Abstract:Health systems face multimorbidity as the leading cause of the burden of disease and demand for healthcare services. Although non-communicable diseases are well known, and countries in Latin America have implemented strategies to its approach, tackling multimorbidity is still a challenge and an emerging topic. In response to this, Centro de Innovación en Salud ANCORA UC, in association with Servicio de Salud Metropolitano Sur Oriente and the National Health Fund, implemented a pilot study a Multimorbidity Patient-Centered Care Model in the health network. The objective was to evaluate the health care services utilization and mortality. The authors performed a cohort study with adults with non-communicable diseases, stratified by ACG® System. Analysis used logistic regression, adjusted by confounding variables. The intervened group had a significantly lower incidence of hospital admissions, length of stay, number of consultancies to hospital emergency, and number of consultancies to primary care emergency than the control group. Further, it was associated with significantly less mortality (OR 0.54; 95% CI 0.47 – 0.63). Barriers and facilitators proper from a complex intervention were approached. The model showed positive results in mortality and health services utilization. A description is provided to contribute to this emerging topic and facilitate its reproducibility. Abstract:INTRODUCTION: Value-based systems will place financial risk on providers. If these payments are not adequately adjusted for comorbidity burden, systems will be disincentivized to treat marginalized socioeconomic groups who have disproportionately high comorbidity burden. METHODS: Comorbidity-focused note-writing templates were incorporated into the physicians’ workflow within Jefferson University Hospital’s Neurosurgery Department. A list of high-risk comorbidities was identified and incorporated into the top of each note. Emphasis was given to comorbidities which have high historical impact on mortality and which are commonly overlooked in the acute neurosurgical decisionmaking process (e.g. obesity). The selected comorbidities were populated into the top of every note which included treatment plans for each condition. The process measure included the percentage of notes in compliance with the template. The outcome measures were expected mortality (composite variable representing comorbidity burden) and the capture rate of major comorbidities within the coding process. Measures were compared between one full year pre and post-intervention. Coders were blinded to the presence of our intervention. RESULTS: There was 100% compliance after the second week post-intervention (as measured by full audit of all notes written). There was a 44% (3.4% to 4.9%) improvement in expected mortality for the department’s encounters. There was an increased capture rate for several major comorbidities which are disproportionately associated with marginalized socioeconomic groups. These included malnutrition (233% increase, from 3% to 10%), coagulopathies (71% increase from 7% to 12%), obesity (50% increase from 22% to 33%), congestive heart failure (43% increase from 7% to 10%), hypertension (31% increase from 13% to 17%), and deficiency anemias (31% increase from 16% to 21%). CONCLUSIONS: We present a low-cost framework to increase capture rate of high-risk comorbidities which traditionally contribute to socioeconomic disparities.

Abstract:Patients with multimorbidity have increased and more complex healthcare needs, posing their management a challenge for healthcare systems. This study aimed to describe their primary healthcare utilization and associated factors. A population-based cross-sectional study was conducted in a Spanish basic healthcare area including all patients with chronic conditions, differentiating between having multimorbidity or not. Sociodemographic, functional, clinical and service utilization variables were analyzed, stratifying the multimorbid population by the Adjusted Morbidity Groups (AMG) risk level, sex and age. A total of 6036 patients had multimorbidity, 64.2% being low risk, 28.5% medium risk and 7.3% high risk. Their mean age was 64.1 years and 63.5% were women, having on average 3.5 chronic diseases, and 25.3% were polymedicated. Their mean primary care contacts/year was 14.9 (7.8 with family doctors and 4.4 with nurses). Factors associated with primary care utilization were age (B-coefficient [BC] = 1.15;95% Confidence Interval [CI] = 0.30–2.01), female sex (BC = 1.04; CI = 0.30–1.78), having a caregiver (BC = 8.70; CI = 6.72–10.69), complexity (B-coefficient = 0.46; CI = 0.38–0.55), high-risk (B-coefficient = 2.29; CI = 1.26–3.32), numerous chronic diseases (B-coefficient = 1.20; CI = 0.37–2.04) and polypharmacy (B-coefficient = 5.05; CI = 4.00–6.10). This study provides valuable data on the application of AMG in multimorbid patients, revealing their healthcare utilization and the need for a patient-centered approach by primary care professionals. These results could guide in improving coordination among professionals, optimizing multimorbidity management and reducing costs derived from their extensive healthcare utilization.

