Abstract:Background: The aim of this study is to assess the central auditory processing (CAP) function and its electroencephalogram (EEG) in patients with mild cognitive impairment (MCI) and the early stage of Alzheimer's disease (AD).Methods: In this study, 25 patients with early AD, 22 patients with MCI, and 22 matched healthy controls (HC) were included. After cognitive assessment, binaural processing function was assessed using the staggered spondaic word (SSW) test, and auditory working memory was assessed by auditory n-back paradigm, while EEG was recorded. Patients' behavioral indicators, event-related potentials (ERPs) components, and function connection (FC) were compared be-tween groups and the related factors were analyzed. Results: The difference of the accuracy of behavioral tests for the three groups of subjects was significant and all the behavioral indicators were positively correlated with cognitive function scores. Intergroup differences in amplitude (p < 0.05) and latency (p < 0.01) were significant for P3 in the 1-back paradigm. In the SSW test, AD and MCI patients showed reduced connectivity between the left frontal lobe and the whole brain in the 8-band, while in the n-back paradigm, patients with MCI and early AD showed reduced association of frontal leads with central and parietal leads in the 8-band.Conclusions: Patients with MCI and early AD have reduced CAP functions including binaural processing function and auditory working memory functions. This reduction is significantly associated with reduced cognitive function, and is reflected in different patterns of changes in ERP as well as functional connectivity in the brain.

Abstract:Background: Specific learning disabilities (SLDs) are a significant factor in children's academic underachievement. Around 46% of SLDs children have comorbid auditory processing disorder (APD). Cognitive functions are the ability to think and plan. Consequently, learning and academic progress are significantly affected by these disabilities. Objective: Our study sought to evaluate whether children with SLDs have both central auditory processing (CAP) and cognitive abnormalities. Patients and methods: A case-control study was conducted on 36 children from different schools of Zagazig city, Alsharquia government, Egypt, allocated into 2 groups, 18 with SLDs and academic underachievement (study group I) and 18 without SLDs (control group II). Medical history, otological evaluation, basic audiological examination, central screening tests (speech in noise (SPIN), dichotic digit (DDT), and pitch pattern sequence (PPS)), test for temporal resolution (Gaps in noise, (GIN)), IQ, and visuomotor ability evaluation were used to collect data. Results: The SLDs group had lower DDT, PPS, total score and percent of GIN test versus the control groups. At the same time, the approximate threshold of GIN test in the SLDs group was significantly greater relative to the normal group. Regarding Bender-Gestalt test (BGT) test, there was highly statically significant increase scores on copy and recall phases among control group. A significant positive relationship was detected between the BGT copy and recall phases and PPS on both sides. Conclusion: Our findings endorsed our hypothesis that most children with SLDs had subnormal scores on CAP abilities and cognitive function. The most severely affected CAP were temporal resolution and temporal ordering. Most SLDs children had visuomotor affection.

