Abstract:In recent years, master–slave vascular robots have been developed to address the problem of radiation exposure during vascular interventions for surgeons. However, the single visual feedback reduces surgeon immersion and transparency of the system. In this work, we have developed a haptic interface based on the magnetorheological fluid (MRF) on the master side. The haptic interface can provide passive feedback force with high force fidelity and low inertia. Additionally, the manipulation of the master device does not change the operating posture of traditional surgery, which allows the surgeon to better adapt to the robotic system. For the slave robot, the catheter and guidewire can be navigated simultaneously which allows the two degrees of action on the catheter and axial action of a guidewire. The resistance force of the catheter navigation is measured and reflected to the user through the master haptic interface. To verify the proposed master–slave robotic system, the evaluation experiments are carried out in vitro, and the effectiveness of the system was demonstrated experimentally.

Abstract:Neuroendovascular catheterization using fluoroscopy poses the problem to operators and staffs of cumulative radiation exposure. To solve this problem, we are developing a remote-controlled master-slave robot. Because a wire-like elongated treatment device is inserted into a blood vessel using a catheter, the robot requires a sensor to detect the insertion force of the wire. The proposed sensor is integrated into a robot installed in an X-ray fluoroscopy room that is remotely controlled from another room. The features of this sensor include measurement of the insertion force with sufficient accuracy, simple wire attachment, and an inexpensive disposable sensor head, rendering it very suitable for practical application. In this paper, we report on these features, as well as the results of a practical test of the sensor using a cerebrovascular model.

Abstract:Robot-assisted vascular interventional surgery (RVIS) is an emerging technology for the treatment of vascular diseases. It has obvious advantages over traditional manual operation, such as increased accuracy, reduced fatigue, and reduced tremor. However, current research suggests that natural human–robot interaction in RVIS is still a challenge that needs to be addressed. In this article, we developed a novel robotic platform that realized magnetorheological (MR) fluids-based haptic feedback to improve the interventionist’s tactile presence. In addition, we proposed a force sensing method to accurately detect the real-time force of the flexible instrument and a collaborative operation method of the guidewire and the catheter to assist the flexible instruments in selecting the target blood vessel branch and reduce the operation difficulty of the interventionist. To verify the developed robotic platform and the proposed methods, we conducted the performance evaluation experiments in a blood vessel model and an endo vascular evaluator. The results indicated that the developed robotic platform and the proposed methods have great potential to improve the natural human–robot interaction in RVIS and guarantee safety.

Abstract:The robot-assisted endovascular catheterization system (RAECS) has the potential to address some of the procedural challenges and separate interventionalists from the X-ray radiation during the endovascular surgery. However, the employment of robotic systems is partly changing the natural gestures and behavior of medical professionals. This paper presents a RAECS that augments surgeon's motions using the conventional catheter as well as generates the haptic force feedback to ensure surgery safety. The magnetorheological fluids based haptic interfaceexperiment is proposed to measure the actions of the operator and provide the haptic force. The slave catheter manipulator is presented to actuate the patient catheter in two degrees of freedom (radial and axial direction). In addition, a force model is provided to characterize the kinematics of the catheter intervention. Afterward, the “pseudocollision” and “real collision” are utilized to descript the catheter tip–vessel interaction. The in vitro results are presented, which demonstrate that catheter-based haptic force feedback has a benefit for improving catheterization skills as well as reducing the cognitive workload of operators.

Abstract:In a vascular interventional surgery robot system, accurately pushing the guidewire into the patient-specific branch vessel is the core step of the entire operation, so it has become the focus of the master-slave co-control of the guidewire propulsion and manipulation mechanism. Some vascular interventional surgery robots have been used for delivery of the guidewire; however, problems such as the inability to reliably clamp the guidewire and the lack of accurate force feedback prevent doctors from using robots for accurate delivery. In addition, failure to disinfect surgical robots quickly and completely increases the risk of surgery. This article introduces a new type of master-slave vascular interventional robot with reliable clamping of the catheter/guidewire, accurate perception of the lead-in force of the guidewire for precise delivery and fast disinfection. In response to the problem of the nonlinear and uncertain disturbance of the catheter/guidewire resistance affecting the delivery mechanism, an adaptive sliding controller based on master-slave tracking is designed. Through experiments and analysis of the fuzzy sliding mode controller (FSMC) experimental platform, the results show that a vascular interventional robot control system with good tracking performance and strong robustness has been designed.

