Abstract:Background The hypertension care cascade has been characterized worldwide, yet it has not been quantified how far above the blood pressure control threshold people with uncontrolled treated hypertension are. We summarized the mean systolic blood pressure (SBP; mmHg) in people treated for hypertension but SBP not <130/80.Methods We did a cross-sectional analysis of 55 WHO STEPS Surveys (n = 10,658), comprising six world regions (Africa, Americas, Eastern Mediterranean, Europe, Southeast Asia and Western Pacific); we only included the most recent survey by country regardless of when it was conducted. Adults, men and women, aged between 25 and 69 years, with self-reported hypertension receiving antihypertensive medication and whose blood pressure was >130/80 mmHg were included. We quantified the mean SBP overall and by socio-demographic (sex, age, urban/rural location, education) and cardiometabolic (current smoking, self-reported diabetes) risk factors.Findings The lowest SBP was observed in Kuwait (146.6; 95% CI: 143.8-149.4 mmHg) and the highest in Libya (171.9; 95% CI: 167.8-176.0 mmHg). In 29 countries, the SBP was higher in men, and SBP tended to be higher in older groups except in six countries. In 17 countries, the SBP was higher in rural than in urban sites, for example in Turkmenistan the SBP was 162.3 (95% CI: 158.4-166.2) mmHg in rural versus 151.6 (95% CI: 148.7-154.4) mmHg in urban areas. In 25 countries, the SBP was higher in adults with no education, for example in Benin the SBP in people without formal education was 175.3 (95% CI: 168.8-181.9) mmHg versus 156.4 (95% CI: 148.8-164.0) mmHg in people with higher education. Interpretation Stronger interventions to improve and secure access to effective management are needed in most countries and specific groups, to reach hypertension control in people with hypertension already receiving antihy-pertensive medication.

Abstract:Background and Objective: Hypertension (HTN) is the leading cardiovascular risk factor, yet its control remains low. Identifying factors influencing diagnosis, potential clinical inertia, and the measuring devices used is crucial. Objective: This study aims to understand factors in medical practice affecting HTN knowledge and control, and to assess the availability and use of blood pressure (BP) measurement devices. Methods: This observational, cross-sectional study is based on a 25-question self-administered survey by the Argentine Society of Hypertension (SAHA) and the Argentine Society of Medicine (SAM), targeting doctors treating HTN patients in Argentina. This report focuses on BP measurement. Results: A total of 1,042 responses were received, with women comprising 45.8% (n:471). Responses came from 430 cardiologists (41.7%), 380 internists (36.8%), 68 HTN specialists (6.6%), 54 primary care physicians (5.2%), and 100 from other specialties (9.7%). An own validated automatic arm BP monitor was used by 44.5%, a manual aneroid by 36.5%, an institution's validated automatic arm monitor by 10.9%, and a fixed wall aneroid by 8.2%. BP was measured in all patients by 89% of respondents, sometimes by 8.5%, and only if the patient was hypertensive by 1.7%. Ambulatory blood pressure monitoring (ABPM) was always requested before treatment by 25.2%, sporadically by 34%, never by 4.1%, and only for BP between 130/85 mmHg and 159/99 mmHg by 36.7%. The 24-hour ABPM values for initiating pharmacological treatment were >140/90 mmHg (40.9%), >135/85 mmHg (39%), and >130/80 mmHg (20.1%). Conclusions: Discrepancies exist in HTN patient care. Only 20% of doctors start pharmacological treatment at 24-hour ABPM values above 130/80 mmHg. Enhancing education and information could improve HTN control strategies.

Blood pressure and 10-year all-cause mortality: Findings from the PERU MIGRANT Study

