Abstract:OBJECTIVE To evaluate the effects of different intracoronal bleaching methods on the shear bond strengths (SBS) of orthodontic brackets. MATERIALS AND METHODS Sixty freshly extracted mandibular incisors were randomly divided into four groups (n = 15 per group). After filling the root canals, root fillings were removed 2 mm apical to the cementoenamel junction, and a 2-mm-thick layer of zinc-phosphate cement base was applied. Group 1 served as the control. Intracoronal bleaching was performed with hydrogen peroxide (Opalacence Endo, Ultradent products Inc, South Jordan, Utah) in group 2, sodium perborate (Sultan Healthcare, Englewood, NJ) in group 3, and 37% carbamide peroxide (Whiteness Super Endo, Dentscare, ltda, Joinville, Brazil) in group 4. Orthodontic brackets were bonded with a light cure composite resin and cured with an LED light. After bonding, the SBS of the brackets was tested with a Universal testing machine. RESULTS Analysis of variance indicated a significant difference between groups (P < .001). The highest values for SBS were measured in group 1 (10.15 ± 1.15 MPa). The SBS was significantly lower in groups 2, 3, and 4 than in group 1 (P < .001). The lowest values for SBS were measured in group 3 (6.17 ± 0.85 MPa). SBS was significantly higher in group 4 than in group 3 (P < .05). CONCLUSIONS Intracoronal bleaching significantly affected the SBS of orthodontic brackets on human enamel. Bleaching with sodium perborate affects SBS more adversely than does bleaching with other agents.

Abstract:Abstract Background The High Intensity Users (HIU) Team supports individuals with single or multi-morbidities or complex care needs who are high users of Health and Social Care Services. Introduction Stoke-on-Trent has an aged population rising: 65 years plus by 17% increase to 280853; 85 years plus by 38% increase to 39425. Frail older people are the most frequent cohort of patients converted to admissions from ED. From April 2021 to October 2022 there has been an increase of 18% in inpatient length of stay which may be detrimental to the older person. HIU was developed to support this cohort of patients by creating an MDT to create personalised anticipatory care plans. Methods Individuals who have had three or more acute admissions and forty days non-elective in-patient days during the previous twelve months were included. An MDT facilitated by a Case Manager formulated an action plan to streamline the care; this includes the Frailty Passport which will identify the advanced/anticipatory care wishes of the individual. This is shared across all relevant teams. Once the actions are complete the patient is then monitored, if stable the patient then has an identified case worker to manage their on-going care. Results In the twelve months post consent: ED attendances reduced by 108; Admissions reduced by 477; Length of Stay reduced by 7086 (bed days); Cost saving £2,480,100; Service Cost £400,000; Qualitative feedback 93% patients would recommend; Staff 88% feel there are benefits for patients/services. Conclusion The results quite clearly demonstrate the benefits of patient centred care and approach when introducing an anticipatory care plan, including: Raised patient, family and staff satisfaction/ reassurance; Improved access to services; Right care, right time, right professional; Value for money. Next steps Useful tool. More studies needed. Expand and share learning with others.

