Abstract:Introduction. The article discusses the features of nutrition and the formation of the metabolic syndrome in workers in dust-hazardous industries. The relevance and urgency of studying the relation of nutrition to the development of obesity, arterial hypertension, impaired carbohydrate and lipid metabolism is due to the steady progression of the prevalence of metabolic syndrome both in the population as a whole and among workers in hazardous industries. The purpose. Hygienic assessment of the actual nutrition in different age groups from the standpoint of the risk of developing metabolic syndrome in workers of the industrial enterprises. Materials and methods. Sixty four workers aged of 51.0±8.7 years, the average work experience being 18.4±6.9 years were surveyed in the mining and engineering industries,. The ones surveyed were divided into two groups taking into account their age: the first group included people from 30 to 44 years (28 people), the second group included people aged 45 to 60 years (36 ones). The research methods included the assessment of actual nutrition, anthropometry, measurement of blood pressure, clarification of the glycemic profile and lipid spectrum. Results. The actual nutrition of workers in hazardous industries was found to be off-balanced in terms of macronutrient intake. In the age group of 30-44 years, a significant increase in the risk of developing metabolic syndrome was determined due to alcohol consumption, exceeding the norm of added sugar. In the older group (45-60 years), the risk of developing metabolic syndrome increased due to the excessive amount of total fats in the diet and a deficiency of dietary fibers. Limitations. The study included 64 workers aged 30 to 60 years, the correlation analysis of the compared indicators was carried out using a quantitative scale, when calculating the odds ratio, the indicators were measured on a nominal scale. Conclusion. There are revealed age-related features of actual nutrition of workers in dust-hazardous occupations, resulting in the development of abdominal obesity and metabolic syndrome, requiring further study to develop effective measures for the nutritional prevention of cardiovascular diseases and type 2 diabetes mellitus.
Body mass index, blood pressure, and glucose and lipid metabolism among permanent and fixed-term workers in the manufacturing industry: a cross-sectional study

Abstract:BackgroundTemporary employment, a precarious form of employment, is recognized as social determinant of poor health. However, evidence supporting precarious employment as a risk factor for health is mainly obtained from subjective data. Studies using objective clinical measurement data in the assessment of health status are limited. This study compared body mass index (BMI), lipid and glucose metabolism, and health-related lifestyle factors between permanent workers and fixed-term workers employed in the manufacturing industry.MethodsData of 1,701 male manufacturing industry workers <50 years old in Japan were collected and analyzed. Anthropometric data were BMI, calculated using measured height and weight of study participants, and blood pressure. For lipid metabolism, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, and triglyceride levels were determined. For glucose metabolism, fasting plasma glucose and hemoglobin A1c (HbA1c) levels were measured. Multiple regression analysis adjusted for age and lifestyle factors was performed.ResultsBMI was significantly higher in permanent workers (22.9 kg/m2) compared with fixed-term workers (22.4 kg/m2). The leaner population (BMI < 18.5) was greater among fixed-term workers (8.3%) compared with permanent workers (4.0%), whereas the overweight population (BMI ≥ 25.0) was greater among permanent workers (21.4%) compared with fixed-term workers (18.1%). Although fixed-term workers tended not to be overweight, regression analysis adjusted for age and lifestyle factors suggested that fixed-term employment was significantly associated with higher blood pressure (systolic β = 2.120, diastolic β = 2.793), triglyceride (β = 11.147), fasting blood glucose (β = 2.218), and HbA1c (β = 0.107) compared with permanent workers (all p < 0.01).ConclusionsFixed-term workers showed more health risks, such as poorer blood pressure and lipid and glucose metabolism, even when adjusted for age and lifestyle variables, although BMI of fixed-term workers were lower than permanent workers. Precarious work might contribute to a deteriorating health status even among less overweight populations.