Abstract:Background: The long-term impact of elevated blood pressure on mortality outcomes has been recently revisited due to proposed changes in cut-offs for hypertension. This study aimed at assessing the association between high blood pressure levels and 10-year mortality using the Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure (JNC-7) and the American College of Cardiology and the American Heart Association (ACC/AHA) 2017 blood pressure guidelines. Methods: Data analysis of the PERU MIGRANT Study, a prospective ongoing cohort, was used. The outcome of interest was 10-year all-cause mortality, and exposures were blood pressure categories according to the JNC-7 and ACC/AHA 2017 guidelines. Log-rank test, Kaplan-Meier and Cox regression models were used to assess the associations of interest controlling for confounders. Hazard ratios (HR) and 95% confidence intervals (95% CI) were estimated. Results: A total of 976 records, mean age of 60.4 (SD: 11.4), 513 (52.6%) women, were analyzed. Hypertension prevalence at baseline almost doubled from 16.0% (95% CI 13.7%-18.4%) to 31.3% (95% CI 28.4%-34.3%), using the JNC-7 and ACC/AHA 2017 definitions, respectively. Sixty-three (6.4%) participants died during the 10-year follow-up, equating to a mortality rate of 3.6 (95% CI 2.4-4.7) per 1000 person-years. Using JNC-7, and compared to those with normal blood pressure, those with pre-hypertension and hypertension had 2.1-fold and 5.1-fold increased risk of death, respectively. Similar mortality effect sizes were estimated using ACC/AHA 2017 for stage-1 and stage-2 hypertension. Conclusions: Blood pressure levels under two different definitions increased the risk of 10-year all-cause mortality. Hypertension prevalence doubled using ACC/AHA 2017 compared to JNC-7. The choice of blood pressure cut-offs to classify hypertension categories need to be balanced against the patients benefit and the capacities of the health system to adequately handle a large proportion of new patients.

Abstract:PURPOSE: To determine the association of changes in blood pressure (BP) components between baseline examination (1999-2001) and a second visit (2002-2005) with incident cardiovascular diseases (CVD).METHODS: In 3569 (2048 female) Iranian subjects ≥30 y, systolic BP, diastolic BP, mean arterial pressure (MAP) and pulse pressure (PP) were evaluated in two consecutive visits. Subjects were then followed for cardiovascular events. Multivariate sex-adjusted Cox Proportional-Hazards models were built for each BP component's change, and further adjusted for baseline BP values, traditional cardiovascular risk factors, and their changes.RESULTS: During a median follow-up of 6.09 years after the second examination, 303 CVD cases occurred. A 1 SD increase in systolic BP, diastolic BP and MAP were significantly associated with 21%, 22%, and 95% increased CVD risk after adjustments for baseline values of BP components and other common risk factors (all p-values <0.05). Importantly, diastolic BP change resisted after further adjustment with systolic BP change (hazard ratio 1.21, CI 95% 1.05-1.39). PP change showed no significant association with CVD.CONCLUSIONS: In a middle-aged population, three-year rises in systolic BP, diastolic BP, MAP, but not PP were associated with increased incident CVD. The significant association between diastolic BP change and CVD was shown independent of systolic BP change.

Abstract:Background: Hypertension is a major challenge today. Although young individuals present lower risk, it's important to identify factors predisposing to elevated blood pressure from early years. Objectives Identify associated factors with elevated systolic blood pressure (SBP) in medical students, Pereira-2022. Methodology A cross-sectional study was conducted on medical students at a public university in Pereira, in which sociodemographic, anthropometric, physiological, and electrocardiographic variables were collected. A bivariate analysis was made comparing individuals with an SBP>130mmHg to those below. A multivariate analysis was performed using logistic regression. Results: 201 medical students were interviewed (mean age=21.4 years), 56.7% women. 17.4% presented high-normal blood pressure or grade I hypertension. 24 students were found with an SBP>130mmHg (11.9%). The factors significantly related with this variable were being a woman (OR= 0.11 95% CI= 0.03-0.39), family history of deceased by chronic kidney disease (OR= 3.83 95% CI= 1.05-12.86), a parent with hypertension (OR=3.91 95% CI=1.41-10.80), diastolic blood pressure >85mmHg (OR=6.60 95% CI= 1.96- 22.27) and wrong progression of the R-wave on the anteroseptal wall (OR=11.03 95% CI= 2.32-52.22). Conclusions: even though the population of youth has a lower prevalence of arterial hypertension, in this study, it was found that 11.9% of the participants present an SBP>130mmHg. In this demographic group, there must be considered certain factors, such as a family history of chronic kidney disease and hypertension, a personal finding of elevated diastolic blood pressure, and electrographic changes, like a wrong progression of the R-wave on the anteroseptal wall.

