Abstract:The diagnosis of primary hemifacial spasm (pHFS), due to a benign compression of the facial motor nerve by a vessel, within or close to its root exit zone, is often made with delay. Misdiagnosis includes psychogenic spasm, tics, facial myokymia or blepharospasm, but in fact post-facial palsy synkinesis (post-paralytic HFS) is the closest clinical condition, because it is limited to the territory of the facial nerve of a single hemiface. The differential diagnosis between these two entities, whose pathophysiological mechanisms are very different, can be made by electroneuromyographic (ENMG) examination. In addition, magnetic resonance imaging (MRI) is essential to show the offending vessel at the origin of pHFS and the absence of other causes of nerve compression. However, the diagnosis cannot be made on MRI basis alone, since a neurovascular conflict can be present in clinically asymptomatic subjects. This article reviews the clinical, MRI, and ENMG features in favour of a pHFS diagnosis as well as the various differential diagnoses of this involuntary facial movement disorder.

Abstract:Minoxidil is efficacious in inducing hair growth in patients with androgenetic alopecia by inducing hair follicles to undergo transition from the early to late anagen phase. Although the efficacy of 1% topical minoxidil has been confirmed in Japan, no controlled study of 5% topical minoxidil has been conducted using male Japanese subjects. The objective of this trial was to verify the superiority in clinical efficacy of 5% topical minoxidil to 1% topical minoxidil in a double‐blind controlled study with male, Japanese androgenetic alopecia patients as the subjects. The trial included 300 Japanese male patients aged 20 years or older with androgenetic alopecia who were administered either 5% topical minoxidil (n = 150) or 1% topical minoxidil (n = 150) for 24 weeks. The mean change from the baseline in non‐vellus hair/cm2, the primary efficacy variable, was 26.4 (n = 142) in the 5% topical minoxidil group and 21.2 (n = 144) in the 1% topical minoxidil group at 16 weeks, the main time point for the evaluation. The difference between the groups was significant (P = 0.020). The incidence of adverse events was 8.7% (13/150) in the 5% group and 5.3% (8/150) in the 1% group, with no significant difference between the groups (χ2‐test: P = 0.258). Our findings confirmed the superiority of 5% topical minoxidil to 1% topical minoxidil in treating Japanese men with androgenetic alopecia.

[Role of endoscopy and vascular decompression in the treatment of hemifacial spasm]

Abstract:Although it has long been hypothesized that hemifacial spasm could arise from a conflict between the nerve and the artery, it is very difficult to distinguish between a normal arterial loop and a pathological conflict leading to facial symptoms. Several new elements would help in the definition of the cause and allow less traumatic treatment of idiopathic hemifacial spasm. They include magnetic resonance imagery with CISS sequences, limited retrosigmoid approach, endoscopy of the cerebellopontine angle and monitoring the facial nerve. In our experience with 20 cases, retrosigmoid approach with a combined surgical and endoscopic procedure has led to total involution of the spasms in 80% of the cases.

