Abstract:OBJECTIVE: To validate a French version of the speech, spatial, and qualities of hearing scale (SSQ), a subjective evaluation of patients' hearing disability, and to assess SSQ reproducibility across different language versions.DESIGN: The SSQ was translated in accordance with the principles of the 'Universalist approach' of cross-cultural adaptation of patient-reported outcome instruments. Scores from a normal-hearing and a hearing-impaired population were compiled and compared, whenever possible, with data from the literature, collected using other language versions.STUDY SAMPLE: One hundred normal-hearing subjects and 230 hearing-impaired subjects.RESULTS: Good reproducibility of scores and inter-subject variability were obtained between several language versions, even if scores found using the French version were slightly lower than those obtained using Dutch or English versions. A comparison of factor analysis outcomes between the English and French versions confirmed good conceptual equivalence across languages and robustness of the SSQ for use in international settings. The three main subscales (speech, spatial, and qualities) confirmed their usefulness in assessing different aspects of hearing disability.CONCLUSION: This study validated a French-language version of the SSQ, and assessed the reproducibility of the SSQ across subject groups, administration modes, and different countries/languages, confirming its potential as an international standard for hearing disability evaluation.

French-Language Adaptation of the Consensus Auditory-Perceptual Evaluation of Voice (CAPE-V)

Abstract:Summary Objectives This study aimed to adapt the Consensus Auditory-Perceptual Evaluation of Voice (CAPE-V) protocol for perceptual voice assessment to the French language. The primary objective was to achieve consensus among an international panel of voice experts on the content of the adapted protocol. Methods To ensure the relevance and robustness of the French CAPE-V protocol, this study employed a systematic Delphi method and involved an international panel primarily comprising speech therapists and lecturers from France and Belgium. The multi-stage process included an initial panel size of 15 experts. Three rounds of online questionnaires, integrating both quantitative and qualitative data collection, were conducted. Participants provided feedback and ratings on various protocol elements until a consensus was reached. Adaptations targeted the choice of task stimuli (sustained vowel, sentence reading, semi-spontaneous speech), of the rating scales, and vocal quality terminology. Results The Delphi process achieved consensus on all elements of the adapted CAPE-V protocol. Notably, the sustained vowel task saw consensus in favor of the vowel /a/. Sentence adaptations achieved substantial agreement, with the final set unanimously approved. The simple Visual Analog Scale emerged as the preferred rating scale. Agreement on terms for describing vocal qualities marked a crucial step in establishing a shared vocabulary among French-speaking voice experts. Conclusions The study successfully adapted the CAPE-V protocol for perceptual voice assessment to the French language through a systematic Delphi process. The final protocol closely resembles the original English version, maintaining its structure and core objectives. Consensus on sustained vowel tasks, sentence adaptations, rating scales, and vocal quality terminology indicates the relevance and robustness of the adapted protocol. Ongoing validation studies in France demonstrate the potential clinical utility of the adapted CAPE-V in French-speaking contexts, representing a significant step toward standardized and validated voice assessment tools for clinicians and researchers globally.

Abstract:Nowadays non-small cell lung cancer (NSCLC) is the first cause of death for tumor worldwide. In the second line setting there are few results upon survival parameters from the various treatment options. European Society of Medical Oncology (ESMO) guidelines say that patients clinically or radiologically progressing after first-line chemotherapy, irrespective of administration of maintenance chemotherapy, and with an Eastern Cooperative Oncology Group (ECOG) Performance Status (PS) 0-2, should be offered second-line chemotherapy (1). In this setting, single agents chemotherapy improve disease-related symptoms and overall survival (OS) to nearly 6.7-8.3 months, with 30% of patients alive at 1 year (2,3). Comparable options in the second-line therapy consist of pemetrexed—for non-squamous histology only—or docetaxel. Erlotinib is an additional potential option in patients with PS 0-2 (1). Epidermal growth factor receptor (EGFR) tyrosine kinase inhibitors (TKIs) are the standard treatment option for advanced NSCLC patients harboring EGFR-activating mutations.
Reference Data for a Quick Speech-in-Noise Hearing Test in the French Language

