We have described such a patient whose initial presenting symptom was sudden hearing loss. She had no progression of the hearing deficit with long-term anticoagulant therapy.

Abstract:Electrolyte imbalance in leukemia can be due to either organ infiltration and cell death or to a side effect of cytostatic drugs. From the wide variety of these disturbances seen in acute leukemias in childhood, the excess of potassium is most dangerous. Further electrolyte changes, which are however less evident, are hyperphosphataemia, hyperphosphaturia, and hypocalcaemia. The destruction of a large amount of cells during aggressive induction therapy can boost the electrolyte imbalance and therefore lead to renal failure. Such situations are demonstrated in two cases. Following Vincristine and Cyclophosphamide administration, electrolyte changes such as acute or prolonged decrease of sodium in the serum and urinary loss of sodium are seen frequently. Based on the data from 20 patients with acute lymphoblastic leukemias we describe the dynamics of this process. These changes are probably caused by the syndrome of inadequate ADH-secretion. The clinical importance of these findings are discussed and procedures for improving therapy are set out.
Taxanes in Combination with Platinum Derivatives for the Treatment of Ovarian Cancer During Pregnancy: A Literature Review

Abstract:Ovarian cancer is one of the most common types of solid carcinoma diagnosed during pregnancy. Taxane plus a platinum derivative is a combination therapy that is predominantly used in the treatment of ovarian cancer in non-pregnant women. Pregnancy adds various complexities to a course of treatment. In pregnant patients diagnosed with cancer during the first trimester, the risks of fetal malformations and fetal loss increase following the administration of cytotoxic drugs, and this is higher with multi-agent vs. single-agent chemotherapy (similar to 25 vs. 10%). Exposure during the second and third trimester has little influence on teratogenic effects but increases the risk of intrauterine growth retardation, prematurity, low birth weight, and bone marrow toxicity. The present study aimed to review the maternal and fetal safety of treatment with taxane plus platinum derivatives for ovarian cancer during pregnancy. Relevant literature was retrieved from the Embase and PubMed databases using the search terms "ovarian cancer", "pregnancy", "taxane", "paclitaxel", "docetaxel", "platinum", "cisplatin", and "carboplatin". All available data up until September 2016 was synthesized, with no language restrictions. A total of 11 articles (including 13 pregnancies and 14 newborns) were retrieved that reported on the use of standard-dose taxane and platinum chemotherapy, including 9 cases treated with paclitaxel and carboplatin, 3 cases treated with paclitaxel and cisplatin, and 1 case treated with docetaxel and cisplatin. In 13 of the 14 (92.9%) births included, a healthy neonate was born, with follow-up ranging from 2 to 160 months. The average weight of the neonates at the time of delivery was 2,442.1 g. In 7 of 9 the case reports that provided survival data, the mother was alive and disease-free at the end of follow-up (ranging from 2 to 40 months). In conclusion, combination therapy with taxanes and a platinum derivative may play a significant role in the management of pregnant patients with ovarian cancer during the second and third trimester. Exposure to this combination of agents during the second and third trimester does not appear to have a significant bearing on fetal mortality and abortion.

Abstract:Background: Pregnancy-associated breast cancer (PABC), with an incidence rate from 1:3,000 to 1:10,000 deliveries, is the most frequent cancer during pregnancy. PABC appropriate management must take into consideration both the maternal oncological safety and the fetal health, thus posing a challenge for the mother, the baby, and the clinicians. The treatment should adhere as closely as possible to the breast cancer (BC) guidelines. Therefore, surgery is a mainstay, and, when mastectomy is required, breast reconstruction (BR) is a topic of debate. To minimize the risks to the baby, most surgeons postpone BR to delivery. However, a delayed breast reconstruction (DBR) could affect the outcome. In the present case, we report cesarean section concurrent with mastectomy and immediate breast reconstruction (IBR). Methods: A 37-year-old patient, at the 36th week of pregnancy with PABC, underwent simultaneous cesarean delivery, nipple-sparing mastectomy, and IBR. To minimize risks for the newborn, cesarean was firstly performed under spinal anesthesia. Immediately after, breast surgery, including mastectomy and IBR, was performed under general anesthesia. Partial submuscular IBR with an acellular porcine dermal matrix concluded the surgical procedure. Lactation was inhibited, and adjuvant chemotherapy and hormone therapy were administered to the patient. Results: In a single surgical session, cesarean delivery, subcutaneous mastectomy, axillary dissection, and IBR were successfully carried out. No early or late postoperative complications were reported for both the patient and the newborn. Histopathological investigation reported a multifocal and multicentric infiltrating ductal carcinoma. After a 6-year follow-up, the patient is alive and well. Conclusion: To the best of our knowledge, this is the first reported case of concomitant cesarean delivery, PABC mastectomy, axillary dissection, and IBR. This surgical strategy allowed PABC treatment by the BC guideline, minimizing the newborn's disadvantage and permitting, at the same time, the best final BR outcome.

