Abstract:OBJECTIVE:This study aimed to review the available literature on fertility-preserving treatment and pregnancy outcomes in patients with early-stage endometrial carcinoma who desired to preserve their fertility.DATA SOURCES:The PubMed database (1992-2012) was searched for the words "conservative "OR" fertility sparing "OR" fertility preserving" AND "endometrial neoplasms" (MeSH). All relevant articles in English and the relevant references were collected.STUDY SELECTION:Data from published articles about fertility-preserving treatment of endometrial cancer, including the response and recurrence rate of conservative treatment, strategies of infertility treatment, pregnancy, and obstetric outcomes, were selected. Data were mainly extracted from 41 studies, which are listed in the reference section of this review.RESULTS:Hormone therapy was the most common method used for early-stage endometrial carcinoma in patients who wished to preserve fertility. Sixty percent of the patients became pregnant after remission of the carcinoma. The percentage of patients who conceived in the assisted reproductive technology group was higher than that of the natural pregnancy group (80.0% vs. 43.2%, P < 0.01). A higher rate of preterm labor and multiple pregnancies was observed in the assisted reproductive technology group than that in the natural pregnancy group. The majority of pregnancies (71.4%) in the assisted reproductive technology group were achieved by in vitro fertilization-embryo transfer. The clinical pregnancy rate of transfer cycles in patients with endometrial carcinoma was 34.1%.CONCLUSIONS:Assisted reproductive technology is a good option in well-selected patients with early-stage endometrial carcinoma who have completed conservative treatment. In vitro fertilization-embryo transfer offers an opportunity to achieve an immediate pregnancy.
Concept and validation of a simple model of the intrathyroidal iodine kinetics

Abstract:Summary Aim of this study is the introduction and validation of a simple model of the intrathyroidal iodine kinetics, designed for optimizing radioiodine therapy planning and dose measurement in a routine clinical setting. Methods: The new model defines the intrathyroidal iodine kinetics as balance of the thyroidal iodine intake and -excretion, characterized by the two exponential equations At = A0 * (1-exp(-λ1t)) and At = A0 * (exp(-λ2t) -1), respectively. A0 describes the theoretically maximum iodine uptake when the thyroidal iodine excretion is ignored, λ1 and λ2 represent the constants characterizing the iodine intake and excretion, respectively. The thyroidal iodine content at the time t equals the sum of both functions, which is At = A0 * (exp(-λ2t)-exp(-λ1t)). In 25 patients with autonomous goiter / nodules (n = 18), Graves’ disease (n = 5), or endemic euthyroid goiter (n = 2), the iodine uptake in the thyroid during the radioiodine therapy as fraction of the applied activity was determined daily, with the remaining body covered by a lead shield. On average, 7.2 measurements were performed per patient (minimum 4, maximum 13). With these uptake values, individual regression curves were fitted using the above equation, and the difference between the actual measurements and the corresponding values of the regression curves was determined. Results: The average deviation of the 179 uptake values from the calculated points of the respective regression curves was only 1.4%. There was no significant difference between the three disease groups. The distribution of the relative deviations during the individual courses was constant, systematic errors were not detected. Conclusion: Our results suggest that the intrathyroidal iodine kinetics can be precisely described with the model At = A0 * (exp(-λ2t)-exp(-λ1t)). With only three measurements, the trend of the curve can be calculated, which allows to determine the total radioiodine storage in the thyroid.

Abstract:Objective: To report an unusual case of congenital cervical atresia and tubal factor infertility, achieving triplet pregnancy through transmyometrial embryo transfer (ET) resulting in the delivery of healthy twins after multifetal pregnancy reduction (MFPR). Design: Case report and literature review. Setting: University hospital. Intervention(s): Controlled ovarian hyperstimulation, oocyte retrieval, in vitro fertilization, transmyometrial ET, MFPR procedure for triplet pregnancy. Patient(s): A 33-year-old patient with endometriosis-associated and tubal factor infertility who underwent uterovaginal canalization surgery for congenital cervical atresia 9 years ago. Main Outcome Measure(s): Successful delivery after transmyometrial ET followed by MFPR. Result(s): A total of three IVF cycles and four procedures of transmyometrial ET were performed. In the third cycle a triplet pregnancy was achieved. MFPR was performed 35 days after embryos transfer, two healthy babies delivered at 31 weeks gestation. Fertility outcomes several years after urgent fertility preservation for patients with breast cancer