Abstract:444 Background: CMS approved Chronic (CCM) and Principal Care Management (PCM) to improve patient care. These services are typically non-face-to-face; providers can bill ≥ twenty minutes /month. In oncology, this may help with comorbidity and symptom management to reduce the risk and expense of avoidable hospitalizations and ER visits and pay providers for time spent. In 2021, a system of community cancer centers initiated a program with a partner to manage chronic conditions more effectively at home. Approximately three hundred people were enrolled. A review of the data found that patients did not meet the medical criteria for the program, and care was not billable. An internal program was created to improve the enrollment process and financial sustainability. A clinical needs assessment was completed with our oncologists, outlining revised criteria for patient enrollment. Qualified patients were enrolled by their attending oncologist during the initial face-to-face visit and assigned a dedicated RN. Methods: Three hundred unenrolled, actively treating patients were randomly selected from January 2023 to May 2023 by a group of data analysts not associated with the program to be compared to enrolled patients who have been enrolled on average six months. Patients were compared by age, gender, cancer type, and race. The LACE Index for Readmission, a validated tool using the length of stay, acuity of admission, comorbidities, and ER visits to predict the risk of readmission, was done retrospectively by RNs for all patients. Enrolled patients received a comprehensive clinical needs assessment by RNs, and a provider-approved care plan was created. RNs provided patients with education on lifestyle modification and assisted in setting achievable goals. The care plan was monitored with weekly calls, providing the opportunity to respond to patients’ needs proactively. Symptom and medication management were provided, and psychological and nutritional needs were addressed. This coordination of services was documented in the electronic health record, and superbills were created and sent to billing and coding monthly. Results: See table. Conclusions: The purpose of this study was to compare hospital admissions between the groups. Our findings suggest that CCM and PCM may reduce unplanned admissions, with physician engagement pivotal in driving appropriate enrollment. The current program is labor-intensive and further investigation is required to determine the most cost-effective way of scaling the program with the possible use of artificial intelligence, RPM, or other technology.[Table: see text]

Better Care, Same Cost - Reducing Unplanned Care for Multi-visit Patients: A Payer-Provider Model

Abstract:Importance: Many interventions implemented for multi-visit patients (MVP) have been developed to address patient-centric needs of these individuals and reduce unplanned care for ambulatory-sensitive conditions. More rigorous research is needed to better understand the impact of these interventions on changes in care utilization including unplanned care. Objective: To evaluate the impact of the Enhanced Care Program (ECP), a payer-provider collaborative model, on unplanned care use and cost of care. Design: Using propensity methods, a comparison group was constructed using insurer membership files. Comparisons were performed using a difference-in-differences analysis. Participants: Patients enrolled in ECP through December 2019 were considered eligible for the study (n = 357). All patients had five or more ED visits in the past year or two or more inpatient hospitalizations in the past year prior to enrollment. Exposures: ECP is a high-intensity outpatient intervention intended to reduce avoidable unplanned care such as ED visits and inpatient hospital stays through home visits, chronic/acute disease management, and intensive care coordination. Main measures: The primary outcomes of interest were events per 100 members per year of ED use with return to home, unplanned inpatient and observational status admissions, and unplanned behavioral health inpatient admission, and cost of care per member per month. Key results: Overall total unplanned care encounters were significantly reduced with a difference-in-difference of 320 unplanned care encounters per 100 members per year in the intervention group (p < 0.05). The ECP group showed statistically significant decreases in costs of unplanned ED, unplanned observation admission, and unplanned inpatient behavioral medicine costs, but statistically significant increases in overall pharmacy costs and lab costs. Changes in total costs of care for the ECP group were not statistically different than the control group (p = 0.55). Conclusions: ECP showed significant reduction of unplanned care for MVP patients.