Abstract:Background: pregnancy-associated breast cancer (PABC) affects one in 3000 pregnancies, often presenting with aggressive features. Methods: We retrospectively evaluated a cohort of 282 young BC patients (≤45 years old) treated between 1995 and 2019, dividing them into three groups: nulliparous women, women with PABC (diagnosed within 2 years since last pregnancy) and women with BC diagnosed > 2 years since last pregnancy. This last group was further stratified according to the time between pregnancy and BC. The analysis encompassed histological factors (tumor size, histotype, grading, nodal involvement, multifocality, lympho-vascular invasion, hormone receptor expression, Ki-67 index, and HER2 expression), type of surgery and recurrence. Results: Age at diagnosis was younger in nulliparous than in parous women (p < 0.001). No significant differences were noticed regarding histological characteristics and recurrences. At univariate analysis, nodal involvement (OR = 2.4; p < 0.0001), high tumor grade (OR = 2.6; p = 0.01), and lympho-vascular invasion (OR = 2.3; p < 0.05), but not pregnancy (OR = 0.8; p = 0.30), influenced DFS negatively. Multivariate analysis confirmed nodal involvement as the only negative independent prognostic factor for a worse DFS (OR = 2.4; p = 0.0001). Conclusions: in our experience, pregnancy is not an independent adverse prognostic factor for BC DFS.

Abstract:This paper discusses a fundamental problem in natural language generation: how to organize the content of a text in a coherent and natural way. In this research, we set out to determine the semantic content and the rhetorical structure of texts and to develop heuristics to perform this process automatically within a text generation framework. The study was performed on a specific language and textual genre: French instructional texts. From a corpus analysis of these texts, we determined nine senses typically communicated in instructional texts and seven rhetorical relations used to present these senses. From this analysis, we then developed a set of presentation heuristics that determine how the senses to be communicated should be organized rhetorically in order to create a coherent and natural text. The heuristics are based on five types of constraints: conceptual, semantic, rhetorical, pragmatic, and intentional constraints. To verify the heuristics, we developed the spin natural language generation system, which performs all steps of text generation but focuses on the determination of the content and the rhetorical structure of the text.

Abstract:Breast cancer is one of the most common invasive cancers in women and is the second cancer frequently occurring worldwide of newly-diagnosed cancers. [ 1 , 2 ] Pregnancy-associated breast cancer (PABC) is defined as newly diagnosed breast cancer during pregnancy or within 1 year of delivery. [ 3 ] Although PABC is relatively rare, its incidence has increased in recent years. The impact of systemic therapy on the fetus presents a particular challenge to clinicians. Compared with common breast cancer, PABC is characterized by a low rate of early detection and poor prognosis. PABC is usually diagnosed in the middle to late stages, [ 4 ] of which the onset can be extremely detrimental to the fetus and the mother. In order to detect and diagnose PABC in time and standardize the treatment, we retrospectively analyzed 1 case of PABC admitted to our hospital, and the case report is as follows.