Abstract:Objective: To study the fertility outcomes of women who tried to conceive after breast cancer (BC) treatment and fertility preservation. Design: Retrospective observational, bicentric cohort study. Setting: University hospital. Patients: Patients with BC. Intervention: All patients who had undergone fertility preservation before BC treatment between January 2013 and July 2019 were included (n = 844). The endpoint date was March 1, 2022. Patients with missing data on pregnancy attempts after a cancer diagnosis (n = 195) were excluded from the pregnancy analysis. Main outcome measures: Cumulative incidences of pregnancy and live birth (LB) were calculated. For women who became pregnant, the time to conception was calculated between the first fertility preservation consultation and the estimated day of conception. For those who did not conceive, we considered the time between the first fertility preservation consultation and the endpoint date, or the date of patient death. A Cox regression model was used to study the predictive factors for pregnancy and LB. Results: Among the 649 patients with available data on pregnancy attempts after BC diagnosis, 255 (39.3% [35.5-43.2]) tried to conceive (median follow-up of 6.5 years). Overall, 135 (52.9% [46.6-59.2]) of these patients achieved a pregnancy, mainly through unassisted conception (79.3% [72.8-84.8]), and 99 reported an LB (representing 38.8% of patients who attempted conception). In our cohort, 48 months after the first fertility preservation consultation, the cumulative incidence of pregnancy was 33.1% ([27.6-37.9]). After adjustment for age, parity, type of chemotherapy administration, and endocrine therapy, only multiparity at diagnosis and absence of chemotherapy were positive predictive factors of pregnancy after cancer. Of the 793 patients who had vitrified oocytes and embryos, 68 used them (27% [21.3-32.5] of the patients who tried to conceive), resulting in 8 LBs (11.8% [5.2-21.9]). Women who used their cryopreserved oocytes and embryos were older at the first consultation of fertility preservation (hazard ratio 1.71 [1.42-2.21]), and chose more often to vitrify embryos (hazard ratio 1.76 [1.28-2.23]). Conclusion: Although pregnancy rates after fertility preservation for patients with BC are low, most conceptions are achieved without medical assistance. Our findings provide useful information to advise women on the different techniques of fertility preservation, their efficacy, and safety, as well as the relatively high chances of unassisted conception.
Pathways to motherhood: A single-center retrospective study on fertility preservation and reproductive outcomes in patients with breast cancer

Abstract:Background: Breast cancer treatments often have negative effects on fertility, which pose challenges among patients who want to be parents in the future. This study aimed to examine the efficacy of oocyte cryopreservation, embryo cryopreservation, and ovarian tissue cryopreservation in patients with breast cancer. Methods: This retrospective review evaluated 42 patients with breast cancer who underwent fertility preservation at our center from January 2012 to December 2022. This review encompassed the demographic characteristics of the patients, cancer stages, treatment details, and types of fertility preservation procedures and their outcomes. Results: The average age at disease diagnosis was 33.4 years. Approximately 90.4% of patients presented with early-stage cancer (≤2). Of 42 patients, 26 underwent oocyte cryopreservation; 17, embryo cryopreservation; and 2, ovarian tissue cryopreservation. Further, three patients received mixed treatment. The overall live birth rate was 63.2%. There are more live births in embryo cryopreservation group. The successful pregnancy group was significantly younger and had a remarkably higher quantity of preserved oocytes/embryos than the nonsuccessful pregnancy group. The oocyte and embryo utilization rates in cryopreservation were 7.69% and 52.94%, respectively. These findings underscored the importance of prompt, informed discussions about fertility preservation options. Conclusion: Fertility preservation in patients with breast cancer have promising reproductive outcomes, with embryo cryopreservation being particularly effective. Prompt counseling and individualized fertility preservation strategies are important for improving the likelihood of posttreatment pregnancy. Nevertheless, future research on the long-term psychological and emotional effects of different fertility preservation methods must be performed.

