Abstract:Objective To investigate changes of calcium transport protein activities of aortic smooth muscle cell in progress of hypertension,and to elucidate the relationship between blood-presurelowering effect of irbesartan and changes of this protein.Methods Sixteen 16-week-old SHR were randomized into irbesartan treated groupGBT group,n = 8) and distilled water group(SHR group,n = 8) ,and Wistar Kyoto rats(WKY,n = 8) served as normal controls.Irbesartan and appropriate amount of distilled water were administered respectively for 14 weeks by gavage.Blood pressure of caudal artery was examined before and after treatment,activities of Na~+-K~+-ATPase and Ca~(2+)-Mg~(2+)-ATPase of thoracic aorta smooth muscle cell were measured.Results After medication for 14 weeks,blood pressure in IBT group and WKY group was much lower than that in SHR group.Compared with SHR group,activities of Na~+-K~+-ATPase and Ca~(2+)-Mg~(2+)-ATPase of thoracic aorta smooth muscle cell were significantly higher in both IBT group and WKY group. Activities of Na~+-K~+-ATPase and Ca~(2+)-Mg~(2+)-ATPase of thoracic aorta smooth muscle cell were negatively correlated to blood pressure (r =— 0.446,r =— 0.387,P < 0.01).Conclusion The changes of calcium transport protein activities of aortic smooth muscle cell play an important role in the development of hypertension.Administration of Irbesartan for 14 weeks ameliorated effectively calcium transport protein activities of vascular smooth muscle cells.

Abstract:Objective To investigate the efficacy and safety of valsartan in treatment of essential hyˉpertensive patients.Methods Seventy three patients with essential hypertension received valsartan80mg daily for8weeks;for some non-responded patients hydrochlorothiazide(12.5mg/25mg daily)was added.Results Valsartan reduced sitting blood pressure from(156.43±10.46)/(95.76±3.79)mmHg to(134.56±9.15)/(82.38±6.01)mmHg(t=19.020,18.983,P0.001).Meanwhile standing blood pressure was reduced from(154.54±11.06)/(94.37±4.67)mmHg to(133.75±9.40)/(81.76±6.00)mmHg(t=16.584,17.169,P0.001),and no patient had orthostatic hypotension caused by valsartan.The effective rate of valsartan was77.78%.Valsartan also reduced proteinuria.Conclusions In mild-moderate essential hypertensive patients,Valsartan was effective and well tolerated and offers a promising approach to the treatment of hypertension.
Changes to the gut microbiota induced by losartan contributes to its antihypertensive effects

Abstract:Background and purpose: Hypertension is associated with gut dysbiosis. Here we have evaluated the effects of the angiotensin receptor antagonist losartan on gut microbiota in spontaneously hypertensive rats (SHR) to assess their contribution to its antihypertensive effects. Experimental approach: Twenty-week-old Wistar Kyoto rats (WKY) and SHR were treated with losartan for 5 weeks (SHR-losartan). Faecal microbiota transplantation (FMT) was performed from donor SHR-losartan group to recipient untreated-SHR. Blood pressure (BP) was measured using tail-cuff plethysmography. Composition of the gut microbiota was assessed by amplification of the V3-V4 region of 16S rRNA gene. T cells were analysed in gut/aorta by flow cytometry. Key results: Faeces from SHR showed gut dysbiosis, characterised by higher Firmicutes/Bacteroidetes ratios, lower acetate- and higher lactate-producing bacteria, and lower levels of strict anaerobic bacteria, effects which were restored to normal by losartan. Improvement of gut dysbiosis was linked to higher colonic integrity and lower sympathetic drive in the gut. In contrast, hydralazine reduced BP, but it neither restored gut dysbiosis nor colonic integrity. FMT from SHR-losartan to SHR reduced BP, improved the aortic endothelium-dependent relaxation to ACh, and reduced NADPH oxidase activity. These vascular changes were accompanied by both increased Treg and decreased Th17 cell populations in the vascular wall. Conclusion and implications: In SHR, losartan treatment reduced gut dysbiosis and sympathetic drive in the gut, thus improving gut integrity. The changes induced by losartan in gut microbiota contributed, in part, to protecting the vasculature and reducing BP, possibly by modulating the immune system in the gut.
The use of medication compliance devices by district nursing services.

