Age of Complementary Foods Introduction and Risk of Anemia in Children Aged 4–6 years: A Prospective Birth Cohort in China

Abstract:Age of complementary foods introduction is associated with childhood anemia, but the ideal age for the introduction of complementary foods to infants is a continuing topic of debate. We examined the longitudinal association between complementary foods introduction age and risk of anemia in 18,446 children from the Jiaxing Birth Cohort, who had detailed complementary feeding records at 3 and 6 months of age and had hemoglobin concentrations measured at 4–6 years. Early introduction of complementary foods at 3–6 months of age was significantly associated with a higher risk of anemia (odds ratio = 1.14; 95% confidence interval: 1.01–1.28) and a lower hemoglobin concentration of −0.84 g/L (95% confidence interval: −1.33 to −0.35) in children aged 4–6 years, compared with those fed complementary foods starting at 6 months of age. When it comes to the specific type of complementary foods, early introduction of all plant-based foods was associated with increased anemia risks and lower hemoglobin concentrations, while early introduction of most animal-based foods was not. These findings may be informative regarding the appropriate time to introduce complementary foods in infants.

Abstract:[Objective I To understand the complementary food of children under the age of three years in rural countries of western China. [Methods] Cross-sectional study with probability-proportional-to-size(pps) sampling was used. The information on complementary food was obtained by recall of the child's mother. 10655 children were studied. The data of complementary food in rural countries of western China was compared with those of the children from the survey of same rural countries in 2001. [Results] In 2005, it was at 6th months old for children to start complementary food on average. The percents of babies had begun to take any foods before 4 months old in 34 counties sample in 2005 were 29. 7%, and 42. 6% in 34 counties sample in 2001; The percents of babies did not take any foods at age of 6 months were 9. 0% in 34 counties sample in 2005, and 17. 1% in 34 counties sample in 2001. In 2005 from 4 to 6 months the percents of starting complementary foods were 59. 2% for cereal, 54. 8% for eggs, 11. 7% for milk, 24. 0% for bean products, 34. 1% for meat and 45.4% for vegetables and fruits respectively; at 9 months 10.1%, 23.2%, 71.3%, 49.5%, 34.1% and 24. 8% of babies had not begun to take cereal, eggs, milk, bean products, meat and vegetables and fruits respectively. In 2005 the 34 study counties sample showed the time, proportions and frequencies of complementary foods were better than in 34 study counties sample in 2001. [Conclusion] Cereal and eggs were popular complementary foods in this survey, vegetables and fruits, meat and bean products were secondary, it is not popular that the children take milk in this survey. The economy, health education and delivery in hospital should be strengthen. The mothers should be educated on how to raise their children. In order to reduce prevalence of malnutrition, it is important that improving proportions and frequencies of timely bean products and animal foods respectively.

Abstract:Background/purpose: Introducing complementary foods to infants is a critical stage in their healthy growth and development process. In this study, we analyzed the feeding habits and choices of caregivers in Taiwan during this essential transition period. Methods: Data from the 2013-2016 Nutrition and Health Survey in Taiwan were analyzed. Using a multistage stratified sampling technique, we collected data through questionnaires on the feeding practices, challenges, and information sources associated with complementary feeding. We also investigated the timing of introducing various foods. Certain feeding indicators recommended by the World Health Organization were modified and used for analysis. Results: Among 439 caregivers of infants aged 2-24 months, complementary feeding typically began at 5 months, and it primarily began with cereals and pureed foods. At 6-8 months of age, 98.29% of the infants received these foods. Only 48.39% of infants aged 6-24 months consumed eggs or flesh foods. Among the primary challenges for caregivers were concerns regarding infants' adaptation to new foods (38.2%), refusal of foods by infants (37.1%), and uncertainty regarding which foods to introduce (11.4%). The main sources of information were friends and family (46.9%), the Internet (46.5%), and baby care books (35.6%). Notably, approximately 51% of the respondents used commercial baby foods. Conclusion: In Taiwan, although the majority of infants begin complementary feeding on time, the introduction of eggs and flesh foods is typically delayed. Given the growing reliance on the Internet in the modern age, accurate feeding information should be disseminated online.
Complementary Feeding and Childhood Adiposity in Preschool-Aged Children in a Large Chinese Cohort