Abstract:PURPOSE We investigated time to pregnancy, efficacy and safety of fertility preservation, and assisted reproductive technologies (ARTs) in women with early hormone receptor–positive breast cancer (BC) desiring future pregnancy. PATIENTS AND METHODS POSITIVE is an international, single-arm, prospective trial, in which 518 women temporarily interrupted adjuvant endocrine therapy to attempt pregnancy. We evaluated menstruation recovery and factors associated with time to pregnancy and investigated if ART use was associated with achieving pregnancy. The cumulative incidence of BC-free interval (BCFI) events was estimated according to the use of ovarian stimulation at diagnosis. The median follow-up was 41 months. RESULTS Two hundred seventy-three patients (53%) reported amenorrhea at enrollment, of whom 94% resumed menses within 12 months. Among 497 patients evaluable for pregnancy, 368 (74%) reported at least one pregnancy. Young age was the main factor associated with shorter time to pregnancy with cumulative incidences of pregnancy by 1 year of 63.5%, 54.3%, and 37.7% for patients age <35, 35-39, and 40-42 years, respectively. One hundred and seventy-nine patients (36%) had embryo/oocyte cryopreservation at diagnosis, of whom 68 reported embryo transfer after enrollment. Cryopreserved embryo transfer was the only ART associated with higher chance of pregnancy (odds ratio, 2.41 [95% CI, 1.75 to 4.95]). The cumulative incidence of BCFI events at 3 years was similar for women who underwent ovarian stimulation for cryopreservation at diagnosis, 9.7% (95% CI, 6.0 to 15.4), compared with those who did not, 8.7% (95% CI, 6.0 to 12.5). CONCLUSION In POSITIVE, fertility preservation using ovarian stimulation was not associated with short-term detrimental impact on cancer prognosis. Pregnancy rates were highest among those who underwent embryo/oocyte cryopreservation followed by embryo transfer.

Abstract:There is considerable interest in transplanting stem cells or progenitors into the injured nervous system and enhancing their differentiation into mature, integrated, functional neurons. Little is known, however, about what intrinsic or extrinsic signals control the integration of differentiated neurons, either during development or in the adult. Here we ask whether purified, postmitotic, differentiated retinal ganglion cells (RGCs) directly isolated from rat retina or derived from in vitro-differentiated retinal progenitor cells can survive, migrate, extend neurites, and form morphologic synapses in a host retina, in vivo and ex vivo. We found that acutely purified primary and in vitro-differentiated RGCs survive transplantation and migrate into deeper retinal layers, including into their normal environment, the ganglion cell layer (GCL). Transplanted RGCs from a wide range of developmental ages, but not from adults, were capable of extending lengthy neurites in the normal and injured adult rat retina ex vivo and to a lesser degree after transplantation in vivo. We have also demonstrated that RGCs may be differentiated and purified from retinal precursor cultures and that they share many of the same cell biological properties as primary RGCs. We have established that progenitor-derived RGCs have similar capacity for integration as developing primary RGCs but appear to form a lower number of presynaptic punctae. This work provides insight for further understanding of the integration of developing RGCs into their normal environment and following injury.

Abstract:(1) Background: Currently, an increasing number of women postpone pregnancy beyond the age of 35. Gynecological cancers affect a significant proportion of women of reproductive age, necessitating the development of fertility preservation methods to fulfill family planning. Consequently, providing treatment options that preserve fertility in women diagnosed with gynecological cancers has become a crucial component of care for survivors. (2) Methods: We conducted an extensive search of relevant scientific publications in PubMed and Embase databases and performed a narrative review, including high-quality peer-reviewed research on fertility after being treated for gynecologic cancers, reporting pregnancy rates, birth rates, and pregnancy outcomes in cancer survivors as well as therapeutic options which partially preserve fertility and methods for obtaining a pregnancy in survivors. (3) Discussion: The medicine practiced today is focused on both treating the neoplasm and preserving the quality of life of the patients, with fertility preservation being an important element of this quality. This leads to an improved quality of life, allowing these women to become mothers even in the seemingly adverse circumstances posed by such a pathology. However, although there are guidelines on female fertility preservation in the context of neoplasms, an analysis shows that physicians do not routinely consider it and do not discuss these options with their patients. (4) Conclusions: Advancements in medicine have led to a better understanding and management of gynecological neoplasms, resulting in increased survival rates. Once the battle against these neoplasms is won, the issue of preserving the quality of life for these women arises, with fertility preservation being an important aspect for women who have not yet fulfilled their family planning desires at the time of diagnosis. It is important for patients to be informed about the available options for fertility preservation and to be encouraged to make informed decisions in collaboration with their medical team. Standardized recommendations for onco-fertility into guidelines should be taken into consideration in the future.

