Abstract:Objective: To reshape medical education about pain to present it as a population-based public health process as well as a neuron-centered phenomenon.Design: Collaborate with students to apply a recent inventory of pain-related preclinical curricular content and clinical training in order to modify the current multiyear presentation and offer a broadened social perspective on pain. Appraise fourth-year medical students' pain-related educational needs by surveying their knowledge, attitudes, experience with the curriculum, and self-reported assessment of pain-related competencies.Setting and subjects: University-affiliated medical school and its fourth-year medical students.Methods: Analysis of a detailed inventory of first- and second-year curricula. Survey of graduating medical students assessing attitudes, skills, and confidence. Construction of a fourth-year pain education elective and collaboration with enrollees to better integrate pain throughout the four-year curriculum.Results: This student-faculty collaboration produced an evidence-guided proposal to reorganize pain-related content across the longitudinal medical curriculum. An attitudes/skills/confidence survey of graduating medical students (104 respondents of 200 polled) found that 70% believed chances for successful outcomes treating chronic pain were low. Self-evaluated competency was high for evaluating (82%) and managing (69%) acute pain; for chronic pain, both were lower (evaluating = 38%; managing = 6%). Self-evaluated knowledge of pain physiology and neurobiology was poor (14%), fair (54%), or good (30%), but rarely excellent (2%).Conclusions: To meet graduating students' desire for increased competency in pain, pain-related curricula can and should be reorganized to include pain as a disease state and a widespread public health burden, not merely a symptom.

Abstract:Background Nursing students are graduating ill-prepared to assess and manage pain in older adults. To address this gap, we developed an e-learning module on the topic. Aim To examine nursing students' perceptions on a pain management e-learning module focused on older adults. Methods Utilizing an exploratory quantitative design, we assessed nursing students' perceptions of the e-learning module. We used a feedback survey (four 5-point, Likert-type items) and one open-ended question to assess students' perceptions. Descriptive statistics were used to summarize students' perceptions and demographic characteristics. Responses to the open-ended question were content analyzed. Results A total of 181 of 249 students completed the module, of whom two-thirds were female. Students perceived that the module enhanced their knowledge, confidence, and perception in working with older people. Students also found the method of instruction interactive and enjoyable. Conclusions The e-learning module on pain management was viewed by nursing students to be helpful and its interactive method of learning improved their knowledge, confidence, and perceptions of working with older adults in pain.

Abstract:Effective pain management in Emergency Departments (EDs) is vital for improving patient comfort and clinical outcomes. This review provides a comprehensive analysis of current pain management practices in ED settings, focusing on the challenges and opportunities for optimization. The review examines pharmacologic and non-pharmacologic pain management strategies, evaluating their effectiveness and identifying inconsistencies and gaps in current practices. Key challenges in the ED environment include time constraints, variability in clinical protocols, and the need to address diverse patient needs, including those of paediatric, geriatric, and chronic pain patients. The review highlights the importance of standardized pain assessment tools and protocols to improve consistency in pain management. Innovations, such as technological advances and multimodal approaches, are explored for their potential to enhance pain management practices. Recommendations address identified challenges, including improved training for ED staff, the development of evidence-based protocols, and the integration of multimodal pain management strategies. By addressing these areas, the review aims to contribute to the development of more effective and uniform pain management practices in emergency care, ultimately leading to better patient outcomes and experiences. Abstract:Introduction Healthcare providers (HCPs) practicing in community settings are critical to improving access to pain care, yet there are significant gaps in training opportunities designed for interprofessional learners. Project Extension for Community Healthcare Outcomes (Project ECHO®) is an established model for delivering online HCP education through virtual clinics and cultivating a community of practice. However, to our knowledge, the integration of pain core competency education into the ECHO® model has not been previously attempted. This innovation could enhance the ECHO® model while also addressing the growing calls for more accessible interprofessional pain curricula. This paper describes efforts to implement and evaluate core competency curricula within the context of Pediatric Project ECHO for Pain , one of the first pediatric-pain focused ECHO programs in the world. Methods Needs assessments informed curricula development. The first delivered core competency model consisted of synchronous webinar-style sessions while the second model included a mixture of asynchronous (eLearning course) and synchronous (virtual clinical debrief) elements. A convenience sample of HCPs was recruited from ECHO program registrants. Participants completed baseline and follow-up surveys to assess core competency acceptability as well as impact on knowledge and self-efficacy related to managing pediatric pain. Usability of the eLearning platform (model 2 only) was also evaluated. Surveys used 5-point Likert scales to capture outcomes. A priori targets included mean scores ≥4/5 for acceptability and ≥80% of learners reporting knowledge and self-efficacy improvements. The study received local research ethics approval. Results The core competency was found to be highly acceptable to interprofessional learners ( n = 31) across delivery models, surpassing a priori targets. Specifically, it was characterized as a worthwhile and satisfactory experience that was helpful in supporting learning. The core competency was also associated with improvements in knowledge and self-efficacy by 97% and 90% of learners, respectively. The eLearning platform was reported to have high usability with clinically realistic cases (100% of respondents) that were helpful to inform care delivery (94% of respondents). Conclusion The integration of core competency learning within the Project ECHO® model was a successful approach to deliver pediatric pain education to interprofessional HCPs.
Case-Based Peer Teaching for Pain Medicine Fellows: A Curriculum for Diverse Residency Backgrounds

