Abstract:OBJECTIVE To clarify whether neonatal jaundice may cause myocardial damage to term infants with normal birth weight (BW). METHODS Totally 178 term neonates admitted during March, 2004 to December, 2010 with normal BW were enrolled. Infants with antenatal or neonatal asphyxia, temperature abnormality, septicemia, antenatal viral infection, congenital dysmorphia, congenital heart disease, 21-trisomy, and polycythemia were excluded. There was no maternal complications during the pregnancy. Serum total bilirubin (TB), creatine kinase (CK), MB isoenzymes of creatine kinase (CK-MB), and cardiac troponin-I (cTnI) were measured. Patients with transcutaneous bilirubin level (TcB) ≥ 342 µmol/L (20 mg/dl) were in Group A (n = 32), and those with TcB below phototherapy level at matched time point were in Group B (n = 25). ECG, for correct Q-T intervals (QTc) and correct QT intervals dispersion (QTcd), and ECHO, for left ventricular ejection fraction (EF), the ratio of the peak velocity of early stage and advanced stage of diastolic phase at the mitral orifice (E/A), were applied to patients in Group A and B. SPSS 13.0 software was used for the data analysis. The coefficients of correlation among age in hours on admission (hr), TB, CK, CK-MB, CK-MB/CK, and cTnI were studied by multiple and partial correlation analysis. Data in Group A and B were compared by independent-samples Mann-Whitney U test (nonparametric method) or Student t-test. RESULTS When the data were analyzed by multiple correlation, there were significant correlation between TB and cTnI, CK-MB, respectively (r = 0.212, -0.161, respectively, all P < 0.05). But, when the data were analyzed by partial correlation, there was no correlation between TB and cTnI, CK-MB, respectively (r' = 0.112, -0.112, respectively, all P > 0.05), negative correlation between hr and TB, cTnI, respectively (r' = -0.490, P = 0.000; r' = -0.162, P = 0.032). There was no significant difference in CK (Z = -1.384, P = 0.166), CK-MB (Z = -0.821, P = 0.412), cTnI (Z = -1.159, P = 0.246), QTc (t = 1.146, P = 0.257), QTcd (t = 1.342, P = 0.185), EF (t = 1.558, P = 0.125), E/A (t = -0.640, P = 0.525) between group A and B. There was significant difference in CK-MB/CK (Z = -3.187, P = 0.001) between group A and B with a lower value in group A [0.075 (0.032 - 0.102)] comparing to that in group B [0.160 (0.073 - 0.284)]. CONCLUSION There is no sufficient evidence to support the hypothesis that neonatal jaundice may induce myocardial damage in normal birth weight term infants.

Abstract:The author mistakenly feels that while mentioning Indian work on chyluria, a significant work on povidone‐iodine instillation published in the British Journal of Urology in 2004 was omitted.[1] We simply aimed to highlight that research on chyluria has been scant from India, especially when India contributes to 40% of the global burden of lymphatic filariasis.[2] To justify this statement, giving examples of 2016 and 2017, we mentioned that only a handful of articles were published on this topic from India. We do not mention anything about 2004 or the many other articles that have been published from India on chyluria in the last many years.
Efficacy of ramucirumab combination chemotherapy as second‐line treatment in patients with advanced adenocarcinoma of the stomach or gastroesophageal junction after exposure to checkpoint inhibitors and chemotherapy as first‐line therapy

Abstract:What's new? Evidence suggests that immunochemotherapy followed by second‐line treatment with ramucirumab, a vascular endothelial growth factor receptor (VEGF) inhibitor, potentially boosts treatment response in patients with advanced gastroesophageal cancer (aGEC). Here, to more thoroughly explore this possibility, the authors analyzed data from a phase II trial involving aGEC patients treated with ramucirumab after failing first‐line immunochemotherapy. Improved tumor control and lengthened survival were observed in patients following second‐line ramucirumab‐containing therapy. The benefit was greatest among patients who initially responded to first‐line therapy and whose tumors expressed programmed‐death ligand‐1. The findings indicate that integration of VEGF‐targeted agents into immunochemotherapy‐based regimens improves aGEC survival. FOLFOX plus nivolumab represents a standard of care for first‐line therapy of advanced gastroesophageal cancer (aGEC) with positive PD‐L1 expression. The efficacy of second‐line VEGFR‐2 inhibition with ramucirumab (RAM) plus chemotherapy after progression to immunochemotherapy remains unclear. Medical records of patients with aGEC enrolled in the randomized phase II AIO‐STO‐0417 trial after treatment failure to first‐line FOLFOX plus nivolumab and ipilimumab were retrospectively analyzed. Patients were divided into two groups based on second‐line therapy: RAM plus chemotherapy (RAM group) or treatment without RAM (control group). Eighty three patients were included. In the overall population, progression‐free survival (PFS) in the RAM group was superior to the control (4.5 vs 2.9 months). Responders (CR/PR) to first‐line immunochemotherapy receiving RAM containing second‐line therapy had prolonged OS from start of first‐line therapy (28.9 vs 16.5 months), as well as second‐line OS (9.6 vs 7.5 months), PFS (5.6 vs 2.9 months) and DCR (53% vs 29%) compared to the control. PD‐L1 CPS ≥1 was 42% and 44% for the RAM and the control, respectively. Patients with CPS ≥1 in the RAM group showed better tumor control (ORR 25% vs 10%) and improved survival (total OS 11.5 vs 8.0 months; second‐line OS 6.5 vs 3.9 months; PFS 4.5 vs 1.6 months) compared to the control. Prior exposure to first‐line FOLFOX plus dual checkpoint inhibition followed by RAM plus chemotherapy shows favorable response and survival rates especially in patients with initial response and positive PD‐L1 expression and has the potential to advance the treatment paradigm in aGEC.