Abstract:A method is described which in principle is a quantitative "spot analysis". 0.5-5 microliter of a protein solution in the concentration range between 0.01-10 mg/ml is used for one determination. The sample is taken up by capillary attraction in a 0.5-, 1-, 2- or 5-microliters capillary and transferred to a cellulose acetate strip. The protein is fixed and stained simultaneously by dipping the cellulose acetate strip into a solution of Amido Black or a benzoxanthene derivative (Hoechst 2495) dissolved in in methanol/acetic acid. After elution of the excess of dye (3 x 5 min) the quantitative evaluation can be performed in different ways: 1) The sample is fixed and made transpartent by incubation in dioxane/1-butanol and evaluated densitometrically (Amido Black 10B) or 2) the evaluation is performed in situ by spot fluorometry (Hoechst 2495). 3 The sample can be dissolved together with the acetate layer completely in dioxane, dimethylsulfoxide or N,N-dimethylformamide and evaluated photometrically or 4) fluorometrically. 5) Highest sensitivity is reached if the fluorochrome (Hoechst 2495) bound to the protein is eluted with 15% NH4OH and measured fluorometrically. There is a linear correlation with a correlation coefficient of 0.999 between the fluorescence and a protein amount of 10 ng-20 micrograms. In addition to its simplicity, the method has the advantage of being independent of or well-defined against other external influences, e.g., sodium dodecyl sulfate, mercaptoethanol, Triton X-100, etc. The stainability of a protein with Amido Black is influenced stoichiometrically by sodium dodecyl sulfate (not by mercaptoethanol) whilst the staining with Hoechst 2495 is not at all affected. As there is linear correlation between the area of a spot on an acetate layer and the volume applied in the range between 0.5 and 5 microliters, (only influenced stoichiometrically by the protein concentration in that volume, which in turn is measured by staining with Amido Black), then with a simple iterative calculation on the basis of suitable calibration curves, it is easily possible to determine a protein concentration in mg/ml even in an unknown volume between 0.5 and 5 microliters.

Abstract:The cranial nerve rhizophathy, commonly presented with trigeminal neuralgia (TN) or hemifacial spasm (HFS), is a sort of hyperexcitability disorders with higher incidence in senior Asian. In this paper, a novel hypothesis on the pathogenesis is proposed and with which some clinical phenomena are explained. In those with crowded cerebellopontine angle in anatomy, the cranial nerve root and surrounding vessel are getting closer and closer to each other with aging and finally the neurovascular conflict happens. As the interfacial friction associated with pulse, the nerve incurs demyelination. Since this pathological change develops to a certain degree, some transmembrane proteins emerge from the nerve due to a series of signaling pathway mediated by inflammatory cytokines. Among them, voltage-gated (Nav1.3) and mechanosensitive (Piezo2) ion channels may play the important role. With pulsatile compressions, the Piezo2 drives the resting potential toward depolarization forming a state of subthreshold membrane potential oscillation. Under this condition, just an appropriate pressure can make the membrane potential easy to reach threshold and activate the sodium channel, eventually generating conductible action potentials from the axon. When these ectopic action potentials propagate to the central nerve system, an illusion of sharp pain is perceived; while to the nerve-muscle junctions, an attack of irregular muscle constriction occurs. This hypothesis can well explain the symptomatic manifestation of paroxysmal attacks aroused by emotions. When we get nervous or excited, our heart rate and blood pressure alter correspondingly, which may give rise to "a just right pressure" - with specific frequency, amplitude and angle - impacting the suffered nerve to reach the threshold of impulse ignition. After a successful microvascular decompression surgery, the trigger is gone (there is no compression anymore) and the symptom is alleviated. While the postoperative recurrence could be attributable to Teflon granuloma development if had been placed improperly - for this nerve root has been susceptible no matter to arteries or to neoplasms. Besides, it may illustrate the clinical phenomenon that secondary TN or HFS cases are seldom caused by schwannoma: with a proliferative sheath, the nerve root is actually insulated. By contrast, not all neurovascular contacts can lead to the onset: it demands an exclusive extent of demyelination firstly.
Hemifacial spasm caused by a tortuous recurrent perforating artery: A case report