Abstract:Introduction: Pain medicine is an advanced medical subspecialty incorporating trainees from diverse primary disciplines. A curriculum that does not assess or promote the trainees' differential expertise developed from their primary specialties may silo instead of promoting collaborative learning in a multidisciplinary subspecialty. Methods: We created a Kern-guided curriculum for the pain medicine fellowship utilizing case-based modules and a novel note-taking and knowledge-sharing format. Nine faculty-led, case-based modules of increasing complexity were developed based on pain medicine competency content. Pain medicine fellows were instructed to actively designate prior and new knowledge into four categories: Prior knowledge that is accurate and relevant, inaccurate prior knowledge, new knowledge gained from peers, and new knowledge for all trainees. After each case study, each fellow acknowledged "new knowledge gained from peers" and prepared a brief teach-back of "new knowledge for all trainees" at the beginning of the next session. Fellow participants were assessed for their reactions to the curriculum via anonymized surveys. Results: The curriculum was implemented at a single institution pain medicine fellowship from 2021 to 2023. At the end of this curriculum, 90% of participants reported improved knowledge retention through participating in this case-based curriculum compared to the traditional didactic format, and 80% participants reported increased respect for their peers. Discussion: We created a novel note-taking and knowledge-sharing platform, anchored by a well-established case-based educational strategy, to improve the learning environment and knowledge retention for multidisciplinary trainees with heterogeneous baseline knowledge.