Exploratory Analysis of Patients With Gastric/Gastroesophageal Junction Adenocarcinoma With or Without Liver Metastasis From the Phase 3 RAINBOW Study

Abstract:Purpose: Liver metastasis (LM) is reported in approximately 40% of patients with advanced/metastatic gastric/gastroesophageal junction adenocarcinoma (metastatic esophagogastric adenocarcinoma; mGEA) and is associated with a worse prognosis. This post-hoc analysis from the RAINBOW trial reported the efficacy, safety, and biomarker outcomes of ramucirumab and paclitaxel combination treatment (RAM+PAC) in patients with (LM+) and without (LM-) LM at baseline. Materials and methods: Patients (n=665) were randomly assigned on a 1:1 basis to receive either RAM+PAC (LM+: 150, LM-: 180) or placebo and paclitaxel (PL+PAC) (LM+: 138, LM-: 197). The overall survival (OS) and progression-free survival (PFS) were evaluated using stratified Kaplan-Meier and Cox regression models. The correlation of dichotomized biomarkers (VEGF-C, D; VEGFR-1,2) with efficacy in the LM+ versus LM- subgroups was analyzed using the Cox regression model with reported interaction P-values. Results: The presence of LM was associated with earlier progression than those without LM, particularly in patients receiving PL+PAC (hazard ratio [HR], 1.68). RAM+PAC treatment improved OS and PFS irrespective of LM status but showed greater improvement in LM+ than that in LM- (OS HR, 0.71 [LM+] vs. 0.88 [LM-]; PFS HR, 0.47 [LM+] vs. 0.76 [LM-]). Treatment-emergent adverse events were similar between patients with and without LM. No predictive relationship was observed between biomarker levels (VEGF-C, D; VEGFR-1,2) and efficacy outcome (OS, PFS) (all interaction P-values >0.05). Conclusions: RAM provided a significant benefit, irrespective of LM status; however, its effect was numerically stronger in patients with LM. Therefore, RAM+PAC is a clinically meaningful therapeutic option for patients with mGEA and LM. Trial registration: ClinicalTrials.gov Identifier: NCT01170663.

Abstract:199 Background: Ramucirumab (RAM), a fully human IgG1 monoclonal antibody targeting vascular endothelial growth factor receptor 2, has been approved in combination with paclitaxel (PTX) in patients with advanced gastric cancer in the second-line setting outside of China in around 80 countries based on the significant overall survival (OS) benefit in the global phase 3 RAINBOW study. RAINBOW-Asia was a bridging study of RAINBOW to evaluate the efficacy and safety of RAM in a predominantly Chinese population. Methods: Patients with advanced gastric or gastro-esophageal junction (GEJ) adenocarcinoma who experienced disease progression on first-line chemotherapy (platinum plus fluoropyrimidine with or without an anthracycline) were randomized at a 2:1 ratio to receive RAM (8 mg/kg) on day 1 and 15 plus PTX (80 mg/m2) on day 1, 8, and 15 of a 28-day cycle or placebo (PL) plus PTX. Randomization was stratified by ECOG Performance Status (0 versus 1) and peritoneal metastases (yes versus no). The co-primary endpoints were PFS and OS. Results: From Mar 2017 to Jun 2020, 440 pts were randomized (RAM+PTX: 294; PL+PTX: 146). Median PFS was significantly improved with RAM+PTX compared to PL+PTX (4.14m vs. 3.15m; HR = 0.765 [95% CI: 0.613 to 0.955]; p = 0.0184). Median OS was 8.71m with RAM+PTX and 7.92m with PL+PTX (HR = 0.963 [95% CI: 0.771 to 1.203]; p = 0.7426). Grade ≥ 3 treatment-emergent adverse events (TEAEs) occurring in > 5% of patients with RAM+PTX were: neutrophil count decreased (54.3% RAM+PTX; 38.6% PL+PTX), white blood cell count decreased (43.3% vs. 29.0%), anaemia (15.7% vs. 16.6%), hypertension (7.2% vs. 6.2%), and febrile neutropenia (6.1% vs. 0.7%). Conclusions: The combination of RAM+PTX as 2nd-line therapy demonstrated a statistically significant PFS benefit, and OS benefit consistent with RAINBOW study in a predominantly Chinese population with advanced gastric and GEJ cancer. The safety profile of RAM was consistent with previous studies. No new safety signal was observed. Clinical trial information: NCT02898077.

