Abstract:In recent years,the incidence of coronary artery disease in China has tended to increase.Early screening of patients with coronary artery disease may lead to the appropriate therapeutic intervention and reduce the incidence of serious adverse cardiac events.Nowadays,the common diagnostic imaging methods include invasive coronary angiography,echocardiography,multi-slice spiral CT,radionuclide imaging and magnetic resonance imaging.With the rapid development of CT imaging technology,in addition to coronary angiography,dual-source CT can conduct stress myocardial perfusion imaging.This "one-stop" examination can obtain coronary anatomical features and myocardial hemodynamic change at the same time,which provides the basis for proper clinical intervention.The research progress of clinical application of dual-source CT myocardial perfusion imaging is reviewed in this paper.
[Recent advances in echocardiographic diagnosis of ischemic heart disease]

Abstract:Before the early 1990s, the diagnostic usefulness of echocardiography for ischemic heart disease had been relatively limited compared with that for other cardiac diseases such as valvular disease, congenital anomalies and cardiomyopathies. The principal role of echocardiography was to assess persistent regional wall motion abnormality as well as to detect complications of myocardial infarction. However, recent technological advances have created many newer applications of echocardiography in this field. One of the most important advances was seen in the field of stress echocardiography. Dobutamine stress echocardiography has become an established method of diagnosing transient myocardial ischemia due to coronary stenosis and assessing the myocardial viability of a persistently akinetic segment. More recently, several new contrast agents have been developed or will be available in the near future to visualize the blood stream within the left heart cavity and myocardial blood flow. Simultaneously, new ultrasound technologies including harmonic imaging and gated intermittent imaging have enhanced the selective visualization of contrast agents and will contribute to noninvasive imaging of coronary microcirculation. Harmonic imaging has also been shown to improve quality of B-mode image without a contrast agent and will play an important role in the clinical recognition of wall motion abnormality in patients with ischemic heart disease. Recent advances in three-dimensional technology have enabled accurate measurements of left ventricular volume and ejection fraction without any geometrical assumption, which may be especially important for the evaluation of ischemic patients who often have a deformed left ventricular cavity due to remodeling.
Clinical Analysis of 373 Patients with Non-ST Elevation Acute Coronary Syndrome in a Teaching Hospital

Abstract:Aim To analysis of patients with non-ST elevation acute coronary syndrome in 2008,and scale the gap between real world pratice and evidence-based guidelines.Methods Clinical data of the dianosis and management of 373 patients with non-ST elevation acute coronary syndrome from January to December in 2006,and scale the gap between real world pratice and evidence-based guidelines.Results Elevated cardiac markers,chest X-ray,echocardiography,dynamic electrocardiogram,exercise electrocardiograph (ECG),coronary angiography were used more extensively,but the usage of cardiac stress imaging and electron-beam computed tomography was low.Significant increases in the use of coronary angiography,aspirin,using 300 mg first dosage of aspirin,β-blocker,angiotensin converting enzyme inhibitor (ACEI),thienopyridine,statins and invasive strategies such as percutaneous coronary intervention (PCI) (51.1%),early invasive intervention occurred.The utilization rate of morphine sulfate,nitrates,unfractionated heparin were low.Conclusions There are great progress in the dianosis and management of patients with non-ST elevation acute coronary syndrome.Still,further improvements are needed for optimal implementation of these guidelines such as the use of β-blocker,adding dosage of β-blocker,platelet glycoprotein Ⅱb/Ⅲa inhibitor,low-molecular-weight heparin.Prompt and effective measures should be taken to enhance the education of evidence-based guidelines.

