also the power deposition values in target plates during a 7 days irradiation interval. Thermal-hydraulic analysis has been also carried out to obtain thermal behavior of these plates. Using Thermal-hydraulic analysis, it can be concluded that the safety parameters are satisfied in the current study. Consequently, the present neutronic and thermal-hydraulic calculations show efficient 99Mo production is accessible at significant activity values in TRR current core configuration.
Optimization of Deuteron Irradiation of 176Yb for Producing 177Lu of High Specific Activity Exceeding 3000 GBq/mg

Abstract:The irradiation of 176Yb with deuterons offers a promising pathway for the production of the theranostic radionuclide 177Lu. To optimize this process, calculations integrating deuteron transport, isotope production, and decay have been performed. In pure 176Yb, the undesired production of 174g+mLu occurs at higher deuteron energies, corresponding to a distribution slightly shallower than that of 177Lu. Hence, 174g+mLu can be effectively filtered out by employing either a low-energy deuteron beam or stacked foils. The utilization of stacked foils enables the production of 177Lu using a high-energy linear accelerator. Another unwanted isotope, 176mLu, is produced roughly at the same depth as 177Lu, but its concentration can be significantly reduced by selecting an appropriate post-irradiation processing time, owing to its relatively short half-life. The modeling approach extended to the mapping of yields as a function of irradiation time and post-irradiation processing time. An optimized processing time window was identified. The study demonstrates that a high-energy deuteron beam can be employed to produce 177Lu with high specific activity exceeding 3000 GBq/mg. The effect of different purity levels (ranging from 98% to 100%) was also discussed. The impurity levels have a slight impact. The modeling demonstrates the feasibility of obtaining 177Lu with a specific activity > 3000 GBq/mg and radionuclidic purity > 99.5% when using a commercially available 176Yb target of 99.6% purity.

Abstract:Recent clinical success with metal-based radiopharmaceuticals has sparked an interest in the potential of these drugs for personalized medicine. Although often overlooked, the success and global impact of nuclear medicine is contingent upon the purity and availability of medical isotopes, commonly referred to as radiometals. For nuclear medicine to reach its true potential and change patient lives, novel production and purification techniques that increase inventory of radiometals are desperately needed. This tutorial review serves as a resource for those both new and experienced in nuclear medicine by providing a detailed explanation of the foundations for the production and purification of radiometals, stemming from nuclear physics, analytical chemistry, and so many other fields, all in one document. The fundamental science behind targetry, particle accelerators, nuclear reactors, nuclear reactions, and radiochemical separation are presented in the context of the field. Finally, a summary of the latest breakthroughs and a critical discussion of the threats and future potential of the most utilized radiometals is also included. With greater understanding of the fundamentals, fellow scientists will be able to better interpret the literature, identify knowledge gaps or problems and ultimately invent new production and purification pathways to increase the global availability of medical isotopes.

Abstract:Background: the gamma-emitting radionuclide Technetium-99m (99mTc) is still the workhorse of Single Photon Emission Computed Tomography (SPECT) as it is used worldwide for the diagnosis of a variety of phatological conditions. 99mTc is obtained from 99Mo/99mTc generators as pertechnetate ion, which is the ubiquitous starting material for the preparation of 99mTc radiopharmaceuticals. 99Mo in such generators is currently produced in nuclear fission reactors as a by-product of 235U fission. Here we investigated an alternative route for the production of 99Mo by irradiating a natural metallic molybdenum powder using a 14-MeV accelerator-driven neutron source. Methods: after irradiation, an efficient isolation and purification of the final 99mTc-pertechnetate was carried out by means of solvent extraction. Monte Carlo simulations allowed reliable predictions of 99Mo production rates for a newly designed 14-MeV neutron source (New Sorgentina Fusion Source). Results: in traceable metrological conditions, a level of radionuclidic purity consistent with accepted pharmaceutical quality standards, was achieved. Conclusions: we showed that this source, featuring a nominal neutron emission rate of about 1015 s-1, may potentially supply an appreciable fraction of the current 99Mo global demand. This study highlights that a robust and viable solution, alternative to nuclear fission reactors, can be accomplished to secure the long-term supply of 99Mo.

Abstract:Targeted radionuclide therapy (TRT) is an emerging field and has the potential to become a major pillar in effective cancer treatment. Several pharmaceuticals are already in routine use for treating cancer, and there is still a high potential for new compounds for this application. But, a major issue for many radiolabeled low-to-moderate-molecular-weight molecules is their clearance via the kidneys and their subsequent reuptake. High renal accumulation of radioactive compounds may lead to nephrotoxicity, and therefore, the kidneys are often the dose-limiting organs in TRT with these radioligands. Over the years, different strategies have been developed aiming for reduced kidney retention and enhanced therapeutic efficacy of radioligands. In this review, we will give an overview of the efforts and achievements of the used strategies, with focus on the therapeutic potential of low-to-moderate-molecular-weight molecules. Among the strategies discussed here is coadministration of compounds that compete for binding to the endocytic receptors in the proximal tubuli. In addition, the influence of altering the molecular design of radiolabeled ligands on pharmacokinetics is discussed, which includes changes in their physicochemical properties and implementation of cleavable linkers or albumin-binding moieties. Furthermore, we discuss the influence of chelator and radionuclide choice on reabsorption of radioligands by the kidneys.

Abstract:Abstract Technitium-99m (99mTc), a widely used radioisotope, is used in tens of millions of medical diagnostic procedures annually. However, it is hard to store and must be immediately used upon production due to its short half-life (i.e., 6 h); thus, it is currently produced from 99Mo, which itself is a result of 235U fission. The majority of 99Mo supplies to U.S. patients are currently provided by foreign producers and produced using highly enriched uranium (HEU). In order to minimize the proliferation risks of HEU-based medical isotope production, the U.S. Department of Energy’s National Nuclear Security Administration has funded a program to accelerate the development of technologies to produce 99Mo without the use of HEU. Today, the global supply of 99Mo depends on a limited number of nuclear reactors, and production has been interrupted unexpectedly since 2009 due to the fleet’s advanced age. Alternative options for 99Mo production are discussed in this paper, and one potential option is to obtain 99mTc from molten salt reactors (MSRs). A MSR is a nuclear fission reactor that can operate at or close to atmospheric pressure with liquid fuel, which allows for producing isotopes in a timely manner. In this paper, the past and current production of 99Mo via nuclear reactors is described, and the future of 99Mo production by MSRs is discussed. The behavior and chemical properties of molybdenum in fluoride salts in MSRs and the possible extraction methods are also examined in addition to the limitation of current studies.

Abstract:The objective of this study was to examine the effect of postactivation treatment with latrunculin A (LatA), an actin polymerization inhibitor, on in vitro and in vivo development of somatic cell nuclear transfer (SCNT) embryos derived from kidney fibroblasts of an aged Clawn miniature boar (12 years old). After electric activation, SCNT embryos were treated with 0, 0.5 or 1 μM LatA and cultured in vitro. The rate of blastocyst formation was significantly higher (P<0.05) in SCNT embryos treated with 0.5 μM LatA (38%) than those in control (14%). When cloned embryos treated with 0.5 μM LatA were transferred into the oviducts of two recipient miniature gilts to assess their development in vivo, both recipients became pregnant; one maintained pregnancy to term, and a live piglet (weighing 220 g) was delivered by Caesarean section. The results of this study indicated that the postactivation treatment with LatA was effective in improving in vitro developmental capacity of SCNT miniature pig embryos derived from kidney fibroblasts of an aged animal and that miniature pig cloned embryos treated with LatA had the ability to develop to term.
