Abstract:Intergenerational consequences of extensive trauma experienced by parents for the loneliness experienced by their children were explored in 52 adults (26 men and 26 women) who grew up in Holocaust survivor families. These adults, children of mothers who had survived Nazi concentration camps, were recruited from a random nonclinical Israeli sample. A narrative analysis of their recollected accounts of loneliness in childhood and adolescence yielded 4 major categories of loneliness experiences in the context of growing up in Holocaust survivor families: (a) echoes of parental intrusive traumatic memories; (b) echoes of parental numbing and detachment; (c) perceived parents' caregiving style; and (d) social comparison with other families, in particular the lack of grandparents. The echoes of the parental trauma in the recollected loneliness accounts are conceptualized as representing a sense of failed intersubjectivity in these interpersonal processes. The experiences of not being understood by others, not understanding others, and the lack of shared understanding involved in failed intersubjectivity are discussed and related to the importance of opening lines of communication between survivors and their descendents.

Abstract:Purpose - The purpose of this paper is to propose a robotic system for percutaneous surgery. The key component in the system, a robotic arm that can manipulate a puncture needle is presented. The mechanical design, the motion control and the force control method of the robotic arm are discussed in the paper.Design/methodology/approach - The arm with an arc mechanism placed on a 3D Cartesian stage is developed as a puncture needle manipulator to locate the position of the needle tip, tune the needle's posture and actuate the puncture motion under the visual guidance of two orthogonal X-ray images of a patient by a surgeon. A focusing method by using two laser spots is proposed to automatically move the needle tip to a surgery entry point on the skin. A dynamics model is developed to control the position of the needle mechanism and an explicit force control strategy is utilized to perform the needle insertion.Findings - With the surgical system, a surgeon can easily perform puncture operation by taking two orthogonal real-time X-ray images as a visual feedback and accurately navigating the needle insertion. The laser-guided focusing method is efficient in placement of the needle tip. The explicit force control strategy is proved to be effective for holding constant and stable puncture force in experiments.Originality/value - The robotic arm has an advantage in easy redirection of the needle because the rotation and the translation are decoupled in the mechanism. By adopting simple laser pens and a well-developed kinematics model, the system can handle the entry point, locating task automatically. The focusing method and the force control method proposed in the paper are useful for the present system and could be intuitive for similar surgical robots.

Abstract:BackgroundAlthough robotic laparoscopic surgery has various benefits when compared with conventional open surgery and minimally invasive surgery, it also has issues to overcome and one of the issues is the discontinuous surgical flow that occurs whenever control is swapped between the endoscope system and the operating robot arm system. This can lead to problems such as collision between surgical instruments, injury to patients, and increased operation time. To achieve continuous surgical operation, a wireless controllable stereo endoscope system is proposed which enables the simultaneous control of the operating robot arm system and the endoscope system.MethodsThe proposed system consists of two improved novel master interfaces (iNMIs), a four-degrees of freedom (4-DOFs) endoscope control system (ECS), and a simple three-dimensional (3D) endoscope. In order to simultaneously control the proposed system and patient side manipulators of da Vinci research kit (dVRK), the iNMIs are installed to the master tool manipulators of dVRK system. The 4-DOFs ECS consists of four servo motors and employs a two-parallel link structure to provide translational and fulcrum point motion to the simple 3D endoscope. The images acquired by the endoscope undergo stereo calibration and rectification to provide a clear 3D vision to the surgeon as available in clinically used da Vinci surgical robot systems. Tests designed to verify the accuracy, data transfer time, and power consumption of the iNMIs were performed. The workspace was calculated to estimate clinical applicability and a modified peg transfer task was conducted with three novice volunteers.ResultsThe iNMIs operated for 317 min and moved in accordance with the surgeon’s desire with a mean latency of 5 ms. The workspace was calculated to be 20378.3 cm3, which exceeds the reference workspace of 549.5 cm3. The novice volunteers were able to successfully execute the modified peg transfer task designed to evaluate the proposed system’s overall performance.ConclusionsThe experimental results verify that the proposed 3D endoscope system enables continuous surgical flow. The workspace is suitable for the performance of numerous types of surgeries. Therefore, the proposed system is expected to provide much higher safety and efficacy for current surgical robot systems.