Abstract:Objective To study the natural changes course of blood pressure in a cohort of middle-aged Chinese population.Methods The study comprised two cross-sectional surveys conducted in 1998 and 2004.A total of 6 129 participants aged 35-59 were enrolled.After excluding 864 persons in treatment,5265 people were classified as normal BP(n=2621),normal-high BP(n=1 904),mild hypertension(n=587) and were moderate heavy hypertension(n=153) according to Chinese guideline for prevention and treatment of hypertension(revised edition 2005).The participarts were followed up for 6 years.Results After 6 years without antihypertensive treatment,the majority(85%,3834/4525) of normotensives remained as normal or high normal;78%(457/587) mild hypertensives persisted,only 25% moderate or heavy hypertension regressed to non-hypertension.Multi-variable logistic regression showed that high-normal had 4 times more likely to progress to hypertension compared with normal blood pressure(OR=4.3,95% CI:3.6-5.1),while the mild hypertension patients who had a 2-fold greater chance regress to normotensive compared with moderate or heavy hypertension(OR=0.5 95% CI: 0.3-0.7).Weight change significantly influenced BP progression and regression.Heavy alcohol drinking affected progression significantly.Conclusion Data from this study support the strategy of intensively treatment moderate or severe hypertension and aggressively treatment mild hypertension.Persons with high-normal BP should be monitored and lifestyle modification should be implicated.
Prevalence, Trends, and Associated Factors of Isolated Systolic, Diastolic, and Systolic-Diastolic Hypertension in Peru: A Nine-Year Analysis of the Demographic and Family Health Survey

Abstract:Introduction: While HTN is widely seen as a primary threat to cardiovascular conditions worldwide, it is essential to recognize that not all HTN is identical. Objective: To determine the prevalence, trend, and factors associated with each type of HTN: isolated systolic (ISH), isolated diastolic (IDH), and systolic-diastolic (SDH). Methods: A secondary analysis of data from the Demographic and Family Health Survey from 2014 to 2022 was conducted. For the analysis of associated factors, a Poisson regression model with robust variance was implemented to calculate adjusted Prevalence Ratios (aPR) along with their 95% confidence intervals. Results: The prevalence was 7.02%, 1.55%, and 3.28% for ISH, IDH, and SDH, respectively. ISH showed a decline in 2022, unlike the other two types, which seem to be on the rise. A statistically significant association was found in men and an increased risk with age for ISH and SDH, unlike IDH, where age acts as a protective factor. Additional factors identified include smoking and excessive alcohol consumption, while a high intake of fruits/vegetables offers a protective effect. Obesity and diabetes were associated with a higher risk, and significant variations by region and altitude, as well as among ethnic groups, were observed. Conclusions: Significant differences in the prevalence of HTN subtypes have been found, underscoring the heterogeneity of this chronic condition, both in related factors and in trends over the years.

Abstract:Background Data for trends in blood pressure are needed to understand the effects of its dietary, lifestyle, and pharmacological determinants; set intervention priorities; and evaluate national programmes. However, few worldwide analyses of trends in blood pressure have been done. We estimated worldwide trends in population mean systolic blood pressure (SBP).Methods We estimated trends and their uncertainties in mean SBP for adults 25 years and older in 199 countries and territories. We obtained data from published and unpublished health examination surveys and epidemiological studies (786 country-years and 5.4 million participants). For each sex, we used a Bayesian hierarchical model to estimate mean SBP by age, country, and year, accounting for whether a study was nationally representative.Findings In 2008, age-standardised mean SBP worldwide was 128.1 mm Hg (95% uncertainty interval 126.7-129.4) in men and 124.4 mm Hg (123.0-125.9) in women. Globally, between 1980 and 2008, S BP decreased by 0.8 mm Hg per decade (-0.4 to 2.2, posterior probability of being a true decline=0.90) in men and 1.0 mm Hg per decade (-0.3 to 2.3, posterior probability=0.93) in women. Female SBP decreased by 3.5 mm Hg or more per decade in western Europe and Australasia (posterior probabilities >= 0.999). Male SBP fell most in high-income North America, by 2.8 mm Hg per decade (1.3-4.5, posterior probability >0.999), followed by Australasia and western Europe where it decreased by more than 2.0 mm Hg per decade (posterior probabilities >0.98). SBP rose in Oceania, east Africa, and south and southeast Asia for both sexes, and in west Africa for women, with the increases ranging 0.8-1.6 mm Hg per decade in men (posterior probabilities 0.72-0.91) and 1.0-2.7 mm Hg per decade for women (posterior probabilities 0.75-0.98). Female SBP was highest in some east and west African countries, with means of 135 mm Hg or greater. Male SBP was highest in Baltic and east and west African countries, where mean SBP reached 138 mm Hg or more. Men and women in western Europe had the highest SBP in high-income regions.Interpretation On average, global population SBP decreased slightly since 1980, but trends varied significantly across regions and countries. SBP is currently highest in low-income and middle-income countries. Effective population-based and personal interventions should be targeted towards low-income and middle-income countries.