Abstract:BACKGROUND: In the global phase 3 RAINBOW study, ramucirumab plus paclitaxel significantly improved overall survival compared with placebo plus paclitaxel in patients with advanced gastric or gastro-oesophageal junction (GEJ) adenocarcinoma. RAINBOW-Asia, a bridging study with similar design to RAINBOW, aimed to evaluate the efficacy and safety of ramucirumab plus paclitaxel for advanced gastric or GEJ adenocarcinoma in Asian, predominantly Chinese, patients.METHODS: RAINBOW-Asia was a randomised, double-blind, placebo-controlled, phase 3 trial done at 32 centres in China, Malaysia, the Philippines, and Thailand. Adult patients (≥18 years) with metastatic or locally advanced, unresectable gastric or GEJ adenocarcinoma who previously received fluoropyrimidine-platinum-based chemotherapy were randomly assigned with a centralised interactive web response system in a 2:1 ratio to receive ramucirumab 8 mg/kg or placebo intravenously on days 1 and 15 plus paclitaxel 80 mg/m<sup>2</sup> intravenously on days 1, 8, and 15 of every 28-day cycle. Randomisation was stratified by Eastern Cooperative Oncology Group performance status and presence of peritoneal metastases. The co-primary endpoints were progression-free survival and overall survival. Efficacy analyses were done in the intention-to-treat population, and safety analysis included patients who received at least one dose of study treatment. This trial is registered with ClinicalTrials.gov, NCT02898077, and has been completed.FINDINGS: Between March 2, 2017, and June 30, 2020, 440 patients were randomly assigned to receive ramucirumab plus paclitaxel (n=294) or placebo plus paclitaxel (n=146). Median progression-free survival was 4·14 months (95% CI 3·71-4·30) in the ramucirumab plus paclitaxel group compared with 3·15 months (2·83-4·14) in the placebo plus paclitaxel group (hazard ratio [HR] 0·765, 95% CI 0·613-0·955, p=0·0184). Median overall survival was 8·71 months (95% CI 7·98-9·49) in the ramucirumab plus paclitaxel group and 7·92 months (6·31-9·10) in the placebo plus paclitaxel group (HR 0·963, 95% CI 0·771-1·203, p=0·7426). The most common grade 3 or worse treatment-emergent adverse events were decreased neutrophil count (159 [54%] of 293 patients in the ramucirumab plus paclitaxel group vs 56 [39%] of 145 in the placebo plus paclitaxel group), decreased white blood cell count (127 [43%] vs 42 [29%]), anaemia (46 [16%] vs 24 [17%]), hypertension (21 [7%] vs nine [6%]), and febrile neutropenia (18 [6%] vs one [<1%]).INTERPRETATION: These findings, along with the results from RAINBOW, support the use of ramucirumab plus paclitaxel as second-line therapy in a predominantly Chinese population with advanced gastric or GEJ adenocarcinoma.FUNDING: Eli Lilly and Company, USA.TRANSLATION: For the Chinese translation of the abstract see Supplementary Materials section.

Abstract:TPS430 Background: Anti-VEGFR2 antibody (ramucirumab) has efficacy in gastric cancer (GC), both as monotherapy & in combination with paclitaxel based on the REGARD & RAINBOW trials that led to its FDA approvals in the 2 nd line setting. Preclinical data demonstrate significant tumor immune microenvironment modulatory effects from antiangiogenic agents, supporting the clinical study of dual VEGFR with immune checkpoint blockade. This has resulted in at least 7 FDA-approved combinations of anti-VEGF/VEGFR with anti-PD-1/PD-L1 agents in lung, renal, endometrial & liver cancers. Recent data showed encouraging activity with ramucirumab plus nivolumab & other trials with pembrolizumab and durvalumab in GC & esophageal adenocarcinoma. Notably, the addition of nivolumab to 2 nd line ramucirumab plus paclitaxel was evaluated in a multi-center phase I/II trial. Study population consisted of 60% (26/43) harboring tumors positive for PD-L1 CPS ≥ 1. Results revealed encouraging efficacy (ORR 37.2%, 6-month PFS 46.5%) in the overall population. Median PFS and OS were found to be numerically higher in PD-L1 CPS ≥ 1 (6.4 months & 13.8 months respectively) compared to CPS-negative participants (5.1 months & 8.0 months), suggesting a predictive impact of PD-L1 CPS in this treatment setting. Methods: SWOG2303 is a national, randomized, open label, phase II/III trial to assess the efficacy and safety of nivolumab + paclitaxel + ramucirumab versus paclitaxel + ramucirumab in adult patients with advanced stage MSS/pMMR PD-L1 CPS ≥ 1 gastric and esophageal adenocarcinoma. Patients must have documented MSI and PD-L1 CPS status by standard of care tissue-based analysis, evaluable disease on imaging, and Zubrod performance status (PS) 0-1. They should also have clinical or radiographic progression or intolerance to frontline standard of care systemic therapy with PD-1 inhibitor containing fluoropyrimidine based chemotherapy regimen. Patients with HER2 positive disease are excluded as well as patients with prior significant immunotherapy related adverse events (iRAEs) that prompted permanent discontinuation of the PD-1 agent. Planned enrollment is 224. Participants will be randomized using a dynamic balancing algorithm with stratification based on Zubrod PS (0 vs 1), tumor location (gastric vs GEJ or esophageal) and PD-L1 CPS (≥5 vs <5). Primary endpoints are PFS for phase II and OS for phase III. Secondary endpoints include overall response rate, disease control rate, & safety. Baseline and pharmacodynamic changes in cellular and plasma angiogenic, inflammatory, and immune biomarkers will be explored & correlated with the efficacy. Other endpoints include health quality of life measured using the Functional Assessment of Cancer Therapy-Gastric (FACT-Ga). Enrollment is ongoing.

