Abstract:The seat of consciousness has not always been thought to reside in the brain. Its "source" is as varied as the cultures of those who have sought it. At present, although most may agree that the central nervous system is held to be the root of individualism in much of Western philosophy, this has not always been the case, and this viewpoint is certainly not unanimously accepted across all cultures today. In this paper the authors undertook a literary review of ancient texts of both Eastern and Western societies as well as modern writings on the organic counterpart to the soul. The authors have studied both ancient Greek and Roman material as well as Islamic and Eastern philosophy. Several specific aspects of the human body have often been proposed as the seat of consciousness, not only in medical texts, but also within historical documents, poetry, legal proceedings, and religious literature. Among the most prominently proposed have been the heart and breath, favoring a cardiopulmonary seat of individualism. This understanding was by no means stagnant, but evolved over time, as did the role of the brain in the definition of what it means to be human. Even in the 21st century, no clear consensus exists between or within communities, scientific or otherwise, on the brain's capacity for making us who we are. Perhaps, by its nature, our consciousness--and our awareness of our surroundings and ourselves--is a function of what surrounds us, and must therefore change as the world changes and as we change.
Heart in art: cardiovascular diseases in novels, films, and paintings

Abstract:Background: Understanding representations of disease in various art genres provides insights into how patients and health care providers view the diseases. It can also be used to enhance patient care and stimulate patient self-management. Methods: This paper reviews how cardiovascular diseases are represented in novels, films, and paintings: myocardial infarction, aneurysm, hypertension, stroke, heart transplantation, Marfan's disease, congestive heart failure. Various search systems and definitions were used to help identify sources of representations of different cardiovascular diseases. The representations of the different diseases were considered separately. The Common Sense Model was used a theoretical model to outline illness perceptions and self-management in the various identified novels, films, and paintings. Results: Myocardial infarction followed by stroke were the most frequently detailed diseases in all three art genres. This reflects their higher prevalence. Representations ranged from biomedical details through to social and psychological consequences of the diseases. Conclusions: Artistic representations of cardiovascular diseases reflect cognitions, emotions, and images of prevalent disease. These representations shape views and behaviour of ill and healthy persons regarding heart diseases. As these representations are amenable to change, they deserve further research, which may be instrumental in improving the quality of life of persons struck by cardiovascular diseases. Changing illness perceptions appears to be a method to improve self-management and thereby quality of life in patients with various cardiovascular diseases.

Abstract:Despite rapid advancements in modern medicine, the treatment and prevention of cardiovascular diseases have achieved significant progress. However, issues such as high incidence, recurrence rates, and poor prognosis still profoundly threaten human life and quality of living. The correlation between the heart and brain in anatomy and function, supported by neurological regulation and pathological foundations, validates the association between them. Traditional medicine, characterized by holistic concepts and differential treatment, presents the therapeutic principle of treating different diseases similarly, offering new insights into the clinical treatment of cardiovascular diseases. This paper aims to summarize the theoretical basis of the heart-brain correlation from functional, material, neurologic, and pathological perspectives, aiming to provide more evidence for the theory of treating the brain and heart simultaneously.

The haunted heart and the Holy Ghost: on retrieval, donation and death

Abstract:This enquiry examines problems which haunt the 'heart' and its donation. It begins by examining the heart's enduring significance for culturally mediated self-understanding, its vulnerability to misunderstanding and abuse and its relevance to challenging the determination of death by neurological criteria. Despite turns to brain-centred self-conceptions, the heart remains haunted by the hybrid experiences of identity accompanying organ transplant, the relational significance attached to dead hearts witnessed in the Alder Hey scandal and claims that heart transplants commonly constitute the legitimate killing of a person. To explore these phenomena, traditions are retrieved in which the heart-as-organ was construed in terms of a person's core identity. Influential Abrahamic beliefs about 'the heart' are considered in order to explore explanations for why the heart remains culturally pre-eminent, to make intelligible our haunted hearts and to examine possible violations of solidarity in organ donation practice. Jewish and Christian Scriptures are exegeted to illumine the sources of our haunting and address the desire for holistic bodily life. In these sources, the heart is the seat of affections, intelligence and agency but requires healing, conceived via the surgical metaphors of heart transplant and circumcision, if people are to join the insightful, solidary path of pilgrimage. Absent healing, the heart experiences a judgement of the whole person-organ-and-core-at the moment of death. Through such exegesis, the doctrine of the Holy Ghost emerges as a way to make intelligible, though not dispel, the heart's haunting. The doctrine's practical significance concerns the possibility of social unity among hearts, 'intercordiality', which construes people within a covenantal life of pilgrimage which encourages heart donation in certain circumstances, makes intelligible the Alder Hey parents' experience of social misunderstanding and rejects ascribing any legitimacy in medical culture to the consensual killing of patients for the sake of retrieving their organs.