Abstract:Background and Aim: The Buffalo Model Questionnaire (BMQ) has been proposed for the screening, helping to diagnose and also monitoring the effect of rehabilitation on the improvement of Central Auditory Processing Disorder ((C)APD). In this regard, the applicability and accuracy of Persian-BMQ (P-BMQ) are evaluated by examining the correlation between the results of this questionnaire and the Buffalo model test battery. Methods: Overall, 254 children, normal and with Specific Learning Disorder (SLD) aged between 7–12 years old participated in this cross-sectional study. The questionnaire was completed by the parents of children who were subjected to the Buffalo model test battery evaluations. Results: In the normal group, the highest correlation (0.648) was shown between the Decoding (D) component of the P-BMQ with the Row Staggered Spondaic Word (RSSW) variable. In the SLD group, the highest correlation (0.318) was shown between the Variance-tolerance fading memory (V) component of the P-BMQ and the qualitative Persian version of the Phonemic Sentence Test (P-PST) variable. The highest correlation was considered to be between the D component of the P-BMQ with the Right Competitive word, started in the Left ear (RC-LEF) variable in the SSW test (0.498), and qualitative P-PST variable (0.471); and 0.360 between the V component of the P-BMQ and Persian version of the Speech in Noise with S/N=4 in the Left ear (P-SIN4L) of variable in the P-SIN test. Conclusion: P-BMQ is a useful screening tool complementing the Persian Buffalo model test battery for evaluating auditory processing evaluation in children. Abstract:Summary Objective This study aimed to compare the effect of experience on central auditory processing skills in voice therapists and other Speech-Language Pathologists in the auditory perceptual evaluation of voice. Study Design This research is a descriptive-analytical cross-sectional observational study. Methods Our study population includes three groups; first-year speech-language pathology students who have not undergone any training or education in the evaluation of auditory samples (group 1; n = 10), Speech-Language Pathologists who do not have any activity in the field of voice disorders (group 2; n = 10), and therapists who have at least 2 years of experience in auditory-perceptual judgment (group 3; n = 10). Initially, the participants underwent a basic audiological evaluation (Pure Tone Audiometry, Speech Recognition Threshold, Speech Discrimination Score, tympanometry , and acoustic reflex) to assess peripheral hearing. In the next step, the auditory processing skills of the participants were evaluated with the Buffalo model (staggered spondaic word (SSW) test, phonemic synthesis test (PST), speech in noise (SPN) test). In the third step, four recorded voice samples of patients with voice disorders and healthy individuals were randomly selected for auditory perceptual judgment. These samples were the same for all participants. All the voices were previously subjected to auditory perceptual analysis by two Speech-Language Pathologist who had at least 6 years of experience in the evaluation and treatment of voice disorders; the voices were played for participants to assess auditory perception using the grade, roughness, breathiness, asthenia, strain (GRBAS) scale. Results All subjects had normal hearing and auditory processing abilities but there were some differences among the groups in processing skills A difference was found in the SSW-qualifier and SPN-R components between the three groups. The third group had significant differences with the other two groups in the evaluation of auditory perception in the components of R, B, and S (from the GRBAS scale) in voice number one, and the components of G, R, and A in voice number three. In voice number two, there was a correlation between the SSW order effect component and the G component, as well as between the PST component and the A component. In voice number three, there was a correlation between the SPN-L component and the G component, and between the SSW condition, SSW qualifier, and SSW All errors components with the S component. In voice number four, there was a correlation between the SSW-RC, SSW-LC, SSW condition, SSW competing, and SSW order effect components with the A component. Lastly, there was a correlation between the SSW order effect and SSW type A components with the R component in voice number three in group three. Conclusion The results of the present study indicate that experience has an impact on auditory processing skills and the participants of the third group, who had more experience in the evaluation of voice disorders obtained better scores than the other two groups in the auditory processing skills. Additionally, the level of experience is related to the accuracy of auditory perceptual judgment of voice. The participants of the third group performed better than the other two groups, and the performance of the first and second groups was almost the same.

Diagnosis of amblyaudia in children referred for auditory processing assessment

Abstract:Children (n = 141) referred to 5 clinical sites for auditory processing disorder assessment were tested with two dichotic listening tests, one with word pairs and the other with pairs of digits, as part of a comprehensive diagnostic battery. Scores from the Randomized Dichotic Digits Test and the Dichotic Words Test were compared to age-appropriate norms and used to place children into one of four diagnostic categories (normal, dichotic dysaudia, amblyaudia, or amblyaudia plus) or to identify them as undiagnosed. Results from the two dichotic tests led to diagnosis of 56% of the children tested, leaving 44% undiagnosed. When results from a third dichotic listening test were used as a tie-breaker among originally undiagnosed children, a total of 79% of the children's scores were placed into diagnostic categories (13% normal, 19% dichotic dysaudia, 35% amblyaudia, 12% amblyaudia plus). Amblyaudia, a binaural integration deficit evident only from dichotic listening test results, was most prevalent (35% + 12% = 47%) in this population of children suspected of auditory processing weaknesses. Since amblyaudia responds to treatment with Auditory Rehabilitation for Interaural Asymmetry (ARIA), clinicians are guided through the protocol for identifying diagnostic categories so that they can make appropriate referrals for rehabilitation.