Abstract:Robotic surgery is preferred over open or laparoscopic surgeries due to its intuitiveness and convenience. However, prolonged use of surgical robots can cause neck pain and joint fatigue in wrist and fingers. Also, input systems are bulky and difficult to maintain. To resolve these issues, we propose a novel input module based on real-time 3D hand tracking driven by RGB images and MediaPipe framework to control surgical robots such as patient side manipulator (PSM) and endoscopic camera manipulator (ECM) of da Vinci research kit. In this paper, we explore the mathematical basis of the proposed 3D hand tracking module and provide a proof-of-concept through user experience (UX) studies conducted in a virtual environment. End-to-end latencies for controlling PSM and ECM were 170 ± 10 ms and 270 ± 10 ms, respectively. Of fifteen novice participants recruited for the UX study, thirteen managed to reach a qualifiable level of proficiency after 50 min of practice and fatigue of hand and wrist were imperceivable. Therefore, we concluded that we have successfully developed a robust 3D hand tracking module for surgical robot control and in the future, it would hopefully reduce hardware cost and volume as well as resolve ergonomic problems. Furthermore, RGB image driven 3D hand tracking module developed in our study can be widely applicable to diverse fields such as extended reality (XR) development and remote robot control. In addition, we provide a new standard for evaluating novel input modalities of XR environments from a UX perspective.

Integrated Planning and Control of Robotic Surgical Instruments for Tasks Autonomy

Abstract:Agile maneuvers are essential for robot-enabled complex tasks such as surgical procedures. Prior explorations on surgery autonomy are limited to feasibility study of completing a single task without systematically addressing generic manipulation safety across different tasks. We present an integrated planning and control framework for 6-DoF robotic instruments for pipeline automation of surgical tasks.We leverage the geometry of a robotic instrument and propose the nodal state space (NSS) to represent the robot state in SE(3) space. Each elementary robot motion could be encoded by regulation of the state parameters via a dynamical system. This theoretically ensures that every in-process trajectory is globally feasible and stably reached to an admissible target, and the controller is of closed-form without computing 6-DoF inverse kinematics. Then, to plan the motion steps reliably, we propose an interactive (instant) goal state of the robot that transforms manipulation planning through desired path constraints into a goal-varying manipulation (GVM) problem. We detail how GVM could adaptively and smoothly plan the procedure (could proceed or rewind the process as needed) based on on-the-fly situations under dynamic or disturbed environment. Finally, we extend the above policy to characterize complete pipelines of various surgical tasks. Simulations show that our framework could smoothly solve twisted maneuvers while avoiding collisions. Physical experiments using the da Vinci Research Kit (dVRK) validates the capability of automating individual tasks including tissue debridement, dissection, and wound suturing. The results confirm good task-level consistency and reliability compared to state-of-the-art automation algorithms.

Abstract:In this paper, a safety-enhanced collaborative control framework is proposed for tele-operated minimally invasive surgery (MIS) using a redundant 7-DoF serial robot. The redundant manipulators offer a safe physical collaborative flexible workspace for nurse or surgeon (assisting physicians, patient support) undergoing surgery. The novel framework integrates a Cartesian compliance control strategy to guarantee that the tele-operated surgical tool always goes through the trocar position, and a safety-enhanced null-space collaborative strategy to constrain the swivel motion in an assumed safe range. Two event-based operative procedures (hands-on motion and teleoperation) are performed in torque level to achieve the whole surgical task. Finally, experimental studies with virtual surgical tasks were conducted to validate the effectiveness of the proposed framework, using the KUKA LWR 4+ robot and Sigma 7 master device. It provided an online flexible and safe collaborative way without degradation of the quality of the surgical task.