Abstract:Abstract Background Person-centred care (PCC) focusing on personalised goals and care plans derived from “What matters to you?” has an impact on single disease outcomes, but studies on multi-morbid elderly are lacking. Furthermore, the combination of PCC, Integrated Care (IC) and Pro-active care are widely recognised as desirable for multi-morbid elderly, yet previous studies focus on single components only, leaving synergies unexplored. The effect of a synergistic intervention, which implements 1) Person-centred goal-oriented care driven by “What matters to you?” with 2) IC and 3) pro-active care is unknown. Methods Inspired by theoretical foundations, complexity science, previous health service research and a patient-driven evaluation of care quality, we designed the Patient-Centred Team (PACT) intervention across primary and secondary care. The PACT team collaborate with the patient to make and deliver a person-centred, integrated and proactive multi-morbidity care-plan. The control group receives conventional care. The study design is a pragmatic six months prospective, controlled clinical trial based on hospital electronic health record data of 439 multi-morbid frail elderly at risk for emergency (re) admissions referred to PACT and 779 propensity score matched controls in Norway, 2014–2016. Outcomes are emergency admissions, the sum of emergency inpatient bed days, 30-day readmissions, planned and emergency outpatient visits and mortality at three and six months follow-up. Results The Rate Ratios (RR) for emergency admissions was 0,9 (95%CI: 0,82-0,99), for sum of emergency bed days 0,68 (95%CI:0,52-0,79) and for 30-days emergency readmissions 0,72 (95%CI: 0,41-1,24). RRs were 2,3 (95%CI: 2,02-2,55) and 0,9 (95%CI: 0,68-1,20) for planned and emergency outpatient visits respectively. The RR for death at 3 months was 0,39 (95% CI: 0,22-0,70) and 0,57 (95% CI: 0,34-0,94) at 6 months. Conclusion Compared with propensity score matched controls, the care process of frail multi-morbid elderly who received the PACT intervention had a reduced risk of high-level emergency care, increased use of low-level planned care, and substantially reduced mortality risk. Further study of process differences between groups is warranted to understand the genesis of these results better. Risk stratification at primary care centres in Valencia (Spain) to activate integrated care pathways for elderly patients with chronic conditions

Abstract:Introduction: The activation of integrated care pathways is not always easy and is time-consuming, so the standardized use of stratification tools (ST) can support health care professionals in decision-making processes. Several ST –developed and validated in the United States– have been implemented in the Valencia Region (Spain) at primary care (PC) centres to identify elders with multimorbidity at risk of suffering future hospital admissions (FHA). In spite of the usefulness of these instruments, it would be recommended to apply STs which have been specifically designed on the basis of the Spanish healthcare system and the characteristics of its population. Thus, the objective of this study was to design a ST aimed at the identification of elders at risk of hospital admissions in the following 12 months in the Valencian Healthcare System. Methods: The study started with the organization of focus groups (FG) with six multidisciplinary PC professionals aimed at selecting potential variables to be included in the ST. From the FG’s results a retrospective study was carried out with the objective of identifying the most significant variables. These variables were tested in a sample of 107 elders. In the last stage, the combination of variables derived from the previous steps was tested in a sample of 1,000 patients in order to select a final combination of variables and to build a predictive algorithm through binary logistic regression analysis.Results: A set of 13 potential variables to be included in our ST were identified in the FG. These variables comprised data on socio-demographics, clinical and social aspects and registers on the previous use of health resources. As part of the retrospective studies, binary logistic regression analysis determined that the following variables were statistically significant as predictors of FHA: ‘chronic respiratory disease’ (OR= 2.32, p= 0.015), ‘chronic heart disease’ (OR= 2.10, p= 0.019), ‘palliative care’ (OR= 22.53, p= 0.04), and the ‘number of previous visits to emergency department at hospital’ (OR= 1.82, p< 0.001). The model derived a risk score from 0 to 1 for each patient, classifying patients as high risk of FHA at a risk score threshold of 0.5 or higher. The model had a sensitivity of 42% and specificity of 96% and the AUC was 0.76.Discussions: The model developed within this study has shown an optimal performance, and an acceptable ability to identify patients at high risk of FHA who really were admitted(sensitivity of 42%), an excellent ability to identify patients at low risk who really did not suffer any admission (specificity of 96%). The accuracy detecting patients was similar to other comparable studies or even higher with reported AUC of 0.764. Moreover, the model incorporates variables that can be collected automatically from electronic health information systems (eHIS). Furthermore, the experts participating in the FG highlighted some social variables – usually not registered within eHIS – as determinants of health, especially in elders with chronic conditions (such as availability of knowledge and skills for appropriate self-care, social and/or family support or economic difficulties). Thus, it is proposed a 2-phase stratification system: the first one will calculate the developed algorithm through eHIS, and the second one will assess social variables by consulting the professional of reference.Conclusions: The final predictive model is composed by variables available at eHIS, so it can be easily introduced into health administration system of the Valencia Region. From this screening and decision-making support ST, professionals will be able to implement a wide range of care plans in accordance with patients’ health and social needs which can potentially improve quality of care and favor that older people can remain at home longer.Lessons learned: It is possible to design new ST on the basis of the target population profile and the characteristics of the healthcare system.Limitations: The sample is not representative of the whole elderly population in the Valencia Region. Moreover, the sensitivity of the model is limited.Suggestions for future research: Future studies are needed to validate these results with a larger sample from the Valencia Region and other regions in Spain. Moreover, it would be interesting to explore their applicability in other health and social care settings and even in other European contexts assessing the degree of similarity or variance of discriminating factors or the available electronic data sets. Currently, we continue this study developing an integrated care protocol to be implemented in those patients identified as high risk of suffering FHA by a multidisciplinary team of PC professionals (general practitioners, nurses and social workers).

