Abstract:12043 Background: Impaired nutrition is associated with greater treatment toxicity and reduced overall survival in patients with advanced cancer. We aimed to evaluate the association of impaired nutrition with psychological health and quality of life among older adults with advanced cancer who were starting cancer treatment with palliative intent. Methods: This secondary analysis was performed on baseline data from a nationwide cluster randomized clinical trial (ClinicalTrials.gov identifier: NCT02107443; PI: Mohile; funding NCI UG1CA189961). Adults age ≥70 with advanced cancer and≥1 geriatric assessment impairment were enrolled from 2014 to 2017. Patients with BMI < 21 kg/m 2 , > 10% involuntary weight loss in the past 6 months, or Mini Nutritional Assessment Short Form (MNA-SF) score of ≤11 were considered to have impaired nutrition. We used separate multivariable linear regressions to evaluate the association of impaired nutrition with each measure of psychological health and quality of life, which included Geriatric Depression Scale (GDS-15, range 0-15), Generalized Anxiety Disorder-7 (GAD-7, range 0-21), NCCN Distress Thermometer (NCCN DT, range 0-10) and Functional Assessment of Cancer Therapy-General (FACT-G, range 0-108). Analyses were adjusted for baseline patient demographics, clinical characteristics, and geriatric assessment. Results: Among 541 patients, mean age was 77 (range 70-96) and 60% had impaired nutrition. Among the 326 patients with impaired nutrition, 95% had MNA-SF ≤11, 23% had > 10% weight loss in the last six months, and 20% had BMI < 21 kg/m 2 . Mean baseline GDS-15 was 3.1 (standard deviation [SD] 2.7), GAD-7 was 2.9 (SD 4.0), NCCN DT was 2.9 (SD 2.7), and FACT-G was 80 (SD 14). In the adjusted model, compared to those with normal nutrition, older adults with impaired nutrition had greater depression (mean 0.65 points higher on the GDS-15, p < 0.01) and lower quality of life (mean 6.0 points lower on the FACT-G, p < 0.01). There was insufficient evidence of an association between nutrition and anxiety (β = 0.67, p = 0.06) or distress (β = 0.26, p = 0.28). Conclusions: Recognition of malnutrition by oncology providers is crucial for older adults with advanced cancer. Impaired nutrition is associated with greater depression and lower quality of life among older adults with advanced cancer. Tailored resources to improve psychological health and quality of life are needed to support older adults with impaired nutrition receiving cancer treatment.

Abstract:A previous Phase 1 trial demonstrated that Shigella flexneri 2a deleted in guaBA, sen and set (strain CVD 1208) is well-tolerated and immunogenic after a single oral dose of 108 or 109 CFU. To facilitate further clinical development, the strain was reconstructed using animal-free media to conform to regulatory guidelines, and designated CVD1208S. Healthy inpatient volunteers were randomized (double-blind) to receive a single oral dose of either CVD 1208S (108 [n = 7] or 109 [n = 7] CFU) or placebo (n = 2). Both vaccine dosage levels were generally well-tolerated. Anti-lipopolysaccharide responses, measured as IgA antibody secreting cells, serum IgG, or fecal IgA levels, occurred in 7 (100%), 3 (43%) and 2 (29%) subjects, respectively, following inoculation with 109 CFU. Interferon gamma production in response to Shigella antigens was observed in 1 of 4 (25%) and 4 of 7 (57%) subjects, respectively, following inoculation with 108 and 109 CFU. We conclude that CVD 1208S retains a favorable safety and immunogenicity profile after reconstruction on animal-free media, comparable to that seen with CVD 1208, which was constructed on media containing animal products, and shows promise as a live, oral Shigella vaccine.
Relationship between markers of malnutrition and clinical outcomes in older adults with cancer: systematic review, narrative synthesis and meta-analysis