Abstract:Objective To examine the association between the timing and type of complementary feeding and childhood adiposity in Mainland China.Study design During 1999-2009, 97 424 singletons were enrolled in the Jiaxing Birth Cohort, a population-based prospective cohort study in Southeast China. Of these children, 43 848 children provided complementary diet information and other anthropometric measurements at 1, 3, and 6 months of age and were followed up until 4-5 years of age. Obesity and overweight were identified as body mass index (BMI)-for-age z-score (SD) >= 2 and between 1 and 2, respectively.Results Among 40 510 children in the statistical analysis, 3.18% were overweight and 64.8% were fed complementary food before 3 months of age. Early introduction of complementary foods was associated with greater BMI z-score (P-trend < .001) and higher risk of overweight (P-trend = .033). Compared with introduction of complementary foods between 4-6 months of age, before 3 months of age of introduction was associated with 11% greater risk of overweight (OR 1.11, 95% CI 1.03-1.19). No significant association between timing of complementary feeding and obesity was observed. Fish liver oil was the major type of complementary food associated with adiposity. Early introduction of fish liver oil was associated with greater BMI z-score (P < .001) and greater risk of overweight (P-trend = .004).Conclusions Early introduction of fish liver oil is associated with greater childhood BMI and risk of overweight in Chinese children at 4-5 years of age.

Abstract:Inappropriate complementary feeding practices have been identified as the major causes of malnutrition in young children in developing countries. This article reviewed literature on complementary feeding educational interventions from 1998 onwards. The purpose of this study was to provide evidence from published studies in the past decade on the effectiveness of educational intervention programs on improving complementary feeding practices in the developing world. The literature search was performed through PubMed/Medline database with the following key words: infant feeding, weaning and intervention, and identified a total of 15 original educational intervention studies. Evidence from these studies supports that educational intervention can effectively improve complementary feeding practices and child nutrition and growth. The intervention should be culturally sensitive, accessible and integrated with local resources.
[Association Between the Introducing Time of Complementary Food and Growth of Infants and Young Children in Poor Rural Areas in Shaanxi Province].

Abstract:OBJECTIVE:To study the introducing time of complementary food in poor rural areas and its association with the growth of infants and young children.METHODS:In total of 1802 infants and young children aged 6-12 months from 11 poverty counties in southern Shaanxi Province were selected by stratified random cluster sampling. These infants were surveyed four times, and once every 6 months. Data on complementary feeding for children were collected through the questionnaires. Cognitive development was measured by BSID. Body weight and length for them were measured using standard gauges to generate height for age Z score( HAZ), weight for age Z score( WAZ) and weight for height Z score( WHZ). The analysis of variance was used to identify the association between the introducing time of complementary food and growth of children.RESULTS:at baseline, only 32. 93% of children were introduced complementary food at the age of 6 months. This study indicated that the introducing time of complementary food was significantly associated with the cognitive development in the longer run. The third and fourth survey showed that children with introducing complementary food after 6 months of age( mental development index were 81. 24 and78. 40 respectively) had significantly lower cognitive development ability( F = 11. 86, P<0. 05; F = 4. 24, P< 0. 05). There were no long-term significant correlation between the introducing time of complementary food and WAZ, HAZ and WHZ.CONCLUSION:There are still lots of children that were not feeding reasonably in poor rural China. The introducing time of complementary food is related to growth of infants and young children.