Abstract:Rational Studies are needed to determine if multimorbidity screening and management reduce the rate of multimorbidity accumulation in patients with chronic inflammatory rheumatic diseases (IRD). Objectives This study evaluates the impact of systematic screening programme on patient care and hospitalisation rates. Methods Patients with IRD who participated in the screening programme (exposed patients) were identified within the French national health database and matched with controls. Two sets of analysis were performed: one with multivariate analysis and a second using a propensity score matching to ensure comparability between exposed patients and controls. The primary endpoint (PE) was a composite score assessing the dispensation of multimorbidity-preventing drugs, including vaccines, lipid-lowering agents, antiosteoporotic medications and antiplatelet drugs, during the year following the index date. Results The first analysis included 286 exposed patients and 858 controls, demonstrating a higher rate of meeting the PE in exposed patients (adjusted OR=1.6 (1.2–2.2), p<0.01). Propensity score matching resulted in 281 exposed patients and 281 controls. Exposed patients exhibited a significantly higher rate of meeting the PE compared with controls (54.8% vs 44.5%; OR=1.5; p=0.015), with increased utilisation of vaccines, cholesterol-lowering drugs and antiosteoporotic medications. Furthermore, emergency admission and hospitalisations for fracture, cardiovascular events or infection were significantly less frequent in the exposed group (7.1% vs 15.3%; OR=0.42, p<0.01), with a reduction in severe infections (0.7% vs 3.9%; p=0.03). Conclusion Systematic multimorbidity screening in patients with IRD boosted preventive medication use and reduced hospital admissions, justifying time and resource allocation for screening.

Abstract:The purpose of the present study was to investigate the influence of a polyhydroxy base, N-acetyl glucamine (also know as Meglumine), as a ternary component on the complexation of DRF-4367, a poorly water-soluble and weakly acidic anti-inflammatory molecule, with 2-hydroxypropyl-β-cyclodextrin (HPβCD). The molecular inclusion of DRF-4367 with HPβCD alone and in combination with ternary component was aimed at improvement in solubility and, subsequently, dissolution rate-limited oral bioavailability. The solid complexes of DRF-4367 and HPβCD with or without meglumine (binary and ternary systems, respectively) were prepared as coevaporated product in different stoichiometric ratios and compared against physical mixture. The formation of inclusion complexes was confirmed by using classical instrumental techniques. Phase solubility studies suggested that meglumine was responsible for solubility improvement via multiple factors rather than just providing a favorable pH. Mechanisms and factors governing solubility enhancement were investigated by using phase solubility and thermodynamic parameters. The complexation of DRF-4367 with HPβCD is thermodynamically favored because the Gibbs free energies of transfer of the drug to the cyclodextrin cavity are negative. The solubilization efficiency and stability were further improved while retaining the favorable Gibbs free energies of transfer with the addition of meglumine. Inclusion ternary complex of DRF-4367 with HPβCD and meglumine showed significant improvement in dissolution compared with uncomplexed drug and binary system. Moreover, the phenomena of reprecipitation observed with binary system during dissolution could be avoided with meglumine as an enabling ternary component. This improved physicochemical behavior of ternary complex with the novel inclusion of a polyhydroxy base translated into an enhanced oral bioavailability of DRF-4367 compared with either uncomplexed drug or nanosuspension.
Integrated approach for multimorbid patients in a hospitalist setting: Survival analysis of a two-year prospective study