Abstract:282 Background: Patients who have been diagnosed with cancer have an increased risk of complications and potential hospitalization instances. Those in more advanced age groups have an additional risk, especially those of the age group of 65 and higher. Avoidable hospitalizations increase the total cost of care and decrease patient quality of life during treatment. Methods: To reduce potentially avoidable hospital admissions, patients enrolled in a commercial insurance contract were reviewed weekly for a high-risk potential. Criteria for high-risk inclusion included: Any patient >75 years of age receiving treatment; Any patient receiving chemotherapy/radiation combination therapy; Any patient with the following diagnosis: Metastatic lung cancer, Metastatic pancreatic cancer, Multiple myeloma, Breast cancer, Prostate cancer with multiple HCC; Any patient receiving: Cisplatin, Adriamycin, Taxanes, Ifosfimide, Doxorubicin, or Carbo with AUC 4 or greater; Any patient that has had one visit to ER/hospital during the program During the recording period, the high-risk list of patients was reviewed weekly. Before the weekend, each patient identified was contacted by phone by a registered nurse for a weekly “touch call” to address side effects, treatment questions, complications, or concerns. Patients were reminded to contact the practice first when a non-life-threatening concern occurs. Results: The program began on April 1, 2020, with a baseline admission rate of 1.8%. At the conclusion of the program on March 31st, 2023, this cohort of patients had a 53.1% reduction in admission rates from their baseline. Additionally, the reimbursement PPPM was under target yearly, concluding at $65.64, $14.46 below the target amount of $80. The cohort consisted of a range of 36-75 patients per quarter, program total of 185 enrolled patients. Conclusions: Identifying potentially high-risk patients and conducting weekly, personalized calls before the weekend reduced the overall avoidable admission rate. Timely identification and support for high-risk patients reduces the total cost of care and potentially increases patient care quality during treatment. Abstract:House call programs may save health care costs in high-risk patients. This retrospective cohort study aimed to evaluate the effect of a house call program on health care costs and utilization in Medicare beneficiaries who received care at Beth Israel Deaconess Medical Center, Boston, MA. In November 2012-June 2013, 548 beneficiaries who were at high risk for hospitalizations were contacted for a house call program; 307 who agreed received at least monthly home visits by nurse practitioners who collaborated with their physicians for disease management and care coordination. The remaining 241 beneficiaries received usual care without home visits or care coordination. The outcomes of interest were changes in the 12-month total Medicare part A and B cost and health care utilization before and after their contact date. Compared with the usual care group, the house call group was older (82 vs. 79 years) and had higher burden of chronic conditions. After propensity score matching, the average total Medicare cost per member per month increased by $494 in the house call group ($1875 to $2369) vs. $47 in the usual care group ($1845 to $1892) (p=0.01). The number of emergency room visits, hospitalizations, skilled nursing facility admissions did not differ between the groups; home health agency use per 100 person-months changed by 125 days in the house call group (313 to 438 days) vs. -48 days (309 to 261 days) (p=0.01). In conclusion, our house call program was associated with increased health care cost and home health agency use.

Abstract:Abstract Background Person-centred care (PCC) focusing on personalised goals and care plans derived from “What matters to you?” has an impact on single disease outcomes, but studies on multi-morbid elderly are lacking. Furthermore, the combination of PCC, Integrated Care (IC) and Pro-active care are widely recognised as desirable for multi-morbid elderly, yet previous studies focus on single components only, leaving synergies unexplored. The effect of a synergistic intervention, which implements 1) Person-centred goal-oriented care driven by “What matters to you?” with 2) IC and 3) pro-active care is unknown. Methods Inspired by theoretical foundations, complexity science, previous health service research and a patient-driven evaluation of care quality, we designed the Patient-Centred Team (PACT) intervention across primary and secondary care. The PACT team collaborate with the patient to make and deliver a person-centred, integrated and proactive multi-morbidity care-plan. The control group receives conventional care. The study design is a pragmatic six months prospective, controlled clinical trial based on hospital electronic health record data of 439 multi-morbid frail elderly at risk for emergency (re) admissions referred to PACT and 779 propensity score matched controls in Norway, 2014–2016. Outcomes are emergency admissions, the sum of emergency inpatient bed days, 30-day readmissions, planned and emergency outpatient visits and mortality at three and six months follow-up. Results The Rate Ratios (RR) for emergency admissions was 0,9 (95%CI: 0,82-0,99), for sum of emergency bed days 0,68 (95%CI:0,52-0,79) and for 30-days emergency readmissions 0,72 (95%CI: 0,41-1,24). RRs were 2,3 (95%CI: 2,02-2,55) and 0,9 (95%CI: 0,68-1,20) for planned and emergency outpatient visits respectively. The RR for death at 3 months was 0,39 (95% CI: 0,22-0,70) and 0,57 (95% CI: 0,34-0,94) at 6 months. Conclusion Compared with propensity score matched controls, the care process of frail multi-morbid elderly who received the PACT intervention had a reduced risk of high-level emergency care, increased use of low-level planned care, and substantially reduced mortality risk. Further study of process differences between groups is warranted to understand the genesis of these results better. Team functioning as a predictor of patient outcomes in early medical home implementation