Abstract:In early infancy, introducing complementary foods is critical to infant health and development. During the weaning period, the dietary patterns provide essential nutrients and facilitate the development of a diverse gut microbiome, which plays significant roles in the regulation of immune, metabolic, and neurological functions. In this study, we enrolled 200 families to assess the impact of complementary feeding on infant growth and health outcomes. Data collection included detailed records of complementary feeding practices, measurements of infant growth parameters, health status assessments, and the collection of infant fecal samples and maternal breast milk between weeks 12 and 32 postpartum. Gut microbiome composition was analyzed using 16S rRNA sequencing, while metabolite profiling, including human milk oligosaccharides (HMOs), monosaccharides, and short-chain fatty acids (SCFAs), was conducted through chromatography coupled with mass spectrometry. Results revealed a high prevalence of breastfeeding, with complementary food first introduced around 16 weeks. Significant alterations in the infant gut microbiome were observed, particularly in the genera Lactobacillus, Akkermansia, and Staphylococcus. Additionally, the levels of HMOs, monosaccharides, and SCFAs were found to be influenced by the introduction of complementary foods. Significant correlations emerged between complementary feeding practices, gut microbiome diversity, specific bacterial genera (e.g., Streptococcus, Lactobacillus, Bifidobacterium, Clostridioides), and key metabolites (such as Lacto-N-tetraose, Lacto-N-neotetraose, mannose, and butyric acid). This study provides valuable insights into the complex interactions between complementary feeding, the gut microbiome, and metabolite profiles during early infant growth. Future research involving larger cohorts and targeted dietary interventions is recommended to further elucidate the underlying mechanisms.

Abstract:Background: Appropriate infant and young child feeding (IYCF) plays a crucial role in promoting the healthy growth of children. Currently, many Chinese urban parents are seeking care of children from the early childhood education (ECE) institutions, however, little is known about the feeding practices of infants and young children in ECE institutions. This study aims to investigate the complementary feeding practices for Chinese urban children aged 6-23 months in ECE institutions and explore potential factors influencing their feeding practices.Methods: This cross-sectional study was conducted among primary caregivers of children aged 6-23 months in ECE institutions across 31 provinces in China from 1 March to 30 April 2023. Convenience sampling was used to recruit caregivers from centres of Gymboree Play & Music (an ECE institution). Self-administered questionnaires were designed using the online survey tool Sojump and distributed through WeChat platform, which collected information on 1) children's complementary feeding practices; 2) food frequency of seven food groups; 3) caregivers' feeding knowledge and practices; 4) frequency of children attended ECE classes and the primary caregivers' daily nurturing care time; 5) source of information on complementary feeding.Results: A total of 2731 children and their caregivers were surveyed, with 416 children aged 6-11 months and 2315 children aged 12-23 months. The prevalence of minimum dietary diversity (MDD), minimum meal frequency (MMF), and minimum acceptable diet (MAD) was 59.4, 60.6, and 39.2%, respectively. Only 17.3% caregivers believed that continued breastfeeding should be at two years or above, and 29.5% children were continuously breastfed at 12-23 months (CBF). The proportion of non-responsive feeding practices among caregivers ranged from 28 to 64%. Except for CBF, the prevalence of other key complementary feeding practices was higher among children who attending ECE classes than those not attending ECE classes (all P < 0.05). Moreover, children aged 12-23 months who received long-nursing care time (≥4h/d) had significantly higher MMF and MAD prevalence than those in short-nursing care time group (MMF = 66.2 vs. 58.8%, P = 0.0003; MAD = 44.2 vs. 38.3%, P = 0.0047).Conclusions: The complementary feeding practices of children aged 6-23 months in ECE institutions in urban China remained suboptimal, and non-responsive feeding practices among caregivers were common. The attendance of ECE classes and the caregivers' daily nurturing care time could be beneficial in ensuring children to comply with complementary feeding recommendations.

[The Introduction of Complementary Food of Children under the Age of Three Years in Tibet of China].

Abstract:OBJECTIVE:To understand the feeding status of Tibetan children aged below three years old.METHODS:Cross-sectional study and randomly sampling were used. The information on complementary food was obtained through the recollection from mother of the child. Data of complementary food of Tibet children was compared with those of the children from the surveys of nine cities in China and 40 poor counties in western China.RESULTS:One thousand six hundred and fifty-five children were studied. The findings indicated that the age of introduction of Zanba (a kind food of mixture of highland barley flour and butter) was 1.98 months after birth and the age of introduction of egg and meat was about 8 - 9 months. Boys were fed with Zanba earlier than girls but there was no significant difference in the introduction of other complementary food between sexes. Urban children were introduced with complementary food earlier than rural children, and children in the agricultural region earlier than those in the pasturing region. The proportion of no introduction of any complementary food by the age of 4 months was about 10.6% for urban children, but 21.7% for rural children while 14.2% for children in the agricultural region and 30.9% for children in the pasturing region. The proportion of no introduction of complementary food was 11.5% for Tibetan children compared with 45.2% for the children of 40 poor counties in western China. The proportions of introduction of the following complementary foods were 89.1% for Zanba, 22.1% for egg, 29.1% for fresh milk, 23.5% for meat, 17.5% for vegetable, 0.75% for bean products, 1.03% for fish and 3.35% for chicken respectively.CONCLUSIONS:The main complementary foods for Tibetan children included Zanba, milk, mutton and beef. There was a small amount of vegetable in diet. The introduction of bean, chicken and fish was not common. Early and monotony seemed to be the character of introduction of complementary food for Tibetan children. Improving introduction of complementary food is important in enhancing the nutritional status of Tibetan children.