Abstract:This article presents a critical review of the literature relating to medication compliance devices and the findings of a survey that examined the use of such devices by district nursing services. The UKCC (1992) does not regard the loading of compliance devices by nurses as safe practice; however, compliance devices continue to be used by district nurses. The evidence base concerning the value and use of medication compliance devices is examined and significant gaps in the literature relating to the use of such devices are identified. There is an absence of studies that focus on the effect of compliance devices on adherence among older patients and the nature and frequency of drug administration errors involving these devices. The survey findings show that nurse-loaded compliance devices are used in over one-third of the sample. Further research is necessary to assess the clinical effectiveness of, and clinical risk attached to, compliance devices for older patients in the community. It is suggested that this is an issue of serious concern for primary care groups considering the principles of clinical governance.

Abstract:Objective To study the natural changes course of blood pressure in a cohort of middle-aged Chinese population.Methods The study comprised two cross-sectional surveys conducted in 1998 and 2004.A total of 6 129 participants aged 35-59 were enrolled.After excluding 864 persons in treatment,5265 people were classified as normal BP(n=2621),normal-high BP(n=1 904),mild hypertension(n=587) and were moderate heavy hypertension(n=153) according to Chinese guideline for prevention and treatment of hypertension(revised edition 2005).The participarts were followed up for 6 years.Results After 6 years without antihypertensive treatment,the majority(85%,3834/4525) of normotensives remained as normal or high normal;78%(457/587) mild hypertensives persisted,only 25% moderate or heavy hypertension regressed to non-hypertension.Multi-variable logistic regression showed that high-normal had 4 times more likely to progress to hypertension compared with normal blood pressure(OR=4.3,95% CI:3.6-5.1),while the mild hypertension patients who had a 2-fold greater chance regress to normotensive compared with moderate or heavy hypertension(OR=0.5 95% CI: 0.3-0.7).Weight change significantly influenced BP progression and regression.Heavy alcohol drinking affected progression significantly.Conclusion Data from this study support the strategy of intensively treatment moderate or severe hypertension and aggressively treatment mild hypertension.Persons with high-normal BP should be monitored and lifestyle modification should be implicated.

Abstract:The treatment of hypertension has improved in the last century; attention has been directed to restoring several altered pathophysiological mechanisms. However, regardless of the current treatments, it is difficult to control blood pressure. Uncontrolled hypertension is responsible for several cardiovascular complications, such as chronic renal failure, which is frequently observed in hypertensive patients. Therefore, new approaches that may improve the control of arterial blood pressure should be considered to prevent serious cardiovascular disorders. The contribution of purinergic receptors has been acknowledged in the pathophysiology of hypertension; this review describes the participation of these receptors in the alteration of kidney function in hypertension. Elevated interstitial ATP concentrations are essential for the activation of renal purinergic receptors; this becomes a fundamental pathway that leads to the development and maintenance of hypertension. High ATP levels modify essential mechanisms implicated in the long-term control of blood pressure, such as pressure natriuresis, the autoregulation of the glomerular filtration rate and renal blood flow, and tubuloglomerular feedback responses. Any alteration in these mechanisms decreases sodium excretion. ATP stimulates the release of vasoactive substances, causes renal function to decline, and induces tubulointerstitial damage. At the same time, a deleterious interaction involving angiotensin II and purinergic receptors leads to the deterioration of renal function.

Abstract:Over the past five decades, we have witnessed significant developments in the management of patients with arterial hypertension and elucidation of basic mechanisms involved in the disease. Many of these progresses resulted from experimental and clinical studies conducted in Italy. Several randomized clinical trials have been carried out worldwide and in Italy according to the best available evidence-based medicine rules, often before the initiation of comparable studies in different cardiovascular areas (acute coronary syndromes, arrhythmias, heart failure). Because of these progresses, we currently dispose of a huge therapeutic armamentarium of effective and generally well tolerated antihypertensive drugs. Ablation of renal nerves is a procedure which is re-gaining attention. We are also learning how to correctly measure blood pressure not only in the hospital setting, but also during normal daily activities using 24 h ambulatory blood pressure monitoring and self-measured home blood pressure. Out-of-office blood pressure proved to be superior to office blood pressure in its relationship with hypertensive organ damage and risk of major cardiovascular complications and mortality. Currently, we should improve our understanding of out-of-office blood pressure measurements and the clinical use of several available drug combinations according to their efficacy and tolerability in the single patients. We should also learn more about optimal blood pressure targets to be achieved in the general hypertensive population is specific subgroups at different cardiovascular risk.