We have described such a patient whose initial presenting symptom was sudden hearing loss. She had no progression of the hearing deficit with long-term anticoagulant therapy.

Abstract:Objective To report pregnancy and live birth resulting from intracytoplasmic sperm injection of ex vivo retrieved mature oocytes from a woman with bilateral ovarian carcinoma. Design Case report. Subject(s) A 34-year-old nulliparous woman with bilateral ovarian tumor, with a risk of malignancy at 96.1% according to IOTA-ADNEX, who desired fertility preservation before definitive surgical treatment. Intervention(s) Cryopreservation of ex vivo retrieved mature (MII) oocytes followed by fertilization with donor sperm, and embryo transfer to a gestational carrier. Main outcome measure(s) Fertility preservation. Results After controlled ovarian stimulation, 12 MII oocytes were retrieved from oophorectomized specimens, and vitrified. ICSI with donor sperm was performed in remission, resulting in 9 cleavage-stage embryos, two of which were transferred to a gestational carrier, resulting in a normal healthy singleton pregnancy, and the live birth of a healthy baby. Conclusion (s): Ex vivo oocyte retrieval after oophorectomy may be a safe alternative to standard oocyte retrieval for fertility preservation in women with ovarian malignancies.

Abstract:Probable pathways of food contamination with environmental chemical mutagens and factors responsible for the formation of mutagens in food during cooking are reviewed, and natural and synthetic food components with mutagenic properties are described. The data discussed provide evidence for the expediency of further analyzing mutagenicity of various food components and developing chemical analytical methods for their detection and identification. Antimutagenic compounds present in food are characterized. The adequacy of methods for estimating the activity of these compounds and perspectives for their use as antimutagenic chemopreventive agents are discussed. Emphasis is placed on the necessity of being extremely cautious in employing food antimutagens because of the probable inversion of their effects. Several examples are provided to demonstrate the possibility of reducing mutagenicity of prepared food by modifying food processing technology.

The clinical management of such patients is often disappointing. It is rare to have two consecutive successful pregnancies. We present a child-bearing-age woman who underwent fertility preservation therapy due to endometrial carcinoma. Following fertility preservation therapy, she underwent in vitro fertilization and embryo transfer. After receiving her first fresh embryo transfer, she successfully conceived and gave birth to a healthy child. Two years after the first embryo transfer and regular follow-up, she had another frozen embryo transfer of two cleavage embryos and successfully gave birth to another healthy baby. After the delivery of her second child, she underwent surgical treatment for endometrial carcinoma. For endometrial carcinoma patients who intend to preserve fertility, high-quality long-term follow-up and personalized treatment are necessary. Plain Language Summary: In this case report, we share the story of one young woman who had endometrial cancer but desired to have children. She received fertility-sparing treatment and in vitro fertilization to increase her chances of conceiving. She successfully delivered a healthy child after the first embryo transfer. Two years later, she had another healthy child through a second frozen embryo transfer. Rigorous monitoring showed no cancer recurrence throughout the entire treatment. There are currently few reported cases of a patient with endometrial cancer successfully and safely giving birth twice through assisted reproductive technology. This case report emphasizes that, with personalized treatment and monitoring, endometrial cancer patients can have multiple pregnancies safely. In summary, this case report brings hope to young women with early-stage endometrial cancer who aspire to become mothers. With the right support, they can overcome the challenges of cancer and have their own babies. Keywords: endometrial carcinoma, fertility preservation, pregnancy outcome, in vitro fertilization-embryo transfer Endometrial carcinoma (EC) is a malignancy that originates from the endometrium, ranking as the fifth most prevalent cancer in females. 1 The standard treatment for EC is surgery, which includes total extra-fascial hysterectomy, bilateral salpingo-oophorectomy, and pelvic and para-aortic lymph node dissection. 2 Patients with early-stage EC can usually achieve satisfactory survival outcomes after surgery. The 5-year survival rate for these patients is up to 96%. 3 Approximately 14–25% of EC patients are premenopausal, and 4–7% of them are under 40 years of age. 4,5 These patients often express a strong desire for fertility preservation. Fortunately, patients diagnosed with early-stage and well-differentiated EC usually have a favorable prognosis. However, for women of childbearing age who wish to preserve their fertility, this definitive approach may not be an optimal option. Review of fertility preservation issues for young women with breast cancer

