Abstract:A precise relative stopping power map of the patient is crucial for accurate particle therapy. Charged particle imaging determines the stopping power either tomographically - particle computed tomography (pCT) - or by combining prior knowledge from particle radiography (pRad) and x-ray CT. Generally, multiple Coulomb scattering limits the spatial resolution. Compared to protons, heavier particles scatter less due to their lower charge/mass ratio. A theoretical framework to predict the most likely trajectory of particles in matter was developed for light ions up to carbon and was found to be the most accurate for helium comparing for fixed initial velocity. To further investigate the potential of helium in particle imaging, helium computed tomography (HeCT) and radiography (HeRad) were studied at the Heidelberg Ion-Beam Therapy Centre (HIT) using a prototype pCT detector system registering individual particles, originally developed by the U.S. pCT collaboration. Several phantoms were investigated: modules of the Catphan QA phantom for analysis of spatial resolution and achievable stopping power accuracy, a paediatric head phantom (CIRS) and a custommade phantom comprised of animal meat enclosed in a 2 % agarose mixture representing human tissue. The pCT images were reconstructed applying the CARP iterative reconstruction algorithm. The MTF10% was investigated using a sharp edge gradient technique. HeRad provides a spatial resolution above that of protons (MTF1010%=6.07 lp/cm for HeRad versus MTF10%=3.35 lp/cm for proton radiography). For HeCT, the spatial resolution was limited by the number of projections acquired (90 projections for a full scan). The RSP accuracy for all inserts of the Catphan CTP404 module was found to be 2.5% or better and is subject to further optimisation. In conclusion, helium imaging appears to offer higher spatial resolution compared to proton imaging. In future studies, the advantage of helium imaging compared to other imaging modalities in clinical applications will be further explored.
Energy painting: helium-beam radiography with thin detectors and multiple beam energies

Abstract:Objective. Compact ion imaging systems based on thin detectors are a promising prospect for the clinical environment since they are easily integrated into the clinical workflow. Their measurement principle is based on energy deposition instead of the conventionally measured residual energy or range. Therefore, thin detectors are limited in the water-equivalent thickness range they can image with high precision. This article presents our energy painting method, which has been developed to render high precision imaging with thin detectors feasible even for objects with larger, clinically relevant WET ranges. Approach. A detection system exclusively based on pixelated silicon Timepix detectors was used at the Heidelberg ion-beam therapy center to track single helium ions and measure their energy deposition behind the imaged object. Calibration curves were established for five initial beam energies to relate the measured energy deposition to water-equivalent thickness (WET). They were evaluated regarding their accuracy, precision and temporal stability. Furthermore, a 60 mm × 12 mm region of a wedge phantom was imaged quantitatively exploiting the calibrated energies and five different mono-energetic images. These mono-energetic images were combined in a pixel-by-pixel manner by averaging the WET-data weighted according to their single-ion WET precision (SIWP) and the number of contributing ions. Main result. A quantitative helium-beam radiograph of the wedge phantom with an average SIWP of (1.82 ± 0.05) % over the entire WET interval from 150 mm to 220 mm was obtained. Compared to the previously used methodology, the SIWP improved by a factor of 2.49 ± 0.16. The relative stopping power value of the wedge derived from the energy-painted image matches the result from range pullback measurements with a relative deviation of only 0.4 %. Significance. The proposed method overcomes the insufficient precision for wide WET ranges when employing detection systems with thin detectors. Applying this method is an important prerequisite for imaging of patients. Hence, it advances detection systems based on energy deposition measurements towards clinical implementation.

Mixed particle beam for simultaneous treatment and online range verification in carbon ion therapy: Proof-of-concept study