Abstract:Introduction: When the culprit vessel in hemifacial spasm (HFS) is hard to determine, this is a challenge in microvascular decompression (MVD) surgery. In such a situation, small arteries such as perforators to the brainstem might be suspected. But small arteries are omnipresent near the facial nerve root exit/entry zone (fREZ). How to decide whether a given small artery is responsible for HFS is unclear. Method: We report a case with a previously unreported form of neurovascular impingement, in which the culprit was found to be the recurrent perforating artery (RPA) from the anterior inferior cerebellar artery (AICA). An aberrant anatomic configuration of the RPA was found intraoperatively, which we thought was responsible for generating focal pressure on the facial nerve. Case report: A 62-year-old woman presented with a 1-year history of paroxysmal but increasingly frequent twitching in her right face. MRI showed tortuosity of the vertebral artery and apparently marked neurovascular impingement on the asymptomatic left side, while only the right AICA could be implicated as the possible culprit. Hemifacial spasm was diagnosed based on the typical clinical manifestation, and MVD was performed. The pre-meatal segment of the AICA was found not to be impinging the facial nerve at any susceptible portion near the fREZ: root exit point, attached segment, or root detachment point. The real culprit was in fact the RPA. This occult culprit vessel was tortuous, forming a coil-shaped twist which was interposed between the facial nerve and the intermediate nerve near the root detachment point. Focal pressure atrophy of the nerve was clearly observed at the compressing site. The patient achieved total spasm relief immediately after surgery, and remained spasm-free at 1-year follow-up, without any postoperative complications. Conclusion: MVD is the only curative treatment for hemifacial spasm, but with a failure rate of around 10%. Mistaking the real culprit has been reported to be the most likely reason for surgical failure. Therefore, intraoperative identification of atypical occult forms of vascular compression is of importance to improve surgical outcome. In the present case, the RPA formed a coil-shaped twist, which inflicted focal vascular compression causing hemifacial spasm. We recommend careful inspection of the recurrent perforating artery during MVD for HFS, and decompressing any such neurovascular impingement.

Abstract:Object: It is generally accepted that hemifacial spasm (HFS) is caused by pulsatile vascular compression upon the facial nerve root exit zone. This 2-3 mm area, considered synonymous with the Obersteiner-Redlich zone, is a transition zone (TZ) between central and peripheral axonal myelination that is situated at the nerve's detachment from the pons. Further proximally, however, the facial nerve is exposed on the pontine surface and emerges from the pontomedullary sulcus. The incidence and significance of neurovascular compression upon these different segments of the facial nerve in patients with HFS has not been previously reported. Methods: The nature of neurovascular compression was determined in 115 consecutive patients undergoing their first microvascular decompression (MVD) for HFS. The location of neurovascular compression was categorized to 1 of 4 anatomical portions of the facial nerve: RExP = root exit point; AS = attached segment; RDP = root detachment point that corresponds to the TZ; and CP = distal cisternal portion. The severity of compression was defined as follows: mild = contact without indentation of nerve; moderate = indentation; and severe = deviation of the nerve course. Success in alleviating HFS was documented by telephone interview conducted at least 24 months following MVD surgery. Results: Neurovascular compression was found in all patients, and the main culprit was the anterior inferior cerebellar artery (in 43%), posterior inferior cerebellar artery (in 31%), vertebral artery (in 23%), or a large vein (in 3%). Multiple compressing vessels were found in 38% of cases. The primary culprit location was at RExP in 10%, AS in 64%, RDP in 22%, and CP in 3%. The severity of compression was mild in 27%, moderate in 61%, and severe in 12%. Failure to alleviate HFS occurred in 9 cases, and was not related to compression location, severity, or vessel type. Conclusions: The authors observed that culprit neurovascular compression was present in all cases of HFS, but situated at the RDP or Obersteiner-Redlich zone in only one-quarter of cases and rarely on the more distal facial nerve root. Since the majority of culprit compression was found more proximally on the pontine surface or even pontomedullary sulcus origin of the facial nerve, these areas must be effectively visualized to achieve consistent success in performing MVD for HFS.

