Abstract:Introduction: T wave oversensing (TWOS) is a major drawback of implantable cardioverter defibrillator (ICD) and data on predictors of TWOS in ICD is limited. We aimed to calculate a novel index of T wave safety margin (TWSM) and assess its potential for evaluating TWOS during the procedure of ICD implantation. Methods and Results: Thirty-two consecutive patients with ICD implantation were enrolled. During each procedure of ICD implantation, different ICD generators were connected to implanted sensing lead through active-fixation leads and bridging cables. R and T wave amplitudes were measured on ICD printouts according to the gain. The ICDs were programed to the most sensitive settings to reveal possible TWOS. A novel index TWSM was calculated according to the corresponding sensing algorithm of ICD. There was discrepancy of R wave amplitudes measured by different ICDs (P < 0.01). In Fortify and Teligen ICDs, T wave amplitudes showed no difference (P > 0.05) and TWSMs were sufficiently high (post sensing: 13.0 ± 7.6 and 28.3 ± 16.5, respectively, post pacing: 5.0 ± 2.2 and 4.6 ± 0.9, respectively). In nine patients with 10 TWOS episodes detected during the procedure of ICD implantation, generators with the highest TWSM were chosen. Only one TWOS episode during pacing was recorded during the 25 ± 7 mo follow-up period. Conclusions: We first propose the index of TWSM during ICD implantation as a potentially efficient predictor for TWOS. Evaluation of TWSM might help to reduce TWOS episodes in patients with high risk of TWOS. Prospective studies are warranted to validate this index and its potential to reduce TWOS episodes.
Analysis of causes of inappropriate discharge of an implantable cardioverter defibrillator in elderly patients

Abstract:Objective To analyze the causes of inappropriate discharge of implantable cardioverter defibrillator (ICD) in elderly patients and to discuss the corresponding solutions.Methods Totally 95 elderly patients with ICD were collected.They were followed up and the inappropriate ICD discharges were recorded and analyzed.Results All patients were followed up for 235 times/year totally.There were 16 patients with inappropriate ICD discharges.The causes of inappropriate ICD discharges included ventricular oversensing in 7 patients (43.7％),supraventricular tachycardia misclassified into ventricular tachycardia in 4 patients (25.0％),electrode fracture in 2 patients (12.5％),and electromyogram (EMG) interference,electromagnetic interference and electrode fracture with EMG interference in 1 patients respectively (6.3％ each).11 inappropriate ICD discharges were reduced and avoided by reprogrammed and noninvasive treatment in 16 patients.1 patient with the underdetected ventricular fibrillation after reprogramming and the problem was solved by external defibrillation.The other 5 patients got better by the invasive treatment.Conclusions Different causes account for inappropriate discharges in patients with ICD.Reprogramming the corresponding parameters in different ICD can reduce or avoid inappropriate discharges without affecting the sensitivity of ICD.

Abstract:This paper explores the effect of the load imposed by implantable cardioverter defibrillators (ICDs) on their lithium battery power sources longevity using a simulation approach that incorporates a coupled electro-thermal dynamic model. ICDs are one of the effective treatments available to significantly improve survival of patients with fatal arrhythmia (abnormal heart rhythm) disorders. Using a lithium battery power source, this life-saving device sends electrical shocks or pulses to regulate the heartbeat. The service life and reliability of an ICD is primarily expressed by its battery's lifespan and performance. In this paper we investigate the terminal voltage, depth of discharge and temperature dynamics of the implantable lithium battery with a combined cathode material, namely silver vanadium oxide and carbon-monofluoride (Li/SVO-CF x ). Modeling the implantable batteries characteristics is a well-established topic in literature; however, to the best of the author's knowledge, the impact of the high-energy shocks (defibrillation) and low-energy device power supply (housekeeping) on the ICD's battery operation is relatively less-explored. Our analysis reveals that the battery terminal voltage is primarily influenced by the continuous low-level housekeeping discharge current within the range of micro amps, rather than the intermittent high-level demand of defibrillation current within the range of several amperes. A Monte Carlo simulation and model prediction comparison with real-time experimental data from literature were used to assess the accuracy and applicability of the model. The results can be used to improve the device battery design, control and operation, thus extending the service life in patients and reducing the need for invasive replacement surgery.