Abstract:The use of a large number of cardiovascular biomarkers, meant to complement the use of the electrocardiogram, echocardiography cardiac imaging, and clinical symptom assessment, has become a routine in clinical diagnosis, differential diagnosis, risk stratification, and prognosis and guides the management of patients with suspected cardiovascular diseases. There is a broad consensus that cardiac troponin and natriuretic peptides are the preferred biomarkers in clinical practice for the diagnosis of the acute coronary syndrome and heart failure, respectively, while the roles and possible clinical applications of several other potential biomarkers are still under study. This review mainly focuses on the recent studies of the roles and clinical applications of troponin and natriuretic peptides, which seem to be the best-validated markers in distinguishing and predicting the future cardiac events of patients with suspected cardiovascular diseases. Additionally, the review briefly discusses some of the large number of potential markers that may play a more prominent role in the future.

Abstract:An activation-specific cDNA library was made from phytohaemagglutinin (PHA)-activated haematopoietic cells of the rainbow trout (Oncorhynchus mykiss) using the technique of suppression subtractive hybridization. Several immune system genes were identified, including an interleukin (IL)1 receptor related protein and two invariant chain-like proteins. Many clones showed no similarity by BLAST search, but had AU-rich elements. These fragments were labelled and used for hybridization with a PHA-activated head kidney cDNA library. Several immune system genes were isolated by this technique, including a tumour necrosis factor (TNF) decoy receptor and a novel chemokine, designated trout chemokine 2. The TNF receptor is 285 amino acids in length and is 32-36% identical to a brook trout and human homologue. The CC chemokine is 44% identical at the amino acid level to a carp CC chemokine and approximately 20% identical to several mammalian CC chemokines. However, it has a 91 amino acid stalk-like structure at its COOH end, which is similar to the glycosylated stalk of fractalkine, a mammalian CX(3)C chemokine. In summary, AU-rich fragments obtained from an activation-specific library proved useful as hybridization probes for isolating trout immune system genes.

Abstract:This article comments on the role of the most important biochemical markers that are already applied in clinical practice or are still under research, in Acute Coronary Syndromes (ACS). Cardiac troponin (cTn) is established as the 'gold standard' in the diagnosis of ACS. C-reactive protein (CRP) and especially high-sensitivity CRP (hs-CRP) are considered to be the most useful inflammatory markers for clinical practice in the setting of acute coronary syndrome. Brain-type natriuretic peptide (BNP) and the amino terminal fragment of the prohormone BNP (NT-proBNP) appear to provide prognostic information in individuals admitted for acute coronary syndromes. Microalbuminuria in nondiabetics appears to be a signal from the kidney that the vasculature, particularly the endothelium, is not functioning properly. Increased plasma levels of cystatin C, neopterin, myeloperoxidase, and pregnancy associated protein are associated with adverse cardiovascular outcomes, cardiovascular and noncardiovascular death, and possibly cerebrovascular disease. Furthermore, recent evidence suggests that serum levels of CD40-CD40L pathway exert important roles in progression, and outcome of acute coronary syndrome. In the future further, studies are necessary to elucidate the exact role of the new biochemical markers in ACS.

Abstract:Recent advances in intravascular imaging techniques have made it possible to assess the culprit lesions of acute coronary syndrome (ACS) in the clinical setting. Intravascular ultrasound (IVUS) is the most commonly used intravascular imaging technique that provides cross-sectional images of coronary arteries. IVUS can assess plaque burden and vessel remodeling. Optical coherence tomography (OCT) is a high-resolution (10 μm) intravascular imaging technique that uses near-infrared light. OCT can identify key features of atheroma, such as lipid core and thin fibrous cap. Near-infrared spectroscopy (NIRS) can detect lipid composition by analyzing the near-infrared absorption properties of coronary plaques. NIRS provides a chemogram of the coronary artery wall, which allows for specific quantification of lipid accumulation. These intravascular imaging techniques can depict histological features of plaque rupture, plaque erosion, and calcified nodule in ACS culprit lesions. However, no single imaging technique is perfect and each has its respective strengths and limitations. In this review, we summarize the implications of combined use of multiple intravascular imaging techniques to assess the pathology of ACS and guide lesion-specific treatment.

