Abstract:Objective Scimitar syndrome is a rare congenital disease, and it is characterized almost exclusively by anomalous right pulmonary venous drainage into the inferior vena cava coexisting with pulmonary anomalies, abnormal systemic arterial supply of the affected lung, vascular ring or complex congenital heart disease. The anomalous pulmonary vein produces a unique radiographic sign as a tubular structure, paralleling the heart border in the shape of a Turkish sword (scimitar). This syndrome can be presented in infancy, or it may present later in childhood or adulthood. We report an exceedingly rare total anomalous pulmonary venous return (TAPVR) in a two-month-old baby with successful surgical treatment of an isolated left sided Scimitar vein in which all the left pulmonary veins were connected to the vena portae. The right pulmonary veins were connected into one collector that flows to the dilated sinus coronarius, coexisting with atrial septal defect, persistent left superior vena cava and arteria lusoria. Design and method The diagnosis was made with transthoracic echocardiography, computed tomography and selective pulmonary angiography and personalized 3D heart model derived from patient CT imaging data.To our knowledge, this is the first report of such a unique constellation as TAPVR. We surgically corrected the left scimitar vein with a side-to-side anastomosis to the left atrium, ligated and divided the distal part, and closed the ASD with an autologous pericardial patch. Sternal closure was delayed due to bleeding issues. The child remained stable in the ICU, the sternum was closed eventually, and the child was extubated. The patient recovered without complications. Results and conclusions To our knowledge, isolated left-sided scimitar vein connected to vena portae has not been previously reported. The treatment of patients with this cardiac anomaly is very complex. Therefore, a highly experienced team of specialists should be engaged to manage such abnormalities successfully.