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background/Purpose</h3><p>Multimorbidity is a worldwide issue when aging is rapidly. The aim of this study was to evaluate the impact of demography, morbidity, disability and depression on short-term and long-term mortality for multimorbid inpatients.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>The participants' information were assessed upon recruitment. Multimorbidity and disability were measured by modified Charlson comorbidities Index (CCI) and Barthel Index for Activity of Daily Living (ADL), respectively. Depression was screened over one-item self-reported perceptions of depressed mood rated as yes or no. The factors of in-hospital mortality and periodic mortality after discharge were examined by Cox proportional hazard regression and Kaplan–Meier survival analyses.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>A total of 201 inpatients from a hospitalist's ward were recruited. The in-hospital mortality was 14.4%, while 24-month mortality was 57.8%. After adjustment, severe ADL dependence (<35) was the only contributing factor for in-hospital mortality (Hazard Ratio [HR] = 12.94, <em>p</em> = 0.018). The hazard ratios of 3-6-12-24-months of high CCI (≥6) and severe ADL dependence were 8.12-13.57 (<em>p</em> < 0.001) and 2.91-5.39 (<em>p</em> < 0.001) respectively; both trends of impacts were decreasing overtime. Gender rather than age effect was evident. Besides, self-reported depression was associated with 12-month (HR = 1.72, <em>p</em> = 0.04) and 24-month (HR = 1.65, <em>p</em> = 0.038) mortality. Moreover, severe ADL dependence (<em>p =</em> 0.001) and depression (<em>p =</em> 0.01) contributed to higher mortality in non-cancer patients.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Our findings suggested that gender, multimorbidity, and disability influenced the two-year survival, while depression was the strongest factor related to long-term mortality. Clinicians should notice the importance of integrated approach and mental health care for those with severe disabilities and morbidity.</p>

Promising algorithms to perilous applications: a systematic review of risk stratification tools for predicting healthcare utilisation

Abstract:Objectives: Risk stratification tools that predict healthcare utilisation are extensively integrated into primary care systems worldwide, forming a key component of anticipatory care pathways, where high-risk individuals are targeted by preventative interventions. Existing work broadly focuses on comparing model performance in retrospective cohorts with little attention paid to efficacy in reducing morbidity when deployed in different global contexts. We review the evidence supporting the use of such tools in real-world settings, from retrospective dataset performance to pathway evaluation. Methods: A systematic search was undertaken to identify studies reporting the development, validation and deployment of models that predict healthcare utilisation in unselected primary care cohorts, comparable to their current real-world application. Results: Among 3897 articles screened, 51 studies were identified evaluating 28 risk prediction models. Half underwent external validation yet only two were validated internationally. No association between validation context and model discrimination was observed. The majority of real-world evaluation studies reported no change, or indeed significant increases, in healthcare utilisation within targeted groups, with only one-third of reports demonstrating some benefit. Discussion: While model discrimination appears satisfactorily robust to application context there is little evidence to suggest that accurate identification of high-risk individuals can be reliably translated to improvements in service delivery or morbidity. Conclusions: The evidence does not support further integration of care pathways with costly population-level interventions based on risk prediction in unselected primary care cohorts. There is an urgent need to independently appraise the safety, efficacy and cost-effectiveness of risk prediction systems that are already widely deployed within primary care.