Abstract:The association between the age at introduction of complementary feeding and the risk of overweight or obesity during childhood has been hotly debated, but the result remains uncertain. This meta-analysis of prospective cohort studies attempted to evaluate this association, as well as provide evidence for infant feeding recommendations. The PubMed, Embase, and Cochrane databases were systematically searched for relevant original articles published prior to March 1, 2015 that met predefined inclusion criteria. The pooled relative risks (RRs) and corresponding 95% confidence intervals (CIs) were calculated using fix-effect or random-effect models, which were chosen based on heterogeneity among studies. Ten articles consisting of 13 studies, where 8 measured being overweight as an outcome and 5 measured being obese, were included in this meta-analysis. There were a total of 63,605 participants and 11,900 incident cases in the overweight studies, and 56,136 individuals and 3246 incident cases in the obese studies. The pooled results revealed that introducing complementary foods before 4months of age compared to at 4 to 6months was associated with an increased risk of being overweight (RR, 1.18; 95% CI, 1.06-1.31) or obese (RR, 1.33; 95% CI, 1.07-1.64) during childhood. No significant relationship was observed between delaying introduction of complementary foods after 6months of age, and being overweight (RR, 1.01; 95% CI, 0.90-1.13) or obese (RR, 1.02; 95% CI, 0.91-1.14) during childhood. The results of this study suggest that the introduction of complementary foods to infants before 4months of age should be avoided to protect against childhood obesity.
Parental Decision-Making Around Introducing Complementary Foods: An Integrative Review

Abstract:A primary role in infant parenting is feeding, and this role undergoes a significant transition when introducing complementary foods (CF), with important long-term health implications. Understanding the influences on parental decision-making around timing the introduction to CF can help health care providers provide parents with effective support for feeding; however, the factors that influence parental decision-making have not been recently reviewed in the United States. To determine influences and information sources, this integrative review examined the literature from 2012 to 2022. Results indicated that parents are confused and distrustful of inconsistent and changing guidelines around CF introduction. Instead, developmental readiness signs may be a more appropriate way for practitioners and researchers to support parents in appropriate CF introduction. Future work is needed to evaluate interpersonal and societal influences on parental decision-making, as well as to develop culturally sensitive practices to support healthful parental decisions.
[Construction and acceptability of different complementary foods texture in infants and young children aged 6 to 23 months]

Abstract:Objective: To construct the complementary food texture in infants and young children aged 6 to 23 months, and observe the acceptability of complementary food of different months old infants. Methods: Based on the domestic and foreign guidelines, consensus and literatures on complementary feeding, and combined with the characteristics of children's growth and development in China. The complementary food texture index of 6-23 months old infants and young children was constructed. One province was selected in the south and north respectively, one city and one rural area was selected as the observation point in each province. The stratified random sampling principle was adopted in each observation point, 240 infants and young children were selected for the acceptability study. According to the food type, 12 common foods were selected to make the complementary food toolkit. The parents were instructed to make complementary food at home according to the corresponding month age, observe and record the acceptability of single/mixed complementary food feeding. Results: The complementary food texture index of 6 months, 7-8 months, 9-11 months, 12-17 months, 18-23 months was constructed. Caregivers could make complementary food at the corresponding month age according to the established complementary food texture index. The acceptability of single complementary food for infants and young children aged 6-23 months was 98.3%, 98.7%, 99.8%, 96.9% and 97.5%, respectively. The acceptability of mixed complementary food for children aged 7-23 months was 98.3%, 99.6%, 93.8% and 97.5%, respectively. Conclusion: The complementary food texture index of different month age can be made at home, and the acceptability of different texture is good.