Abstract:Although primary hemifacial spasm (HFS) is mostly related to a vascular compression of the facial nerve at its root exit zone (REZ), its occurrence in association with distal, cisternal portion has been repeatedly reported during the last two decades. We report two patients with typical HFS caused by distal neurovascular compression, in which the spasm was successfully treated with microvascular decompression (MVD). Vascular compression of distal, cisternal portion of the facial nerve was identified preoperatively in the magnetic resonance imaging (MRI). It was confirmed again with intraoperative findings of compression of cisternal portion of the facial nerve by the meatal loop of the anterior inferior cerebellar artery (AICA) and absence of any offending vessel in the REZ of the facial nerve. Immediate disappearance of lateral spread response (LSR) after decompression and resolution of spasm after the operation again validated that HFS in the current patients originated from the vascular compression of distal, cisternal portion of the facial nerves. According to our literature review of 64 patients with HFS caused by distal neurovascular compression, distal compression can be classified by pure distal neurovascular compression (31 cases, 48.4%) and double compression (both distal segment and the REZ of the facial nerves, 33 cases [51.6%]) according to the presence or absence of simultaneous offender in the REZ. Eighty-four percent of 64 identified distal offenders were the AICA, especially its meatal and postmeatal segments. Before awareness of distal neurovascular compression causing HFS and sophisticated MRI imaging (before 2000), the rate of reoperation was high (58%). Preoperative MRI and intraoperative monitoring of LSR seems to be an essential element in determination of real offending vessel in MVD caused by distal offender.

Abstract:Hemifacial spasm is a neurological disorder due to abnormal hyperactivity of the facial nerve. The most common cause of hemifacial spasm is a neuro-vascular conflict in the cerebellopontine angle between a vascular loop and the root of the facial nerve (96% of cases). Tumors are the cause of hemifacial spasm in only 1% of cases). The authors present their results in 100 patients who underwent microvascular decompression for essential hemifacial spasm between 1990 and 1995. They used microsurgical and endoscopic procedures by a minimal retrosigmoid approach in all cases. The most common offending vessels were the posterior inferior cerebellar artery (70%), the vertebral artery (41%) and the anterior inferior cerebellar artery (28%). An aberrant vein was found in 2 cases. There were 38% of multiple artery-nerve conflicts. Physiopathology of hemifacial spasm is explained by two principal theories: in the ephaptic theory, hyperactivity and an abnormal nervous impulse pathway are due to a short demyelinated area on the nerve trunk caused by the offending vessel, inducing short circuiting between adjacent nerve fibers. In the nuclear theory, hyperactivity of the facial nerve is due to an abnormal and automatic activity of the facial nerve nucleus itself, induced by the vessel. The authors used pre and postoperative electromyographic tests and intraoperative electromyographic tests. Their results tend to prove the nuclear theory. Ninety per cent of the patients had a good result, with a mean follow-up time of 30 months in 60 cases. In 82% of the cases, there was a total recovery after a single procedure. There was no mortality and no facial palsy. Hearing loss occurred in less than 5%.
Release of endogenous GABA from the rat brain: methodological aspects and the effects of catecholamines.

Abstract:The release of endogenous GABA from the striatum, the substantia nigra and the ventral thalamic nucleus was measured in push-pull canula perfusates with high pressure chromatography and fluorimetric detection. The release of GABA per min is dependent on the flow rates, when low perfusion rates were applied. The GABA release was enhanced by K+ (maximal effect at 50 mM). The effects of various drugs added to the perfusion medium was measured. Imipramine, propranolol and other lipophilic bases (10 microM) enhanced the GABA release in the thalamus. Dopamine (DA), noradrenaline (NA), isoprenaline, clonidine and dibutyryl cyclic AMP decreased the GABA release from the striatum. Haloperidol blocked the effect of DA, but not that of NA. Stimulation of the locus coeruleus decreased GABA output. Destruction of striatal GABA-ergic neurons by kainic acid did not lead to a decreased number of NA-receptors as measured by the formation of cyclic AMP. The results indicate that both catecholamines influence GABA release and that DA and NA act at different receptors. The beta responses are mediated by beta-adrenoceptors that are not confined to striatal GABA neurons.