Abstract:Introduction: Difficulty in understanding speech in noise is the most common complaint of people with hearing impairment. Thus, there is a need for tests of speech-in-noise ability in clinical settings, which have to be evaluated for each language. Here, a reference dataset is presented for a quick speech-in-noise test in the French language (Vocale Rapide dans le Bruit, VRB; Leclercq, Renard, & Vincent, 2018). Methods: A large cohort ( N = 641) was tested in a nationwide multicentric study. The cohort comprised normal-hearing individuals and individuals with a broad range of symmetrical hearing losses. Short everyday sentences embedded in babble noise were presented over a spatial array of loudspeakers. Speech level was kept constant, while noise level was progressively increased over a range of signal-to-noise ratios. The signal-to-noise ratio for which 50% of keywords could be correctly reported (speech reception threshold, SRT) was derived from psychometric functions. Other audiometric measures were collected for the cohort, such as audiograms and speech-in-quiet performance. Results: The VRB test was both sensitive and reliable, as shown by the steep slope of the psychometric functions and by the high test-retest consistency across sentence lists. Correlation analyses showed that pure tone averages derived from the audiograms explained 74% of the SRT variance over the whole cohort, but only 29% for individuals with clinically normal audiograms. SRTs were then compared to recent guidelines from the French Society of Audiology [Eur Ann Otorhinolaryngol Head Neck Dis. 2022;139(1):21–7]. Among individuals who would not have qualified for hearing aid prescription based on their audiogram or speech intelligibility in quiet, 18.4% were now eligible as they displayed SRTs in noise impaired by 3 dB or more. For individuals with borderline audiograms, between 20 dB HL and 30 dB HL, the prevalence of impaired SRTs increased to 71.4%. Finally, even though five lists are recommended for clinical use, a minute-long screening using only one VRB list detected 98.6% of impaired SRTs. Conclusion: The reference data suggest that VRB testing can be used to identify individuals with speech-in-noise impairment.

Analysis of Voice in Parkinson’s Disease Utilizing the Acoustic Voice Quality Index

Abstract:OBJECTIVES: The purpose of this study was to analyze the acoustic voice quality index (AVQI) in relation to perceptual analysis and disease stage in speakers with Parkinson's disease (PD).STUDY DESIGN: Cross-sectional study.METHODS: The following data were gathered from the Parkinson's Disease Speech corpus of Tampere University (PDSTU): prolonged vowels and reading samples from native Finnish speakers with PD (n = 34), speaker demographic information, and Hoehn and Yahr scale scores. AVQIv03.01 analysis was completed with Praat. Expert raters utilized the GRBASI scale (GRBASI: grade, roughness, breathiness, asthenia, strain, instability) to assess voice quality. AVQI scores were analyzed in relation to GRBASI ratings and disease stage, as measured by the Hoehn and Yahr scale, utilizing correlation analysis.RESULTS: AVQI scores ranged from -0.11 to 4.46 (mean = 1.64, SD = 1.39), GRBASI average total scores ranged from 0-1.83 (mean = 0.66, SD = 0.50), and Hoehn and Yahr scale scores ranged from 1- 4. A statistically significant correlation between AVQI scores and GRBASI ratings was found (r<sub>s</sub> = 0.508, P = 0.01). Several significant correlations were also found between the parameters of AVQI and scores for G, R, B and A from GRBASI. Hoehn and Yahr scores correlated significantly with GRBASI total score (r<sub>s</sub> =0.437, P = 0.01) but not with AVQI.CONCLUSION: A significant correlation was found between GRBASI ratings and Hoehn and Yahr scores, but not between AVQI and Hoehn and Yahr scores.DATA AVAILABILITY: Due to the nature of this research, participants of this study did not agree for their data to be shared publicly. Data may be made available on a case-by-case basis. Data will be available after the active phase of the study in 2028.

[Singing Voice Handicap Index-12 : Development and validation of a German version]

Abstract:Background: The aim of this study was to develop a shortened German version of the Singing Voice Handicap Index (SVHI). The SVHI is a one-dimensional instrument for self-assessment of a voice disorder in singers. The questionnaire developed in the USA comprises 36 items and has been available in a validated German version since 2013. Methods: Bicentric data from a total of 200 patients formed the basis for item analysis and selection. Using corrected item-total correlations, 12 items were selected for the abridged version. The internal consistency was calculated. The SVHI-12 was subsequently validated in 97 vocal patients and 105 vocally healthy singers (control group) using the test-retest procedure. Results: The SVHI-12 achieved a good internal consistency (Cronbach's alpha = 0.93) and a good test-retest reliability (intra-class correlation r = 0.88 ; p < 0.001). The patients had significantly higher overall scores (18 ± 13 vs. 7 ± 6) compared to the healthy control group. The SVHI-12 overall score correlated significantly positively with the severity of the voice disorder as reported by the patient (r = 0.68; p < 0.001). As a threshold value above which a voice can be described as disturbed, a total score > 7 points was calculated using receiver operating curve analysis. As an indication of a voice disorder, a sensitivity of 81% and a specificity of 71% is thus achieved (Youden index 0.523, area under the curve 0.827, 95% confidence interval 0.769-0.885). Conclusion: The shortened SVHI has similarly good psychometric characteristics to the original SVHI. With the SVHI-12, a valid and effective instrument for the detection of singing voice disorders is available for German-speaking countries.