A meta-analysis of the cognitive and motivational effects of serious games

Abstract:It is assumed that serious games influences learning in 2 ways, by changing cognitive processes and by affecting motivation. However, until now research has shown little evidence for these assumptions. We used meta-analytic techniques to investigate whether serious games are more effective in terms of learning and more motivating than conventional instruction methods (learning: k = 77, N 5,547; motivation: k = 31, N 2,216). Consistent with our hypotheses, serious games were found to be more effective in terms of learning (d= 0.29, p .05) than conventional instruction methods. Additional moderator analyses on the learning effects revealed that learners in serious games learned more, relative to those taught with conventional instruction methods, when the game was supplemented with other instruction methods, when multiple training sessions were involved, and when players worked in groups.

Abstract:AIM: To study the atherosclerotic plaque composition in patients with intermediate coronary stenosis using multidetector computed tomography angiography(MDCTA) and to explore the relationship between serum matrix metalloproteinase 9,soluble CD40 ligand level and plaque composition.METHODS: Enrolled in the study were 65 patients aged(62±8) years,including 36 patients aged(61±1) years with stable angina pectoris(SAP) and 29 patients age(64±8) years with unstable angina pectoris(UAP) with at least one segment of coronary diameter stenosis 30%-70% confirmed by contrast-enhanced 64-DCTA.Plaque composition in coronary arteries was determined by plaque CT value based on Hounsfield units(HU): noncalcified,calcified,or mixed,and serum MMP-9 and sCD40L expressions were measured in both patients and 37 volunteers age(46±15) years without coronary heart disease(CHD).RESULTS: Plaques found in patients with intermediate coronary stenosis were mixed(118,46%),noncalcified(85,33%) and calcified(54,21%) and the number of mixed plus noncalcified was significantly higher than the number of calcified(P0.05).The levels of serum MMP-9 and sCD40L were higher in patients and lower in volunteers without CHD(P0.01,P0.05).CONCLUSION: Patients with intermediate coronary stenosis have a higher burden of mixed and noncalcified plaques and higher activities of serum MMP-9 and sCD40L.Thus,it is important to stabilize the vulnerable plaques so as to prevent and reduce acute cardiovascular events.
The diagnostic value of echocardiography combined with serum Hcy and PCSK9 in coronary heart disease.

Abstract:Coronary heart disease (CHD) is one of the most common cardiovascular diseases. This study analyzed the diagnostic value of echocardiography combined with serum homocysteine (Hcy) and proprotein convertase subtilisin 9 (PCSK9) in CHD. Here, 108 CHD patients were selected as the study group. Additionally, 108 patients with suspected CHD excluded by coronary angiography were selected as the control group. Serum Hcy and PCSK9 levels were detected by circulating enzymatic and biochemical analysis assay. The contrast echocardiography showed that the contrast agent filling velocity (β) and the maximum number of microbubbles (A) in the study group were declined compared to the control group. Serum Hcy and PCSK9 levels in the study group were higher than those in the control group. In addition, β, A, Hcy and PCSK9 are important influencing factors of CHD. The coronary artery disease branch numbers and the degree of stenosis in CHD patients were negatively correlated with β and A values, and positively correlated with serum Hcy and PCSK9 levels. β, A combined with serum Hcy and PCSK9 levels have diagnostic value for CHD, and are significantly correlated with the severity of CHD.