Abstract:The review on the problem of the pulmonary heart pursues two goals: firstly, to restore historical justice and to show the priority studies of doctor Dmitry D. Pletnev on such problems as diagnosis of right ventricular myocardial infarction, clinical characteristics of congestive heart failure of the right ventricle; secondly, to outline the modern concept of the pulmonary heart. The review provides an analysis of the pathogenetic mechanisms of the development of heart failure in the pulmonary heart. Much attention is paid to echo cardiography data and biological markers are emphasized in assessing the function of the right atrium, right ventricle, tricuspid valve regurgitation, pulmonary artery pressure. Prognostically unfavorable signs of the course of the pulmonary heart have been identified, which include a high degree of tricuspid valve regurgitation, the amplitude of movement of the fibrous valve ring (TAPSE) and atrial fibrillation developing with dilation of the right atrium.

Abstract:Heart rate variability (HRV), the change in the time intervals between adjacent heartbeats, is an emergent property of interdependent regulatory systems that operate on different time scales to adapt to challenges and achieve optimal performance. This article briefly reviews neural regulation of the heart, and its basic anatomy, the cardiac cycle, and the sinoatrial and atrioventricular pacemakers. The cardiovascular regulation center in the medulla integrates sensory information and input from higher brain centers, and afferent cardiovascular system inputs to adjust heart rate and blood pressure via sympathetic and parasympathetic efferent pathways. This article reviews sympathetic and parasympathetic influences on the heart, and examines the interpretation of HRV and the association between reduced HRV, risk of disease and mortality, and the loss of regulatory capacity. This article also discusses the intrinsic cardiac nervous system and the heart-brain connection, through which afferent information can influence activity in the subcortical and frontocortical areas, and motor cortex. It also considers new perspectives on the putative underlying physiological mechanisms and properties of the ultra-low-frequency (ULF), very-low-frequency (VLF), low-frequency (LF), and high-frequency (HF) bands. Additionally, it reviews the most common time and frequency domain measurements as well as standardized data collection protocols. In its final section, this article integrates Porges' polyvagal theory, Thayer and colleagues' neurovisceral integration model, Lehrer et al.'s resonance frequency model, and the Institute of HeartMath's coherence model. The authors conclude that a coherent heart is not a metronome because its rhythms are characterized by both complexity and stability over longer time scales. Future research should expand understanding of how the heart and its intrinsic nervous system influence the brain.

Whereas is that the whole picture of wound healing? Decades have passed; we are still following these protocols. Do we really have nothing better to do with wound healing? Rachel Clarke tells the story of Keira's heart, which beats on today in Max's body. It is, of course, a tragic story. A beautiful, vivacious, perfectly healthy and adored daughter and sister is in the wrong place — a known accident black spot — at the wrong time. Fatally wounded and likely brain dead, she is transported by road to a nearby hospital as she is too sick to travel farther. Her mother and brother, with multiple fractures and internal injuries, respectively, are airlifted to a regional trauma centre; her two sisters, miraculously, are away at a sleepover. Keira's father, away on a motorcycling weekend, learns the news when he stops for fuel and makes a desperate, sickening pilgrimage across country to begin reuniting his broken family. It is also a hopeful story. We follow Max's progressive decline from energetic schoolboy to pallid invalid as a (presumed) virus wreaks havoc with his myocardium. He is unable to concentrate or play. Abstract:Professor Marks is a colleague, mentor, and friend. His illustrious and influential career as a haematologist and bone marrow transplanter has resulted in numerous scientific publications, and as such he has firmly established his reputation as an experienced and eloquent scientific writer. When asked to review his book, Life Blood – part semi-fictional work, part real life narrative - I realised that I knew little of his broader literary skill as a creative writer. It was with excitement and a little trepidation that I began reading. The first thing that struck me about this book was its honesty. On a fundamental level – the text is a vulnerable reflection on being a doctor as well as a human being. The reader is introduced to a series of patient anecdotes drawn from real-life experience, and through these interactions is encouraged to view how the clinical expertise that we offer as healthcare professionals, is wholly complemented by the richness and depth of human interaction between patient and doctor. As haematologists, we are often privileged to look after patients for many years. Therapeutic relationships that we hope to develop with our patients are bi-directional. No-one goes away empty handed. We cannot fail but to witness, learn, understand things about each other. Professor Marks shows us how influential and ultimately humbling these interactions can be for the clinician – in the consulting room and beyond. 'I learnt a lot from Sally and her indomitable spirit. It taught me to not give up too easily.'