Abstract:China is confronted with the challenge of increasing childhood obesity. Although interventions for childhood obesity have been developed, their effectiveness and implementation can vary considerably across nations. In this paper, we review and consolidate the evidence on childhood obesity intervention strategies aimed at multiple levels and components across life, in China. School-based interventions, primarily delivered with group-based dietary and physical education, have shown potential efficacy and can be enhanced by integrating food environment reform and family support. However, most interventional studies were done in eastern and urban areas of China, which highlights the need for attention in regions with insufficient health resources where policies and initiatives are less accessible. In addition, the sparse reporting on implementation design and delivery might hinder the scalability of potentially effective strategies. Amid rapid social and economic development over the past few decades, China faces unique challenges on urbanisation, physical inactivity, and nutritional transitions, necessitating an updated policy agenda for primary prevention of childhood obesity. Structural longitudinal evaluations of public initiatives led by the government and organisations along with inclusive and equitable interventions targeting children of different ages, urban–rural regions, and different ethnic and socioeconomic groups are warranted. Tackling childhood obesity in China needs a cohesive approach that integrates social, economic, cultural, and environmental strategies, combining school-based, family-based, and individual-based approaches, and concerted efforts from multiple sectors and entities within China.

Abstract:Background: An urgent need exists for evidence-based dietary guidance early in life, particularly regarding protein intake. However, a significant knowledge gap exists in the effects of protein-rich foods on growth and development during early complementary feeding. Methods: This is a randomized controlled trial of infant growth and gut health (primary outcomes). We directly compare the effects of dietary patterns with common protein-rich foods (meat, dairy, plant) on infant growth trajectories and gut microbiota development (monthly assessments) during early complementary feeding in both breast- and formula-fed infants. Five-month-old infants (up to n = 300) are randomized to a meat-, dairy-, plant-based complementary diet or a reference group (standard of care) from 5 to 12 months of age, with a 24-month follow-up assessment. Infants are matched for sex, mode of delivery and mode of feeding using stratified randomization. Growth assessments include length, weight, head circumference and body composition. Gut microbiota assessments include both 16S rRNA profiling and metagenomics sequencing. The primary analyses will evaluate the longitudinal effects of the different diets on both anthropometric measures and gut microbiota. The secondary analysis will evaluate the potential associations between gut microbiota and infant growth. Discussion: Findings are expected to have significant scientific and health implications for identifying beneficial gut microbial changes and dietary patterns and for informing dietary interventions to prevent the risk of overweight and later obesity, and promote optimal health. Professional Feeding Guidance Improved Infants' Self-Feeding Proportion During Complementary Foods Introduction in Beijing, China: an Exploratory Study.

Abstract:An exploratory study was undertaken to examine the prevalence of infants’ feeding patterns in Beijing, China, as well as the factors linked to infants’ self-feeding proportion during the introduction of complementary foods, and the impact of professional feeding guidance on this proportion. A total of 122 families with infants aged 6–11 months from Beijing were included in the study. A descriptive analysis was employed to assess the prevalence of infants’ feeding patterns, while generalized linear model analysis was utilized to investigate the factors associated with these patterns. All families were provided with comprehensive and personalized professional guidance regarding the introduction of complementary foods for infants. However, 64 families were lost to follow-up, leaving 58 families who were re-evaluated and queried after one month. To exclude the influence of infants aging, both the 64 families prior to receiving feeding guidance, and the 58 families after receiving feeding guidance, were included in the analysis. The families with infants aged 6–8 months and 9–11 months were compared separately based on the presence or absence of feeding guidance. Statistical tests, including the Wilcoxon rank-sum test and χ2 test, were conducted to assess any significant differences. The study revealed that the proportion of infants engaging in self-feeding was found to be remarkably low (10% [0%, 40%]). Furthermore, a significant positive association was observed between the proportion of infants engaging in self-feeding and their age (p < 0.001). Notably, after receiving professional feeding guidance, the proportion of infants engaging in self-feeding significantly increased (from 1% [0%, 20%] to 30% [10%, 50%], p < 0.001 for infants aged 6–8 months; from 20% [10%, 50%] to 40% [30%, 50%], p < 0.001 for infants aged 9–11 months). These findings contribute valuable insights for improving postnatal care practices during the introduction of complementary foods for infants.