Abstract:Purpose: Radiation therapy with ion beams provides a better conformation and effectiveness of the dose delivered to the tumor with respect to photon beams. This implies that a small uncertainty or variation in the crossed tissue shape and density may lead to a more important underdosage of the tumor and/or an overdosage of the surrounding healthy tissue. Although the online control of beam fluence and transverse position is well managed by an appropriate beam delivery system, the online measurement of the longitudinal position of the Bragg peak inside the patient is still an open issue. In this paper we propose a proof-of-concept study of a technique that would allow the online verification of the patient thickness along the beam direction, which could permit detecting a subset of possible range error causes, such as morphological variations. Methods: The nuclei 12 C and 4 He have the same magnetic rigidity: the two species could be accelerated together in an accelerator and a mixed particle beam delivered to the patient. In the same medium and with the same energy per nucleon, the range of 4 He2+ is about three times the 12 C6+ one. It is, thus, conceivable to achieve a dual goal with a single mixed beam: carbon, stopping into the tumor, is appointed to cure, while helium, emerging from the patient, to control: by detecting and measuring the residual range and position of He, it would be possible to determine the integrated relative stopping power of the patient and prove that it is the expected one. For the detection of helium particles, a plastic scintillator and an optical sensor are proposed. Being helium ions not available at CNAO, the detection system has been characterized using a proton beam. Nevertheless, since the light emitted by a proton is less than the one produced by a helium ion, the helium signal is expected to be more pronounced than the proton one (for the same number of particles). To predict the magnitude of the light signal measured by the sensor, two Monte Carlo models have been setup and validated by measuring the photons per pixel impinging on the sensor. To deal with the many optical issues and to reliably describe the physical process, some corrections have been included into the models. Results: The predictions of both the models are in good agreement with the measurements (within the 20% in terms of absolute photons per pixel). The proposed detection system is able to measure the range of a proton beam with sub-millimetrical precision also in the presence of the background produced by carbon ion fragments and discrepancies in the expected range were detected with a resolution better than 1 mm. Conclusions: Although many technical issues have still to be addressed for a real implementation in a clinical environment, the preliminary results of this study suggest that a surrogate of real-time verification of the beam range inside the patient during a treatment with carbon ions is possible by adding a small fraction of helium ions to the primary beam.
Three dimensional reconstruction of therapeutic carbon ion beams in phantoms using single secondary ion tracks

Abstract:Carbon ion beam radiotherapy enables a very localised dose deposition. However, already small changes in the patient geometry or positioning errors can significantly distort the dose distribution. A live monitoring system of the beam delivery within the patient is therefore highly desirable and could improve patient treatment. We present a novel three-dimensional imaging method of the beam in the irradiated object, exploiting the measured tracks of single secondary ions emerging under irradiation. The secondary particle tracks are detected with a TimePix stack, a set of parallel pixelated semiconductor detectors. We developed a three-dimensional reconstruction algorithm based on maximum likelihood expectation maximisation. We demonstrate the applicability of the new method in an irradiation of a cylindrical PMMA phantom of human head size with a carbon ion pencil beam of 226MeV/u. The beam image in the phantom is reconstructed from a set of 9 discrete detector positions between -80 and 50 degrees from the beam axis. Furthermore, we demonstrate the potential to visualise inhomogeneities by irradiating a PMMA phantom with an air gap as well as bone and adipose tissue surrogate inserts. We successfully reconstructed a 3D image of the treatment beam in the phantom from single secondary ion tracks. The beam image corresponds well to the distribution expected from the beam direction and energy. In addition, cylindrical inhomogeneities with a diameter of 2.85cm and density differences down to 0.3g/cm$^3$ to the surrounding material are clearly visualised. This novel 3D method to image a therapeutic carbon ion beam in the irradiated object does not interfere with the treatment and requires knowledge only of single secondary ion tracks. Even with detectors with only a small angular coverage, the 3D reconstruction of the fragmentation points presented in this work was found to be feasible.

Abstract:Particle therapy is a well established clinical treatment of tumors. More than one hundred particle therapy centers are in operation world-wide. The advantage of using hadrons like protons or carbon ions as particles for tumor irradiation is the distinct peak in the depth-dependent energy deposition, which can be exploited to accurately deposit doses in the tumor cells. To guarantee this, high accuracy in monitoring and control of the particle beam is of the utmost importance. Before the particle beam enters the patient, it traverses a monitoring system which has to give fast feedback to the beam control system on position and dose rate of the beam while minimally interacting with the beam. The multi-wire chambers mostly used as beam position monitors have their limitations when a fast response time is required (drift time). Future developments such as MRI-guided ion beam therapy pose additional challenges for the beam monitoring system, such as tolerance of magnetic fields and acoustic noise (vibrations). Solid-state detectors promise to overcome these limitations and the higher resolution they offer can create additional benefits. This article presents the evaluation of an HV-CMOS detector for beam monitoring, provides results from feasibility studies in a therapeutic beam, and summarizes the concepts towards the final large-scale assembly and readout system.