Abstract:PurposeThe Auditory Vocal Hallucination Rating Scale Questionnaire (AVHRS-Q) is a short self-report measure assessing several characteristics of auditory vocal hallucinations (AVH) that was derived from a validated clinical interview (the auditory vocal hallucination rating scale; AVHRS). This study investigated the internal reliability, convergent validity, and divergent validity of the AVHRS-Q using two clinical samples.MethodsIn sample I, 32 psychiatric patients with AVH were recruited from an academic hospital service and assessed with the AVHRS and the AVHRS-Q. Data for sample II were retrospectively retrieved from a pseudonymised Routine Outcome Monitoring (ROM) database collected in the context of mental healthcare at the same academic hospital service. Data from 82 psychiatric patients with AVH were retrieved, who completed the AVHRS-Q, and measures of psychological distress (the Outcome Questionnaire; OQ-45, and the Symptom Checklist; SCL-90) and quality of life (the Manchester Short Assessment of Quality of Life; MANSA).ResultsThe AVHRS-Q showed good internal consistency in both samples. Severity scores of the AVHRS-Q were strongly correlated to the severity scores of the AVHRS (r = 0.90, p X2 (1, N = 32) = 15.71]. AVHRS-Q severity scores had moderate correlations with measures of psychological distress (OQ-45, r = 0.43, p r = 0.50, p r = − 0.22, p ConclusionsThe AVHRS-Q demonstrated good reliability, convergent validity, and divergent validity, suggesting it can be applied in both clinical and research settings for a quick and reliable assessment of AVH.

Abstract:Purpose: Vocal fatigue–related symptoms, frequent in patients with voice disorders, are reliably quantifiable with the Vocal Fatigue Index (VFI) in several languages. The main aim of this work was to investigate the content and construct validity of the German Vocal Fatigue Index (VFI-D) by comparison with the German Voice Handicap Index 9 international (VHI9i) and the Vocal Tract Discomfort Scale (VTD). Moreover, the scale and retest reliability were investigated. Method: In a prospective cohort study at two tertiary care hospitals, 93 adults with all types of voice complaints filled out the VFI-D, the VHI-9i, and the VTD. Eighty-one adults ( M age = 53.5 years, SD = 15.7) completed them one time, and additional 12 patients ( M age = 53.8 years, SD = 15.6) did a test–retest, completed them twice at an interval of 1 week. The validity was assessed by Spearman's rho and Mokken scaling and the reliability by a factor analysis and a two-tailed t test. Results: Mokken scaling has proven the VFI-D as one-dimensional questionnaire (one cluster, r = .95). All VFI-D items could be assigned to one of three factors according to principal component and principal axis factor analysis. Although Factor 1 related best to the VHI9i (∅ r s = .76) and Factor 2 to the VTD (∅ r s = .74), Factor 3 correlated moderately with the VHI9i ( r s = .34) and weakly with the VTD ( r s = .25). Results from the first and second VFI-D completion agreed strongly ( t = −1.78, p = .10). Conclusions: In this study, the content and construct validity of the VFI-D were confirmed. Also, both scale reliability with identical factors as those in the original study and retest reliability were verified. The VHI9i and VTD were highly indicative of VFI-D Factors 1 and 2, respectively. No correlation between Factor 3 and the other questionnaires was determined. Future research should evaluate which characteristics are specific to vocal fatigue versus general voice disorder–related symptoms.