Abstract:By means of a sensitive semiquantitative electrophoretic technique involving tryptic digestion of immobilized casein embedded in agarose gel, the acid-stable proteinase-inhibitory fraction of human serum was shown to consist of two fractions with alpha 1- and prealbumin mobility, respectively. The prealbumin-like migrating inhibitor (pA-PI) was previously thought to be the only inhibitor present in the acid supernatant. The pA-PI was demonstrated in native serum as well, but was not recovered quantitatively upon acid precipitation. Using antibody-containing intermediary gels pA-PI was shown to react with inter-alpha-trypsin inhibitor antibodies, whereas the alpha 1-inhibitor reacted with alpha 1-proteinase inhibitor antibodies. The pA-PI activity was compared to serum levels of creatinine and C-reactive protein using a limited number of patients. The inhibitor level was clearly increased in patients with high serum creatinine and in some cases of high concentrations of C-reactive protein in the serum. Only small amounts of pA-PI were detected in sera of healthy people.
Using strength-duration analysis to identify the afferent limb of the lateral spread response in hemifacial spasm patients during microvascular decompression surgery

Abstract:Strength-duration analysis has been used to identify excitability differences between motor and sensory axons in human peripheral mixed nerves. The trigeminal and facial nerves have both been suggested to play a role in mediating the lateral spread response (LSR) in patients with hemifacial spasm (HFS). We sought to investigate this hypothesis by analyzing strength-duration properties of spasm side mentalis M wave and o. oculi LSR in 22 patients undergoing microvascular decompression surgery for HFS. Simultaneous recordings of mentalis M wave and o. oculi LSR prior to dural opening were collected following marginal mandibular facial nerve branch stimulation. Threshold responses were observed at stimulus pulse widths from 0.05 to 1.0 ms and the chronaxie and rheobase calculated from charge versus stimulus pulse width plots. The mean chronaxie (±SEM) of mentalis M wave was 0.34 ± 0.03 ms and 0.33 ± 0.04 ms for the LSR (p = 0.42, one-tailed t-test). The rheobase for the M wave (8.0 ± 1.0 mA) was found to be significantly different than the LSR rheobase (5.7 ± 0.7 mA; p = 0.03, one-tailed t-test) likely due to differences in the threshold amplitudes of the M wave versus the LSR. These results are highly suggestive of the facial nerve and not the trigeminal nerve in mediating the LSR.

Antibiotic serum activities against bacterial isolations from cases of bovine pneumonic pasteurellosis in feedlot calves.

Abstract:The serum bactericidal activity test was carried out in two groups of 7 calves each suspected to be suffering from pneumonic pasteurellosis. The diagnosis was based on clinical signs, endoscopic examination and bacteriological investigations. The first group of seven calves received oxytetracycline at a dosage rate of 10 mg kg-1 once a day. A second similar group received penicillin at 30,000 IU kg-1 once a day. Results showed that the correlation between antibiograms, serum bactericidal activity and clinical response may be good for oxytetracyclines and poor for penicillin. The serum bactericidal activity test may be a good method to establish effective dosages for certain antibiotics in the treatment of pneumonic pasteurellosis.

Abstract:Background: Primary hemifacial spasm (HFS) is usually caused by arterial compression of the facial nerve at the root exit zone at the brainstem. Rarely, a purely venous compression is seen. However, arachnoid bands strangulating the facial nerve have not been recognized as a cause of hemifacial spasm. Observations: The authors present a case of a 24-year-old female who had experienced HFS for 9 years. Endoscopic inspection of the root exit zone revealed no vascular compression but an arachnoid band strangulating the facial nerve. After cutting the band, the lateral spread response disappeared, and the patient was immediately spasm free after the surgery. Two years later, she was still doing well without any spasms. Lessons: This is the third patient in the authors' series of 535 patients who had no vascular conflict but rather a strangulation of the nerve by arachnoid bands. All patients have remained spasm free. To the authors' knowledge, no other group has reported arachnoid bands as an etiological factor of HFS so far.