Abstract:Abstract Recently, it has been proposed that a mixed helium/carbon beam could be used for online monitoring in carbon ion beam therapy. Fully stripped, the two ion species exhibit approximately the same mass/charge ratio and hence could potentially be accelerated simultaneously in a synchrotron to the same energy per nucleon. At the same energy per nucleon, helium ions have about three times the range of carbon ions, which could allow for simultaneous use of the carbon ion beam for treatment and the helium ion beam for imaging. In this work, measurements and simulations of PMMA phantoms as well as anthropomorphic phantoms irradiated sequentially with a helium ion and a carbon ion beam at equal energy per nucleon are presented. The range of the primary helium ion beam and the fragment tail of the carbon ion beam exiting the phantoms were detected using a novel range telescope made of thin plastic scintillator sheets read out by a flat-panel CMOS sensor. A 10:1 carbon to helium mixing ratio is used, generating a helium signal well above the carbon fragment background while adding little to the dose delivered to the patient. The range modulation of a narrow air gap of 1 mm thickness in the PMMA phantom that affects less than a quarter of the particles in a pencil beam were detected, demonstrating the achievable relative sensitivity of the presented method. Using two anthropomorphic pelvis phantoms it is shown that small rotations of the phantom as well as simulated bowel gas movements cause detectable changes in the helium/carbon beam exiting the phantom. The future prospects and limitations of the helium/carbon mixing as well as its technical feasibility are discussed.

Abstract:Objective: There is an increasing interest in calculating and measuring linear energy transfer (LET) spectra in particle therapy in order to assess their impact in biological terms. As such, the accuracy of the particle fluence energy spectra becomes paramount. This study focuses on quantifying energy depositions of distinct proton, helium, carbon, and oxygen ion beams using a silicon pixel detector developed at CERN to determine LET spectra in silicon. Approach: While detection systems have been investigated in this pursuit, the scarcity of detectors capable of providing per-ion data with high spatial and temporal resolution remains an issue. This gap is where silicon pixel detector technology steps in, enabling online tracking of single-ion energy deposition. The used detector consisted of a 300-μm thick silicon sensor operated in partial depletion. Main Results: During post-processing, artifacts in the acquired signals were identified and methods for their corrections were developed. Subsequently, a correlation between measured and Monte Carlo-based simulated energy deposition distributions was performed, relying on a two-step recalibration approach based on linear and saturating exponential models. Despite the observed saturation effects, deviations were confined below 7% across the entire investigated range of track-averaged LET values in silicon from 0.77 keV/μm to 93.16 keV/μm. Significance: Simulated and measured mean energy depositions were found to be aligned within 7%, after applying artifact corrections. This extends the range of accessible LET spectra in silicon to clinically relevant values and validates the accuracy and reliability of the measurements. These findings pave the way towards LET-based dosimetry through an approach to translate these measurements to LET spectra in water. This will be addressed in a future study, extending functionality of treatment planning systems into clinical routine, with the potential of providing ion-beam therapy of utmost precision to cancer patients.

Results of Images Acquired with Backscattered Mv Radiation Using A Pinhole Collimator

Abstract:Purpose:To ascertain the feasibility of acquiring real time images of small lung tumors from scattered photons while undergoing radiation treatment. There are several methodologies currently used to track tumor location such as MV‐cine acquisition and kV fluoroscopy. However, MVcine offers no information parallel to the beam axis while kV fluoroscopy offers little potential for soft tissue discernability while also increasing the patient dose. This study investigates the feasibility of observing an actual simulated tumor while exploring techniques that may improve image quality.Methods:A prototype imager consisting of a gamma camera pinhole collimator and a computed radiography (CR) plate were used in conjunction with a Varian TrueBeam linac. One study consisted of a 2.5 cm diameter solid water cylinder representing a solid tumor imbedded within a lung equivalent material slab. The cylinder with the lung slab was sandwiched between 1 cm lung equivalent slabs and these were sandwiched between 2 slabs of solid water. The top water slab was 1 cm thick. The other imaging study consisted of three different density plugs, 0.46, 1.09, and 1.82 g/cm3 placed on the accelerator couch. The gantry was orientated 70° relative to the CR plate. The slabs and plugs were irradiated with 2000 MU and 500 MU respectively using the 6FFF mode.Results:The solid water plug was visually discernible in the slab phantom. The ratio of the signal coming from the higher density plugs (placed on the treatment couch) to that between the plugs increased from 1.02 to about 3.0 after subtracting the background image acquired with no plugs present.Conclusion:Preliminary results indicate that a lung tumor could be visualized with scattered radiation during treatment. Improvements in discerning an object can be enhanced by filtering out the head leakage and background scattered radiation not emanating from the imaged object.
Offline imaging of positron emitters induced by therapeutic helium, carbon and oxygen ion beams with a full-ring PET/CT scanner: experiments in reference targets