Abstract:Introduction Parkinson's patients with dysarthria often suffer from multiple impairments in speech subsystems, including phonation. The Acoustic Voice Quality Index (AVQI) may be considered as a predictor of the onset and severity of Parkinson's disease. Objective Investigating the AVQI in Persian-speaking Parkinson's patients compared to healthy controls and its association with disease severity based on Unified Parkinson's Disease Rating Scale-Part III (UPDRS-III) and dysarthria severity. Method This is an epidemiological descriptive-analytical study with a sample size of 30 Parkinson's patients with hypokinetic dysarthria and 30 healthy controls. Disease severity was first assessed using the UPDRS-III, followed by speech sample recording (sustained vowel/a/phonation and reading of the text "grandfather") using a specialized microphone and dysarthria severity assessment. Finally, AVQI scores were extracted using the Praat software. Results An independent t test was used to compare mean AVQI between the healthy and patient groups. The mean voice quality index was higher in the patient group than in the healthy controls, and the difference between the two groups was significant ( P value ≤ 0.001). Correlation analysis of AVQI with disease severity and dysarthria severity using Spearman's test showed that voice quality index was significantly positively correlated with disease severity (UPDRS-III) and dysarthria severity ( P value ≤ 0.001). Conclusion The AVQI can differentiate between the presence of voice disorders in Persian-speaking Parkinson's patients with hypokinetic dysarthria and healthy controls. This index showed a positive correlation with Parkinson's disease severity and dysarthria severity in Persian. It is recommended that this correlation be investigated in other languages as well.

Abstract:Transplant glomerulopathy (TG) is a histologic entity described more than four decades ago. In the last few years, our understanding of TG has improved significantly. Current evidence supports the postulate that TG is a unique pathologic and pathogenic entity distinct from other forms of chronic allograft injury. Detailed electron microscopic studies have shown basement membrane abnormalities in glomerular and peritubular capillaries, indicating that this is a disease of the entire renal capillary network. Staining biopsies for the complement fragment, C4d, showed positivity in subgroups of TG, suggesting the participation of antidonor antibodies. Consistent with this postulate, the incidence of TG is increased in patients with antidonor HLA antibodies prior to the transplant. The use of surveillance biopsies has demonstrated that TG can develop during the first few months after transplantation, although it may remain clinically quiescent for several years. However, TG is progressive, leading to reduced graft survival. Recent studies demonstrated a close association between TG and anti‐HLA class II antibodies. Current therapies for TG are likely of limited value. However, it is also likely that an improved understanding of TG pathogenesis will result in the development of effective therapies for this form of progressive kidney allograft damage.
Mandarin Chinese version of the Aging Voice Index

Abstract:Objectives: This study developed the Mandarin Chinese version of the Aging Voice Index (AVI), with preliminary validation of the scale for potential clinical applications. Study design: Scale development. Methods: The experimental procedure involved: (1) cross-cultural adaptation of the original AVI into the Mandarin Chinese version (CAVI); (2) evaluation by expert panel; (3) back translation; (4) pilot testing; (5) development of the final CAVI; (6) scale validation with 68 older adults of 60-89 years old (29 females and 39 males), 34 with voice disorders and 34 age-matched with normal voice. Internal consistency reliability, test-retest reliability, content validity, criterion-related validity, and discriminatory ability (diagnostic accuracy) of the CAVI were evaluated. Results: There were high internal consistency (Cronbach's alpha = 0.9733), high test-retest reliability (intraclass correlation coefficient = 0.9578, p < 0.01), high content validity (content validity index = 0.9710), high criterion-related validity (Pearson's r = 0.9439, p < 0.01 between CAVI and Voice Handicap Index-10; r = 0.8070, p < 0.01 between CAVI and voice-related quality of life [V-RQOL]), and significant difference in CAVI scores between the two groups with huge effect size (t(34.69) = -11.59, Cohen's d = 2.81, p < 0.001). Receiver operating characteristic analysis revealed a high diagnostic accuracy of the CAVI, with an area under the curve of 0.9974 (p < 0.001) and a cut-off score of 12.0 with 100% sensitivity and 97.1% specificity. Conclusion: Our findings suggested that the CAVI could be a reliable and valid standardized self-assessment questionnaire tool for clinical evaluation of the impact of voice problems specifically for Mandarin-speaking older adults. Further studies should explore a full-scale validation of the CAVI for being a standard clinical tool, including for older adults in Mainland China. Level of evidence: 3b (case-control study).
Integrated Vocal Deviation Index (IVDI): A Machine Learning Model to Classifier of the General Grade of Vocal Deviation