Abstract:Introduction. Dyslipidemia is one of the most common risk factors for cardiovascular disease (RFCVD). In coal industry workers, working conditions are considered as an additional risk factor to the well-known RFCVD. The study aims to analyze the prevalence of RFCVD in coal industry workers in comparison with the general population of the Kemerovo region. Materials and methods. The study included 2,356 male respondents, 1,656 (70.3%) coal industry workers (the main group) and 700 (29.7%) people from the population sample of participants in the multicenter epidemiological study of ESSE-RF included in the territory of Kuzbass (control group). The traditional FSSR was evaluated, including the parameters of lipid metabolism (total cholesterol (TCH), triglycerides (TC), low-density lipoproteins (LDL) and high-density lipoproteins (HDL)). Results. A comparative analysis of the FSSRP of coal industry workers and the control group showed their high prevalence in the control group: the prevalence of diabetes mellitus by 12.3 times, previous stroke by 4.8 times, myocardial infarction by 4 times, arterial hypertension by 3.8 times, obesity by 1.8 times, dyslipidemia by 1.2 times. The average values of systolic (SBP) and diastolic (DBP) blood pressure and plasma glucose levels were within the normal range in both groups, but statistically significantly higher in the control group. There were 1.3 times more smokers among coal industry workers (60.4% compared to the control group). 45.7%, p<0.001). The median of TCH and its fractions, TC were within the limits of the normative values for workers in the coal industry, whereas in the control group the LDL level was statistically significantly higher. The incidence of hypertriglyceridemia (34.2% vs. 22.4%) and low HDL (15.8% vs. 2.0%) was statistically significantly higher in miners. The researchers observed hypercholesterolemia and high LDL levels in more than half of the patients (50.6% and 65.3%, respectively) of the control group. A correlation analysis of work experience in the coal industry and the FSSR demonstrated an association with age (r=0.83, p≤0.001), body mass index (BMI) (r=0.37, p≤0.001), SBP level (r=0.33, p≤0.001) and DBP (r=0.36, p≤0.001), concentration of TCH (r=0.65, p≤0.001), LDL (r=0.51, p≤0.001) and TC (r=0.84, p≤0.001). The authors found that coal miners had an 11.5-fold increased risk of HDL reduction, 2.3-fold increased hypertriglyceridemia, and 2-fold increased risk of smoking compared to the population of the region. The proportion of people with hypertriglyceridemia and low HDL prevailed in all age groups of coal industry workers. Taking into account the work experience of more trained individuals, the risk of hypercholesterolemia and high LDL levels increases by 2 times, the risk of hypertriglyceridemia and low HDL levels is slightly lower. Limitation. The design of the study did not allow to determine the causal relationship between the profession and the RFCVD. The analysis of the nature of nutrition, intake/abuse of alcoholic beverages was not carried out and metabolic syndrome was not diagnosed as possible causes of dyslipidemia. Inclusion in the main and control groups was carried out for different periods of time. Conclusion. Smoking and dyslipidemia are among the most common RFCVD among workers in the Kuzbass coal industry. The largest proportion is hypercholesterolemia, one third has hypertriglyceridemia and elevated LDL, half as often as low HDL. The duration of work in underground conditions is associated with age, BMI, SBP, DBP, concentrations of TCH, LDL, TC. The proportion of people with hypertriglyceridemia and low HDL prevails in all age groups of coal miners. In more trained workers, the risk of hypercholesterolemia and high LDL increases by 2 times, the risk of hypertriglyceridemia and low HDL is slightly lower. Ethics. The study was performed in accordance with the standards of good clinical practice and the principles of the Helsinki Declaration. Prior to inclusion, all respondents signed an informed consent form.

Abstract:One third of Chinese exporters sell more than ninety percent of their production abroad. We argue that this distinctive pattern is attributable to the widespread use of subsidies that require firms to export the vast majority of their output. We study this type of subsidy in the context of a heterogeneous-firm model, and show that it is worse from a welfare standpoint than a regular export subsidy, partly because it increases protection of the domestic market. A counterfactual analysis suggests that eliminating these subsidies would result in a welfare gain for China comparable to that of halving its trade costs.

Abstract:Introduction. Cardiovascular diseases (CVD) are a leading cause of morbidity and mortality. The role of occupational hazards in the CVD prevalence remains to be clarified. Material and methods. Here we report the results of the study of risk factors and CVD prevalence in 590 workers at the largest copper production plants in the Sverdlovsk region, exposed to heavy metals in the workplace. The workers` health information was obtained during a regular medical examination in 2018. The lead concentration increase to 1.3-1.8 occupational exposure limits was registered in the working areas of the concentrating mill (for bunkerman) and copper smelting workshops (transporter, smelter, converter, non-ferrous metal spreader, repairman, electrician). Results. We studied the exposure indices (Pb level in blood), the response markers (reticulocyte count, erythrocytes basophilic stippling, coproporphyrin, and aminolevulinic acid in the urine), and their correlation to a working tenure. Based on this analysis, we attributed CVD risk factors and cardiovascular diseases to the occupation, in order to potentially modify some of those risk factors and ultimately inform the risk management. Hypertension occurred in 57% of the examined workers, which is higher than in the general population. We calculated relative risk, confidence intervals and attributable fraction. We developed a predictive mathematical model (stepwise logistic regression) to predict high-stage hypertension and identified the risk factors associated with its development. Conclusions. Correlation analysis revealed direct correlations between stages 2 and 3 hypertension and a working tenure over 20 years. We think it’s reasonable to consider the documented CVDs as related to the toxic effects of heavy metals (lead and cadmium).