Abstract:Background and Objectives: Hypertension is the main modifiable cardiovascular risk factor. However, 46% of adults with hypertension are unaware of their diagnosis. Ambulatory blood pressure monitoring (ABPM) can be very helpful in the identification of white coat and masked hypertension, offering advantages when compared to in-office measurement. Unfortunately, this method is not available at all levels of care. Particularly in Colombia, there is no information regarding the frequency of use or geographic distribution; however, it's inferred that it's more available in the most developed regions. Therefore, the purpose of this study is to evaluate the frequency of use, geographic distribution, and demographic characteristics of patients who underwent ABPM in Colombia. Methods: Cross sectional study, carried out through the review of the SISPRO records of Colombians aged 18 years or older, with indication of performing ABPM between 2019 and 2023. Results: 405,396 individuals in Colombia underwent ABPM (7.7 ABPMs per 1000 inhabitants per year). 63.1% were females. The departments with the highest number of ABPM studies were Bogotá (71,709 ABPMs – 9.9 ABPMs per 1000 inhabitants per year), followed by Antioquia (45,097 ABPMs - 7.5 ABPMs per 1000 inhabitants per year) (Figure 1). Concerning age groups, the highest number of ABPM requests was between 70 and 74 years (50,103 patients), followed by the 55-59 years group (46,944 patients). 72.25% of the requests were for patients older than 50 years. Conclusions: Although Colombia has improved in the implementation and use of ABPM at the national level, its use predominates in women, patients aged between 70 and 74 years, and at the level of the 2 largest cities in the country. Collective efforts are required at national level to improve the implementation and registration of the ABPM.

Abstract:Introduction: Hypertension is one of the main concerns in public health, since it is related with increased morbidity, and potential years of life lost in addition to loss of quality of life. This study aimed to assess: (1) the distribution of indicators of life course SEP in a cohort of Colombian patients with hypertension and (2) to assess the association of life course SEP and control of hypertension among this cohort of patients. Methods: Data were obtained using the baseline survey of 258 patients from the Social Determinants and Inequities in the Control of Blood Hypertension Program (ProDSICHA). Mother occupation and housing conditions were measured with the Event History Calendar. Mother educational level was measured with the questionnaire developed by the Project on Ethnicity and Race in Latin America (PERLA). Socioeconomic position during adulthood was measured using education, occupation, and income level based in the MacArthur Network. Results: The group with a higher lifelong social position and the group of lower lifelong social position showed better control of hypertension (OR = 1.21; p <0.05; OR = 1.33; p < .05, respectively) compared to those whose social position throughout life varied the most. No statistical differences were found in the relations between single lifetime social position variables, and hypertension control in the three time points analyzed. Discussion: These findings warrant further research to deeper our understanding on the role of a multidimensional and cumulative approach of social position in hypertension control.

Urinary sodium‐to‐potassium ratio and body mass index in relation to high blood pressure in a national health survey in Chile

Abstract:<p>Several lifestyle and sociodemographic factors are associated with blood pressure (BP). The authors conducted a retrospective study of 4870 subjects from the National Health Survey 2009 in Chile to identify exposure factors associated with increasing BP levels. Subjects with isolated urinary excretion of sodium (n = 2873), potassium, and creatinine were included to estimate daily salt intake and urinary sodium/potassium (Na/K) ratio. Hypertension was defined according to European guidelines 2018 and American guidelines ACC/AHA 2017. Proportional odds models were developed to analyze education level, sedentarism, smoking, alcohol intake, estimated urinary Na/K ratio, estimated daily salt intake, and body mass index (BMI) as factors associated with increasing BP levels (from high‐normal BP to hypertension). Logistic regression models were checked for overdispersion. Mean age and BMI of the population were 42 years old and 27 kg/m<sup>2</sup>, respectively; 19% had low education level and 27% had hypertension according to European guidelines, whereas 47% according to ACC/AHA criteria. Mean estimated urinary Na/K ratio was 4 ± 2, and mean salt consumption was 10 ± 2 g/day. Estimated urinary Na/K ratio (OR, 1.11; 95% CI, 1.01‐1.21), BMI (OR, 1.10; 95% CI, 1.07‐1.13), estimated daily salt intake (OR, 1.10; 95% CI, 1.03‐1.17), and alcohol intake (OR, 1.03; 95% CI, 1.01‐1.05) were significantly associated with hypertension. This study highlights that a healthy diet and weight control should be important components of BP management plans, and it suggests that public policies should include close monitoring of these factors to reduce hypertension prevalence and improve its management in a Latino population.</p>