Abstract:Abstract Malnutrition predicts poorer clinical outcomes for people with cancer. Older adults with cancer are a complex, growing population at high risk of weight-losing conditions. A number of malnutrition screening tools exist, however the best screening tool for this group is unknown. The aim was to systematically review the published evidence regarding markers and measures of nutritional status in older adults with cancer (age ≥ 70). A systematic search was performed in Ovid Medline, EMBASE, Web of Science, CINAHL, British Nursing Database and Cochrane CENTRAL; search terms related to malnutrition, cancer, older adults. Titles, abstracts and papers were screened and quality-appraised. Data evaluating ability of markers of nutritional status to predict patient outcomes were subjected to meta-analysis or narrative synthesis. Forty-two studies, describing 15 markers were included. Meta-analysis found decreased food intake was associated with mortality (OR 2.15 [2.03–4.20] p = < 0.00001) in univariate analysis. Prognostic Nutritional Index (PNI) was associated with overall survival (HR 1.89 [1.03–3.48] p = 0.04). PNI markers (albumin, total lymphocyte count) could be seen as markers of inflammation rather than nutrition. There a suggested relationship between very low body mass index (BMI) (<18 kg/m 2 ) and clinical outcomes. No tool was identified as appropriate to screen for malnutrition, as distinct from inflammatory causes of weight-loss. Risk of cancer-cachexia and sarcopenia in older adults with cancer limits the tools analysed. Measures of food intake predicted mortality and should be included in clinical enquiry. A screening tool that distinguishes between malnutrition, cachexia and sarcopenia in older adults with cancer is needed.

Abstract:Abstract Background This study was sought to report the prevalence of malnutrition in elderly patients with cancer. Validate the predictive value of the nutritional assessment tool (Patient-Generated Subjective Global Assessment Short Form, PG-SGA SF) for clinical outcomes and assist the therapeutic decision. Methods This is a secondary analysis of a multicentric, observational cohort study. Elderly patients with cancer older than 65 years were enrolled after the first admission. Nutritional status was identified using the PG-SGA SF. Results Of the 2724 elderly patients included in the analysis, 65.27% of patients were male ( n = 1778); the mean age was 71.00 ± 5.36 years. 31.5% of patients were considered malnourished according to PG-SGA SF. In multivariate analysis, malnutrition(PG-SGA SF > 5) was significantly associated with worse OS (HR: 1.47,95%CI:1.29–1.68), affects the quality of life, and was related to more frequent nutrition impact symptoms. During a median follow-up of 4.5 years, 1176 death occurred. The mortality risk was 41.10% for malnutrition during the first 12 months and led to a rate of 323.98 events per-1000-patient-years. All nutritional assessment tools were correlated with each other (PG-SGA SF vs. PG-SGA: r = 0.98; PG-SGA SF vs. GLIM[Global Leadership Initiative on Malnutrition]: r = 0.48, all P < 0.05). PG-SGA SF and PG-SGA performed similarly to predict mortality but better than GLIM. PG-SGA SF improves the predictive ability of the TNM classification system for mortality in elderly patients with cancer, including distinguishing patients’ prognoses and directing immunotherapy. Conclusions The nutritional status as measured by PG-SGA SF which is a prognostic factor for OS in elderly cancer patients and could improve the prognostic model of TNM.

Abstract:Elderly cancer survivors (patients with any stage of cancer or a history of cancer) are precious members of our society and they can be easily found in various types of surveys. As is well known, good nutrition is important in elderly people suffering from cancer. Proper nutritional evaluation and intervention not only improves their quality of life but also helps them to receive adequate treatment, thereby prolonging individual survival and reducing social healthcare costs. In this study, we retrieved elderly cancer survivors from national survey data and assessed their nutritional status as good or bad. Then, we described the individual, physical, and mental health factors between people with good and bad nutrition. Physical and psychological variables associated with poor nutritional status were evaluated through regression analysis. We investigated data from the 2017 National Survey of Older Persons, and cancer patients aged 65 years or over were eligible. A total of 360 adults were enrolled and more than half (57.2%, n = 206) were in a poor nutritional status. We applied individual variable-adjusted statistical models and discovered that limited instrumental activities of daily living (IADL) (OR 2.15, 95% CI 1.08–4.28) and poor subjective health status (OR 1.74, 95% CI 1.00–3.02) were significantly associated with poor nutrition on logistic regression. Our research findings suggested that IADL and self-rated health status needed to be addressed in old cancer survivors at nutritional risk. The early recognition and management of nutrition in these populations might help them to live longer and have a better quality of life, eventually reducing socioeconomic burdens.