Abstract:102 Background: The most used 1 st line (1L) chemotherapy backbone for mCRC is fluoropyrimidine (FP) + oxaliplatin. In the 2L, iri alone or with 5FU (FOLFIRI), ± a biologic agent (biologic), are options. It is unclear whether adding 5FU to iri is beneficial following disease progression on prior FP. Methods: This study used the nationwide Flatiron Health electronic health record (EHR)-derived de-identified database to identify patients diagnosed with mCRC from 01-01-2013 to 02-13-2023, who received 1L with FP + oxaliplatin ± biologic, and 2L with irinotecan ± 5FU ± biologic. Baseline patient/tumor characteristics were obtained. The number of major comorbidities was computed using ICD-9-CM/10 codes within 1 year prior to 2L (Quan, 2005). Time to next treatment (TTNT) was defined from the start of 2L to the start of next line treatment (excluding maintenance). For survival outcomes, the median survival time and 95% confidence interval (CI) were estimated using the Kaplan-Meier method. Uni- and multi-variable analyses (UVA and MVA) were performed with the cox proportional hazard model. All covariates were evaluated in UVA, and those significant were included in MVA with adjusted hazard ratio (HR) and 95% CI. Results: 3,215 patients met criteria; most previously received a biologic with 1L (70.5% prior anti-angiogenesis and 5.9% prior anti-EGFR) and had 0 structured major comorbidity diagnoses (82.1%). In the 2L, FOLFIRI was the most used chemotherapy backbone (90.6%, vs iri 9.4%), 79% of patients also received a biologic, and patients who received FOLFIRI were younger (median age 61 vs 65 years) and had better ECOG performance status (87.8% vs 81.8% ECOG 0-1). Overall survival (OS) was longer with FOLFIRI than iri (median 14.4 vs 9.8 months, p<0.001), as was TTNT (median 7.6 vs 5.3 months, p<0.001). In MVA, when controlling for age, comorbidities, CEA, ECOG performance status, and 1L, iri was associated with shorter OS than FOLFIRI (HR 1.38, 95% CI 1.21-1.57, p<0.001). In MVA, the addition of a biologic to FOLFIRI and iri prolonged OS and TTNT in some subgroups (Table). Conclusions: In this retrospective real-world analysis of 3,215 patients with mCRC who received 1L with FP + oxaliplatin ± biologic, 2L FOLFIRI was associated with longer OS compared to iri alone (both ± biologic). Abstract:423 Background: Patients (pts) with advanced hepatocellular carcinoma (HCC) and elevated α-fetoprotein (AFP) have poor prognosis, and need effective, well-tolerated treatment options. Ramucirumab has been shown to improve survival and is currently approved as second-line therapy for pts with a baseline elevated AFP that have progressed on prior sorafenib. However, new options now exist in the front-line setting. This global open-label expansion cohort of REACH-2 (NCT02435433) was initiated to study ramucirumab in pts with advanced HCC and baseline AFP ≥400 ng/mL following a non-sorafenib based systemic therapy, representing one of the first sequencing studies in HCC. Methods: This single-arm study investigated ramucirumab in pts with advanced HCC (BCLC stage C or B disease), Child-Pugh A, ECOG PS 0/1, and baseline AFP ≥400 ng/mL who received 1-2 prior systemic regimens for advanced HCC, excluding prior sorafenib or chemotherapy. Enrolled pts received ramucirumab 8 mg/kg IV once every 14 days. The primary endpoint was safety. Secondary endpoints included overall survival (OS), progression-free survival (PFS) objective response rate (RECIST 1.1) and time to progression (TTP). Final analysis occurred after all enrolled pts completed ≥ 3 treatment cycles or discontinued treatment. Results: 47 pts were treated at 21 investigative sites in the USA (n = 12), Taiwan (n = 10), mainland China (n = 8), Hong Kong (n = 8), Germany (n = 8), and Switzerland (n = 1). At baseline, these pts with 2nd to 3rd+ line advanced HCC had ECOG PS 1 (51%), with vascular invasion or extrahepatic spread (85%), viral hepatitis B (55%), BCLC stage C disease (92%), and a median AFP of 3236 ng/mL (IQR: 1332, 18210). Prior systemic regimens included lenvatinib (n = 20), checkpoint inhibitor (CPI) monotherapy (n = 11), CPI + antiangiogenic (n = 15), and CPI + CPI (n = 4). Grade ≥ 3 treatment-emergent adverse events (AEs) were reported in 27 (57%) pts and were deemed to be treatment-related in 11 (23%) pts. Grade ≥ 3 AEs occurring in ≥ 5% pts were hypertension (n = 5; 11%), proteinuria (n = 3; 6%), hyponatremia (6%) and AST increased (6%). Two deaths occurred due to treatment-related AEs on therapy or within 30 days of treatment discontinuation (myocardial infarction and upper GI hemorrhage). Median OS was 8.7 months (95% CI 4.6-12.2), median PFS was 1.7 months (95% CI 1.5-4.1), and median TTP was 2.8 months (95% CI 1.5-4.2). The number of pts achieving an objective response was 5 (10.6%, 95% CI 1.8-19.5), with a median duration of response of 8.3 months (95% CI 2.4-NR). Conclusions: This expansion cohort of REACH-2 represents a non-sorafenib sequencing study in pts with advanced HCC. The safety/efficacy profile of ramucirumab following a non-sorafenib based systemic therapy was consistent with that observed in pts who received prior sorafenib in the Ph3 REACH-2 study. Paclitaxel/Ramucirumab vs. Paclitaxel in 2nd-line therapy of advanced esophageal squamous cell carcinoma: Randomized Phase II IKF-AIO-RAMOS Trial