Abstract:Introduction: Hypertension, diabetes mellitus, dyslipidemia, obesity and smoking are risk factors that impact the emergence of subclinical lesions and contribute to increased cardiovascular (CV) morbidity and mortality. Objective: to evaluate the cardiometabolic profile and blood pressure levels in young adults treated in primary care in the Brazilian Unified Health System. Methods: Multicenter cross-sectional population study (arm of the LapARC study – Rio de Janeiro) approved by the Ethics Committee under registration: 5,046,133. Our center is located in the municipality of Santarém, Pará, Brazilian Amazon. Young adults (20 to 50 years old) were evaluated in relation to their cardiovascular risk profile, sociodemographic, anthropometry and laboratory tests. Office blood pressure (BP) was measured and patients underwent Home Blood Pressure Monitoring (HBPM), with a seven-day protocol. Results: 161 individuals were included (81.4% female; mean age 39.6 ± 7.9 years), 90% declared themselves black and mixed race and 27.3% had low education. More than half of the population has 3 or more CV risk factors (55.9%), with dyslipidemia being the most prevalent (60.2%), followed by hypertension (55.9%) and women with significantly more abdominal obesity (67.2% vs 30.0%, p<0.001). Among hypertensives, 84.4% knew the diagnosis, of which 71 were treated, with only 31.1% reaching blood pressure control. The most frequently mentioned antihypertensives were renin-angiotensin-aldosterone system inhibitors (36.6%) and diuretics (18.6%). Men had higher office and home systolic BP levels than women (136 ± 22 vs 128 ± 23 and 127 ± 21 vs 116 ± 15, p<0.05), respectively. Furthermore, the mean ankle-brachial index was 1.12 [1.07-1.20]. After HBPM, 20.9% of individuals were identified with masked hypertension, only 1.2% with white coat hypertension and 26.7% with sustained hypertension. Conclusion: although apparently healthy, this population have an adverse cardiometabolic profile, marked by low blood pressure control and the prevalence of masked hypertension, which justifies the continuation of the study.

Abstract:Abstract Background Despite the high burden of anxiety and hypertension in Hispanic/Latino adults, little is known about their association in this population. Purpose To examine the associations of anxiety symptoms with 6-year changes in blood pressure (BP) and incident hypertension in Hispanic/Latino adults. Methods We examined data from a probability sample of 10,881 Hispanic/Latino persons aged 18–74 who attended visits 1 (V1; 2008–2011) and 2 (V2; 2014–2017) of the Hispanic Community Health Study/Study of Latinos (HCHS/SOL), a prospective cohort study. Anxiety symptoms were assessed at V1 using the 10-item Spielberger Trait Anxiety Scale (M = 17.1; Range = 10–40) and dichotomized using a cut-point of 20, the highest quartile in this cohort. BP was measured at both visits using a standardized protocol. Results Adults with elevated anxiety symptoms had a 1.02 mm Hg greater increase in systolic (p = .02) and a 0.75 mm Hg greater increase in diastolic BP (p = .02) over 6.1 years than those with lower symptoms, after adjusting for sociodemographic and clinical covariates. These associations differed by sex. Elevated anxiety was associated with a greater increase in systolic and diastolic BP in men only. Among persons without hypertension at V1 (N = 7,412), those with elevated anxiety symptoms at V1 had a 22% higher incidence of hypertension (p = .02) 6.1 years later. Conclusions Our findings underscore the importance of screening for and treating elevated anxiety symptoms to help prevent hypertension. Further research on the role of sex and underlying mechanisms is warranted.