Abstract:In vivo verification of light ion therapy based on positron-emission tomography (PET) imaging of irradiation induced patient activation relies on activity predictions from Monte-Carlo (MC) or analytical computational engines for comparison to the measurements. In order to achieve the necessary accuracy, experimental data are indispensable for the validation of the calculation models. For this we irradiated thick reference targets with mono-energetic helium, carbon and oxygen ion beams and measured the resulting material activation offline with a commercial full-ring PET/CT scanner located nearby the treatment room. Acquired PET data were analysed over time to separate the activity contribution of different radionuclides. Determined production yields were compared to published findings obtained from in-beam activation measurements with a limited-angle double-head PET camera. In addition, we investigated the time-dependence of the measured radionuclide-specific contributions and of the distal activity range, as well as the lateral spread of the activity signal as a function of beam penetration depth. We present radionuclide-specific depth-resolved activity distributions and production yields for the radionuclides [Formula: see text], [Formula: see text] and [Formula: see text], dominating irradiation-induced patient activation. We observe systematically lower production yields with a ratio between the dual-head and our full-ring PET measurements of, on average, 1.7 and 1.3 for the oxygen and carbon beam irradiations, and 1.7 (2.1) for the high (low) energy helium beam irradiations. Findings on the temporal development of the activity range confirm the expectation, with the oxygen beam induced signal being the most sensitive scenario. The experimental data reported in this work, acquired with a state-of-the-art full ring PET scanner, provide a comprehensive and consistent basis for the benchmarking of PET signal calculation engines. In particular, they can support a fine-tuning of the underlying physics models used by the respective implementation and therefore improve the accuracy of PET-based therapy verifications at current and future treatment facilities.
Inhomogeneity detection within a head-sized phantom using tracking of charged nuclear fragments in ion beam therapy

Abstract:Objective: The highly conformal carbon-ion radiotherapy is associated with an increased sensitivity of the dose distributions to internal changes in the patient during the treatment course. Hence, monitoring methodologies capable of detecting such changes are of vital importance. We established experimental setup conditions to address the sensitivity of a monitoring approach based on secondary-fragment tracking for detecting clinically motivated air cavity dimensions in a homogeneous head-size PMMA phantom in 40 mm depth. Approach: The air cavities were positioned within the entrance channel of a treatment field of 50 mm diameter at three lateral positions. The measured secondary-fragment emission profiles were compared to a reference measurement without cavities. The experiments were conducted at the Heidelberg Ion-Beam Therapy Center in Germany at typical doses and dose rates. Main Results: Significances above a detectability threshold of 2σ for the larger cavities (20 mm diameter and 4 mm thickness, and 20 mm diameter and 2 mm thickness) across the entire treatment field. The smallest cavity of 10 mm diameter and 2 mm thickness, which is on the lower limit of clinical interest, could not be detected at any position. We also demonstrated that it is feasible to reconstruct the lateral position of the cavity on average within 2.8 mm, once the cavity is detected. This is sufficient for the clinicians to estimate medical effects of such a cavity and to decide about the need for a control imaging CT. Significance: This investigation defines well-controlled reference conditions for the evaluation of the performance of any kind of treatment monitoring method and its capability to detect internal changes within head-sized objects. Air cavities with volumes from 0.31 cm3 to 1.26 cm3 were narrowed down around the detectability threshold of this secondary-fragment-based monitoring method.