Abstract:Objective To develop a multiparametric index based on machine learning (ML) to predict and classify the overall degree of vocal deviation (GG). Method The sample consisted of 300 dysphonic and non-dysphonic participants of both sexes. Two speech tasks were sustained vowel [a] and connected speech (counting numbers from 1 to 10). Five speech-language pathologists performed the auditory-perceptual judgment (APJ) of the GG and the degrees of roughness (GR), breathiness (GB), instability (GI), and strain (GS). We extracted 47 acoustic measurements from these tasks. The APJ result and the acoustic measurements were used to develop the multiparametric index. We used mean absolute error, root mean square error, and coefficient of determination ( R 2) to select the best model of ML to predict GG and feature importance to select the best set of variables for the index. After classifying the GG between nondysphonic, mild, moderate, and severe, the final model was validated using accuracy, sensitivity, specificity, predictive values, likelihood ratios, F1-Score, and weighted kappa. Results The gradient boost model showed the best performance among the ML models. Eight features were selected in the model, including four acoustic measures (jitterLoc, smoothed cepstral peak prominenc, mean harmonic-to-noise ratio (HNRmean), and correlation) and four APJ measures (GR, GB, GS, and GI). The final model correctly classified 93.75% of participants and obtained a weighted kappa index of 0.9374, demonstrating the model's excellent performance. Conclusion The Integrated Vocal Deviation Index includes four acoustic measures and four auditory-perceptual measures and showed excellent performance in classifying voices according to GG.

Alouette‐R normative data for French‐speaking school‐aged children living in Quebec

Abstract:Background The Alouette‐R (2005) by Lefavrais is one of the most widely used tools to assess reading skills in French. However, this instrument does not have normative data specific to the French‐speaking population of Quebec, Canada. Aims The validity of an assessment being strongly compromised when using inappropriate norms, the first objective of this study was to establish local norms for the Alouette‐R. The second objective was to provide sensitivity and specificity data for each Alouette‐R measure in the French‐speaking Quebec population. The third objective was to compare Quebec and French normative data and their sensitivity to better understand the applicability and effectiveness of the Alouette‐R test at the regional level. Methods & Procedures A total of 347 fluent readers and 48 children with dyslexia from 3rd to 6th grades were recruited from different regions in Quebec. Participants had to read aloud the 265‐word text of the Alouette‐R in a maximum of 3 min. Outcomes & Results Norms (means, standard deviations and percentiles) by school grades were created for each test measure: reading time, number of words read, number of errors, number of words correctly read, reading accuracy index and reading fluency index. The sensitivity (i.e., the ability to correctly identify children with dyslexia) and specificity (i.e., the ability to correctly identify children without dyslexia) of these measurements were also documented. The norms and their sensitivity were then compared with those of the original French study by Lefavrais in 2005. Conclusions & Implications The presence of differences between European and Quebec norms supports the importance of using local norms when assessing language skills. The reading accuracy and fluency indexes are the measurements that best discriminated children with dyslexia from those without a reading disorder in our study. This study will allow clinicians working in Quebec to have a better interpretation of the Alouette‐R measurements and ultimately avoid erroneous conclusions resulting from the use of foreign normative data. What this paper adds What is already known on this subject The Alouette‐R is a reading test validated and standardized in France to screen for dyslexia in children. The validity of existing norms with the Quebec population in Canada is questionable due to socio‐linguistic differences with the population of France. What this study adds to existing knowledge This study provides for the first time normative and sensitivity/specificity data of the Alouette‐R for French‐speaking school‐aged children living in Quebec. Differences were noted with the normative data from France, which supports the importance of using local normative data when administering reading tests in Quebec. What are the potential or actual clinical implications of this work? When administering the Alouette‐R, clinicians in Quebec will now be able to use normative data adapted to the local population, which will limit erroneous conclusions resulting from the use of foreign normative data. In addition, the sensitivity and specificity values reported in the article will allow these clinicians to better interpret their results when screening for a developmental reading disorder.

Cross-Cultural Adaptation and Validation of the Vocal Fatigue Index (VFI) to Danish

Abstract:Objectives: The aim was to cross-culturally adapt and validate the Vocal Fatigue Index (VFI) to Danish (DK-VFI). Study design: Cross-sectional design. Methods: The original American English VFI was cross-culturally adapted to Danish using a forward-backward translation procedure combined with focus group interviews. The DK-VFI was distributed to 26 patients with vocal fatigue (VF) and 103 vocally healthy controls. A matched subsample based on age and sex of the control group was included. Internal consistency was determined using Cronbach's alpha and test-retest reliability was determined using Spearman's Rho as well as Wilcoxon signed-rank tests. Validity was confirmed using the Wilcoxon rank-sum test. Sensitivity and specificity were assessed using the Receiver Operating Characteristics test and the discriminative ability was determined using the Area Under the Curve (AUC) test. The original VFI consists of three factors, and cutoff values were calculated for each separate factor in the DK-VFI. Results: Minor changes were made in the phrasing of specific questions in order to appropriate the VFI to the Danish language. The internal consistency for the DK-VFI was high, with a Cronbach's alpha of α = 0.95. The results showed statistically significant differences (P < 0.05) in mean scores of the DK-VFI between patients and controls. The cutoff values between patients and controls for each separate factor were ≥11.5 for factor 1, ≥2.5 for factor 2, and ≤5.5 for factor 3. The results of the AUC analysis showed that the DK-VFI has discriminative abilities of 0.94 for factor 1, 0.92 for factor 2, and 0.82 for factor 3. Analysis of test-retest reliability showed strong reliability for the patient group and acceptable reliability for the control group. Conclusion: The DK-VFI is a valid and reliable tool for identifying symptoms of VF and can sensitively discriminate people with VF from vocally healthy controls.