Abstract:Significance. The conducted studies are devoted to the analysis of the factors of disease development and prevention of major non-communicable diseases, the most common of which are cardiovascular diseases. The relevance of this work is determined by the possibility and need for timely identification of pre-pathological and pathological conditions, in order to develop optimal schemes of preventive measures to preserve professional longevity of workers. The purpose of the study was to develop a program of preventive measures based on the risk assessment of cardiovascular diseases in workers of metallurgical production, exposed to a complex of occupational risk factors. Material and methods. Results of periodic medical examination of workers of certain professional groups of a metallurgical enterprise have been analyzed. To determine the total fatal cardiovascular risk among workers, the SCORE (Systematic Coronary Risk Evaluation) was used. Results. A general sanitary audit of the working conditions of the metallurgical plant workers corresponds to hazardous class 3, degree 2-3 (3.2-3.3). According to the survey results, tobacco smoking is common among more than half of the surveyed workers - 56.8%. Among the professional group of temperers, tobacco smoking is more common (67.0%) compared to the professional group of wire-drawers (50.0%) and the control group (25.6%). The average body mass index among workers was 26.1±1.7 kg/m2, and 27.8±1.5 kg/m2 in the control group. Analysis of the average cholesterol group level was within the reference values of 4.8±0.1 mmol/l. 31.6% of the workers had high and very high levels of fatal cardiovascular risk, and 14.1% - a low one. Conclusion. Preventive measures to preserve professional longevity of metallurgical production workers should be carried out based on the cardiovascular risk assessment using the developed programs. Scope of application. The developed algorithm for selecting medical measures with due regard to cardiovascular risk assessment can be used as a corporate program for preserving health of the working population.

Abstract:Introduction. The main cause of the able-bodied population’s death in the Russian Federation is cardiovascular diseases. It is known that one of the most important factors of cardiovascular risk is a violation of lipid metabolism, which is an indicator of atherogenesis processes activation. In this regard, it is relevant to study the lipid spectrum and its effect on the cardiovascular risk’s level for workers of various industries, who are exposed to a complex of harmful production factors. The study aims to investigate lipid metabolism and atherosclerotic processes in chemical production workers for the opportune implementation of therapeutic and preventive measures. Materials and methods of research. Comprehensive clinical and hygienic studies in the chemical industry have been carried out. Hygienic studies and assessment of working conditions of employees were accomplished using the current regulatory documents. Surveyed were divided into 3 groups: machine operators (I group), machinery repair locksmiths (II group), control and measuring devices’ locksmiths (CMD) (III group - comparison). To identify risk factors for cardiovascular diseases (CVD), a screening examination was conducted within the framework of periodic medical examinations of 3761 male workers, a study of the lipid spectrum in 846 workers. Employees with high and very high cardiovascular risk according to the SCORE system underwent in-depth medical examination using clinical laboratory, functional, ultrasound examination methods (179 people). The relative risk and etiological proportion were calculated in order to establish a causal relationship between the complex of harmful production factors and the formation of hypercholesterolemia. Results. The data of the conducted studies showed that the factors of cardiovascular risk in workers of I and II groups have harmful working conditions on the workplace (general class of working conditions 3.2), smoking, obesity, hypertension, which are more common in the group of machinery repairmen which was compared with workers of other professions. It was found that the average values of total cholesterol in blood serum, LDL cholesterol were higher in the examined I-II groups than III group. Analysis of the state of the cardiovascular system main indicators for workers of various professions obtained as a result of in-depth examination in a hospital revealed, that signs carotid arteries atherosclerotic changes were more often registered in workers of I-II groups. The revealed indicators of dyslipidemia and atherosclerosis increased in proportion to the length of service in harmful working conditions. Conclusion. The conducted research allowed to substantiate the algorithm of stage-by-stage examination, dynamic monitoring of the chemical production workers’ health’s state in order to detect atherosclerosis earlier and integrate therapeutic and preventive measures timely.
Evaluation of the selenium deficiency impact on metabolic parameters of people of working age