Abstract:Hypertension affects approximately one third of the world's adult population and is a major cause of premature death despite considerable advances in pharmacological treatments. Growing evidence supports the use of lifestyle interventions for the prevention and adjuvant treatment of hypertension. In this Review, we provide a summary of the epidemiological research supporting the preventive and antihypertensive effects of major lifestyle interventions (regular physical exercise, body weight management and healthy dietary patterns), as well as other less traditional recommendations such as stress management and the promotion of adequate sleep patterns coupled with circadian entrainment. We also discuss the physiological mechanisms underlying the beneficial effects of these lifestyle interventions on hypertension, which include not only the prevention of traditional risk factors (such as obesity and insulin resistance) and improvements in vascular health through an improved redox and inflammatory status, but also reduced sympathetic overactivation and non-traditional mechanisms such as increased secretion of myokines.

Differing contributions of the gut microbiota to the blood pressure lowering effects induced by first‐line antihypertensive drugs

Abstract:Background and Purpose This study analyses whether first‐line antihypertensive drugs ameliorate the dysbiosis state in hypertension, and to test if this modification contributes to their blood pressure (BP) lowering properties in a genetic model of neurogenic hypertension. Experimental Approach Twenty‐week‐old male Wistar Kyoto rats (WKY) and spontaneously hypertensive rats (SHR) were untreated or treated with captopril, amlodipine or hydrochlorothiazide. A faecal microbiota transplantation (FMT) experiment was also performed by gavage of faecal content from donor SHR‐treated groups to SHR recipients for 3 weeks. Key results Faeces from SHR showed gut dysbiosis, characterized by lower acetate‐ and higher lactate‐producing bacteria and lower strict anaerobic bacteria. All three drugs increased the anaerobic bacteria proportion, captopril and amlodipine restored the proportion of acetate‐producing bacterial populations to WKY levels, whereas hydrochlorothiazide decreased butyrate‐producing bacteria. Captopril and amlodipine decreased gut pathology and permeability and attenuated sympathetic drive in the gut. Both drugs decreased neuroinflammation and oxidative stress in the hypothalamic paraventricular nuclei. Hydrochlorothiazide was unable to reduce neuroinflammation, gut sympathetic tone and gut integrity. FMT from SHR‐amlodipine to SHR decreased BP, ameliorated aortic endothelium‐dependent relaxation to acetylcholine, lowered NADPH oxidase activity, aortic Th17 infiltration and reduced neuroinflammation, whereas FMT from SHR‐hydrochlorothiazide did not have these effects. Conclusions and Implications First‐line antihypertensive drugs induced different modifications of gut integrity and gut dysbiosis in SHR, which result in no contribution of microbiota in the BP lowering effects of hydrochlorothiazide, whereas the vasculo‐protective effect induced by amlodipine involves gut microbiota reshaping and gut‐immune system communication.

Natural Variances in Blood Pressure Category among Chinese Adults

Abstract:Little is known about the natural progression and regression of blood pressure status, even though such knowledge would help determine the best intervention strategies. Our study aimed to explore natural changes in blood pressure status in a middle-aged Chinese population. A total of 6,129 Chinese men and women, aged 35 to 59 years at baseline, from the China Multi-center Collaborative Study of Cardiovascular Epidemiology, were reexamined 6 years later to determine the probability of progression (from non-hypertension to hypertension) and regression (from hypertension to non-hypertension). The majority (80%) of non-hypertensives among the respondents in this study remained normal or pre-hypertensive; about two-thirds of stage 1 hypertensives either stayed at the same stage or regressed to non-hypertension. However, only 9% of stage 2 hypertensives regressed to non-hypertension. Multi-variable logistic regression analysis showed that the stage 1 hypertension group had a 5-fold chance of regressing to non-hypertension in comparison with the stage 2 hypertension group (odds ratio [OR]=0.2, 95% confidence interval [CI]: 0.1−0.3), whereas the pre-hypertension group had a 4-fold likelihood of progressing to hypertension compared with normotensive subjects (OR=4.4, 95% CI: 3.7−5.3). After excluding participants ever on drug treatment in either examinations, the OR of regression for stage 2 hypertension was over twice that for stage 1 hypertension (OR=0.5, 95% CI: 0.3−0.7), and the possibility of progression decreased, though very slightly (OR=4.3, 95% CI: 3.6−5.1). Weight change significantly influenced progression and regression. Alcohol drinking affected progression significantly. In conclusion, the present findings support the strategy of intensively treating stage 2 hypertension and moderately treating stage 1 hypertension. Persons with pre-hypertension should be monitored for progression and advice on lifestyle modifications should be used.