In the adjusted model, compared to those with no nutritional impairment, older adults with nutritional impairment had greater depression (β = 0.79, 95% CI 0.36–1.23) and anxiety severity (β = 0.86, 95% CI 0.19–1.53), and worse QOL (β = −6.31, 95% CI −8.62 to −4.00). Of the measures of nutrition, MNA‐SF ≤11 demonstrated the strongest associations with depression, anxiety, distress, and QOL. Conclusion Nutritional impairment is associated with impaired psychological health and worse QOL. Clinicians should use the MNA‐SF to screen older adults for nutritional impairment and offer tailored supportive interventions.

Abstract:The scored Patient-Generated Subjective Global Assessment (PG-SGA) is considered to be the most appropriate tool for detecting malnutrition in cancer patients. In particular, malignant tumours derived from the gastrointestinal tract may impair nutrient intake and absorption and cause malnutrition. We carried out a cross-sectional study to assess the nutritional status and related factors of patients with gastrointestinal cancer. Nutritional status was determined using the scored PG-SGA in patients (n 498) with advanced gastrointestinal cancer admitted to the Gastrointestinal Medical Oncology Unit at Beijing Cancer Hospital between 1 August 2012 and 28 February 2013. The possible related factors including age, sex, hospitalisation frequency and pathology were explored. We found that 98% of the patients required nutrition intervention and 54% of the patients required improved nutrition-related symptom management and/or urgent nutritional support (PG-SGA score ≥9). Factors related to malnutrition were age (r 0.103, P<0.01), hospitalisation frequency (r -0.196, P<0.01) and sex (the prevalence of malnutrition was higher in men than in women (9.88 v. 8.54, P<0.01)). Patients with rectal cancer had a lower risk of malnutrition than patients with other types of gastrointestinal cancer (F=35.895, P<0.01). More attention should be paid to the nutritional status of gastrointestinal patients, especially those at a higher risk of malnutrition, such as elderly patients, those hospitalised for the first time, male patients and those with other types of gastrointestinal cancer except rectal cancer. The nutritional status of these patients should be evaluated and they should be given proper nutrition education and nutritional support in a timely manner.

Abstract:254 Background: Malnutrition is prevalent among cancer patients placing them at an increased risk for treatment-related complications and poor clinical outcomes. This study investigated the relationship between malnutrition risk and healthcare utilization in a diverse sample of adult ambulatory cancer patients from the Sylvester Comprehensive Cancer Center. Methods: A retrospective analysis was conducted with data collected through the My Wellness Check (MWC), an electronic health record (EHR)-based assessment. Malnutrition was assessed through the NUTRISCORE, an integrated and validated nutritional screening tool within MWC that evaluates self-reported weight loss, appetite, cancer type, and treatment modality to assess risk of malnutrition. A NUTRISCORE greater or equal to 5 is considered malnutrition risk. Time to an emergency room (ER) visit and hospitalization were calculated in days from the first MWC questionnaire assignment and the first event, respectively. All ER visits and hospitalizations in the University of Miami Health System, self-reported patient demographics, and clinical characteristics were captured from the EHR. Uni- and multi-variate logistic regression models were used to examine factors associated with malnutrition risk. For survival analyses, the Kaplan-Meier method was used to calculate the cumulative incidence function of events and the cox proportional hazards regression models adjusting for patient demographics and clinical characteristics. Results: A total of5,984 ambulatory patients completed the MWC assessment between October 2019, and August 2023, with 31.2% (n=1,868) identified at malnutrition risk (NUTRISCORE ≥5). Patients were predominately White (n=5,092, 85.2%), and Hispanic (n=2,341, 39.1%). Hematologic cancer was the most common diagnosis (n=1284, 21.5%) followed by malignancies in digestive system (n=903, 15.1%) and male genitourinary (n=892, 14.9%). After adjusting for covariates, patients at malnutrition risk were at significantly higher risk for both ER visits (HR 1.57, 95% CI; 1.29 - 1.91, p<0.0001) and hospitalizations (17.4% vs. 7.4% at 100 days, p<0.0001) compared to those with low malnutrition risk. Factors associated with malnutrition risk included Hispanic ethnicity (aOR 1.17, 95% CI; 1.01-1.35), fatigue (aOR 1.62, 95%CI; 1.07-2.45), declined physical function (aOR 1.75, 95% CI;1.37 - 2.24), physical inactivity (aOR 1.20, 95%CI;1.04 - 1.38) and low HRQOL (aOR 2.48, 95%CI; 1.93-3.19). Conclusions: Malnutrition was associated with higher healthcare utilization in a diverse sample of ambulatory cancer patients. Hispanic ethnicity and worse PROs are factors associated with higher risk for malnutrition. Results from this work underscore the need of early nutritional interventions to prevent malnutrition and improve clinical outcomes.