For example, the International Association for the Study of Pain (IASP) was one of the first of these organizations, and continues today with a mission that includes "exchange of ideas and education to stimulate and support the study of pain and to translate that knowledge into improved pain relief worldwide". 1 However, as commercial industry sponsors found an audience for the marketing of their products and services, a number of pain educational groups went awry. By the late 1990s, external forces seemingly brought increased accountability to the field, and efforts were made to improve educational programs with transparency regarding conflicts of interest and to deliver more evidence-based content and decrease bias. Nonetheless, the problem remains especially challenging within the field of chronic pain, an area of practice in which a number of treatments continue to be supported despite inconsistent evidence for safety and efficacy. Multiple challenges continue to face pain educators today, such as upholding standards of academic integrity, minimizing conflicts of interest and bias, and maintaining interdisciplinary communication, while remaining accessible for healthcare providers, researchers, patients, and other stakeholders. The Lyceum for Pain Education and its weekly Interdisciplinary Pain and Headache Rounds offer one such template to meet these challenges and mitigate common problems with pain education. Upon switching to a virtual format after 20 years due to the COVID-19 crisis, these seminars provide evidence-based continuing education (CE) to clinicians, researchers, and public policy makers who have an interest in a wide variety of pain topics. The programs are provided at no cost to attendees, with sponsorship derived from philanthropic efforts as opposed to industry funding. In this article, we discuss the challenges that face CE and our experience in navigating them. One significant problem that faced many CE offerings in the late 1990s was not only due to the impact of industry bias, but due to the absence of independent scientific reviewers. 2 For example, a 2007 annual report from the Accreditation Council for Continuing Medical Education (ACCME) determined that more than 30,000 CME activities each year were sponsored by for-profit medical education and communication centers that received approximately 90% of their income from commercial support. 2,3 In our experience, many CE committees play a minimal role in vetting the materials that they sponsor. Rather, speakers are recruited by program planners who are thought to be experts in the field. This model of recruitment is in contrast to the model typically utilized in scientific writing and publishing, in which a critical independent review of the scientific content is necessary. The field of pain medicine can easily find itself susceptible to "junk science", alongside any other field in which patient suffering is great, treatment pathways are weak, and financial incentives persist. Some may be surprised that these issues are not new, and in fact, clinicians and academicians in the 19th century fought the same battles. Improving Pain Management for Marginalized Communities: Educating the Next Generation of Health Care Professionals

Abstract:Inadequately managed pain has immense negative impacts on children, families, health care systems, and societies. Historically and presently, inadequately treated pain disproportionally affects marginalized communities. Deficiencies in pain education for health care providers are widely recognized as a leading contributor to poorly managed pain. Existing training for providers prioritizes physiological systems and pharmacological interventions for pain, despite decades of evidence supporting the biopsychosocial model and interdisciplinary treatment of pain. Moreover, education for health care providers rarely acknowledges the role of bias, prejudice, and systemic racism in perpetuating disparities in pain care. To address this gap, we sought to develop an innovative curriculum for health care teams to combat racial injustice related to pain management. This curriculum was developed and refined collaboratively with community partners representing diverse expertise, including lived experiences of pain, interdisciplinary pain management, cultural humility and authentic allyship, and curriculum development and evaluation. Four modules delivered across 1 to 2 hours were developed and pilot-tested extensively with multidisciplinary providers across a large pediatric health system. Learner feedback indicated high acceptability and informed iterative changes to the curriculum. Additional research is needed to examine impacts of the curriculum on health care provider knowledge and behavior (eg, clinical decision-making) and patient-reported outcomes as well as to test dissemination and implementation strategies. PERSPECTIVE: We present the development and evaluation of a curriculum for health care professionals to combat racial injustice in pain management. By engaging diverse community partners, using design thinking, applying an antiracist lens, and designing for dissemination, we aim to equip the next generation of providers to deliver equitable pain care.

Abstract:Abstract The modified Delphi technique is widely used to develop consensus on group opinion within health services research. However, digital platforms are offering researchers the capacity to undertake a real-time Delphi, which provides novel opportunities to enhance the process. The aim of this case study is to discuss and reflect on the use of a real-time Delphi method for researchers in emergency nursing and cognate areas of practice. A real-time Delphi method was used to develop a national survey examining knowledge, perceptions and factors influencing pain assessment and management practices among Australian emergency nurses. While designing and completing this real-time Delphi study, a number of areas, emerged that demanded careful consideration and provide guidance to future researchers.