Abstract:UNLABELLED We report on a 2-y-old male with intussusception accompanying nutcracker syndrome detected by renal Doppler ultrasound. Renal Doppler ultrasound revealed a significant difference in the peak velocity between the hilar and aortomesenteric portions of the left renal vein. CONCLUSION Renal Doppler ultrasound has been very useful and effective in detecting nutcracker syndrome in a young child.

A Rare Complication of Implantable Cardiac Defibrillator Placement

Abstract:Implantable cardiac defibrillators (ICDs) are a popular and effective option in heart failure with left ventricular systolic dysfunction patients. Although frequently underdiagnosed, inadvertent malposition can lead to endocardial damage and thrombotic events. As ICD implants tend to increase in the following years, the recognition of their complications is critical. The authors present a case of a 64-year-old female with advanced heart failure and ICD malposition. This accidental discovery was denounced by the presence of a right bundle branch block (RBBB) pattern and later confirmed by echocardiography which showed the lead tip in contact with the midsegment of the left ventricular anterolateral wall. As the patient's hospitalization was complicated with refractory ascites and cardiogenic shock, she underwent cardiac transplantation, with no recurrence of heart failure symptoms. An electrocardiogram showing a RBBB pattern during VVI pacing should arise the suspicion of inadvertent incorrect placement of a pacing/ICD lead. The many facets of echocardiography should be used for the diagnosis of this complication, as they were paramount in this case, as highlighted.

Abstract:Insulation defects in the leads are a common complication in implantable cardioverter-defibrillators (ICDs). Although the new multilumen leads have significantly reduced the incidence of insulation defects, they have introduced the new and rare problem of outer-insulation abrasions. In this report, we present the case of a 53-year-old man with dilated cardiomyopathy who was found to have an outer-insulation abrasion on fluoroscopy 3 years after the implantation of an ICD. Several electrical storms experienced by the patient were considered to be responsible for the abrasions; results of routine evaluations were normal. A CRT-D device was finally implanted.

Abstract:Implantable cardioverter-defibrillators (ICDs) are increasingly used for the primary and secondary prevention of sudden cardiac death (SCD). Currently, transvenous (TV) and subcutaneous (S) ICDs are the two different types used. Preservation of central venous vasculature, no risk of vascular or myocardial injury during implant, easier explantation, and lower risk of systemic infections have driven the increased use of S-ICDs. The shocks delivered by ICDs for non-life-threatening arrhythmias or because of oversensing T waves or noise are known as inappropriate shocks. Here, we present the case of a 33-year-old man who had an S-ICD implanted in 2019 for hypertrophic cardiomyopathy. He had a TV-ICD implanted in 2010 which was explanted in 2013 due to infective endocarditis, and the patient underwent a mechanical mitral valve replacement. He was at intermediate risk for SCD over the next five years. He had an S-ICD implanted in 2019 and had never received any shock before. Electrocardiogram showed normal sinus rhythm, left axis deviation, QRS 110 ms, hyperacute T waves in inferior leads, and T-wave inversion in lateral leads. He then began experiencing inappropriate shocks three years after S-ICD placement due to a drop in R wave amplitude secondary to noise oversensing in October 2022. Despite reprogramming the device from the primary vector to an alternate vector, the patient had further inappropriate shocks two months later due to noise oversensing. The patient was discussed in a multidisciplinary team meeting and the S-ICD was explanted according to the patient's wishes and a loop recorder was implanted.
Incidence and Causes of Inappropriate Detection and Therapy by Implantable Defibrillators of Cardioversion in Patients with Ventricular Tachyarrhythmia