Abstract:It is well documented that cancer treatment may temporarily or permanently impair childbearing potential of young women with breast cancer. Given that many patients have not initiated or completed their families when diagnosed, fertility issues are of utmost importance in this clinical population. This review addresses the importance of incorporating fertility issues into the clinical care of young breast cancer patients, focusing on recent knowledge and counselling practices about fertility-related issues and the complexity of fertility-related decisions in this population. Multiple studies report cancer-related infertility may have serious psychological consequences and reduce Quality of Life for some patients. To guide health care providers and patients regarding appropriate, safe, and cost-effective fertility care for women who desire biological children, several professional organizations have developed clinical practice guidelines. However, the extent to which health professionals use these guidelines and provide timely and appropriate fertility-related information to patients is questionable. Fertility should not be neglected by health care providers and a multidisciplinary team is needed to target fertility issues at diagnosis and into survivorship care, providing timely, clear information that includes support, resources, and appropriate referral to fertility specialists. Analysis of Pregnancy-Associated Factors after Fertility-Sparing Therapy in Young Women with Early Stage Endometrial Cancer or Atypical Endometrial Hyperplasia

Abstract:Abstract Background Fertility-sparing therapy is an alternative conservative treatment for patients with early stage endometrioid cancer or atypical endometrial hyperplasia. In this study, we investigated pregnancy outcomes and pregnancy-associated factors in young patients receiving hormonal therapy. Methods We retrospectively analyzed 68 patients who attempted to conceive after fertility-sparing therapy and achieving complete remission (CR). They were divided into a pregnancy group and a non-pregnancy group. A Cox proportional hazard regression model was applied for univariate and multivariate analysis to determine factors associated with pregnancy. Kaplan–Meier analysis, combined with the log-rank test, was used to calculate a patient’s pregnancy probability and the distribution of recurrence-free survival (RFS). Results A total of 36 patients became pregnant with 47 pregnancies. Univariate and multivariate Cox analysis revealed that several factors were associated with pregnancy, including BMI at the time of pregnancy permission, the time to CR, prolonged treatment time, the number of hysteroscopy procedures, the endometrium thickness after CR, and relapse before pregnancy. The mean RFS of patients who achieved pregnancy, and those who did not, was 27.6 months and 14.8 months, respectively (P = 0.002). No significant difference was detected in terms of cumulative RFS when compared between assisted reproductive technology (ART) cases and those involving natural conception (NC) (P = 0.707). Conclusions Normal BMI, a shorter time to CR, a prolonged three-month treatment, fewer hysteroscopy procedures, and a thicker endometrium may be positive indicators for successful pregnancies, while relapse before pregnancy may have a negative effect on conception. Moreover, a successful pregnancy protects the endometrium while ART does not increase the risk of recurrence.
Fertility preservation and infertility treatment in breast cancer patients

