Abstract:Hybrid imaging modalities combine two or more medical imaging techniques offering exciting new possibilities to image the structure, function and biochemistry of the human body in far greater detail than has previously been possible to improve patient diagnosis. In this context, simultaneous Positron Emission Tomography and Magnetic Resonance (PET/MR) imaging offers great complementary information, but it also poses challenges from the point of view of hardware and software compatibility. The PET signal may interfere with the MR magnetic field and vice-versa, posing several challenges and constrains in the PET instrumentation for PET/MR systems. Additionally, anatomical maps are needed to properly apply attenuation and scatter corrections to the resulting reconstructed PET images, as well motion estimates to minimize the effects of movement throughout the acquisition. In this review, we summarize the instrumentation implemented in modern PET scanners to overcome these limitations, describing the historical development of hybrid PET/MR scanners. We pay special attention to the methods used in PET to achieve attenuation, scatter and motion correction when it is combined with MR, and how both imaging modalities may be combined in PET image reconstruction algorithms.

Abstract:The combination of clinical MR and PET scanners has received increasing attention in recent years. In contrast to currently used PET/CT machines, PET/MR offers not only improved soft-tissue contrast and reduced ionizing radiation but also a wealth of available MR variations such as functional, spectroscopic, and diffusion tensor imaging. This combination, however, has proven to be very challenging, due to the detrimental effect of the scanners on each other’s performance. Significant progress has been made in the last 10 years to solve the various technical issues, culminating with the recent release of clinical whole-body hybrid scanners.In this chapter, we review the technological challenges of PET/MR design, briefly describing the different available architectures for hybrid clinical scanners, their capabilities, and limitations.

Abstract:Simultaneous positron emission tomography and MRI (PET/MRI) is a technology that combines the anatomic and quantitative strengths of MR imaging with physiologic information obtained from PET. PET and computed tomography (PET/CT) performed in a single scanning session is an established technology already in widespread and accepted use worldwide. Given the higher cost and complexity of operating and interpreting the studies obtained on a PET/MRI system, there has been question as to which patients would benefit most from imaging with PET/MRI versus PET/CT. In this article, we compare PET/MRI with PET/CT, detail the applications for which PET/MRI has shown promise and discuss impediments to future adoption. It is our hope that future work will prove the benefit of PET/MRI to specific groups of patients, initially those in which PET/CT and MRI are already performed, leveraging simultaneity and allowing for greater degrees of multiparametric evaluation.Level of Evidence: 5Technical Efficacy: Stage 5J. Abstract:In just over a decade, hybrid imaging with FDG PET/CT has become a standard bearer in the management of cancer patients. An exquisitely sensitive whole-body imaging modality, it combines the ability to detect subtle biologic changes with FDG PET and the anatomic information offered by CT scans. With advances in MR technology and advent of novel targeted PET radiotracers, hybrid PET/MRI is an evolutionary technique that is poised to revolutionize hybrid imaging. It offers unparalleled spatial resolution and functional multi-parametric data combined with biologic information in the non-invasive detection and characterization of diseases, without the deleterious effects of ionizing radiation. This article reviews the basic principles of FDG PET and MR imaging, discusses the salient technical developments of hybrid PET/MR systems, and provides an introduction to FDG PET/MR image acquisition.

Abstract:The integration of positron emission tomography (PET) and magnetic resonance imaging (MRI) has been an ongoing research topic for the last 20 years. This paper gives an overview of the different developments and the technical problems associated with combining PET and MRI in one system. After explaining the different detector concepts for integrating PET-MRI and minimising interference the limitations and advantages of different solutions for the detector and system are described for preclinical and clinical imaging systems. The different integrated PET-MRI systems are described in detail. Besides detector concepts and system integration the challenges and proposed solutions for attenuation correction and the potential for motion correction and resolution recovery are also discussed in this topical review.