Abstract:Background. Among the population living in Russia, the frequency of selenium deficiency in the body is more than 80%. An unbalanced diet in terms of the level of selenium supply to the body leads to metabolic disorders among the adult population. Aim. Study of the relationship between the insufficient level of selenium supply of the body in different periods of the year and changes in body composition, basal metabolism among the adult population aged 2550 years. Material and methods. In the autumn, winter and spring periods, the adult population aged 2550 years, living in the Leninogorsk district of the Republic of Tatarstan, of the 1st group of physical activity (n=143, of which 47% were men) was examined. The body composition was assessed using the ABC-01 Medass analyzer for bioimpedansometry. The content of selenium in the hair was studied using an atomic emission spectrometer. An assessment of the obesity and overweight prevalence in the adult population with selenium content below the reference values and the boundaries of the 95% confidence interval was made. Laboratory studies of dishes and foodstuffs for selenium content were carried out using an atomic absorption spectrometer. To assess the actual consumption of food products and dishes, the method of 24-hour (daily) reproduction of nutrition by questioning respondents on a 14-day menu was used. Statistical processing of the studied indicators was performed by the Spearman correlation method, mathematical and centile methods. Results. In bioassays with a low level of selenium in the hair (25 centile interval), the largest proportion of subjects with selenium deficiency was noted in autumn in more than 60% of cases, both among men with a 95% confidence interval Z-score=248, and among women with 95% confidence interval Z-score=240, while in spring only 18% and 42% at 95% confidence interval Z-score=72 and 168, respectively. When studying the relationship of selenium content in hair with morphofunctional indicators, significant correlations were found using the Spearman method with body mass index (RNovember=0.389), active cell mass (RFebruary=0.260; RMay=0.318), and basal metabolism (RMay=0.313; RNovember=0.294). Analysis of diets revealed a lack of selenium content in food products and ready-made meals in the autumn-winter period in both men (4046%, n=67) and women (5461%, n=76), in spring-summer period in 35 and 42% of cases, respectively. The lower the level of selenium in the hair (in the body), the higher the body mass index, the lower the active cell mass and the basal metabolism. There is no correlation with fat and skeletal muscle mass. The basic exchange is reduced at an insufficient of selenium supply in the body. Conclusion. The results indicate an insufficient intake of selenium in the body with a daily diet and the relationship of body mass index, basal metabolism with an insufficient level of selenium in the body.

Abstract:The commonality of the goals of occupational health and safety and medicine is to ensure working conditions that contribute to the preservation of the employee health. For professions, in which it is not possible to exclude the possibility of a negative impact of occupational hazards and harmfulness, timely detection of health disorders and medical restrictions on the performance of certain types of work help to preserve the worker health. However, in the occupational medicine, little attention is paid to the subjective assessment of the health of industrial employees. The purpose of the study is to establish the degree of influence on the psychosomatic state of the mining and processing plant underground workers of working conditions associated with the profession, experience, the presence of an assigned preferential pension, as well as comparison with similar indicators for workers in other manufacturing industries. The analysis of the subjective assessment of the quality of life of miners was carried out using the Russian version of the SF-36 questionnaire, according to the results of which the self-assessment of the quality of life of miners admitted by the medical commission to work exceeds 50 points, for those who did not pass the medical commission (23.28 % of the total) subjective indicators of physical and mental health are less than 50 points. There is a trend towards a decrease in the quality of the life indicators with an increase in length of service to the minimum values by 35 years of work, which is noted in the published questionnaire studies of the highly-skilled workers of the enterprises in other branches of industry. In the group of highly-skilled workers over 36 years, the majority of whom are pensioners (less than 60 years, according to the legislation of the Russian Federation) of pre-retirement age, the workers with physical and mental health indicators comparable to the self-assessment of the quality of life of miners who worked at the enterprise for less than 20 years remain in the profession. A review of studies at other enterprises with harmful and hazardous working conditions allows to judge that the decrease in the physical and mental tone of the quality of life of the highly-skilled workers is aggravated by the presence and severity of the diseases received, as well as the level of industrial potential of the enterprise. The obtained results of the objective and subjective dynamics of the health status of workers in harmful and hazardous working conditions can form the basis for the development of preventive measures to reduce occupational risks.

Abstract:Metastasis to the oral cavity from cutaneous melanoma is rare: fewer than 30 cases of metastatic melanoma to the palatine tonsil have been reported. Tonsil metastasis is haematogenously disseminated and therefore usually has a poor prognosis. We present a case of metastatic melanoma to the palatine tonsil occurring 6½ years after removal of the primary cutaneous lesion. The patient has remained disease‐free for 18 months since the removal of skin and tonsil metastases. Immunohistopathologically, HLA class II and costimulatory factor B7‐2 molecules were concomitantly expressed on melanoma cells: we suggest that the patient was therefore able to develop antimelanoma T‐cell activation resulting in prevention of further metastasis, and thus a favourable prognosis.