Abstract:Chronic pain, opioid use, and opioid use disorder (OUD) are increasingly common in older adults. Outpatient clinician education may improve outcomes. Our aim was to create and evaluate a novel curriculum for outpatient clinicians on older adult pain management, opioid prescribing, and OUD leveraging Project ECHO®, a model for delivering subspecialized medical knowledge to community outpatient clinicians via videoconferencing.The series is comprised of 8-hour-long sessions, each with an expert-led interactive lectures followed by participant-led case discussions. Topics include pain assessment & treatment, considerations in older adults (e.g. cognition, caregiver support), shared decision-making, behavioral health, and OUD diagnosis and treatment.Sixty-nine clinicians attended the series and completed the pre-series survey; 78% (n = 54) completed the post-series survey. Clinicians reported a significant increase in self-efficacy in 14/14 queried competencies and greater frequency of performing 7/10 practice behaviors regarding older adult pain management, opioid prescribing, and OUD management. The most liked curricular components were opportunities to learn with subspecialists and discussions with other community clinicians. Qualitative analysis suggests that clinicians may benefit from education on specific geriatrics-related content areas.This novel curriculum has potential to improve management of older adult chronic pain, opioid prescribing, and OUD in a virtual and scalable format.
Implementing Evidence‐Based Pain Management Interventions Into an Emergency Department: Outcomes Guided by Use of the Ottawa Model of Research Use

Abstract:Aim To implement strategies to improve the care of patients with acute pain in the emergency department (ED). Design Pre–post implementation study using a Type 2 hybrid effectiveness–implementation design. Methods Implementation strategies were introduced and monitored through the Ottawa Model of Research Uses' assessment, monitoring and evaluation cycles, supported by focused and sustained facilitation. Results Improvements in time‐to‐analgesia within 30 min (21%–27%), administration of nurse‐initiated analgesia (NIA) (17%–27%) and measurement of pain (65%–75%) were achieved post‐implementation. NIA was the strongest predictor of receiving analgesia within 30 min. Adoption of pain interventions into practice was not immediate yet responded to sustained facilitation of implementation strategies. Conclusion Collaboration with local clinicians to introduce simple interventions that did not disrupt workflow or substantially add to workload were effective in improving analgesia administration rates, and the proportion of patients receiving analgesia within 30 min. The assessment, monitoring and evaluation cycles enabled agile and responsive facilitation of implementation activities within the dynamic ED environment. Improvements took time to embed into practice, trending upward over the course of the implementation period, supporting the sustained facilitation approach throughout the study. Implications Sustained adoption of evidence‐based pain interventions into the care of people presenting to the ED with acute pain can be achieved through sustained facilitation of implementation. NIA should be at the centre of acute pain management in the ED. Impact This study addressed the lingering gap between evidence and practice for patients with acute pain in the ED. Implementation of locally relevant/informed implementation strategies supported by focused and sustained facilitation improved the care of patients with acute pain in the ED. This research will have an impact on people presenting to EDs with acute pain, and on clinicians treating people with acute pain in the ED. Abstract:Pain and addiction are one of the most common reasons for adults to seek health care, yet educational programs focused on pain are often underrepresented in medical school curricula. In January 2021, the Association of Faculties of Medicine of Canada (AFMC) launched an online national, bilingual, competency-based curriculum for undergraduate medical (UGME) students in pain management and substance use in response to the opioid crisis and to bridge the content gaps in programs across Canada. The purpose of this study is to evaluate the pilot of this national curriculum.