Abstract:Abstract Single Incision Laparoscopic Surgery (SILS) is a fast-growing method in the field of MIS (Minimally Invasive Surgery) that has the potential to represent the future of laparoscopic surgeries. The major benefits of SILS results from a single incision which makes surgeries essentially scar-less, and it can reduce wound infection substantially as well as recuperation time. Many new researches are now focusing on developing cutting edge technologies to support SILS; however, the practical applications of SILS are constrained by a number of intricacies such as space limitation, absence of dexterous multitasking tools, lack of sufficient actuation force and poor visualization. Deployment and retraction of surgical tools or robots are done manually in the absence of a multitasking tool manipulator which increases the surgery time, risk of injury and surgeon’s fatigue. Our research focuses on designing a novel operative hardware (multitasking manipulator) to facilitate the SILS technique with automatic tool changing capability. A wire driven mechanism has been implemented in the design to minimize the damage to the electronic hardware during sterilization since the electronic actuation and sensing components are located remotely from the end-effector which requires heat or chemical sterilization before surgery. And a wire-driven articulated robotic arm has also been designed to support the manipulator. The details of the robotic design and analysis are conducted in the paper. The feasibility of this robotic method has been demonstrated by experiments.

Abstract:<p class="a-plus-plus">In endoscopic endonasal transsphenoidal surgery, for the treatment of deep brain tumors, such as craniopharyngiomas, an endoscope and surgical instruments are inserted through a nasal cavity into the lesion to remove the tumor. This has recently become the preferred technique because there is less likelihood of neural damage and a low complication rate. Manually controlled rigid surgical instruments are available for this procedure, but they provide limited dexterity and field of view. Therefore, some areas remain inaccessible when these surgical instruments are used. To solve these problems, we propose a surgical robot system for endoscopic endonasal transsphenoidal surgery. We defined a target surgical space based on an analysis by a surgeon and designed surgical instruments to reach this target space. The system consists of two robot arms, end-effectors, surgical instruments, a master device, a control device, and a robot base. The robot arm has an end-effector exhibiting two degrees of freedom (DOFs) and an inner channel, into which flexible surgical instruments are inserted. The flexible surgical instrument can reach the target space by steering the robot arm and end-effector. The outer diameter of the end-effector is 4 mm, and the diameter of the instrument channel, into which commercial surgical instruments can be integrated, is 2 mm. We motorized the motion of the robot arms, end-effectors, and instruments and included motion capability with the necessary precision, and developed a master device and control device to operate them. The surgical robot base is used to place the surgical robot before the operation and allow for manual operation. In a cadaver experiment, it was confirmed that the robot system can reach a larger area than is accessible with current surgical instruments, and it can support or remove tissues in the target surgical space. We received productive feedback from the surgeon conducting the experiment, and further research is underway to improve the robot system.</p>

Abstract:Recent advances in telecommunications have enabled surgeons to operate remotely on patients with the use of robotics. The investigation and testing of remote surgery using a robotic arm is presented. The robotic arm is designed to have four degrees of freedom that track the surgeon's x, y, z positions and the rotation angle of the forearm {\theta}. The system comprises two main subsystems viz. the detecting and actuating systems. The detection system uses infrared light-emitting diodes, a retroreflective bracelet and two infrared cameras which as a whole determine the coordinates of the surgeon's forearm. The actuation system, or robotic arm, is based on a lead screw mechanism which can obtain a maximum speed of 0.28 m/s with a 1.5 degree/step for the end-effector. The infrared detection and encoder resolutions are below 0.6 mm/pixel and 0.4 mm respectively, which ensures the robotic arm can operate precisely. The surgeon is able to monitor the patient with the use of a graphical user interface on the display computer. The lead screw system is modelled and compared to experimentation results. The system is controlled using a simple proportional-integrator (PI) control scheme which is implemented on a dSpace control unit. The control design results in a rise time of less than 0.5 s, a steady-state error of less than 1 mm and settling time of less than 1.4 s. The system accumulates, over an extended period of time, an error of approximately 4 mm due to inertial effects of the robotic arm. The results show promising system performance characteristics for a relatively inexpensive solution to a relatively advanced application.

Abstract:A novel separate robotic system for minimally invasive surgery (MIS) has been presented in this paper. Control system architectures were designed, basing on versatile performance criteria. The compact control and mechanical structure were suitable for medical environment. Function and safety design satisfying medical application were integrated into the robot system as well. Additionally, intuitive control algorithm solved the problems of hand-eye incoordination and workspace mismatch between master hands and slave arms during the master–slave control process. A series of experiments have been accomplished to evaluate the performance of the robotic system at last. The results demonstrated that the robotic system was capable of executing surgical operation intuitively and implementing auxiliary functions perfectly, which meant that the control system was feasible and reliable.