Abstract:Purpose: High blood pressure (HBP) is the leading cause of mortality and years of disability, and its prevalence is increasing. Therefore, diagnosis and effective treatment of HBP is one of the main goals to prevent and reduce its complications, and pharmacological treatment is the cornerstone of hypertension management. Materials and Methods: The gradual introduction of different drug families has led to the development of new molecules that have improved efficacy and reduced adverse effects. Results: Current drugs include a large number that target key mechanisms of blood pressure regulation as well as those that contribute to hypertension-induced organ damage. Recently, new antihypertensive drugs have been introduced that not only aim to lower blood pressure but also provide additional protection against organ damage and metabolic disorders. Some of them were introduced for specific indications other than hypertension and other are based in a pharmacogenomic approach. Other routes of administration, such subcutaneous injection, are also being explored to improve protection and compliance. Conclusions: The main characteristics of each class of antihypertensive drug are summarised.

Abstract:Systemic arterial hypertension (SAH) can be conceptualized as a chronic degenerative disease of multifactorial nature, in most cases asymptomatic, that fundamentally compromises the balance of the vasodilator and vasoconstrictor systems that maintain the vasomotor tone, leading to a reduction in the lumen of the vessels and damage to the organs they irrigate. In practice, SAH is characterized by an increase in blood pressure levels above what is recommended for a given age group. The objective was to analyze four non-drug measures in the treatment of systemic arterial hypertension which are: dietary control, alcohol control, tobacco control, and physical exercise. This study was based on a bibliographic review in databases and considered systematic review, case report, clinical trial, and included publications in Portuguese and English. SAH is a highly prevalent disease with a high negative social impact. Early identification of hypertensive patients and effective treatment are of great clinical importance. Identification of possibly reversible causes is a fundamental part in the clinical management of these patients. It was concluded that nutritional therapy and lifestyle changes associated with pharmacological therapy can promote benefits in the reduction of systemic blood pressure. Thus, it is of utmost importance to stimulate and make the population aware of the need for adherence to pharmacological antihypertensive treatment along with non-drug therapy, thus contributing to the reduction of morbidity, mortality and health care costs.
Prevention of disease progression, left ventricular hypertrophy and congestive heart failure in hypertension treatment trials

Abstract:OBJECTIVES: This work was done to determine the role of hypertension treatment in the prevention of disease progression, left ventricular hypertrophy and congestive heart failure.BACKGROUND: Lowering of blood pressure in hypertensive patients has been reported to reduce morbidity and mortality from strokes and myocardial infarction. Data on primary prevention of disease progression, left ventricular hypertrophy and congestive heart failure have not previously been carefully quantified.METHODS: All the major long-term hypertension treatment trials over the past 20 years were reviewed.RESULTS: One thousand four hundred ninety-three of 13,342 subjects in the control groups compared with only 95 of 13,389 in the treated groups progressed from less severe to severe hypertension. The incidence of left ventricular hypertrophy in treated compared with control or placebo subjects was 140 of 6,150 and 216 of 6,098 subjects, respectively; congestive heart failure occurred in 240 of 6,923 subjects in the control group compared with only 112 of 6,914 treated subjects.CONCLUSIONS: The lowering of blood pressure over a 3- to 5-year period of time is effective in preventing severe disease, left ventricular hypertrophy and congestive heart failure in addition to strokes and myocardial infarction. In an era when expensive and often complicated methods are being used to prevent recurrence of congestive heart failure or myocardial infarction, it is important to highlight the role of antihypertensive therapy in primary prevention.