Abstract:Background Implantable cardioverter defibrillator (ICD) is the only effective therapy in patients with life threatening ventricular arrhythmias. Inappropriate detection and therapy by ICDs are the most common causes of side effects that affect the quality of life in ICD recipients. This study evaluated the incidence and causes of inappropriate detection and therapy by ICDs in patients in our hospital. Methods From January 2000 to December 2005, fifty patients who received ICD implantation for ventricular arrhythmias for prevention of sudden cardiac death were evaluated in this study. Each ICD was programmed using clinical arrhythmic and cardiac data of the patient before discharge. Patients were followed up by standard schedule after implantation and all data retrieved from each device were collected and saved for further analysis. Results No arrhythmic event was detected in 12/50 (24%) patients during the period of follow-up. Among the remaining patients, 11 (22%) experienced inappropriate detections and therapies during follow-up in this study. ICD detected 383 ventricular tachyarrhythmia (VT) and 108 ventricular fibrillation (VF) episodes and delivered 678 therapies. In VT group, ICD delivered 413 antitachycardiac pacings (ATPs) and 118 shocks, among which 78 ATPs and 9 shocks were initiated by 55/383 (14.3%) inappropriate detections. In VF group ICD delivered 147 shocks, among which 56 shocks were initiated by 28/108 (26.9%) inappropriate detections. Overall, more than 50% of these episodes were caused by atrial fibrillation (AF) with rapid ventricular response, followed by electromagnetic or myopotential interference. In addition, most inappropriate therapies occurred within one year after ICD implantation. Conclusions About one fifth of patients experienced ICD inappropriate detection and therapy after implantation. The main cause was AF with rapid ventricular response, followed by electromagnetic or myopotential interference.

Abstract:Abstract Background Subcutaneous implantable cardiac defibrillators (S-ICDs) protect against sudden cardiac death and avoid transvenous ICD lead complications. Objective To investigate S-ICD real-world battery longevity estimates and the impact of device parameters and advisory status. Methods Second and third generation S-ICD device longevity labeling is 7.3 years (yr) under normal use (e.g remote monitoring, pre-arrhythmia EGM storage, etc.). De-identified data from S-ICD devices implanted ≥3 months, with recent uploads from the patient devices to the LATITUDE system, were obtained in Nov 2022. For early to mid-life devices (15-100% battery life), longevity estimates were derived from storage time, implant time, and number of high voltage charges (HVC). For late-life devices, longevity estimates were calculated more accurately from battery voltage. Monte Carlo simulations estimated future HVCs. Multivariable maximum likelihood parameter models were used to analyze battery longevity determinants for the overall cohort and for patients who received >1 shock, assuming 1 shock due to conversion testing at implant. Variables were considered clinically significant if univariable difference and multivariable estimate’s magnitude were both ≥0.2 yr. Results are reported in median and interquartile range. Results The 32,678 EMBLEM devices had a 2.5 yr [1.2,4.1] device dwell-time with 8.7 yr [8.3,8.9] longevity estimate. Of these patients, 28,544 (87.3%) received ≥1 shocks and 11,480 (35.1%) received >2 shocks, including shocks delivered during conversion testing. Late life devices (n=3091, 9.5%) had a 5.2 yr [4.6,5.9] dwell time. For devices under the battery depletion advisory (n=13,900), the Monte Carlo model does not account for premature battery depletion (PBD), which is projected to impact 11.5% of advisory devices at 5 yrs. Longevity for PBD is 6.6 yr (Q4 2022 product performance report). While all variables except AF trend, AF episodes/yr, and post shock pacing statistically impact device longevity, only advisory status and number of device charges per yr impacted longevity by ≥0.2 yr (Figure, panel A). Patients who received >1 shock have 8.5 yr [8.0,8.8] longevity estimates, vs 8.8 [8.4,8.9] for patients with 0 or 1 shock. For patients with >1 shock, (Figure, panel B), only advisory status had a ≥0.2 yr impact on longevity in multivariable and univariable analyses. Dwell time, shocks delivered per yr, and system impedance software upgrade impacted longevity by only 0.1 yr in univariable analysis. Conclusion Median EMBLEM S-ICD longevity estimate is 8.7 years, which is longer than device labeling. Longevity is reduced by 0.26 years for each device charge. AF monitoring and post shock pacing did not impact device longevity significantly. Low and high voltage impedance as well as remote monitoring transmission impacted longevity by less than 0.1 years. An Unusual Case of Takotsubo Cardiomyopathy with Cardiac Arrest: The Importance of Clinical Context in Implanted Cardioverter Defibrillator Candidacy