Abstract:Background The purpose of this study was to evaluate left ventricular systolic function in patients with different types of ischemic heart disease using two-dimensional speckle tracking imaging (2D-STI). Methods We retrospectively studied patients who were admitted to Peking University Third Hospital from January 2011 to December 2017 due to chest tightness and chest pain. Two hundred forty-two patients were divided into control group, CMD group and obstructive CAD group. The main coronary artery stenosis was confirmed by coronary angiography or coronary computed tomography and coronary flow reserve (CFR) in patients was measured by transthoracic Doppler echocardiography. Left ventricular strain and strain rate (SR) measured by 2D-STI. Cardiac structure and function were measured by conventional echocardiography. Results Conventional echocardiography showed that there was no significant difference in cardiac structure and function among the three groups ( P > 0.05). Moreover, the longitudinal strain (LS) of each ventricular wall in CMD group was notably lower than that in control group ( P < 0.01). In addition, global longitudinal SR and longitudinal SR in CMD group and obstructive CAD group were obviously lower than those in control group ( P < 0.01). GLS, endocardial LS and epicardial LS were negatively correlated with CFR ( P < 0.01). Conclusions Early left ventricular systolic dysfunction was found in patients with CMD and patients with obstructive CAD, with similar degree. CFR is an independent influencing factor of GLS. GLS and stratified LS have certain diagnostic value for CMD.
The Effect of Pressure-controlled Ventilation on Pulmonary Mechanics in the Prone Position During Posterior Lumbar Spine Surgery: A Comparison With Volume-controlled Ventilation

Abstract:Background: When an anesthetized patient is turned to the prone position using the Wilson frame, dynamic compliance (Cdyn) decreases and peak airway pressure (Ppeak) increases. As pressure-controlled ventilation (PCV) decreases the Ppeak, this prospective, randomized study was designed to compare the effect of PCV and volume-controlled ventilation (VCV) on lung mechanics in the prone position using the Wilson frame during posterior lumbar spine surgery. Methods: Forty patients scheduled for posterior lumbar spine surgery were randomly allocated to receive mechanical ventilation using either the VCV (n=20) or PCV (n=20) mode. Respiratory variables (including Ppeak and Cdyn) and oxygenation parameters were recorded at the supine position (Tsupine) and 30 minutes after prone positioning (Tprone). Results: During the study period, tidal volume, minute volume, end-tidal carbon dioxide tension, and positive end-expiratory pressure were comparable between the 2 groups. The Ppeak in the PCV group was lower than that in the VCV group throughout the study period (P=0.007 and 0.003 at Tsupine and Tprone, respectively) and was increased at Tprone compared with Tsupine in both groups. Cdyn was decreased from Tsupine to Tprone in both groups. However, the Cdyn in the PCV group was higher than that in the VCV group during the study period. The arterial oxygen tension was comparable between the 2 groups during the study period. Conclusions: PCV provides lower Ppeak compared with VCV when the ventilator is set to deliver the same tidal volume and variable respiratory rate to maintain a constant end-tidal carbon dioxide tension in patients undergoing posterior lumbar spine surgery in the prone position using the Wilson frame.
Emerging cardiac markers in coronary disease: role of brain natriuretic peptide and other biomarkers

Abstract:Brain natriuretic peptides (BNP and pro-BNP) represent useful biomarkers in heart failure diagnosis and risk stratification; more recently their clinical use has been applied to acute coronary syndrome with and without ST segment elevation. Few studies have demonstrated that hormone dosage can add clinical and prognostic information with respect to the traditional laboratory analysis (i.e. troponin, MB-creatine-kinase, C-reactive protein). Besides these traditional biomarkers, growing data indicate potential laboratory indexes that can predict endothelial dysfunction, vascular thrombosis and platelet aggregation. However, their prognostic value is currently unknown. We describe the most widely used laboratory tools in coronary artery disease with potential prognostic power that could provide incremental predictive information in clinical practice.
Proposal on the extent of lymph node dissection for gallbladder carcinoma.