Abstract:Objective: Aortic stiffness is considered a valuable index of subclinical damage in hypertensives offering to cardiovascular risk estimation. The integrity of endothelial glycocalyx plays a vital role in vascular permeability, inflammation and elasticity. We aimed to explore any changes in both aortic stiffness and endothelial glycocalyx in hypertensive patients under medical treatment (MT) or just life style changes (LSC). Design and method: We studied 114 patients with newly diagnosed essential hypertension (mean age 48+10 years, 80 males). At baseline, we performed carotid-femoral artery PWV to evaluate aortic stiffness. Increased perfusion boundary region (PBR) of the sublingual arterial microvessels (ranged from 5–25 micrometers) was measured as a non-invasive accurate index of reduced endothelial glycocalyx thickness. Twenty six patients (23%) decided to follow only LSC (Group A) while 88 patients (77%) started MT (Group B). All patients re-evaluated at 12+7 months but no earlier than 6 months. Blood pressure (BP) <140/90 mmHg was considered as controlled (office and/or home measurements). Results: There were no significant differences within Groups A and B regarding baseline evaluation of systolic blood pressure (SBP), PWV and PBR 5–25. In Group A, systolic blood pressure (SBP), PWV and PBR were not different from baseline evaluation. However, in the subgroup of LSC with controlled hypertension (Group AC, 9/26), we found that SBP and PWV were reduced (p < 0.05). In Group B, SBP (p < 0.001) and PWV (p = 0.002) were reduced. In the subgroup of MT with uncontrolled hypertension by the physician but controlled by the patient and treated for at least 12 months (Group BUC, 10/88 patients) we found that SBP, PWV and PBR5–25 were decreased (p < 0.05). Conclusions: Cardiovascular risk reduction in hypertensives is based not only in SBP control but also in target organ damage improvement. Our results underline that the expected improvement of aortic stiffness induced by BP control is present independently of the choice of the treatment. Secondly, improvement of endothelial glycocalyx needs long term treatment. Finally, our study shows the importance of the information received by the patients regarding their BP control at home regarding the white coat effect phenomenon in office measurements.

Analysis on the Trend of Long-Term Change of Blood Pressure in Hypertensive Patients Treated with Benazepril

Abstract:OBJECTIVE:To introduce the application of mixed linear model in the analysis of secular trend of blood pressure under antihypertensive treatment.METHODS:A community-based postmarketing surveillance of benazepril was conducted in 1831 essential hypertensive patients (age range from 35 to 88 years) in Shanghai. Data of blood pressure was analyzed every 3 months with mixed linear model to describe the secular trend of blood pressure and changes of age-specific and gender-specific.RESULTS:The changing trends of systolic blood pressure (SBP) and diastolic blood pressure (DBP) were found to fit the curvilinear models. A piecewise model was fit for pulse pressure (PP), i.e., curvilinear model in the first 9 months and linear model after 9 months of taking medication. Both blood pressure and its velocity gradually slowed down. There were significant variation for the curve parameters of intercept, slope, and acceleration. Blood pressure in patients with higher initial levels was persistently declining in the 3-year-treatment. However blood pressures of patients with relatively low initial levels remained low when dropped down to some degree. Elderly patients showed high SBP but low DBP, so as with higher PP. The velocity and sizes of blood pressure reductions increased with the initial level of blood pressure.CONCLUSION:Mixed linear model is flexible and robust when applied to the analysis of longitudinal data but with missing values and can also make the maximum use of available information.

Abstract:Study design: An open- label, multicentre study was conducted to evaluate the antihypertensive efficacy of a 4-week course of losartan 50 mg plus hydrochlorothiazide 12.5 mg in Asian patients with essential hypertension whose blood pressure had previously been treated with but not controlled by valsartan 80 mg.Methods: A total of 237 eligible patients with mean trough sitting diastolic blood pressure SiDBP) 95 - 115 mmHg and a mean trough sitting systolic blood pressure ( SiSBP) < 190 mmHg entered the baseline period of treatment with valsartan 80 mg/ day for 4 weeks. Those ( n = 165) whose SiDBP remained > 90 mmHg and who were not excluded for other reasons were then switched to a single-tablet formulation of losartan 50 mg/ hydrochlorothiazide 12.5 mg combination once daily for a further 4 weeks.Results: Mean SiDBP ( study primary endpoint) at the end of combination therapy was reduced to 86.9 mmHg from 95.2 mmHg. SiSBP ( study secondary endpoint) was reduced to 132.6 mmHg from 140.7 mmHg. Mean reductions after switching from valsartan 80 mg to losartan 50 mg/hydrochlorothiazide 12.5 mg were thus 8.3 and 8.1 mmHg for SiDBP and SiSBP, respectively ( p <= 0.001 for both outcomes). The goal of SiDBP <= 90 mmHg was attained in 72% of the patients previously not controlled to the same level by valsartan 80 mg/day. Combination therapy with losartan 50 mg/hydrochlorothiazide 12.5 mg was generally well tolerated. Mean compliance with the losartan 50 mg/hydrochlorothiazide 12.5 mg combination was > 99%.Conclusion: These results demonstrate that in Asian patients who do not reach the goal of mean trough SiDBP <= 90 mmHg with valsartan monotherapy at 80 mg once-daily, switching to a single-tablet combination of losartan 50 mg/hydrochlorothiazide 12.5 mg once-daily is well tolerated, provides effective control of blood pressure and is an excellent choice to achieve blood pressure reduction goals.