Abstract:'Dysarthria' is a group of motor speech disorders resulting from a disturbance in neuromuscular control. Most individuals with dysarthria cope with communicative restrictions due to speech impairments and reduced intelligibility. Thus, language-sensitive measurements of intelligibility are important in dysarthria neurological assessment. The Frenchay Dysarthria Assessment, 2nd edition (FDA-2), is a validated tool for the identification of the nature and patterns of oro-motor movements associated with different types of dysarthria. The current study conducted a careful culture- and linguistic-sensitive adaption of the two intelligibility subtests of the FDA-2 to Hebrew (words and sentences) and performed a preliminary validation with relevant clinical populations. First, sets of Hebrew words and sentences were constructed, based on the criteria defined in FDA-2, as well as on several other factors that may affect performance: emotional valence, arousal and familiarity. Second, the new subtests were validated in healthy older adults (n = 20), and in two clinical groups (acquired dysarthria, n = 15; and developmental dysarthria, n = 19). Analysis indicated that the new subtests were found to be specific and sensitive, valid and reliable, as scores significantly differ between healthy older adults and adults with dysarthria, correlated with other subjective measures of intelligibility, and showed high test-retest reliability. The words and sentences intelligibility subtests can be used to evaluate speech disorders in various populations of Hebrew speakers, thus may be an important addition to the speech-language pathologist's toolbox, for clinical work as well as for research purposes. WHAT THIS PAPER ADDS: What is already known on the subject 'Dysarthria' is a group of disorders reflecting impairments in the strength, speed and precision of movements required for adequate control of the various speech subsystems. Reduced speech intelligibility is one of the main consequences of all dysarthria subtypes, irrespective of their underlying cause. Voice disorder discrimination using vowel acoustic measures in female speakers

Abstract:Background Sustained vowels are important vocal tasks that have been investigated in discriminating voice disorders using acoustic analysis. To date, no study has combined vowel acoustic measures only that evaluate major aspects of the pathological voice signals in voice disorder discrimination. Aims To investigate the value of vowel acoustic measures that quantify glottal noise, signal stability, signal periodicity, spectral slope and overall voice quality in discriminating female speakers with and without voice disorders. Methods & Procedures Sustained vowel /ɑ/ samples were extracted from 133 voice‐disordered female patients and 97 non‐voice disordered female speakers and were signal typed prior to analysis. Praat software was used to measure harmonics‐to‐noise ratio (HNR), glottal‐to‐noise excitation ratio (GNE), the standard deviation of fundamental frequency (F0SD) and cepstral peak prominence (CPPp); and the Analysis of Dysphonia in Speech and Voice (ADSV) program was used to measure CPPadsv, low/high spectral ratio (LH) and the cepstral/spectral index of dysphonia (CSID). Outcome measures included sensitivity, specificity, and discrimination accuracy. Outcomes & Results As individual acoustic measures, only spectral‐based measures showed good (CPPadsv) and acceptable (CSID) discrimination results. The HNR, GNE and CPPp measures had acceptable sensitivity but poor or non‐acceptable specificity and discrimination accuracy. Logistic regression models with all Praat measures (F0SD, HNR, GNE, CPPp) plus ADSV measures (CPPadsv, LH or CSID) provided excellent sensitivity, good‐to‐excellent specificity and excellent discrimination accuracy. ROC analysis for all individual measures showed that CPPadsv, CSID, CPPp, GNE and F0SD had the highest area under the curve (AUC) values. Conclusions & Implications A combination of acoustic measures that evaluate the major aspects of vocal dysfunction resulted in good to excellent voice discrimination outcomes. Individual acoustic measures had lower discrimination ability than combined measures. The findings implied that acoustic measures extracted from a prolonged vowel were useful in voice disorder discrimination. WHAT THIS PAPER ADDS What is already known on this subject Acoustic measures hold great value in discriminating voice disorders from normal voices. However, no study has evaluated discrimination values of a combination of sustained vowel acoustic measures that quantify additive noise, signal stability, signal periodicity, spectral slope and overall voice quality in single‐gender cohorts. Previous studies have not used signal typing (the classification of the acoustic signals) for time‐based measures, impacting the reliability of discrimination. What this study adds to the existing knowledge This study was the first to implement signal typing to include sustained vowel samples of Types 1 and 2 signals for discrimination statistics. We showed that a combination of vocal acoustic measures using time‐ and spectral‐based extraction from the sustained /ɑ/ vowel evaluating additive noise, signal stability, signal periodicity, spectral slope and overall voice quality resulted in good to excellent sensitivity, specificity and discrimination accuracy. As individual measures, traditional time‐based measures such as HNR had rather limited discrimination values whilst spectral‐based measures provided higher discrimination values. Measures that are sensitive to signal types have low discrimination ability. What are the potential or actual clinical implications of this work? The sustained vowel /ɑ/ is a relevant, universal vocal task for clinical application using acoustic measures to discriminate female speakers with and without voice disorders if signal typing is implemented. Clinical voice assessment using vowels may not be effective if relying solely on time‐based measurements. Spectral‐based measures perform better in voice disorder discrimination given their insensitivity to signal types. The most effective voice disorder discrimination could only be obtained using a combination of acoustic measures that quantify major phenomena in the signals of disordered voices. Using measures extracted from both programs, Praat and ADSV, is useful given that specific settings in a program may impact on discrimination accuracy.