Abstract:Despite the early recognition of the potential of proton imaging to assist proton therapy (Cormack 1963 J. Appl. Phys. 34 2722), the modality is still removed from clinical practice, with various approaches in development. For proton-counting radiography applications such as computed tomography (CT), the water-equivalent-path-length that each proton has travelled through an imaged object must be inferred. Typically, scintillator-based technology has been used in various energy/range telescope designs. Here we propose a very different alternative of using radiation-hard CMOS active pixel sensor technology. The ability of such a sensor to resolve the passage of individual protons in a therapy beam has not been previously shown. Here, such capability is demonstrated using a 36 MeV cyclotron beam (University of Birmingham Cyclotron, Birmingham, UK) and a 200 MeV clinical radiotherapy beam (iThemba LABS, Cape Town, SA). The feasibility of tracking individual protons through multiple CMOS layers is also demonstrated using a two-layer stack of sensors. The chief advantages of this solution are the spatial discrimination of events intrinsic to pixelated sensors, combined with the potential provision of information on both the range and residual energy of a proton. The challenges in developing a practical system are discussed.

Abstract:Ion-beam radiotherapy is an advanced cancer treatment modality offering steep dose gradients and a high biological effectiveness. These gradients make the therapy vulnerable to patient-setup and anatomical changes between treatment fractions, which may go unnoticed. Charged fragments from nuclear interactions of the ion beam with the patient tissue may carry information about the treatment quality. Currently, the fragments escape the patient undetected. Inter-fractional in-vivo treatment monitoring based on these charged nuclear fragments could make ion-beam therapy safer and more efficient. We developed an ion-beam monitoring system based on 28 hybrid silicon pixel detectors (Timepix3) to measure the distribution of fragment origins in three dimensions. The system design choices as well as the ion-beam monitoring performance measurements are presented in this manuscript. A spatial resolution of 4 mm along the beam axis was achieved for the measurement of individual fragment origins. Beam-range shifts of 1.5 mm were identified in a clinically realistic treatment scenario with an anthropomorphic head phantom. The monitoring system is currently being used in a prospective clinical trial at the Heidelberg Ion Beam Therapy Centre for head-and-neck as well as central nervous system cancer patients.

Abstract:<p>Ion beam therapy has become a frequently applied form of cancer therapy over the last years. The advantage of ion beam therapy over conventional radiotherapy using photons is the strongly localized dose deposition, leading to a reduction of dose applied to surrounding healthy tissue. Currently, treatment planning for proton therapy is based on X-ray computed tomography, which entails certain sources of inaccuracy in calculation of the stopping power (SP). A more precise method to acquire the SP is to directly use high energy protons (or other ions such as carbon) and perform proton computed tomography (pCT). With this method, the ions are tracked prior to entering and after leaving the patient and finally their residual energy is measured at the very end. Therefore, an ion imaging demonstrator, comprising a tracking telescope made from double-sided silicon strip detectors and a range telescope as a residual energy detector, was set up. First measurements with this setup were performed at beam tests at MedAustron, a center for ion therapy and research in Wiener Neustadt, Austria. The facility provides three rooms for cancer treatment with proton beams as well as one which is dedicated to non-clinical research.</p><p>This contribution describes the principle of ion imaging with proton beams in general as well as the design of the experimental setup. Moreover, first results from simulations and recent beam tests as well as ideas for future developments will be presented.</p>

Abstract:Charged particle beams are used in Particle Therapy (PT) to treat oncological patients due to their selective dose deposition in tissues and to their high biological effect in killing cancer cells with respect to photons and electrons used in conventional radiotherapy. Nowadays, protons and carbon ions are used in PT clinical routine but, recently, the interest on the potential application of helium and oxygen beams is growing due to their reduced multiple scattering inside the body and increased linear energy transfer, relative biological effectiveness and oxygen enhancement ratio. The precision of PT demands for online dose monitoring techniques, crucial to improve the quality assurance of treatments. The beam range confined in the irradiated target can be monitored thanks to the neutral or charged secondary radiation emitted by the interactions of hadron beams with matter. Prompt photons are produced by nuclear de-excitation processes and, at present, different dose monitoring and beam range verification techniques based on the prompt {\gamma} detection have been proposed. It is hence of importance to perform the {\gamma} yield measurement in therapeutical-like conditions. In this paper we report the yields of prompt photons produced by the interaction of helium, carbon and oxygen ion beams with a PMMA target. The measurements were performed at the Heidelberg Ion-beam Therapy center (HIT) with beams of different energies. A LYSO scintillator has been used as photon detector. The obtained {\gamma} yields for $^{12}$C ion beams are compared with results from literature, while no other results from $^{4}$He and $^{16}$O beams have been published yet. A discussion on the expected resolution of a slit camera detector is presented, demonstrating the feasibility of a prompt-{\gamma} based monitoring technique for PT treatments using helium, carbon and oxygen ion beams.