Abstract:About 12% of breast cancer cases occur before the age of 35 years. Thus, young patients may consider preservation of ovarian function during cytotoxic therapy and/or childbearing after treatment. Besides the exposure to cytotoxic agents over several months, a significant number of patients will need sequential antihormonal therapy for years. Thus, even young patients at diagnosis may have to postpone pregnancy for years. At that time, the probability of pregnancy may per se be considerably reduced. Young patients per se carry a worse prognosis compared to their postmenopausal counterparts. Ovaries may harbor viable tumor cells. Premenopausal patients have a higher prevalence of BRCA gene mutation than older patients. Thus, even if the ovaries are preserved, the risk of developing cancer in the ovary may be high. Normal menstrual cyclicity after chemotherapy/antihormonal therapy does not preclude premature ovarian failure. After antineoplastic therapy, it is recommended to wait at least for two months before the evaluation of the ovarian function. The most reliable parameter to assess the ovarian reserve is the anti-muellerian factor (AMH; Muellerian Inhibiting Factor, MIF). Four randomized studies have investigated the protective effect of the GnRH analogue goserelin on ovarian function. Due to the inconsistent results observed, the concurrent administration of GnRH analogues with cytostatic chemotherapy now has to be considered experimental and cannot be recommended. Emergency in vitro fertilization may be performed between breast cancer diagnosis and the start of chemotherapy in selected cases. Extracorporal in vitro fertilization may be performed and the resultant embryo cryopreserved to preserve fertility.

Abstract:Background: to prove the effectivity of fertility-sparing procedures in early-stage ovarian cancer by assessing pregnancy rates and obstetrical outcomes. Methods: we performed a retrospective multicenter study among 55 Spanish hospitals, collecting patients from 18 to 40 years old with diagnosis of early-stage ovarian cancer, epithelial (EOC) or non-epithelial (non-EOC), from January 2010 to December 2019. Data on the use of assisted reproductive techniques, pregnancy attempts and obstetrical outcomes were collected. Results: a total of 150 patients met inclusion criteria, 70 (46.6%) EOC and 80 (53.4%) non-EOC. Pregnancy attempts were reported in 51 (34%) patients, with 42 (28%) patients carrying the pregnancy to term. Among them, 30 (71.4%) underwent surgery alone and 12 (28.6%) had additional postoperative chemotherapy. A total of 32 (76.1% patients) had spontaneous pregnancies and 10 (23.9%) required in vitro fertilization. There was only one (2.4%) complication reported. Vaginal delivery was reported in twenty-nine (69%) patients and cesarean section in five (11.9%) patients. Conclusions: fertility-sparing management for ovarian cancer seems to be an option with proven good pregnancy rates and low complications. The selection of patients must consider strict criteria in order to maintain a good prognosis.

Abstract:Childhood cancer incidence, especially in high-income countries, has led to a focus on preserving fertility in this vulnerable population. The common treatments, such as radiation and certain chemotherapeutic agents, though effective, pose a risk to fertility. For adult women, established techniques like embryo and egg freezing are standard, requiring ovarian stimulation. However, for prepubescent girls, ovarian tissue freezing has become the primary option, eliminating the need for hormonal preparation. This review describes the beginning, evolution, and current situation of the fertility preservation options for this young population. A total of 75 studies were included, covering the steps in the current fertility preservation protocols: (i) ovarian tissue extraction, (ii) the freezing method, and (iii) thawing and transplantation. Cryopreservation and the subsequent transplantation of ovarian tissue have resulted in successful fertility restoration, with over 200 recorded live births, including cases involving ovarian tissue cryopreserved from prepubescent girls. Despite promising results, challenges persist, such as follicular loss during transplantation, which is attributed to ischemic and oxidative damage. Optimizing ovarian tissue-freezing processes and exploring alternatives to transplantation, like in vitro systems for follicles to establish maturation, are essential to mitigating associated risks. Further research is required in fertility preservation techniques to enhance clinical outcomes in the future. Ovarian tissue cryopreservation appears to be a method with specific benefits, indications, and risks, which can be an important tool in terms of preserving fertility in younger women.