Abstract:Introduction: In squamous cell carcinoma of the esophagus (ESCC), therapeutical options in 2nd-line treatment are scarce with immune checkpoint inhibition being the only approved one. Ramucirumab/paclitaxel is an approved 2nd-line treatment in metastatic esophagogastric adenocarcinoma. We assessed safety and efficacy of ramucirumab/paclitaxel for ESCC. Methods: This prospective, randomized, open-label, multicenter, phase II trial evaluated paclitaxel (80 mg/m2 d1, 8, 15) plus ramucirumab (8 mg/kg d1, 15) (investigational arm A) vs. paclitaxel alone (80 mg/m2 d1, 8, 15) (standard arm B), both q4w, in advanced/metastatic ESCC refractory or intolerant to fluoropyrimidine and platinum-based drugs. Primary endpoint was overall survival (OS) rate at 6 months. Results: From 3/2019 to 4/2021, 21/186 planned patients were included (arm A 11 pts; arm B 10 pts) in 9 German centres. Due to slow accrual, the study was terminated prematurely. OS at 6 months was 72.7% for ramucirumab/paclitaxel and 50.0% for paclitaxel. The study design did not allow statistical comparison of the arms. PFS (3.8 vs. 3.5 months), OS (12.1 vs. 9.2 months), ORR (18.2% vs. 20.0%) and DCR (54.5% vs. 60.0%) were comparable in both arms. Most common treatment related adverse events (TRAEs) in arm A were leucopenia (54.5%), fatigue (27.3%) and peripheral sensory neuropathy (18.2%). 27.3% in arm A and 50.0% in arm B had TRAEs ≥ grade 3. Conclusion: Ramucirumab/paclitaxel shows an acceptable tolerability and numerically improved OS at 6 months. Due to the small number of patients the current trial must be considered exploratory and more data are needed in this indication. Registration: ClinicalTrials.gov, NCT03762564. IKF-s627 RAMOS Study.

Abstract:Purpose To evaluate the efficacy and safety of FOLFIRI regimen in patients with advanced colorectal cancer refractory to fluoropyrimidine and oxaliplatin. Methods The FOLFIRI regimen consisted of intravenous infusion of irinotecan 180 mg/m 2 on day 1 plus leucovorin (LV) 400 mg/m 2 on day 1 plus 5-fluorouracil (5-FU) 400 mg/m 2 bolus on day 1 plus 46-hour intravenous infusion of 5-FU 2,400 mg/m 2 , every 2 weeks as one cycle. The main selection criterion for this study was the advanced colorectal cancer refractory to fluoropyrimidine and oxaliplatin. Results Of the 57 evaluable patients for efficacy, 4 (7.5%) had a partial response, 36 (67.9%) had stable disease, and 13 (24.5%) had progressive disease. Median progression-free survival was 4.8 months (95% CI 3.9–5.7 months), and median overall survival was 7.8 months (95% CI 13.1–16.5 months). Safety analysis was based on the data of 57 evaluable patients. The most frequently observed grade 3 or 4 toxicities were neutropenia 16 (27.8%), nausea/vomiting 7 (12.3%), and diarrhea 1 (1.8%). Conclusion FOLFIRI regimen is effective and well tolerated in patients with advanced colorectal cancer refractory to fluoropyrimidine and oxaliplatin in Chinese population.