The weak definition of what is under the umbrella of HTN is a consequence of the lack of knowledge on the players involved in environment-gene interaction and their impact on blood pressure (BP) and mechanisms. The disclosure of these mechanisms that sense and (mal)adapt to toxic-environmental stimuli might at least determine some phenotypes of essential HTN and will have important therapeutic implications. In the present manuscript, we looked closer to the environmental sensor aryl hydrocarbon receptor (AHR), a ligand-activated transcription factor involved in cardiovascular physiology, but better known by its involvement in biotransformation of xenobiotics through its canonical pathway. This review aims to disclose the contribution of the AHR-canonical pathway to HTN. For better mirror the complexity of the mechanisms involved in BP regulation, we privileged evidence from in vivo studies. Here we ascertained the level of available evidence and a comprehensive characterization of the AHR-related phenotype of HTN. We reviewed clinical and rodent studies on AHR-HTN genetic association and on AHR ligands and their impact on BP. We concluded that AHR is a druggable mechanistic linker of environmental exposure to HTN. We conclude that is worth to investigate the canonical pathway of AHR and the expression/polymorphisms of its related genes and/or other biomarkers (e.g. tryptophan-related ligands), in order to identify patients that may benefit from an AHR-centered antihypertensive treatment.

The role of eprosartan in the management of essential hypertension: literature review and expert opinion

Abstract:Introduction: Eprosartan is an angiotensin receptor blocker (ARB) used for management of essential hypertension. With unique pharmacological characteristics, dual action mechanism, and clinical effectiveness, eprosartan offers additional advantages over other ARBs in specific patient populations. Areas covered: A comprehensive review of the literature was performed across publicly available databases, with no time limitations, to ensure the inclusion of all relevant studies. The review focuses on presenting the efficacy and safety profile of eprosartan, alone or in combination with other agents. Additionally, it explores the etiology of hypertension concerning the structure and function of angiotensin II type 1 receptors. Further, the efficacy of eprosartan in special populations and its additional benefits are also discussed. Expert opinion: Eprosartan effectively reduces blood pressure (BP), with a 24-hour BP-lowering effect at 600 mg/day. Eprosartan demonstrates similar or better efficacy than other ARBs, such as telmisartan and losartan, particularly in managing coagulation-related abnormalities and peripheral resistance. In combination therapy, eprosartan with hydrochlorothiazide significantly enhances BP reduction. Eprosartan is well-tolerated, with a low incidence of adverse events, making it a reliable choice for long-term hypertension management across various patient populations, such as those with comorbid diabetes and renal disease and older adults.

Abstract:Objective: To study the change of L Arg/NO system in erythrocytes of essential hypertension patients. Methods: The 3H L Arg transport , NO synthase (NOS) production, NOS activity and the contents of cGMP were measured. Results: The Vmax for total transport of L Arg via system Y + deceased in erythrocytes of hypertension patients compared with control subjects, while the Vmax for the transport of L Arg via system Y +L was unaffected in erythrocytes of hypertension patients. Km values for total L Arg influx via Y + system were not significantly different in erythrocytes in control or hypertension groups. The NOS activity produced and cGMP decreased significantly compared with those of the control and hypertension groups. Conclusion: The ability of erythrocytes utilizing L Arg to produce NO is reduced when blood pressure rises.