Abstract:Background: In-hospital care of chronic patients is based on their characteristics and risk levels. Adjusted morbidity groups (AMG) is a population stratification tool which is currently being used in Primary Care but not in Hospitals. The objectives of this study were to describe the use of hospital services by chronic patients according to their risk levels assigned by AMG and to analyze influencing variables. Material and methods: In this cross-sectional study, patients aged ≥18 years from a healthcare service area classified as chronically ill by the AMG classification system who used their referral hospital services from June 2015 to June 2016 were included. Predisposing and needs factors were collected. Univariate, bivariate and multiple linear regressions were performed. Results: Of the 9,443 chronic patients identified (52.1% of the population in the selected area), 4,143 (43.9%) used hospital care services. Their mean age was 62.1 years (standard deviation (SD) = 18.4); 61.8% were female; 9% were high risk; 30% were medium risk, and 61% were low risk. The mean number of hospital service contacts was 5.0 (SD = 6.2), with 3.8 (SD = 4.3) visits to outpatient clinic, 0.7 (SD = 1.2) visits to emergency departments, 0.3 (SD = 2.8) visits to day hospital, and 0.2 (SD = 0.5) hospitalizations. The factors associated with greater service use were predisposing factors such as age (coefficient B (CB) = 0.03; 95% confidence interval (CI) = 0.01-0.05) and Spanish origin (CB = 3.9; 95% CI = 3.2-4.6). Among the needs factors were palliative care (CB = 4.8; 95% CI = 2.8-6.7), primary caregiver status (CB = 2.3; 95% CI = 0.7-3.9), a high risk level (CB = 2.9; 95% CI = 2.1-3.6), multimorbidity (CB = 0.8, 95% CI = 0.4-1.3), chronic obstructive pulmonary disease (COPD) (CB = 1.5, 95% CI = 0.8-2.3), depression (CB = 0.8, 95% CI = 0.3-1.3), active cancer (CB = 4.4, 95% CI = 3.7-5.1), and polymedication (CB = 1.1, 95% CI = 0.5-1.7). Conclusions: The use of hospital services by chronic patients was high and increased with the risk level assigned by the AMG. The most frequent type of contact was outpatient consultation. Use was increased with predisposing factors such as age and geographic origin and by needs factors such as multimorbidity, risk level and severe diseases requiring follow-up, home care, and palliative care.

Abstract:OBJECTIVE To evaluate the effects of different intracoronal bleaching methods on the shear bond strengths (SBS) of orthodontic brackets. MATERIALS AND METHODS Sixty freshly extracted mandibular incisors were randomly divided into four groups (n = 15 per group). After filling the root canals, root fillings were removed 2 mm apical to the cementoenamel junction, and a 2-mm-thick layer of zinc-phosphate cement base was applied. Group 1 served as the control. Intracoronal bleaching was performed with hydrogen peroxide (Opalacence Endo, Ultradent products Inc, South Jordan, Utah) in group 2, sodium perborate (Sultan Healthcare, Englewood, NJ) in group 3, and 37% carbamide peroxide (Whiteness Super Endo, Dentscare, ltda, Joinville, Brazil) in group 4. Orthodontic brackets were bonded with a light cure composite resin and cured with an LED light. After bonding, the SBS of the brackets was tested with a Universal testing machine. RESULTS Analysis of variance indicated a significant difference between groups (P < .001). The highest values for SBS were measured in group 1 (10.15 ± 1.15 MPa). The SBS was significantly lower in groups 2, 3, and 4 than in group 1 (P < .001). The lowest values for SBS were measured in group 3 (6.17 ± 0.85 MPa). SBS was significantly higher in group 4 than in group 3 (P < .05). CONCLUSIONS Intracoronal bleaching significantly affected the SBS of orthodontic brackets on human enamel. Bleaching with sodium perborate affects SBS more adversely than does bleaching with other agents.
Unplanned Hospital Admissions Stratification Model With Heart Failure Metrics: A Novel Approach For Measuring Disease Progression