Abstract:Introduction: The infant-toddler meaningful auditory integration scale (IT-MAIS) can evaluate toddlers and infants’ early auditory development in aspects of detection, discrimination, and identification of sounds. The present study aims to evaluate the reliability and criterion validity of the Persian version of the IT-MAIS (IT-MAIS-P) for children with hearing loss (HL). Materials and Methods: In this study, due to the limitations of COVID-19 and the lack of access to samples, 23 available children with HL were sampled. The cause of participants› HL was 43.5% congenital, 30.4% acquired, and 26.1% unknown. The Mean±SD of the age of these children was 21.6±6.3 months. To evaluate the predictive validity and concurrent validity, the speech intelligibility rating (SIR) and categories of auditory performance (CAP) were used, respectively. Also, to assess the inter-rater reliability, both parents of each child completed the IT-MAIS-P. Results: A significant correlation (concurrent validity) was observed between the IT-MAIS-P and the CAP scores (r=0.87, P 0.05). Also, the inter-rater reliability (kappa=0.44, P<0.001) of the IT-MAIS-P was confirmed. Conclusion: The IT-MAIS-P is a valid tool to investigate the progress of auditory skills in infants, although the predictive validity was not confirmed.
A Survey on Screening and Diagnostic Criteria of Auditory Processing Disorders in India

Abstract:The current study aimed to determine the criteria used for screening and diagnosing cases with central auditory processing disorders (CAPD) in India. A cross-sectional questionnaire-based survey design was used in the present study. A questionnaire was developed to determine the criteria used for screening and diagnosing CAPD across clinics in India. Responses were obtained from 83 participants from all over India. Results indicated that 78% of respondents were currently doing CAPD evaluation. In that, the majority of respondents (63%) had a predetermined minimum battery that was relatively adaptable depending on the case history and age of the patient. In screening, most respondents used a screening questionnaire (SCAP, 75%) and a screening test (STAP, 60%). In the diagnostic protocol, the most used tests by the respondents were masking level difference (MLD), repetition of words (RW), gap detection test (GDT), pitch pattern test (PPT), speech perception in noise (SPIN), digit span test (DST), dichotic digit test (DDT), binaural fusion test (BFT), auditory brainstem response (ABR), dichotic CV test (DCVT), and duration pattern test (DPT). The current study's result will help professionals choose the minimum test battery for diagnosing CAPD.

Abstract:The goals of the current study were to evaluate audibility and cortical speech processing, and to provide insight into binaural processing in children with single-sided deafness (CHwSSD) using a cochlear implant (CI). The P1 potential to acoustically-presented speech stimuli (/m/, /g/, /t/) was recorded during monaural [Normal hearing (NH), CI], and bilateral (BIL, NH + CI) listening conditions within a clinical setting in 22 CHwSSD (mean age at CI/testing 4.7, 5.7 years). Robust P1 potentials were elicited in all children in the NH and BIL conditions. In the CI condition: (1) P1 prevalence was reduced yet was elicited in all but one child to at least one stimulus; (2) P1 latency was prolonged and amplitude was reduced, consequently leading to absence of binaural processing manifestations; (3) Correlation between P1 latency and age at CI/testing was weak and not significant; (4) P1 prevalence for /m/ was reduced and associated with CI manufacturer and duration of CI use. Results indicate that recording CAEPs to speech stimuli in clinical settings is feasible and valuable for the management of CHwSSD. While CAEPs provided evidence for effective audibility, a substantial mismatch in timing and synchrony of early-stage cortical processing between the CI and NH ear remains a barrier for the development of binaural interaction components.