Rational Pharmacotherapy in Cardiology

Abstract:Aim. To analyze prevalence of cardiovascular risk factors in the Kemerovo region based on the results of epidemiological studies (2013 and 2016). Material and methods . The study was based on two large epidemiological studies of the Kemerovo region: on 2013, «The epidemiology of cardiovascular diseases and their risk factors in the Russian Federation» and on 2016, «The prospective study of urban and rural epidemiology: study of the influence of social factors on chronic non-infectious diseases in low, middle and high income countries». In the study we analyzed cardiovascular risk factors using identical questionnaires, functional, anthropometric, biochemical means and measured on identical scales. As a result, we analyzed the prevalence of smoking, diabetes mellitus, overweight and obesity, abdominal obesity, hypercholesterolemia and hypertriglyceridemia, high levels of low-density lipoprotein (LDL). Results. Univariate analysis indicates that in the sample of 2016, compared to the sample of 2013, the prevalence of smoking is statistically significantly lower, as well as the proportion of participants with high cholesterol levels, but not taking lipid-lowering drugs. In contrast, the prevalence of diabetes, hypercholesterolemia and hypertriglyceridemia is higher. In women, the frequency of abdominal obesity on 2016 is lower than on 2013: at 35-44 age group odds ratio (OR) =0.67 with 95% confidence interval (CI) 0.44-1.03, at 45-54 age group OR =0.47 with 95% CI 0.31-0.72, 55-65 age group OR =0.49 with 95% CI 0.30-0.79. A high incidence of diabetes, hypercholesterolemia and hypertriglyceridemia is characteristic mainly of older women (55-65 age group): accordingly, OR =1.96 with 95% CI 1.19-3.22, OR =1.42 with 95% CI 1,02-1.97, OR =1.51 at 95% CI 1.08-2.12. In the 45-54 age group of men, they smoked statistically significantly less often on 2016 compared to 2013, OR =0.59 with 95% CI 0.36-0.96. The prevalence of overweight and obesity in both samples is the same: for women, the OR for overweight in different age groups is within 0.74-0.87, for men - within 0.95-1.78; for obesity OR in women is from 0.70 to 0.79, in men - from 1.03 to 1.34. Conclusion. A significant advantage of the study is the analysis of changes in prevalence in age and gender groups, which showed significant differences in the dynamics of men and women in different age categories for a number of risk factors. Analysis of the dynamics of the prevalence of cardiovascular risk factors makes it possible to assess the effectiveness of state and regional policies in the field of health protection and, first of all, "risk groups” that require closer attention, development and implementation of targeted health-saving technologies.

Abstract:Introduction. Due to the constant presence of harmful chemical components in the air of the working area at an oil-producing enterprise, the length of work experience exacerbates their negative impact on the human body, which can lead to dysmetabolic disorders. The purpose of the work is to assess changes in the biochemical parameters of metabolism in workers of an oil producing enterprise. Materials and methods. A study and assessment of the content of polycyclic aromatic hydrocarbons in the bioenvironments of workers, biochemical indicators of metabolic processes, statistical analysis, modelling of cause-and-effect relationships were carried out. Results. In oil and gas production operators, benzene and o-xylene are identified in the blood, the concentration of which is up to 1.5 times higher than that of workers not associated with the oil production process. There is and gain in the level of benzene in the blood up to 1.4 times, depending on the increase in work experience. Significant differences were found in the content of C-peptide, glucose, total cholesterol, HDL cholesterol, LDL cholesterol and triglycerides in oil and gas production operators relative to similar indicators in workers not exposed to chemical production factors of the enterprise. Among workers involved in oil and gas production, a dependence of an elevation of glucose and C-peptide in the blood serum on an increase in the concentration of benzene in the blood and the length of work experience was established; an increase in the level of total cholesterol, LDL cholesterol, triglycerides in the blood serum - only depending on the increase in work experience. Limitations. The conducted study takes into account the influence of only chemical factors and the duration of work experience on the health of oil production workers. Conclusions. The constant presence of polycyclic aromatic hydrocarbons in the air, in particular benzene and O-Cixille (up to 1.5 times higher than the MPC of the working area), leads to their increased concentration in the blood. In operators of oil and gas production there was established an increase in the level of a number of biochemical indicators of the negative effects of metabolic syndrome, which are dependent on the length of service and the content of benzene in the air of the working area.