Abstract:Dense distribution of varicose fibres containing neuropeptide Y-like immunoreactivity (NPY-LI) was found in the atrioventricular valves of the Japanese monkey, and moderately in the rat. The immunoelectron microscopy using immunogolds resulted in the localization of NPY-LI within the dense-cored vesicles which existed with the small clear vesicles in the unmyelinated axons near the endocardium. These NPY-LI-containing fibres may participate in regulation of vasomotor role or other functions of the atrioventricular valves.

Abstract:283 Background: Doublet platinum or taxane-based therapies are the current standard backbone of treatment for advanced gastric/GEJ adenocarcinoma. Previously, anthracycline based triplets have been aborted due to lack of efficacy and toxicity. We hypothesize that the combination of FOLFOX and nab-paclitaxel (FOLFOX-A) will have a higher efficacy, a better tolerability, and enhanced patient reported outcomes. Methods: Phase II single arm trial of FOLFOX + nab-paclitaxel in chemotherapy–naïve unresectable, advanced gastric/GEJ adenocarcinoma. Nab-paclitaxel 150 mg/m2 · Oxaliplatin 85 mg/m2 · Leucovorin 400 mg/m2 · 5-FU 2400 mg/m2 over 46-48 hrs every 14 days. Primary objective was response rate.Evaluable disease according to RECIST v1.1 for solid tumors was required. Her-2 positive gastric tumor patients and those with clinically significant peripheral neuropathy were excluded. A Simon (1989), optimal two-stage design was employed. Response rates and Clopper-Pearson confidence intervals are reported. Progression free (PFS) and overall (OS) survival were estimated using the method of Kaplan-Meier. Results: A 29% response rate was observed in Stage 1 and therefore the study continued enrolling all planned 39 patients. Median age was 63 (range 20-80) years, 29 (74%) were male, 30 patients now are off treatment due to progression (17), initiation of alternative therapy (5) and side effects (3). 38 patients started treatment. Of the 35 patients evaluable for response, 14 (40.0%; 95%CI: 23.9% - 57.9%) had CR/PR as best response, with disease control rate of 80.0%. Progression-free (PFS) and overall survival (OS) were available for 36 patients, with median follow-up duration of 20 months (range: 1.9 - 34.45 months). Median PFS was 6.6 months (95%CI: 5.7 – 13.0), with 28.6% (95%CI: 15.2% - 53.9%) 12-month PFS rate. There were 21 deaths, median OS was 11.0 months (95%CI: 9.6 – 21.7), 12-month OS rate was 46.6% (95%CI: 30.9% - 70.3%). Treatment-related grade 3–4 toxicities included peripheral sensory neuropathy, anemia (18.4% each), decreased neutrophil count (15.8%), and diarrhea (7.9%). One patient died due to multi-organ failure, not treatment-related. 23 patients required dose reductions or discontinuation of at least 1 component of the 3-drug regimen due to toxicity. Median number of cycles completed was 4 (range: 0-30). Conclusions: FOLFOX-A has a significant response rate, expected toxicities, and should be considered for future investigation in combination with immunotherapy given the recent approvals.

Abstract:469 Background: Patients with gastroenterpancreatic neuroendocrine carcinomas (NECs) have a very poor prognosis. And the role of the second-line therapy remains unknown. Methods: We retrospectively analyzed 11 patients with gastroenteropancreatic NEC after failure of first-line chemotherapy in the last two years in our cancer center. Data examined included clinical and pathological characteristics at the time of diagnosis, efficacy and safety. Results: Median age was 51 (17-64) years old. Most were male (81.8%). Tumor origin included pancreas (6pts, 54.5%), colon-rectum (3pts, 27.3%), one stomach and one liver. Metastatic disease was evident at diagnosis in all patients (liver metastases: 100%, celiac lymph nodes: 72.7%, bone: 18.2%, pleural effusion and peritoneal metastasis: 18.2%). Median Ki-67 index was 80%. Etoposide-cisplatin combinations as first line chemotherapy were administered in 8 (72.7%) patients and 2 (18.2%) capecitabine based regimen and 1(9.1%) gemcitabine with nab-paclitaxel. All patients received the FOLFIRI regimen with a median number of 8 (3-36) courses. Three patients (27.3%) had partial response, 4 (36.4%) stable disease, and 4 (36.4 %%) tumor progression. But in 6 patients received the FOLFIRI plus bevacizumab, disease control rate was 66.7%, 3 (50.0%) partial response, 1(16.7%) stable disease. The median PFS in all 11 pts was 3.77 (1.77-24.07) months, in 6pts with bevacizumab was 4.77(1.83-24.07) months. The most severe toxicities was grade 3 neutropenia (27.2%), the other adverse effects were mild to moderate, including mild anemia, transient transferase elevation and proteinuria. Conclusions: FOLFIRI regimen is a potentially effective and safe chemotherapy given as second-line in patients with NEC. Additional anti-VEGF therapy with bevacizumab may improve the disease control and PFS time.