Abstract:BACKGROUND/AIMS To evaluate the value of performing extended regional lymph node dissection for gallbladder carcinoma, the mode of recurrence after curative resection was analyzed. METHODOLOGY Records of 45 patients who underwent surgical resection for gallbladder carcinoma from 1973 to August 1997 were reviewed. RESULTS Thirty-three cases underwent a curative resection and 12 received a non-curative resection. Among the 32 patients who survived the curative resection, cancer recurred in 7 with lymph node metastasis, whereas recurrence was found in only 1 of the remaining 25 patients without lymph node metastasis (p < 0.0001). At the 1st diagnosis of recurrence in these 8 patients, lymph node recurrence was detected in 7, and the site of recurrence was limited to the lymph nodes, which were confined to the peripancreatic region and the interaortocaval nodes near the left renal vein in 4 cases. CONCLUSIONS In view of the site of the metastatic lymph nodes and the lymphatic drainage system of the gallbladder, it was considered that lymph node dissection was inadequate in 5 of the 8 patients and that 2 might have been cured by extended regional lymph node dissection, including complete resection of the retroportal, posterior pancreatoduodenal, right celiac and interaortocaval nodes.
New avenues for the assessment of stable ischemic heart disease

Abstract:Myocardial ischemia is a complex condition which may result from epicardial and/or microvascular causes involving functional and structural mechanisms. These mechanisms may overlap in a given patient illustrating the difficulties for appropriate management. Assessment of myocardial ischemia can be performed using noninvasive and invasive tools. However, despite living in the era of individualized precision medicine, these tools are not yet used in a broader fashion. Emerging noninvasive techniques such as quantitative perfusion cardiac magnetic resonance imaging (CMR) and stress perfusion computed tomography (CT) or photon-counting CT techniques may contribute to new standards in the assessment of stable angina patients. Invasive evaluation of myocardial ischemia should not only focus on hemodynamically relevant epicardial disease but also involve coronary vasomotor function testing (coronary spasm, coronary flow reserve, and microvascular resistance) where appropriate. Optimal patient management will depend on accurate and comprehensive diagnostic evaluation of myocardial ischemia and development of new treatment options in the future.

Abstract:Objective: To investigate the clinical value of quantitative assessment of myocardial perfusion(MP) for patients with coronary heart disease using global acoustic density technique(GAD) and real-time contrast echocardiography.Methods: Nineteen patients with coronary heart disease were performed contrast echocardiography using SonoVue (Bracco) intravenously.Four-chamber and twochamber view of left ventricular at apical were obtained.The MP were assessed quantitatively by GAD.Results: Quantitative methods using GAD: the MP were significantly higher in segments supplied by normal coronary arteries than those supplied by narrowed ones(22.09±18.12dB2/svs 61.94±31.01dB2/s, p<0.01).The MP were still lower in segments supplied by coronary arteries after PTCA and stenting than that by normal supply(28.84±23.94dB2/s vs 66.12±33.46dB2/s,p<0.01).However, the MP in segments supplied by coronary arteries after CABG were similar to that by normal supply (64.11±23.61dB2/svs 61.94±31.01dB2/s,p>0.05).Conclusions: Real-time contrast echocardiography and GAD can quantitatively evaluate the MP in patients with coronary heart disease.

Abstract:Modern imaging techniques occupy an important place in the diagnosis, selection of treatment and prognosis of patients with coronary artery disease. Hybrid imaging is a combination of two sets of diagnostic data that complement and enhance each other by comparing anatomical and functional characteristics. As a rule, hybrid imaging is synergistic, that is, more powerful, since the addition of new information leads to an increase in the sensitivity and specificity of each of the modalities separately. The review provides brief information on the diagnostic efficacy of myocardial perfusion scintigraphy (MPS), computerized tomography (CT) coronary angiography in comparison with invasive coronary angiography with fractional flow reserve. The diagnostic and prognostic significance of assessing calcium index with MPS, as well as CT coronary angiography combined with MPS in the diagnosis, risk stratification and prognosis of patients with coronary artery disease, is characterized in detail. A separate section is devoted to the importance of hybrid imaging in making decisions about myocardial revascularization.