Abstract:Objective To observe the changes in blood pressure (BP) over 10 years and to investigate current BP association to serum uric acid (SUA) levels and cardiovascular risk factors in the epidemiological data of a target group of patients with prehypertension in 2007. Design Cross-sectional study. Setting Mlati Subdistrict, Sleman District, Yogyakarta Province, Indonesia. Participants A total of 733 patients from ‘Mlati Study Database’ in 2007 were selected by simple random sampling using statistical software. Subjects had both physical and laboratory examinations. Outcome measures Morning home BP and laboratory examination of urine (uric acid excretion and creatinine) and blood samples (SUA, blood urea nitrogen, creatinine, a lipid profile and fasting blood glucose levels). Results About 31.1% of 733 subjects with prehypertension became hypertensive after 10 years, 24.6% returned to normal tension and the rest of it remained in prehypertensive state. Mean (SD) of SUA levels in 2017 was significantly higher in men than in women (5.78 (1.25) mg/dL vs 4.52 (1.10) mg/dL, p<0.001). Furthermore, men tended to have high-normal (5–7 mg/dL) or high SUA levels (≥7 mg/dL) compared with women (p<0.001, Relative Risk (RR)=2.60). High-normal and high SUA levels in population with a history of prehypertension were significantly associated with current prehypertension and hypertension only in women (p=0.001, RR=1.21). Age and body mass index was found to be significantly associated with both systolic and diastolic BP in men, but only with systolic BP in women. Fasting blood glucose and SUA levels were significantly associated with systolic and diastolic BP only in women. Conclusion We concluded that after 10 years, of 733 subjects with prehypertension, 31.1% became hypertensive. The SUA levels in men are significantly higher than those in women. Moreover, high-normal and high SUA levels were significantly associated with prehypertension and hypertension in women but not in men.

High and low affinity binding of folate to proteins in serum of pregnancy women.

Abstract:Binding of [3H]folate to proteins in serum of pregnant women was studied in equilibrium dialysis experiments (pH 7.4, 37 degrees C). A Scatchard analysis revealed the presence of high-affinity (Kass = 10(10) M(-1), N = 0.4 nM folate) and low-affinity sites. The high-affinity folate binding protein (Mr approximately 30 000--35 000) appeared in front effluent after application of serum to a DEAE-Sepharose CL-6B column equilibrated with 0.05 M imidazole buffer (pH 6.3)/30 mM NaCl. Low-affinity binding protein eluted from the column after a rise in NaCl concentration to 1 M was mainly similar to albumin. A minor part was, however, associated with a large molecular size (Mr > 200 000) protein, probably alpha 2-macroglobulin. High-affinity binding which displayed positive cooperativity was saturated at folate concentrations above 10(-10) M. Folate dissociation was a complex process consisting of an initial rapid phase (terminated within 48 h) followed by a slow release. At pH 3.5 dissociation became rapid and complete. Purified methotrexate had no effect on high-affinity binding, whereas N10-methylfolate (an impurity in the methotrexate preparation) acted as a potent inhibitor. Low-affinity binding was proportional to the folate concentration within the range 10(-10)--10(-7) M. Dissociation of folate was rapid.

Abstract:Introduction: Hypertension (HTN) has been classified into stages according to elevated blood pressure levels. It has been detailed that higher levels correlate with an increased risk of long-term complications. Objective: To determine the prevalence and trends of different hypertension stages among Peruvians and to identify factors associated with these stages. Methods: Data from the Demographic and Family Health Survey from 2014 to 2022 were analyzed. Blood pressure levels were categorized into normotension, high blood pressure (HBP), and HTN stages I, II, and III. The study included individuals aged 18 years and older, excluding those who had not had their blood pressure measured or had known histories of hypertension. Results: The prevalences of HBP, HTN stage I, II, and III were 14.73%, 9.64%, 2.16%, and 0.64%, respectively. Factors such as gender, age, wealth index, alcohol consumption, daily smoking, and health conditions like obesity and diabetes significantly influenced the prevalence of the various stages. Conclusions: The prevalence of HTN varies significantly depending on the stage. These findings underscore the necessity for public health strategies that are personalized and differentiated according to the specific stage of HTN to enhance effectiveness.