Abstract:OBJECTIVE To detect the correlation between the clinical phenotype of spinal muscular atrophy (SMA) and survival motor neuron gene (SMN2) copy number. METHODS The SMN2 gene copy numbers of 57 different types of SMA were detected by real-time fluorescence quantitative PCR method with TaqMan technique. RESULTS Average SMN2 copy number was 1.017 +/- 0.090, 2.019+/- 0.080, 3.104+/- 0.170 in predicting one, two, three copy numbers, respectively, and CV was 8.9%, 3.9%, 5.4%, respectively. Average SMN2 copy number was 1.926+/- 0.460, 2.508+/- 0.460, 2.876+/- 0.270, in type I, II and III SMA, respectively. The SMN2 gene copy number in type II and III SMA were higher than that of type I SMA (P < 0.01). The SMN2 gene copy number in type III SMA was higher than that of type II SMA (P < 0.01). 85.72% of type I SMA patients usually had 2 SMN2 copies; 40% and 60% of type II SMA patients had 2 and 3 SMN2 copies, respectively; 82% of type III SMA patients had 3 SMN2 copies. CONCLUSION There is significant correlation between the change of SMA clinical phenotype and SMN2 cope number. The distributions of the SMN2 gene copy number are various in different types of SMA patients. All types of SMA patients have at least one copy SMN2. The SMN2 gene copy numbers in type II, III SMA are higher than that of type I. All of these findings suggest that the severity of SMA patients depend on the change of the SMN2 copy numbers.

Abstract:In minimally invasive surgery, endoscopes serve as the eyes of surgeon. To avoid fatigue in manual endoscope steering, robotic endoscope holders have been developed. Unfortunately, existing robotic endoscope holders are not widely adopted due to the poor surgeon-robot cooperation. In this work, we developed an intelligent flexible endoscope system based on the da Vinci Research Kit. In the system, surgical instruments are detected and classified in real time with an oriented bounding box-based object detection method. A custom dataset of 6243 images is established to train the detection neural network. Then, a surgeons preference guided visual servoing control method is proposed for automatically tracking the detected instruments during minimally invasive surgery. In order to realize 3-degree of freedom control on the image plane, an image moment-based visual servoing control method is adopted. To improve the dynamic performance of the system control, a modified genetic algorithm is developed to select the optimal gains of the robot controller. Both simulation and experimental results show the feasibility of the proposed intelligent flexible endoscope system.

Abstract:An innovative single-port surgical robot has recently been developed by the Samsung Advanced Institute of Technology (SAIT). The robot can reach various surgical sites inside the abdominal cavity from a single incision on the body. It has two 7-DOF surgical tools, a 3-DOF endoscope, a flexible hyper-redundant 6-DOF guide tube, and a 6-DOF manipulator. This paper primarily focuses on the manipulator, called a slave arm, which is capable of setting the location of a Remote Center Motion (RCM) point. Because the surgical tools can explore the abdominal area through a small incision point when the RCM point is aligned with the incision area, the RCM mechanism is an integral part of the manipulator for single-port surgery. The mechanical feature, operational principle, control method, and the system architecture of the slave arm are introduced in this paper. In addition, manipulation experiments conducted validate its efficacy.

Abstract:Robotic surgery and novel surgical instrumentation present great potentials towards safer, more accurate and consistent minimally invasive surgery. However, their adoption is dependent to the access to training facilities and extensive surgical training. Robotic instruments require different dexterity skills compared to open or laparoscopic. Surgeons, therefore, are required to invest significant time by attending extensive training programs. Contrary, hands on experiences represent an additional operational cost for hospitals as the availability of robotic systems for training purposes is limited. All these technological and financial barriers for surgeons and hospitals hinder the adoption of robotic surgery. In this paper, we present a mobile dexterity training kit to develop basic surgical techniques within an affordable setting. The system could be used to train basic surgical gestures and to develop the motor skills needed for manoeuvring robotic instruments. Our work presents the architecture and components needed to create a simulated environment for training sub-tasks as well as a design for portable mobile manipulators that can be used as master controllers of different instruments. A preliminary study results demonstrate usability and skills development with this system.