552 Pain in the Assessment: Improving Reassessment of Burn Patients in the Emergency Department

Abstract:Abstract Introduction Treatment of burn patients in the emergency department (ED) is challenging and often complicated by excruciating pain sparked by the burn injury. Unmanaged burn pain causes undue stress to the patient, both mentally and physically, and may result in physiologic manifestations. The physiologic impact of extreme burn pain may convolute a provider’s assessment and priorities of care. To facilitate appropriate treatment, RNs in the ED must perform timely pain reassessments. Our institutional benchmark for pain reassessment following medication administration is 90%. In 2021, we observed a drop in compliance in the ED to 73% and instituted a multidisciplinary, quality improvement (QI) approach to improve pain reassessment. The purposes of this project were to pinpoint education needs of nursing staff, improve compliance of pain reassessment, and to evaluate the efficacy of our interventions. Methods A survey was disseminated to Burn ED nursing staff to identify barriers to pain reassessment. QI interventions included a collaboration between Nursing, Informatics, and Quality Assurance to overcome barriers related to education, optimization of documentation in the electronic health record (EHR), and timely performance feedback. Education was disseminated via pre-shift nursing huddle, weekly newsletters, and educational posters. Modifications were made to the EHR software to alert nurses to reassessment needs and accounted for medication ‘indication.’ Quality Assurance increased report frequency to furnish reports weekly instead of monthly, enabling nursing leadership to expedite individualized feedback and education. Feedback was well-received by staff. Reassessment metrics were added to the quality board and posted in staff lounges. Top performers were acknowledged monthly in the department newsletter’s ‘Benchmark Brief.’ Performance was analyzed from April 2021 through June 2023, and was grouped quarterly. Results Following initiation of the QI project, pain reassessment increased by 18.8% to a 93.6% rate of timely reassessment. Conclusions Our multidisciplinary quality improvement project was successful, increasing complete and timely pain reassessment in the ED by 18.8%. Further, we discovered the need to reinforce education related to medication administration route, as PO medications had a higher incidence of pain reassessment non-compliance. To reduce the limitations of staff education, we continue to work with Informatics and the EHR vendor to refine the use of technology and better facilitate reassessment. Applicability of Research to Practice Further research is needed to evaluate efficacy of education related to pain reassessment following PO medication administration.

Abstract:OBJECTIVE: To evaluate statewide emergency department assessment and management of pain in pediatric patients as a quality improvement initiative.METHODS: 2002 Survey of Illinois Hospital emergency department's pediatric pain assessment and management strategies, in conjunction with a retrospective chart review of children, ages 0 to 15 years, treated for an extremity fracture. Survey results were available for 123 (59.4%) hospitals; 933 charts (107 hospitals) were reviewed for pain management. Survey results were compared with practices identified by chart review.RESULTS: Use of a pain assessment scale estimated by the survey was 92%, compared with 59% use by chart review. Use of pain assessment scales for infants was limited. Fifty percent of patients in moderate to severe pain would be offered an analgesic. Six- to 15-year-old children would be offered opioids more often than children aged 0 to 1 and 2 to 5 years. Offering higher potency narcotic analgesics was associated with patient's age, geographic location of the facility, and emergency department volume. Providing an analgesic (odds ratio 4.53, 95% confidence interval 2.89-7.10), offering supportive care (odds ratio 2.37, 95% confidence interval 1.44-3.89), and pediatric-focused annual nurse competencies (odds ratio 1.90, 95% confidence interval 1.18-3.06) correlated with reduction of the patient's pain.CONCLUSIONS: Disparity exists between perceived and documented emergency department pain management practices for children. Quality improvement initiatives should focus on improving pain assessment in infants, treating moderate to severe pain in children of all age groups, and education of health care providers in pain management strategies. Resources should target health care processes effective in decreasing pediatric pain.

Abstract:Background: Relief of pain for patients requires that palliative care practitioners have knowledge and skill in both pain assessment and the use of pharmacologic and complementary therapies. Method: Pain assessment and management and the teaching strategies suggested within the End-of-Life Nursing Education Consortium curriculum are presented. Results: By addressing the pain experienced by patients with life-limiting illnesses and those at the end of life, the quality of care can be greatly improved. As a multidimensional phenomenon that relates to physical, emotional, and spiritual well-being, the relief of pain enhances the quality of life of patients and lessens the stress experienced by family caregivers. Conclusion: The relief of pain and suffering is consistent with the philosophy and goals of nursing as a profession and is a nursing responsibility.