Abstract:Introduction Central auditory processing disorders (CAPD) can significantly affect the daily functioning of a child, and the first step in determining whether rehabilitation procedures are required is a proper diagnosis. Different guidelines for making diagnoses have been published in the literature, and in various centers normative values for psychoacoustic tests of CAPD have been used internally. The material presented in this paper is based on more than 1000 children and is the largest collection so far published. The aim of this study is to present normative values for tests assessing CAPD in children aged 6 to 12 years, divided by age at last birthday. Method We tested 1037 children aged 6 to 12 years who were attending primary schools and kindergartens. The criteria for inclusion were a normal audiogram, intellectually normal, no developmental problems, and no difficulties in auditory processing. To evaluate auditory processing all children were given three tests on the Senses Examination Platform: the Frequency Pattern Test (FPT), Duration Pattern Test (DPT), and Dichotic Digit Test (DDT). Results The results from 1,037 children allowed us to determine normative values for FPT, DPT, and DDT in seven different age groups (6 through to 12 years). We developed a newapproach, based on quantile-based norms, to determine normative values in each group. Three categories – average, below-average, and above-average – allow for a broader but more realistic interpretation than those used previously. We compare our results with published standards. Conclusions Our study is the largest normative database published to date for CAPD testing, setting a standard for each child by age in years. We used the Senses Examination Platform, a universal tool, to unify standards for the classification of CAPD. Our study can serve as a basis for the development of a Polish model for the diagnosis of CAPD.

Abstract:Background: Various studies have shown that individuals with autism spectrum disorder (ASD) experience significant cognitive impairments during childhood. Individuals often experience various language disorders that can manifest in different ways. There are also studies indicating that these impairments persist into adulthood for individuals with ASD. This study aimed to evaluate and identify cognitive impairments among Persian-speaking adults with ASD. Methods: This research is of a quantitative nature and has been conducted using an experimental method in which two subtests from the Persian Version of the Montreal Protocol for the Evaluation of Communication (P.M.E.C.), including the Metaphor Interpretation and Speech Act Interpretation subtests, were utilized. Thirteen Persian-speaking men with ASD participated in this research, with ages ranging from 25 to 44 years (mean age 32.84, standard deviation 4.17), whose educational levels varied from primary school to 20 years of formal education. The control group consisted of 26 healthy Persian-speaking men who were matched in terms of age and educational level with the ASD group. The Kolmogorov-Smirnov test and a paired t-test were used to compare the two groups. Results: The results indicated that the ASD group performed significantly poorer in both the Metaphor Interpretation subtest (P < 0.001) and the Speech Act Interpretation subtest (P = 0.033) compared to the healthy control group, suggesting cognitive impairments in their abilities. Conclusion: The findings of this research can be valuable for assessment and intervention purposes in rehabilitation centers, as well as in academic and research settings.

Abstract:Background Children with visual challenges rely on their hearing ability to accomplish daily tasks more efficiently. Many investigations show changes in the auditory pathway in children and adults with early blindness. It can be hypothesized that the auditory processing abilities of children with congenital blindness will be superior to those of typically developing children without any visual challenges. Purpose The current study was designed to compare some of the auditory processing abilities of children with congenital blindness and typically developing children who do not have any visual challenges. Specifically, the study compared the perception of speech in noise, temporal patterning ability, binaural integration ability as well as auditory memory and sequencing abilities of children in two groups. Research Design It was a prospective cross-sectional study comparing two groups. Study Sample A total of 160 children, 80 typically developing children without any visual challenges (reference group) and 80 children with congenital blindness, in the age range of 7 to 11 years, participated in the study. Data collection and analysis Pitch pattern test (PPT), perception of speech perception in noise test in Marathi (PSIN-M), dichotic digit test in Marathi (DDT-M), and auditory memory and sequencing test in Marathi (AMST-M) were administered. Independent sample T test and Mann Whitney U test were used to compare the performance of the two groups on these tests. Results Children with congenital blindness showed significantly better scores on PPT, PSIN-M, and DDT-M as compared to children in the reference group in all the age groups. A clear trend was not observed on DDT-M Conclusion It can be concluded that children with congenital blindness have better auditory processing, auditory memory, and sequencing abilities compared to typically developing children without any visual challenges.