[Validation of the French version of the situational characteristics questionnaire in the measurement of space and motion discomfort]

Abstract:Introduction: Space and motion discomfort (SMD) refers to various symptoms that occur in environments with unreliable visual and kinesthetic information that do not permit adequate spatial orientation. Some studies have demonstrated that there is a stable and predictable relationship between vestibular dysfunction and anxiety disorders. Further, vestibular dysfunction can predispose or trigger the development of panic disorder with or without agoraphobia (PD/A) or reinforce phobic avoidance. It therefore seems clinically useful to develop and validate instruments for evaluating SMD in various populations. Measuring SMD could facilitate identification of individuals with PD/A who present comorbid vestibular dysfunction. Jacob et al. developed and validated such a questionnaire: the Situational characteristics questionnaire (SitQ). This questionnaire evaluates the presence of symptoms such as dizziness, vertigo, and instability under specific conditions. The SitQ comprises two subscales that measure SMD and one subscale (agoraphobia) that measures agoraphobic avoidance behaviours. The instrument has two sections. The first section is composed of the SMD-I and agoraphobia subscales, containing 19 and seven items, respectively. Each item consists of two contrasting descriptors of a specific situation or environment. The respondent is required to indicate to what extent the two described situations or environments cause discomfort. Each item includes a "criterion" descriptor for the situation (i.e., a descriptor that is presumed to engender SMD) and an alternative (non-criterion) descriptor. The second section comprises the SMD-II scale; this scale is composed of nine criterion situations, for which non-criterion situations are not supplied. The instrument takes approximately 20 minutes to complete. Objective: The present study focuses on the validation of the French-language version of the SitQ: the questionnaire des caractéristiques situationnelles (QCS). Method: The sample was composed of French Canadians recruited across Quebec from an anxiety disorders treatment clinic, general psychiatric care clinics, a community organization for individuals with anxiety disorders, advertisements in local newspapers, and ads posted in various public locations. The sample included 141 participants who met the criteria for lifetime PD/A. Participants reported current PD/A (n=73) or PD/A in remission (n=68). The control sample was recruited from undergraduate courses in various disciplines. Two hundred and thirty-five (n=235) students completed the questionnaires. Data from 63 (26.8%) participants were excluded from the analyses due to failure to complete all of the research questionnaires. Results: Analysis of the global descriptive data and the descriptive data for each dependent variable revealed that the data were independent of sociodemographic variables and respected the assumptions of normal distribution (skewness and kurtosis). Parametric tests were subsequently conducted. Using the combined data from the control and clinical groups, the internal consistency of the scales was analyzed using Cronbach's alpha. The SMD-I and SMD-II scales demonstrated good homogeneity. The results were comparable or superior to those obtained with the English-language version of the questionnaire. The agoraphobia scale demonstrated weaker internal consistency and corresponding weaker homogeneity. This result was consistent with that of the original version of the agoraphobia scale; this scale was eliminated for the subsequent analyses. Construct validity was analyzed via t-tests comparing clinical and control groups. Effect sizes were estimated using percentage of variance explained. The SMD-I scale demonstrated weak construct validity and was also eliminated from subsequent analyses. The SMD-II scale demonstrated good construct validity and provided an adequate measure of the theoretical construct of SMD. This scale permitted discrimination of participants according to the presence or absence of PD/A. It is therefore possible to identify participants with PD/A by their level of SMD. This result is comparable to that of Jacob et al. Conclusion: The results of the present study are generally consistent with the results of the validation of the original version of the questionnaire. However, the SMD-I and agoraphobia scales in the French-language version of the measure did not achieve a level of significance sufficient to definitively establish validity.
Validation of the Japanese Version of the Singing Voice Handicap Index