Abstract:Introduction Heart Failure (HF) is the leading cause of hospitalization for patients over 65, generating the highest rate of 30-day readmissions, accounting for 33% of all Fee for Service Medicare costs. Moreover, many instances of hospitalization for HF patients are considered preventable, making this group a perfect target for preventative care management programs (PCMP) aiming to improve outcomes and reduce costs by directly targeting avoidable and unplanned hospital admissions (UHAs). Hypothesis By leveraging novel Galectin-3 blood tests and HF classification methods with state-of-the-art ML risk stratification techniques, we can improve accuracy in identifying members at higher risk of adverse outcomes with HF. Methods We enable our PCMP team to preemptively address HF by providing them with the most cutting-edge information (e.g., Galectin-3 blood tests, ML risk stratification) on HF predisposition, progression, and overall adverse event risk. Using 21,009 unique outcome-stratified and continuously enrolled members (mean age 73 years, 56% female), we built an XGBoost model to identify members at high risk of having a UHA within 6 months (prevalence 5.24%). Comorbidities include hypertension 77%, obesity 22%, chronic kidney disease 17%, coronary artery disease 27%, and HF 10% with an average Charlson Comorbidity Index (CCI) of 5.21. We use claims data, EHR, and research-based HF features for model training with a 10-fold cross-validation (80/20 split) and 950 features. The key HF model features are a claims-based estimation of "reduced Left Ventricular Ejection Fraction," HF medication adherence, and the HLM System, which seeks to classify HF progression using a person's claims medical history regarding Heart damage (H), Lung involvement (L), and Malfunction (M) of peripheral organs in analogy to TNM classification used in oncology. Results After running the UHA model with HF features for a year, the model identified members at the 90th risk percentile with more developed HF (HLM Score*: 2.40 vs 2.18), higher costs (PMPM***: $2,500 vs $2,227), higher CCI (CCI***: 16 vs 14), and higher probability of UHA (UHA*: 19.7% vs 18.00%) than a similar UHA model without the specialized HF features. Conclusions Overall, enhancing our existing PCMP with HF-specific information and new technologies allows us to utilize our existing workflows to address HF within our attributed population more accurately. Moreover, it has shown that we can apply these new technologies to proactively identify members with HF, opening the door to future HF-targeted PCMP that can have significant impacts on HF progression within our population.

Implementing Risk Stratification in Primary Care: Challenges and Strategies

Abstract:Introduction: Primary care risk stratification (RS) has been shown to help practices better understand their patient populations' needs and may improve health outcomes and reduce expenditures by targeting and tailoring care to high-need patients. This study aims to understand key considerations practices faced and practice experiences as they began to implement RS models. Methods: We conducted semistructured interviews about experiences in RS with 34 stakeholders from 15 primary care practices in Oregon and Colorado and qualitatively analyzed the data. Results: Three decisions were important in shaping practices' experiences with RS: choosing established versus self-created algorithms or heuristics, clinical intuition, or a combination; selecting mechanisms for assigning risk scores; determining how to integrate RS approaches into care delivery. Practices using clinical intuition found stratification time-consuming and difficult to incorporate into existing workflows, but trusted risk scores more than those using algorithms. Trust in risk scores was influenced by data extraction capabilities; practices often lacked sufficient data to calculate their perceived optimal risk score. Displaying the scores to the care team was a major issue. Finally, obtaining buy-in from care team members was challenging, requiring repeated cycles of improvement and workflow integration. Discussion: Practices used iterative approaches to RS implementation. As a result, procedural and algorithmic changes were introduced and were influenced by practices' health IT, staffing, and resource capacities. Practices were most successful when able to make iterative changes to their approaches, incorporated both automation and human process in RS, educated staff on the importance of RS, and had readily accessible risk scores.

Abstract:Objective To compare morbidity burden captured from multimorbidity indices and aggregated measures of clinically meaningful categories captured in primary care community‐based health center (CBHC) patients. Data Sources and Study Setting Electronic health records of patients seen in 2019 in OCHIN's national network of CBHCs serving patients in rural and underserved communities. Study Design Age‐stratified analyses comparing the most common conditions captured by the Charlson, Elixhauser, and Multimorbidity Weighted (MWI) indices, and Classification Software Refined (CCSR) and Chronic Condition Indicator (CCI) algorithms. Data Collection/Extraction Methods Active ICD‐10 conditions on patients' problem list in 2019. Principal Findings Approximately 35%–56% of patients with at least one condition are not captured by the Charlson, Elixhauser, and MWI indices. When stratified by age, this range broadens to 9%–90% with higher percentages in younger patients. The CCSR and CCI reflect a broader range of acute and chronic conditions prevalent among CBHC patients. Conclusion Three commonly used indices to capture morbidity burden reflect conditions most prevalent among older adults, but do not capture those on problem lists for younger CBHC patients. An index with an expanded range of care conditions is needed to understand the complex care provided to primary care populations across the lifespan.