Abstract:Background: New models of patient-centered primary care such as the patient-centered medical home (PCMH) depend on high levels of interdisciplinary primary care team functioning to achieve improved outcomes. A few studies have qualitatively assessed barriers and facilitators to optimal team functioning; however, we know of no prior study that assesses PCMH team functioning in relationship to patient health outcomes. Purpose: The aim of the study was to assess the relationships between primary care team functioning, patients' use of acute care, and mortality. Methodology/approach: Retrospective longitudinal cohort analysis of patient outcomes measured at two time points (2012 and 2013) after PCMH implementation began in Veterans Health Administration practices. Multilevel models examined practice-level measures of team functioning in relationship to patient outcomes (all-cause and ambulatory care-sensitive condition-related hospitalizations, emergency department visits, and mortality). We controlled for practice-level factors likely to affect team functioning, including leadership support, provider and staff burnout, and staffing sufficiency, as well as for individual patient characteristics. We also tested the model among a subgroup of vulnerable patients (homeless, mentally ill, or with dementia). Results: In adjusted analyses, higher team functioning was associated with lower mortality (OR = 0.92, p = .04) among all patients and with fewer all-cause admissions (incidence rate ratio [IRR] = 0.90, p < 0.01), ambulatory care-sensitive condition-related admissions (IRR = 0.91, p = .04), and emergency department visits (IRR = 0.91, p = .03) in the vulnerable patient subgroup. Conclusion: These early findings give support for the importance of team functioning within PCMH models for achieving improved patient outcomes. Practice implications: A focus on team functioning is important especially in the early implementation of team-based primary care models.

Abstract:Context: The Center for Medicare and Medicaid Innovation (CMMI) awarded Denver Health's (DH) integrated, safety net health care system $19.8 million to implement a "population health" approach into the delivery of primary care. This major practice transformation builds on the Patient Centered Medical Home (PCMH) and Wagner's Chronic Care Model (CCM) to achieve the "Triple Aim": improved health for populations, care to individuals, and lower per capita costs. Case description: This paper presents a case study of how DH integrated published predictive models and front-line clinical judgment to implement a clinically actionable, risk stratification of patients. This population segmentation approach was used to deploy enhanced care team staff resources and to tailor care-management services to patient need, especially for patients at high risk of avoidable hospitalization. Developing, implementing, and gaining clinical acceptance of the Health Information Technology (HIT) solution for patient risk stratification was a major grant objective. Findings: In addition to describing the Information Technology (IT) solution itself, we focus on the leadership and organizational processes that facilitated its multidisciplinary development and ongoing iterative refinement, including the following: team composition, target population definition, algorithm rule development, performance assessment, and clinical-workflow optimization. We provide examples of how dynamic business intelligence tools facilitated clinical accessibility for program design decisions by enabling real-time data views from a population perspective down to patient-specific variables. Conclusions: We conclude that population segmentation approaches that integrate clinical perspectives with predictive modeling results can better identify high opportunity patients amenable to medical home-based, enhanced care team interventions.