Abstract:Purpose: The aim of the present study was to translate the Singing Voice Handicap Index (SVHI) into Japanese and validate the Japanese version of the SVHI. Methods: The SVHI was translated into Japanese from the validated original version, and the questionnaire was administered to 102 singers with voice problems and 88 healthy singers. Internal consistency and test-retest methods were implemented to evaluate the reliability of this index. The internal consistency method assessed validity via Cronbach's α, and test-retest reliability was analyzed by the intraclass correlation coefficient (ICC) and limits of agreement (LoA) according to the Bland Altman method. Construct validity was verified by confirming correlations between SVHI scores and visual analog scale (VAS) scores for disability in singing using Spearman correlation. Discriminant validity was evaluated by comparing SVHI scores between singers with voice problems and healthy singers using t tests. Using the Tukey's honestly significant difference (HSD) test, we also compared the Voice Handicap Index (VHI) and SVHI scores among three groups: healthy singers, singers with voice problems solely during singing, and singers with voice problems during both speaking and singing. Results: The Japanese version of the SVHI showed excellent internal consistency (Cronbach's α = 0.981) and test-retest reliability (ICC: 0.93). The 95 percent LoA was calculated to be between -20.8 and 33.9. Construct validity was verified through correlated SVHI and VAS scores (r = 0.736, P < 0.001). Discriminant validity was verified as the SVHI scores of singers with voice problems were higher than those of healthy singers (77.8±37.5 vs. 30.0±26.5, P < 0.001). There were no statistically significant differences in VHI scores between singers with voice problems solely during singing and healthy singers; however, the SVHI scores of singers with voice problems solely during singing were significantly higher than those of healthy singers (63.4±36.8 vs. 30.0±26.5, P < 0.001). Conclusion: We confirmed that the Japanese version of the SVHI is a valid and reliable self-rated questionnaire for measuring the patient-perceived impact of singing voice problems among Japanese singers.
Adaptation and Validation of the Persian Version of the Voice Handicap Index-10 (VHI-10)

Abstract:Objectives: Voice disorders can profoundly impact health, quality of life, job performance, and social interactions. Traditional evaluations have expanded to include quality-of-life assessments, emphasizing self-reported outcomes. The Voice Handicap Index (VHI) stands out among relevant questionnaires, with the VHI-10 being a concise derivative. This study was conducted to translate and validate the Voice Handicap Index-10 (VHI-10) questionnaire for Persian speakers, enhancing clinical assessments of voice disorders and quality of life. Methods: This cross-sectional study, conducted in Iran, involved (1) translating the VHI-10 into Persian, (2) confirming face and content validity using Content Validity Ratio (CVR), and Content Validity Index (CVI), and (3) evaluating its reliability through a survey. A panel of experts confirmed the validity, and reliability of the study, which was assessed using Cronbach's alpha, Spearman-Brown, and Guttman coefficients. The survey involved 225 participants, including 150 healthy people and 75 patients with voice disorders, who were selected using a convenience sampling method. Results: All question items demonstrated a CVI greater than 0.79 and a CVR between 0.62 and 1. Reliability analysis yielded high Cronbach's alpha values for functional, physical, and emotional domains (0.909) and total (0.961). The mean overall scores of VHI-10 for healthy and disordered groups were 18.78 and 0.74, respectively. The VHI-10 effectively discriminated between healthy and disordered groups in all domains, with an accuracy of 97.33%. The determined cut-off point was 4.5, with a strong area under the Receiver Operating Characteristic (ROC) curve (0.989). Conclusion: This study successfully adapted and validated the Persian version of the VHI-10. The questionnaire demonstrated high reliability and validity, distinguishing between individuals with and without voice disorders. This Persian version is now a valuable tool for speech and language pathologists conducting clinical voice evaluations in Iran and also it could be applied in studies to determine the effects of voice problems on participant's quality of life.