Abstract:Abstract Background: More than 12,000 new diagnoses of breast cancer occur annually in women <40 years old and most will become survivors. It is recommended that young women with breast cancer be counseled about fertility preservation (FP) options prior to initiation of systemic therapy if they are interested in having biologic children in the future. Decisions to pursue FP are multifactorial and current data suggest only a small proportion of women pursue FP before breast cancer treatment. This study examines the decisions and outcomes of women who pursue FP before treatment and family building after therapy. Methods: An IRB approved web-based cross-sectional survey examining decisions about FP, factors influencing decisions, and post-decision regret was administered to young (age 18-45) breast cancer survivors with stage I-III breast cancer who were either English or Spanish speaking. All women received counseling by a MSK Fertility Nurse Specialist prior to initiating cancer treatment at MSKCC in 2009-2017. Clinical data was also extracted from the medical record .Results: Our sample consisted of 211 women with a mean and median age of 34 (age range was 20-45) and 82% had ER+ disease, 19% had HER-2+ disease and 12% had triple negative breast cancer. At the time of diagnosis, 46 women (22%) had children and 173 women (61%) were married or living with a partner. 151 (72%) of study participants wanted to have (more) children, 48 (23%) were unsure and 12 (6%) did not want additional children. Prior to treatment, 172 (82%) women met with a reproductive endocrinologist (RE), and 122 (58%) underwent FP: 73 froze eggs and 49 froze embryos. After treatment, 28 (13%) women met with a RE, and 13 underwent FP: 9 froze eggs and 4 froze embryos. 26 (12%) patients opted to just use a GnRH agonist during chemotherapy. For women who froze eggs, the average number obtained was 14 pre-treatment and 18 post-treatment. Of those who froze embryos, the average number obtained was 7 both pre- and post-treatment. Concern with financial burden was noted in 58% of women. Insurance did not cover any FP treatment for 56/122 (46%) of women. 32% (39/122) of women paid $10,000 or more for their FP. Women reported that the most important factor that contributed to their FP decision was the ability to feel hopeful about their future followed by concern that they would have regrets if they did not undergo FP. Additional factors that contributed to the FP decision were the desire to have biologic children, feeling overwhelmed by the cancer diagnosis, the cost of freezing eggs or embryos and concern about taking hormone medication. Reflecting back 176 women (83%) believe they made the right decision regarding FP. After completing primary breast cancer treatment, 61 women (29%) opted to try to build their family and 42 had children: 35 gave birth and 7 used a surrogate. Currently 3 women are pregnant, and 10 are pursuing pregnancy. 19 (31%) women conceived without fertility treatment and 22 (36%) women used or are currently pursuing assisted reproduction. 1 woman used a donor egg and 15 successfully used frozen eggs/embryos harvested prior to treatment. Delay of endocrine therapy to pursue pregnancy was uncommon, reported by only 6 women. However, 23 women interrupted endocrine therapy to conceive. Conclusions: The majority of women who attempted family building after treatment were able to have children. Many of these women used eggs/embryos frozen before breast cancer treatment either by getting pregnant themselves or via surrogacy. These data stress the importance of early discussion with a fertility nurse specialist regarding risks and preservation options followed by prompt referral to a reproductive endocrinologist. In addition, FP helped women to feel hopeful about their future and the majority of women believe they made the right decision. Abstract:Abstract Introduction An increasing number of young patients with early-stage endometrial cancer are opting for fertility-sparing treatment, and they have achieved reassuring rates of complete remission, with some women achieving pregnancy. However, surgical treatment is still recommended for patients with advanced endometrial cancer. We reported a woman with IVb endometrial cancer (EC) who reached complete remission (CR) and gained a live birth successfully with the help of in vitro fertilization and embryo transfer (IVF-ET). Case report A 30-year-old woman found to have stage IVb EC after hysteroscopy, laparoscopy, and pathological examination. After a combination of chemotherapy and progesterone therapy, the woman achieved complete remission. The woman became pregnant with IVF-ET and got a full-term live birth. In addition, there was no recurrence after 19 months of observation. The molecular classification of POLE and the combination of chemotherapy and progesterone therapy may be associated with a good prognosis of this patient. The use of progestin-primed ovarian stimulation (PPOS) and the combination of letrozole and levonorgestrel-releasing intrauterine device (LNG-IUD) during ovulation induction seemed to be safe for the woman Conclusion This is the first report on fertility-sparing treatment and live birth for advanced EC. IVF-ET is feasible for women with EC who preserve reproductive function. The progestin-primed ovarian stimulation combined with levonorgestrel-releasing intrauterine device and letrozole seems to be safe for these women. Patients still need to be closely watched for tumor recurrence.