Abstract:Background Hypertension, diabetes and hypercholesterolemia are the most frequent and diagnosed chronic diseases in Argentina. They contribute largely to the burden of chronic disease and they are strongly influenced by a small number of risk factors. These risk factors are all modifiable at the population and individual level and offer major prospects for their prevention. We are interested in socioeconomic determinants of prevalence of those 3 specific diseases. Design and methods We estimate 3-equation probit model, combined with 3 separate probit estimations and a probit-based Heckman correction considering possible sample selection bias. Estimations were carried out using secondary self-reported data coming from the 2013 Risk Factor National Survey. Results We find a negative association between socioeconomic status and prevalence of hypertension, cholesterolemia and diabetes; main increases concentrate in the transition from low to high SES in hypertension and diabetes. In cholesterol, the major effect takes place when individual crosses from low to middle SES and then vanishes. Anyway, in Argentina SES exhibit and independent effect on chronic diseases apart from those based on habits and body weight. Conclusions Public strategies to prevent chronic diseases must be specially targeted at women, poorest households and the least educated individuals in order to achieve efficacy. Also, as the probability of having a condition related to excessive blood pressure, high levels of cholesterol or glucose in the blood do not increase proportionally with age, so public campaigns promoting healthy diets, physical activity and medical checkups should be focused on young individuals to facilitate prophylaxis.

Abstract:Objectives To assess the risk of progression to hypertension across baseline blood pressure (BP) level among rural Chinese adults.Methods A population-based sample of 24 052 rural Chinese adults aged at least 35 years and free from hypertension at baseline were followed up from 2004-2006 to 2008. Incident hypertension was defined as SBP of at least 140 mmHg, DBP of at least 90 mmHg, current use of antihypertensive medication, or all.Results During a median follow-up of 28 months, 26.5% participants developed clinical hypertension, most of which were untreated and uncontrolled. The cumulative incidence of hypertension in participants with optimal, normal, and high-normal BP was 21.2, 25.2, and 32.4%, respectively. The higher incidence of hypertension (per 100 person-years) was obviously observed in higher BP category at baseline among both older and younger groups (both P for trend <0.05). Compared with optimal BP, the adjusted hazard ratios of participants with normal and high-normal BP for incident hypertension were 1.16 (95% confidence interval 1.08-1.24) and 1.28 (95% confidence interval 1.20-1.36), respectively. In addition, older age, men, Mongolian ethnicity, alcohol drinking, family history of hypertension, BMI, and change in BMI during follow-up were also independently associated with incident hypertension.Conclusion There was a stepwise increase in risk of progression to hypertension across baseline BP level. This information could help us define a population at high risk of progression to hypertension, to underscore the importance of frequent BP monitoring, and early interventions in rural Chinese adults. Abstract:Background: The progression of Central Blood Pressure (CBP) values and central hemodynamic parameters and its relationship with cardiovascular risk factors is quite unknown. We sought to investigate this association in a Spanish adult population without cardiovascular diseases. Methods: Prospective observational research with a five-year follow-up. Randomly sampled 501 individuals (mean age 56±14 years, 50.3% women). After five years, 480 individuals had a follow-up. Measurements taken using the SphygmoCor® (AtCor Medical Pty Ltd., Head Office,West Ryde, Australia), following all the recommendations established in the "International task force"1, giving an estimate of central blood pressure relative to measured brachial blood pressure (type 1 device). Results: Progressions during follow-up: central systolic blood pressure (cSBP): 4.16±13.71 mmHg; central diastolic blood pressure (cDBP): 2.45±11.37 mmHg; central pulse pressure (cPP): 1.72±12.43 mmHg; pulse pressure amplification (PPA): 2.85±12.20 mmHg; ejection duration (ED): 7.00±47.87 ms; subendocardial viability ratio (SEVR): -8.04±36.24%. In multiple regression analysis: cSBP positively associated with: BMI (β=0.476); waist size (β=0.159); number of cigarettes per day (β=0.192). Inversely associated with peripheral systolic blood pressure (β=-0.282). cDBP increase positively associated with number of cigarettes per day (β=0.174). Inversely associated with peripheral diastolic blood pressure (β=-0.292). cPP increase positively associated with BMI (β=0.330). Inversely associated with peripheral pulse pressure (β=-0.262). PPA increase positively associated with: BMI (β=0.276); number of cigarettes per day (β=0.281). ED progress inversely associated with basal plasma glucose (β=-0.286). Conclusions: All measures increased except for SEVR. Progressions in CBP and PPA were positively associated with anthropometric parameters and number of cigarettes and CBP inversely associated with peripheral blood pressure, although this association was different according to sex.