Abstract:Background Malnutrition is common among elderly cancer patients which greatly affects patients' quality of life. Malnutrition is a key factor in the development of systemic chronic low-grade inflammation that can exacerbate. Methods A total of 633 elderly cancer patients from 29 tertiary public hospitals in 14 cities in China were selected as the research subjects. Physical examination, questionnaire survey, medical history collection, and blood index detection were performed on the research subjects to analyze the relationship between malnutrition, nutritional support and infection status. Results There was no significant difference in infection rate among elderly hospitalized cancer patients with different cancer types, cancer stages or therapy methods. ( P >0.05). The proportion of vomiting, loss of appetite, depression, and physical fatigue in patients with good nutritional status were lower than that in patients with moderate or severe malnutrition( P<0.05 ). Nutritional support is crucial to the nutritional status of elderly cancer patients, patients without special nutritional guidance are more likely to develop infection ( P<0.05 ). After adjusting for confounding variables, logistic regression analysis showed that ONS (OR 0.088, 95% CI 0.015-0.514), PN(OR 0.091, 95% CI 0.011-0.758), and individualized nutritional guidance (OR 0.026, 95% CI 0.003-0.202) were all protective factors associated with cancer infection. Conclusion Many elderly hospitalized cancer patients have local or systemic infections, and good nutrition is associated with lower rates of infection. Receiving nutritional guidance or support is associated with lower infection rates in elderly cancer patients.

Abstract:Objective: To assess the association between the risk of malnutrition, as estimated by the Patient-Generated Subjective Global Assessment (PG-SGA) numerical scores, and adverse outcomes in oncology patients. Design: Systematic review and meta-analysis. Settings: A comprehensive search was conducted in PubMed, Web of Science, Embase, CKNI, VIP, Sinomed, and Wanfang database. Studies that examined the association between the risk of malnutrition, as estimated by the PG-SGA numerical scores, and overall survival or postoperative complications in oncology patients were included. Patients were classified as low risk (PG-SGA≤3), medium risk (PG-SGA 4-8), and high risk of malnutrition (PG-SGA>8). Subject: Nineteen studies reporting on 20 articles (n = 9,286 patients). Results: The prevalence of medium and high risk of malnutrition ranged from 16.0% to 71.6%. A meta-analysis showed that cancer patients with medium and high risk of malnutrition had a poorer overall survival (adjusted hazard ratios [HR] 1.98; 95% confidence intervals [CI] 1.77-2.21) compared to those with a low risk of malnutrition. Stratified analysis revealed that the pooled HR was 1.55 (95% CI 1.17-2.06) for medium risk of malnutrition and 2.65 (95% CI 1.90-3.70) for high risk of malnutrition. Additionally, the pooled adjusted odds ratio for postoperative complications was 4.65 (95% CI 1.61-13.44) for patients at medium and high risk of malnutrition. Conclusions: The presence of medium and high risk of malnutrition, as estimated by the PG-SGA numerical scores, is significantly linked to poorer overall survival and an increased risk of postoperative complications in oncology patients.