Abstract:Background and Aim: Hearing handicap, as one of the common health problems among older people, affects life activities. The Hearing Handicap Questionnaire (HHQ) is one of the scales that provide criteria for social withdrawal, participation restriction, and emotional distress. The present study aims to translate the HHQ into Persian and determine its psychometric properties. Methods: After translation into Persian, the content validity of the questionnaire was determined based on the Lawashe’s method. Then, the Persian HHQ (P-HHQ) and the Persian Hearing Handicap Inventory for Elderly-Screening version (P-HHIE-S) were completed by 110 hearing-impaired seniors (49 females) over 60 years. The concurrent validity was determined by Spearman correlation test, and the discriminant validity was analyzed by Kruskal-Wallis test and independent t-test. The test-retest reliability was assessed in 47 subjects after two weeks by Spearman correlation test and paired t-test. Results: The P-HHQ had high face validity. The mean total score of P-HHQ was 1.89±1.05. It had a significant positive correlation with the score of P-HHIE-S (r=0.87) and pure tone average of the better ear (r=0.72). There was a significant difference between three groups of elderly with different degrees of hearing impairment (p<0.001). Cronbach’s α values were in the range of 0.94–0.97. There was a strong correlation between test and retest scores of P-HHQ (r=0.97) which indicates a high test-retest reliability. Conclusion: The P-HHQ has acceptable validity and reliability and can be used as a suitable instrument to evaluate hearing handicap of the elderly in research studies and clinical settings. Abstract:Purpose: This study aims to evaluate the effect of auditory neuropathy spectrum disorder (ANSD) on postoperative auditory perception and listening difficulties in pediatric cochlear implant (CI) recipients. Method: The Children's Auditory Perception Test (CAPT) assesses auditory perception skills, and the Children's Home Inventory of Listening Difficulties (CHILD) Scale evaluates daily listening difficulties. The study involved pediatric CI recipients ( n = 40) aged between 5 and 7 years, with and without diagnosis of ANSD. The research ensured homogeneity across various factors, including chronological age, age at diagnosis, age at initial implantation, bilateral simultaneous surgery, etiologies of hearing loss, and family education level. Results: The findings have demonstrated that children without ANSD exhibited better performance in integrating visual–auditory stimuli and overall listening performance, distant sound source scores, and noisy environment scores (respectively p = .047, p = .001, p = .028, and p = .010). Additionally, children with better speech perception also have a better ability to integrate audiovisual stimuli ( p = .005, r = .438). Conclusions: There are significant differences in postoperative listening skills and auditory perceptions between children with and without an ANSD who have CIs. Accordingly, children without an ANSD perform better.

Abstract:Introduction Parkinson's patients with dysarthria often suffer from multiple impairments in speech subsystems, including phonation. The Acoustic Voice Quality Index (AVQI) may be considered as a predictor of the onset and severity of Parkinson's disease. Objective Investigating the AVQI in Persian-speaking Parkinson's patients compared to healthy controls and its association with disease severity based on Unified Parkinson's Disease Rating Scale-Part III (UPDRS-III) and dysarthria severity. Method This is an epidemiological descriptive-analytical study with a sample size of 30 Parkinson's patients with hypokinetic dysarthria and 30 healthy controls. Disease severity was first assessed using the UPDRS-III, followed by speech sample recording (sustained vowel/a/phonation and reading of the text "grandfather") using a specialized microphone and dysarthria severity assessment. Finally, AVQI scores were extracted using the Praat software. Results An independent t test was used to compare mean AVQI between the healthy and patient groups. The mean voice quality index was higher in the patient group than in the healthy controls, and the difference between the two groups was significant ( P value ≤ 0.001). Correlation analysis of AVQI with disease severity and dysarthria severity using Spearman's test showed that voice quality index was significantly positively correlated with disease severity (UPDRS-III) and dysarthria severity ( P value ≤ 0.001). Conclusion The AVQI can differentiate between the presence of voice disorders in Persian-speaking Parkinson's patients with hypokinetic dysarthria and healthy controls. This index showed a positive correlation with Parkinson's disease severity and dysarthria severity in Persian. It is recommended that this correlation be investigated in other languages as well.