Abstract:Treatment planning in ion beam therapy requires accurate knowledge of the relative stopping power (RSP) distribution within the patient. Currently, RSP maps are obtained via conventional x-ray computed tomography (CT) by converting the measured attenuation coefficients of photons into RSP values for ions. Alternatively, to avoid conversion errors that are inherent to this method, ion computed tomography (iCT) can be used since it allows determining the RSP directly. In typical iCT systems, which usually consist of a tracking system and a separate residual energy detector, the RSP is obtained by measuring the particle trajectory and the corresponding water equivalent path length (WEPL) of single ions travelling through the patient. Within this work, we explore a novel iCT approach which does not require a residual energy detector. Instead, the WEPL is estimated indirectly by determining the change in time of flight (TOF) due to the energy loss along the ion's path. For this purpose, we have created a Geant4 model of a TOF-iCT system based on low gain avalanche detectors (LGADs), which are fast 4D-tracking detectors that can measure the time of arrival and position of individual particles with high spatial and time precision. To assess the performance of this TOF-iCT concept, we determined the RSP resolution and accuracy for different system settings using the Catphan CTP404 sensitometry phantom. Within the set of investigated system parameters, the lower limit of the RSP accuracy was found at 0.91%, demonstrating the proof-of-principle of this novel TOF-iCT concept. The main advantage of using this approach is that it could potentially facilitate clinical integration due to its compact design, which, however, requires experimental verification and an improvement of the current WEPL calibration procedure.

Evaluation of a pixelated large format CMOS sensor for x-ray microbeam radiotherapy

Abstract:Purpose: Current techniques and procedures for dosimetry in microbeams typically rely on radiochromic film or small volume ionization chambers for validation and quality assurance in 2D and 1D, respectively. Whilst well characterized for clinical and preclinical radiotherapy, these methods are noninstantaneous and do not provide real time profile information. The objective of this work is to determine the suitability of the newly developed vM1212 detector, a pixelated CMOS (complementary metal-oxide-semiconductor) imaging sensor, for in situ and in vivo verification of x-ray microbeams. Methods: Experiments were carried out on the vM1212 detector using a 220 kVp small animal radiation research platform (SARRP) at the Helmholtz Centre Munich. A 3 x 3 cm2 square piece of EBT3 film was placed on top of a marked nonfibrous card overlaying the sensitive silicon of the sensor. One centimeter of water equivalent bolus material was placed on top of the film for build-up. The response of the detector was compared to an Epson Expression 10000XL flatbed scanner using FilmQA Pro with triple channel dosimetry. This was also compared to a separate exposure using 450 µm of silicon as a surrogate for the detector and a Zeiss Axio Imager 2 microscope using an optical microscopy method of dosimetry. Microbeam collimator slits with range of nominal widths of 25, 50, 75, and 100 µm were used to compare beam profiles and determine sensitivity of the detector and both film measurements to different microbeams. Results: The detector was able to measure peak and valley profiles in real-time, a significant reduction from the 24 hr self-development required by the EBT3 film. Observed full width at half maximum (FWHM) values were larger than the nominal slit widths, ranging from 130 to 190 µm due to divergence. Agreement between the methods was found for peak-to-valley dose ratio (PVDR), peak to peak separation and FWHM, but a difference in relative intensity of the microbeams was observed between the detectors. Conclusions: The investigation demonstrated that pixelated CMOS sensors could be applied to microbeam radiotherapy for real-time dosimetry in the future, however the relatively large pixel pitch of the vM1212 detector limit the immediate application of the results.
Nanodosimetric investigation of the track structure of therapeutic carbon ion radiation. Part 1: Measurement of ionization cluster size distributions