Abstract:Background: Advances in robotic technology have been adopted in various subspecialties of both open and minimally invasive surgery, offering benefits such as enhanced surgical precision and accuracy with reduced efforts and fatigue. Despite the advantages, robotic applications to endovascular neurosurgery have remained largely unexplored due to technical challenges such as the miniaturization of robotic devices that can navigate the complex and tortuous cerebral vasculature. Although some commercial robotic systems enable precise manipulation of conventional guidewires for coronary and peripheral vascular interventions, they remain unsuited for neurovascular applications due to the considerably smaller and more tortuous anatomy of cerebral arteries. Methods: We present a teleoperated robotic system designed for neurovascular interventions based on magnetic steering and navigation. Our system consists of a magnetically controlled guidewire, a robot arm with a permanent magnet attached to its end-effector for steering control of the guidewire, a set of motorized linear drives to advance or retract the guidewire and a microcatheter, and a remote-control console to operate the system under feedback from real-time fluoroscopic imaging and visualization. Results: With a life-sized neurovascular phantom, we demonstrate the navigational capability to reach several branches of circle of Willis and distal cerebral vasculature with the magnetic guidewire under teleoperation of the system. We further demonstrate telerobotically-assisted therapeutic procedures including coil embolization and clot retrieval thrombectomy for intracranial aneurysms and ischemic stroke, respectively. Conclusion: Our telerobotic neurointerventional platform could enable safer and quicker access to hard-to-reach lesions, significantly reduce operative time and perioperative risk by reducing intravascular manipulation, minimize the radiation exposure to interventionalists, and open the possibility of remote procedural services to address challenges in current stroke systems of care. Note: Animal testing will be performed in Q3 2021 and the results will be presented at ISC.

Abstract:Interventional surgery is widely used in the treatment of cardiovascular and cerebrovascular diseases, and the development of surgical robots can greatly reduce the fatigue and radiation risks brought to surgeons during surgery. In this paper, we present a novel interventional surgical robot which allows surgeons to fully use their operating skills during remote control. Fuzzy control theory is used to guarantee control precision during the master-slave operation. The safety force feedback control is designed based on the catheter and guidewire spring model, and the force-position control is designed to decrease the potential damage due to the control delay. This study first evaluates the force-position control strategy using a vascular model experiment, and then an in vivo experiment is used to evaluate the precision of the surgical robot controlling the catheter and guidewire to the designated position. The in vivo experiment results and surgeon's feedback demonstrate that the proposed surgical robot is able to perform complex remote surgery in clinical application.Surgeons perform remote interventional animal surgery using interventional surgical robots.

Abstract:In this paper, we proposed a novel master-slave robotic catheterization system with force feedback for VIS (Vascular Interventional Surgery). The force feedback to the operator on the master side is the key factor to improve the safety during VIS. The developed system used the MR (magneto rheological) fluid to realize force feedback, and it used the developed multidimensional monitoring interface to realize the visualization of force feedback, the developed multidimensional monitoring interface can monitor the motion information of the catheter and contact force between catheter tip or side wall and blood vessel wall, and the motion data of the catheter was collected and generated diagram for reference to surgeon. We have developed a force sensor array to detect the contact force between catheter tip or side wall and blood vessel wall. The force information was detected by the developed contact force sensor array when the catheter contacted with the blood vessel. The force feedback and multidimensional information monitoring interface evaluation experiments were done, the tracking characteristic evaluation experiments were also carried out, the experimental results indicated that the developed novel robotic catheterization system with force feedback and visualization of force feedback is effective for VIS, it can improve the safety during VIS.