Abstract:<h2 class="section-title u-h3 u-margin-l-top u-margin-xs-bottom">Summary</h2><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background & aims</h3><p>Elderly cancer patients are at particularly high risk for malnutrition because both the disease and the old age threaten their nutritional status. The Global Leadership Initiative on Malnutrition (GLIM) released new universal criteria for diagnosing and grading malnutrition, but the validation of these criteria in elderly cancer population is not well documented. Our objective was to investigate the application of the GLIM criteria in nutrition assessment and survival prediction in elderly cancer patients.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>This retrospective cohort analysis was conducted on a primary cohort of 1192 cancer patients aged 65 years or older enrolled from a multi-institutional registry, and a validation cohort of 300 elderly cancer patients treated at the First Affiliated Hospital of Sun Yat-sen University. Patients considered at-risk for malnutrition based on the NRS-2002 were assessed using the GLIM criteria. The association between the nutritional status and patients' overall survival (OS) was then analyzed by the Kaplan-Meier method and a Cox model. A nomogram was also established that included additional independent clinical prognostic variables. To determine the predictive accuracy and discriminatory capacity of the nomogram, the C-index, receiver operating characteristic (ROC) curve and calibration curve were evaluated.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The percentage of patients considered "at-risk" for malnutrition was 64.8% and 67.3% for the primary and validation cohorts, respectively. GLIM-defined malnutrition was diagnosed in 48.4% of patients in the primary cohort and 46.0% in the validation cohort. In the primary cohort, patients at risk of malnutrition (NRS-2002 ≥3) showed a worse OS than those with a NRS-2002<3 (HR 1.34, 1.10-1.64; <em>p</em>=0.003). Additionally, patients with GLIM-defined severe malnutrition (HR1.71, 1.37-2.14; <em>p</em><0.001) or moderate malnutrition (HR1.35, 1.09-1.66; <em>p</em>=0.006) showed a significantly shorter OS compared to those without malnutrition. The nomogram incorporating the domains of the GLIM with other variables was accurate, especially for predicting the 1- and 2-year overall survival rates.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>The GLIM criteria can be used in elderly cancer patients not only to assess malnutrition, but also to predict survival outcome. The nomogram developed based on the GLIM domains can provide a more accurate prediction of the prognosis than existing systems.</p>

Abstract:The negative influence of malnutrition on patients with malignant tumors runs through the whole course of a disease. To the best of our knowledge, the nutritional status of cancer patients has not yet been reported in a large sample size. Therefore, this study aimed to investigate the status of malnutrition and nutritional support therapy in a large sample of patients with common malignant tumors in China. In this observational multicenter study, 80 tertiary hospitals in 22 major provinces and cities were selected using a two-stage random sampling method. A total of 47,488 hospitalized patients with 16 kinds of common malignant tumors were enrolled in this study. The nutritional status of the patients was assessed by the scored Patient-Generated Subjective Global Assessment (PG-SGA). The severity of malnutrition was identified by a quantitative assessment where 0–1 was labelled as no malnutrition and 2–3, 4–8, and ≥9 were defined as suspected/mild, moderate and severe malnutrition. The PG-SGA scores of patients with different tumor characteristics were compared by a variance analysis. The incidence rate of moderate and severe malnutrition was 58.2%, of which 32.1% and 26.1% fell under the category of moderate and severe malnutrition, 22.2% were suspected/mild and only 19.6% of the sample were not malnourished. Abstract:BACKGROUND AND OBJECTIVES Malnutrition has adverse impacts on survival of cancer patients. The aim of the present study was to investigate the prevalence of malnutrition, and the nutrition support status in hospitalized patients with cancer in China. METHODS AND STUDY DESIGN A multi-center, cross-sectional study was conducted in 29 tertiary public hospital wards in 14 Chinese cities. Malnutrition was defined as weight loss (WL) >5% over the past 6 months or body mass index (BMI) <20 kg/m2 with WL >2%. The nutrition risk index (NRI) and performance status (PS) were evaluated. RESULTS 1138 hospitalized cancer patients (93.4% of the initial sample, 662 men, 60.6±14.5 years) were enrolled. Overall, 41.3% of patients were malnourished. The percentage of nutritional disorders as determined by the NRI was 51.4%. PS was 0 in 50.3%, 1 in 15.4%, 2 in 13.9%, and 3 or 4 in 20.4%. Compared with patients with PS of 0-1, patients with PS of 3-4 had a relative risk of malnutrition of 1.275 (95% CI 0.250-0.488, p<0.0001). Only 38.6% of patients received nutrition support, of whom 45.0% of the malnourished and 31.9% of the non-malnourished patients did; 63.2% of patients complained of poor appetite, while merely 14.0% of patients had received nutrition counseling. CONCLUSIONS The prevalence of malnutrition is high in hospitalized cancer patients, and inappropriate use of nutritional interventions highlights the urgent need to define standard operating procedures and quality control process.

