Abstract:Salisbury says that physician associates (PAs) have no place in general practice,1 but the 2000 already employed would indicate differently. The Royal College of General Practitioners had an opportunity to take a lead, support and develop the role, and make it fit for purpose. Instead, it has chosen the status quo. Progressive healthcare systems, such as those in the Netherlands and Sweden, have multidisciplinary teams, led by doctors, providing better care than patients can currently get in the United Kingdom. Rising complexity in primary care means that doctors need time and space to manage this while other, simpler, cases are managed by others. The PA role can help with this. Abstract:Like other health care systems, the National Health Service (NHS) in England has looked to new staffing configurations faced with medical staff shortages and rising costs. One solution has been to employ physician associates (PAs). PAs are trained in the medical model to assess, diagnose and commence treatment under the supervision of a physician. This paper explores the perceived effects on professional boundaries and relationships of introducing this completely new professional group. It draws on data from a study, completed in 2014, which examined the contribution of PAs working in general practice. Data were gathered at macro, meso and micro levels of the health care system. At the macro and meso level data were from policy documents, interviews with civil servants, senior members of national medical and nursing organisations, as well as regional level NHS managers (n = 25). At the micro level data came from interviews with General Practitioners, nurse practitioners and practice staff (n = 30) as well as observation of clinical and professional meetings. Analysis was both inductive and also framed by the existing theories of a dynamic system of professions. It is argued that professional boundaries become malleable and subject to negotiation at the micro level of service delivery. Stratification within professional groups created differing responses between those working at macro, meso and micro levels of the system; from acceptance to hostility in the face of a new and potentially competing, occupational group. Overarching this state agency was the requirement to underpin legislatively the shifts in jurisdictional boundaries, such as prescribing required for vertical substitution for some of the work of doctors.

Abstract:I am a general practitioner of 30 years' standing, and I've mostly agreed with Salisbury's opinions over the years, but I don't share her view that physician associates (PAs) have no role in general practice.1I seemed to spend the first 20 years of my working life wondering why I was seeing patients with self-limiting illnesses—now I see almost none, largely because they now see PAs or advanced nurse practitioners. The "unsupervised" care of patients with highly complex conditions by GPs is arguably more risky than the supervised care of patients with relatively minor conditions by PAs.We have two outstanding PAs in our practice. They are conscientious, knowledgeable, and popular with patients who understand that they are not doctors and might need to discuss care with a supervisor. Abstract:A few weeks have passed since the extraordinary general meeting at the Royal College of Physicians (RCP) debating physician associate (PA) roll-out and scope of practice. During the opening presentation, data were shown stating that the majority of PAs are currently employed in primary care settings in England. David Nicholl, one of the RCP fellows responsible for tabling the motions at the meeting, opened his remarks with 'We are not primary care physicians' when addressing the college audience. Abstract:Introduction New Zealand's health care system faces significant shortages in health care workers. To address workforce challenges and meet the population's health needs, health care systems around the world have introduced new clinical roles, such as physician associates/assistants (PAs) into existing health care teams. Aim This article aims to examine the benefits, challenges, and broader implications of regulating PAs in the context of New Zealand's primary care sector, with a specific emphasis on how it may impact general practice. Methods A range of literature surrounding the role, impact, and perception of PAs were selected and included in this article. Results The PA profession can significantly strengthen New Zealand's primary care workforce, improving patient access and continuity of care. However, the global deployment of PAs has faced scrutiny due to concerns about its potential risks to patient safety and the overall viability of such a role. Discussion If regulated, the PA profession can reshape New Zealand's primary care, offering a partial solution to current medical staff shortages. Trained under a generalised medical model similar to doctors, PAs possess the necessary skills to perform both routine and non-routine medical tasks. This dual capability can significantly improve primary care service provision, reduce existing workloads, and allow for a more efficient deployment of doctor expertise. However, medico-legal issues and the supervisory burden can impede widespread integration into general practice. Despite challenges, the success of the PA role relies on mutual trust, respect, and support from other clinical team members within primary health care.