Abstract:Details of the substrate, p-hydroxybenzoate, and substrate analog, p-aminobenzoate, binding to p-hydroxybenzoate hydroxylase have been elicited by Raman difference spectroscopy. Deep red (752 nm) excitation was used to avoid interference from a fluorescence background. The Raman data provide information on changes in the ligand upon binding as well as changes in the flavin ring system of the enzyme in the enzyme-substrate complex. For p-aminobenzoate, its three most intense Raman features, due to a phenyl mode (1607 cm-1) and carboxylate stretching (1383 cm-1) and scissoring (863 cm-1) motions, are little perturbed upon binding and show no changes in the pH range 6.5-8.5. However, changes in a number of spectral features associated with isoalloxazine modes in this pH range are evidence for a protonation/deprotonation event occurring in or near the active site. A feature in the difference spectrum of the complex at 1700 cm-1 is assigned to the stretch of the 4C&dbd;O group of the isoalloxazine; the relatively narrow profile of this feature is due to the ring being held in a rigid network of hydrogen bonds as demonstrated by the X-ray-derived structure [Schreuder, H. A., Prick, P. A. J., Wierenga, R. K., Vriend, G., Wilson, K. S., Hol, W. G. J., & Drenth, J. (1989) J. Mol. Biol. 208, 679-696]. The absence of a corresponding negative band in the spectrum near 1725 cm-1 shows that in the enzyme, in the absence of ligand, the 4C&dbd;O peak is "washed out" by a fluctuating series of hydrogen bonds to water molecules which penetrate to the flavin ring, resulting in a broad C&dbd;O stretching feature which escapes detection in the difference spectrum. For p-hydroxybenzoate, upon complexation, the -COO- symmetric stretch shifts 10 cm-1, which is ascribed to the formation of the salt bridge to the guanidinium of Arg 214, seen in the X-ray structure. This is in contrast with the results for the complex involving the p-amino analog where no shift in the carboxylate mode is detected and demonstrates an advantage of using vibrational spectroscopy as a fine probe of active site interactions, since the X-ray structures for the p-amino and p-hydroxy analog complexes indicate that the structures in the -COO- group guanidinium regions are the same. The Raman difference data for the substrate complex in the 1700 cm-1 region closely resemble those for the p-amino analog, indicating that in both cases the 4C&dbd;O group is participating in a rigid hydrogen bonding network in the complexes with ligand but is in a more dynamic hydrogen bonding environment involving water molecules in the unliganded enzyme. In order to measure the pKa of the -OH group in bound p-hydroxybenzoate, the substrate was labeled with 18O in both -COO- oxygen atoms. By subtracting the Raman spectrum of the complex with labeled substrate from that with unlabeled substrate, a simple difference spectrum was obtained with features involving the -COO- group alone. These features were used to measure the pKa of the ring hydroxyl group which was found to be 8.3. The value determined from absorption spectroscopy is 7.4, and possible reasons for the discrepancy are discussed. Both methods are in accord, however, in that they show that the pKa of the bound substrate is substantially below that for the free, a device which assists in the hydroxylation at the 3-position.

Abstract:An analysis of occupational risks to the health of workers, the fundamental components of which are injuries and occupational morbidity, is extremely relevant, primarily for those industries where working conditions remain difficult and harmful and there are restrictions on hiring for health reasons, including at mining enterprises mineral. The purpose of the work is to study the consequences of exposure to harmful and (or) dangerous working conditions for employees of the main types of economic activity and mining of the Republic of Bashkortostan for the period 2011–2020. and comparing them with similar data for the Russian Federation. Sources of information — statistical reports, reports and materials published on the official websites rosstat.gov.ru, rospotrebnadzor.ru, 02.rospotrebnadzor.ru, bashstat.gks.ru, mintrud.gov.ru, mintrud.bashkortostan.ru. The average long-term level of occupational morbidity in Russia for the study period was 1.45 per 10 thousand workers, in the Republic of Bashkortostan — 0.910/000. A steady decrease in intensive indicators of occupational morbidity in dynamics by 2.5 times in Russia and almost 3 times in the republic was noted. The average annual rates of occupational morbidity in the production of minerals in the country ranged from 21.2 in 2019 to 32.75 in 2013 per 10 thousand employees (average 29.04 0/000), in the republic — 6.0 per 10 thousand employees, which is 7.3 times higher than the national average. The average number of victims with disability for all types of economic activity in Russia was 1.2 times higher than the national indicator, the average injury severity indicator in the region exceeded the same value in the country by 11.4 %. An excess of the average long-term indicator of the frequency and severity of injuries at the country's mining enterprises in comparison with the indicators of general injuries was noted. The data convincingly testify to the existing problems in the production of minerals, causing increased rates of occupational morbidity and occupational injuries with severe outcomes.