Risk factors of poor nutrition in non-small cell lung cancer patients after chemotherapy: cross-sectional study

Abstract:This cross-sectional study aimed to analyze the associated factors of poor nutrition in non-small cell lung cancer (NSCLC) patients after chemotherapy. Concretely, 176 NSCLC patients who attended our hospital from June 2020 to December 2022 were enrolled. Standard-compliant patients were categorized into nutrition group (n = 38) and malnutrition group (n = 70) according to different nutrition statuses. Baseline characteristics and nutrition level were assessed. Associated factors of poor nutrition were analyzed by logistic regression analysis. There were obvious differences between nutrition group and malnutrition group in terms of age (P = 0.041), body mass index (BMI, p = 0.021), residence (P = 0.023), per capita monthly income of family (P = 0.023), tumor staging (P = 0.017), Karnofsky (KPS) score (P < 0.001), effect of chemotherapy (P = 0.045), and nutrition support before chemotherapy only (P = 0.023) and perichemotherapy (P = 0.011). The higher proportion of NSCLC patients was found in malnutrition group relative to nutrition group in terms of having poor nutritional cognition (67.14% vs. 47.37%, P = 0.045), and lacking access to vitamins (65.71% vs. 44.74%, P = 0.047) and trace elements (57.14% vs. 36.84%, P = 0.044). BMI <18.5 (OR = 3.707, P = 0.007, 95%CI (1.434-9.586)), residence in village (OR = 3.426, P = 0.013, 95%CI (1.291-9.092)), and KPS score ≤70 (OR = 7.608, P < 0.001, 95%CI (2.842-20.367)) were associated factors for poor nutrition. Collectively, BMI, residence, and KPS score were associated factors of poor nutrition in NSCLC patients after chemotherapy.

Abstract:Objective To investigate the nutritional status of patients undergoing concurrent chemoradiotherapy for ma-lignant tumor and its influencing factors.Methods A self-designed questionnaire was used to collect related clinical data of the patients who underwent concurrent chemoradiotherapy for malignant tumor in Beijing Cancer Hospital from March to May,2019.Patient-generated Subjective Global Assessment(PG-SGA)was used to investigate the nutritional status of these patients and re-lated influencing factors.Results A total of 150 usable questionnaires were collected.The median PG-SGA score was 6.0(3.0,11.0)for all 150 patients.Among these 150 patients,56(37.3%)had severe malnutrition and 49(32.7%)had moderate malnutri-tion.The univariate analysis showed that old age,esophageal cancer or head and neck cancer,stage Ⅳ tumor,more than 15 ses-sions of radiotherapy,more than 4 sessions of chemotherapy,and paclitaxel+platinum regimen or platinum monotherapy were in-fluencing factors for malnutrition in these patients(Z=-3.839--2.087,P＜0.05).The binary logistic regression analysis showed that old age,esophageal cancer or head and neck cancer,and more than 15 sessions of radiotherapy were independent risk factors for severe malnutrition in patients(P＜0.05).Conclusion There is a relatively high proportion of patients with malnu-trition among the patients undergoing concurrent chemoradiotherapy for malignant tumor,especially among elderly patients,pa-tients with esophageal cancer or head and neck cancer,and patients in the late stage of concurrent chemoradiotherapy.Medical staff should dynamically assess and monitor the nutritional status of patients and give timely intervention.
Malnutrition and its risk factors in a home for seniors in Shanghai