Abstract:Recent studies suggest that team-based primary care models could contribute to eliminating the predicted physician shortages. In this article, the authors explore existing team-based clinical care delivery models, comparing specialist and primary care teams, that include patient-centered medical homes and accountable care organizations. Next, the authors describe the barriers to adopting these models on a large scale, particularly the regulatory, financial, and cultural factors as well as scope of practice considerations for nonphysician providers. The authors' aim is not to evaluate the merits of team-based primary care models but, rather, to ascertain whether such models should be at the center of current physician workforce planning policies. The authors argue that although emerging evidence indicates that primary care teams can improve patient outcomes, few data exist to suggest that these models will drastically reduce the need for additional physicians or other providers. Thus, the authors conclude that additional research is needed to evaluate the ability of such models to alleviate provider deficits. And, while policy makers should work toward their ideal health care system, they also must expand the physician workforce to meet the growing demand for health care services in the existing one.

Abstract:The plan to introduce and further expand physician associates through the NHS workforce plan has become a clinical, financial, and strategic mess. The royal colleges—including the Royal College of Physicians, the Royal College of Anaesthetists, and the Royal College of General Practitioners—are finally starting to develop national scope for the role.1Problems will arise with having multiple scopes of practice from different royal colleges setting different baselines or ceilings. This would create further conflict and raises questions about what happens if trusts decide to ignore their guidance—and whether doing so would put them at risk of legal challenge. Abstract:Primary care in the US faces challenges with clinician recruitment, retention, and burnout, with further workforce shortages predicted in the next decade. Team-based care can be protective against clinician burnout, and opportunities for interprofessional education (IPE) on professional development and leadership could encourage primary care transformation. Despite an increasingly important role in the primary care workforce, IPE initiatives training physician assistants (PAs) alongside physicians are rare. We describe the design, curriculum, and outcomes from an interprofessional primary care transformation fellowship for community-based primary care physicians and PAs.

Abstract:Objectives To compare the contribution of physician associates to the processes and outcomes of emergency medicine consultations with that of foundation year two doctors-in-training. Design Mixed-methods study: retrospective chart review using 4 months’ anonymised clinical record data of all patients seen by physician associates or foundation year two doctors-in-training in 2016; review of a subsample of 40 records for clinical adequacy; semi-structured interviews with staff and patients; observations of physician associates. Setting Three emergency departments in England. Participants The records of 8816 patients attended by 6 physician associates and 40 foundation year two doctors-in-training; of these n=3197 had the primary outcome recorded (n=1129 physician associates, n=2068 doctor); 14 clinicians and managers and 6 patients or relatives for interview; 5 physician associates for observation. Primary and secondary outcome measures The primary outcome was unplanned re-attendance at the same emergency department within 7 days. Secondary outcomes: consultation processes, clinical adequacy of care, and staff and patient experience. Results Re-attendances within 7 days (n=194 (6.1%)) showed no difference between physician associates and foundation year two doctors-in-training (OR 0.87, 95% CI 0.61 to 1.24, p=0.437). If seen by a physician associate, patients were more likely receive an X-ray investigation (OR 2.10, 95% CI 1.72 to 4.24), p<0.001), after adjustment for patient characteristics, triage severity of condition and statistically significant clinician intraclass correlation. Clinical reviewers found almost all patients’ charts clinically adequate. Physician associates were evaluated as assessing patients in a similar way to foundation year two doctors-in-training and providing continuity in the team. Patients were positive about the care they had received from a physician associate, but had poor understanding of the role. Conclusions Physician associates in emergency departments in England treated patients with a range of conditions safely, and at a similar level to foundation year two doctors-in-training, providing clinical operational efficiencies.

Abstract:The problem of adequate medical care for the nation's elderly is mounting as this population grows in numbers. While the overall U.S. population has tripled since 1900, the segment over 65 has increased eightfold. Because of the high incidence of chronic illness in persons over 65, they consume a disproportionate amount of health care. Contributing to the problem are pervasive attitudes of ageism in the U.S. culture (including those of physicians), with the result that old people in the U.S. receive fragmented and often substandard medical care, particularly in nursing homes. Although some stirrings of activity in geriatric medicine are apparent in medical schools, not nearly enough faculty are available to teach courses and the interest of medical students is low. Several trends indicate that physician assistants are prepared to help fill the gaps in health care of the elderly. First, as physician extenders, these allied health professionals have demonstrated that they can perform approximately 80% of primary care tasks carried out by physicians at no sacrifice of quality. Second, a large proportion of the current caseload of physician assistants is patients over the age of 65. Third, physician assistant training programs have incorporated a fivefold increase in geriatric courses into their curricula since 1980, and both students and graduates show a high interest in this field. Finally, reimbursement policies of third party payers indicate a trend toward underwriting more physician assistant services.