Abstract:Positron emission tomography (PET) and magnetic resonance imaging (MRI) have both been used for decades in cardiovascular imaging. Since 2010, hybrid PET/MRI using sequential and integrated scanner platforms has been available, with hybrid cardiac PET/MR imaging protocols increasingly incorporated into clinical workflows. Given the range of complementary information provided by each method, the use of hybrid PET/MRI may be justified and beneficial in particular clinical settings for the evaluation of different disease entities. In the present joint position statement, we critically review the role and value of integrated PET/MRI in cardiovascular imaging, provide a technical overview of cardiac PET/MRI and practical advice related to the cardiac PET/MRI workflow, identify cardiovascular applications that can potentially benefit from hybrid PET/MRI, and describe the needs for future development and research. In order to encourage its wide dissemination, this article is freely accessible on the European Radiology and European Journal of Hybrid Imaging web sites.• Studies and case-reports indicate that PET/MRI is a feasible and robust technology.• Promising fields of application include a variety of cardiac conditions.• Larger studies are required to demonstrate its incremental and cost-effective value.• The translation of novel radiopharmaceuticals and MR-sequences will provide exciting new opportunities.

Abstract:PET and MRI are established clinical tools which provide complementary information, but clinical workflow limits widespread clinical application of both modalities in combination. The two modalities are usually situated in different hospital departments and operated and reported independently, and patients are referred for both scans, often consecutively. With the advent of PET/MR as a new hybrid imaging modality there is now a possibility of addressing these concerns. There are two different design philosophies for integrated PET/MR imaging-positioning PET inside the MRI magnet or in tandem, similar to PET/CT. The Ingenuity TF PET/MR by Philips Healthcare is a sequential PET/MR tomograph combining state-of-the-art time-of-flight PET and high-field MRI with parallel transmission capabilities. In this review article we describe the technology implemented in the system, for example RF and magnetic shielding, MR-based attenuation correction, peculiarities in scatter correction, MR system optimisation, and the philosophy behind its design. Furthermore, we provide an overview of how the system has been used during the last two years, and expectations of how the use of PET/MR may continue in the years to come. On the basis of these observations and experiences we discuss the utility of the system, clinical workflow and acquisition times, and possible ways of optimization.

Abstract:Positron emission tomography (PET) and magnetic resonance imaging (MRI) are established imaging modalities for the study of neurological disorders, such as epilepsy, dementia, psychiatric disorders and so on. Since these two available modalities vary in imaging principle and physical performance, each technique has its own advantages and disadvantages over the other. To acquire the mutual complementary information and reinforce each other, there is a need for the fusion of PET and MRI. This combined dual-modality (either sequential or simultaneous) could generate preferable soft tissue contrast of brain tissue, flexible acquisition parameters, and minimized exposure to radiation. The most unique superiority of PET/MRI is mainly manifested in MRI-based improvement for the inherent limitations of PET, such as motion artifacts, partial volume effect (PVE) and invasive procedure in quantitative analysis. Head motion during scanning significantly deteriorates the effective resolution of PET image, especially for the dynamic scan with lengthy time. Hybrid PET/MRI device can offer motion correction (MC) for PET data through MRI information acquired simultaneously. Regarding the PVE associated with limited spatial resolution, the process and reconstruction of PET data can be further optimized by using acquired MRI either sequentially or simultaneously. The quantitative analysis of dynamic PET data mainly relies upon an invasive arterial blood sampling procedure to acquire arterial input function (AIF). An image-derived input function (IDIF) method without the need of arterial cannulization, can serve as a potential alternative estimation of AIF. Compared with using PET data only, combining anatomical or functional information from MRI for improving the accuracy in IDIF approach has been demonstrated. Yet, due to the interference and inherent disparity between the two modalities, these methods for optimizing PET image based on MRI still have many technical challenges. This review discussed upon the most recent progress, current challenges and future directions of MRI-driven PET data optimization for neurological applications, with either sequential or simultaneous acquisition approach.