Abstract:Curiously journals of mass spectrometry for me always have seemed to have something to do with Australia. I migrated from England to Australia in 1975 to work with one of the eminent " Jim's " in mass spectrometry in Australia—Jim Morrison in Melbourne—and later moved to be with the other " Jim " in mass spectrometry in Australia—Jim Shannon in Sydney. Almost on day one, Jim Shannon said I should take over as Australasian Editor of Organic Mass Spectrometry. I agreed, but I'm not sure that I understood exactly what I was agreeing to. And I don't think that I did anything much for the journal. I failed completely to persuade Jim Morrison to publish in the journal, and I noticed that other Australian mass spectroscopists would submit their papers directly to the Editor-in-Chief Allan Maccoll in London. Allan Maccoll was the Australian's Australian in Europe. So as Australasian Editor I didn't learn that much about editing and publishing scientific journals—but I probably did catch the " publishing bug ". Moving back to England in the eighties, I relinquished the Australasian Editorship to John Macleod. I put thoughts of journals aside, until Fred McLafferty opined that I should be Editor-in-Chief of Organic Mass Spectrometry. Allan Maccoll probably saw me as the least objectionable of the possible successors—at least, by that time, I was an Australian (on paper at least)! That was the beginning of five very good years, in which I came to know well and respect greatly Hans-Friedrich Grützmacher—the European Editor of Organic Mass Spectrometry—and also John Holmes—North American Editor—Mamoru Ohashi in Japan and Oleg Chizhov in Russia. I came to know Brian Chait, whose interests had always been close to mine and who joined the team as an Editor. And significantly, I worked alongside and grew close to Allan Maccoll. Metrics were not so important in those days, but in the beginning all of us could see that submissions were not going up, colleagues would from time to time mention losing their libraries' subscription and we had that feeling of being slightly off the pace in terms of where the field of mass spectrometry was going. So we did the " necessary " and we thought by 1994 that we had turned the journal around—certainly there were more pages of mass spectrometry (1679 in 1993 compared to 698 in 1990). Implementation of an Interdisciplinary, Team-Based Complex Care Support Health Care Model at an Academic Medical Center: Impact on Health Care Utilization and Quality of Life

Abstract:INTRODUCTION: The Geriatric Resources for the Assessment and Care of Elders (GRACE) program has been shown to decrease acute care utilization and increase patient self-rated health in low-income seniors at community-based health centers.AIMS: To describe adaptation of the GRACE model to include adults of all ages (named Care Support) and to evaluate the process and impact of Care Support implementation at an urban academic medical center.SETTING: 152 high-risk patients (≥5 ED visits or ≥2 hospitalizations in the past 12 months) enrolled from four medical clinics from 4/29/2013 to 5/31/2014.PROGRAM DESCRIPTION: Patients received a comprehensive in-home assessment by a nurse practitioner/social worker (NP/SW) team, who then met with a larger interdisciplinary team to develop an individualized care plan. In consultation with the primary care team, standardized care protocols were activated to address relevant key issues as needed.PROGRAM EVALUATION: A process evaluation based on the Consolidated Framework for Implementation Research identified key adaptations of the original model, which included streamlining of standardized protocols, augmenting mental health interventions and performing some assessments in the clinic. A summative evaluation found a significant decline in the median number of ED visits (5.5 to 0, p = 0.015) and hospitalizations (5.5 to 0, p<0.001) 6 months before enrollment in Care Support compared to 6 months after enrollment. In addition, the percent of patients reporting better self-rated health increased from 31% at enrollment to 64% at 9 months (p = 0.002). Semi-structured interviews with Care Support team members identified patients with multiple, complex conditions; little community support; and mild anxiety as those who appeared to benefit the most from the program.DISCUSSION: It was feasible to implement GRACE/Care Support at an academic medical center by making adaptations based on local needs. Care Support patients experienced significant reductions in acute care utilization and significant improvements in self-rated health.