Abstract:Purpose: The diagnosis of auditory processing disorder (APD) is controversial particularly due to the influence of higher order factors of language and cognition on the diagnostic APD testing. As a result, there might be a need for testing for other domains (e.g., cognition) along with conducting the diagnostic APD testing to rule out the influence of other domains. In order to make recommendations on whether cognitive testing is needed along with the auditory processing testing, as a starting point, the current study was conducted to examine the relationship between cognitive abilities and basic auditory processing in young adults. Method: A total of 38 young adults with normal audiometric thresholds between 250 and 8000 Hz participated in this study. They were tested on their executive function, language, processing speed, working memory, and episodic memory components of cognitive testing and tests for temporal fine structure and spectrotemporal sensitivity for auditory processing testing. Results: No significant correlation was found between the cognitive tests and the tests for basic auditory processing in young adults. Conclusions: These findings present contrast to the existing findings in children and older adults where a stronger correlation between cognitive abilities and auditory processing has been found. The current findings suggest that testing for cognitive abilities may not be needed when testing for basic auditory processing in young adults.

Abstract:Elimination of drug molecules via tubular secretion is an important pharmacokinetic parameter especially for oral dosage forms where an extremely short half-life would prevent their application. We have studied the inhibition in vitro of the glycination of p-aminobenzoic acid (PABA) using rat liver microsomal preparations. The I50 values, the concentration of the inhibitor that leads to 50% inhibition of glycine conjugation of PABA as compared to the control, have been determined for sulfamoyl benzoic acids, sulfonamides, and penicillins. Statistically significant regression equations were derived explaining the observed variation in I50 values as a function of lipophilicity and steric bulk of the substituents for the combined set of sulfamoyl benzoic acids and sulfonamides (n = 33). For the penicillins studied, only steric effects seem to be important for the explanation of the I50 values. Finally, regression analysis and the use of neural networks allowed the classification of compounds by their in vitro I50 values as being tubular secreted or not. Therefore, it can be concluded that the inhibition in vitro of the glycine conjugation of PABA is a useful model for the estimation of tubular secretion and drug interaction potential of acidic drug molecules in this process in vivo.
Relationship between Auditory Processing Abilities and Hearing Aid Benefit in Elderly Individuals with Hearing Impairment

Abstract:Background and Aim: Cognitive and auditory processing deficits are seen in older individuals with normal hearing sensitivity. Studies on older individuals with hearing impairment have assessed the cognitive function and correlated with hearing devices benefit. Since auditory processing can also affect speech perception abilities, and there is a possible relationship between cognition and some of the auditory processing abilities, it is essential to assess the relationship between auditory processing abilities and hearing aid benefit in naïve and experienced hearing aid users in older adults. Methods: Fifty individuals in the age range of 51 to 70 years with mild to moderate hearing loss participated in the study. There were 30 participants without any hearing aid experience and 20 participants with hearing aid experience for at least six weeks. Their auditory processing abilities were tested using gap detection test, duration pattern test, speech perception in noise, dichotic consonant-vowel test, masking level difference, forward and backward span tests. The hearing aid benefit was assessed using aided speech perception in noise measures and International Outcome Inventory-Hearing Aids questionnaire in Kannada. Results: Spearman’s correlation showed only correlation between auditory closure ability and binaural integration abilities with hearing aid benefit in experienced users. There was no correlation between any other auditory processing abilities and hearing aid benefit. Conclusion: There is a correlation between a few auditory processes and hearing aid use in elderly individuals with mild to moderate hearing loss.