Abstract:Canadian Association of Radiologists Journal, Ahead of Print. Positron emission tomography/magnetic resonance (PET/MR) imaging has gone through major hardware improvements in recent years, making it a reliable state-of-the-art hybrid modality in clinical practice. At the same time, image reconstruction, attenuation correction, and motion correction algorithms have significantly evolved to provide high-quality images. Part I of the current review discusses technical basics, pre-clinical applications, and clinical applications of PET/MR in radiation oncology and head and neck imaging. PET/MR offers a broad range of advantages in preclinical and clinical imaging. In the preclinic, small and large animal-dedicated devices were developed, making PET/MR capable of delivering new insight into animal models in diseases and facilitating the development of methods that inform clinical PET/MR. Regarding PET/MR's clinical applications in radiation medicine, PET and MR already play crucial roles in the radiotherapy process. Their combination is particularly significant as it can provide molecular and morphological characteristics that are not achievable with other modalities. In addition, the integration of PET/MR information for therapy planning with linear accelerators is expected to provide potentially unique biomarkers for treatment guidance. Furthermore, in clinical applications in the head and neck region, it has been shown that PET/MR can be an accurate modality in head and neck malignancies for staging and resectability assessment. Also, it can play a crucial role in diagnosing residual or recurrent diseases, reliably distinguishing from oedema and fibrosis. PET/MR can furthermore help with tumour characterization and patient prognostication. Lastly, in head and neck carcinoma of unknown origin, PET/MR, with its diagnostic potential, may obviate multiple imaging sessions in the near future.

Due to the major improvements in the hardware and image reconstruction algorithms, positron emission tomography/magnetic resonance imaging (PET/MR) is now a reliable state-of-the-art hybrid modality in medical practice. Currently, it can provide a broad range of advantages in preclinical and clinical imaging compared to single-modality imaging. In the second part of this review, we discussed the further clinical applications of PET/MR. In the chest, PET/MR has particular potential in the oncology setting, especially when utilizing ultrashort/zero echo time MR sequences. Furthermore, cardiac PET/MR can provide reliable information in evaluating myocardial inflammation, cardiac amyloidosis, myocardial perfusion, myocardial viability, atherosclerotic plaque, and cardiac masses. In gastrointestinal and hepato-pancreato-biliary malignancies, PET/MR is able to precisely detect metastases to the liver, being superior over the other imaging modalities. In genitourinary and gynaecology applications, PET/MR is a comprehensive diagnostic method, especially in prostate, endometrial, and cervical cancers. Its simultaneous acquisition has been shown to outperform other imaging techniques for the detection of pelvic nodal metastases and is also a reliable modality in radiation planning. Lastly, in haematologic malignancies, PET/MR can significantly enhance lymphoma diagnosis, particularly in detecting extra-nodal involvement. It can also comprehensively assess treatment-induced changes. Furthermore, PET/MR may soon become a routine in multiple myeloma management, being a one-stop shop for evaluating bone, bone marrow, and soft tissues.

Abstract:IntroductionRecently there have been significant advances in the field of machine learning and artificial intelligence (AI) centered around imaging-based applications such as computer vision. In particular, the tremendous power of deep learning algorithms, primarily based on convolutional neural network strategies, is becoming increasingly apparent and has already had direct impact on the fields of radiology and nuclear medicine. While most early applications of computer vision to radiological imaging have focused on classification of images into disease categories, it is also possible to use these methods to improve image quality. Hybrid imaging approaches, such as PET/MRI and PET/CT, are ideal for applying these methods.MethodsThis review will give an overview of the application of AI to improve image quality for PET imaging directly and how the additional use of anatomic information from CT and MRI can lead to further benefits. For PET, these performance gains can be used to shorten imaging scan times, with improvement in patient comfort and motion artifacts, or to push towards lower radiotracer doses. It also opens the possibilities for dual tracer studies, more frequent follow-up examinations, and new imaging indications. How to assess quality and the potential effects of bias in training and testing sets will be discussed.ConclusionHarnessing the power of these new technologies to extract maximal information from hybrid PET imaging will open up new vistas for both research and clinical applications with associated benefits in patient care.