Abstract:BACKGROUND: Nurse care management programs for patients with chronic illness have been shown to be safe and effective.OBJECTIVE: To determine whether a telephone-mediated nurse care management program for heart failure reduced the rate of rehospitalization for heart failure and for all causes over a 1-year period.DESIGN: Randomized, controlled trial of usual care with nurse management versus usual care alone in patients hospitalized for heart failure from May 1998 through October 2001.SETTING: 5 northern California hospitals in a large health maintenance organization.PATIENTS: Of 2786 patients screened, 462 met clinical criteria for heart failure and were randomly assigned (228 to intervention and 234 to usual care).INTERVENTION: Nurse care management provided structured telephone surveillance and treatment for heart failure and coordination of patients' care with primary care physicians.MEASUREMENTS: Time to first rehospitalization for heart failure or for any cause and time to a combined end point of first rehospitalization, emergency department visit, or death.RESULTS: At 1 year, half of the patients had been rehospitalized at least once and 11% had died. Only one third of rehospitalizations were for heart failure. The rate of first rehospitalization for heart failure was similar in both groups (proportional hazard, 0.85 [95% CI, 0.46 to 1.57]). The rate of all-cause rehospitalization was similar (proportional hazard, 0.98 [CI, 0.76 to 1.27]).LIMITATIONS: The findings of this study, conducted in a single health care system, may not be generalizable to other health care systems. The overall effect of the intervention was minor.CONCLUSIONS: Among patients with heart failure at low risk on the basis of sociodemographic and medical attributes, nurse care management did not statistically significantly reduce rehospitalizations for heart failure or for any cause. Such programs may be less effective for patients at low risk than those at high risk.

Abstract:This research seeks to assess the potential of regionally integrated health management for specific sub-populations, including the incorporation of self-management initiatives. It will achieve this by conducting a thorough stratification analysis of hospital data, utilizing the Adjusted Clinical Groups (ACG) classification system. The approach involves a retrospective review of healthcare data spanning five years, which includes patient demographics, health outcomes, and healthcare utilization metrics. We intend to use the ACG method to classify the patient population into pertinent groups that mirror their health requirements and resource use. The insights obtained from this analysis will be used to create a localized adaptation of the Kaiser Permanente Pyramid Model of Care. This adaptation aims to identify the distribution of costs among patients treated in the Rivierenland Hospital. We anticipate that stratifying data with the ACG method will identify distinct multimorbid subgroups. These subgroups will have unique healthcare requirements. Early interventions and customized health management strategies, based on these insights, could enhance health outcomes and resource efficiency for high-risk patients. This analysis will serve as a foundation for constructive discussions with hospital management and clinical staff, fostering a deeper comprehension of the patients' burden of disease. It might also foster multidisciplinary collaboration opportunities between medical specialties as with regional healthcare partners such as general practitioners (GPs), mental health and other long-term care organizations. Moreover, we anticipate that self-care initiatives, supported by customized health information, will encourage increased patient engagement and strategies for enhancing lifestyle improvements. This strategy is expected to enable the personalization of advanced care planning based on individual needs profiles, thereby improving the management of complex and chronic conditions, and encouraging self-care practices. Our anticipated findings highlight the potential benefits of a data-informed approach to advancing healthcare outcomes and present opportunities for future investigations to refine and implement such integrated care models across the region.

Abstract:ABSTRACTBackgroundThe ability of latent class models to identify clinically distinct groups among high-risk patients has been demonstrated, but it is unclear how healthcare data can inform group-specific intervention design.ObjectiveExamine how utilization patterns across latent groups of high-risk patients provide actionable information to guide group-specific intervention design.DesignCohort study using data from 2012 to 2015.PatientsParticipants were 934,787 patients receiving primary care in the Veterans Health Administration, with predicted probability of 12-month hospitalization in the top 10th percentile during 2014.Main MeasuresPatients were assigned to latent groups via mixture-item response theory models based on 28 chronic conditions. We modeled odds of all-cause mortality, hospitalizations, and 30-day re-hospitalizations by group membership. Detailed outpatient and inpatient utilization patterns were compared between groups.Key ResultsA total of 764,257 (81.8%) of patients were matched with a comorbidity group. Groups were characterized by substance use disorders (14.0% of patients assigned), cardiometabolic conditions (25.7%), mental health conditions (17.6%), pain/arthritis (19.1%), cancer (15.3%), and liver disease (8.3%). One-year mortality ranged from 2.7% in the Mental Health group to 14.9% in the Cancer group, compared to 8.5% overall. In adjusted models, group assignment predicted significantly different odds of each outcome. Groups differed in their utilization of multiple types of care. For example, patients in the Pain group had the highest utilization of in-person primary care, with a mean (SD) of 5.3 (5.0) visits in the year of follow-up, while the Substance Use Disorder group had the lowest, with 3.9 (4.1) visits. The Substance Use Disorder group also had the highest rates of using services for housing instability (25.1%), followed by the Liver group (10.1%).ConclusionsLatent groups of high-risk patients had distinct hospitalization and utilization profiles, despite having comparable levels of predicted baseline risk. Utilization profiles pointed towards system-specific care needs that could inform tailored interventions.