Abstract:Background and objectives: Older adults residing in senior homes are at a high risk of malnutrition. In this study, we investigated the nutritional status of these individuals and factors associated with malnutrition in this population. Methods and study design: This cross-sectional study (September 2020-January 2021) included a total of 583 older adults residing in a senior home in Shanghai (mean age, 85.0±6.6 years). The Mini Nutritional Assessment Short Form (MNA-SF) questionnaire was administered to assess the nutritional status of the participants. Patients with possible sarcopenia were identified according to the guidelines recommended by the Asian Working Group for Sarcopenia in its 2019 consensus (AWGS 2019). Moreover, the factors influencing malnutrition were determined through multivariate analyses. Results: The likelihoods of having malnutrition and being at a risk of malnutrition were noted in 10.5% and 37.4% of the participants, respectively. In both male and female participants, handgrip strength (HGS) and calf circumference (CC) increased significantly with increasing scores on the aforementioned questionnaire (p<0.001). Among the participants, 44.6% had ≥3 chronic diseases and 48.2% used multiple medicines. Multivariate analyses revealed that dys-phagia (OR, 3.8; 95% CI, 1.7-8.5), possible sarcopenia (OR, 3.6; 95% CI, 2.2-5.6), and dementia (OR, 4.5; 95% CI, 2.8-7.0) were correlated with a relatively high prevalence of malnutrition/malnutrition risk. Exercise (at least thrice a week) reduced malnutrition risk. Conclusions: Malnutrition is common among older adults residing in senior homes; therefore, the associated factors must be identified, and appropriate interventions should be administered.
Clinical Significance of Preoperative Nutritional Status in Elderly Gastric Cancer Patients Undergoing Radical Gastrectomy: A Single-Center Retrospective Study

Abstract:BACKGROUND The population of elderly patients with gastric cancer is increasing, which is a major public health issue in China. Malnutrition is one of the greatest risk factors for adverse clinical outcomes in elderly patients with gastric cancer. AIM To investigate the preoperative nutritional status and its association with delayed discharge of elderly gastric cancer patients following radical gastrectomy. METHODS A total of 783 patients aged 65 years and older harboring gastric adenocarcinoma and following radical gastrectomy were retrospectively analyzed from the prospectively collected database of Zhongshan Hospital of Fudan University between January 2018 and May 2020. RESULTS The overall rate of malnutrition was 31.8%. The incidence of postoperative complications was significantly higher in the malnourished group compared to the well-nourished group (P < 0.001). Nutritional characteristics in the malnourished group, including body mass index, prognostic nutritional index (PNI), albumin, prealbumin, and hemoglobin, were all significantly lower than those in the well-nourished group. The percentage of patients who received postoperative total nutrient admixture was lower in the malnourished group compared to the well-nourished group (22.1% vs 33.5%, P = 0.001). Age ≥ 70 years (HR = 1.216, 95%CI: 1.048-1.411), PNI < 44.5 (HR = 1.792, 95%CI: 1.058-3.032), operation time ≥ 160 minutes (HR = 1.431, 95%CI: 1.237-1.656), and postoperative complications grade III or higher (HR = 2.191, 95%CI: 1.604-2.991) were all recognized as independent risk factors associated with delayed discharge. CONCLUSION Malnutrition is relatively common in elderly patients undergoing gastrectomy. Low PNI is an independent risk factor associated with delay discharge. More strategies are needed to improve the clinical outcome of these patients.