Abstract:The multimodal imaging technique has gained the spotlight in the present era due to its striking and immense applications. It is the combination of two or more modalities that complement one another to yield detailed information. Indubitably, it is an emerging and crucial technique due to its broad clinical and research applications. The diagnostic techniques with the dual modality are aligned for obtaining molecular data. Positron emission tomography (PET) is a progressive imaging technique in nuclear medicine. To flourish in the imaging industry, PET was combined with computed tomography (CT), but the fusion of the two provides some challenges, such as less soft tissue contrast and inefficiency of acquisition in simultaneous mode. As a result, another hybrid imaging technology, PET and MRI (PET/MRI), has been developed to provide more soft tissue contrast and less radiation dose exposure, leading this technique to be used extensively despite its shortcomings. This review study discusses the fusion of PET/MRI, technical challenges for their combination, commercially available models, and clinical applications observed in the wide areas of oncology, the cardiovascular system, the central nervous system, pediatrics, and inflammatory diseases.

Abstract:Multimodality imaging with PET-CTand SPECT-CT has become commonplace in clinical practice and in preclinical and basic biomedical research.Although PET-CT is already an established clinical tool it still bears some limitations.To overcome these limitations, recent research concentrates on the combination of PET and MRI into one single machine.Combined PET-MRI allows for multi-parametric imaging and reveals one or more functional processes simultaneously along with high-resolution morphology.Especially in small-animal research, where high soft tissue contrast is required, and the scan time as well as radiation dose are critical factors, the combination of PET and MRI would be beneficial compared with PET-CT.

Abstract:Positron emission tomography (PET) facilitates the evaluation of molecular aspects and metabolic alterations that are fundamental in detecting of malignancies, characterization of tumor stage and assessment of therapeutical response, and tumor recurrence. The main advantage of PET is its high sensitivity in identifying of areas of cancerous involvement at an early stage. In general, the accelerated radiotracer activity occurs before anatomical structure changes. The main difficulty with PET is the lack of an anatomical reference frame. Magnetic resonance imaging (MRI) is an excellent morphological imaging modality with a high anatomical resolution. Whole-body MRI produce large amounts of image data, resulting in the possibility of overlooking subtle pathological findings. The fusion of PET with MRI can compensate for their disadvantages and therefore offers several advantages in comparison to PET or MRI alone. The combination of these two excellent diagnostic imaging modalities into a single scanner improves the diagnostic accuracy by facilitating the accurate registration of molecular aspects and metabolic alterations of the diseases with exact correlation to anatomical findings and morphological information. Whole-body PET/MRI is a very promising diagnostic modality for oncological imaging and for use in cancer screening in the decades to come due to the considerably lower radiation exposure in contrast to PET/CT and the high soft tissue resolution of MRI.

Abstract:A persistent theme in much anthropological writing is the concept of the deliberate control of population numbers by hunter-gatherers as a means of achieving moderate family size, adequate nutrition, and constrained adult mortality. An analysis of the mix of theory and field evidence that led to this conclusion finds the case not proven. On the contrary, Malthusian constraints can operate, and probably did operate, to produce a hunter-gatherer society where most adults were reasonably robust and healthy even though child mortality was high and life expectancy short. The absence of population limitation in pre-Neolithic times implies high mortality as well as high fertility, and weakens the argument positing a Neolithic mortality crisis.

Abstract:Imaging plays an increasingly important role in the early diagnosis, prognosis prediction and therapy response evaluation of central nervous system (CNS) diseases. The newly emerging hybrid positron emission tomography/magnetic resonance imaging (PET/MRI) can perform “one-stop-shop” evaluation, including anatomic, functional, biochemical and metabolic information, even at the molecular level, for personalised diagnoses and treatments of CNS diseases. However, there are still several problems to be resolved, such as appropriate PET detectors, attenuation correction and so on. This review will introduce the basic physical principles of PET/MRI and its potential clinical applications in the CNS. We also provide the future perspectives for this field.