Abstract:BACKGROUND: Although intracoronary electrocardiography (IC-ECG) offers direct electrophysiological insights into myocardial ischemia caused by insufficient coronary blood supply, compared to common diagnostic methods like electrocardiography (ECG), it lacks widespread adoption and robust clinical research. OBJECTIVE: To analyze the value and accuracy of intracoronary electrocardiogram in myocardial ischemia diagnosis in coronary heart disease patients. METHODS: Three hundred patients treated at our hospital were included in the study. Patients were categorized into non-ischemic group A (Fraction Flow Reserve [FFR] > 0.8) and ischemic group B (FFR 0.8) and ischemic subgroup B2 (FFR < 0.75). Both subgroups underwent IC-ECG examination. Receiver operating curves were constructed using FFR to assess the clinical utility of different IC-ECG parameters. RESULTS: Group A patients showed a significant decrease in ST-segment shift at J-point, ST-segment integral, T-peak, T-wave integral, and T-peak to end-time, while the Corrected Q-T interval (QTc-time) was significantly higher in the B group (p< 0.05). The parameters, including ST-segment shift at J-point, ST-segment integral, T-wave integral, T-peak, T-peak to end-time, and QTc-time, were found to have clinical significance in predicting the occurrence of myocardial ischemia (p< 0.05). CONCLUSION: Intracoronary electrocardiogram QT interval dispersion and Q-T peak (QTp) interval dispersion have a high diagnostic accuracy for myocardial ischemia in coronary heart disease.

Abstract:In the past 20 years,dysfunction of the coronary microcirculation was recognized as one of the causes of myocardial ischemia and associated with long-term prognosis.Although coronary microvasculature cannot be directly imaged,a number of invasive and noninvasive techniques can be used as special parameters to assess coronary microcirculation function.These techniques include invasive coronary hemodynamic parameters via coronary catheter,and noninvasive imaging such as cardiac magnetic resonance and radionuclide imaging.Each method has relative advantages and limitations, however, these techniques have provided novel insights into the physiological and pathophysiological status,the regulation of the whole coronary artery system,and guide clinical therapy to the coronary artery disease.

Abstract:Contrast angiography is presently the standard for in vivo quantitation and morphologic evaluation of coronary atherosclerosis. Pathologic-angiographic correlation studies have shown marked angiographic underestimation of atheromatous plaques. This is particularly true in the early stages of intimal thickening and atheroma growth where media initially enlarges to accommodate the proliferative process without a chance in lumen size. The miniaturization of transducers allowed to mount them on the tip of a catheter and made it possible to position these catheters inside the arterial lumen and, for the first time, to see in vivo the vessel wall. Intravascular ultrasound is a new imaging technique with the ability to define and measure luminal areas and arterial wall components; therefore it serves as a potential tool to assess vascular wall morphology and dynamic vasomotor responses in epicardial coronary arteries. Its ability to measure luminal areas and diameters and the different components of the arterial wall has been validated by several in vitro, animal and in vivo studies. The definition of normal arterial wall by ultrasound has been defined looking at coronary arteries of young adults. It also showed that young subjects with angiographically normal vessels have a wide range of coronary intimal thickening, including occasional evidence of focal, early atheromatous lesions. Safety studies have shown it does not accelerate progression of angiographically quantifiable coronary artery disease suggesting that it can be safely utilized even in coronary arteries not undergoing interventions. The large potential of intracoronary imaging in the setting of interventional cardiology and future developments are also discussed, including the use of combined devices (ultrasound catheter-balloon angioplasty, ultrasound-atherectomy, ultrasound-laser); three dimensional reconstruction; new devices design, such as looking forward catheters; tissue characterization with the goal of differentiating between different types of plaque, and between soft plaque and thrombus; study of biomechanical properties of the atherosclerotic plaque, trying to distinguish between "good" and "bad" plaques. In conclusion, intravascular ultrasound is a powerful in vivo coronary imaging technique which may serve as a new standard for quantifying coronary atherosclerosis and offers significant potential for enhancing our understanding of the pathophysiology of coronary artery disease.