Abstract:BackgroundEarly dietary patterns are critical to growth and development and play an important role in development of food preferences and habits. The objective of this paper is to describe when different foods enter the diet and how food consumption changes over the period of complementary feeding.MethodsWe analyzed cross-sectional 24-h dietary recall data from the Feeding Infants and Toddlers Study (FITS) 2008, a nationally representative sample of young children. Data are presented for 7 age groups: 6–8.9 months (n = 249), 9–11.9 months (n = 256), 12–14.9 months (n = 243), 15–17.9 months (n = 251), 18–20.9 months (n = 219), 21–23.9 months (n = 212) and 24–47.9 months (n = 1461). Per capita energy was calculated from major food groups and specific types of food within each food group.ResultsThe predominant source of energy in the infant diet was milk (breast milk and infant formula), contributing 98% of energy at 0–3 months, and 91% at 4–5 months. At 6–8 months, when complementary feeding had begun for most infants, 69% of daily energy was coming from milk, 11% from grains, 6% from fruits, 5% from vegetables and 4% from mixed dishes. The diet then quickly transitioned until at 21–23.9 months, 26% of energy came from milk, with other key sources of energy being grains (17%), mixed dishes (14%), meats (12%) and sweets (12%). The period from 4 to 14 months was the most dynamic period of change regarding the introduction of complementary foods for all food groups, with a slower rate of change between 15 and 24 months. After 24 months, the diet stabilized and included less energy from milk (20%) and vegetables (4%) and more energy from mixed dishes (18%), grains (16%), and sweets (17%), and began to resemble adult diets.ConclusionsFeeding in the first 24 months shapes dietary patterns, and thus provides a critical window of opportunity to shape subsequent dietary patterns and eating habits. Findings from FITS identified high sweet and low vegetable consumption as key issues. Education is needed for health care professionals and parents on feeding during this important period. These findings can be used to support the development of specific and actionable feeding guidance for parents.

Abstract:Background: Early introduction of complementary foods has been associated with various immune disorders, oxidative stress, and obesity in childhood. The gut microbiota and the short chain fatty acids (SCFAs) they produce are postulated to be on the causal pathway. The objective of this study was to determine if early complementary feeding (i.e. consumption of solids or non-water/formula liquids at or before 3 months) is prospectively associated with infant gut microbiota composition, diversity and SCFAs at 3 and 12 months of age in the Nurture birth cohort. Results: Mother-infant dyads in the early complementary feeding group (n = 18) had similar baseline characteristics to those in the later feeding group (n = 49). We assessed differential abundance of microbial taxa (measured by 16S rRNA gene sequencing of the V4 region) by timing of complementary feeding using beta-binomial regression models (considering a two-sided FDR corrected p-value of < 0.05 as significant), and we fittted linear regression models to assess the association between early complementary feeding and SCFA concentrations (quantified using gas chromatography). After multivariable adjustment for breastfeeding, delivery method, birth weight, and gestational age, there were 13 differentially abundant microbial amplicon sequence variants (ASVs) by timing of introduction to complementary foods at 3 months and 20 ASVs at 12 months. Infants introduced to complementary foods early (vs. later) had higher concentrations of the SCFA butyric acid (mean difference = 0.65, 95% CI: 0.27, 1.04, p < 0.01) and total SCFAs (mean difference = 38.8, 95% CI: 7.83, 69.7) at 12 months. Bilophila wadsworthia and Lachnospiraceae Roseburia were associated with early (vs. later) complementary feeding and with higher butyric acid concentrations at 3 and 12 months, respectively. Conclusions: Our findings are consistent with the hypothesis that early (vs. later) introduction to complementary foods is associated with altered gut microbiota composition and butyric acid concentrations measured in stool until at least 1 year of age. Further research is needed to determine if these changes mediate future development of metabolic and immune conditions.