Abstract:Cancer diagnosis and treatment in pregnant women is a challenging situation. A multidisciplinary network of specialists is required to guide both, the expecting mother and the unborn child through the diagnostic workup and the cytotoxic therapy, by balancing the respective risks and benefits. Tumor entity, stage, biology and gestational week at diagnosis determine the appropriate approach. As premature delivery emerged as one of the main risk factors for adverse long-term outcome of the progeny, it should be avoided, if reasonable from the oncological perspective. This article offers a comprehensive review with respect to the various aspects of cancer in pregnancy.

Abstract:Background: Pregnancy-associated breast cancer (PABC) is a special type of breast cancer that occurs during pregnancy and within 1 year after childbirth. With the rapid social development and the adjustment of reproductive policies in China, the average age of females at first childbirth is increasing, which is expected to lead to an increase in the incidence of PABC. This study aimed to accumulate clinical experience and to investigate and summarize the prevalence, diagnosis, and treatment of PABC based on large multicenter samples in China. Methods: According to the Chinese Society of Breast Surgery, a total of 164 patients with PABC in 27 hospitals from January 2016 to December 2018 were identified. The pregnancy status, clinicopathological features, comprehensive treatment methods, and outcomes were retrospectively analyzed. Survival curves were plotted using the Kaplan-Meier method. Results: A total of 164 patients of PABC accounted for 0.30% of the total number of cases in the same period; of which, 83 patients were diagnosed during pregnancy and 81 patients during lactation. The median age of PABC was 33 years (24-47 years). Stage I patients accounted for 9.1% (15/164), stage II 54.9% (90/164), stage III 24.4% (40/164), and stage IV 2.4% (4/164). About 9.1% (15/164) of patients were luminal A. Luminal B patients accounted the most (43.3% [71/164]). About 15.2% (25/164) of patients were human epidermal growth factor receptor 2 (Her-2) overexpression and 18.9% (31/164) of patients were triple-negative breast cancer. For pregnancy breast cancer, 36.1% (30/83) of patients received direct surgery and 20.5% (17/83) received chemotherapy during pregnancy. About 31.3% (26/83) chose abortion or induction of labor. The median follow-up time was 36 months (3-59 months); 11.0% (18/164) patients had local recurrence or distant metastasis and 3.0% (5/164) died. Conclusions: It is safe and feasible to standardize surgery and chemotherapy for PABC.

Abstract:Objective: Colorectal cancer (CRC) during pregnancy is a rare occurrence, with a reported incidence of 0.8 cases per 100,000 pregnancies. Managing CRC during pregnancy poses substantial challenges for clinicians: the diagnosis is often complicated and delayed due to symptom overlap with pregnancy-related manifestations, and medical imaging is constrained by safety concerns for the foetus. Methods: This article presents two cases of advanced CRC diagnosed and managed during pregnancy. Additionally, we conducted a systematic review of the literature to assess diagnostic and prognostic factors involved in CRC in pregnant individuals. The systematic review, with pre-registration and approval through Prospero, involved an extensive search of medical databases (Pubmed, Web of Science, Scopus and Scholar) and statistical analysis using t-test for continuous variables and chi square for dichotomous variables. Results: A total of 1058 studies were identified. After applying exclusion criteria, sixty-six studies were included. Women whose initial symptoms were severe abdominal pain not responsive to common medical treatments and constipation (acute abdomen) had a mean gestational age at delivery lower than those who presented with paucisymptomatic onset. In our study groups, women who underwent chemotherapy during pregnancy had a higher mean gestational age at delivery and did not experience worse neonatal outcomes compared to those who did not undergo chemotherapy. Conclusions: CRC during pregnancy poses unique diagnostic and therapeutic challenges. Collaborative efforts among various medical disciplines are essential to manage CRC during pregnancy.

Abstract:A 13-year-old boy had a seven-year history of typical otalgic glossopharyngeal neuralgia, which was relatively unresponsive to medical treatment. Diagnosis was confounded by his young age and the previously unknown onset of glossopharyngeal neuralgia before the age of 16 years. A trigger point was present in the right tonsillar area, anesthesia of which abolished his pain entirely, albeit temporarily. Cervical section of the right glossopharyngeal nerve and its tympanic branch resulted in complete relief to date (13 months).