Abstract:SMIfp (SMILES fingerprint) is defined here as a scalar fingerprint describing organic molecules by counting the occurrences of 34 different symbols in their SMILES strings, which creates a 34-dimensional chemical space. Ligand-based virtual screening using the city-block distance CBD(SMIfp) as similarity measure provides good AUC values and enrichment factors for recovering series of actives from the directory of useful decoys (DUD-E) and from ZINC. DrugBank, ChEMBL, ZINC, PubChem, GDB-11, GDB-13, and GDB-17 can be searched by CBD(SMIfp) using an online SMIfp-browser at www.gdb.unibe.ch. Visualization of the SMIfp chemical space was performed by principal component analysis and color-coded maps of the (PC1, PC2)-planes, with interactive access to the molecules enabled by the Java application SMIfp-MAPPLET available from www.gdb.unibe.ch. These maps spread molecules according to their fraction of aromatic atoms, size and polarity. SMIfp provides a new and relevant entry to explore the small molecule chemical space.

Abstract:In RAINBOW-Asia study (NCT02898077), a bridging study of RAINBOW, RAM/PTX compared with PBO/PTX demonstrated consistent efficacy benefit [PFS: 4.14 vs. 3.15 months, Hazard ratio (HR)=0.765, p=0.0184; OS: 8.71 vs. 7.92 months, HR=0.963] and safety profile with RAINBOW study in a predominantly Chinese population with AGC as 2nd-line therapy. Here we will present results of QoL of the study. Patients with AGC who previously received fluoropyrimidine- and platinum-based therapy were randomized at 2:1 ratio to receive RAM/PTX or PBO/PTX. Patients completed the EORTC QLQ-C30 (v3) at baseline, first day of each 4-week cycle and at discontinuation. Scores were classified as improved or deteriorated if changed by ≥10 points (on 100-point scale) relative to baseline, otherwise classified as stable. Time to deterioration in QoL parameters was defined as duration from randomization to first deterioration. HRs and 95% confidence interval (CI) were estimated using stratified Cox proportional hazard model. Of 440 patients randomized, 272/294 (92.5%) of RAM/PTX and 133/146 (91.1%) of PBO/PTX patients provided both baseline and post-baseline data. Percentage of compliance was high (>95% in most cycles) and similar between arms. For all QoL parameters, the proportion of patients reporting improved or stable scores was generally similar between arms at all on-therapy assessment times. For time to deterioration, HRs for 11 of the 15 QoL scales had 95% CI that included 1, indicating no significant difference in those scales between arms. HRs for 4 symptom scales including dyspnea, insomnia, appetite loss, and diarrhea did not favor RAM/PTX arm, which was consistent with slightly higher incidence of all-grade (primarily low-grade) treatment-emergent adverse events in this arm. Similar proportion of patients reported improved or stable QoL scores between arms and no evidence of detriment to QoL in majority of scales by treatment with RAM/PTX as 2nd-line therapy in this population.
Ramucirumab plus erlotinib versus placebo plus erlotinib in previously untreated EGFR-mutated metastatic non-small-cell lung cancer (RELAY): exploratory analysis of next-generation sequencing results

Abstract:Background: Ramucirumab plus erlotinib (RAM + ERL) demonstrated superior progression-free survival (PFS) over placebo + ERL (PBO + ERL) in the phase III RELAY study of patients with epidermal growth factor receptor (EGFR)-mutated metastatic non-small-cell lung cancer (EGFR+ mNSCLC; NCT02411448). Next-generation sequencing (NGS) was used to identify clinically relevant alterations in circulating tumor DNA (ctDNA) and explore their impact on treatment outcomes. Patients and methods: Eligible patients with EGFR+ mNSCLC were randomized 1 : 1 to ERL (150 mg/day) plus RAM (10 mg/kg)/PBO every 2 weeks. Liquid biopsies were to be prospectively collected at baseline, cycle 4 (C4), and postdiscontinuation follow-up. EGFR and co-occurring/treatment-emergent (TE) genomic alterations in ctDNA were analyzed using Guardant360 NGS platform. Results: In those with valid baseline samples, detectable activating EGFR alterations in ctDNA (aEGFR+) were associated with shorter PFS [aEGFR+: 12.7 months (n = 255) versus aEGFR-: 22.0 months (n = 131); hazard ratio (HR) = 1.87, 95% confidence interval (CI) 1.42-2.51]. Irrespective of detectable/undetectable baseline aEGFR, RAM + ERL was associated with longer PFS versus PBO + ERL [aEGFR+: median PFS (mPFS) = 15.2 versus 11.1 months, HR = 0.63, 95% CI 0.46-0.85; aEGFR-: mPFS = 22.1 versus 19.2 months, HR = 0.80, 95% CI 0.49-1.30]. Baseline alterations co-occurring with aEGFR were identified in 69 genes, most commonly TP53 (43%), EGFR (other than aEGFR; 25%), and PIK3CA (10%). PFS was longer in RAM + ERL, irrespective of baseline co-occurring alterations. Clearance of baseline aEGFR by C4 was associated with longer PFS (mPFS = 14.1 versus 7.0 months, HR = 0.481, 95% CI 0.33-0.71). RAM + ERL improved PFS outcomes, irrespective of aEGFR mutation clearance. TE gene alterations were most commonly in EGFR [T790M (29%), other (19%)] and TP53 (16%). Conclusions: Baseline aEGFR alterations in ctDNA were associated with shorter mPFS. RAM + ERL was associated with improved PFS outcomes, irrespective of detectable/undetectable aEGFR, co-occurring baseline alterations, or aEGFR+ clearance by C4. aEGFR+ clearance by C4 was associated with improved PFS outcomes. Monitoring co-occurring alterations and aEGFR+ clearance may provide insights into mechanisms of EGFR tyrosine kinase inhibitor resistance and the patients who may benefit from intensified treatment schedules.
Ramucirumab in second‑line advanced colorectal cancer therapy: A study on therapeutic outcomes and hepatic sinusoidal platelet aggregation