Abstract:There is currently great interest in combining data from different imaging modalities, either by image registration methods that are performed after the data has been acquired or using new devices that can acquire data from two modalities simultaneously, or near simultaneously. In this paper a small prototype NMR-compatible PET scanner capable of acquiring PET images simultaneously with either NMR images or NMR spectra is described. In an associated paper [1], Pamela Garlick describes some investigations of cardiac metabolism that have been made using this system. One of the main challenges in constructing an NMR-compatible PET scanner is that photomultiplier tubes, which are an essential element of nearly all current PET systems, will not function in a high magnetic field. In collaboration with Simon Cherry and the Crump Institute of Biomedical Imaging at UCLA Medical School, a small (5.4 cm diameter) NMR-compatible PET scanner that will operate within the bore of an NMR magnet has been developed. Long optical fibres are used to transport light from the scintillation crystals that form the detector head to photomultiplier tubes situated in a low magnetic field region several metres from the magnet. This system has been used to perform simultaneous PET and NMR spectroscopy measurements with a 9.4T spectroscopy system, and has also been used to obtain simultaneous PET and MR images in several MRI scanners including a 4.7T small bore animal imaging system. Current efforts in the development of this technology are directed at experimental studies on small animals, both because this is less demanding technically and because it is in this area that applications are likely to appear first. However, there is no reason in principle why human PET-MR would not be feasible. Below, work with the prototype system and the next stage in its development are described, and some of the future possibilities and challenges are discussed.
Update on the Use of PET/MRI Contrast Agents and Tracers in Brain Oncology: A Systematic Review

Abstract:The recent advancements in hybrid positron emission tomography-magnetic resonance imaging systems (PET/MRI) have brought massive value in the investigation of disease processes, in the development of novel treatments, in the monitoring of both therapy response and disease progression, and, not least, in the introduction of new multidisciplinary molecular imaging approaches. While offering potential advantages over PET/CT, the hybrid PET/MRI proved to improve both the image quality and lesion detectability. In particular, it showed to be an effective tool for the study of metabolic information about lesions and pathological conditions affecting the brain, from a better tumor characterization to the analysis of metabolic brain networks. Based on the PRISMA guidelines, this work presents a systematic review on PET/MRI in basic research and clinical differential diagnosis on brain oncology and neurodegenerative disorders. The analysis includes literature works and clinical case studies, with a specific focus on the use of PET tracers and MRI contrast agents, which are usually employed to perform hybrid PET/MRI studies of brain tumors. A systematic literature search for original diagnostic studies is performed using PubMed/MEDLINE, Scopus and Web of Science. Patients, study, and imaging characteristics were extracted from the selected articles. The analysis included acquired data pooling, heterogeneity testing, sensitivity analyses, used tracers, and reported patient outcomes. Our analysis shows that, while PET/MRI for the brain is a promising diagnostic method for early diagnosis, staging and recurrence in patients with brain diseases, a better definition of the role of tracers and imaging agents in both clinical and preclinical hybrid PET/MRI applications is needed and further efforts should be devoted to the standardization of the contrast imaging protocols, also considering the emerging agents and multimodal probes.

Abstract:The implementation of hybrid imaging systems requires thorough and anticipatory planning at local and regional levels. For installation of combined positron emission and magnetic resonance imaging systems (PET/MRI), a number of physical and constructional provisions concerning shielding of electromagnetic fields (RF- and high-field) as well as handling of radionuclides have to be met, the latter of which includes shielding for the emitted 511 keV gamma rays. Based on our experiences with a SIEMENS Biograph mMR system, a step-by-step approach is required to allow a trouble-free installation. In this article, we present a proposal for a standardized step-by-step plan to accomplish the installation of a combined PET/MRI system. Moreover, guidelines for the smooth operation of combined PET/MRI in an integrated research and clinical setting will be proposed. Overall, the most important preconditions for the successful implementation of PET/MRI in an integrated research and clinical setting is the interdisciplinary target-oriented cooperation between nuclear medicine, radiology, and all referring and collaborating institutions at all levels of interaction (personnel, imaging protocols, reporting, selection of the data transfer and communication methods).