Abstract:Objective: To investigate the principles of diagnosis and treatment of breast cancer during pregnancy. Methods: Clinical data of patients with breast cancer during pregnancy admitted to Obstetrics and Gynecology Hospital of Fudan University between January 2012 to July 2017 were analyzed retrospectively. A total of 17 patients were diagnosed with breast cancer in pregnancy, the median age was 32 years (range from 25 to 45 years old), pathological staging revealed 2 patient with stage 0, 1 with stage Ⅱa, 7 with stage Ⅱb, 1 with stage Ⅲa, 2 with stage Ⅲc, 4 with stage Ⅳ. Results: Thirteen patients received surgical treatment in pregnancy, the gestational age at surgery was (27.7±4.6) weeks; 2 patients with ductal carcinoma in situ received mastectomy, 11 patients with breast cancer underwent modified radical mastectomy. In patients undergoing surgery during pregnancy, no prophylactic contractions were used in 4 patients who had been treated earlier, there were 2 patients with frequent contractions within 24 hours after operation in these patients. Follow-up 9 patients were given oral nifedipine to prevent contractions, no obvious contractions occurred after the operation. Seven patients received chemotherapy during pregnancy; the chemotherapy of 4 cases of triple negative breast cancer was weekly paclitaxel sequential epirubicin and cyclophosphamide, the chemotherapy of the other three patients was docetaxel sequential epirubicin and cyclophosphamide. Fifteen patients underwent cesarean section to terminate pregnancy, 2 patients underwent spontaneous labor. The gestational age of birth was (36.9 ±1.3) weeks. Less than 35 weeks of termination of pregnancy occurred in one patient, the fetus was delivered to the neonatal intensive care unit due to neonatal respiratory distress syndrome, and suffered from congenital dysaudia. The prognosis of the other 16 survived infants was good. The median follow-up time was 10 months (range from 4 to 27) months, in 13 patients of stage 0 to Ⅲc, one patient were diagnosed with bone metastasis at 12 months after surgery, the remaining 12 patients had no disease progression, the progression free survival rate was 12/13, the overall survival rate was 13/13. Among the 4 patients with stage Ⅳ, one died in 7 months after delivery, one had new liver metastasis in 8 months after delivery. The remaining 2 patients were in stable condition. Conclusions: Breast cancer in pregnancy can be treated effectively, multidisciplinary cooperation and detailed assessment of maternal-fetal risks and benefits are necessary. Chemotherapy during pregnancy is safe for maternal-fetal, but it needed a large sample of clinical studies and long-term follow-up. The neonatal outcome was associated with gestational age, and therefore premature delivery was avoided as much as possible during treatment.

Abstract:As the pregnant patient with breast cancer is in a special physiological period, both the efficacy of mother and the safety of developing fetus should be considered during the whole process of diagnosis and treatment. It is particularly important for multidisciplinary teams including breast, obstetrics and nursing departments to make a secure and effective individualized plan for those in different gestational week and different stages of breast cancer development. Pregnancy risk assessment and whole-process multidisciplinary case management mode for breast cancer during pregnancy are helpful for the early detection of abnormal health status of pregnant women and fetuses, enabling rapid and efficient treatment, reducing the occurrence of adverse medical events, and maximizing the safety of pregnant women and fetuses. Obstetricians should pay attention to the chief complaints of pregnant women and conduct regular breast ultrasound examinations. Once anything suspicious is found, breast surgeons need to take charge of a multidisciplinary discussion. Not only should the multidisciplinary collaborative outpatient clinic determine the treatment plan for breast cancer during pregnancy, but also the concept of multidisciplinary collaboration should be incorporated into the follow-up treatment process, including active surgical treatment, selection of neoadjuvant chemotherapy and adjuvant chemotherapy, avoidance of endocrine therapy, targeted therapy and radiotherapy, and adherence to multidisciplinary follow-up, etc. Multidisciplinary case management of breast cancer during pregnancy is necessary and feasible, and more prospective clinical studies need to be carried out to help improve clinical diagnosis and treatment strategies.