Abstract:Takotsubo syndrome (TTS), also known as stress-induced cardiomyopathy, is characterized by acute heart failure, reversible left ventricular dysfunction, and other complications such as life-threatening arrhythmias. The management of TTS is challenging due to its unpredictable clinical course and the lack of evidence-based treatment recommendations. In this case report, we present a 71-year-old female who developed TTS with ventricular tachycardia (VT) cardiac arrest following septic shock and an exploratory laparotomy for appendicitis. Despite the presence of VT cardiac arrest and a left ventricular ejection fraction of 30-35%, an implanted cardioverter-defibrillator (ICD) was not indicated due to the rapid and satisfactory recovery of the patient's ventricular function. This case highlights the importance of considering the clinical context and the transient nature of TTS in the decision-making process for ICD candidacy.

Abstract:Through a secondary analysis of existing data, the authors researched the question of what types of aggressive behaviors are learned from an individual's social environment. The findings included data on three student groups: 1) students who had never been in military service; 2) students who had been in military service and had been to Viet Nam; and 3) students who had been in military service, but who had not been to Viet Nam. The results indicated that assault is clearly one aggressive behavior that appears to increase with exposure to an aggressive life experience.

Abstract:Early repolarization syndrome (ERS) was initially thought to be a good prognostic manifestation detectable by electrocardiogram. In recent years, with the gradual deepening of research and understanding, ERS is considered to possess malignant tendency and related to ventricular fibrillation (VF) and sudden cardiac death (SCD). Implantable cardioverter defibrillator (ICD) is currently the first choice of treatment used to reduce the risk of potentially life-threatening arrhythmias and SCD in patients at high risk for ERS. The stress of living with an implanted device and receiving ICD shock has been noted to exert a psychological toll on patients, especially those who have previously experienced VF and syncope. The current case report describes symptoms and signs of a patient with ERS, and discusses the changes of psychological condition after ICD implantation. Moreover, we explored a targeted treatment approach for anxiety and depression in an individual with an ICD for ERS, which combined medication, cognitive behavioral therapy, and physical exercise. We believe that the psychological experience of ICD recipients, which is a critical component in designing the biopsychosocial therapeutic approach for this growing patient population, is worthy of more attention.
An inappropriate pacing threshold increase after repeated electrical storm in a patient with implantable cardioverter defibrillator

Abstract:BackgroundImplantable cardioverter defibrillators (ICD) are capable of effectively terminating malignant ventricular arrhythmia and are the most effective way to prevent sudden cardiac death. However, some evidences demonstrated that both anti-tachycardia pacing (ATP) and ICD shock can also bring adverse prognosis.Case presentationA 66-year-old Han Chinese man with prior ICD implantation was admitted to our hospital because of frequent ICD shocks. Although intravenous amiodarone and esmolol succinate were administered daily, the patient suffered 155 episodes of VT/VF during 8 weeks after implantation. After repeated discharge of the device, the pacing threshold of the patient increased gradually. Considering the inappropriate increase of the pacing threshold, we decided to reposition the right ventricular (RV) lead with good sensing and threshold parameters confirmed. Subsequent 22 months interrogation follow-up revealed a stable lead position and electrical specifications. Furthermore, antiarrhythmic drugs were maximally increased, while ATP burst was remarkably decreased and the inappropriate ICD shock never occurred until now.ConclusionAn inappropriate pacing threshold was increased secondary to repeated ICD electrical storm. A timely active lead position adjustment reduced the pacing threshold and eliminated the risk of premature battery depletion.