Abstract:IF we compare the text-books of physiology of to-day with those of twenty years ago, we cannot fail to be impressed, not only with the vast strides that have been made by the subject within this short time, but also with the fact that a large majority of the latest discoveries, which have an intimate bearing on the understanding and control of disease, could not figure at all in the physiological equipment of the men who studied medicine at that time and are now in the full tide of practice. Even the professional physiologist finds it difficult to keep himself abreast of the course of discovery in his own subject. It would seem, therefore, almost impossible to expect a man in a busy practice to appreciate what recent physiology has done and is doing for his science and for his craft. Many men, and those not the least successful, do. not attempt the task, and trust to their craftsmanship and their powers of naming a diseased condition, that is, of placing it in a category familiar to them which they therefore believe they understand, and to their experience in treating such cases without, at any rate, harming the patient.

Abstract:Stroke-heart syndrome (SHS), a critical yet underrecognized condition, encompasses a range of cardiac complications that arise following an ischemic stroke. This narrative review explores the pathophysiology, clinical manifestations, and implications of SHS, focusing on the complex interplay between the brain and the heart. Acute ischemic stroke (AIS) triggers autonomic dysfunction, leading to a surge in catecholamines and subsequent myocardial injury. Our review highlights the five cardinal manifestations of SHS: elevated cardiac troponin (cTn) levels, acute myocardial infarction, left ventricular dysfunction, arrhythmias, and sudden cardiac death. Despite the significant impact of these complications on patient outcomes, there is a notable absence of specific guidelines for their management. Through a comprehensive literature search, we synthesized findings from recent studies to elucidate the mechanisms underlying SHS and identified gaps in the current understanding. Our findings underscore the importance of early detection and multidisciplinary management of cardiac complications post-stroke. Future research should focus on establishing evidence-based protocols to improve clinical outcomes for stroke patients with SHS. Addressing this unmet need will enhance the care of stroke survivors and reduce mortality rates associated with cardiac complications.
Blood, perceptions, resource and ownership: When transfusion illustrates the complexity

Abstract:Blood is apart from the rest of the tissues as this fluid is overseen by basic and applied life and humanistic sciences. Blood is the essence of human functioning. It is the object of one of the most commonly known cancers, leukemia. It is life-saving in transfusion - a property that also gives blood a special credit and questions blood as a valuable merchandise or as no ones' property but common good. But blood is also scandalous after the tainted blood affair in the 1980s and 1990s. Blood is further inseparable from most religious practices, both forefront and hidden (magic cults). It is frightening as it is versed in legitimate and illegitimate combats; it is poured to compensate offenses or debts in many civilizations. Any time blood comes forefront, rationale science leaves it to irrational digressions. Even the very same life-saving transfusion, is beaten by groups of opponents on religious grounds. Further, at a time blood cells and molecules are scrutinized, no one can claim having a complete understanding of what blood is, off the vasculature, as - to study it - one has to alter it. The study of blood is fascinating for all colleges of an academy and not many topics can share this property: chemists, physicists, geneticists, physiologists, medical doctors, philosophers, ethicists, theologians, artists, historicists, anthropologists, sociologists, etc. have all contributed to depict different, specific, aspects of blood. The present review aims at merging different aspects of blood to give pathophysiologists a platform to better understand fears and hopes related to this special tissue, when dealing with patients of theirs.

Abstract:Music can powerfully evoke and modulate emotions and moods, along with changes in heart activity, blood pressure (BP), and breathing. Although there is great heterogeneity in methods and quality among previous studies on effects of music on the heart, the following findings emerge from the literature: Heart rate (HR) and respiratory rate (RR) are higher in response to exciting music compared with tranquilizing music. During musical frissons (involving shivers and piloerection), both HR and RR increase. Moreover, HR and RR tend to increase in response to music compared with silence, and HR appears to decrease in response to unpleasant music compared with pleasant music. We found no studies that would provide evidence for entrainment of HR to musical beats. Corresponding to the increase in HR, listening to exciting music (compared with tranquilizing music) is associated with a reduction of heart rate variability (HRV), including reductions of both low-frequency and high-frequency power of the HRV. Recent findings also suggest effects of music-evoked emotions on regional activity of the heart, as reflected in electrocardiogram amplitude patterns. In patients with heart disease (similar to other patient groups), music can reduce pain and anxiety, associated with lower HR and lower BP. In general, effects of music on the heart are small, and there is great inhomogeneity among studies with regard to methods, findings, and quality. Therefore, there is urgent need for systematic high-quality research on the effects of music on the heart, and on the beneficial effects of music in clinical settings.