Abstract:Objective To discuss the diagnosis,treatment and prognosis of breast cancer during pregnancy.Methods To review three patients with breast cancer treated in peole's hospital Peking University from 1994 to 2004.Results The breast cancer is very rare in pregnant patients,the incidence was 0.020%,and the mean age was 30.The main symptoms of the three breast cancer patients was breast mass,the final pathologic diagnose was made through fine needle biopsy or lumpectomy.The diagnosis of two patients was delayed more than 9 months.Two patients were at stage Ⅱ,another was stage Ⅳ.Two breast cancer patients with stage Ⅱaccepted modified radical mastectomy as first line method after abandoning the fetus.And adjuvant chemotherapy was regularly used,they were followed up for 8 year 6 months and 10 year 8months respectively.Another breast cancer patient with stage Ⅳ only undertood lumpectomy after cesarean section,although adjuvant chemotherapy was used she died after 15 months.Conclusions The clinical manifestations and diagnostic procedure in breast cancer patients associated with pregnancy was the same as ono-pregnant patients.But they were usually diagnosed delayed.Treatment principle was according to the tumor stage,gestational age and the desire for fetus.Pregnancy was not a prognostic factor for breast cancer.

Abstract:Objective To analyze the clinical and pathological characteristics, treatment and prognosis of pregnancy associated breast cancer (PABC). Method Clinical data of 33 PABC patients were reviewed, of which the treatment, pathological characteristics, histological stage and prognosis were summarized. Result 33 cases of PABC patients had a median age of 30 years (24~38 years) at diagnosis, in which 27.3%(9/33) were in stage I and II, 63.6%(21/33) were in stage III, and 9.1%(3/33) were in stage IV;Axillary nodes metastasis was found in 78.8%(26/33);53.6%(15/28) had pN3 in 28 patients with surgery. 87.9%cases were diagnosed as invasive ductal carcinoma, 6.1%(2/33) were invasive lob-ular carcinoma, and 6.1%(2/33) were considered to be other invasive carcinoma. 54.5%(18/33) were confirmed as hor-mone receptor positive, 24.2% (8/33) were triple-negative, and 21.2% (7/33) were determined to be HER2 over-ex-pressed. One patient received neo-adjuvant chemotherapy during pregnancy, and delivered a healthy matured infant. Me-dian follow-up time was 32 months (9 ~ 142 months), and median disease-free survival was 16.7 months (0.9 ~ 57.9 months) in 28 cases with surgery, and the overall survival was 68.1 months (8.8~142.1 months). Conclusion Diagnosis and treatment of PABC was often delayed, and were at more advanced stage yet with axillary node involvement at diagno-sis, thus it is important to pay more attention to breast mass noticed during pregnancy and lactation, meanwhile early diag-nosis and treatment are recommended.

Abstract:The diagnosis of cancer during pregnancy imposes a medical-ethical dilemma in weighing the risks of both mother and child. Increasing awareness of the feasibility of chemotherapy during pregnancy results in more pregnant patients receiving treatment for cancer. Information on obstetric and pediatric outcome of these high-risk pregnancies is greatly needed to guide physicians in patient counseling. In this review we present reported evidence for the incidence, diagnostic options, therapeutic management, obstetric risks, and neonatal outcome when cancer treatment is initiated during pregnancy. Decision-making when a cancer is diagnosed in a pregnant patient should be multidisciplinary, always taking the patient’s perspective into account. Cancer treatment during pregnancy is associated with low birth weight and preterm delivery, therefore frequent obstetric follow-up during oncological treatment in a specialized center is mandatory. Short-term clinical, cardiac, and cognitive outcome of children pre-natally exposed to cancer treatment is overall reassuring. Long-term follow-up of children is warranted to define the possible effect of pre-natal cancer treatment on general health, fertility outcome, and the risk of secondary cancers.