Retrospective Analysis of the Appropriate and Inappropriate Shock Characteristics in 134 Patients with Implantable Cardioverter-Defibrillator

Abstract:Objective To analyze the appropriate and inappropriate shock characteristics in patients with implantable cardioverter-defibrillator (ICD), explore the influence factors and their impact to mortality. Methods Checking the medical records and follow up data of 134 ICD patients in Pecking University People's Hospital from January 2001 to February 2015, analyze the time, frequency and reasons of their ICD shocks, and look into their impact on mortality. Results The 134 ICD patients aged 25-82 years, mean (48. 0 ± 19. 2) years old, 90 males ( 67. 2% ), 33 ( 24. 6% ) patients implanted ICD for ischemic cardiomyopathy, 101 (75. 4% ) patients implanted ICD for non-ischemic cardiomyopathy. During follow up of (5. 1 ± 1. 6) years, 41 (30. 6% ) patients had appropriate shock therapy and 8 (6. 0% ) patients had inappropriate shock therapy. The most common reasons for inappropriate ICD shocks were atrial fibrillation, supraventricular tachycardia, over-sensing and noise. Among the 88 inappropriate ICD shocks, 12 (13. 6% ) of them were due to atrial arrhythmias or supraventricular tachycardia, 56 (63. 6% ) of them were due to noise and 20 (22. 7% ) of them were due to over-sensing. When compared shock group with non-shock group, appropriate shock group with inappropriate shock group, there were no mortality differences between them. Conclusions Inappropriate shocks are common complications after ICD implantations, atrial fibrillation, atrial flutter, supraventricular tachycardia, atrial tachycardia, noise from lead fracture, and over-sensing are the common reasons. Either appropriate or inappropriate ICD shocks don??t increase all-cause mortality.
Contemporary Management and Prognostic Factors of Arrhythmia Recurrence in Patients with High-Energy Discharge of Cardiac Implantable Electronic Devices

Abstract:Background and Objectives: Understanding the underlying causes of implantable cardioverter-defibrillator (ICD) discharges is vital for effective management. This study aimed to evaluate the characteristics of patients admitted following ICD discharge, focusing on myocardial ischemia as a potential exacerbating factor and potential risk factors for VT recurrence. Materials and Methods: This retrospective, single-center study included 81 patients with high energy discharge from cardiac implantable electronic device admitted urgently to the cardiology department from 2015 to 2022. The exclusion criterion was ST-segment elevation acute coronary syndrome. Data were collected anonymously from electronic medical records. Patients were categorized based on coronary angiography, percutaneous angioplasty, presence of significant stenosis, recurrent ventricular tachycardia (VT), and catheter ablation. Clinical variables, including demographic data, echocardiographic parameters, and pharmacotherapy, were analyzed. The primary endpoint was the recurrence of VT during in-hospital stay. Results: Among 81 patients, predominantly male (86.4%), with a mean age of 63.6 years, 55 (67.9%) had coronary artery disease (CAD) as the primary etiology for ICD implantation. Coronary angiography was performed in 34 patients (42.0%) and showed significant stenosis (>50%) in 18 (41.8%) patients, while 8 (26.0%) individuals underwent percutaneous coronary intervention (PCI). Recurrent VT occurred in 21 subjects (26.3%), while ventricular catheter ablation was performed in 36 patients (44.0%). Referral for urgent coronary angiography was associated with presence of diabetes (p = 0.028) and hyperlipidemia (p = 0.022). Logistic regression analysis confirmed NYHA symptomatic class (OR 4.63, p = 0.04) and LVH (OR 10.59, p = 0.049) were independently associated with relapse of VT. CAD patients underwent catheter ablation more frequently (p = 0.001) than those with dilated cardiomyopathy. Conclusions: The study showed a low referral rate for coronary angiography among patients with ICD discharge. Presence of LVH and preexisting symptomatic class influence arrhythmia recurrence. Understanding these associations can guide personalized management strategies for ICD recipients.