Abstract:OBJECTIVES:To assess the knowledge and use of the Assessment, prevention, and management of pain; spontaneous awakening and breathing trials; Choice of analgesia and sedation; Delirium assessment; Early mobility and exercise; and Family engagement and empowerment (ABCDEF) bundle to implement the Pain, Agitation, Delirium guidelines. DESIGN:Worldwide online survey. SETTING:Intensive care. INTERVENTION:A cross-sectional online survey using the Delphi method was administered to intensivists worldwide, to assess the knowledge and use of all aspects of the ABCDEF bundle. MEASUREMENT AND MAIN RESULTS:There were 1,521 respondents from 47 countries, 57% had implemented the ABCDEF bundle, with varying degrees of compliance across continents. Most of the respondents (83%) used a scale to evaluate pain. Spontaneous awakening trials and spontaneous breathing trials are performed in 66% and 67% of the responder ICUs, respectively. Sedation scale was used in 89% of ICUs. Delirium monitoring was implemented in 70% of ICUs, but only 42% used a validated delirium tool. Likewise, early mobilization was "prescribed" by most, but 69% had no mobility team and 79% used no formal mobility scale. Only 36% of the respondents assessed ICU-acquired weakness. Family members were actively involved in 67% of ICUs; however, only 33% used dedicated staff to support families and only 35% reported that their unit was open 24 hr/d for family visits. CONCLUSIONS:The current implementation of the ABCDEF bundle varies across individual components and regions. We identified specific targets for quality improvement and adoption of the ABCDEF bundle. Our data reflect a significant but incomplete shift toward patient- and family-centered ICU care in accordance with the Pain, Agitation, Delirium guidelines.

Methods: First-year medical students at a major academic medical center enrolled in a required pre-clerkship pain course in June 2020 and completed pre- and post-course online surveys with Likert-scale questions about their attitudes toward pain management and opioid-related issues. Additionally, the surveys included a free-text question where the students listed the first five words that came to mind when hearing the word "opioids". These words were categorized as "professional" or "lay" words and further as having "positive", "negative", or "neutral" connotations. Data analyses included descriptive statistics, as well as non-parametric and parametric tests. Results: Fifty-four of the 119 students responded to pretest and posttest surveys and were included in paired analyses. There was a significant difference between the number of professional words used before (M=1.21, SD=0.97) and after the course (M=2.40 SD=1.33); t(52)=− 6.39, P < 0.001. Students also used more lay-positive words after the course (M=0.81, SD=0.63) than they used pre-course (M=0.23, SD=0.43); t(51)=− 5.98, P < 0.001. Students' post-course responses to several key Likert-scale questions showed significant shifts toward more positive attitudes about caring for patients with pain. For example, students acknowledged greater comfort in providing opioids for chronic pain ( P < 0.001) where appropriate, and enhanced interest in handling complex pain cases ( P < 0.001). Conclusion: Results showed that a comprehensive, multi-disciplinary pain course could greatly enhance first-year medical students' attitudes toward pain management, chronic pain patients, and the complex issues surrounding opioids. Keywords: medical education, opioids, stigma, epidemic, biases, pain Over 100 million Americans suffer from unremitting chronic pain—exceeding the number of Americans affected by heart disease, cancer, and diabetes combined. 1 The annual economic burden of chronic pain falls between 560 and 635 billion United States dollars. Pain mismanagement—particularly in relation to the opioid crisis—has inflicted substantial individual and public health damage, while being associated with higher pharmacy, emergency department, inpatient, and total healthcare costs. 2 There are 5,871 pain medicine physicians actively practicing. Using data from the United States (US) Census Bureau, we calculate 56,453 Americans per pain medicine physician in contrast to 20,366 per oncologist and 14,717 per cardiologist. 3 Unfortunately, 20% of patients with severe chronic pain never reach a pain specialist; 4 those who do experience a 7.8-year average delay post-referral, seeing an average of 8 other physicians first. 4 Pain management pervasively falls on the shoulders of primary care practitioners, surgeons, oncologists, and others without formal pain training. With unfilled American pain medicine fellowship positions tripling in the last three years, 5 effective pain management will more frequently be expected from those without the appropriate skillsets—exponentiating pain-related mistakes and economic burden.