Abstract:Introduction. The leading place in the Russian Federation and in the world is occupied by cardiovascular diseases (ischemic heart disease, hypertension, and peripheral vascular diseases), which are the main cause of disability and death worldwide. The aim of the research was to study the prevalence of cardiovascular pathology in workers of the aluminum industry. Material and methods. We examined 66 workers of the aluminum facility without occupational pathology and 135 employees who never worked in adverse working conditions (the control group). All the examined subjects were men; the average age accounted for 52.08±0.4 years. Results. The study showed the workers of the aluminum facility to develop such diseases of the cardiovascular system as arterial hypertension, atherosclerosis with lesions of peripheral and extracranial arteries, as well as multifocal atherosclerosis more often than in those who do not work in adverse working conditions (the control group). Conclusion. The unfavorable occupational factors of aluminum industry are an additional risk factor for the development of cardiovascular pathology.
The Assessment of the Nutritional Status among the Young-Old and Old-Old Population with Alimentary-Dependent Diseases

Abstract:Background and Objectives: Unhealthy nutrition can contribute to the development or progression of various alimentary-dependent diseases, including obesity, type 2 diabetes mellitus, metabolic syndrome, anaemia, and arterial hypertension. Young-old and old-old individuals often have diets deficient in essential vitamins, minerals, and macronutrients, characterized by high consumption of carbohydrate-rich foods and insufficient intake of plant-based products like vegetables and fruits. This study aims to identify key parameters of nutritional status among the young-old (aged 60-74 years) and old-old (aged 75-90 years) populations in central Kazakhstan, particularly in relation to specific alimentary-dependent diseases. Materials and Methods: The study involved 300 participants aged 60-90 years. The study incorporated a dietary questionnaire, food consumption records (such as 24 h recalls), and measurements of anthropometric indicators including weight and skinfold measurements. Results: Residents in the surveyed regions typically consumed food 3-4 times daily, with breakfast, lunch, and dinner being eaten at consistent times. A significant proportion of individuals, especially older adults, followed this meal schedule. About one-third ate before bedtime, and more than half believed they adhere to a specific diet. The mean BMI for men aged 60-74 years was 28.3 (95% CI: 20.1-43.2) and, for those aged 75-90 years, it was 29.0 (95% CI: 22.1-40.8). Caloric intake among individuals aged 60-74 was higher compared to those aged 75-90, with males consuming an average of 2372.7 kcal and females consuming 2236.78 kcal versus 2101.5 kcal for males and 2099.9 kcal for females in the older age group. Conclusions: The dietary patterns observed among old-old individuals were marked by excessive calorie intakes and imbalances in macronutrient composition, with a predominant emphasis on high-carbohydrate foods at the expense of essential nutrients like proteins, fats, and key vitamins (such as C, E, B vitamins) and minerals (such as potassium, calcium, and iron).

Abstract:Introduction: Trace elements found in natural drinking water can play a key role in making up the chemical composition of the human body in the area of residence. Objective: To study the relationship between indicators characterizing the elemental status and oxidative metabolism of residents of the Khanty-Mansi Autonomous Okrug and poor-quality tap water treatment. Materials and methods: Hair concentrations of iron, manganese, calcium, magnesium, zinc, and selenium were established in 155 regional residents using atomic emission and inductively coupled plasma mass spectrometry. Contents of the products of lipid peroxidation (lipid hydroperoxide and thiobarbituric acid reactive substances) and antioxidant defense of the body (total antioxidant capacity and thiol status) were determined in blood serum using test kits. The oxidative stress index was calculated as lipid hydroperoxide × thiobarbituric acid reactive substances / total antioxidant capacity × thiol status. Results: The residents of the northern Russian cities notorious for poor-quality water treatment have elevated hair levels of Fe and Mn (p < 0.001) and a decreased Se concentration (p = 0.012) combined with a statistically higher level of lipid hydroperoxides, thiobarbituric acid reactive substances, oxidative stress index (p < 0.001), and a low level of antioxidant defense of the body (p < 0.001–0.002). We established a direct correlation between selenium levels in hair and indicators of antioxidant defense status (r = +0,784 ... r = +0,531) and an inverse correlation with lipid peroxidation parameters (r = –0,679 ... r = –0,465). Accumulation of iron and manganese in the human body is closely related to activation of lipid peroxidation (r = +0,472 ... r = +0,413) and suppression of the antioxidant defense status (r = –0,521 ... r = –0,379). Conclusion: The observed relationships between the parameters of oxidative metabolism give evidence of a weakening of antioxidant protection and activation of lipid peroxidation in the population of the cities of the Khanty-Mansi Autonomous Okrug supplied with poor-quality tap water.