Abstract:To determine whether panic disorder patients exhibit physiological hyper‐arousal during rest or during mild, non‐panic‐inducing stress, 18 patients who experienced frequent panic attacks were compared with nonanxious controls on a battery of physiological assessments. During baseline, patients with panic disorder exhibited higher forehead electromyographic activity, higher systolic blood pressure and higher heart rates than non‐anxious volunteers. During psychological stress, heart rate and systolic blood pressure rose more in patients with panic disorder than in nonanxious controls. The skin conductance response, however, was greater and more variable in the nonanxious controls. The results suggest that panic disorder patients with frequent panic attacks exhibit heightened cardiovascular arousal and decreased electrodermal flexibility than nonanxious people, even in nonthreatening situations.
A worldwide experience of the management of battery failures and chronic device retrieval of the Nanostim leadless pacemaker

Abstract:BACKGROUND: The Nanostim leadless pacemaker (LP) met the primary endpoints in an investigational device exemption trial, and was shown to be fully retrievable percutaneously. In October 2016, St Jude Medical issued a worldwide alert of a battery malfunction that caused lost pacing output and LP communication.OBJECTIVE: To report the battery failure mechanism and incidence and the worldwide patient management, including device retrieval experiences.METHODS: The affected LP battery is a custom lithium-carbon monofluoride cell. These were returned after failure and underwent analysis assessing electronics and battery performance. Data were collected in ongoing clinical studies when LPs were abandoned or retrieved.RESULTS: Of 1423 LPs implanted worldwide, there were 34 battery failures, occurring at 2.9 ± 0.4 years with no instances of associated patient injury. Analysis of returned batteries revealed an increase in battery resistance caused by insufficient electrolyte availability at the cathode/anode interface. A total of 66 of 73 retrieval attempts were successful (90.4%; implant duration range: 0.2-4.0 years). The LP docking button was inaccessible in 6 patients, and the docking button detached from the LP during retrieval in 1 patient. There was 1 case of arteriovenous fistula and another case of the LP docking button migrating into the pulmonary artery. There were also 115 non-LP retrieval patients after the advisory who received an additional pacemaker, with no adverse device-to-device interactions reported.CONCLUSION: As with standard pacers, LPs can have critical battery failures. Chronic retrieval of LPs is safe and efficacious.

Implantable cardioverter defibrillator therapy for primary prevention of sudden cardiac death: a single center experience

Abstract:Objective To evaluate the clinical application of implantable cardioverter defibrillator (ICD) / cardiac resynchronization therapy-defibrillator (CRT-D) for primary prevention in sudden cardiac death (SCD) high-risk Chinese patients. Methods Eighty patients treated with ICD / CRT-D implantation for primary prevention of SCD in our unit between 2009 and 2011 were enrolled in the study. Single-chamber or dual-chamber ICDs were implanted in 33 cases and CRT-D in 47 cases. Of the 80 cases, 44 (55.0%) had non-ischemic cardiomyopathy and 22 (27.5%) had ischemic cardiomyopathy. All the patients were followed up at one and three months after implantation, and every six months thereafter or when prompted by an ICD event. Results During an average follow-up of 23 ± 7 months, 11 (13.7%) patients had unplanned readmis sions and 4 (5.0% ) died. Thirty-eight ICD therapy events were recorded, including 26 appropriate ICD therapies and 12 inappropriate therapies, of the later were due to atrial fibrillation. Conclusions ICD / CRT-D for primary prevention of SCD may recognize malignant rapidarrhythmia in a short time so that the patients can be treated timely. Therefore, it can reduce the sudden cardiac death rate in high-risk patients.