Abstract:Interventional surgical robots are widely used in neurosurgery to improve surgeons’ working environment and surgical safety. Based on the actual operational needs of surgeons’ feedback during preliminary in vivo experiments, this paper proposed an isomorphic interactive master controller for the master–slave interventional surgical robot. The isomorphic design of the controller allows surgeons to utilize their surgical skills during remote interventional surgeries. The controller uses the catheter and guidewire as the operating handle, the same as during actual surgeries. The collaborative operational structure design and the working methods followed the clinical operational skills. The linear force feedback and torque feedback devices were designed to improve the safety of surgeries under remote operating conditions. An eccentric force compensation was conducted to achieve accurate force feedback. Several experiments were carried out, such as calibration experiments, master–slave control performance evaluation experiments, and operation comparison experiments on the novel and previously used controllers. The experimental results show that the proposed controller can perform complex operations in remote surgery applications and has the potential for further animal experiment evaluations.

Abstract:Objective: Inadequate visual and force feedback while navigating surgical tools elevate the risk of endovascular procedures. It also poses occupational hazard due to repeated exposure to X-rays. A teleoperated robotic system that augments surgeon's actions is a solution. Method: We have designed and developed an endovascular robotic system that augments surgeon's actions using conventional surgical tools, as well as generates feedback in order to ensure safety during the procedure. The reaction force from vasculature is estimated from motor current that drives the surgical tool. Calibration required for force estimation is based on bilevel optimization. Input shaping is used in conjunction with a cascaded controller to avoid large responses due to faster inputs and to track tool position. The design, realization, and testing of our system are presented. Results: The responses of the system in comparison with the dynamics model is similar vis-à-vis the same input commands. Any error in the position tracking is reduced by the cascaded controller. Phase-portrait analysis of the system showed that the system is stable. The reaction force estimation is validated against load cell measurements. The safety mechanism in the events of excessive reaction forces while interacting with vasculature is demonstrated. Conclusion and significance: Our system is a step toward intelligent robots that can assist surgeons during endovascular procedures by monitoring and alerting the surgeons regarding detrimental parameters. It arrests any unintended excursions of the surgical tools or surgeon's actions. This will also eliminate the need for surgeons to be in radiation environment.

Abstract:This paper presents a robotic catheter system with force sensors, monitor and a master–slave remote control system. We developed micro force sensors and applied them in the system to guarantee the operation safety in intravascular neurosurgery applications, and employed a camera to monitor the operation. Two kinds of force information are obtained through force sensors when the catheter contacted the blood vessel. The experiment shows that the proposed force sensors-based catheter system works well through force feedback and remote control. The system can facilitates the operation and avoid potential damages.

Abstract:Cardiovascular diseases (CVDs) are the deadliest diseases worldwide. Master-slave robotic systems have been widely used in vascular interventional surgery with the benefit of high safety, efficient operation, and procedural facilitation. This paper introduces a remote-controlled vascular interventional robot (RVIR) that aims to enable surgeons to perform complex vascular interventions reliably and accurately under a magnetic resonance imaging (MRI) environment. The slave robot includes a guidewire manipulator (GM) and catheter manipulator (CM) that are mainly composed of a hollow driving mechanism and a linear motion platform. The hollow driving mechanism is based on a traveling wave-type hollow ultrasonic motor (HUM) which has high positional precision, fast response, and magnetic interference resistance and realizes the cooperation of the guidewire and catheter by omitting the redundant transmission mechanism and maintaining good coaxiality. The HUM stator, the core part of the RVIR, is optimized by an adaptive genetic algorithm for better quality and greater amplitude of traveling waves, which are beneficial to the drive efficiency and precision. The robot system features great cooperating performance, small hysteresis, and high kinematic accuracy and has been experimentally verified for its capability to precisely manipulate the guidewire and catheter.

Our system consists of a magnetically controlled guidewire, a robot arm with an actuating magnet to steer the guidewire, a set of motorized linear drives to advance or retract the guidewire and a microcatheter, and a remote-control console to operate the system under real-time fluoroscopy. We demonstrate our system’s capability to navigate narrow and winding pathways both in vitro with realistic neurovascular phantoms representing the human anatomy and in vivo in the porcine brachial artery with accentuated tortuosity for preclinical evaluation. We further demonstrate telerobotically assisted therapeutic procedures including coil embolization and clot retrieval thrombectomy for treating cerebral aneurysms and ischemic stroke, respectively. Our system could enable safer and quicker access to hard-to-reach lesions while minimizing the radiation exposure to physicians and open the possibility of remote procedural services to address challenges in current stroke systems of care.