Abstract:Introduction. In recent years, the use of rare earth elements has been expanding in various innovative research and high-tech industries, which leads to an increase in the volume of their extraction and processing and makes it possible to develop a negative impact on the health of workers under conditions of long-term industrial exposure to rare earth elements with the air of the working area. The purpose of the work is to study and evaluate the deviations of the biochemical parameters of the lipid spectrum in workers, due to the chemical factors of the technological process of processing raw materials and production of rare metal products. Materials and methods. The study and evaluation of the content of rare earth elements in the blood of workers, the study of biochemical parameters, statistical analysis, modelling of cause-and-effect relationships were carried out. Results. Workers exposed to rare-earth elements with the air of the working area (100% of workplaces) showed increased concentrations of holmium, dysprosium, yttrium, neodymium, niobium and terbium in the blood from 1.5 to 2.5 times compared to similar indicators in workers who were not associated with technological process. Elevated levels of total cholesterol, VLDL and triglycerides in blood serum, a decrease in the total antioxidant activity of blood plasma in workers relative to the physiological norm and similar indicators in the comparison group were established. A significant relationship of the identified deviations with an increased blood concentration of niobium, dysprosium and holmium has been proven. Limitations. The study took into account the aerogenic inhalation effect of rare earth elements on the health of workers involved in the processing of raw materials and production of rare metal products. Conclusion. Long-term inhalation exposure to rare earth elements contained in the air of the working area caused changes in biochemical parameters, reflecting the depletion of antioxidant defense resources, inducing disorders of lipid metabolism. The identified negative effects in workers can lead to pro-atherogenic vascular inflammation under conditions of continuing industrial exposure. The obtained results should be taken into account for the development of recommendations aimed at minimizing the negative consequences of the impact of the studied chemical factors of the labour process on the health of workers.

Abstract:Introduction: An urgent hygienic problem in the regions with intensive industrial development is the persistence of ambient air pollutants and their adverse health outcomes in the population leading to higher incidence rates of non-communicable diseases and their chronicity. Objective: To substantiate the relationships between high blood levels of copper, nickel, and chromium and biomarkers of their adverse effects on blood, respiratory and immune systems for the tasks of early detection and prevention of poor health outcomes of the combined inhalation exposure to copper, nickel and chromium oxides on the example of the adult population at risk. Materials and methods: We carried out a hygienic assessment of ambient air quality in residential areas and analyzed general population incidence and prevalence for 2016–2020. We also examined the health status of women of reproductive age environmentally exposed to nickel, chromium and copper oxides in the area of residence and of relatively unexposed controls. About 5,000 chemical, clinical, immunological, and biochemical tests were done in 2022. Results: We established associations between higher blood levels of copper, nickel, and chromium and changes in the indicators of adverse health effects. Biomarkers of nonspecific and specific sensitization, chronic inflammation accompanied by an imbalance in the humoral and cellular immunity were hyperproduction of eosinophils in the nasal mucus, blood lymphocytes, specific IgG to copper, a decrease in the phagocytic index and number; lower ferritin marked disorders of iron homeostasis, higher malondialdehyde indicated induction of oxidative processes, and a decrease in antioxidant activity gave evidence of depletion of the total antioxidant activity. Conclusions: It is expedient to use our findings for early detection and subsequent substantiation of targeted measures aimed at preventing disorders of the respiratory and immune systems, blood, and certain disorders involving the immune mechanism, etiopathogenetically related to the combined exposure to the metal oxides tested.

Abstract:The metabolic syndrome is currently becoming more common. It is a significant public health concern as it is epidemic affecting populations in many regions of the world. In Magadan region no research has been carried out to study the frequency of components of the metabolic syndrome among northerners. This survey was performed to assess the occurrence of the main and additional components of the metabolic syndrome among 17 to 74 year old residents of the Northern region. Two hundred and forty north born Caucasians participated in the study: male subjects at their young age, working age, and retirement age, all belonging to the territory of Magadan region. We used photometric, immunochemiluminescent research methods, as well as standard methods for assessing body mass index and cardiovascular system. The metabolic syndrome factors were determined in accordance with the criteria of the National Cholesterol Education Program (NCEP), the Adult Treatment Program III (ATP III), the International Diabetes Federation (IDF), and the Consensus of International Experts in Cardiology and Endocrinology. We analyzed five main components of the metabolic syndrome (overweight, carbohydrate metabolic impairments hypertension, hypertriglyceridemia, hypoalphacholesterolemia) and three additional components (presence of insulin resistance, purine metabolism disorder, deficient and insufficient concentrations of vitamin D). Combinations of the components were also studied through the examined age groups. According to the ATP III, NCEP and IDF criteria, the metabolic syndrome was more common in elderly subjects (47%) than in working age (21%) or young examinees (3%). Older males tended to exhibit higher frequency of both the main and additional factors of metabolic syndrome. The total index of the occurrence of metabolic syndrome factors in the group of young men was 101%; in the group of men of working age - 180%, and in men of retirement age - 274%. The results on occurrence of the metabolic syndrome components observed in the surveyed groups of northerners can make an information data base, which we assume can be applied when planning and carrying out scientifically grounded preventive measures, which will improve subjective quality of life and its expectancy under the North conditions.