Abstract:Robotic assistance is promising for improving minimally invasive surgery (MIS). This work presents asymmetric bimanual control of a dual-arm serial robot with two remote centers of motion (RCMs) constraints for MIS. In our previous works, general null space controllers to guarantee the fixed RCM constraint have been proposed. However, an incision on a patient's abdominal wall is not fixed owing to the respiration of the patient, which generates an uncertain disturbance at the joints of robotic manipulators. To improve accuracy, a radial basis function neural network is implemented to adapt to these disturbances and control the end-effector position. Finally, the adaptive bimanual control strategy is validated through simulations based on clinical data. The proposed control shows improved accuracy in the end effector position for all the designed surgical tasks. In future works, the algorithm will be validated on an actual dual-arm serial robot making use of a body phantom.
Innovative Effector Design for Simulation Training in Robotic Surgery

Abstract:Feasibility and effectiveness of minimally invasive surgery have made the use of robotic surgical system a rising prevalence with a wide range of applications. However, as a remarkable result, how to adroitly and precisely manipulate the sophisticated robotic surgery instruments is of fundamental importance to new surgeons. In order to provide the robotic surgical skill training for surgeons, we designed and built a training simulator with virtual reality equipped with two new hardware interfaces (effectors) which can emulate the operating process of the actual manipulators in the robotic surgical system. By using our designed framework, medical trainees can use the simulator to practice the fundamental technique of the surgical operation. To develop training platform and analyze performance, personal real time operating data, such like space position, moving speed and acceleration parameters of the virtual robotic instruments, can be collected by the sensors on the effectors and transmitted through wireless communication to the PC terminal for surgical skill performance analysis.

Abstract:Objective: Inadequate visual and force feedback while navigating surgical tools elevate the risk of endovascular procedures. It also poses occupational hazard due to repeated exposure to X-rays. A teleoperated robotic system that augments surgeon's actions is a solution. Method: We have designed and developed an endovascular robotic system that augments surgeon's actions using conventional surgical tools, as well as generates feedback in order to ensure safety during the procedure. The reaction force from vasculature is estimated from motor current that drives the surgical tool. Calibration required for force estimation is based on bilevel optimization. Input shaping is used in conjunction with a cascaded controller to avoid large responses due to faster inputs and to track tool position. The design, realization, and testing of our system are presented. Results: The responses of the system in comparison with the dynamics model is similar vis-à-vis the same input commands. Any error in the position tracking is reduced by the cascaded controller. Phase-portrait analysis of the system showed that the system is stable. The reaction force estimation is validated against load cell measurements. The safety mechanism in the events of excessive reaction forces while interacting with vasculature is demonstrated. Conclusion and significance: Our system is a step toward intelligent robots that can assist surgeons during endovascular procedures by monitoring and alerting the surgeons regarding detrimental parameters. It arrests any unintended excursions of the surgical tools or surgeon's actions. This will also eliminate the need for surgeons to be in radiation environment.
A Robotic System with Force Feedback for Micro-Surgery

Abstract:A robotic system with force feedback for micro-surgery is developed (named “Microhand”). The system designed is based on a master-slave operation mode. The slave manipulators are designed by using macro-micro frames. The PHANToM Desktop developed by the SensaAble Technology Company is used as the master device. The interactive force/torque information of the slave manipulators and the surgical environments measured by the six-dimension force/torque sensors are fed back to the master device, so that the system is capable of providing force feedback to the surgeon during operations. The validity of the system has been proved through animal experiments of vas suturing a rabbit’s 3mm in diameter neck artery and its 1mm in diameter leg artery.

Abstract:The paper presents a new project of a surgery robotic system for minimally invasive. The novel system includes a human-arm-like robot manipulator and a catheter intervention manipulation system. A six-dimensional force sensor is fixed on the manipulator to drag the arm movement and accomplish compliance control to the arm. According to the surgery requirement, the catheter intervention manipulation system should deliver and rotate the catheter and help it insert into the target focus of the vessel to check, diagnose and therapy. Based on the actuality of catheter intervention manipulation, this paper describes the system's structure and master-slave manipulate mode, shows the realization way of quick detachment and force feedback, gives out the deduction of the promoting velocity and feedback force to pave the way for writing the control program.