Abstract:Cancer during pregnancy is defined as a tumor diagnosed in a pregnant woman or up to 1-year post-partum. While being a rare disease, cervical cancer is probably one of the most challenging medical conditions, with the dual stake of treating the cancer without compromising its chances for cure, while preserving the pregnancy and the health of the fetus and child. To date, guidelines for gynecological cancers are provided through international consensus meetings with expert panels, giving insights on both diagnosis, treatment, and obstetrical care. However, these expert guidelines do not discuss the various approaches than can be found within the literature, such as alternative staging modalities or innovative surgical approaches. Also, the obstetrical care of women diagnosed with cervical cancer during pregnancy requires specific considerations that are not provided within our current standard of care. This systematic review aims to fill the gap on current issues with regards to the management of cervical cancer during pregnancy and provide future directions within this evolving landscape.

Abstract:Background: Breast cancer (BC) is considered the second most common malignancy affecting pregnant women. Treatment of this condition is complex due to the potential risks it poses to both the fetus and mother. In this study, we report on the incidence, clinicopathological characteristics, management, and the outcomes of patients diagnosed with breast cancer during pregnancy in a tertiary care institution in Saudi Arabia. Patients and methods: This is a single-institution retrospective cohort analysis. We reviewed the medical records of all breast cancer cases in pregnant women that were diagnosed and treated at a a tertiary care center in Saudi Arabia, during the period from 2001 to 2017. We used Fisher's exact test, chi-square, and Kaplan-Meier (KM) method for various analyses. Demographic data, treatment received, response, and prognostic factors were analyzed. The study was approved by the institutional review board at our center. Results: One hundred seventy-three patients met the search criteria. Median age at diagnosis was 34 (IQR: 30 – 38) years. Of these patients, 100 (58%) had invasive ductal carcinoma (IDC), 42 (24%) had IDC with ductal carcinoma in situ (DCIS), and 15 (20%) had other types. One hundred forty-two (82%) had pathological grades 2 and 3. Hormone receptors were positive in 90 (52%) patients, negative in 79 (45.7) patients and unknown in 4 (2.3%) patients. Her2/neu receptor status was positive in 67 (38.7%) patients and negative in 96(55.5%) patients. Forty five (26%) patients had triple negative disease.Stages I, II,III and IV were identified in 7(4%), 47 (27.2%), 54 (31.3%) and 51 (29.5%) patients respectively. Ninety (52%) and 75 (43%) patients received neoadjuvant and adjuvant anthracycline, platinum or taxane based chemotherapy. After being implemented in different international and our institutional guidelines, anti-Her2/neu therapies were given in 35 (20.2%) patients. The median overall survival (OS) was 127 months. Univariate analysis had shown statistically significant correlation between clinical stage, hormonal status, and overall survival. Conclusion: Breast cancer diagnosed during pregnancy is associated with poor outcomes. Breast cancer awareness and screening programs are essential for early detection and proper management thereby leading to better outcomes. Abstract:There is considerable interest in transplanting stem cells or progenitors into the injured nervous system and enhancing their differentiation into mature, integrated, functional neurons. Little is known, however, about what intrinsic or extrinsic signals control the integration of differentiated neurons, either during development or in the adult. Here we ask whether purified, postmitotic, differentiated retinal ganglion cells (RGCs) directly isolated from rat retina or derived from in vitro-differentiated retinal progenitor cells can survive, migrate, extend neurites, and form morphologic synapses in a host retina, in vivo and ex vivo. We found that acutely purified primary and in vitro-differentiated RGCs survive transplantation and migrate into deeper retinal layers, including into their normal environment, the ganglion cell layer (GCL). Transplanted RGCs from a wide range of developmental ages, but not from adults, were capable of extending lengthy neurites in the normal and injured adult rat retina ex vivo and to a lesser degree after transplantation in vivo. We have also demonstrated that RGCs may be differentiated and purified from retinal precursor cultures and that they share many of the same cell biological properties as primary RGCs. We have established that progenitor-derived RGCs have similar capacity for integration as developing primary RGCs but appear to form a lower number of presynaptic punctae. This work provides insight for further understanding of the integration of developing RGCs into their normal environment and following injury.