Abstract:The present study investigated the role of ramucirumab (RAM) in treating liver metastases (LMs) as a second-line or salvage treatment in patients with advanced CRC. Of the 36 patients, 21 (58%) received RAM plus folinic acid, fluorouracil and irinotecan (FOLFIRI) as second-line treatment, whereas 15 (42%) received it in a salvage setting. The median overall survival time was 23 months [95% confidence interval (CI), 12-34 months] for those in the second-line treatment group and 8 months (95% CI, 5-19 months) for those in the salvage treatment group. Of the 36 patients, 14 (39%) underwent surgical resection of LMs during chemotherapy. A total of 6 patients underwent surgical resection for LMs for the first time during second-line RAM plus FOLFIRI (RAM-LM); of the remaining 8 patients, 6 underwent resection of LMs during first-line bevacizumab (BEV)-based chemotherapy (BEV-LM). Immunohistochemical analysis of CD42b showed that the platelet aggregation score (CD42b score), which ranges from 0 (absence of deposition) to 3 (presence of linear deposition), tended to decrease with the increasing duration of treatment with both RAM and BEV. Although there was no significant difference in the mean duration of anti-VEGF antibody treatment between the BEV-LM and RAM-LM groups, the median CD42b score was higher in the RAM-LM group (median CD42b score, 3; range, 0-3) compared with that in the BEV-LM group (median CD42b score, 1; range, 0-3; P=0.01), suggesting that RAM induces a different degree of platelet aggregation in liver sinusoids compared to BEV.

Abstract:PurposeThis study aimed to compare ramucirumab-paclitaxel versus chemotherapy in second-line (2L) advanced gastroesophageal cancer (aGEC) based on HER2 status and analyze prognostic factors.MethodsThe study includes patients from the AGAMENON-SEOM registry with aGEC and known HER2 status who received 2L between 2016 and 2021. The Kaplan–Meier method was used to calculate progression-free survival (PFS) and overall survival (OS) and multivariable Cox regression analysis was done to adjust for confounding variables.ResultsOf the 552 patients who met the selection criteria, 149 (26.9%) had HER2-positive aGEC, 89 were treated with chemotherapy, and 60 with ramucirumab-paclitaxel, and 403 had an HER2-negative aGEC, 259 were treated with chemotherapy, and 144 with ramucirumab-paclitaxel.In the whole sample, 2L PFS was 3.0 months (95% CI 2.8–3.2), 2L OS, 5.7 months (5.2–6.3), and ramucirumab-paclitaxel versus chemotherapy was associated with increased PFS (HR 0.64, 95% CI 0.53–0.78, p < 0.0001) and OS (HR 0.68, 0.55–0.83, p = 0.0002). Median PFS of ramucirumab- paclitaxel versus chemotherapy was 3.5 vs 2.8 months (HR 0.67, 0.54–0.83, p = 0.0004) in HER2-negative, and 4.7 vs 2.7 months (HR 0.57, 0.40–0.82, p = 0.0031) in HER2-positive aGEC, respectively. Median OS for ramucirumab-paclitaxel versus chemotherapy was 6.6 vs 5 months (HR 0.67, 0.53–0.85, p = 0.0007) in HER2-negative, and 7.4 vs 5.6 months (HR 0.70, 0.53–1.04, p = 0.083) in HER2-positive aGEC, respectively. ECOG-PS, tumor burden, Lauren subtype, and neutrophil–lymphocyte ratio were prognostic factors.ConclusionsIn patients with an aGEC from the AGAMENON-SEOM registry, 2L treatment with ramucirumab-paclitaxel was superior to chemotherapy in PFS, OS and response rate, independent of HER2 status.