Abstract:Objective: The aim was to clarify which pairs or clusters of diseases predict the hospital-related events and death in a population of patients with complex health care needs (PCHCN). Method: Subjects classified in 2012 as PCHCN in a local health unit by ACG® (Adjusted Clinical Groups) System were linked with hospital discharge records in 2013 to identify those who experienced any of a series of hospital admission events and death. Number of comorbidities, comorbidities dyads, and latent classes were used as exposure variable. Regression analyses were applied to examine the associations between dependent and exposure variables. Results: Besides the fact that larger number of chronic conditions is associated with higher odds of hospital admission or death, we showed that certain dyads and classes of diseases have a particularly strong association with these outcomes. Discussion: Unlike morbidity counts, analyzing morbidity clusters and dyads reveals which combinations of morbidities are associated with the highest hospitalization rates or death.

Abstract:This study built a predefined rule-based risk stratification paradigm using 19 factors in a primary care setting that works with rural communities. The factors include medical and nonmedical variables. The nonmedical variables represent 3 demographic attributes and one other factor represents transportation availability. Medical variables represent major clinical variables such as blood pressure and BMI. Many risk stratification models are found in the literature but few integrate medical and nonmedical variables, and to our knowledge, no such model is designed specifically for rural communities. The data used in this study contain the associated variables of all medical visits in 2021. Data from 2022 were used to evaluate the model. After our risk stratification model and several interventions were adopted in 2022, the percentage of patients with high or medium risk of deteriorating health outcomes dropped from 34.9% to 24.4%, which is a reduction of 30%. The medium-complex patient population size, which had been 29% of all patients, decreased by about 4% to 5.7%. According to the analysis, the total risk score showed a strong correlation with 3 risk factors: dual diagnoses, the number of seen providers, and PHQ9 (0.63, 0.54, and 0.45 correlation coefficients, respectively).

Abstract:Objective: To analyze the effects of a standardized community health worker (CHW) intervention on hospitalization. Data sources/study setting: Pooled data from three randomized clinical trials (n = 1340) conducted between 2011 and 2016. Study design: The trials in this pooled analysis were conducted across diseases and settings, with a common study design, intervention, and outcome measures. Participants were patients living in high-poverty regions of Philadelphia and were predominantly Medicaid insured. They were randomly assigned to receive usual care versus IMPaCT, an intervention in which CHWs provide tailored social support, health behavior coaching, connection with resources, and health system navigation. Trial one (n = 446) tested two weeks of IMPaCT among hospitalized general medical patients. Trial two (n = 302) tested six months of IMPaCT among outpatients at two academic primary care clinics. Trial three (n = 592) tested six months of IMPaCT among outpatients at academic, Veterans Affairs (VA), and Federally Qualified Health Center primary care practices. Data collection/extraction methods: The primary outcome for this study was all-cause hospitalization, as measured by total number of hospital days per patient. Hospitalization data were collected from statewide or VA databases at 30 days postenrollment in Trial 1, twelve months postenrollment in Trial 2, and nine months postenrollment in Trial 3. Principal findings: Over 9398 observed patient months, the total number of hospital days per patient in the intervention group was 66 percent of the total in the control group (849 days for 674 intervention patients vs 1258 days for 660 control patients, incidence rate ratio (IRR) 0.66, P < .0001). This reduction was driven by fewer hospitalizations per patient (0.27 vs 0.34, P < .0001) and shorter mean length of stay (4.72 vs 5.57 days, P = .03). The intervention also decreased rates of hospitalization outside patients' primary health system (18.8 percent vs 34.8 percent, P = .0023). Conclusions: Data from three randomized clinical trials across multiple settings show that a standardized CHW intervention reduced total hospital days and hospitalizations outside the primary health system. This is the largest analysis of randomized trials to demonstrate reductions in hospitalization with a health system-based social intervention.