Abstract:Endovascular therapy has emerged as a crucial therapeutic method for treating vascular diseases. Endovascular surgical robots have been used to enhance endovascular therapy. However, to date, there are no universal endovascular surgical robots that support molds of different types of devices for treating vascular diseases. We developed a novel endovascular surgical robotic system that can independently navigate the intravascular region, advance and retract devices, and deploy stents. This robot has four features: (1) The bionic design of the robot can fully simulate the entire grasping process; (2) the V-shaped relay gripper waived the need to redesign special guidewires and catheters for continuous rotation; (3) the handles designed based on the feedback mechanism can simulate push resistance and reduce iatrogenic damage; and (4) the detachable design of the grippers can reduce cross-infection risk and medical costs. We verified its performance by demonstrating six different types of endovascular surgeries. Early evaluation of the novel endovascular robotic system demonstrated its practicability and safety in endovascular surgeries.

Abstract:Remote-controlled vascular interventional robots (RVIRs) are being developed to increase the overall accuracy of surgical operations and reduce the occupational risks of intervening physicians, such as radiation exposure and chronic neck/back pain. Several RVIRs have been used to operate catheters or guidewires accurately. However, a lack of cooperation between the catheters and guidewires results in the surgeon being unable to complete complex surgery by propelling the catheter/guidewire to the target position. Furthermore, it is a significant challenge to operate the catheter/guidewire accurately and detect their proximal force without damaging their surfaces. In this study, we introduce a novel method that allows catheters and guidewires to be operated simultaneously in complex surgery. Our method accurately captures force measurements and enables precisely controlled catheter and guidewire operation. A prototype is validated through various experiments. The results demonstrate the feasibility of the proposed RVIR to operate a catheter and guidewire accurately, detect the resistance forces, and complete complex surgical operations in a cooperative manner.

Abstract:In this paper, a novel robotic catheter system was proposed, which can be used to assist surgeon to do the operation of vascular interventional surgery (VIS), the robotic catheter system can measure the contact force and friction between catheter and blood vessel, we used load cell to measure the friction between catheter and blood vessel, the friction can be transmitted to the operator's hand as feedback force, and this method was not paid attention in the past research, furthermore, we used an optical fiber force sensor to measure the contact force between catheter tip and blood vessel, the contact force information can be transmitted to the operator as visual information in real time. Meanwhile, the optical fiber force sensor is through inside the catheter as a guide wire, which is served to lead catheter forward. We did the simulation experiment (“In Vitro”) by using endovascular evaluator (EVE), the experimental results indicated that the developed robotic catheter system works well, and they can effectively assist surgeon to do the operation and improve safety during VIS.

Abstract:Robot-assisted technology is often used to perform endovascular catheterization surgeries, which generally depend on the flexible operability and the accurate force feedback of a robotic system. In this paper, an endovascular catheterization robotic system (ECRS) was developed to improve collaborative operation and haptic force feedback. A couple of operating handles were designed to maximize the use of the natural operations of surgeons on the master side, which is a flexible and ergonomic device. A magnetically controlled haptic force feedback structure is proposed based on hydrogel and solid magnetorheological (MR) fluid to offer a sense of haptic feedback to operators; this has potential influence on the field of force feedback. In addition, a unique tremor-reduction structure is introduced to enhance operating safety. Tracking performance experiments and in vitro experiments were conducted to evaluate the performance of the developed ECRS. According to these experimental results, the average translation-tracking error is 0.94 mm, and the average error of rotation is 0.89 degrees. Moreover, in vitro experiments demonstrated that haptic feedback has the advantage of reducing workload and shortening surgery completion time. The developed ECRS also has the benefits of inspiring other researchers to study collaborative robots and magnetically controlled feedback.