Abstract:BACKGROUND: Value-based care is an opportunity for medication optimization services to improve medication management and reduce health care spending. The reach of these services may be extended through telehealth. However, as health care systems and payers grapple with the long-term financing of telehealth, real-world assessments are needed to evaluate the potential economic impact of pharmacy-driven telehealth services. OBJECTIVE: To evaluate the impact of a scalable pharmacist-driven telehealth intervention to improve medication management on health care spending for clinically complex patients who were enrolled in a Medicare Next Generation Accountable Care Organization. METHODS: Data for this pretest-posttest nonequivalent group design study came from Medicare claims from 2015 to 2020 and linked pharmacist care activity data derived from the electronic medical record. Patients in the intervention group were identified as those who received the telehealth medication management service. Patients in the control group were offered the service and refused or could not be contacted. The primary outcome was total medical spending over a 6-month period, and impact was assessed using a covariate-adjusted difference-in-difference model. RESULTS: There were 581 patients who received the intervention and 1,765 who served as controls. The telehealth intervention reduced total medical spending by $2,331.85 per patient over the first 6 months of the service ($388.50 per month; P = 0.0261). Across a range of estimates for the cost of service delivery, we find a return on investment of 3.6:1 to 5.2:1. CONCLUSIONS: The $388.50 monthly savings found in this study represent a substantial reduction in health care spending and emphasize the opportunity for telehealth delivery of medication management services to improve value as a part of alternative payment models. Abstract:Background: Chronic diseases are the leading cause of disability and death in the United States. Clinical pharmacists have been shown to optimize health outcomes and reduce healthcare expenditures in patients with chronic diseases through improving medication adherence. Objective: The primary objective of this study was to evaluate a pharmacist-led, artificial intelligence-supported medication adherence program on medication adherence, select disease control measures, and healthcare expenditures. Methods: This was a multicenter, retrospective, quasi-experimental evaluation from January 2019 to December 2019 (pre-implementation) and January 2021 to December 2021 (post-implementation). This pharmacy-driven service focuses on improving medication adherence and patient outcomes through artificial intelligence-supported analytics, individual patient case review, and pharmacist-led individual patient outreach. The primary endpoint was to determine if implementation improved medication adherence in three medication-related measures: medication adherence for hypertension (MAH), medication adherence for cholesterol (MAC), and medication adherence for diabetes (MAD). Secondary outcomes were to evaluate reductions in select chronic diseases control measures and cost savings of this service following implementation of this service. Results: This medication adherence service was deployed across 10,477 patients: 60.6% of patients were in at least one medication-related measure, generating 2,762 actionable medication adherence gaps. Following the implementation of this pharmacist-led program, medication adherence improved in all three disease state measures (MAH: 5.9% improvement; MAC: 7.9% improvement; MAD: 6.4% improvement), and Medicare Star ratings also improved. The percentage of patients with diabetes who reached their A1c goal also increased (75.5% to 81.7%). Furthermore, reductions in overall healthcare expenditures were seen per member per month in patients that were adherent in comparison to those who were non-adherent (hypertension: 31% cost savings; hyperlipidemia 25% cost savings; diabetes: 32% cost savings). Conclusion: This clinical pharmacist driven service leveraged technology and patient connection to increase medication adherence in patients with chronic disease states and led to improvement in select disease control measures and substantial healthcare cost savings.