Abstract:At the Heidelberg Ion-Beam Therapy Center, the track structure of carbon ions of therapeutic energy after penetrating layers of simulated tissue was investigated for the first time. Measurements were conducted with carbon ion beams of different energies and polymethyl methacrylate (PMMA) absorbers of different thicknesses to realize different depths in the phantom along the pristine Bragg peak. Ionization cluster size (ICS) distributions resulting from the mixed radiation field behind the PMMA absorbers were measured using an ion-counting nanodosimeter. Two different measurements were carried out: (i) variation of the PMMA absorber thickness with constant carbon ion beam energy and (ii) combined variation of PMMA absorber thickness and carbon ion beam energy such that the kinetic energy of the carbon ions in the target volume is constant. The data analysis revealed unexpectedly high mean ICS values compared to stopping power calculations and the data measured at lower energies in earlier work. This suggests that in the measurements the carbon ion kinetic energies behind the PMMA absorber may have deviated considerably from the expected values obtained by the calculations. In addition, the results indicate the presence of a marked contribution of nuclear fragments to the measured ICS distributions, especially if the carbon ion does not cross the target volume.

Flat-panel imager energy-dependent proton radiography for a proton pencil-beam scanning system

Abstract:In proton-based radiotherapy, proton radiography could allow for direct measurement of the water-equivalent path length (WEPL) in tissue, which can then be used to determine relative stopping power (RSP). Additionally, proton radiographs allow for imaging in the beam's-eye-view. In this work, a proton radiography technique using a flat-panel imager and a pencil-beam scanning (PBS) system is demonstrated in phantom. Proton PBS plans were delivered on a Varian ProBeam system to a flat-panel imager. Each proton plan consisted of energy layers separated by 4.8 MeV, and a field size of 25 cm × 25 cm. All measured data is binned into a layer-by-layer delivery in post processing. To build a calibration curve correlating detector response to WEPL, the plans were delivered to slabs of solid water of increasing thickness. Pixel-by-pixel detector response in the time/energy domain is then fit as a function of WEPL. Tissue equivalent phantoms are imaged for evaluation of WEPL accuracy. A spatial resolution phantom and a head phantom are also imaged. For all experiments, the detector was run with an effective pixel size of 0.4 mm × 0.4 mm. The proposed method reconstructed RSP with mean errors of 2.65%, -0.14%, and 0.61% for lung, soft tissue, and bone, respectively. In a 40 mm thick spatial resolution phantom, a 2 mm deep pinhole with 1 mm diameter can be seen. The accuracy and spatial resolution of the method show that it could be implemented for patient position verification. Further development could lead to patient-specific verification of RSP to be used for treatment guidance.

Abstract:Proton beam therapy benefits from the Bragg peak and delivers highly conformal dose distributions. However, the location of the end-of-range is subject to uncertainties related to the accuracy of the relative proton stopping power estimates and thereby the water-equivalent path length (WEPL) along the beam. To remedy the range uncertainty, an in vivo measurement of the WEPL through the patient, i.e. a proton-range radiograph, is highly desirable. Towards that goal, we have explored a novel method of proton radiography based on the time-resolved dose measured by a flat panel imager (FPI). A 226 MeV pencil beam and a custom-designed range modulator wheel (MW) were used to create a time-varying broad beam. The proton imaging technique used exploits this time dependency by looking at the dose rate at the imager as a function of time. This dose rate function (DRF) has a unique time-varying dose pattern at each depth of penetration. A relatively slow rotation of the MW (0.2 revolutions per second) and a fast image acquisition (30 frames per second, ~33 ms sampling) provided a sufficient temporal resolution for each DRF. Along with the high output of the CsI:Tl scintillator, imaging with pixel binning (2 × 2) generated high signal-to-noise data at a very low radiation dose (~0.1 cGy). Proton radiographs of a head phantom and a Gammex CT calibration phantom were taken with various configurations. The results of the phantom measurements show that the FPI can generate low noise and high spatial resolution proton radiographs. The WEPL values of the CT tissue surrogate inserts show that the measured relative stopping powers are accurate to ~2%. The panel did not show any noticeable radiation damage after the accumulative dose of approximately 3831 cGy. In summary, we have successfully demonstrated a highly practical method of generating proton radiography using an x-ray flat panel imager.
